Description

本発明は、一般的に医療用機器、より具体的には、医療用機器と共に用いられる液体用キャニスタの充填状態を判定する方法およびシステムに関する。
褥創性潰瘍や糖尿病患者の足に見られる潰瘍のような開放創とその痛みの治療は解決が難しい。しばしば細菌性の病原体に感染したこのような創傷の適切な治療は多元的なものである。外科医は通常、壊死した組織を切除することになる。病原性バクテリアの繁殖を抑え、あるいはこれを殺すために、可能であれば抗生物質が用いられる。同時に、患者の健康状態を維持するためには適切な栄養補給が必要であり、より重要なことは、創傷の適切な処置が傷と痛みの治癒に必須であることである。
創傷の適切な処置として、創傷部を清潔に且つ乾燥した状態に保つことが求められる。創傷が感染すると、下層組織が漿液を滲出させ、場合によっては化膿した液が出て、この双方は速やかな治癒のために除去されねばならない。上記の液を取り除く従来の方法においては、創傷部を機械的に洗浄することとスポンジを用いて創傷部を乾燥させることが求められる。このやり方は患者にとって痛みを伴うばかりでなく、治療中に創傷部にバクテリアが侵入することによって、重複感染の危険性が増大してしまう恐れがある。
開放創の機械的洗浄の代替法は、負圧による創傷密封デバイスである。この負圧による創傷密封デバイスは、創傷部の上に配置して創傷周辺に比較的気密な漏れ防止を形成する気泡パッドで構成される。負圧による創傷密封デバイスはまた、患者の創傷部の上に被せたパッドに流体連結させたバキュームポンプを具える。バキュームポンプが始動すると、創傷部を覆っているパッドの内部に負圧が作られる。創傷部から滲出する液は負圧によって気泡パッドを通じて除去され、気泡パッドに連結したチューブを通ってキャニスタに運ばれる。キャニスタは創傷部から除去された液を回収する。キャニスタが創傷部から除去された液で満杯になると、キャニスタを空にして真空システムに再び組み込む。キャニスタが創傷液で満杯になったらユーザにアラームを発するという試みがなされている。これらの方法は、キャニスタ内の創傷液の量を直接計測することを目指していた。
しかしながら一般的に創傷液は粘性が高いために、キャニスタ内の創傷液を直接計測する既存の方法は信頼性が無いことが明らかになった。創傷液はまた頻繁に発泡し、気泡を生じて空気を取り込み、これらはキャニスタ内部の底にたまること無く液中に残存する。そこで、この分野では創傷液を回収するキャニスタが満杯になり空にする必要が生じたときに、ユーザに警報を発する信頼性の有るシステムと方法が必要とされている。
既存のキャニスタ検知システムの問題点は、本発明によるシステムとその方法によって解決される。本発明の実施例によれば、キャニスタの満杯状態検出能力を有する減圧治療システムにおいて、当該システムが:減圧を提供する減圧源と;組織部位に配置され、前記減圧源と流体連結されたポーラス状のパッドと;前記ポーラス状のパッドと前記減圧源の間に流体連結されて、前記組織部位から出る液体を回収するキャニスタと;前記キャニスタと前記減圧源の間で導管と流体連結されているソレノイドバルブと;前記減圧源とポーラス状のパッドの間のブロッケージを検出するよう構成され、ブロッケージ状態が検出された場合に前記ソレノイドバルブを開く処理ユニットと;前記ソレノイドバルブが開いたときに、所定時間における前記導管内の圧力減衰を判定する手段と;前記圧力減衰に基づいて前記キャニスタの満杯状態を判定する手段と;を具えることを特徴とする
本発明のその他の目的、特徴、利点は、以下に続く図面ならびに詳細な説明によって明らかになる。
図1は、本発明の実施例によるキャニスタの満杯状態検出能力を具える減圧治療システムを示す。 図2は、図1の減圧治療システムにおける処理ユニットを示すブロック図である。 図3は、図に示す本発明の実施例によるキャニスタの満杯状態検出方法を用いたシステムによって実施したテスト結果を示すグラフである。 図4は、図に示す本発明の実施例によるキャニスタの満杯状態検出方法を用いたシステムによって実施したテスト結果を示すグラフである。 図5は、図に示す本発明の実施例によるキャニスタの満杯状態検出方法を用いたシステムによって実施したテスト結果を示すグラフである。 図6は、図に示す本発明の実施例によるキャニスタの満杯状態検出方法を用いたシステムによって実施したテスト結果を示すグラフである。
図に示す実施例の以下の詳細な説明において、実施例の一部をなす添付図面に符号が付されている。本発明は、これら実施例を用いて実施することができる。これらの実施例は、当業者が本発明を実施できるよう充分詳細に記載されている。図に示す実施例の精神と範囲から外れることなく、理論的・構造的・機械的・電気的そして化学的変更を行うことができる、その他の実施例を用いることができる。当業者が本発明を容易に実施するうえで必要ではない細事を避けるために、当業者に公知である一定の情報は割愛されている。従って、以下の詳細な説明は限定的に解されるべきではない。この実施例の範囲は、添付された請求項によって定められる。
ここで用いられている用語「減圧」とは、一般的に、治療の対象となる組織部位周囲の圧力よりも低い圧力を意味する。殆どの場合にこの減圧状態とは、患者が置かれている場所の大気圧よりも低い。用語「真空」および「負圧」は、ここでは組織部位に適用される圧力を記述するうえで使用されるが、組織部位に適用される実際の圧力は、通常完全な真空に使用される圧力よりも有意に小さい。この学術名称に従って、減圧あるいは真空圧における増加は、絶対圧力の相対的な減少として捉えられ、減圧あるいは真空圧における減少とは、絶対圧力の相対的な増加として捉えられる。
ここで用いられている「組織部位」とは、骨組織・脂肪組織・筋肉組織・皮膚組織・血管組織・結合組織・軟骨・腱あるいは靱帯等を含むが、これに限定されるものではなく、あらゆる細胞組織上あるいは組織中の損傷部分を指すものである。この「組織部位」なる語は、不必要に傷付きあるいは破壊された細胞組織のエリアを指すが、更に組織の成長促進が望まれる代わりのエリアでもある。例えば、採取された、あるいは他の組織の部位に移植された更なる組織を成長させるための特定の組織エリアで減圧組織治療が行なわれることにもなる。
図1を参照すると、減圧治療システム11はポーラス状のパッド20あるいは組織部位15に配置したその他の分散マニフォールドを具える。ポーラス状のパッド20はターゲット導管21によってキャニスタ17と流体連結されている。減圧源19は源導管24によってキャニスタ17に流体連結されている。動作中は、減圧源19が、源導管24およびキャニスタ17及びターゲット導管21によって集合的に構成された液体流路を介して、減圧を組織部位に送達する。
ソレノイドバルブあるいはその他の適当なバルブ18が源導管24に取り付けられ、このバルブが開いた時に、液体流路内の減圧を大気中に解放するよう作動する。ソレノイドバルブ18は処理ユニット26に通信して、このユニットによって制御されている。これは以下において更に詳述する。処理ユニット26は更に、源導管24内の圧力を検出するのに用いられる検知デバイス28と通信する。
減圧治療システム11は、キャニスタの満杯状態を(すなわち、キャニスタが液で満杯であるか否か)信頼性を持って判定することができる。このシステム11は、取り付けた液体流路の容積に依拠しており、従って、この既知の容積からバキュームの開放がバルブ18が開いた後の所定時間内に行なわれるべきである。信頼性のある何らかのブロッケージ検知手段によって、処理ユニット26が液体流路内にブロッケージを検出した場合は、処理ユニット26が所定時間(例えば1秒間)バルブを開くように信号を送って、液体流路から大気中に減圧を解放する。処理ユニット26は検知デバイス28から検知データを受けとって、この1秒間における圧力減衰をモニタする。この圧力が急速に減衰すれば、そこで処理ユニット26は、放出されているチャンバの量が小さいのでキャニスタ17は満杯である、と判定する。従って、キャニスタが満杯状態と判定されれば、処理ユニット26はアラーム信号を発する。
もし、減衰が遅い、すなわち、ソレノイドバルブ18が開いている1秒間でバキュームが予め設定されているレベルまで減衰しない場合には、処理ユニット26はキャニスタ17が満杯ではないと結論付ける。次いで、処理ユニットは、元々検出されているブロッケージ状態がターゲット流路21での実際のブロッケージによって生じており、キャニスタは満杯状態ではない、と見なす。この特殊な例では、ブロッケージが存在していることを表示する別のアラームを処理ユニット26が発する。キャニスタが創傷液で満杯になっていることと、創傷液が流れるターゲット導管21にブロッケージが発生したことをユーザに知らせるのに使われるアラームは、別のものであり、判別し易いものであることが望ましい。
減圧状態の減衰は、様々な方法で検出することができる。例えば、バルブ18が開いた後の所定時間における減圧の減少(すなわち絶対圧力の増加)の計測によって、減衰を検出できる。別の例としては、スレッシュホールド圧までの減圧が低下するのに要した経過時間の計測によって、減衰を検出できる。減圧状態の減衰は、バルブ18が開いた後の所定時間における源導管24内の流量の減少を計測することによっても検出できる。減圧状態の減衰を計測するその他の方法も同様に用いられ、この実施例の範囲内にあることを意図している。
図に示す実施例とともに追加の構成要素を用いて、キャニスタ17の満杯状態をより良く判定するようにしてもよい。例えば、キャニスタ17とポーラス状のパッド20を連結しており、その中を創傷液が流れるターゲット導管21に、追加のバルブ25を設けても良い。バルブ18を経て大気中に解放する時にバルブ25を閉じることで、キャニスタ内の容量が空であることを示すようにしても良い。この空容量の表示からの推定と、キャニスタのジオメトリを知ることで、キャニスタ内の液面レベルを判定することができ、また、一定間隔でモニタされるならば、組織部位15からの液の流量を検出してもよい。
図1に加えて図2も参照すると、処理ユニット26は、減圧源19および検知デバイス28ならびにバルブ18を収納もしている治療ユニットハウジング40内に格納されている。処理ユニット26は、同じタイプあるいは異なるタイプの一又はそれ以上のプロセッサで構成される。例えば、処理ユニット26は、一またはそれ以上のプロセッサ、ロジック、アナログ部品、あるいは、システム中の様々な場所で受取る液圧のような情報などの情報を送信することができるその他の電子機器を具えていても良い。
処理ユニット26は、ソフトウェア208を実行し、更に、(i)データやソフトウェア・コードを格納するメモリ210、(ii)バルブや検知デバイスなどのその他のデバイスやシステムと、ワイヤレスあるいはワイヤを介して、あるいはメモリ入力デバイスを介して通信する入出力(I/O)ユニット218、(iii)一またはそれ以上のファイルを有するデータベースなどの、一またはそれ以上のデータレポジトリ226a−226n(集合的に226)を保存するストレージユニット222、(iv)タッチセンサ・タイプである、あるいはこのタイプでない電子ディスプレイ234、及び(v)音響、視覚、あるいはその他の信号を用いて、減圧治療システムのユーザに信号を出すことができる一またはそれ以上のアラーム238、のそれぞれと通信することができる。ソフトウェア208は、その他のデバイス(例えば、電子ディスプレイ234)とそれぞれインターフェイスを取って、減圧治療の管理運営ならびに監視をできるように構成されている。
テストデータ
図3を参照すると、この図は減圧治療システム11と同様のシステムを用いて行なったテストの結果をグラフ表示したものである。このテストにおいては、密閉したキャニスタとして125mmHgの満杯レベルに対する減衰時間の変化が記録されている。キャニスタは、空の状態から50mlづつ水を入れて500mlとなるまで様々な水面レベルにして行って満杯にした。創傷治療用の負圧は125mmHgにセットされ、目標となる圧力を僅かに上回ってポンプが停止するまでシステムを稼動させた。治療をそこで停止して、自然に圧力が漏れて圧力が125mmHgになるまでそのままとした。この時点で、バルブを開いてキャニスタを大気に開放した。圧力曲線は毎秒10サンプルで記録されている。同じテストを標準的な大きさの創傷をシステムに供して行い、この結果を図6のグラフに示す。
図4を参照すると、この図は、様々な圧力の下で大きな創傷を用いて、キャニスタが満杯状態とほぼ満杯状態との間の減衰時間の差異を観察したテスト結果をグラフ表示したものである。標準的な大きさの創傷泡とドレッシングをプラスティックの基台の上に用意して、シリンジで液体を注入して、キャニスタを満杯にしてブロッケージ状態のアラームを発するようにする減圧送達パッドを取り付けた。50mmHgから400mmHgの治療圧で二組の曲線をプロットした。一つは液面レベルが450mlであり、一つはブロッケージアラームが発せられるまでキャニスタを創傷排液一杯にした。この450mlと云う値は、可能な限りの高い充填レベルでありながらブロッケージのアラームを発生させない、という条件に沿った特定のキャニスタが必要であるために選択された値である。この状況は、高レベルでの二つの状態の間でシステムが差別化できた場合、レベルを下げるほど一層容易になるだけであると云う解釈で、最悪のケースのシナリオとなることを予測したものである。
キャニスタは450mlまで満たされ、目標となる圧力を僅かに上回る圧力とした。バキュームポンプが停止して、漏れによって圧力が目標のレベルに下がったので、上述のとおり解放バルブを開いた。ブロッケージ状態となった曲線群用に、創傷に水を噴射し通常の治療条件下で稼動させることによって、キャニスタを満杯にした。このテストの結果を図4に示す。
図5を参照すると、この図は、大きな創傷を用いて満杯状態と、350mmHgの他の液面レベルの間で減衰時間の差異を観察したテスト結果をグラフ表示したものである。図3及び図4のテストと同様に、標準的な大きい創傷を用意して、外部から液を満たして、空の状態から液面レベルが500mlの状態までについて、350mmHgで圧力曲線をプロットした。この処理は上記のテストと同様、直接制御の下で実行した。システムを200mmHgで通常の治療条件下で稼動させ、ブロッケージアラームを発するまで創傷に液を注入された。350mmHgにて直接制御を再開し、最終段階の曲線をプロットした。
テスト結果
図3における密閉キャニスタを用いて得た座標曲線は、満杯レベルと、圧力がゼロおよび5mmHgの双方に低下する時間との間に良好な直線性を示している。減衰の初期段階では多少のクロスオーバが有るが、座標曲線の傾向は、減衰時間が充填レベルに対して概ね比例していることを示すように出ている。
図3に示す結果は密閉キャニスタのものである。図6に反映されているような追加の作業は、創傷が付された際にクロスオーバ効果がかなり低下したことを示している。
図4のグラフは、データの中に明らかなグループ分けが有ることを示している。フィルタに目詰まりが発生したときの満杯状態になったキャニスタでは、セラピー圧に関係なく0.8秒以下で10mmHgを下回って圧力が低下している。キャニスタがほぼ満杯状態ながら目詰まりが発生しない時には、10mmHgまでの所用時間は3秒もしくはそれ以上であった。この差異は、絶対圧力ではなく初期のパーセンテージとなる時間が計測されるならば改善された筈である。
図5は、空の状態から500mlまでの全ての充填レベルにおける減衰時間が、350mmHgからほぼゼロのレベルに降下する実時間を要するものであることを明らかにしている。最も速い減衰速度(500ml時)は4秒後に約20mmHgまで低下し、10秒後に概ね10mmHgであった。比較によればブロッケージ状態に在る曲線は、1秒以下で事実上ゼロ(3mmHg以下)に低下した。
本出願に開示されているシステムと方法は、創傷液キャニスタが満杯になり空になったことを信頼性あるアラームでユーザに知らせることによって、現行の負圧治療システムを改良するものである。液体流路中の圧力をバルブで解放することによって、ここに開示したシステムと方法で負圧の減衰を計測することが可能である。液体流路中の負圧が急速に減少したときには、システムはキャニスタが満杯であると判定し、キャニスタが創傷液で満杯になっていることをユーザに知らせるアラームを起動する。液体流路中の負圧が緩やかに減衰したときには、システムは、システム内に取除くべきブロッケージが起きていると判定し、アラームを起動することによってユーザに知らせる。
ここで検討されている例示の多くが、創傷治療と云う医療分野での本発明の適用例であるが、本発明はまた、医療分野および医療分野以外の用途での吸引および集液機器を含め、あるいはこれに限らず、負圧を用いた他のタイプの適用例にも応用し得る。
当該技術分野の当業者は、何らかの物質を収容する容器について、その満杯状態の表示手段が必要な多岐に亘る範囲に本発明が適用可能である。これまで述べたことから、有意義であり且つ利点のある発明が提示されたことが明らかである。この発明はその態様の幾つかが示されたに過ぎず、これに限定されるものではなく、その主旨から外れることなく様々な変更や改良へ発展し得る。

Claims (5)

  1. キャニスタの満杯状態検出能力を有する減圧治療システムにおいて、当該システムが:
    減圧を提供する減圧源と;
    組織部位に配置され、前記減圧源と流体連結されたポーラス状のパッドと;
    前記ポーラス状のパッドと前記減圧源の間に流体連結されて、前記組織部位から出る液体を回収するキャニスタと;
    前記キャニスタと前記減圧源の間で導管と流体連結されているソレノイドバルブと;
    前記減圧源とポーラス状のパッドの間のブロッケージを検出するよう構成され、ブロッケージ状態が検出された場合に前記ソレノイドバルブを開く処理ユニットと;
    前記ソレノイドバルブが開いたときに、所定時間における前記導管内の圧力減衰を判定する手段と;
    前記圧力減衰に基づいて前記キャニスタの満杯状態を判定する手段と;
    を具えることを特徴とするシステム。
  2. 請求項1に記載のシステムにおいて、前記圧力減衰を判定する手段が更に、前記導管と通信する流量センサを具えることを特徴とするシステム。
  3. 請求項1に記載のシステムにおいて、前記圧力減衰を判定する手段が更に、前記導管と通信する圧力センサを具えることを特徴とするシステム。
  4. 請求項1に記載のシステムにおいて、前記圧力減衰を判定する手段が更に、前記導管内の圧力が所定のスレッショルド圧に至るに要する時間を計測するタイマを具えることを特徴とするシステム。
  5. 請求項1に記載のシステムが更に、キャニスタの満杯状態を表示するために判定された圧力減衰に応じてアラームを発する手段を具えることを特徴とするシステム。
