Description
・生体適合性であり(すなわち、一般に非毒性であり、ヒト及び/又は動物への投与に適し)、
・投与後適切な時間枠内で、例えば標的組織又は標的器官の毛細血管又は細動脈に留まることによって、粒子の動きを遅延させるのに十分な粒子完全性(particle integrity)を維持するのに寄与し、そして
・活性成分を放出するために、及び活性成分を放出した後に、生体により生分解可能である(すなわち、処理又は代謝可能である)。
近年、対象の器官又は組織が損失した細胞の一部を置換することを標的とした、器官移植の代替法としての実験的アプローチが開発されている。これらの手法は半世紀を越えて行われている骨髄移植の成功を手本にしている。免疫適格個体に移植すると、骨髄中の細胞の小集団が全ての血液細胞種を生成することができる能力によって、成体組織が生成能を有する「幹細胞」を含有し、組織又は器官全体を再生することが説得力をもって証明された。この概念的ブレイクスルーによって、治療対象の個体から単離した(自己細胞治療)又は受容する個体とは異なる個体から単離した(異種細胞治療)様々な種類の幹細胞を使用して損傷組織を修復する実験的アプローチの開発に至った。これらの細胞は、大量に単離するか、又は移植前にまず培養液で増殖させて、損傷組織の所望の修復を引き起こす。これらの細胞治療アプローチは、組織再生のために幹細胞の本来の再生特性を利用する。
第一中隔枝の遠位にある冠動脈前下行枝の一時的バルーン閉塞によって、雌ブタに前壁心筋梗塞を引き起こした。この手法によって、再現性のある中程度のサイズの前壁−心尖部梗塞が生じた。インスリン様増殖因子1と肝細胞増殖因子の組み合わせの心筋再生能を、梗塞を起こしたブタの心筋に様々な用量で因子を局所投与することによって試験した。対照動物を偽薬で処置した。
心筋梗塞後様々な時点で、数日〜1ヶ月にわたり心臓を解析した。結果は、ヒトIGF-1及びHGFで処置したブタの虚血領域及びその境界において活性化した幹細胞数及び前駆細胞の数の劇的な増加を示した。虚血領域では筋肉の著しい再生が見られ、新生した細動脈及び血管も含有していた。再生応答は、投与した増殖因子の用量に比例するようであった。これらの予備データから、CSCの治療的in situ活性化は、ヒトの心臓とサイズ及び生体構造において類似する動物の心臓において、広範囲に及ぶ新たな心筋組織形成を引き起こし、並びに左心室機能を有意に改善する可能性がある。
雌のヨークシャー白ブタ(23±4kg;n=3)の様々な心臓領域(右房及び左房、右心室及び左心室並びに心尖部)から複数の心臓試料(各約2g)を得た。一部の試料を固定し、組織化学的解析のためにパラフィンに包埋した。その他の部分を酵素消化し、以前の記載に従い、改変を加えて、心筋細胞を欠失した心臓細胞懸濁液を調製した(Beltrami, A.P.ら, 2003. Cell 114:763)。簡単に説明すると、刻んだ心臓組織を、ハンクス平衡塩類溶液(HBSS)中の0.1%コラゲナーゼ(Worthington Biochemicals)、0.1%トリプシン(Sigma)、0.1%DNAse Iを用いて37℃で消化し、遠心により小さい心臓細胞の画分を回収した。心臓小細胞を、抗ヒトCD117(c-kit)抗体(Miltentyi Biotechnology)とインキュベートし、蛍光励起細胞分取(FACS;MoFlo(Dako Cytomation)セルソーター)又は磁性活性化微小免疫ビーズ(MACS)によって選別した。死細胞を除去するためFACS前にヨウ化プロピジウム(PI;2μg/mL)を添加した。
c-kit陽性細胞を、7〜10日間、細胞2 x 104個/mlで、10%FBS、bFGF(10ng/ml)、インスリン−トランスフェリン−セレナイト(ITS)、及びEPO(2.5U)を含有するDulbecco's MEM/Ham’s F12(DMEM/F12)改変培地中に播種した。回収後、一部の細胞を、心臓球(cardiosphere)生成のため、改変心臓球形成培地(mCSFM):DMEM/F12、bFGF(10ng/ml)、EGF(20ng/ml)、ITS、2-β-メルカプトエタノール(0.1mM)と、B27及びN2サプリメント(Gibco)を添加した神経基本培地との1:1比に移した。クローン形成能を試験するため、単一のc-kit陽性細胞を、フローサイトメトリー又は連続希釈によって、96ウェルのゼラチンコートしたテラサキプレートのウェルに一つずつ播種した。クローンが同定され大きくなる1〜3週間にわたり、個々のc-kit陽性細胞を、DMEM/F12改変培地中で増殖させた。各96ウェルプレートに生成したクローン数を数えることによってckit陽性細胞のクローン形成能を決定し、パーセンテージとして表した。心臓の領域毎に計10個のプレートを解析した。クローン化した細胞及び心臓球を、筋細胞、血管平滑筋及び内皮細胞特異化のための特殊な心臓形成分化培地(42から改変)に移した。
細胞を、ガラスチャンバースライド(BD Falcon)上で2日間培養し、4%PFAで20分間固定し、次いで染色した。細胞内染色では、細胞を0.1%Triton X-100を用いて透過させた。細胞を一次抗体とともに4℃で一晩インキュベートし、3回洗浄し、次いでFITC結合二次抗体又はテキサスレッド結合二次抗体とともに37℃で1時間インキュベートした。その後、細胞を3回洗浄し、核をDAPIで対比染色した。蛍光を可視化し、像を共焦点顕微鏡(Zeiss LSM510)で取得した。細胞染色には以下の抗体を使用した:Oct3/4、Nanog、Isl-1、c-kit、Flk-1、及びNkx2.5(R&D Systems);Bmi-1、c-met及びIGF-1r(Santa Cruz Biotechnology)、テロメラーゼ(Abcam)。ガラスチャンバースライド中で24時間培養した後、心臓球をc-kitについて染色した。球から細胞を増殖及び分化させる培養において、4〜6日後、平滑筋アクチン、α-サルコメアアクチン(Sigma)及びフォンヴィレブランド因子(von Willebrand factor)(DAKO)に対する抗体で染色した。全ての二次抗体は、Jackson Immunoresearch社から購入した。
血清飢餓培地に24時間さらし、その後200ng/ml IGF-1又は200ng/ml HGFを10〜20分間添加したc-kit陽性pCSCから得たタンパク質溶解物を用いて、IGF-1受容体(IGF-1R)及びHGF受容体(c-met)を検出するイムノブロッティングを、以前記載されたように行った(Ellisonら 2007. J. Biol. Chem. 282:11397)。以下の抗体を、製造業者の指示する希釈率で使用した:ウサギポリクローナル抗体IGF-1R、ホスホル-IGF1R、Akt、ホスホル-Akt、c-met(Cell Signalling)、ホスホル-c-met(Abcam)、FAK、及びホスホル-FAK(Upstate)。
心房摘出後、長軸に垂直な切断により心尖部から基部まで、心臓を5個の冠状スライスに分割した。各ブタの各レベルから、梗塞した心筋、梗塞周囲の心筋及び遠位にある心筋の試料を得た。試料をPBSで洗浄し、10%ホルマリンで固定し、パラフィン包埋した。5μm切片をミクロトーム(Sakura)で調製し、顕微鏡スライド上にマウントした。切片を、ヘマトキシリン及びエオシン(H&E)で標準的手法に従って染色した(Ellisonら 2007. J. Biol. Chem. 282:11397)。光学顕微鏡(Nikon E1000M)上でLucia Gソフトウェアを用いて、レベルC及びDに由来する梗塞周囲領域のH&E切片(動物1匹につき3枚のスライド)の核にわたって、筋細胞の直径を測定した。各ブタについて、1切片につき計200個の筋細胞を解析した。
CSCを同定するため、ブタ心臓の横断切片を、幹細胞抗原であるc-kit(ウサギポリクローナル、Dako)に対する抗体で染色した。c-kit陽性CSCは、造血系統、神経系統、及び骨格筋系統(21)のマーカーについて染色が陰性であることにより、系統陰性(Lin陰性)と同定された。対照ブタの様々な心臓領域におけるCSC心筋分布の定量のため、c-kit陽性(lin陰性)細胞数及び心筋細胞数を倍率63倍で計5個の切片についてカウントした。次いで、各横断切片の面積を測定し、単位面積あたりのCSC数及び心筋細胞数を決定した。左心房及び右心房においてc-kit陽性CSC数にわずかな差しか見られなかったため、心房についてのデータをプールした。CSC数を筋細胞106個あたりで表した。
データを平均±SDとして報告する。2群間の有意性を、スチューデントのt検定によって、及び分散分析(ANOVA)による多重比較において決定した。ボンフェローニ事後法(Bonferroni post hoc method)を使用して差を見出した。有意性をP<0.05で設定した。
2セットのPLGA及びアルギン酸塩のマイクロスフェアを調製した。一方のセットはヒト血清アルブミン(HSA)及びインスリン様増殖因子1(IGF-1)の混合物を含有し、もう一方のセットは、HSA及び肝細胞増殖因子(HGF)の混合物を含有する。要する増殖因子がごくわずかな量であることを考えて、乳化のための十分な体積(bulk)を提供するためにHSAを使用した。
投与するマイクロスフェア数を低下させるため、封入効率を最適化するために、以下のパラメーターの変更によって使用する条件を最適化した:
a. - 液相中の乳化剤の取り込み。最適な組み合わせは、5時間まで安定なエマルジョンを生成するTween 80及びSpan 60の混合物であることがわかった。
b. - 5%から20%HSA(ヒト血清アルブミン)へのタンパク質濃度の最適化。
c. - 0.45%から0.2%への水相中のNaCl濃度の最適化。
d. - 5%から5.5%へのEtOAc中のPLGA濃度の最適化。
e. - 初期混合物中のHGF-1濃度を0.4%とした。
マイクロスフェアの製造に使用した試薬及び装置は以下の通りであった:
-アルギン酸塩:プロタナール(Protanal)LF 10/60;FMCBioPolymer(G/M≧1.5);プロタナール LF10/60LS;FMCBioPolymer(G/M≦1)

-液体−気体の設定で単純な噴霧器FF:L2(D=100μm、H=100)及びL3(D=100μm、H=100)
-ハーバードポンプ11プラス。
雌のヨークシャー白ブタ(n=2)(27kg)をテラゾール(terazol)(100mg、I.M.)で鎮静させ、挿管し、毛を剃った。静脈内カテーテルを末梢耳静脈中に入れた。動物を手術室に移動させ、支持板上に置き、肢固定具で手術台に固定した。動物をイソフルレン(O2中2.5%)で麻酔下に維持し、手術中、絶えずEKGをモニターした。透視ガイドのため携帯型放射源(GE STENOSCOP, GE Medical Systems USA)を用いて、本プロトコールのために特別に設計した長さ40 cmの6FガイディングカテーテルJR 3.5を左主冠動脈に挿管した(Cordynamic-Iberhospitex S.A. Barcelona, Spain)。ベースラインの冠動脈造影法を行った。
15μmの蛍光ポリスチレンマイクロスフェア(Invitrogen, Cat # F8842, フルオスフェアズ(FluoSpheres)(登録商標)ポリスチレンマイクロスフェア)のPBS中1x106個/mL濃度の20mL懸濁液を調製し、5分間ボルテックスにかけた。この懸濁液を、左主冠動脈の起始部に、血管造影カテーテルを介して、速度1mL/minのハーバードポンプによって投与した。1mL(マイクロスフェア100万個)投与する毎に、注入を3分間中断し、この間、完全なEKGを行い、冠状静脈洞血液試料を採取した。血液試料を得た直後に、蛍光微粒子の存在を確認するために血液塗抹スライドを調製した。残りの試料を酵素測定のために保存した。心電図が心筋虚血に合致した最小変化を示すまで、処置を続けた。実験の開始時及び粒子懸濁液の投与後に、冠血流量(TIMI)を測定した。2匹のブタを覚醒させ、24時間に再検査し、その後屠殺した。
15μmのマイクロスフェアの安全な用量範囲を決定したため、同じプロトコールを用いて、心筋の同じ領域に10x106個のPLGAマイクロスフェア(直径15μm)を投与した。マイクロスフェア懸濁液は、計2μgのヒト組換えインスリン様増殖因子1(IGF-1)を積載したPLGAマイクロスフェア4x106個;計1μgのヒト組換え肝細胞増殖因子(HGF)を積載したPLGAマイクロスフェア4x106個で構成された。これら2種類のマイクロスフェアは、血中及び組織学的切片中で可視化をより容易にするため、蛍光緑色色素も積載した。さらに、懸濁液は、Invitrogen社の橙色範囲のポリスチレン蛍光2x106個を含有した。Invitrogen社のスフェアは、PLGAマイクロスフェアの安定性及び分布のための対照として機能するために含めた。10mLの生理的PBS中の懸濁液は、上述のように装置を付けたブタに投与した。
上述したように、冠動脈を介して投与したIGF-1及びHGFの組み合わせは、内在心臓幹細胞の活性化を刺激するのに非常に有効であった。この予備的アッセイでは、本発明者らは、マイクロスフェアが送達された領域における幹細胞の活性化をモニターし、左冠動脈によって供給されない左心室の領域と比較した。図11の像に見られるように、未処置の心筋における大部分の内在幹細胞は休止状態にあるのに対し(矢印/矢頭で強調)、処置した領域のものは細胞周期マーカーki-67の発現が示すように細胞周期に入っている(核内の黄色いシグナル、図では強調した領域における明るい「スポット」)。従って、増殖因子を毛細血管に送達し、周囲の間質腔へ荷降ろしするまで増殖因子をその場に保持する固形基体上の増殖因子の投与は、内在性幹細胞集団の刺激に対して有効な増殖因子投与法である。
体重24±3kgの3匹のヨークシャーブタに由来する心筋の組織学的切片を、共焦点顕微鏡で、大部分のCSCで発現することが知られている一般的幹細胞マーカーc-kit(幹細胞因子(SCF)の受容体)について陽性な細胞の存在について調べた。c-kitについて陽性(c-kit陽性)な小さい細胞が、心房及び心室の心筋中に(図1A〜B)、他の心臓領域と比べて心房(左心房と右心房との間に差はない、データ示さず)及び心尖部においてより高密度で分布していた(図1C)。この分布パターンは、ヒトをはじめとする他の動物種の心臓におけるc-kit陽性CSCの解剖学的位置と一致する。従って、ブタの心臓におけるc-kit陽性細胞の密度はヒト及び齧歯動物の心筋と類似している:約1,000個の筋細胞あたり1細胞、又は組織1gあたり約50,000個のc-kit陽性細胞。
この結果は、ブタ心臓における真のpCSCの存在を示す。
全ての動物実験は、Escuela Veterinaria y Hospital de Leon、レオン、スペインの適切な委員会によって認証された。雌のヨークシャー白ブタ(n=26)(27±3kg)をテラゾール(100mg、I.M.)で鎮静させ、挿管し、毛を剃った。静脈内カテーテルを末梢耳静脈中に入れた。動物を手術室に移動させ、支持板上に置き、肢固定具で手術台に固定した。動物をイソフルレン(O2中2.5%)で麻酔下に維持した。26匹の動物全てにおいて、冠動脈バルーンカテーテルをガイドワイヤーに沿って進め、第一貫通動脈の起始部直下の、左前冠動脈(LAD)の近位部に位置した。梗塞を誘導する前に125UI/kgのヘパリンをブタに与え、その後、梗塞術中にヘパリン点滴(10UI/kg/h)した。梗塞を誘導するため、LAD冠動脈をバルーン拡張(2.5mm直径)によって75分間閉塞した。抗不整脈剤として、アミオダロナ(Amiodarona)(トランゴレックス(Trangorex))(5mg/kg/h)を梗塞の15分前に開始して手術中、ブタに絶えず点滴した。心室性期外収縮又は心室細動が多い場合には、1〜3mg/kgのリドカインを静脈内投与した。手術1日前に、75mgクロピドグレル(プラビックス)及び250mgアスピリンの術前医薬を投与した。術後医薬は、屠殺まで毎日の75mgクロピドグレル(プラビックス)及び125mgアスピリンで構成された。
心臓機能を、ベースライン、冠動脈閉塞直後及び屠殺前に心電図で測定した。簡単に説明すると、Mモード及び2次元エコー図を用いて、胸骨傍の長軸及び短軸図を得た。LV径(LV dimension)(LVEDD及びLVESD)を中弦レベル(midchordal level)で心室の長軸に対して垂直に測定した。LV駆出率及び放射方向のストレイン(radial strain)を計算した。
ヒト組換えIGF-1及びHGF(以降IGF-1/HGF又はGFと略す)を、様々な用量でブタに冠動脈内注入によって急性心筋梗塞30分後に投与した。別のブタに、生理食塩水のみの同じ容量を注入し、対照群(CTRL)を構成した。
正常心臓(示さず)及びMI後の心臓では、免疫組織化学によって検出されるように、in situでc-kit陽性pCSCの約90%がIGF-1受容体及びc-met(HGF)受容体を発現する(図4A〜B)。その結果、GF処置した梗塞ブタ心臓は、MI21日後に、境界領域においてc-kit陽性pCSC数の有意な増加を示し、梗塞領域でははるかに大きな増加を示す(図4C〜D)。c-kit陽性pCSCのこの増加がGF投与の結果であることは、投与したGF用量との直接的相関によって確かめられる(図4D)。最も高いGF用量では、梗塞領域におけるc-kit陽性pCSCの数は、CTRL心臓に比べて、>6倍多い(図4D、補足表)。さらに、1x用量と4x用量との間の直線的増加は、最大の再生応答を引き起こす飽和用量には達していないことを示唆している。pCSCの多くがBrdU陽性であったが、このことは、MI発生後にそれらが生まれたことを実証している(図4E)。それらの周期する性質は、現在細胞周期にある、又は最近細胞周期にあった細胞を標識するKi-67染色によって確かめた(データ示さず)。多くのc-kit陽性細胞は、主要心臓系統、すなわち筋細胞、内皮細胞及び平滑筋細胞への分化の指標となる転写因子Nkx-2.5、Ets-1又はGata6を発現していた(図4F〜I)。定量解析によって、c-kit陽性Nkx2.5陽性細胞(筋細胞/血管前駆体細胞へ運命決定している)の数は、GF処置したブタ心臓の梗塞領域及び境界領域においてGF用量依存的に有意に増加することが明らかになり(図4G)、CTRL心臓におけるより>10倍高いレベルに達した。
GF処置心臓は、梗塞領域及び梗塞周辺/境界領域のいずれにおいても、まだ最終分化段階に達していない、非常に小さい新生BrdU陽性筋細胞を多数有していた(図5)。これらのデータは、小さい新生筋細胞中のKi67の発現によって確かめられ(図5C及びF)、これらの一部は有糸分裂及び細胞質分裂にあり、それらの未成熟な性質を裏付けた(図5I)。新生BrdU陽性筋細胞は、未処置の生理食塩水を注入したCTRLブタの梗塞周辺/境界領域にも存在した。しかし、それらの数は、処置した心臓の約1/10であり、梗塞領域にはほとんど存在しなかった(図5)。
心エコー画像は、左心室駆出分画率(LVEF)が、冠動脈閉塞後、CTRLブタ及びGF処置ブタで有意に低下することを示した(図6G)。しかし、AMI28日後、LVEFはCTRLでわずかに悪化したのに対し、CF処置により、CTRLと比べると有意に維持/改善された(図6G)。部位的心臓機能についてさらに洞察を得るために、組織ドップラー心エコー検査を用いて、前壁中隔の放射方向のストレイン(antero-septal radial strain)を測定したところ、CTRLに比べてGF処置ブタでは有意に改善していた(図6H〜I)。心臓機能の維持/改善は、GF用量の増加と相関した(図6)。
実施例5で示したように、直径15μmのマイクロスフェアの≧99%は標的組織、及び具体的には心筋の毛細血管ネットワークにトラップされる。しかし、これらのデータは、活性分子が放出されると組織内に保持されるかどうか、又はそれが毛細血管循環及び静脈還流に浸出するかどうかという問題には対処しない。この問題を調べるため、実施例5〜7に述べた同じ投与プロトコールに従って、計50μgのrhIGF-1を積載したマイクロスフェア5 x 106個を左前下行枝の起始部に冠動脈投与した。主な違いは、頸静脈を通して冠状静脈洞にカテーテルを残したことであった。投与中、手術3時間後及び翌3日間にわたり12時間毎に、血液試料を冠状静脈洞から、及び耳静脈を介して静脈血から回収した。血清を調製し、回収完了まで試料をLN2中で凍結した。全ての試料を、ブタIGF-1と交差反応しないヒトIGF-1検出キット(R&D, Minneapolis, Minnesota, USA)を用いたELISAにより解析した。冠状静脈洞由来の試料も、体静脈還流由来の試料も陽性を示さなかった。本発明者らの方法では、アッセイの最小検出限界はIGF-1について52.5ng/mlであった。従って、ELISAの検出レベル未満の漏出が生じた可能性はあるものの、IGF-1の大部分が心筋を離れなかったことは明らかである。
損傷した心筋を治療するために使用したプロトコールが他の組織の治療において有効かどうか試験するために、同じプロトコールを用いて、大腿動脈の45分間の完全バルーン閉塞によって虚血性損傷を引き起こした3匹のブタの右肢の虚血後の骨格筋を治療した。心筋の場合のように、バルーンの収縮による再かん流の30分後、総用量8μgのIGF-1及び2μgのHGFについて実施例2に記載するように調製した、直径15μmのIGF-1及びHGFマイクロスフェアを含有するPBS20mLの懸濁液を投与した。動物を3週間後に屠殺し、大腿四頭筋の生検を免疫組織学によって解析し、病変における幹細胞の活性化の程度を決定した。
上の実施例に記載したプロトコールに新たな因子を付加することにより梗塞後の心筋の再生応答が改善するかどうか試験するために、3匹の動物群に実施例9で使用した高用量のIGF-1(8μg)及びHGF(2μg)を4μgのSCFとともに投与した。実施例2に記載したように、これらの因子はそれぞれ直径15μmのPLGAマイクロスフェアに封入した。梗塞の発生、モニタリング及びマイクロスフェア懸濁液の投与についてのプロトコールは実施例5〜7に記載した通りであった。動物を処置4週間後に屠殺した。

(A)新生動脈構造(BrdU、白色;α-平滑筋アクチン、SMA、白色;ミオシン重鎖、MHC、灰色;DAPI、青色)は、GF処置したブタ心臓の梗塞領域において顕著であった。(B〜C)新生毛細血管も、IGF-1及びHGF注入後、梗塞領域で顕著であった(BrdU、白色;vWF、灰色;DAPI、濃灰色)。(D〜F)GF処置ブタにおける毛細血管数は、生理食塩水処置(濃灰色染色)CTRLと比べて、梗塞領域において有意に増加した。CTRLに対して*p<0.05、IGF-1/HGF 1xに対して†p<0.05、IGF-1/HGF 2xに対して‡p<0.05。像(倍率20倍)は、生理食塩水処置CTRL(D)及びGF-4x(E)処置した心臓におけるvWF染色(濃灰色)を示す。毛細血管は、1又は2個の内皮細胞からなる血管と定義した。(G〜H)GF処置心臓は、生理食塩水処置CTRLと比べて、左心室(LV)駆出分画率(G)及び放射方向のストレイン(H)の改善を示した。ベースラインに対して*p<0.05、AMIに対して#p<0.05、CTRLに対して†p<0.05、GF-1xに対して‡p<0.05。(I)CTRL(a〜c)及びGF-4x(d〜f)処置ブタから明らかとなった代表的な組織ドップラー放射方向のストレイン(Tissue Doppler radial strain)。CTRL(b)及びGF-4x(e)処置ブタは、90分間の冠動脈閉塞(AMI)後、前壁中隔収縮の同程度の脱同調を有した。屠殺時に(MI後)、脱同調収縮はCTRL(c)で悪化したのに対し、GF処置(f)ブタでは改善した。
対照及び損傷した大腿四頭筋の組織学的像。パネルA:右の筋肉に病変を引き起こした5日後の左の筋肉(対照)の組織学的像。この肢には治療薬を投与していない。パネルB:損傷を引き起こした5日後の治療していない右大腿四頭筋の組織学的切片(損傷対照)。矢頭は、細胞壊死のいくつかの広範な領域のうち2つを示しており、ここでは再生応答を開始するための核の凝縮が見られる。パネルC:計16μgのIGF-1及び4μgのHGFの投与に相当するIGF-1を積載したマイクロスフェア及びHGFを積載したマイクロスフェアの混合物で処置した、損傷3日後の右大腿四頭筋の右の生検。矢頭は、まさに再生過程にある損傷領域中の若い微小繊維を指し示す。パネルD:パネルCに示される同じ筋肉の2日後(損傷5日後)の生検。小さな色の濃い繊維は、再生した繊維のマーカーである胚性ミオシン重鎖に対する抗体で標識された再生した繊維である。このパネルの像は、パネルBの像に対応する。2つの像の顕著な違いは、治療の有効性を示す。
図13Aの棒グラフは、2種類のマイクロスフェアの組み合わせで処置したAMI後のブタ(白棒)(一方はIGF-を積載し、もう一方はHGFを積載)と、3種類のマイクロスフェア(hrIGF-1、hrHGF及びhrSCF)の組み合わせで処置した動物(黒棒)において、再生した心臓筋細胞の数における効果を比較する。使用した3つの異なる濃度において、それぞれ異なる因子を積載した3種類のマイクロスフェアの組み合わせが、2種類のみの組み合わせより優れていることは明らかである。CTRL=偽薬で処置した対照動物;白棒:1X=生物学的に活性な2μgのIGF-1及び0.5μgのHGF相当を積載したマイクロスフェアを投与した動物;2X=4μgのIGF-1及び1μgのHGF、並びに4X用量=8μgのIGF-1及び2μgのHGF。黒棒:白棒で表す動物に対するのと同量のIGF-1及びHGFに加えて、2、4及び8μgの生物学的に活性なhrSCF相当のSCFを積載したマイクロスフェア。
Claims (13)
- 活性成分及び生分解性賦形剤を含有する球状粒子を含む、標的組織の上流への動脈内投与用医薬製剤であって、
該粒子の平均直径が15μmであり、該粒子の99%以上が15±1μmの直径を有し、
該15±1μmの直径によって、標的組織の上流に医薬製剤を投与した後にその標的組織に粒子が保持され、
該製剤は直径50μmを超える粒子及び直径5μm未満の粒子を実質的に含まず、
その結果、該製剤が標的組織の上流に投与された場合に、活性成分が標的組織を通過して体循環に入る能力が、標的組織の毛細血管への該粒子のトラップによって制限される、前記医薬製剤。 - 活性成分が、HGF(肝細胞増殖因子);IGF-1を含むIGF(インスリン様増殖因子);PDGF-βを含むPDGF(血小板由来増殖因子)、aFGF(FGF-1)又はbFGF(FGF-2)及びFGF-4を含むFGF(線維芽細胞増殖因子);SDF-1(ストロマ細胞由来因子1);EGF(上皮増殖因子);VEGF(血管内皮増殖因子);エリスロポエチン(EPO);TGFβ(トランスフォーミング増殖因子β);G-CSF(顆粒球・コロニー刺激因子);GM-CSF(顆粒球・マクロファージ・コロニー刺激因子)、骨形成タンパク質(BMP、BMP-2、BMP-4);アクチビンA;IL-6;NGF(神経増殖因子)、BDNF(脳由来神経栄養因子)、NT-3(ニューロトロフィン-3)、NT-4(ニューロトロフィン-4)及びニューロトロフィン-1を含むニューロトロフィン;TPO(トロンボポエチン);GDF-8(ミオスタチン);GDF9(増殖分化因子-9);並びにペリオスチンからなる群より選択される増殖因子である、請求項1に記載の医薬製剤。
- HGF、IGF-1、及びそれらの組み合わせからなる群から選択される活性成分を含有する、請求項2に記載の医薬製剤。
- 活性成分としてSCFを含有する粒子、活性成分としてIGF-1を含有する粒子、及び活性成分としてHGFを含有する粒子を含む、請求項1に記載の医薬製剤。
- 活性成分の濃度が粒子1 x 106個あたり1ng〜粒子1 x 106個あたり4mgまでの範囲である、請求項1〜4のいずれか1項に記載の医薬製剤。
- 標的組織が心臓組織である、請求項2〜5のいずれか1項に記載の医薬製剤。
- 心臓幹細胞を刺激することによる心臓組織の再生に使用するための請求項6に記載の医薬製剤。
- 心筋梗塞(MI)、又は心筋梗塞を有するかもしくは有していない急性もしくは慢性の虚血性心疾患の治療用の、請求項7に記載の医薬製剤。
- 哺乳動物における固形組織の再生に使用するための請求項2〜4のいずれか1項に記載の医薬製剤。
- 生分解性賦形剤が、ポリ乳酸、ポリグリコリド、ポリ乳酸-グリコール酸コポリマー、ポリカプロラクトン、ポリヒドロキシ酪酸、ポリウレタン、多糖、タンパク質、ポリアミノ酸、炭水化物、キトサン、ヘパリン及びポリヒアルロン酸からなる群より選択される、請求項1〜9のいずれか1項に記載の医薬製剤。
- 生分解性賦形剤が、ポリ乳酸-グリコール酸コポリマー(PLGA)及びアルギン酸塩からなる群より選択される、請求項1〜9のいずれか1項に記載の医薬製剤。
- 活性成分としてのHGF又はIGF-1及び生分解性賦形剤を含有する粒子を含む医薬製剤であって、
該粒子の平均直径が15μmであり、該粒子の99%以上が15±1μmの直径を有し、
該15±1μmの直径によって、標的組織の上流に医薬製剤を投与した後にその標的組織に粒子が保持され、
該製剤は直径50μmを超える粒子及び直径5μm未満の粒子を実質的に含まない、前記医薬製剤。 - 活性成分としてHGF又はIGF-1を含有する、心筋梗塞(MI)、又は心筋梗塞を有するかもしくは有していない急性もしくは慢性の虚血性心疾患の治療のための、又は哺乳動物における固形組織の再生のための医薬製剤であって、該製剤は活性成分及び生分解性賦形剤を含有する粒子を含み、
該粒子の平均直径が15μmであり、該粒子の99%以上が15±1μmの直径を有し、
該15±1μmの直径によって、標的組織の上流に医薬製剤を投与した後にその標的組織に粒子が保持され、
該製剤は直径50μmを超える粒子及び直径5μm未満の粒子を実質的に含まない、前記医薬製剤。
