Description
更なる別例においてステント構造体は1つの略管状のステントを使用する。ステントは特別な側枝開口部を有し、開口部を通じて付加的なステントや構造体要素が設けられる。これらのシステムの多くは分岐を有するステントの主枝及び側枝を配置するために、2つのバルーンからなるシステムを有するステント搬送アセンブリ使用する。
上述した情報は、ここで言及される当該情報が本発明に関する「従来技術」であることを容認することを意図したものではない。
特に本発明は主枝及び側枝開口部の両者を有する分岐ステントを容易に配置できるように構成される1つの拡張可能な医療用バルーンに関する。
バルーンは少なくとも1つの内側層及び少なくとも1つの外側層を備え、外側層は少なくとも部分的に通過して延びる腔を有する。腔は内側層の外側表面が暴露されるべく開口部を形成するように外側層を貫通して延びてもよい。一実施例において、腔は完全に外側層を通過して延び、これにより内側層が暴露される開口部を形成する。腔により内側層は通過して延びることが可能となる。
ステントの搬送及び配置の際にステントは、ステントの側枝開口部がバルーンの外側層の腔の上方に配置されるようにバルーンの上方に配置される。拡張時に多層からなるバルーンの少なくとも1つの内側層は少なくとも1つの外側層の腔内を通過して突出する。これによりステントの側枝を血管の分岐部の副次的な血管の小孔内に拡張する。
以下図面を参照する。図1は本発明において非拡張状態、即ち静的状態にあるバルーン10を示す長尺方向断面図である。ここで使用されるように、用語「静的」は収縮及び拡張される前の、形成されたままの状態のバルーンを示すべく使用される。例えば、モールド成形されたバルーンにおいて静的状態はバルーン型枠から取り払われたバルーンの状態である。
図1,1A,2,2Aに示すバルーン10は血管の分岐部にて使用可能な主枝及び側枝開口部を有するステントの搬送に特に有用である。
図9乃至11に示す別例の工程において、多層管状パリソン15は共押出等の上述したような好適な技術を使用して形成される。図9は内側層18及び外側層20を有する多層管状パリソン15を示す長手方向部分断面図である。内側層18は内側表面17及び外側表面19を有する。管状パリソンはまず層からなるパリソンの共押出等の周知の方法を使用して形成可能である。或いは管がまず形成され続いて相互に挿入されるチューブ−イン−チューブ法を使用して形成可能である。これらの方法を使用して、材料は外側層20の部分的な深みを備える腔22又は外側層を貫通して内側層の外側表面を暴露する開口部を形成する腔22を残したまま管状パリソン15から取り除かれる。材料は周知の技術によるレーザーアブレーション、化学的エッチング、グラインド等の好適な技術を使用して取り除かれる。
管状パリソン15はバルーン型枠内に配置され、径方向に拡張され、図11に示すようにバルーン10となる。
形成されるとバルーンは周知の技術のようにその長軸の周囲にて折り畳まれ巻き付けられる。
更に、所望の場合に上述した工程を変形し、外側層の2つ以上の腔と内側層から腔への2つ以上の突出部を設けてもよい。
内側層及び外側層に対して好適なバルーン材料が使用可能である。好適には外側層は内側層とは異なる材料から形成される。好適には少なくとも1つの内側層は外側層のコンプライアンスとは異なるコンプライアンスを有する材料から形成される。即ち外側層より大きなコンプライアンスを有するか、外側層より小さなコンプライアンスを有する。
リブチレンテレフタレート(PBT)等のポリアルキレンテレフタレート、ポリイミド、熱可塑性ポリイミド、ポリアミド、ポリエステル、ポリカーボネート、ポリフェニレン・サルファイド樹脂、ポリプロピレン、剛性を備えたポリウレタンの他これらの混合物が挙げられるが、これらに限定されるものではない。
本発明による拡張可能なバルーン部材は血管系の疾患、特に血管の分岐部における疾患の処置に有用である。これらのバルーンが使用される処置は例えば風船療法(plain old balloon angioplasty; POBA)及び経皮的バルーン冠動脈形成術(percutaneous transluminal coronary angioplasty; PTCA)の他、ステント搬送等の移植可能な医療器具の搬送を含む。
側枝の配置に影響するその他の要因として腔の形状及び寸法、並びに腔領域の外側層の厚みが挙げられる。
Claims (13)
- 分岐した血管の処置に有用な拡張可能な医療用バルーンであって、同バルーンは本体部と、胴体部と、円錐部とを備えるとともに非拡張状態及び少なくとも1つの拡張状態を有し、前記バルーンは非拡張状態において内側表面及び外側表面を有する少なくとも1つの連続した内側層と、同内側層と一体的に形成される少なくとも1つの外側層とを備え、前記バルーンの本体部の外側層は外側層を通過して延びる腔を含み、同腔は、外側層を通過して延び内側層の外側表面を暴露する開口部であり、バルーンが該少なくとも1つの拡張状態にある場合に、前記少なくとも1つの内側層は少なくとも1つの外側層の腔を通過して突出し、前記バルーンは非拡張状態及び拡張状態において前記内側層及び前記外側層を備えることを特徴とする拡張可能な医療用バルーン。
- 前記少なくとも1つの外側層はエチレン、ポリアミド、ポリアルキレンテレフタレート、ポリイミド、ポリエステル、共重合ポリエステル、ポリカーボネート、ポリフェニレン・サルファイド樹脂、ポリプロピレン、ポリウレタンの単独重合体、共重合体、及び三元重合体、並びにこれらの混合物からなる一群から選択される少なくとも1つの材料から形成されることを特徴とする請求項1に記載の拡張可能な医療用バルーン。
- 前記内側層はエチレン、ポリエステル、共重合ポリエステル、ポリウレタン、ポリカーボネート、ポリアミド、ポリ塩化ビニル、アクリロニトリルとブタジエンとスチレンの共重合体(ABS樹脂)、ポリエーテル−ポリエステル共重合体、ポリエーテル−ポリアミド共重合体、スチレンのエンドブロック並びにイソプレン、ブタジエン、エチレン−プロピレン、イソブチレン、及びエチレン−ブチレンのミッドブロックを有するブロック共重合体、ポリエステル−エステルエラストマー、ポリエーテル−エステルエラストマー、ポリエーテル−エーテルケトン、ポリエーテルブロックアミド、並びにこれらの混合物からなる一群から選択されることを特徴とする請求項1に記載の拡張可能な医療用バルーン。
- 前記少なくとも1つの外側層は同外側層が第1のコンプライアンスを備えるような材料から形成されることと、前記内側層は同内側層が第2のコンプライアンスを備えるような材料から形成されることと、同第2のコンプライアンスは第1のコンプライアンスより小さいこととを特徴とする請求項1に記載の拡張可能な医療用バルーン。
- 前記少なくとも1つの外側層は同外側層が第1のコンプライアンスを備えるような材料から形成されることと、前記内側層は同内側層が第2のコンプライアンスを備えるような材料から形成されることと、同第2のコンプライアンスは第1のコンプライアンスより大きいこととを特徴とする請求項1に記載の拡張可能な医療用バルーン。
- 前記少なくとも1つの外側層に1つ以上の腔を有することと、前記内側層は各腔内に突出することとを特徴とする請求項1に記載の拡張可能な医療用バルーン。
- 分岐したステントと組み合わされることと、同分岐したステントは主枝及び側枝を有し、且つ拡張状態に変化することと、ステントの側枝は内側層が通過して突出する腔に対向することと、拡張可能な医療用バルーンの内側層は拡張状態においてステントの側枝開口部内に突出することとを特徴とする請求項1に記載の拡張可能な医療用バルーン。
- ステント搬送カテーテルと組み合わされることを特徴とする請求項6に記載の拡張可能な医療用バルーン。
- 請求項1に記載の拡張可能な医療用バルーンの製造方法であって、該医療用バルーンは少なくとも1つの内側層と少なくとも1つの外側層とを有し、前記製造方法は、
第1の高分子材からなる前記少なくとも1つの内側層を備える管状パリソン形成工程にて、内側層が内側表面及び外側表面を有し、前記少なくとも1つの外側層が第2の高分子材からなる、管状パリソン形成工程と、
同管状パリソンをバルーン型枠に配置する工程と、
多層からなる拡張可能なバルーンを形成すべく管状パリソンをバルーン型枠内にて径方向に拡張する工程とを含み、内側層の突出部が該少なくとも1つの外側層の少なくとも1つの腔に形成され、
該少なくとも1つの腔は、外側層を通過して延び内側層の外側表面を暴露する開口部であることを特徴とする、拡張可能なバルーンの製造方法。 - 前記外側層の腔は内側層の外側層が暴露される開口部であることを特徴とする請求項9に記載の方法。
- 前記管状パリソン形成工程は前記少なくとも1つの内側層と前記少なくとも1つの外側層とを共押出する工程を含むことを特徴とする請求項10に記載の方法。
- 前記少なくとも1つの外側層はエチレン、ポリアミド、ポリアルキレンテレフタレート、ポリイミド、ポリエステル、共重合ポリエステル、ポリカーボネート、ポリフェニレン・サルファイド樹脂、ポリプロピレン、ポリウレタンの単独重合体、共重合体、及び三元重合体、並びにこれらの混合物からなる一群から選択される少なくとも1つの材料から形成されることを特徴とする請求項11に記載の方法。
- 前記内側層はエチレン、ポリエステル、共重合ポリエステル、ポリウレタン、ポリカーボネート、ポリアミド、ポリ塩化ビニル、アクリロニトリルとブタジエンとスチレンの共重合体(ABS樹脂)、ポリエーテル−ポリエステル共重合体、ポリエーテル−ポリアミド共重合体、スチレンのエンドブロック並びにイソプレン、ブタジエン、エチレン−プロピレン、イソブチレン、及びエチレン−ブチレンのミッドブロックを有するブロック共重合体、ポリエステル−エステルエラストマー、ポリエーテル−エステルエラストマー、ポリエーテル−エーテルケトン、ポリエーテルブロックアミド、並びにこれらの混合物からなる一群から選択されることを特徴とする請求項11に記載の方法。
