Description
13.ガイドワイヤをシースから取り外す。
概して、本発明は、先に記載したような先行技術の尿管アクセスシースの上記の欠点に対処する尿管アクセスシースを含む。先行技術の装置と同様に、提案される尿管アクセスシースは、2つの別々の部品、すなわち、シースと拡張器を含む。しかし、提案されるシースは、複数の管腔(主作業チャネル、及び、1以上の補助管腔)を有するという点で、周知の先行技術の装置と異なる。尿管アクセスシースに一体化された補助管腔が、手術野を洗浄するために用いられることができる。従って、洗浄のために、尿管鏡の作業チャネル(一般に、手術器具により占有される)に依存する必要がない。さらに、補助管腔は、安全ワイヤを収容するために用いられることができる。ガイドワイヤは尿管アクセスシースの主作業チャネル内に受け入れられる。安全ワイヤは、補助管腔を通して導入されることができ、この一方で、第1(主)ガイドワイヤは作業チャネルから、例えば、尿管鏡を収容するために除去される。安全ワイヤは、補助管腔を介して、適切な位置に簡単に保持されることができる。或いは、安全ワイヤは、既存の実施例と同様に、シースの外側に保持されることもできる。
Claims (15)
- 内視鏡を受け入れるように構成された主管腔(96)と前記主管腔の少なくとも一部に沿って延びる補助管腔(100)とへのアクセスをもたらす複数管腔ハブ(18)と、
前記主管腔及び前記補助管腔を含んで身体内の通路に挿入されるように構成された細長いシースチューブ(16)を有し、前記複数管腔ハブから延びる細長いシースと
を含むシース組立体(12)と、
前記細長いシースチューブの前記主管腔内に挿入されるように構成された拡張器チューブ(26)を含む拡張器組立体(14)と
を含むアクセスシース(10)であって、
前記主管腔の壁は、前記主管腔を前記補助管腔から分離するように前記複数管腔ハブに連結し、
前記拡張器チューブは、ガイドワイヤを受け入れるように構成された内部通路を含み、
前記身体内の通路内に配置されたガイドワイヤ上で前記シース組立体及び前記拡張器組立体を前進させた後に前記ガイドワイヤ及び前記拡張器組立体を前記シース組立体から除去することにより、前記シース組立体が前記身体内の通路に配置されるように構成される、アクセスシース。 - 前記補助管腔が前記主管腔のほぼ全長に沿って延びる、請求項1に記載のアクセスシース。
- 前記補助管腔が前記細長いシースチューブの遠位端に近接する地点を終端とする、請求項1に記載のアクセスシース。
- 前記補助管腔が前記主管腔よりも小さい、請求項1に記載のアクセスシース。
- 前記補助管腔が手術器具を受け入れるように構成された、請求項1に記載のアクセスシース。
- 前記補助管腔が、安全ワイヤ、レーザファイバ、ストーンバスケット及び把持具の少なくとも1つを受け入れるように構成された、請求項5に記載のアクセスシース。
- 前記補助管腔が洗浄流体をターゲット部位に配送するように構成された、請求項1に記載のアクセスシース。
- 前記主管腔が細長いチューブ(90)により画成され、前記補助管腔が、前記細長いチューブ上に設けられたU字状ハウジング(98)により画成された、請求項1に記載のアクセスシース。
- 前記主管腔の断面がほぼ円形であり、前記補助管腔の断面がほぼ三日月形状である、請求項8に記載のアクセスシース。
- 前記拡張器組立体が、前記拡張器チューブの内部通路へのアクセスをもたらすルアー(28)を含む、請求項1に記載のアクセスシース。
- 前記ルアーがロックタブ(29a,29b)を含み、前記ロックタブ(29a,29b)が前記ハブのロックスロット(41a,41b)により受け入れられ、前記拡張器組立体を前記シース組立体にロックする、請求項10に記載のアクセスシース。
- 前記シース組立体が、前記主管腔の少なくとも一部に沿って延びる追加の補助管腔(100B)をさらに含む、請求項1に記載のアクセスシース。
- 前記追加の補助管腔が、前記細長いシースチューブの遠位端に近接する地点を終端とする、請求項12に記載のアクセスシース。
- 前記補助管腔が、前記主管腔の両端部から軸方向に内側に移動した位置を両終端とする、請求項1に記載のアクセスシース。
- 前記ハブが、ほぼ円筒状の本体部(30)と、前記本体部から分岐するフィッティング(36)とを含み、
前記本体部が、長円形の前方チャンバ(54)と、前記チャンバと連通する円筒状ボア(56)とを有し、前記フィッティングが、前記チャンバに向って開放された円筒状ボア(64)を有する、請求項1に記載のアクセスシース。
