Description
本発明は、脈管閉塞に関するものであり、特に、動脈瘤内に脈管閉塞装置を保持する装置に関する。
様々なインプラント可能な医療装置が、身体の内腔に関連する多くの病気の治療用に開発されている。特に、閉塞装置は血管または体内のその他の空間を塞ぐのに有益である。血管動脈瘤など、いくつかの体内の空間は、動脈壁が弱くなるために形成される。これらの動脈瘤は、多くの場合内出血部位及び梗塞部位である。様々な塞栓物質が、少なくともほぼ間違いなく、これらの異常に対する好適な治療法として知られている。これらの治療法は、一般的に「人工脈管閉塞術」として知られている。
本発明は、動脈瘤頚及び動脈瘤内壁を有する動脈瘤を閉塞する装置に関する。一般的に、本発明にかかる装置は、ニッケルチタニウム(NiTi)などの形状記憶合金で一体的に構成されたメッシュ状構造を有する。この装置は、動脈瘤頚を介して動脈瘤内で高温にさらすことによって及び/又は圧縮力か展開する。この装置は、展開状態もしくは非展開状態に形成することができ、更に、よりら開放されることによって、非展開状態から展開状態に遷移するように形成することができる。この装置は、脈管閉塞コイル、及び/又は液体塞栓物質などのより微細な脈管閉塞装置を保持するように機能することができる。更に、この装置自体が、脈管閉塞装置として機能する。この装置は、例えばカテーテルによって、動脈瘤へ送出することができる。
図1を参照すると、本発明の好ましい実施例によって構成された脈管閉塞エレメント10は、血管60の動脈瘤40内で完全に展開された状態で示されている。動脈瘤40は、対向して位置する頚部70とドーム45を有する。二次的形状に展開されたとき、つまり展開状態にあるとき、脈管閉塞エレメント10は、通常、中央管状エレメント15と、近位端20及び遠位端30を具え、これらは傘状に開く。図に示すように、広がった端部20/30は、動脈瘤40の形状に合致する。脈管閉塞エレメント10の近位端20の径は、頚部70の径より大きく、したがって、近位端20は、頚部70を完全に覆うことができる。
Claims (11)
- 動脈瘤頚部と動脈瘤内壁を有する動脈瘤を閉塞するのに使用される脈管閉塞装置において、
全体が形状記憶合金で形成され、近位端と、遠位端と、中央管状エレメントと、を具えたメッシュ状構造を備え、前記近位端及び前記遠位端は、前記中央管状エレメントの反対側の端部に形成され、前記遠位端は、非展開形状のときに前記動脈瘤頚部を介して挿入され、展開形状のときに前記動脈瘤内壁の一部に対向して広がって開くように構成され、前記近位端は、非展開形状のときに動脈瘤頚部を介して挿入され、展開形状のときに前記動脈瘤に隣接した前記動脈瘤内壁の一部に対向して平坦ディスク形状または広がりながら開いた端部形状に開くように構成されている、ことを特徴とする脈管閉塞装置。 - 動脈瘤頚部と動脈瘤内壁を有する動脈瘤を閉塞するためのカテーテルアッセンブリにおいて、
請求項1に記載の脈管閉塞装置;と、
脈管閉塞装置のメッシュ状構造に接続された遠位端を有する送出カテーテル;と、
を具えることを特徴とするカテーテルアッセンブリ。 - 請求項2に記載のカテーテルアッセンブリにおいて、前記メッシュ状構造が、電解的に切断可能な方法、引き出し可能なシースを用いた方法、及び押し出し法からなる群から選択された方法を用いて、前記動脈瘤へ送出することが可能であることを特徴とするカテーテルアッセンブリ。
- 請求項1に記載の脈管閉塞装置または請求項2に記載のカテーテルアッセンブリにおいて、前記メッシュ状構造の遠位端が、熱活性によって展開状態に開くように構成されていることを特徴とする脈管閉塞装置またはカテーテルアッセンブリ。
- 請求項1に記載の脈管閉塞装置または請求項2に記載のカテーテルアッセンブリにおいて、前記メッシュ状構造の遠位端が、圧縮力が開放されたときに展開状態に開くように構成されていることを特徴とする脈管閉塞装置またはカテーテルアッセンブリ。
- 請求項1から5のいずれか1項に記載の脈管閉塞装置またはカテーテルアッセンブリにおいて、前記メッシュ状構造が、複数のブレード状の微細なワイヤから形成されていることを特徴とする脈管閉塞装置またはカテーテルアッセンブリ。
- 請求項1から6のいずれか1項に記載の脈管閉塞装置またはカテーテルアッセンブリにおいて、前記メッシュ状構造が、NiTi合金でできていることを特徴とする脈管閉塞装置またはカテーテルアッセンブリ。
- 請求項6に記載の脈管閉塞装置またはカテーテルアッセンブリにおいて、前記微細なワイヤが異なる径を有することを特徴とする脈管閉塞装置またはカテーテルアッセンブリ。
- 請求項6に記載の脈管閉塞装置またはカテーテルアッセンブリにおいて、前記微細なワイヤの一つかそれ以上が放射線不透過物質でなることを特徴とする脈管閉塞装置またはカテーテルアッセンブリ。
- 請求項1から9のいずれか1項に記載の脈管閉塞装置またはカテーテルアッセンブリにおいて、前記メッシュ状構造が放射線不透過物質で被覆されていることを特徴とする脈管閉塞装置またはカテーテルアッセンブリ。
- 請求項1から10のいずれか1項に記載の脈管閉塞装置またはカテーテルアッセンブリが、更に、前記メッシュ状構造に連結された放射線不透過物質のマーカを具えることを特徴とする脈管閉塞装置またはカテーテルアッセンブリ。
