Description
本発明は、概して、植込み可能な用具と血管障害を治療する方法とに関する。
最小限に侵襲的な或いは非侵襲的なインターベンショナル処置で使用される、数多くの医療用具及び処置は、外科的処置では容易に到達できない血管障害を治療することに使用されている。脆弱化、或いは閉塞した血管壁を支持し、流体が流れるようにするために体内管腔に植込まれるように構成されるこのような医療用具は、周知であり、市場で販売されている。このような用具の一つに、例えば、血管用ステントがある。ステントは、管壁を支持し、冠状動脈系や脳、尿路、胆道、食道、気管、気管支等の開存性を維持するために用いることができる。
本発明は、概して、動脈瘤等の体内管の治療のために血管系の一部に挿入できる血管塞栓具とその使用方法に関する。本発明の血管塞栓具は、通常、ステントやステントグラフト等の支持構造と組み合わせて使用される。本発明の塞栓具は、血管障害部位において容易に固定され、移動しないように設計される。当然ながら、2個以上の支持構造も、処置において用いることができる。
塞栓具は、血管障害のある領域への流体の流入を遮断する、或いは完全に遮断しないまでもある程度まで遮断することが望ましい血管障害の治療を行うための、侵襲性が最小限のインターベンショナル処置において用いることができる。
本発明の別の実施例は、ステントと、少なくとも1個の固定部材を有する血管塞栓具との組み合わせであって、少なくとも1個の固定部材は、ステントとは別体として構成されるとともに、血管障害のある位置に血管塞栓具が展開された後に血管塞栓具をステントに対して固定する、ステントと血管塞栓具との組み合わせを提供する。
一実施例においては、本発明の方法は、支持構造を血管障害部位に対して展開する工程と、血管塞栓具を血管障害部位に対して展開する工程と、血管塞栓具を支持構造の開口を介して血管障害部位へ挿入する工程と、少なくとも1個の塞栓部材を展開する工程と、少なくとも1個の固定部材を展開する工程とを含む。
本発明のこれらの特徴及び他の特徴ならびに本発明の利点は、本発明の特徴の例として示される、以下の詳細な説明及び図面から明らかになるであろう。
本発明は様々な形態で実施することができるが、本明細書においては、特定の実施例について詳細に記載する。実施例の記載は、本発明の原理を例示するものであり、本発明を例示された特定の実施例に限定するものではない。
図3は、本発明の一実施例における塞栓具(図2においては図示されていない)をマイクロカテーテル等のマイクロカテーテル装置50を用いて非拡張状態で搬送する工程の最初の段階を示すものである。マイクロカテーテル50は、血管12内、続いてステント10内を案内され、ステントストラット15間に位置する開口17内を通り、動脈瘤14内へ案内される。血管塞栓具10(図示しない)は、マイクロカテーテル50内に押込まれ、保持されることが可能なように、巻き付けられるか、圧縮されるか、或いはその他の非拡張状態とすることができる。図4においては、マイクロカテーテル50は、血管12内のステント10内に配置され、ストラット15により形成された開口17を貫通して上方へ突出し、動脈瘤14に入り込んだ状態となっている。固定部材30は、マイクロカテーテル50の先端チップ55に配置されている。
図10においては、引き戻し可能なシース116が完全に引き戻されて、血管塞栓具100の固定部材30も解放されている状態が示される。
上述した各実施例においては、固定部材が1個だけ用いられているが、本発明においては、1個以上の固定部材を用いてもよい。固定部材は、任意の形状のものを用いることができる。
本発明の塞栓具の塞栓部分は、ポリマー材料等の様々な材料を用いて製造することができるが、材料はポリマー材料に限定されるものではない。有用なポリマー材料の例には、合成材料と天然材料の両方を含む。さらに、材料は、生体適合性材料及び/又は生体分解性材料であってもよい。有用なポリマー材料の例には、ポリエチレン及びポリプロピレン等のポリオレフィン、ポリエチレンテレフタレート(PET)及びポリブチレンテレフタレート(PBT)等のポリエステル、ポリウレタン、アクリル、ポリペプチド、ポリエーテル、ポリアミド、延伸ポリテトラフルオロエチレン等のフッ素ポリマー等が含まれるが、これらに限定されるものではない。
コポリマー及び上述した材料の架橋可能なものも、本願での使用に好適である。また、上述した様々な材料の混合物も、本発明の塞栓具の製造に用いることができる。
血管塞栓具が形成される材料又は血管塞栓具自体を改変し、或いは添加物を加えて、血管塞栓具が従来のイメージング技術を用いて視認可能となるようにしてもよい。例えば、塞栓具は、蛍光透視技術を用いて視認可能となるようにしたり、MRIを用いて視認可能となるようにしたり、その両方において視認可能となるようにしたりすることができる。これは、巻回したワイヤ等のマーカーや、マーカーバンド、リベット、プラグ等を用いることにより、或いは放射線不透過性材料又はMRIで視認可能となる材料を血管塞栓具を形成する材料に組み込むことにより、可能となる。放射線不透過材料又はMRIにおいて視認可能となる材料は、任意の好適なものを使用することができる。
a)上述したような支持構造を血管障害部位に対して展開する工程であって、前記支持構造は血管塞栓具が入る開口を有していることと、
b)上述したような拡張形状と非拡張形状とを有する少なくとも1個の塞栓部材を備えた血管塞栓具と、上述したような少なくとも1個の固定部材とを、前記支持構造の開口及び血管障害部位の口部を介して、血管障害部位内へ展開する工程と、
c)少なくとも1個の塞栓部材を拡張させる工程と、
d)少なくとも1個の固定部材を用いて、血管塞栓具を支持構造に対して固定する工程と
を含む。
a)上述したような支持構造を動脈瘤の部位において展開する工程であって、前記支持構造は上述したように血管塞栓具が入る少なくとも1個の開口を有することと、前記血管塞栓具は、非拡張形状及び拡張形状を有する少なくとも1個の塞栓部材と、少なくとも1個の固定部材とを備えることと、前記支持構造は、支持構造の少なくとも1個の開口が動脈瘤の口部と位置が揃うように、動脈瘤の口部に配置されることと、
b)少なくとも1個の塞栓部材が非拡張状態にある状態で、前記支持構造の少なくとも1個の開口及び動脈瘤の口部を介して、血管塞栓具を動脈瘤内に展開する工程と、
c)血管塞栓具の少なくとも1個の塞栓部材を拡張状態となるように拡張させ、血管塞栓具がその拡張状態において動脈瘤の口部を親血管から遮断する工程と、
d)少なくとも1個の固定部材を用いて、血管塞栓具を支持構造に対して固定する工程と
を含む。
Claims (13)
- 血管障害の治療具であって、
第1の非拡張形状及び第2の拡張形状を有する少なくとも1個の塞栓部材と、
前記治療具を血管障害のある位置において開口を有する支持構造に対して固定する少なくとも1個の固定部材と、該固定部材は、前記支持構造の開口を通り抜けることができる非拡張形状と、前記支持構造の開口を通り抜けることができない拡張状態とを有することと
を備える治療具。 - 前記支持構造はステント又はステントグラフトである請求項1に記載の治療具。
- 前記少なくとも1個の固定部材もしくは前記少なくとも1個の塞栓部材又はこれら両方が金属又は合金からなる請求項1に記載の治療具。
- 前記少なくとも1個の塞栓部材もしくは前記少なくとも1個の固定部材又はこれら両方がポリマー材料で形成される請求項1に記載の治療具。
- 前記少なくとも1個の塞栓部材もしくは前記少なくとも1個の固定部材又はこれら両方が生物由来の材料で形成される請求項1に記載の治療具。
- 前記少なくとも1個の塞栓部材は、少なくとも一つのポリマー材料と少なくとも一つの金属又は合金とを組み合わせて形成される、請求項1に記載の治療具。
- 前記少なくとも1個の塞栓部材は、傘状、放物線状、球状、又は円盤状のうちのいずれかの形態をとる請求項6に記載の治療具。
- 前記ポリマー材料は膨張可能なポリマー材料である請求項4に記載の治療具。
- 前記膨張可能なポリマー材料はハイドロゲルである請求項8に記載の治療具。
- 前記少なくとも1個の固定部材がハイドロゲルで形成される請求項1に記載の治療具。
- 前記少なくとも1個の塞栓部材及び前記少なくとも1個の固定部材が膨張可能なポリマー
材料又は圧縮された発泡体で形成される請求項1に記載の治療具。 - 前記少なくとも1個の塞栓部材の少なくとも一部が、網目状、編成された、フェルト状、又はメッシュ状の構造を有する、請求項1に記載の治療具。
- 開口を有するステントと、少なくとも1個の固定部材を有する血管塞栓具との組み合わせであって、少なくとも1個の固定部材は、ステントとは別体として構成されるとともに、血管障害のある位置に血管塞栓具が展開された後に血管塞栓具をステントに対して固定し、前記少なくとも1個の固定部材は、前記ステントの開口を通り抜けることができる非拡張形状と、前記ステントの開口を通り抜けることができない拡張状態とを有する、ステントと血管塞栓具との組み合わせ。
