Description

本発明は、概括的には脊椎移植片に関し、より厳密には、排他的ではないが、横方向に拡張可能な椎骨移植片に関する。
持続性の、時には身体に障害を引き起こすほどの、背中の痛みの主な原因は、円板環の破壊、円板の慢性炎症、又は変性疾患により発症する様な所与の円板を取り囲む椎体が比較的不安定であること、に起因することがある。更に深刻な場合には、対象の円板の何れかの側の椎骨の動きに対してある種の形態の機械的制限が必要になる。そのような場合には、円板組織は回復できないほどに損傷を受けており、そのため円板全体を取り出すことが必要になる。しかしながら、その後の安定化なしに円板核を除去してしまうと、持続性の炎症及び/又は不安定さによって、問題の障害を引き起こす背中の痛みが再発することもしばしばである。
この物質の切除に続いて隣接する椎体を安定化するために、各種対応策が開発されている。或る対応策では、椎骨の間に移植される融合装置を通して、2つの隣接する椎体が一体に融合される。これら既存の移植片設計の多くには、脊椎融合率を低下させる欠点がある。これら設計上の欠点の中でも、1つの不備な点として、移植片が椎骨終板内に沈下して、これにより椎体間の間が狭くなるということがある。従来の融合装置、及び場合によっては人工装具装置でも、そのような装置では、荷重の大部分が椎体の最も弱い部分にかかり、その結果、装置が周囲の椎骨終板に空洞化を引き起こし、次いで内部円板空間が崩壊し、更には椎骨自体も傷つく原因となる。存続に関する別の頻度の高い原因としては、移植片と終板の間の接触面積が小さいことにより生じる。ご存知のように、移植片と終板の間の接触面積が小さいと、存続の危険が高まる。
多くの移植片の別の欠点は、移植後に生じる安定性の欠如である。安定性は融合の成功を左右する重要な要素である。移植片と椎体の間が確実に動かないようにするため、移植片は椎体にしっかりと固定せねばならない。椎体と移植片との間に動きが生じると、骨は正しく融合せず、これにより安定性上の問題が発生することになる。更に、設計によっては、移植片と共に使用できる移植組織材料の量が制限される場合もある。終板と接触することができる移植組織材料の面積が大きければ、2つの椎骨の間で良好且つ中実な骨が成長する可能性が高くなる。
設計によっては、存続の可能性を小さくするために、移植片の大部分が椎骨の骨端環(apophyseal rings)のより堅い皮質骨を覆うように配置される移植片を創作している。しかしながら、この様な設計では、移植片は、複数の別個の構成要素で作られ、それを円板空間内で、個別に組み立てることになる。各構成要素は、別々に移植され、次いで円板空間内で互いに取り付けられる。理解頂けるように、円板空間内でこのような移植片を組み立てることはかなり難しい。このような移植片は、移植片の安定性並びに全体的な融合構成に欠くことのできない剛性のある中央本体が欠ける傾向もある。更に、このような移植片には、移植片を椎体に固定するための機構がない。通常は、移植片を椎体に固定するために骨ねじを使用せねばならないが、これが移植工程をより複雑且つ困難にしている。更に、このような移植片は横方向の幅が一律であり、従って、大きさの異なる椎骨に合わせて調整することは、不可能ではないにしても、概して非常に難しい。別の欠点は、これらの設計では、スペーサが正しく配置されることを保証する機構が提供されないことである。これらの種類の移植片の横方向スペーサは、円板空間内で独立して組み立てられるので、横部材は骨端環に沿って不同の位置に配置されることになり、これにより移植片が椎骨終板に沈下する危険が高まる。
或る態様では、脊椎移植片は、内部空洞を画定するケージと、ケージの空洞内に入っている拡張機構を含んでいる。拡張機構には一対のウイングが作動的に連結されており、ウイングは、それぞれ、対向する椎骨に係合するように作られている互いに反対側の椎骨係合面を有している。拡張機構は、ウイングを、少なくともウイングの大部分がケージの空洞内に入っているコンパクト形態から、ウイングがケージから伸張して各ウイングの椎骨係合面が椎骨に係合している拡張形態へと、椎骨の間で横方向に動かせるようになっている。
別の態様は、円板空間を画定する対向する椎骨の間に移植するための融合装置に関する。この装置は、中央部材と、中央部材に滑動可能に連結された少なくとも一対の横部材を含んでいる。この装置は、横部材を、中央部材から椎骨の間の円板空間内に伸張させ、各横部材が両方の椎骨と係合した状態にするための手段を更に含んでいる。
別の態様では、装置は脊椎移植片を含んでいる。この脊椎移植片は、内部空洞を画定する中央部材と、中央部材の両側に内部空洞内に開口して形成された一対の開口部とを含んでいる。中央部材の開口部には一対のウイングが滑動可能に入っている。ウイングにはシャフトが連結され、シャフトは、ウイングの少なくとも一方に螺合する少なくとも1つのねじ部を有している。
本発明の原理の理解を深めるために、これより図面に示す実施形態を参照してゆくが、その際、特別な用語を使いながら説明する。しかしながら、これによって本発明の範囲を限定するものではない。本発明が関係する技術分野における当業者であれば、ここに例示する処理、システム、又は装置に対する様々な変更及び修正、並びにここに示す本発明の原理の別の使用法を、当然のこととして想起するであろう。本発明の幾つかの実施形態を詳細に示しているが、当業者には自明のように、幾つかの特徴については明解さを期すために示していない場合もある。
以下、本発明の或る実施形態による横方向に拡張可能な脊椎移植片100について、図1から図6までを参照しながら説明する。図1及び図2に示すように、移植片100は、中央部材即ちケージ102と、ケージ102から横方向に伸張するように作られている一対の横部材即ちウイング104と、ウイング104を伸張させるように作動可能な拡張機構106(又は手段)を含んでいる。図示の実施形態では、拡張機構106は、ウイング104を一体に接続する、ターンバックル即ちねじシャフト108を含んでいる。他の実施形態では、拡張機構は、液圧式ピストン、機械的リンクなどを含んでいる。シャフト108は、互いに反対側のねじ部112と114の間のシャフト108の中央に配置されたギヤ110を含んでいる。或る実施形態では、ねじ部112と114のねじ116は逆方向にねじが切られている(即ち、一方は左ねじで他方は右ねじである)。本発明の或る形態では、ねじ部112と114のねじ116は、ウイング104が中央部材102から同じ割合で伸張できるように、等しいピッチを有している。これにより、移植片は対称な構成を有し、これによって椎骨上での移植片100の心出しがやり易くなる。ねじ部112と114は、ウイング104それぞれに画定されたねじ付開口部118と螺合する。別の実施形態では、シャフト108の一方の端部にだけねじが切られており、シャフト108の他方の端部にはねじが切られていない。この実施形態でも、ウイング104は、シャフト108を回転させることによって伸張させることができる。
移植片100は、ウイング104を、ウイング104が横方向にケージ102から伸張した拡張形態に係止するために使用する係止機構120を更に含んでいる。図1に示す実施形態では、係止機構120は、各ウイング104内のねじ切り開口部118の横に形成された係止空洞122を含んでいる。図2に示すように、係止空洞122は、ウイング104内のねじ付開口部118に開口している。或る実施形態では、各係止空洞122は、ウイング104が拡張形態になると、シャフト108がねじ付開口部118から滑動して係止空洞122に入るように、部分的にのみねじが切られている。空洞122の入口付近だけに部分的にねじが切られていることにより、シャフト108は回転できなくなり、ウイング104は後退できなくなる。別の実施形態では、係止空洞122にはねじが切られていないが、ねじ付開口部118よりも深さが浅く、シャフトが係止空洞122に入ると、ウイング104を拡張形態に維持するようになっている。
図2に示すように、各ウイング104は、対向する椎骨に係合するように作られた互いに反対側の椎骨係合面202、並びに中央側面204と側方側面206を含んでいる。図示のように、ウイング104は、図示の実施形態によれば、椎骨の終板形状に一致するように概ね先細の形状を有している。椎骨係合面202は、中央側面204から側方側面206に向けて概ね先細になっている。移植片100を挿入する時の外傷を更に低減するため、ウイング104は、椎骨係合面202と側方側面206の間に面取りされた縁部208を有している。図示の実施形態では、ウイング104の中央側は略平坦で、ウイング104がケージ102内に後退した際には、コンパクト形態で互いに接することができるようになっている。ウイング104の中央側面204には、ねじ付開口部114と係止空洞122の周りにアクセスチャネル210が形成されている。或る形態では、アクセスチャネル210は、シャフト108上のギヤ110を受け入れる大きさになっている。アクセスチャネル210は、医師がアクセスしてギヤ110を回転させ、移植片100を拡張できるようにする開口部212を有している。図示の実施形態では、側方側面206は、椎骨の骨端環(apophyseal rings)の形状に一致させるため、全体的に湾曲した形状を有している。
引き続き図2に示すように、ケージ102は、近位又は工具係合端壁部214と、反対側の遠位端壁部216と、一対の互いに反対側の側壁部218を有し、それらは全体で内部空洞220を画定している。ケージ202は、更に、対向する椎骨と係合するように作られた互いに反対側の一対の椎骨係合面222を有している。椎骨の形状に一致させるため、図示の実施形態の一対の面222は、近位端壁部214から遠位端壁部216まで互いに向けて傾斜するように先細になっている。図示のように、内部空洞220は、両方の椎骨係合面222を貫通して伸張している。図示の実施形態では、ケージ102は、概ね矩形の形状をしている。椎骨係合面222は、移植片100が椎骨から抜け出さないようにするためにテクスチャー加工(texturing)をされている。例えば、図示の実施形態の椎骨係合面222は、移植片100が抜け出るのを防ぐ助けとなるリッジ224を含んでいる。理解頂けるように、本発明の他の形態では、椎骨係合面222は、移植片100の抜け出しを防ぐために他のタイプのテクスチャー加工をされてもよい。近位端壁部214は、挿入工具を内部空洞220に挿入する際に通す工具用開口部226を画定しており、側方壁218は、ウイング104を滑動させて内部空洞220に入れる際に通すウイング用開口部228を画定している。
図3及び図4は、ウイング104が内部空洞220の内側に後退したコンパクト状態にある移植片100を示している。図3に示すように、ウイング104は、ウイング用開口部228の周りに形成された対応するガイドチャネル304に係合する1つ又はそれ以上のガイドレール302を有している。図示の実施形態では、各ウイング104は4つのガイドレールを有し、各一対が、ウイング104の互いに反対側の椎骨係合面202それぞれに沿って配置されている。安定性を更に増すために、図示の実施形態のガイドレール302と、対応するチャネル304は、概ねあり継ぎのための形状を有している。更に、下に詳しく論じるが、ガイドレール302のあり継ぎのための形状は、一旦移植されると、ウイング104が椎骨に固定された状態で確実に維持されるようにする。移植片100がコンパクト状態にあるとき、ウイング104の大部分は、ケージ102の内部空洞220に入っている。コンパクト状態では、中央側面204は互いに接触しており、アクセスチャネル210の入口212は、シャフト108を回すために挿入工具をシャフト108のギヤ110に接近させるためのアクセス開口部306を画定している。
先に説明したように、ギヤ110を使用してシャフト108を回し、これによってウイング104をケージ102から伸張させる。図5及び図6は、ウイング104がケージ102から伸張している横方向拡張状態にある移植片100を示している。理解頂けるように、拡張機構106は、選択された椎骨の大きさに合わせて移植片100の大きさを調整できるように、ウイング104がケージ102から様々な距離だけ伸張できるようにしている。図6に示すように、ガイドレール302の外部側方端402は、外部側方端402が切刃406を形成するように、内向きのノッチ404を画定している。ウイング104が伸張すると、切刃406は椎骨にチャネルを切り込む。切刃406は、ウイング104を椎骨終板に埋め込むためのスパイクのような機能を果たす。ウイング104が伸張してしまうと、ガイドレール302のあり継ぎのための形状は、ウイング104が椎骨に確実にしっかりと固定されるのを助ける。ウイング104が望ましい伸張位置に達すると、ウイング104を所望の伸張位置に係止するため、シャフト108を滑動させ係止空洞122(図2)に入れる。移植後、椎骨の融合を促進させるため、工具用開口部226を通して内部空洞220に骨移植組織材料が充填される。骨移植組織材料は、ウイング104を僅かに伸張させた状態で、移植前に充填してもよい。移植の後、内部空洞220は、融合塊を椎骨の間に形成できる広い領域を提供する。
本発明の或る実施形態による、挿入器702に連結された移植片100を含んでいる移植片挿入器アッセンブリ700を、図7及び図8に示している。挿入器702は、駆動ハンドル704と、作動ノブ706と、シャフト部708と、把持ノブ710と、頭部712を含んでいる。図示の実施形態では、ハンドル部704は中実で、衝撃面714を含んでおり、この衝撃面714にハンマーなどを打ち付けて移植片100を椎骨の間に駆動する。作動ノブ706は駆動シャフト802に接続されており、駆動シャフト802は、作動ノブ706から、シャフト708を通り、頭部712を通って伸張している。移植片100を挿入器702に係合すると、作動ノブ706はウイング104を伸張させることができるようになる。図8に示すように、駆動シャフト804は、一方の端に、キャリア部材810を介して頭部712に連結されている中間ギヤ808と係合する歯806を備えた駆動ギヤ804を有している。移植時には、中間ギヤ808は、移植片100のシャフト108のギヤ110と係合する。作動ノブ706を回すと、駆動シャフト802が駆動ギヤ804を回転させる。すると、駆動ギヤ804は中間ギヤ808を回転させ、これを利用して、ウイング104を伸張させるためにシャフト108を回転させる。把持フィンガ812を内部空洞220の内側に伸張させるために把持ノブ710を回し、挿入器702を移植片100の工具用開口部226と係合させる。把持ノブ710と把持フィンガ812は随意的なもので、或る実施形態では、ノブ710とフィンガ812は含まれていない。衝撃用に広い面積を提供するために、頭部712は、移植片100の近位端壁部214の形状に概ね一致した略矩形形状を有している。
図9、図10、図11は、移植片100が隣接する椎骨902、904の間に移植された状態を各方向から見た図である。移植前に、移植片100の移植用により広い円板空間が作り出すため、環の一部が除去される。椎骨終板は、結合している軟骨性物質を除去することにより整えられる。ケージ102の形状と大きさに概ね一致しているウインドウ906が、両椎骨902と904の間に形成される。移植前には、ウイング104は、移植片100の内部空洞220の内側の後退位置に置かれており、移植片100は、図7に示したように挿入器702に取り付けられる。移植片100は、次いで、椎骨902と904の間に形成されたウインドウ906に打ち込まれる。挿入器702の作動ノブ706を回転させることにより、移植片100のシャフト108を回転させ、これによって移植片100を伸張させる。先に説明してきたように、これによって、ウイング104は、椎骨の間をケージ102から横方向に拡張する。或る実施形態では、移植片100が椎骨902と904の間の心出し位置に確実に保たれるように、両ウイング104は、ケージ102から同じ割合で伸ばされる。ウイング104が伸張するにつれ、ガイドレール302の切刃406は椎骨902と904に切り込み、これにより、移植片が確実にしっかりと椎骨902と904に固定される。ウイング104は、より硬い皮質骨を含んでいる骨端環を覆う位置に達するまで拡張される。図9に示すように、ウイング104の形状は、椎骨902と904の終板の形状に概ね対応している。移植片100によってより広い面積が提供され、且つ骨端環のより硬い皮質骨上に支持されていることにより、移植片100が椎骨902と904内に沈下する危険性が小さくなる。更に、移植片100の構造は、移植片100が様々な大きさの椎骨に対応できるように、移植片が可変寸法を有することができるようにしている。移植片100を所望の拡張形態まで拡張させた後、ターンバックル108をブロック空洞122に動かして、ウイング104がその位置に係止されるようにする。
図12に示す本発明の別の実施形態による移植片1200には、以下に特記した事項を除いて、上記と同じ数多くの特徴が組み込まれている。理解頂けるように、この実施形態の係止機構120は、先に説明した係止機構とは異なっている。図12に示す実施形態では、係止機構120は、ケージ102の遠位端壁部216に取り付けられた板ばね1202を含んでいる。図示のように、板ばね1202は、シャフト108のギヤ110と係合している。板ばね1202は、ウイング104が横方向の拡張方向にだけ動くように、シャフト108が一方向にしか回転しないように配置されている。ばね1202は、シャフトが反対方向に回転するのに抵抗するので、ウイング104が一旦所望の位置まで拡張してしまうと、ばね1202はウイング104をその位置に係止する。
以上、本発明を詳細に示し説明してきたが、本発明の広がりと範囲は、上記代表的な実施形態によって限定されるものではなく、特許請求の範囲の内容及びその等価物によってのみ定義されるものである。なお、選択された実施形態を示し説明してきたに過ぎず、本発明の趣旨の中に含まれる全ての変更及び修正は、保護の対象とされることを要求する旨理解されたい。

Claims (37)

  1. 内部空洞を画定するケージと、
    前記ケージの前記空洞内に入っている拡張機構と、
    前記拡張機構に作動的に連結された一対のウイングであって、それぞれ、対向する椎骨と係合するように作られた互いに反対側の椎骨係合面を有しているウイングと、を備えており、
    前記拡張機構は、前記ウイングを、前記ウイングの少なくとも大部分が前記ケージの前記空洞内に入っているコンパクト形態から、前記ウイングが前記ケージから伸張して前記各ウイングの前記椎骨係合面が椎骨に係合している拡張形態へと、前記椎骨の間で横方向に動かすことができる、脊椎移植片。
  2. 前記拡張機構は、前記ウイングに螺合するねじ部が両端部に設けられたシャフトを含んでいる、請求項1に記載の脊椎移植片。
  3. 前記シャフトには、シャフトを回転させるためのギヤが、前記ねじ部の間に設けられている、請求項2に記載の脊椎移植片。

  30. 円板空間を画定している対向する椎骨の間に移植するための融合装置において、
    中央部材と、
    前記中央部材に滑動可能に連結された少なくとも一対の横部材と、
    前記横部材を、前記中央部材から前記椎骨の間の円板空間内へ伸張させ、前記横部材それぞれが前記両椎骨に係合した状態にするための手段と、を備え
    前記横部材を伸張させるための前記手段が前記横部材を伸張するとき、当該横部材の各々は前記対向する椎骨の前記骨端環のより硬い皮質骨に係合する、融合装置。
  31. 前記横部材を伸張させるための前記手段は、前記横部材に連結されたねじ部を備えているシャフトを含んでいる、請求項30に記載の融合装置。
  32. 脊椎移植片を備えている装置において、
    前記脊椎移植片は、
    内部空洞を画定している中央部材であって、前記中央部材の両側面には、前記内部空洞に開口する一対の開口部が形成されている中央部材と、
    前記中央部材の前記開口部に滑動可能に入っている一対のウイングと、
    前記ウイングに連結されているシャフトであって、前記ウイングの少なくとも一方と螺合する少なくとも1つのねじ部を有するシャフトと、を含んでいる、装置。
  33. 前記開口部はガイドチャネルを有し、
    前記ウイングは前記ガイドチャネル内に滑動可能に入るガイドレールを有している、請求項32に記載の装置。
  34. 前記少なくとも1つのねじ部は、前記シャフトの両端部に一対の対向するねじ部を有している、請求項32に記載の装置。
  35. 前記ウイングを伸張させるために前記移植片に係合される挿入器を更に含んでいる、請求項32に記載の装置。
  36. 前記脊椎移植片の前記シャフトはギヤを含んでいる、請求項35に記載の装置。
  37. 前記挿入器は、
    作動ノブと、
    前記ノブに連結された駆動シャフトと、
    前記駆動シャフトに連結された駆動ギヤと、
