Description

発明の背景
1.発明の分野
本発明は、拡張すると緊張構造又は骨格を形成するように巻き戻される渦巻状の結合を行う改良された形状を有するステントに関する。
2.従来技術の簡単な説明
ステントは、典型的には、管腔貫通的に移植でき、経皮的に導入された後放射状に拡張できる血管内移植体である放射状拡張可能体内人工器官である。ステントはまた泌尿器管や胆管にも移植された。ステントは、血管系での血管形成後、身体血管を再強化し、再発狭窄症を予防するために使用される。ステントは、自己拡張型か、又はバルーン上に搭載されたときのように、内部放射状力によって拡張できる。
ステントの一例は、コイル状ヒンジによって連結されるセグメントを有するステントを開示する欧州特許第0421729号に示される。
過去において、ステントの形状は多様であり、ステントは、金属やプラスチックを含む多くの材料から作製された。ステンレススチールのような通常の金属は、ニチノールなどの形状記憶金属として使用された。ステントはまた、生分解性プラスチック材料から作製された。ステントは、ワイヤ、チューブストックなどから形成された。
発明の概要
本発明は、上記の従来技術のステントに種々の型の全てに適合しうるステントの新規形状を提供する。該新規形状には多数の利点がある。とりわけ、該形状はリコイルを制限し、拡張ステントへの圧縮に対する抵抗性を加える。該形状は非拡張状態と拡張状態の両方で長手軸方向に柔軟性を有する。それは幾つかの実施形態を有する。
新規形状の重要な部分は、渦巻状をなし、又は屈曲しており、ステントの拡張の際に、より大きく、又はより小さく直線状態になるように巻き戻され、渦巻が解かれ、又は非屈曲化される連結要素からなる渦巻構造又は渦巻状構造を含む。このような構造は以後、集合的に渦巻、渦巻形、又は渦巻様構造という。これらの構造は、拡張時、ステント中に低ひずみの領域を与える。これらの要素は、互いに、又は任意の種類の任意の放射状拡張部材と連結できるが、環状蛇行部材が好ましい。

好適な実施形態の詳細な説明
本発明の一好適実施形態は図1−8に示される。それは、図1、図2及び図4に示される形状に、エッチング、又はレーザー切削にて形成された、ニチノール又はステンレススチールのような、特に図3と4にて示される金属管状構造10を含む。その形状は、直線部位及び屈曲部位からなるループ14を形成する一連の蛇行環状拡張可能要素、即ちセグメント12から作製され、放射環状に拡張が可能となる。セグメント12の形状は他の態様もありうるが、蛇行状が好適である。要素12は、図2の詳細図で最も良く見られるように、一端でセグメント12の連続的ループ14と結合し、他端で隣接する対の長尺部材13a、13bと結合する一対の長尺部材13a、13bによって相互に連結させられる。好ましくは、部材13a、13bは部材12より細く、符号16にて示されるような渦巻状又は渦巻形配置にて結合される。部材12の放射状拡張時に、渦巻形の巻き方向に応じて、反時計方向又は時計方向にほぼ直線状態へ巻き戻されうる。この実施形態では、渦巻形16は、一方向に巻き戻されるものがあり、他方向に巻き戻されるものがあるような選択的な巻き構造で形成される。勿論、任意の実施形態で、渦巻形は時計方向か反時計方向の一方向に全て巻き戻され、より多い、又はより少ない部材13を有しうるように渦巻形は形成できる。また、より多い、又はより少ない渦巻形がセグメント間に含まれうる。図4−8に関しより詳細に記載したように、部材13に関し、渦巻が巻き戻されるときには直線化作用を伴う。この構成を有する拡張されたステントにおける最終的な構成は複数のセルからなり、その各々の周は、セグメント12のループ部14及び一対の部材13によって定められることが、図4−8からわかる。セル同士は、図8に最もよく示されるように、符号16において結合される。より詳細には、図8に示すようにセルには2種類ある。第1の対のセルはA、Bよりなり、セルAではセグメント12と2個の部材13a、又はセルBではセグメント12と2個の部材13bから作出される。第2の対のセルはC、Dよりなり、セルC及びDのために、セグメント12の内向きループ14と、部材13a及び部材13bから作出される。
図1−4に示されるもののような本発明のステントが、図4の実施形態からのような、拡張をするとき、本発明のステントは図5に示すように完全拡張状態になる。図5はステントの斜視図を示す。
ステントの拡張で、渦巻要素16が受ける巻き戻し作用は図6−8で最良に示されるが、図6−8は単純性と明瞭性のために正面図のみを示す。
放射状拡張が始まるとき(図6に示す)、渦巻要素16は、部材13の全てを引っ張る力による巻き戻し作用又は直線化作用を受けることが理解できる。即ち、拡張が起るとき、図6の拡張初期に見ることができるように、要素13は直線化作用を受ける。
更に拡張すると(図7に示す)、渦巻形16は更に巻き戻される、即ち、要素13は更なる直線化を受ける。
最後に、図8で、ほぼ完全な拡張により、ほぼ直線の要素13が得られ、その条件で、ステントリコイルは制限され、ステント圧縮への抵抗性は増加する。
図9は改変実施形態を示すが、要素13a、13bはループ14の屈曲部位近傍でセグメント12と接触する。また、この実施形態では、渦巻16は全て同一方向に巻かれることに注意されたい。
図10は、渦巻形部材13はより屈曲の度合いが大きく、より曲線的でないものの渦巻様形状16をなお形成する本発明の実施形態を示す。形状の残りは図9のそれと同様である。図10では、長尺部材13は、ステントの拡張前のものが示される。ステントが拡張するとき、部材13は反時計方向に巻き戻され、やや直線となる。完全拡張で、部材13は更に直線となり、ステント圧縮への抵抗性とリコイルの起こりにくさを与えるようにほぼ直線となる。拡張形状は、図8に見られるものと類似のセル形状を示す。
他の実施形態は、異なる渦巻配置の次なる図で示される。例えば、図11−13の実施形態は、同一方向に巻かれた渦巻状配置16とループ14の屈曲部位近傍に結合した要素13を示すが、セグメント間の幾つかの隣接する渦巻は部材15によって相互に連結されている。
図14は、ループ14の屈曲部位近傍と接触する渦巻形16中の幾つかの要素13と幾つかの接触セグメント12そのものを示す。また、幾つかの隣接渦巻は部材17で相互に連結されている。
図15は、扁平状の、又は長尺状の渦巻構成16を示し、要素12は、ステントの長手軸に対し傾く。上記実施形態では、これらの要素又はセグメントは、軸に対し平行か、又は水平に配置されていた。図13におけるような長尺状の渦巻と前の図の渦巻は一緒に混合できる(図示せず)。
既に述べた実施形態では、セグメント12のような環状拡張可能セグメントには渦巻配置16が散在する。しかし、図16で見ることができるように、ステント体の少なくとも実質的な部分又は全部は、互いに連結した渦巻要素16からだけなることができる。実際にステント体の全ては渦巻形からなりうる。この実施形態では、要素13は、ステントのほぼ全体で渦巻形間で相互連結する。場合によっては、端部は、示すようにセグメント12を含みうる。
図17に示す実施形態は、交互に反対方向に傾いたセグメント12を示し、異なる長さのその脚と要素13は異なる位置、即ち、ループ部14とセグメント部そのものでのような異なる位置でセグメントと接触する。
図18−28は、本明細書で用いる用語、渦巻、渦巻形、及び渦巻状が意味するものの例を示す。渦巻には他の部材も含まれている。
本発明は多数の異なる形で具体化できるが、本明細書では、本発明の特別の好適実施形態が詳細に説明されている。この説明は、本発明の原理の例示であり、記載した特定の実施形態に本発明を限定するものではない。
上記実施例と開示は実例であり、あますところのないものではない。これらの実施例と説明は、当業者に多数の変形と代替を示唆する。これらの代替と変形の全ては請求の範囲に含まれるものとする。当業者は本明細書記載の特定の実施形態の他の均等物を理解できるが、その均等物も請求の範囲に含まれるものとする。

Claims (10)

  1. ステントの長手軸に沿って配列される複数の蛇行環状要素(12)と、隣接する蛇行環状要素(12)同士を連結する複数の渦巻要素(16)とを備える拡張可能な管状ステントにおいて、
    各蛇行環状要素(12)は、交互に反対方向へ屈曲する複数のループ(14)がステントの周方向に連続してなり、
    各渦巻要素(16)は、前記蛇行環状要素(12)のループ(14)の屈曲部位近傍から延びて該渦巻要素(16)の中心において結合する複数の部材(13a、13b)からなるステント。
  2. 前記渦巻要素(16)の巻き方向が全て同じである請求項1に記載のステント。
  3. 第1の巻き方向に巻かれた渦巻要素(16)と、前記第1の巻き方向とは異なる第2の巻き方向に巻かれた渦巻要素(16)とを有する、請求項1に記載のステント。
  4. 前記渦巻要素(16)が近接する渦巻要素(16)に対して連結される請求項1乃至3のいずれか一項に記載のステント。
  5. 前記蛇行環状要素(12)は、複数の渦巻要素(16)を含む一列の渦巻要素群を介して相互に連結される、請求項1乃至4のいずれか一項に記載のステント。
  6. 前記蛇行環状要素(12)は、それぞれ複数の渦巻要素(16)を含む相互連結された二列の渦巻要素群を介して相互に連結される、請求項1乃至4のいずれか一項に記載のステント。
  7. 複数の渦巻要素(16)を備える拡張可能な管状ステントにおいて、
    各渦巻要素(16)は、その中心から複数の部材(13a、13b)が渦巻状に延びてなり、該渦巻要素(16)がステントの周方向に配列されて渦巻要素群を構成し、複数の渦巻要素群がステントの長手方向に配列され、近接する渦巻要素群同士が相互に連結されるステント。
  8. 各渦巻要素(16)がステントの周方向において近接する渦巻要素(16)の一方に対して連結される、請求項に記載のステント。
  9. 前記ステントが相互連結された渦巻要素のみにより構成される請求項又はに記載のステント。
  10. 近接する渦巻要素群同士は、連続する複数のループ(14)よりなる蛇行環状要素(12)を介して連結される請求項又はに記載のステント。
