Description
本発明は、表皮ケラチン細胞成長因子(KGF)を表皮ケラチン細胞以外の細胞の増殖、成長及び分化の刺激に適用すること、及び前記のような生物学的作用が傷害された、または罹患した細胞及び組織の再生に有用である場合に患者にKGFを投与することに係わる。

近年、バイオテクノロジーの進歩は、エリスロポイエチン及び顆粒球コロニー刺激因子を含めたポリペプチドの新規な組み換え体を開発して多くのヒト患者の治療に用いるまでに至った。以来、in vitroで生物学的活性を示す他のポリペプチドが多数発見されている。しかし、ヒトにおいて可能な治療用途を確定するには、これらの因子の標的細胞及び該因子のin vivoでの生物学的作用の解明が必須である。
KGFは、上皮細胞、特に表皮ケラチン細胞に特異的であることが以前から確認されている公知のマイトジェンである。Rubin等,Proc.Natl.Acad.Sci.USA 86,pp.802−806,1989;Finch等,Science 245,pp.752−755,1989;Marchese等,J.Cell.Phys.144,pp.326−332,1990。KGFをコードするメッセンジャーRNAの発現は、ヒト胚、新生児及び成人ソースの上皮組織に由来する幾つかの支質線維芽細胞系において、また正常な成人の腎臓及び胃腸管から抽出されたRNAにおいて検出されている。このようなRNAは神経膠細胞、肺、脳において、または偏平上皮癌、哺乳動物上皮細胞、永久分裂(immortalized)気管支上皮細胞、永久分裂表皮ケラチン細胞、及び表皮ケラチン細胞一次培養物を含めた様々な上皮細胞系においては検出されていない。上掲のFinch等,Science。しかし、KGFは連続マウス細胞系BALB/MKの細胞に対してはマイトジェンとして活性である。Weissman及びAaronson,C ell 32,p.599,1983;上掲のPNAS及びScienceも参照されたい。このような観察は、皮膚におけるKGFのパラクリン作用を、KGFが(表皮でなく)真皮において産生され、かつ表皮ケラチン細胞に作用することと共に示す証拠を補佐するものである。
加えて、ケラチン及びfilagrin遺伝子の発現を用いて、KGFが表皮ケラチン細胞の分化及び成熟に影響することが証明された。上掲のJ.Cell.Phys.を参照されたい。
KGFの生物学的活性を調べる上述の研究の多くは組み換えKGFを用いて行われており、この組み換え体はKGFのより広範な研究を可能にした。国際特許出願公開第90/08771号には、ヒト胚線維芽細胞系の馴らし培地からのKGFの精製、単離したポリペプチドの部分的アミノ酸配列決定、遺伝子のクローニング、及び細菌細胞(大腸菌)での発現により生物学的に活性なKGFの組み換え体を得ることが開示されている。
表皮ケラチン細胞のための刺激剤としてのKGFのin v itroでの役割は明確に確認されたが、成長因子としてのKGFに関しては、KGFが標的とし得るその他の種類の細胞、及びそのような細胞に対するKGFのin vivoでの作用を含めた多くのことが未知のままである。このような情報は、治療薬としてのKGFの効力を完全に認識するうえで肝要である。
発明の概要
手短に言えば、本発明はKGFの、表皮ケラチン細胞以外の上皮細胞に対する刺激作用に関する発見に係わる。特に、in vivoで用いたKGFが皮脂腺細胞(sebocytes)及び毛包細胞などの通常の付属器構造体、肝細胞などの肝臓細胞、及びタイプII肺細胞などの呼吸粘膜上皮細胞の増殖及び分化を誘導することが今や判明した。KGFが腸粘膜の細胞、例えば腺性胃及び小腸のムチン産生さかずき細胞、結腸の腺窩細胞、及び腸内の他の上皮細胞の増殖及び成長を刺激することの無視できない証拠も存在する。
本発明の基礎を成すこれらの発見は、上記特定種類の細胞の傷害または欠失によって特異的に特徴付けられた組織へのKGFの適用を可能にするという観点から重要な意味を有する。次に、本発明によりKGFで治療可能な疾患及び医学的状態について述べる。
毛包、汗腺及び皮脂腺などの付属器構造体の増殖及び分化を刺激することは、火傷その他の皮膚部分層及び全層損傷を負った患者の表皮及び真皮を再生させるうえで決定的に重要である。現在、表面欠損は瘢痕の形成と表皮ケラチン細胞による再被覆とによって治癒するが、皮膚の完全な再生は未だ不可能である。毛包、汗腺及び皮脂腺の再集団化は現在のところ、火傷を含めた皮膚全層欠損においては生起しない。KGFの使用は上記再集団化を可能にし得る。
表皮水疱症は表皮とその下層の真皮との付着の欠陥であり、しばしば破裂して痛い水疱を生じ、この水疱が症状を重くしかねない。このような外傷の再上皮化をKGFでの治療などによって加速すれば、感染の危険性が低下し、痛みが減少し、創傷の手当も軽減されよう。
化学療法誘発性脱毛症は、患者を悪性であるために何クールもの化学療法で治療した結果として起こる。目下のところ、一過性の毛髪消失をもたらす毛包細胞の死を予防するのに有効な治療法は無い。KGFはそのような手段を提供する。
男性型禿頭症はきわめて一般的であり、かつ実質的に治療不能である。男性及び女性の毛髪が次第に失われることは美容上の重大問題である。この状態はKGFを用いて全身的に、または薬物が頭皮を介して適用及び吸収可能である場合は局所的に、即ちエアガンもしくは類似の技術を用いて頭皮中へ噴射注入することによって治療し得る。
胃潰瘍は、H2拮抗薬で治療可能ではあるが罹患率及び再発率が高く、粘膜被覆(mucosal lining)の瘢痕形成によって治癒する。腺粘膜の再生を、例えばKGFでの治療によってより急速に実現できれば、胃潰瘍の治療は著しく改善されよう。
十二指腸潰瘍は胃潰瘍同様治療可能であるが、十二指腸の粘膜被覆をより完全かつ急速に再生させる治療薬を開発すれば重要な進歩となる。加えて、これらの潰瘍を再生によって治癒させ、その再発を低減する治療薬は有益であろう。KGFはそのような効力を発揮する。
(主に小腸を冒す)クローン病及び(主に大腸を冒す)潰瘍性大腸炎などの炎症性腸疾患は病因が未知の慢性疾患であり、罹患個体に粘膜表面の破壊、炎症、修復時の瘢痕及び癒着形成、並びに重大な罹患状態をもたらす。現行の治療法は炎症の制御を目差している。粘膜表面の再被覆を刺激してより急速な治癒を実現するKGFなどの治療薬は、疾患の進行を制御するのに有益であり得る。
消化管毒性(gut toxicity)は、放射線及び化学療法による治療方式の主要な制約要因の一つである。KGFでの前処理は小腸粘膜に対して細胞保護作用を及ぼし得、上記のような治療法での用量を増加させつつ、腸毒性の潜在的かつ致命的副作用を軽減する。
KGF治療には、胃腸管全体にわたる粘液生成への著しい効果が有る。この特性は、摂取される傷害性物質からの消化管粘膜の保護や、炎症性腸疾患などの状態における損傷の拡大の制限に有用であり得る。
早産児のヒアリン膜症は、肺内でタイプII肺細胞により界面活性剤が産生されない事態を招き、その結果肺胞の虚脱が起こる。28週齢以降の胎児における成熟及び分泌はコルチコステロイドによって大幅に加速できるが、それより幼い胎児のための治療法は現在のところ無く、そのためにこの集団の罹患率及び死亡率はきわめて高くなっている。タイプII肺細胞の増殖及び分化を誘導するKGFなどの治療薬は、この疾患の治療に著しく有益であろう。
煙の吸入は、細気管支上皮及び肺胞の壊死に起因して、火傷を負った次の週における罹患及び死亡の重大な一因である。上記構造体の増殖及び分化を刺激してその修復及び再生を誘導し得るKGFなどの成長因子は吸入損傷の治療に有益であろう。
気腫は、肺胞が徐々に失われた結果として生じる。再成長を刺激し得るか、または残存肺胞に対して細胞保護的であるKGFなどの成長因子は治療に有益であろう。現在のところ、有効かつ利用可能な治療法は無い。
ウイルス性肝炎及び慢性アルコール摂取に続発する肝硬変は、罹患及び死亡の重大な一因である。KGFなどによって肝細胞を保護し、増殖させ、かつ分化させて肝機能を増強することは、硬変の進行を遅らせ、または阻止するのに有益であろう。
激症肝不全は末期硬変と共に生起する、生命を脅かす状態である。残存肝細胞の増殖を誘導し得るKGFなどの薬物は、目下のところ肝臓移植でしか治療し得ないこの疾患に対して直ちに有益であろう。
急性ウイルス性肝炎はしばしば無症状で、かつ自己限定的である。しかし、少数の患者においては重篤な肝臓障害が数週間にわたって残る恐れが有る。KGFなどの細胞保護剤は肝細胞変性の予防に有用であろう。
アセトアミノフェン、ハロタン、四塩化炭素その他の毒素によって惹起される肝臓の中毒損傷は、肝細胞に対して細胞保護的である成長因子(KGF)によって回復できよう。
従って本発明は、KGFを治療に(または適当であれば予防に)用いて上述の諸状態を処置することと、KGFを治療に有効な適量で含有する医薬製剤とを包含する。

特定実施態様の説明
KGFが、治療に有用であり得ることを示唆する著しい生物学的作用を示す特定種類の上皮細胞を同定するべく、動物生体及びin vivoでのKGF投与を用いて広範な研究を行ない、結果を観察及び分析した。以下の各実施例では組み換えKGFを用い、この組み換え体は次のように調製した。
ヒト包皮線維芽細胞(AG1523)RNAから組み換えKGF遺伝子をPCRによって単離し、得られたDNAをpCFM1156発現ベクターのNde I及びBamH I制限部位間に連結した。プラスミドpCFM1156は本明細書に参考として含まれる米国特許第4,710,473号に開示されたプラスミドpCFM836から、2個の内因性Nde I制限部位をT4ポリメラーゼ酵素での末端補足(end filling)によって破壊し、続いて平滑末端を連結してユニークなCla I及びKpnI制限部位間の小型DNA配列を、配列番号1及び2を付した次のオリゴヌクレオチド対によって置換することにより取得可能である。
次に、標準的なエレクトロポレーション(BioRad Gene Pulser)形質転換操作を用いてプラスミドpCFM1156KGFをFM5宿主細胞(ATCC #53911)にクローン化した。観察された内部翻訳開始を低減するために、KGF遺伝子のKpn I及びEcoR I間のDNA配列を、内部リボソーム結合部位の使用を減らすべく設計された合成オリゴヌクレオチドDNAによって置換した(図1)。連結及びFM5細胞(ATCC #53911)へのエレクトロポレーションによる導入の後、pCFM1156KGF−dsdを有するクローンを選択した。KGF遺伝子をコードするDNAの配列を確認した。その後、細胞を、有毒副産物の蓄積を防止するためのグルコース制限下に細胞が指数関数的速度で成長することを可能にする標準的な流加培地を収容した10l容の発酵器内で30℃で培養した。指数増殖期のさなかの細胞密度において、温度を一時42℃に上昇させてKGF遺伝子の転写を誘導し、その後発酵の残りのKGF誘導相の間約37℃に維持した。細胞ペーストを回収し、凍結貯蔵した。機械的に溶解させた細胞ペーストから生物学的に活性なKGFを、このタンパク質の非常に高い等電点(Pharmacia社のS−Sepharose Fast Flow)及び分子量(Pharmacia社のSuperdex 75)を利用する標準的なクロマトグラフィーによって精製した。精製した組み換えKGFタンパク質を、上掲のRubin等,PNASに述べられているマイトジェンアッセイにおいて内毒素及び生物学的活性に関して測定した。
実施例1
in vivo創傷治癒モデルにおける付属器構造体の、KGF によって刺激される増殖及び分化
この実施例では、改良したウサギ耳皮膚部分層創傷モデルを用いた。Mustoe等がJ.Clin.Invest.87,pp.694−703,1991に述べているウサギ耳皮膚潰瘍モデルを改良し、軟骨を貫通して耳の裏側の真皮に達する創傷を6mm穿孔器を用いて設けた。その結果、創傷は軟骨下側の上皮要素の急速成長(sprouting)によって治癒し、収縮は治癒の際のパラメーターでなくなり、従って新しい組織の正確な定量が可能となる(図2参照)。先に述べたように調製した組み換えKGFを、創傷を治癒させる治療薬として用いた。
物質及び方法
KGF、またはリン酸緩衝溶液賦形剤のみを手術日に1回適用し、創傷(0.25cm2)をTegaderm密封包帯(3M Company,St.Paul,MN製造)で被覆した。殺す前の各動物にBrdU(Aldrich Chemical Company,Milwaukee,WI)を、体重1kg当たり50mgの量で静脈内注射した。BrdUの投与は、基底表皮ケラチン細胞増殖の程度と、基底細胞の有棘層及び角質層へ向かっての移動の速度(kinetics)とをより良く定量するために用いた。動物を殺した後、各創傷を二分し、一方の切片は最適冷却温度培地(OCT;Miles Inc.,Elkhart,IN)中で凍結させ、第二の切片はOmnifix(Al−Con;Genetics,Inc.,Melville,NY)に固定して通常の組織学的方法に従い加工した。各創傷の3μm厚切片に、マッソン三色染色、オイルレッドO染色及び免疫組織化学(IHC)染色を行なった。
再上皮化の測定
各創傷に関して、創傷全体の空隙及び上皮空隙を目盛付きのステージマイクロメーターを用いて測定した。各創傷に関して全体空隙と上皮空隙との差を求め、これを全体空隙で除することにより、各創傷の再上皮化のパーセンテージを算出した。創傷一つ当たり2個の切片から得たデータを平均した。上皮空隙測定値の差、及び再上皮化のパーセンテージを、片側検定不対Student t試験(one−tailed,unpaired Student's t−test)によって分析した。
治療した創傷及び治療しない創傷において生成した上皮の面積を、各投与量グループに関して算出した。各投与量に関して一方向ANOVA及びDunnett t試験(one−way ANOVA and Dunnett's t−test)を、対照グループに対して行なった。
上皮細胞の増殖及び分化の測定
パラフィンに包埋した3μm厚の組織切片を、抗BrdU(Dako Corp.,Carpinteria,CA)、アビジン−ビオチン複合体(Vector Laboratories,Inc.,Burlingame,CA)及びジアミノベンジジン基質(DAB;Sigma Chemical Co.,St.Louis,MO)を用いて染色した。切片を0.1%プロテアーゼ溶液で消化し、続いて2N HClで処理した。内因性ペルオキシダーゼを、3%過酸化水素溶液への暴露によってクエンチした。スライドを、ウマ正常血清をリン酸緩衝溶液(PBS)に加えた10%溶液でブロックし、その後1%ウシ血清アルブミンで1:400に稀釈した抗BrdUと共にインキュベートした。洗浄後、切片を、1% BSAで1:100に稀釈したペルオキシダーゼ結合アビジン−ビオチン複合体と共に20分間インキュベートした。その後、スライドをDAB基質(10mgのDAB、20mlのPBS、20μlの30%過酸化水素)に10分間暴露した。切片をヘマトキシリンで対比染色した。
BrdUで処理した毛包の組織分析
BrdUを添加し、かつIHC染色を行なった後、各創傷床の毛包総数を計数し、Kruskall−Wallis多重比較試験を用いて分析した。更に、各処理グループにおいて10個以上の増殖細胞を有する毛包が存在する創傷のパーセンテージについてのχ2乗試験なども行なった。加えて、6個以上の増殖細胞を有する総ての毛包においてBrdU陽性染色細胞を計数した。
皮脂腺のオイルレッドO染色
中性脂肪に特異的な染色材であるオイルレッドOを用いて皮脂腺細胞及び皮脂腺を同定し、この同定はF.L.CarsonがHistotechnology:A Self−Instructional Text,ASCP Press,Chicago,1990に述べている操作を用いて凍結切片に対して行なった。手短に言えば、7μm厚の凍結切片を自然乾燥させ、亜鉛−ホルマリン中で10分間固定した。スライドを0.3% w/vオイルレッドO(Sigma Chemical Co.,St.Louis,MO)に浸漬し、次に室温で30分間60%イソプロパノールに浸漬して脱色し、Learnerヘマトキシリンで対比染色した。皮脂腺の寸法、及び皮脂腺1個当たりの細胞数を評価した。
結果及び検討
4〜40μg/cm2のKGFで治療した創傷では、加速された再上皮化と上皮厚の増加とが観察された。治療後5日目の創傷は対照に比較して著しく促進された再上皮化を示した(1μgのKGFを用いた場合で76.7%対52.5%)。新しい上皮の厚みは治療後5日目及び7日目までに投与量に応じて増加し、創傷一つ当たり10μgのKGF投与量では対照創傷の上皮の約2倍の面積に達した(図3)。組織分析は、上皮の再生が主に真皮中の付属器要素、即ち皮脂腺、汗腺及び毛包からの急速成長によって生起したことを示唆した。
基底表皮ケラチン細胞の増殖及び基底細胞の移動の分析は、KGF治療の初日を過ぎて多数の増殖表皮ケラチン細胞が創縁に存在することを明示した(図4)。KGF治療2日目までに、表皮の基底上層の多数の増殖表皮ケラチン細胞によって、増殖細胞が有棘層に向かって移動する時間の短縮が示唆された。KGF治療5日目までに、創傷の基底層では増殖が低下し、このことは上皮の修復及び分化の加速が自己限定性であることを示している(図4)。表皮ケラチン細胞は上方へ移動して角質層を構成したので、正常に成熟したと考えられる。
意外にも、皮脂腺細胞の増殖及び分化の促進、並びに毛包の増殖の促進も観察された。毛包も皮脂腺も、KGF治療創傷では対照創傷に比較してより大きくかつより多くなったと考えられる(図5及び図6をそれぞれ参照)。皮脂腺を選択的に同定するべくオイルレッドOで染色した組織切片はきわめて多数の皮脂腺を示し、これらの皮脂腺はまた著しく肥大しており、このことは皮脂腺細胞の増殖と皮脂産生細胞への分化とが促進されたことを示している。KGF治療創傷では、毛包1個当たりの増殖細胞の数が投与量に応じて増加することも観察された。
以前、表皮成長因子(EGF)及び塩基性の線維芽細胞成長因子(FGF)がウサギ耳モデルにおいて検討された。上掲のJ.Clin.Invest.及びPierce等,Amer.J.Patho l.140,pp.1375−1388,1992。上記両成長因子は再上皮化を刺激することが知られているが、EGFも塩基性FGFも皮脂腺及び毛包などの付属器構造体の増殖や分化には影響を及ぼさない。皮膚中の多様な上皮細胞の増殖及びその後の分化を刺激するKGFの能力は、KGFが本来線維芽細胞から単離されることと相俟って、KGFが皮膚再生過程の強力なパラクリン刺激物質であることを示唆している。
実施例2
in vivoタイプII肺細胞の、KGFによって刺激される増 殖及び分化
この実施例は、KGFの投与が健康なラットの気道の上皮細胞に及ぼす影響を評価するために行なった。
物質及び方法
Ulich等がAmer.J.Pathol.138,pp.1485−1496,1991に述べているプロトコルを用いて、各々200〜250gの体重を有する複数の雄のLewisラットに食塩液またはリン酸緩衝溶液で稀釈したKGFを様々な投与量で1回気管内注入した。KGFの注入は0.1、1.0、5.0及び10.0mg/kgの投与量で行なった。対照ラットには賦形剤を気管内注入した。KGF注入の6時間後並びに1、2、3、4、5及び6日後の時点にラットを殺し、肺を気管内カテーテルを用いてブアン固定液で膨張させ、肺の矢状切片をパラフィンに包埋し、組織切片をヘマトキシリン及びエオキシンで染色した。
結果及び検討
0.1mg/kgのKGFは、組織学的に認識できる肺胞上皮細胞形成を惹起しなかった。1.0mg/kgのKGFは肺胞上皮細胞の増加を、軽度にではあるが明らかに惹起した。5.0及び10.0mg/kgの投与量では、それぞれ肺胞上皮細胞過形成の助長は認められなかった。KGFは、気管内注入の6時間後または1日後には肺胞上皮細胞の組織学的に認識できる増加を惹起しなかった。2日後には、KGF処理ラットの肺内に肺胞中隔上皮細胞の瘤状の微小乳頭状発育過度が認められた(図7、図8)。3日後には、肺胞上皮細胞の低立方体様成長から立方体様成長までが肺の大区域の全肺胞を覆って散在するのが認められた(図9、図10)。しかし、肺の幾つかの領域は正常な組織を保った。3日目のラットの過形成肺胞上皮の組織の外観は、ヒトの肺における反応性タイプII肺細胞過形成の外観と実質的に同じであった。過形成肺胞上皮は、気管内注入後4日目及び5日目のKGF処理ラットの肺では僅かしか識別できなかった。6日目、KGF処理ラットの肺と対照ラットの肺との区別は不能となった。
気管内注入の3日後に、KGF処理ラットの肺を超微細構造に関して調べた。肺胞中隔を被覆する過形成肺胞上皮細胞はほぼ必ず、様々な寸法及び形状のラメラ封入体を1個以上有した(図11、図12)。KGF処理ラットの気管支上皮は3日目に過形成となったが、この過形成は肺胞上皮細胞の過形成ほどは直ちに目立たなかった。過形成は、通常単一の上皮層によって被覆される比較的小型の遠位気管支の被覆上皮が、気管支上皮の房状分岐化または微小乳頭状成長によって、及び気管支上皮中での有糸分裂像の増加によって擬似層化することと理解された(図13A、図13B)。
気管内投与したKGFは、肺胞上皮細胞の顕著な過形成を惹起する。細胞内にラメラ細胞質封入体が超微細構造レベルで存在することは、過形成細胞がタイプII肺細胞であるという提言と合致する。ラメラ封入体は、ラットにおける実験的タイプII肺細胞過形成の幾つかの事例で説明されているほど多数存在する、もしくはオスミウム親和性であるとは考えられなかったが、正常なラット肺に関して説明されているラメラ封入体にきわめて類似している。肺細胞過形成の惹起に加えて、KGFは気管支上皮細胞の過形成も惹起する。過形成肺胞上皮が、気管支上皮の終末細気管支から肺胞実質中への下方増殖を意味することは可能である。しかし、KGF注入の2日後に肺胞中隔上の肺細胞の多病巣性微小乳頭状突起形成(budding)成長パターンが、3日目の立方体様細胞のよる肺胞の融合性被覆の前に出現するので、KGFがタイプII肺細胞に直接作用する可能性の方が高いと考えられる。加えて、タイプII肺細胞は有糸分裂応答性の肺胞上皮細胞集団であると考えられ、KGFなどの、肺胞細胞成長の潜在的な内因性メディエイタに応答する肺胞細胞種であることが予測される。最後に、過形成細胞内でのラメラ封入体の同定は、前記細胞がタイプII肺細胞に分化することのみでなく、該細胞がタイプII肺細胞を起源とすることも示唆する。
KGFがタイプII肺細胞に及ぼす刺激作用は、肺に存在するKGFレセプターのメッセンジャーRNAの、全RNAの等量試料当たりの量が若い成体ラットに由来する他の様々な器官に関して見出されるレベルの2〜3倍であるという観察と合致する。この実験ではKGFレセプターのメッセンジャーRNAのレベルを、改良Ambion RNアーゼ保護アッセイ(RPA II No.1410)を用いて検出した。
このアッセイで用いた“アンチセンス"RNAプローブは次の配列(配列番号3)を有した。
アンチセンスプローブはPromega Riboprobe Gemini IIキット(#P2020)を用いて、対応する上記RNA配列をコードする直鎖状の鋳型DNAを用いて調製した。尿素−ポリアクリルアミドゲルからRNAプローブを、適正寸法のバンドを切り取り、Ambion溶液F中にRNAプローブを溶離することによって精製した。
“アンチセンス”プローブに対して相補的であるRNA配列をコードする“センス"RNA基準配列は次のとおりである(配列番号4)。
センス基準配列は、Promega Riboprobe Gemini IIキット(#P2020)を用いて(放射性標識を用いずに)低温で調製し、Sephadex G50クイックスピンカラムで精製した。
標準的なAmbion(RPA II #1410)プロトコルに続いて、50μgの全RNAを105cpmのプローブと共にまず95℃で4分間、次いで45℃で12〜18時間インキュベートした。RNアーゼ(1:500Ambion溶液R)をハイブリダイゼーション溶液と共に37℃で40分間インキュベートし、その後不活性化/沈澱試薬(溶液Dx)を添加した。
次に、尿素−ポリアクリルアミドゲル電気泳動を用いてRNアーゼ保護断片を大きさに基づいて分離し、ゲル全体をPhosphorimagerスクリーンに対して露出た。その後、バンドをMolecular Dynamics Phosphoimagerにおいて定量した。結果を図14に示す。
KGFをタイプII肺細胞のための成長因子として同定することで、KGFが胎児肺成熟の刺激剤としてグルココルチコイドに類似の潜在的治療能力を示す可能性が出現する。KGFは、成体において肺傷害後に気管支肺胞修復を刺激する臨床上の有用性も有し得る。KGFがタイプII肺細胞に及ぼす増殖促進作用は、KGFが界面活性剤の合成及び分泌の調節において重要な役割を果たし得ることを示唆している。
実施例3
肝細胞の、KGFによって刺激される増殖及び分化
全身投与したKGFが成体ラットの肝臓の上皮組織に及ぼす作用を評価した。
物質及び方法
複数の雄のラットに水及び食物を随意に与えた。総ての動物にKGFまたはPBSを、1日、または4日間もしくは7日間毎日腹腔内注射によって与えた。いずれの実験でも、1グループ当たり少なくとも5匹の動物を分析した。
全ラットをCO2窒息によって安楽死させ、死亡直後の心臓穿刺によって血清化学試験用の血液を得、アッセイ時まで凍結貯蔵した。血清を、標準的な血清化学物質及び電解質に対して分析した。肝臓を予備知識無しで(in a blinded fashion)計量し、重量を体重100g当たりのグラム数(体重に基づくパーセンテージ)として表わした。組織試料を、通常の組織加工のために10%緩衝ホルマリン中に置いた。
肝臓から取得した切片をヘマトキシリン及びエオシン(H及びE)で染色した。殺す1時間前、ラットに50mg/kgのBrdUを腹腔内注射した。抗BrdU免疫組織化学染色を行なって、複製細胞のDNA中へのBrdUの組み込みを可視化した。
肝臓の標識指数(LI)を、目盛付きの視認用対物鏡グリッド(eye objective grid)を用いて決定した。動物1匹当たり10の視野について計数し、平均値を求めた。単位面積当たりの全核数及び標識核数を計数した。同様面積の肝臓実質を代表するものとして計数を行なった視野を標準化するべく、計数を行なった視野はいずれも門脈三分岐付近に定めた。
全データを、両側検定不対Student t試験または多グループ比較用の一方向ANOVAを用いて分析した。
結果及び検討
投与量応答試験を行なって、KGFの有効投与量を確認した。KGFを4日間毎日注射し、器官を採取た。体重に基づくパーセンテージで表わした肝臓は、ANOVAによって比較した場合、1日当たり5mg/kgの最高投与量においてのみ対照と甚だしく相違した(図15)。留意するべきことに、肝臓によって合成される化合物である血清タンパク質、コレステロール及びトリグリセリドは1及び5mg/kgのKGFのいずれの下でも対照に比較して甚だしく高レベルであった(図16)。
動物を、5mg/kgのKGFで1日、または4日間もしくは7日間毎日処理した後に分析した。肉眼的剖検の際、7日経過時点の肝臓は甚だしく肥大していた。KGF処理ラットの肝臓重量はいずれの検査時点においても甚だしく増加していた(7日目でKGF 4.08±0.08%に対して対照3.49±0.06%;p=0.0003)。
BrdU陽性核を定量したところ、KGF処理ラットにおいては1日で対照の3〜4倍に増加したことが判明した(図17)。高倍率視野一つ当たりの有糸分裂像として表わされる有糸分裂指数は、1日の処理では対照よりはるかに大きくなり、この結果はBrdU計数結果と一致した。
これらの結果は、KGFが肝臓に及ぼす強力なマイトジェン作用及び分化促進作用を証明するものである。KGF処理から1日で肝臓重量が急速に増加したことは、有糸分裂指数が増大し、かつBrdU陽性画分が増加したことと相俟って、主に上皮から成るこの組織へのKGFの急速かつ強力な作用を明示している。KGFの有糸分裂促進作用は急速に低下するが、体重に基づくパーセンテージで表わした肝臓重量は試験の間中増加し続けた。重量の増加は主として細胞の増加に起因し、細胞の増加は1日目という早期に組織学的に観察され得、かつ4日及び7日の処理の間中継続した。肝臓質量は肝臓再生の際、2週間より短期間で2倍より多く増加し得るので、1日の治療の後に生じ始める著しい差異を見出しても意外ではない。
アルブミンレベルの上昇は、脱水症以外のいかなる既知病態とも関連しない。KGFは恐らく、多数の肝細胞を存在させることによってより高い血清中レベルのアルブミン(及び他のタンパク質)を誘導していた。KGFによって誘導されるより盛んなタンパク質合成は、末期肝疾患または肝硬変に罹患した個体の救命にすばらしく有益であろう。
実施例4
部分肝切除後のin vivo肝臓細胞の、KGFによって刺激 される増殖及び回復
この実験では検体として、各々300〜340gの体重を有する複数の雄のSpraque−Dawleyラットを用いた。
物質及び方法
Higgins及びAnderson,Archives of Pathology,Volume 12,p.186,1931の操作を用いて、各ラットに2/3部分肝切除を行なった。手術の8時間後から始めてその後は継続的に1日1回、ラットにPBS(対照グループ)またはKGFを1mg/kgの投与量で皮下注射した。手術の4日、7日、10日または14日後、対照ラット及びKGF治療ラットを体重測定して殺した。肝臓残遺物を取り出し、その重量を計測した。
結果及び検討
部分肝切除したラットをKGFで治療すると肝臓質量の回復が、図18及び図19にそれぞれ示したように絶対的にも相対的にも著しく加速されることが判明した。KGF治療グループでは、肝臓質量は7日以内に手術前の値に戻った。これに対して、対照グループ(KGFで治療せず)では絶対的肝臓質量も相対的肝臓質量も、14日経っても完全には回復しなかった。
実施例5
化学的に誘導した肝臓毒性に対するKGFの回復作用
各々300〜340gの体重を有する複数の雄のSpraque−Dawleyラットを、化学毒素導入後の肝臓に対するKGFの治療作用を評価するべく試験した。
投与
0日目、各々12匹の雄のSpraque−Dawleyラットから成る複数のグループに、トウモロコシ油賦形剤中の1ml/kgまたは2ml/kgの四塩化炭素(CCl4)を与えた。各投与量グループを、各々6匹のラットから成る二つのサブグループに分割した。CCl4投与の3時間後から始めてその後毎日、各サブグループにPBSか、または1mg/kg/日のKGFを皮下注射した。CCl4投与の22時間、72時間及び144時間後に各ラットから血液試料を採取し、血清グルタミン酸−オキサロ酢酸トランスアミナーゼ(SGOT)レベルを測定した。
結果及び検討
図20に示したように、CCl4経口投与後にKGFを皮下注射すると、公知の肝臓損傷指示物質である酵素(SGOT)の血清中レベルが著しく低下した。この効果は投与後1日目に、特に2ml/kgの投与量において顕著である。
実施例6
腸粘膜の上皮細胞の、KGFによって刺激される増殖及び 分化
この実施例では、KGFが健康なラットの腸粘膜の上皮細胞に及ぼす生物学的作用を評価する。
物質及び方法
雄のラットに水及び食物を随意に与えた。総ての動物にKGFまたはPBSを1日、または4日間もしくは7日間毎日腹腔内注入した。いずれの実験でも、1グループ当たり少なくとも5匹の動物を分析した。
全ラットをCO2窒息によって安楽死させた。主要器官の重量を予備知識無しで計測し、計測した重量を体重100g当たりのグラム数(体重に基づくパーセンテージ)で表わした。消化管を非腺性前腸と、腺性胃と、小腸と、大腸とに分割した。組織試料を、通常の組織加工のために10%緩衝ホルマリン中に置いた。
全器官から取得した切片をヘマトキシリン及びエオシン(H及びE)で染色した。殺す1時間前のラットに50mg/kgのBrdUを腹腔内注入した。標準的な方法を用いて抗BrdU免疫組織化学染色を行ない、複製細胞のDNAへのBrdUの組み込みを可視化した。選択した組織を、過ヨウ素酸−シッフ試薬(PAS)、pH2.5のアルシアンブルー、及びマッソン三色染色液で染色した。
目盛付きの接眼鏡を用いて胃粘膜の厚みと、胃粘膜におけるPAS染色の深さとを測定した。同様に、十二指腸の絨毛の長さ及び腸腺窩の深さ、並びに結腸の結腸腺窩の深さも測定した。さかずき細胞生成の変化を定量するべく、十二指腸絨毛の基部から100μmまでの長さに沿ってPAS陽性さかずき細胞を計数した。消化管のいずれの領域においても、測定は下側に位置する筋に対して垂直である腺または絨毛についてのみ行なった。測定結果はミクロンで、または小腸さかずき細胞の場合単位面積当たりの平均個数で報告する。加えて、結腸腺窩数、及び結腸の所定長を占める過形成(二分岐または三分岐)腺窩の数も測定した。動物1匹当たり5〜10回の反復測定を行ない、平均値を求めた。全データを、両側検定不対Student t試験または多グループ比較用の一方向ANOVA(Statview II,Abacus Concepts,Berkeley,CA)を用いて分析した。
結果及び検討
最初に投与量応答試験を行なって、KGFの有効投与量を確認した。KGFを4日間毎日注射し、器官を採取た。腺性胃、小腸及び結腸において顕著な投与量応答性が観察された(図15)。
5mg/kgのKGFで1日、並びに4日間及び7日間毎日処理した動物を分析した。肉眼的剖検の際、4日間及び7日間処理した動物では前腸が甚だしく肥大し、かつ粘膜が褶曲していた。前腸、腺性胃、十二指腸及び結腸は4日及び7日の処理では甚だしい重量増加を示したが、1日の処理ではそのようなことは無かった。7日処理した腺性胃の重量は、KGF処理動物では0.65±0.01g、対照では0.48±0.02であった(p=0.0001)。
非腺性の前腸の重量は、4日及び7日の処理では甚だしく増加した。組織学的にみて、KGF処理動物では非腺性前腸の偏平上皮が軽度(4日目)から重度(7日目)に過形成となった。増殖上皮は噴門において突然、食道の正常な偏平上皮から処理前腸の増殖上皮へと変化した。腺性胃粘膜は1日の処理後、組織学的に正常と考えられた。4日及び7日の処理では、多数のさかずき細胞が透明でオープンな細胞質を有する細胞であることが組織学的に明らかとなった。KGFで4日間及び7日間処理した動物の粘膜の厚みは軽度に増加した(図21)。7日の処理では粘液頚細胞層が伸張して漿膜表面の方へ変位した。4日及び7日処理した時のPAS染色は、PAS陽性(ムチン産生)さかずき細胞の数及び大きさが著しく増大したことを示した。更に、BrdU染色切片から、上記さかずき細胞の層は腺性胃の分裂細胞層として確認され、処理を行なった場合の増殖細胞の増加が判明した。胃粘膜の内腔面のPAS陽性層の厚みも、4日及び7日のKGF処理では著しく増大した(図21)。
小腸は、いずれの試験時点においても肉眼的には正常であった。PAS染色は粘液産生さかずき細胞が多数存在することを示した。この相違は、4日及び7日の処理ではなはだしかった(図22)。絨毛の長さはいずれの時点にも異ならなかったが、腺窩の深さは1日及び7日の処理では著しく増大した(図22)。
7日処理グループでは結腸粘膜に襞様の重なりが生じ、その結果総表面積が甚だしく増加した。過形成、二分岐及び三分岐結腸腺窩の指示物質は対照を含めた総てのグループで観察されたが、短期間、例えば1日及び4日の処理のグループにおいて最も大量かつ顕著であり、過形成腺窩の数は処理が長期となると共に減少した(図23)。対応して、結腸腺窩の深さは処理の継続と共に増大した(図24)。
PAS及びアルシアンブルー染色は、KGF処理後の結腸全体における粘液産生さかずき細胞の増加を示した。加えて、内腔面に存在するアルシアンブルー陽性さかずき細胞の数は、7日の処理では全処理動物において増加した。処理動物は4日目という早期に、結腸腺窩の上半分及び1/3においてPAS陽性細胞の著しい増加を示した。
これらの結果は、GI管内の上皮組織に対するKGFの強力なマイトジェン作用及び分化促進作用を証明している。BrdU標識から、KGFが胃粘膜の粘液頚層の細胞の増殖を開始することは明らかである。上記細胞は腺性胃の前駆細胞を構成する。分裂後、上記層由来の細胞は腸内腔に向かって上昇して表面細胞となるか、または漿膜表面に向かって下降して、胃腺を占める細胞となる。(Toner等,Gastrointestinal and Esophageal Pathology,pp.13−28,Churchill Livingstone,New York,NY,1989)。加えて、KGFは、粘液頚細胞が上方へ移動する細胞及びさかずき細胞となる細胞へと選択的に分化するのを促進する。胃粘膜の総体的な厚みは4日及び7日の処理で増加した。これに類似する作用は小腸及び大腸でもみられた。KGFは細胞分裂を促進し、その結果前駆細胞の刺激を介してより深い腺窩をもたらした。KGFはまた、粘液産生の増加、及び絨毛または腺窩のさかずき細胞数の増加が証明するように、分裂腺窩細胞の分化を誘導した。
ここに本発明を、特定の具体例及び実施例と関連付けて説明したが、この説明に限定の意図が無いことは理解されるべきである。即ち、例えば天然KGFのものと実質的に同じ生物学的特性を有する任意形態のKGFを上述の用途に用いることができる。前記形態には、精製された天然KGFそのもの、化学的操作によって合成されたKGF、及び先に説明した以外の発現系を用いて組み換え技術により取得されたKGF、並びに天然アミノ酸配列に基づく類似体、変異体、キメラ等が含まれる。これらの形態はいずれも、本発明の実施において用いるべく企図されたものである。
当業者も理解するように、KGFタンパク質コーディング配列の発現には様々な宿主−ベクター系を用い得る。そのような系には、ウイルス(例えばワクシニアウイルス、アデノウイルス等)に感染させた哺乳動物細胞系;ウイルス(例えばバキュロウイルス)に感染させた昆虫細胞系;酵母ベクターを含む酵母などの微生物;またはバクテリオファージDNA、プラスミドDNAもしくはコスミドDNAで形質転換した大腸菌以外の細菌が非限定的に含まれる。上記ベクターの発現要素はその強さ及び特異性において様々である。用いる宿主−ベクター系に応じて、幾つかの適当な転写及び翻訳要素のうちのいずれかを用い得る。
本明細書に述べたような方法、または当業者に公知である他の操作を用いてKGF cDNA発現のタンパク質産物を単離し、精製し、かつKGF活性に関してアッセイしたら、該タンパク質産物を様々な医薬組成物中に配合することができる。上記組成物には典型的には、治療薬物(KGF)に適した、普通化学的に定義されたキャリヤまたは賦形剤と、所期の投与形態に応じた他の成分とを含有させる。組成物は水性キャリヤを含有し得、またはKGFがコラーゲン、ヒアルロン酸及び様々なポリマーなどの非水性キャリヤ中に存在する固相製剤から成り得る。組成物は、注射、経口投与、局所的投与、鼻腔内投与及び肺内放出を含めた様々な方途で投与するべく適宜調製し得る。
当業者には理解されるように、投与するべきKGFの量は治療する疾患及び投与経路次第で様々となるが、例えば体重1kg当たり0.01〜500mgが有効であり得る。KGFは、患者の疾患及び状態に応じて1回用いても、また繰り返し投与してもよい。KGFを、皮膚、肺、肝臓及び胃腸管の疾患の治療に用いられる他のサイトカイン及び成長因子並びに他の医薬調製物と共に用いることも可能である。

Claims (12)
- 医療上許容し得る担体及び有効量のケラチノサイト成長因子産物を含み、肝臓、肺及び胃腸管に含まれる肝細胞、タイプII肺細胞、ムチン産生杯細胞及び他の上皮細胞並びにこれらの前駆体細胞からなる群から選択される上皮細胞の増殖及び分化を刺激することにより、患者の状態を治療または予防するための医薬組成物。
- (A)胃潰瘍の治療または予防を含んでなる、処置、
(B)十二指腸潰瘍の治療または予防を含んでなる、処置、
(C)胃および食道の糜爛、処置、
(D)炎症性腸疾患の治療または予防を含んでなる、処置、
(E)放射性または化学療法に誘発される消化管毒性の治療または予防を含んでなる、処置
(F)ヒアリン膜症の治療または予防を含んでなる、処置、
(G)気道熱傷に起因する気道上皮壊死の治療または予防を含んでなる、処置、
(H)気腫の治療または予防を含んでなる、処置、
(I)肺炎症および肺線維症の治療または予防を含んでなる、処置、
(J)肝硬変の治療または予防を含んでなる、処置、
(K)劇症肝不全の治療または予防を含んでなる、処置、
(L)急性ウイルス性肝炎の治療または予防を含んでなる、処置、
(M)毒性肝炎の治療または予防を含んでなる、処置、
のための医薬上許容し得る担体及び有効量のケラチノサイト成長因子産物を含む医薬組成物。 - 胃および食道の糜爛が糜爛性胃炎、食道炎、食道逆流の治療または予防を含んでなる、請求項2に記載の医薬組成物。
- 医薬上許容しうる担体が非水性担体である、請求項1乃至3に記載の医薬組成物。
- 非水性担体がコラーゲン、ヒアルロン酸またはカルボキシメチルセルロースである、請求項4に記載の医薬組成物。
- ケラチノサイト成長因子産物が天然のケラチノサイト成長因子であるか、化学的に合成されたケラチノサイト成長因子であるか、または組換え生成ケラチノサイト成長因子である、請求項1乃至5のいずれか一項に記載の医薬組成物。
- 該組換えケラチノサイト成長因子が微生物細胞により産生される、請求項1から8のいずれか一項に記載の医薬組成物。
- 微生物細胞が大腸菌である、請求項9に記載の医薬組成物。
- In vivoで使用される、請求項1から10のいずれか一項に記載の医薬組成物。
- 該ケラチノサイト産物が注射により投与される、あるいは経口、鼻腔、または肺から投与される、請求項1から11のいずれか一項に記載の医薬組成物。
