Description
気外科用切断凝固電極に関する。
患者の組織を通って、接地パッド又はケーブルに流れさ
せる。電流は、活性切断電極で組織を切断し、切断速度
はその領域の組織を通る電流密度によって決まる。低い
電流密度では、熱が発生し、切断は達成されない。高い
電流密度では、急速な切断が起こる。
患者の組織を通って、切断電極から短い距離離れて患者
の組織内に置かれ、又は、これと接触しているリターン
電極に流れる。切断電極及びリターン電極は、一般的に
は、単一の器具によって支えられる 電流密度は、加えられる(ワットで測定した)電流に
よって決まり、この目的のために設計された従来の発電
機にある調節装置を利用して制御することができる。電
流密度はまた、全回路の直列インピーダンスによっても
決まる。直列インピーダンスは、回路を通る電流に対す
る抵抗の総計に等しい。直列インピーダンスは活性電極
の材料と設計によって,患者によって,切断すべき組織

電極は、尿生殖路の手術、特に、前立腺手術に適応され
ている。しかし、本発明による電極は、電気外科的電極
の適するどんな外科処置にも用いることができる。その
ような場合には、適当に、他の形状寸法を利用すること
ができる。本発明による電気外科用電極を、従来の市販
の切除用内視鏡,高周波発生器及び接地パッドと一緒に
利用してもよい。

す装置は、1989年1月18日に出願された出願第.07/298.
477号に、より完全に説明されている。電気外科用切断
電極10は、長手方向に延びた膨張可能なブラダー15にほ
ぼ平行に沿って延びるワイヤ20の性質を帯びている。使
用中、ブラダー15を、身体の導管の中へ外科的切断の必
要な位置まで長向に挿入する。次いで、高周波電流がワ
イヤ20に流しながら、ブラダー15を膨らませる(一般的
には、内側に延びないブラダーを用いる)。これによっ
て、ワイヤを外方に動かし、隣接した組織をその方向に
切開する。ブラダー15と反対に向いて設けられたスリッ
ト24をもつ鞘22がワイヤ20を取り囲む。本発明の改良

- 【請求項1】電気信号に応答して、電流を患者の身体組
織に差し向け、この身体組織に外科的機能を行う、側面
及び長手方向軸線を有する細長い電気導体と、 この電気導体を取り囲み、前記長手方向軸線に沿って前
記電気導体と面接触している絶縁鞘と、を有し、 この絶縁鞘は、前記電気導体の側面の露出を制限するス
リットを有し、この露出は、身体組織に供給される電流
密度を設定するために、前記スリットのサイズを変える
ための調節手段によって制限される、電気外科用電極。 - 【請求項2】前記絶縁鞘は、前記電気導体の側面と接触
した絶縁材料の被覆を有している、請求項1に記載の電
気外科用電極。 - 【請求項3】前記絶縁材料は、身体組織と前記電気外科
用電極との付着を最小にするためにくっつかない表面を
前記被覆に与える特性を有している、請求項2に記載の
電気外科用電極。 - 【請求項4】前記電気導体はブレードを含み、このブレ
ードは、少なくとも2つの大きい表面と、これらの2つ
の大きい表面との間に且つ前記ブレードの周りに延びる
側面とを有し、前記絶縁手段は、前記ブレードの大きい

シーバーを有し、前記電気外科用電極は、前記レシーバ
ーによって受信されるようになっている高周波電気信号
で活性可能であり、外科機能を行うためにスパーキング
する、請求項1乃至8に記載の電気外科用電極。 - 【請求項10】前記調節手段は、窓を備えた絶縁スリー
ブを有し、この窓は、前記スリットとの重なりが大きく
なったり小さくなったりするように移動される、請求項
1に記載の電気外科用電極。 - 【請求項11】前記支持通路は近位端と遠位端との間に
延び、前記電気導体は、この電気導体の長手方向軸線が
前記支持通路を横切るように前記支持通路から曲げら
れ、前記支持通路は、前記スリットを構成している前記
絶縁鞘の部分が前記支持通路の近位端に面する関係をな
