Abstract
Description
流入端部および流出端部を備える環状フレームであって、前記環状フレームが径方向収縮構成と径方向拡張構成との間で径方向に収縮可能かつ拡張可能であり、前記流入端部から前記流出端部に延在する軸方向を画定する環状フレームと、
前記環状フレーム内に位置決めされ、前記環状フレームに固定される弁尖構造体と、
前記環状フレームの外方表面の周囲に位置決めされる環状外方スカートと、
を備え、前記環状外方スカートが、
第1の位置にて前記環状フレームに固定される流入エッジ、
第2の位置で前記環状フレームに固定される流出エッジ、
前記人工弁が前記径方向拡張構成にある場合に、前記環状外方スカートの前記流入エッジおよび前記流出エッジから径方向に外方に座屈する弛みを有する前記流入エッジと前記流出エッジとの間の中間部分、ならびに
(a)前記弛みの径方向外方座屈を強化するために周方向に比べて前記人工弁の軸方向においてより高い剛直性の布帛、および/または
(b)前記環状外方スカートの前記弛みの前記径方向外方座屈を強化するように設定された形状記憶を有する形状記憶材料から作製された繊維を含む自己拡張可能布帛
を備える、植込み可能な人工弁。
(2)前記環状外方スカートの前記流出エッジは、複数の交互する突出部および切欠部を備え、前記突出部は、前記第2の位置にて前記環状フレームに固定され、前記切欠部は、前記環状フレームに直接的には固定されない、(1)に記載の植込み可能な人工弁。
(3)前記環状フレームが前記径方向収縮構成にある場合に、前記環状外方スカートの前記流入エッジと前記環状外方スカートの前記流出エッジとの間の軸方向距離が、前記人工弁が前記径方向拡張構成にある場合よりも大きく、それにより前記環状外方スカートの前記中間部分の前記弛みが軽減される、(1)または(2)に記載の植込み可能な人工弁。
(4)前記布帛は、前記環状フレームが前記径方向収縮構成から前記径方向拡張構成へと拡張された後に、残留ひずみを有していない繊維を含む、(1)から(3)のいずれか一項に記載の植込み可能な人工弁。
(5)前記環状外方スカートは、周方向に比べて前記人工弁の軸方向により高い剛直性の布帛を備え、前記布帛は、
前記環状フレームの前記軸方向に対して平行な複数の第1の繊維と、
前記複数の第1の繊維に対して垂直な複数の第2の繊維と
を含み、
前記複数の第1の繊維中の繊維の少なくともいくつかが、前記複数の第2の繊維中の繊維よりも高い剛直性である、(1)から(4)のいずれか一項に記載の植込み可能な人工弁。
(6)前記複数の第1の繊維は、モノフィラメント繊維を含む、(5に記載の植込み可能な人工弁。
(7)前記複数の第2の繊維は、マイクロフィラメント繊維、マルチフィラメント繊維、またはマイクロフィラメント繊維およびマルチフィラメント繊維の組合せを含む、(5)または(6)に記載の植込み可能な人工弁。
(8)前記複数の第2の繊維は、前記環状フレームが前記径方向収縮構成から前記径方向拡張構成に拡張された後に、残留ひずみを有していない繊維を含む、(5)から(7)のいずれか一項に記載の植込み可能な人工弁。
(9)前記自己拡張可能布帛は、経糸繊維および緯糸繊維の織成体を備える、(1)から(4)のいずれか一項に記載の植込み可能な人工弁。
(10)前記緯糸繊維は、形状記憶材料から作製された繊維を含む、(9)に記載の植込み可能な人工弁。
(11)前記経糸繊維および前記緯糸繊維の前記織成体は、
非形状記憶材料から作製された繊維を含む平織パターンと、
前記形状記憶材料から作製された前記繊維を含むサテン織パターンと
を含む、(9)または(10)に記載の植込み可能な人工弁。
(12)前記サテン織パターンは、非形状記憶材料から作製された経糸繊維と、前記形状記憶材料から作製された緯糸繊維とを含む、(11)に記載の植込み可能な人工弁。
(13)前記形状記憶材料は、ニッケルチタン合金である、(1)または(9)から(12)のいずれか一項に記載の植込み可能な人工弁。
(14)形状記憶材料から作製された前記繊維は、0.5~15ミルの直径を有する、(1)または(9)から(13)のいずれか一項に記載の植込み可能な人工弁。
(15)前記環状フレームは、複数の弁尖装着部分を備え、前記弁尖構造体は、前記環状フレームの前記弁尖装着部分に固定される、(1)から(14)のいずれか一項に記載の植込み可能な人工弁。
(16)患者の体内に人工心臓弁を植え込むためのアセンブリであって、
細長シャフトを備える送達装置と、
前記体内に送達するための径方向収縮構成にて前記細長シャフト上に取り付けられる(1)から(15)のいずれか一項に記載の前記人工心臓弁と、
を備える、アセンブリ。
(17)人工弁を径方向に圧縮する方法であって、
径方向拡張構成にある際に、クリンプ化顎部を備えるクリンプ化デバイスに前記人工弁を部分的に挿入するステップであって、外方スカートを備える前記人工弁の部分がクリンパ顎部の外部に延在する、ステップと、
第1の部分収縮構成へと前記人工弁をクリンプ状にするステップと、
前記外方スカートが前記クリンパ顎部同士の間に位置するように、前記クリンプ化デバイスに前記人工弁をさらに挿入するステップと、
第2の部分収縮構成へと前記人工弁をクリンプ状にするステップと、
完全クリンプ状構成へと前記人工弁を任意選択でクリンプ状にするステップと、
を含む、方法。
(18)前記完全クリンプ状構成へと前記人工弁をクリンプ状にするステップを含む、(17)に記載の方法。
(19)(a)前記第1の部分収縮構成にある前記人工弁は、前記径方向拡張構成にある前記人工弁の直径の約60%である直径を有する、
(b)前記第2の部分収縮構成にある前記人工弁は、前記径方向拡張構成にある前記人工弁の直径の約40%である直径を有する、
(c)完全収縮構成にある前記人工弁は、前記径方向拡張構成にある前記人工弁の直径の約10%である直径を有する、または
(d)(a)から(c)の2つ以上の組合せである、(17)または(18)に記載の方法。
(20)前記クリンプ化デバイスは、自動クリンプ化デバイスである、(17)から(19)のいずれか一項に記載の方法。

Claims (20)
- 植込み可能な人工弁であって、
流入端部および流出端部を備える環状フレームであって、前記環状フレームが径方向収縮構成と径方向拡張構成との間で径方向に収縮可能かつ拡張可能であり、前記流入端部から前記流出端部に延在する軸方向を画定する環状フレームと、
前記環状フレーム内に位置決めされ、前記環状フレームに固定される弁尖構造体と、
前記環状フレームの外方表面の周囲に位置決めされる環状外方スカートと、
を備え、前記環状外方スカートが、
第1の位置にて前記環状フレームに固定される流入エッジ、
第2の位置で前記環状フレームに固定される流出エッジ、
前記人工弁が前記径方向拡張構成にある場合に、前記環状外方スカートの前記流入エッジおよび前記流出エッジから径方向に外方に座屈する弛みを有する前記流入エッジと前記流出エッジとの間の中間部分、ならびに
(a)前記弛みの径方向外方座屈を強化するために周方向に比べて前記人工弁の軸方向においてより高い剛直性の布帛、および/または
(b)前記環状外方スカートの前記弛みの前記径方向外方座屈を強化するように設定された形状記憶を有する形状記憶材料から作製された繊維を含む自己拡張可能布帛を備える、植込み可能な人工弁。 - 前記環状外方スカートの前記流出エッジは、複数の交互する突出部および切欠部を備え、前記突出部は、前記第2の位置にて前記環状フレームに固定され、前記切欠部は、前記環状フレームに直接的には固定されない、請求項1に記載の植込み可能な人工弁。
- 前記環状フレームが前記径方向収縮構成にある場合に、前記環状外方スカートの前記流入エッジと前記環状外方スカートの前記流出エッジとの間の軸方向距離が、前記人工弁が前記径方向拡張構成にある場合よりも大きく、それにより前記環状外方スカートの前記中間部分の前記弛みが軽減される、請求項1または2に記載の植込み可能な人工弁。
- 前記布帛は、前記環状フレームが前記径方向収縮構成から前記径方向拡張構成へと拡張された後に、残留ひずみを有していない繊維を含む、請求項1から3のいずれか一項に記載の植込み可能な人工弁。
- 前記環状外方スカートは、周方向に比べて前記人工弁の軸方向により高い剛直性の布帛を備え、前記布帛は、
前記環状フレームの前記軸方向に対して平行な複数の第1の繊維と、
前記複数の第1の繊維に対して垂直な複数の第2の繊維と
を含み、
前記複数の第1の繊維中の繊維の少なくともいくつかが、前記複数の第2の繊維中の繊維よりも高い剛直性である、請求項1から4のいずれか一項に記載の植込み可能な人工弁。 - 前記複数の第1の繊維は、モノフィラメント繊維を含む、請求項5に記載の植込み可能な人工弁。
- 前記複数の第2の繊維は、マイクロフィラメント繊維、マルチフィラメント繊維、またはマイクロフィラメント繊維およびマルチフィラメント繊維の組合せを含む、請求項5または6に記載の植込み可能な人工弁。
- 前記複数の第2の繊維は、前記環状フレームが前記径方向収縮構成から前記径方向拡張構成に拡張された後に、残留ひずみを有していない繊維を含む、請求項5から7のいずれか一項に記載の植込み可能な人工弁。
- 前記自己拡張可能布帛は、経糸繊維および緯糸繊維の織成体を備える、請求項1から4のいずれか一項に記載の植込み可能な人工弁。
- 前記緯糸繊維は、形状記憶材料から作製された繊維を含む、請求項9に記載の植込み可能な人工弁。
- 前記経糸繊維および前記緯糸繊維の前記織成体は、
非形状記憶材料から作製された繊維を含む平織パターンと、
前記形状記憶材料から作製された前記繊維を含むサテン織パターンと
を含む、請求項9または10に記載の植込み可能な人工弁。 - 前記サテン織パターンは、非形状記憶材料から作製された経糸繊維と、前記形状記憶材料から作製された緯糸繊維とを含む、請求項11に記載の植込み可能な人工弁。
- 前記形状記憶材料は、ニッケルチタン合金である、請求項1または請求項9から12のいずれか一項に記載の植込み可能な人工弁。
- 形状記憶材料から作製された前記繊維は、0.5~15ミルの直径を有する、請求項1または請求項9から13のいずれか一項に記載の植込み可能な人工弁。

前記外方スカートが前記クリンパ顎部同士の間に位置するように、前記クリンプ化デバイスに前記人工弁をさらに挿入するステップと、
第2の部分収縮構成へと前記人工弁をクリンプ状にするステップと、
完全クリンプ状構成へと前記人工弁を任意選択でクリンプ状にするステップと、
を含む、方法。 - 前記完全クリンプ状構成へと前記人工弁をクリンプ状にするステップを含む、請求項17に記載の方法。
