本願は、2018年6月2日に出願された米国仮出願第62/679,810号、2019年1月30日に出願された同第62/798,809号および2019年2月22日に出願された同第62/809,354号に対する利益および優先権を主張し、各号の内容は、その全体を参照によって本明細書に援用するものとする。
本発明は、展開可能な磁気加圧装置に関し、より詳細には、組織間、臓器間などでの吻合部形成の精度を向上させるように、磁気加圧装置を所望の部位に送給し、展開し、位置決めするためのシステム、装置および方法に関する。
消化器(gastroenterological;GI)系、心血管系または泌尿器系のバイパスは、典型的に、組織内の2つの位置に穴を開け、縫合糸またはステープルで穴を接合することによって形成される。バイパスは、典型的に、疾患または機能不全組織をバイパスしながら、系のより健康的な部分間で流体(例えば、血液、栄養素)を経由させるために留置される。処置は、典型的に侵襲的であり、出血、感染、痛みおよび麻酔に対する有害反応などのリスクに患者をさらす。加えて、縫合糸またはステープルによって形成されたバイパスが、術後の漏れおよび癒着によって病状を悪化させる可能性がある。漏れは、感染または敗血症をもたらす可能性があり、癒着は、腸絞扼や腸閉塞などの合併症をもたらす可能性がある。従来のバイパス処置は、内視鏡、腹腔鏡またはロボットによって完了することができるが、組織に開けられた穴を接合するのに時間を要する場合がある。さらに、そのような処置は、多くの外科施設では利用できない特殊な専門知識および機器を必要とする。
本発明は、体内、例えば胃腸管内で、吻合部を低侵襲的に形成するための改良された装置および技術を提供する。そのような装置および技術は、肥満や糖尿病などの慢性疾患のためのより速く、より安価な治療を容易にする。そのような技術はまた、胃癌や結腸癌のような癌などの疾患の緩和治療に伴う時間および痛みを低減させる。
本発明は、体内、例えば胃腸管内で吻合部を低侵襲的に形成するための改良された装置および技術を提供する。そのような装置および技術は、肥満や糖尿病などの慢性疾患のためのより速く、より安価な治療を容易にする。そのような技術はまた、胃癌や結腸癌のような癌などの疾患の緩和治療に伴う時間および痛みを低減させる。

Claims (14)
- 患者内の所望の標的部位への1つ以上の磁気加圧装置の留置を支援するための送給装置であって、
内腔を含む細長い本体であって、前記内腔が、前記細長い本体の全体を通して延び、1つ以上の磁気加圧装置を収容して通過させるように形成および/または寸法設定されている、細長い本体と、
中空の前記細長い本体から延びる遠位部分であって、中空の前記細長い本体から延びる一対のアームを含み、各アームが、それぞれの遠位端部で終端しており、それぞれの前記遠位端部が、組織を貫通するように構成された実質的に1つの先端部を形成するように互いに集合し、前記一対のアームが協働して、前記細長い本体の前記内腔と流体連通しているスロットを形成している、遠位部分と
を備える送給装置。 - 前記細長い本体がカテーテルである、請求項1記載の送給装置。
- 前記細長い本体が、前記標的部位に向かって前進する間の実質的な変形を阻止するのに十分な程度の剛性を有している、請求項1記載の送給装置。
- 前記スロットが、中空の前記本体の全体を通して延びている、請求項1記載の送給装置。
- 前記一対のアームが、標準状態と曲げ状態との間で変形可能である、請求項1記載の送給装置。
- 前記内腔から前記スロットを介して標的部位に送給される1つ以上の磁気アセンブリに解離可能に結合可能な留置部材をさらに備える、請求項1記載の送給装置。
- 前記留置部材がワイヤを含む、請求項6記載の送給装置。
- 前記ワイヤが形状記憶合金を含む、請求項7記載の送給装置。
- 前記送給装置が、内視鏡によって前記患者内に導入されるように形成および/または寸法設定されている、請求項1記載の送給装置。
- 前記送給装置が、腹腔鏡によって前記患者内に導入されるように形成および/または寸法設定されている、請求項1記載の送給装置。
- 前記遠位部分が操縦可能である、請求項1記載の送給装置。
- 前記1つ以上の磁気加圧装置がそれぞれ、可撓性の外骨格要素を介して結合された一対の磁気セグメントを含む、請求項1記載の送給装置。
- 前記一対の磁気セグメントが、互いに直線状に並んで配列され、外骨格の中央部分を介して離間して配置されている、請求項12記載の送給装置。
- 前記一対の磁気セグメントが前記送給装置の前記遠位部分の前記スロットを通して送給されたときに、前記一対の磁気セグメントの各1つの磁気セグメントは、前記外骨格の前記中央部分が前記スロット内にあるままで、前記送給装置の前記細長い本体の向かい合うそれぞれの側で前記スロットから進出する、請求項13記載の送給装置。
