シクロベンザプリン、または3−(5H−ジベンゾ[a,d]シクロヘプタン−5−イリデン)−N,N−ジメチル−1 プロパンアミンは、局所由来の急性筋痙攣の処置について、1977年に米国食品医薬品局によって初めて承認された(Katz and Dube, 1988)。強力な5−HT2Aおよびα1Aアンタゴニストは、睡眠時間の間に、セロトニン作動性2A(5−HT2A)レセプターおよびαアドレナリン作動性1A(α1A)レセプターの拮抗を通じて神経精神障害および線維筋痛症において回復睡眠を改善することが、その後の研究において示された(Moldofsky et al., 2011, Moldofsky et al., 2015)。
TNX−102 SLは、5−HT2Aレセプターおよびα1Aレセプターの拮抗を含む治療上重要な生物学的活性をナノモル濃度範囲ですら保持し(WO2013188847を参照のこと)、低用量で高い安全性および耐容性プロフィールを有するシクロベンザプリンを含む。本明細書で記載されるシクロベンザプリン舌下(SL)製剤は、嚥下困難が共通する高齢集団においてさらなる利点を付与する(Sura et al 2012)。
一局面において、本出願は、認知症または神経変性状態における興奮、精神病、および/または認知機能低下ならびにその関連症状を処置または防止するための方法を開示する。上記症状は、認知症および/または神経変性状態と関連する睡眠疾患または非睡眠疾患であり得る。上記方法は、認知症もしくは神経変性状態に罹患しているか、または認知症もしくは神経変性状態において興奮、精神病、および/もしくは認知機能低下ならびにその関連症状を発生させるリスクにある被験体に、治療上有効な量のシクロベンザプリンおよび薬学的に受容可能なキャリアを含む薬学的組成物を投与する工程を包含する。いくつかの実施形態において、上記組成物は、0.1mg〜30mg シクロベンザプリン/日の間の用量において、または0.1mg〜20mg シクロベンザプリン/日の間の用量において投与され得る。いくつかの実施形態において、上記組成物は、10mg シクロベンザプリン/日未満または5mg シクロベンザプリン/日未満の用量で投与され得る。好ましい実施形態において、上記組成物は、約5.6mg シクロベンザプリン/日の用量で投与され得る。いくつかの実施形態において、上記組成物は、2.8mg シクロベンザプリン/日の用量で投与され得る。上記組成物は、毎日または1日に1回投与され得る。いくつかの実施形態において、上記組成物は、2投与単位(各々2.8mg シクロベンザプリン)として同時に投与される。いくつかの実施形態において、上記組成物は、2投与単位として同時に投与され、ここで上記2投与単位でのシクロベンザプリンの合計量は、約5.6mgである。

本出願の開示の1つの舌下性剤(TNX−102 SL)は、シクロベンザプリン塩酸塩(活性成分)およびD−マンニトールの共晶錯体(eutectic complex)を含む。それはまた、カリウム塩(二塩基性)を含む。表1は、TNX−102 SL錠剤の組成を示す。
ヒト被験体における夜間投与に関してTNX−102 SL製剤の安全性を決定するために、TNX−102 SLを使用する合計10の臨床試験を現在までに完了している: 健常ボランティアにおける5件のフェーズ1試験(データは示さず)、FMおよびPTSDを有する患者における2件のフェーズ2試験;FMを有する患者における1件のフェーズ3試験;ならびにFMおよびPTSDを有する患者における2件のオープンラベルの継続投与試験(open−label extension studies)。FMおよびPTSDを有する患者における5件の完了した臨床試験の中で、合計641名の患者が、1日に1回就寝時に投与される少なくとも1用量のTNX−102 SLを受けた(「組み合わされたTNX102 SL安全性集団」): 197名のPTSD患者および444名のFM患者。そのPTSD患者の中で、50名は5.6mg(2錠の舌下錠)を受け、その残りは2.8mg(1錠の舌下錠)のTNX−102 SLを受けた。表2を参照のこと。
組み合わされたTNX−102 SL安全性集団
全体として、TNX−102 SLは、FM患者およびPTSD患者の両方において十分な耐容性を示した。TNX−102 SL投与と関連したと考えられる最も共通する有害事象(≧ 5% 発生率)は、口腔感覚鈍麻および口腔知覚異常、ならびに傾眠および疲労のような全身的な影響であった。報告された全ての全身的な影響は、シクロベンザプリンの副作用プロフィールと一致したが、市販の経口摂取される製剤(例えば、即時放出FLEXERIL(登録商標)および持続放出AMRIX(登録商標))において報告されたものより概して頻度は低かった。新たな薬物間相互作用は現在まで同定されていない。
少なくとも1用量のTNX−102 SLを受けた患者は、21歳齢〜75歳齢の範囲に及んだ。表2にまとめられるように、少なくとも1用量のTNX−102 SLを受けた641名の患者のうち、241名は、≧50歳齢であり、400名は、<50歳齢であった;大部分は、その2.8mg用量を受け、わずか3名の≧50歳齢の患者が、その5.6mg用量を受けた(年齢範囲: 54〜59歳)。処置に関連した年齢群特異的安全性のサインは同定されなかった。
全体として、抗コリン作動性作用に一部起因し得る、TNX−102 SLと関連するとして組み合わせたTNX−102 SL安全性集団において同定された有害事象(組み合わせたTNX102 SL安全性集団、または年齢群部分セットのいずれかにおいて>2名の患者および>プラシーボ群で起こった)は、傾眠(5.8%)/鎮静(2.2%)、疲労(5.0%)、および便秘(2.2%)である(注: 傾眠および鎮静は、類似の有害事象を表すように見えた)。
特定の憂慮される他の有害事象(失神、嚥下障害、挫傷、転倒、攻撃性/攻撃的行動(報告されない)、興奮(報告されない)、低血圧症(報告されない)、血圧低下(報告されない)または任意の心臓もしくは肝臓事象が挙げられる)は、いずれの年齢群にもTNX−102 SLに関連しないようであった。
TNX−102 SLの2種の異なる用量(すなわち、2.8mgおよび5.6mg)を評価した唯一の臨床上の安全性および有効性の研究は、従軍関連PTSDを有する患者において行ったAtEase Study/P201である(表2を参照のこと)。そのAtEase Studyでは、安全性データは、少なくとも1用量のTNX−102 SL 5.6mgを受けた50名のPTSD患者に関して入手可能である。その5.6mg用量は、プラシーボ群におけるより高い完遂率によって示されるように、十分な耐容性を(それぞれ、83.6% 対 72.8%)、およびプラシーボ群より低い有害事象に起因する試験中止の発生率(それぞれ、0.0% 対 3.2%)を示した。その5.6mg用量の安全性プロフィールは、その2.8mg用量のものに匹敵し、以下の有害事象は、数値の差異に基づいて考えられる用量関係性を示す: 傾眠/鎮静、頭痛、およびおそらく舌痛症。重要なことには、いずれの用量においても、TNX−102 SL処置と関連する自殺念慮または行動に関して増大したリスクの証拠はなかった。
経口シクロベンザプリン市販製品の安全性プロフィールおよび市販のアルツハイマー病薬物療法との薬物相互作用についての潜在的懸念を評価するために、公的なデータベースおよび文献(PubMed、NIH Drug Reaction Navigator、Medscape Drug Interaction Checker、Drugs.comおよび米国食品医薬品局(FDA)のAdverse Event Reporting System(FAERS)が挙げられる)を使用して、徹底した検索を行った。2タイプの経口シクロベンザプリン製剤が、市販されている− シクロベンザプリンIR(5mgまたは10mg TID; すなわち、FLEXERIL)およびシクロベンザプリンER(15mg BIDもしくはQDまたは30mg QD; すなわち、AMRIX)。両方の製剤は、認知症における興奮の処置のために提唱される、1日に1回就寝時に舌下投与されるTNX−102 SL 5.6mgより代表的には少なくとも2〜3倍高い用量で投与される。
精神運動機能に対するシクロベンザプリンIR 5mgの効果を、シクロベンザプリンIR 5mg TID、ジフェンヒドラミン50mg TID、およびプラシーボのクロスオーバー試験において、高齢の健常ボランティア(62〜80歳齢、N=17)で調査した(Lines et al., 1997)。各処置を、4日間にわたって10用量として投与した。この患者集団において、眠気または認知試験成績の低下の証拠はなかった。注目すべきは、以前の試験における若年集団と比較した、この高齢集団においてより少ない鎮静および認知機能障害が認められた。この所見と一致して、製造業者(Merck; FLEXERIL(登録商標) OTC Switch NDA 21079 FDA Safety Review: 1999年7月20日)によって行われた精神運動機能試験の結果はまた、高齢患者(≧ 65歳齢、N=32)における運転関連技術の評価を含め、コンピューター化された試験バッテリー(computerized test batteries)によって測定される場合の精神運動機能の機能障害の一致したパターンはなく、その成績は、若年集団(21〜49歳齢)に類似であることを示した。
高齢者には、潜在的に不適切な薬物使用の勧告が存在する(例えば、the Beers Criteria; American Geriatric Society, 2015)。筋弛緩薬である抗コリン作動性活性を有する薬物は、認知機能に対する影響および神経系効果に二次的な有害事象(例えば、転倒)に主に起因して、潜在的に不適切であるとしてリストに挙げられる。シクロベンザプリン(IR 5〜10mg TIDおよびER 15mg BID/QD〜30mg QD)は、その抗コリン作動性活性、ならびにその長い半減期に主に起因して、抗コリン作動薬負荷スケールにおいて3までのスケールに対して2というスコアが与えられる(Rudolph et al., 2008)。これらの勧告が、介護者を伴う標的集団を研究する本出願において提唱されたTNX−102 SL用量より高い用量に基づくことは、注意されるべきである。その最も共通して報告される、シクロベンザプリンの投与と関連する抗コリン作動性有害事象は、用量関連性のドライマウスである;これは、TNX−102 SLの潜在的な臨床上の利益を考慮すると、耐容性であると考えられ、臨床上の重要性は最小限である。シクロベンザプリンIR 10mgを受けた患者における他の潜在的抗コリン作動性の影響は全て、低発生率で報告された: 頻脈(<1%)、尿閉(<1%)、錯乱(1%〜3%)および目のかすみ(1%〜3%)。データは、シクロベンザプリン5mg IRについては入手可能でない。全体として、高い抗コリン作動薬負荷スコアと関連する薬物における有害な中枢神経系の影響ならびに不十分な認知および機能上の転帰は、概してシクロベンザプリンで観察されなかった(すなわち、発生率 <1%〜3%)。
認知症または神経変性状態における興奮、精神病および/または認知機能低下ならびにその関連症状の処置に関して、大部分の患者は、おそらく、認知症に関する現行の4種の処方薬(ドネペジル、リバスチグミン、ガランタミン、およびメマンチン)のうちの1種を摂取している。これら薬物のうちの3種(ドネペジル、リバスチグミン、およびガランタミン)は、作用機構として、抗コリンエステラーゼ活性を有する。よって、それらの製品表示は、反対の活性に起因して、抗コリン作動薬の共投与を回避することを推奨する。シクロベンザプリンは、PubMed、NIH Drug Reaction Navigator、Medscape Drug Interaction Checker、およびDrugs.comにおいてシクロベンザプリンとこれら4種の薬物との間の薬物間相互作用の検索に具体的に言及しておらず、相互作用のいかなる報告をも特定しなかった。
薬物動態データは、シクロベンザプリンの血漿濃度が。若年被験体と比較して高齢者において増大される(IR製剤およびER製剤のそれぞれに関して1.7倍または1.4倍まで高い)ことを示した。従って、シクロベンザプリンIRに関しては、ゆっくりと上方に滴定するという選択肢を持ちつつ、5mg用量での処置を開始することが推奨される。また、シクロベンザプリンIRに関する製品表示において、「高齢者が幻覚および錯乱のような中枢神経の有害事象、転倒または他の後遺症、薬物間相互作用および薬物−疾患相互作用を生じる心臓事象のより多くのリスクにあり得るとすると、明らかに必要とされる場合にのみ、シクロベンザプリンIRが使用されることが推奨される」ことは注記される(FLEXERILパッケージ挿入物, 2013)。しかし、持続放出シクロベンザプリン(AMRIX 30mgまたは15mgカプセル剤)の投与は、高齢者において推奨されない。なぜならその投与の融通性は、その1日に1回の投与レジメンに起因してより制限されるからである(AMRIXパッケージ挿入物, 2016)。
現在まで、TNX−102 SLおよび他の市販のシクロベンザプリン製品(特に、高齢者にとって代表的な低用量でのもの(例えば、認知症における興奮のためのTNX−102 SLの好ましい用量より2倍より高いシクロベンザプリン5mg IR TID)の安全性プロフィールは、都合が良く、全身性の抗コリン作動性影響の非常に低い発生率とともに、十分に耐容性を示した。上記でまとめられるように、30mgまでの1日用量でのシクロベンザプリンの安全性プロフィールは、大部分は軽度から中程度であり、かつ十分に耐容性を示した。さらに、迅速な経粘膜吸収、初回通過肝臓代謝の迂回、および長い半減期の活性代謝産物であるノルシクロベンザプリンの低減した生成を可能にするTNX−102 SLの特有の製剤は、1日に1回就寝時に投与した場合に、日中への影響を最小限にする。
プラシーボを対照としたアルツハイマー病における興奮(AAD)の処置に関するTNX−102−SLの固定用量の無作為化二重盲検試験を、8週間にわたって行う。TNX−102−SL 5.6mg(2×2.8mg錠剤)の効果を、おそらくアルツハイマー病と診断され、国際老年精神医学会の興奮定義作業グループ(Cummings et al, 2015)によって定義されるとおりの臨床上重大な、中程度または重度の興奮を経験する50〜90歳齢の範囲に及ぶ被験体において試験する。その被験体は、スクリーニング時およびベースライン時に4以上の重篤度に関する臨床上の全般的印象(CGIS)スコア(中程度に病気)を有する。その被験体は、その用量が、その試験において無作為化する前に、少なくとも3ヶ月間にわたって安定であることを条件として、アルツハイマー病の処置のための薬物療法(例えば、ドネペジル、リバスチグミン、ガランタミン、メマンチン)を使用している最中であってもよい。TNX−102−SL(2×2.8mg錠剤を各日に就寝時に舌下で摂取する)を、8週間の評価期間の間にプラシーボと比較する。
主要有効性エンドポイントは、処置の8週間後のコーエン・マンスフィールド興奮評価票(CMAI)におけるベースラインからの平均変化(ベースライン時および各通院時に測定)である。その重要な副次有効性エンドポイントは、以下を含む:(1)8週間の処置後の改訂版アルツハイマー病共同研究の変化に関する臨床上の全般的印象 興奮ドメインスコア(mADCS−CGIC−興奮)、(2)4週間の処置後のCMAI全スコアにおけるベースラインからの平均変化、(3)2週間の処置後のCMAI全スコアにおけるベースラインからの平均変化、(4)8週間の処置後のCMAI身体的/攻撃的サブスケールスコアにおけるベースラインからの平均変化、(5)8週間の処置後のCMAI身体的/非攻撃的サブスケールスコアにおけるベースラインからの平均変化、(6)8週間の処置後のCMAI言語的/攻撃的サブスケールスコアにおけるベースラインからの平均変化、(7)8週間の処置後のアルツハイマー病評価スケール−認知(ADAS−Cog)スコアにおけるベースラインからの平均変化。逐次検定手順を、上記の副次エンドポイントに適用して、多重性について調節し得、全体的なタイプIエラーを制御し得る。

3週間および8週間の毎日の処置後のシクロベンザプリンおよびノルシクロベンザプリンの血中レベルを、決定する。
処置応答の潜在的な遺伝的決定因子を、処置転帰に関するいくつかの遺伝子の機能的改変体を研究することによって試験する。アルツハイマー病における興奮および他の神経行動障害は、モノアミン作動性神経伝達の調節不全と関連していることが長年知られてきた。モノアミン作動性経路に関与するいくつかの遺伝子の遺伝的改変体は、セロトニンおよびドパミンを含め、認知症における興奮と関連している(Pritchard et al., 2007; Proitsi et al., 2012)。5−HT2Aレセプターおよび5−HT2Cレセプターの遺伝的改変体は、アルツハイマー病における興奮のシタロプラム処置に対する治療応答と関連している(Peters, et al., 2016)。本試験において、本発明者らは、処置転帰に関するいくつかの遺伝子の機能的改変体を研究することによって。処置応答の潜在的な遺伝的決定因子を試験する。その遺伝的改変体としては、以下が挙げられるが、これらに限定されない:5−HT2Aセロトニンレセプター(HTR2A−T102C)、5−HT2Cセロトニンレセプター(HTR2C−Cys23Ser)、セロトニントランスポーター(5HTT−LPR)、脳由来神経栄養因子(BDNF−Val66−Met)、およびアポリポタンパク質E(ε2、ε3、ε4改変体)。
少なくとも1用量の試験薬を受ける全ての被験体を、安全性評価において分析する。少なくともベースラインおよび1回のベースライン後のCMAIを評価したときの全ての無作為化した被験体を、改訂版の治療意図に基づく分析(mITT)の評価において評価および分析する。少なくとも1用量のTBX−102 SLを受けかつ評価可能な薬物動態(PK)血液サンプルが得られる全ての無作為化した被験体を、PK評価において分析する。
主要有効性エンドポイントは、複合CMAIスコアにおいてベースラインから8週間目までの変化である。その主要有効性分析を、混合モデル反復測定(MMRM)アプローチを使用して行い、TNX−102 SLで処置される患者と同時のプラシーボで処置される患者との間で比較を行う。そのモデルは、mITT集団の中の全ての患者を含み、その従属変数は、各無作為化後通院時に全CMAIスコアの観察されるベースラインからの変化である。そのモデルにおける共変量は、処置、部位、場所のタイプ(介護施設か地域社会か)、通院、および処置×通院(treatment−by−visit)相互作用の固定されたカテゴリー効果、ならびにベースラインCMAIスコアおよびベースラインスコア×通院相互作用の連続固定共変量を含む。
有害事象を、医薬品規制用語集(Medical Dictionary for Regulatory Activities)(MedDRA)の最新バージョンを使用してコード化し、全体的に、ならびに好ましい用語および器官別大分類によってまとめる。有害事象をまた、重篤度および試験薬に対する関連性によってまとめる。重篤なAEおよび試験薬の中止をもたらすAEも、まとめる。臨床検査結果に関する実際の値およびベースラインからの変化、バイタルサイン測定、ADAS−Cog、DRS−R−98、およびMMSEスコアを、記述統計(n、平均、SD、メジアン、最小、および最大)を使用して、エンドポイントでまとめる。口腔の検査を、舌下投与の安全性を評価するために行い、妊娠検査を、妊娠する可能性のある女性に対して行う。
1:1無作為化において合計およそ160名の被験体(すなわち、TNX−102 SL 5.6mgおよびプラシーボ群に関して80:80の被験体)を、その試験に登録する。最終的な検証試験のデザインのための効果サイズの決定を、1アームにつき80名の患者というサンプルサイズによって可能にする。
血液サンプルを、集団薬物動態分析および薬理ゲノム学分析のために、ベースライン時、ならびに3週間目および8週間目に集める。
TNX−102 SL 5.6mg(2×2.8mg錠剤)は安全で、十分に耐容性を示し、8週間の試験期間にわたるAADの処置に関して有効である。
TNX−102 SLの長期の安全性および耐容性を、アルツハイマー病を有する被験体(n=160; 50〜90歳; AADを有する男性および女性)における興奮の処置のために44週間までにわたるオープンラベルの多施設固定用量試験において評価する。試験薬に関連する重大な有害事象なしに二重盲検試験(実施例5を参照のこと)を安全に終えた被験体は、そのオープンラベルの継続投与試験に適格である。おそらくアルツハイマー病の基準を満たし続け、その試験手順を遵守する意志のある信頼できる介護者がいる被験体を、その試験に留める。治験責任医師または医療モニターの見解で、安全性評価を妨げる可能性のある、重大な医学的状態を発生させている患者は、その試験から除外される。ベースライン通院時に、全ての被験体は、オープンラベルのTNX−102 SL 5.6mgを就寝時に受ける。
TNX−102 SL 5.6mg(2×2.8mg錠剤)は安全であり、十分に耐容性を示し、かつ44週間の研究期間にわたってAADの処置に関して有効である。

- 認知症および神経変性状態における精神病およびその関連症状を処置または防止するための方法であって、前記方法は、その必要性があるかまたはそのリスクにある被験体に、治療上有効な量のシクロベンザプリンおよび薬学的に受容可能なキャリアを含む薬学的組成物を投与する工程を包含する方法。
- 前記薬学的組成物は毎日投与される、請求項1〜3のいずれか1項に記載の方法。

- 前記組成物は、2投与単位として同時に投与され、前記2投与単位での合計量は、5.6mgのシクロベンザプリンである、請求項9に記載の方法。
- 前記組成物は、1日に1回投与される、請求項1〜12のいずれか1項に記載の方法。
- 前記方法は、コリンエステラーゼインヒビター、N−メチル−D−アスパラギン酸(NMDA)レセプターアンタゴニスト、抗鬱薬、抗不安剤、抗精神病薬剤、抗痙攣薬または気分安定薬、抗アミロイド薬剤、および抗タウ薬剤からなる群より選択される1種またはこれより多くの薬剤を、逐次的にまたは同時平行して投与する工程をさらに包含する、請求項1〜3のいずれか1項に記載の方法。

- 前記薬学的組成物は、舌下投与、口内投与、経口投与、坐剤投与、静脈内投与、筋肉内投与、皮下投与、吸入による投与、鼻内投与、薄膜による投与、経皮投与、非経口投与、直腸投与、または膣投与のために製剤化される、請求項1〜3のいずれか1項に記載の方法。
- 前記薬学的組成物は、舌下に、口内に、経口的に、坐剤で、静脈内に、筋肉内に、皮下に、吸入により、鼻内に、薄膜で、経皮的に、非経口的に、直腸に、または膣に投与される、請求項1〜3のいずれか1項に記載の方法。
- 前記薬学的組成物は、精神療法介入、行動的介入または環境的介入と組み合わせて投与される、請求項1〜3のいずれか1項に記載の方法。
- 治療上有効な量のシクロベンザプリン、ならびにコリンエステラーゼインヒビター、NMDAレセプターアンタゴニスト、抗鬱薬、抗不安剤、抗精神病薬剤、抗痙攣薬または気分安定薬、抗アミロイド薬剤、および抗タウ薬剤からなる群より選択される1種またはこれより多くの薬剤を含む、薬学的組成物。
- 前記1種またはこれより多くの薬剤は、ドネペジル、リバスチグミン、ガランタミン、タクリン、アマンタジン、メマンチン、シタロプラム、フルオキセチン、パロキセチン、セルトラリン、ロラゼパム、オキサゼパム、プロマジン、アリピプラゾール、ジプラシドン、クエチアピン、オランザピン、リスペリドン、カルバマゼピン、ジバルプロエクスまたはデキストロメトルファンである、請求項26に記載の薬学的組成物。
- 前記組成物は、0.1mg〜30mgの間のシクロベンザプリンを含む、請求項26に記載の薬学的組成物。
- 前記組成物は、0.1mg〜20mgの間のシクロベンザプリンを含む、請求項26に記載の薬学的組成物。

- 認知症および神経変性状態に罹患しているか、または認知症および神経変性状態において、興奮、精神病または認知機能低下、およびその関連症状のうちの1またはこれより多くに関するリスクにある被験体のためのシクロベンザプリンの治療上有効な用量を選択するための方法であって、前記方法は、
(1)前記被験体から遺伝子サンプルを得る工程、
(2)前記サンプルを使用して、前記被験体のCYP3A、CYP1A2、CYP3A4、またはCYP2D6遺伝子型を決定する工程、
(3)その遺伝子型に基づいて、シクロベンザプリンの治療上有効な用量を選択する工程、
を包含する方法。 - 前記CYP3A、CYP1A2、CYP3A4またはCYP2D6遺伝子型は、遺伝子チップまたはPCR技術を使用して決定される、請求項34に記載の方法。
