Abstract
Description
−本明細書で定義されているアクセスカテーテル(300)、

−本明細書で定義されるようなプッシャ(500)、
−本明細書で定義されているローダ(600)、及び
−1本以上のガイドワイヤ
の1つ又はそれ以上とを備えるキットが本明細書に記載されている。
−二分枝ステント(200)を装填した送達カテーテル(110)を、アクセスカテーテル(300)を通して治療部位まで血管内に進めるステップ、
−アクセスカテーテル(300)を引き込むことによって送達カテーテル(110)を徐々に開くステップ、及び
−送達カテーテル(110)を引き込むことによって、スリット(140)を通して二分枝ステント(200)を展開するステップ
を含む方法である。
治療は、動脈瘤又は動脈閉塞のものであり得る。
−近位端(20)及び遠位端(30)、及びアーム(220、222)を収容するように構成されている遠位端(30)の二分枝部(114)を有する細長い第1の管(102)を備える送達カテーテル(110)であって、二分枝部(114)に配置される長手方向のスリット(140)が二分枝ステント(200)を解放可能に通すように構成されている送達カテーテル(110)

−送達カテーテル(110)の各肢部(120、122)が血管二分枝部の分枝部内に位置するように、アクセスカテーテル(300)を引き出すことによって、送達カテーテル(110)を徐々に開くこと、
−二分枝ステント(200)の各アーム(220、222)が血管二分枝部の分枝部内に位置するように、送達カテーテル(110)を引き出すことによって、スリットを通して二分枝ステント(200)を展開すること
を含む方法に関する。
−二分枝ステント(200)を装填した送達カテーテル(110)を、アクセスカテーテル(300)を通して閉塞部位まで血管内に進めること、
−アクセスカテーテル(300)を引き込むことによって送達カテーテル(100)を徐々に開き、送達カテーテル(110)の各肢部(120、122)が血管二分枝部の分枝部内に位置するようにすること、
−送達カテーテル(110)を引き込むことによって、スリット(140)を通して二分枝ステント(200)を展開して、二分枝ステント(200)の各アーム(220、222)が血管二分枝部の分枝部内に位置し、1つ又は双方のアーム(220、222)が閉塞物質(例えば、凝血塊、血栓、塞栓)内に位置するようにすること、及び
−閉塞物質の少なくとも一部と一緒に二分枝ステント(200)を引き出すこと
を含む方法に関する。
−二分枝ステント(200)の近位端(20)をローダ(600)の第3の管腔(630)の遠位端(30)の遠位端に挿入するステップ、
−第3の管(602)がそれらを覆いながらアーム(120、122)を折り畳むように、二分枝ステント(200)を近位方向(20)に引き出すステップ、
−それぞれの管腔(630、130)が途切れない通り道を形成するように、第3の管(602)の遠位端(30)を第1の管(102)送達カテーテル(110)の近位端に連結するステップ、
−二分枝ステント(200)を第1の管(102)の第1の管腔(130)に入るように遠位方向(30)に前進させるステップ
を含む方法に関する。
図1は、第1の管腔(130、132、134、136)を伴う、配置された第1の管(102)を備える、近位端(20)及び遠位端(30)を有する開放構成の送達カテーテル(110)を示す。第1の管(102)は、二分枝点(124)で遠位端(30)にて二分枝し、第1の管腔(132)と二分枝部(114)とを配置した近位主要部分(112)を形成する。そこでは、第1の管腔(134、136)は、二分枝部(114)の各肢部(120、122)内に前進する。二分枝部(114)には、二分枝ステント(200)を通すための長手方向のスリット(140)が配置されている。
図1のパネルB及びB’は、それぞれのスリット(140’、140’’)及び第1の管腔(134、136)のそれぞれの部分と共に、それぞれ第1の肢部(120)及び第2の肢部(122)を通る横断面図を示す。パネルAは、第1の管(102)の主要部分(112)を通る横断面図を示す。
図2は、近位端(20)及び遠位端(30)を有し、二分枝点(224)で遠位端(30)の一対のアーム(220、222)へと二分枝に別れる近位端(20)のステム部(226)を備え、開放(Y)構成の二分枝ステント(200)を示す。ステント管腔(230)は、ステム部(226)の近位端(232)から各アーム(234、236)の遠位端まで延びる。
図3のパネルA〜Cはそれぞれ、第2の管腔(330)を伴って配置された第2の管(302)を備える、近位端(20)及び遠位端(30)を有するアクセスカテーテル(300)を示し、この場合第2の管腔(330)はスライド式に第1の管(102)を収容している。パネルAでは、第1の管(102)の二分枝部(114)が第2の管腔(330)内に完全に引き込まれて、二分枝部(114)に折り畳み構成をとらせる。パネルBでは、第1の管(102)の二分枝部(114)が第2の管腔(330)内に部分的に引き込まれて、二分枝部(114)が開放構成をとるようにする。パネルCでは、第1の管(102)の二分枝部(114)は第2の管腔(330)に被覆が完全になく、二分枝部(114)に完全に開いた構成をとらせることができる。
Claims (14)
- ステム(226)及び一対のアーム(220、222)を有する二分枝ステント(200)用の送達システム(100)であって、近位端(20)及び遠位端(30)、及び前記アーム(220、222)を収容するように構成されている前記遠位端(30)の二分枝部(114)を有する細長い第1の管(102)を備える送達カテーテル(110)であって、前記二分枝部(114)に配置される長手方向のスリット(140)が前記二分枝ステント(200)を解放可能に通すように構成されている送達カテーテル(110)を備える、送達システム(100)。
- 前記長手方向のスリットが、前記二分枝部(114)の第1の肢部(120)から第2の肢部(122)まで延びる、請求項1に記載の送達システム(100)。
- 前記第1の管(102)の前記二分枝部(114)が、二分枝の身体の血管の分岐部(422、424)を通るようにそれぞれ構成された第1(120)及び第2(122)の肢部を備える、請求項1又は2に記載の送達システム。
- 前記第1(120)及び第2(122)の肢部はそれぞれ、前記二分枝ステント(200)を半径方向に圧縮するように構成されている、請求項3に記載の送達システム。
- 近位端(20)と遠位端(30)とを有し、前記第1の管(102)をスライド式で収容するように適合され、前記第1の管(102)と第2の管(302)のスライド式の相対移動に応答して前記第1の管(102)の前記二分枝部(114)を段階的に開く又は折り畳むのを制御するよう構成される第2の管腔(330)を備える細長い第2の管(302)を備えるアクセスカテーテル(300)をさらに備える、請求項1から4のいずれかに記載の送達システム(100)。
- 前記送達カテーテル(110)が、オーバーワイヤ又は迅速交換動作モード用に構成されている、請求項5に記載の送達システム。
- 自動拡張型の前記二分枝ステント(200)をさらに備える、請求項1から6のいずれかに記載の送達システム(100)。
- 前記二分枝ステント(200)が、場合により抗血栓性、抗凝固性、又は内皮化性を有する、又は細胞移動の促進剤、又は細胞増殖の促進剤である溶出可能な活性医薬成分を備えている、請求項7に記載の送達システム(100)。
- 前記二分枝ステント(200)がレーザ切断又は編組によって準備される、請求項7又は8に記載の送達システム(100)。
- 近位端(20)及び遠位端(30)を有する細長い可撓性ロッド(510)、及び半径方向に自動拡張して開放又は折り畳み構成をとる、その近位端(20)での二分枝ステント(200)への解放可能な取り付けのため前記遠位端(30)にある捕捉要素(520)を備えるプッシャ(500)をさらに備え、前記折り畳み構成は前記第1の管(102)の前記第1の管腔(230)内の通り道用に構成され、前記捕捉要素(520)の前記周縁部は、前記二分枝ステント(200)の前記近位端(20)を掴むために互いにより接近しており、前記開放構成は前記二分枝ステント(200)を解放するよう構成されている、請求項1から9のいずれかに記載の送達システム(100)。
- 近位端(20)及び遠位端(30)を有する、前記折り畳み構成で前記二分枝ステント(200)をスライド式で収容するように適合される第3の管腔(630)を備える細長い第3の管(602)を備える、前記二分枝ステント(200)を前記送達カテーテル(110)に装填するローダ(600)をさらに備え、前記第3の管(602)の前記遠位端(30)は、前記送達カテーテル(110)の前記近位末端と連結するように構成され、前記第1の管腔(130)及び第3の管腔(630)は、前記ローダ(600)から前記送達カテーテル(110)まで前記折り畳み状態で前記二分枝ステント(200)を前進させるための途切れない通り道を形成するように接続される、請求項1から10のいずれかに記載の送達システム(100)。
- 請求項1から4のいずれかに定義の前記送達カテーテル(110)と、
−請求項5又は6に定義のアクセスカテーテル(300)、
−請求項7から9に定義の二分枝ステント(200)、
−請求項10に定義のプッシャ(500)、
−請求項11に定義のローダ(600)、及び
−1本又はそれ以上のガイドワイヤ
のうちの1つ又はそれ以上とを備えるキット。 - 請求項1から10のいずれかに定義の前記送達システム(100)を使用して、二分枝ステントを治療部位に送達するための方法であって、
−前記二分枝ステント(200)を装填した前記送達カテーテル(110)を、前記アクセスカテーテル(300)を通して前記治療部位まで血管内に進めるステップ、
−前記アクセスカテーテル(300)を引き込むことによって前記送達カテーテル(110)を徐々に開くステップ、及び
−前記送達カテーテル(110)を引き込むことによって、前記スリット(140)を通して前記二分枝ステント(200)を展開するステップ
を含む方法。 - 動脈瘤又は動脈閉塞の治療のための、請求項13に記載の方法。
