Abstract
Description
本発明は、ポリペプチドまたはその抗原性部分に特異的に結合する抗体およびその抗原結合断片であって、そのポリペプチドが、a)MUC16外部ドメインポリペプチド、b)MUC16細胞質ドメインポリペプチド、およびc)システインループポリペプチドを含有するMUC16細胞外ドメインポリペプチドから選択される、抗体およびその抗原結合断片に関する。本発明の抗体およびそれらを含有する組成物は、癌などのMUC16が過剰発現している疾患についての診断的および治療的適用に有用である。
細胞表面マーカーおよび脱落抗原(shed antigen)は、いくつかの癌の診断に用いられる。例えば、OC125抗体によって認識されるCA125抗原は、卵巣癌に発現する組織特異的な循環抗原である。CA125抗原は、LloydおよびYinによってクローニングされたMUC16遺伝子によってコードされる。その完全長遺伝子は、おもに様々な婦人科系の組織、特に新生物に存在する複合連結型(complex tethered)ムチンタンパク質を表す。OC125および他の関連抗体は、もっぱらその分子の切断部分に存在するグリコシル化依存性抗原と反応する。

MUC16/CA125抗原の発現は、長い間、婦人科系の組織と関連づけられてきた。「CA125」、「CA−125」、「切断されたCA125」、および「切断されたCA−125」は交換可能に、切断部位に対して遠位にある、連結型ムチンMUC16のグリコシル化外部ドメインを指す(Payneら、米国特許第7,202,346号)。この遊離される外部のドメインは、それぞれが潜在的グリコシル化部位を有する、156個のアミノ酸の16〜20個のタンデムリピートを含有する。19個のアミノ酸の見かけのシステインに基づいたジスルフィドループが、全ての反復配列に存在し、N末端は、高度にO−グリコシル化されているヘアブラシ構造を含有する(11)。推定サイズは、タンパク質部分について2.5MDであり、糖質を加えると、これは5MDまで増加し得る(10、26)。
ヒトMUC16の生物学的特徴をより良く探求するために、本発明者らは、推定の切断部位に対して近位にある、MUC16−カルボキシ末端の細胞外部分に対するモノクローナル抗体、ならびに内部の細胞質ドメインに対する1つのモノクローナル抗体を誘導した。先行の抗体とは対照的に、これらは、MUC16のペプチドバックボーンに対して誘導され、複合糖タンパク質エピトープに向けられていない。これらのエピトープは、切断部位に対して近位にあるので、それらは、循環中に見出される可能性が低く、診断方法および治療的介入に新規な標的を提供する。本明細書におけるデータは、MUC16ペプチドバックボーンに対して発生した例示的な抗体の同定および特徴づけを実証している。

本発明の新規な抗体に加えて、本発明はまた、これらの抗体を産生するハイブリドーマ細胞系を提供する。「ハイブリドーマ細胞」とは、組織培養で成長するその能力について、および抗体鎖を合成しないことについて選択される、骨髄腫(B細胞癌)細胞と特定の抗体産生B細胞とを融合することによって産生される細胞系を指す。ハイブリドーマ細胞によって産生される抗体は、全く単一の特異性であり、それゆえ、(ポリクローナル抗体と対照的に)モノクローナル抗体である。
本発明はコンジュゲート抗体を企図する。「コンジュゲート」抗体とは、細胞傷害性物質および/または細胞傷害性物質のプロドラッグに共有結合により連結された本発明の抗体を指す。
本発明は、被験体においてMUC16の過剰発現を含む疾患を検出するための方法であって、a)i)被験体由来の試料、およびii)本発明の抗体のいずれか1つまたは複数を供給する工程、b)抗体のその同種抗原との特異的結合のための条件下で試料を抗体に接触させる工程、ならびにc)疾患を欠く対照試料と比較して抗体の試料への結合のレベルの増加を検出し、それによって被験体において疾患を検出する工程を含む方法を提供する。抗体を用いて疾患を検出するための一般的な方法は当技術分野において知られている(Payneら、米国特許第7,202,346号)。本発明の方法は、卵巣癌および乳癌などの癌を検出するのに特に有用である。
本発明は、MUC16の過剰発現を含む疾患を処置するための方法であって、上記疾患を有する被験体に、本発明の抗体のいずれか1つまたは複数の治療上有効量を投与する工程を含む方法を提供する。抗体を用いて疾患を処置するための一般的な方法は当技術分野において知られている(Payneら、米国特許第7,202,346号)。本発明の方法は、卵巣癌および乳癌などの癌を処置するのに特に有用である。これらの方法はまた、原発癌、転移癌、および再発癌に適用可能である。
材料および方法
以下は、その後の実施例に用いられる例示的な材料および方法の簡単な記載である。
OVCAR3、SKOV3、およびA2780細胞系を、American Type Culture Collection(ATCC、Manassas、VA)から入手し、ATCC文献に従い、培養で維持した。MUC16+形質移入した細胞系の作製のために、MUC16 cDNAのカルボキシ末端部分を、Vitality phrGFPベクター発現系(Stratagene、La Jolla、CA)を用いて、緑色蛍光タンパク質融合タンパク質として導入した。それらのそれぞれの培地中にジェネティシン(G418、Invitrogen、Grand Island、NY)を用いて安定な細胞系を選択し、緑色蛍光タンパク質の発現によって単離した。安定なトランスフェクタントを、それぞれ、それらの培地中G418の存在下で日常的に維持した。ΔMUC16c114トランスフェクタントは、推定の切断部位からカルボキシ末端まで(アミノ酸1776〜1890)のMUC16タンパク質の細胞表面発現を有する(12)。
MUC16配列を用いて、ΔMUC16c114アミノ酸配列のエレメントをコードするペプチド配列をMemorial Sloan−Kettering Cancer Center(MSKCC)微量化学コア施設で合成した。本発明者らは、3つのポリペプチド(図1)を合成し、ポリペプチド1およびポリペプチド2を、KLHへのより良い結合体化のためにN末端においてシステインで修飾した。等濃度のKLH結合体化ペプチドを混合し、その後、5匹のBALB/cマウスへの免疫原として用いた。本発明者らは、ELISAによって、血清が個々のペプチドに対して最も高い反応性を示した、5匹のマウスのうちの1匹を選択し、MSKCCモノクローナル抗体コア施設は、融合を実施し、標準プロトコールを用いて抗体を選択した。融合から10日後、上清を選択し、個々の合成ペプチドに対するELISAにより、反応性についてスクリーニングした。
個々のペプチドおよびGST−ΔMUC16c114融合タンパク質に対する抗体の陽性を調べるために、ELISAアッセイについての通例のコア施設プロトコールに従ってサンドイッチELISAを実施した。
付着標的細胞を、0.05%のトリプシンおよび0.1%のEDTAによって取り出し、洗浄し、血球計算器によってカウントした。細胞を、複数個のEppendorfチューブへ、チューブあたり少なくとも0.5×106〜1×106細胞で分配した。細胞を、1%FCSおよび0.025%アジ化ナトリウムを含有するリン酸緩衝食塩水(PBS)(FACS緩衝液)で洗浄した。内部のFACS染色のために、Eppendorfチューブ内の細胞を、1:10に希釈したFACS透過処理溶液2(BD BioSciences、San Jose、CA)を用いて室温で10分間、透過処理し、その後、氷冷FACS緩衝液で2回、洗浄した。その後、それらを、氷上で30分間、1μg/チューブのマウスMUC16モノクローナルの生物反応性上清を含まずに(二次抗体の対照として)か、またはそれを含んでかのいずれかでインキュベートした。表面FACS染色のために、細胞を、氷上で30分間、1μg/チューブのマウスMUC16モノクローナル(9B11.20.16、9C9.21.5.13、および4H11.2.5)、マウス抗ヒトOC125(M3519)、マウス抗ヒトM11(M3520)(DakoCytomation、Dako North America Inc.、Carpinteria、CA)、またはVK8(MSKCC、New York、NYのBeatrice Yin博士およびKen Lloyd博士のご厚意により提供された)の生物反応性上清を含まずに(二次抗体の対照として)か、またはそれを含んでかのいずれかでインキュベートした。Eppendorfチューブ内の細胞をまた、1μg/チューブの非特異的アイソタイプ適合対照マウス抗体(MSKCCモノクローナルコア施設から入手された、IgGlモノクローナルについての13C4およびIgG2bモノクローナルについての4E11)で表面染色し、氷上で30分間、インキュベートした。全ての細胞をFACS緩衝液で3回、洗浄した。細胞を、1μg/チューブの二次抗体ヤギ抗マウスIgG1−PEまたはIgG2b−PEと、氷上で30分間、インキュベートし、その後、FACS緩衝液で3回、洗浄した。細胞を、MSKCCフローサイトメトリーコア施設におけるFACS Calibur機によって分析した。
安定な細胞系を、それらのそれぞれの培養培地を含む10cmのディッシュ中で培養し、5%CO2、37℃で3日間、インキュベートした。それらを、氷冷PBSで2回、洗浄して、血清含有培地を除去した。付着細胞を、1〜2mlの氷冷PBSで掻爬し、細胞を、Eppendorf遠心機において4℃で、Eppendorfチューブ内に遠心沈殿させた。上清を捨て、細胞を、0.2mlの改変Ripa溶解緩衝液(20mM Tris−HCL;pH7.4;150mM NaCl;1%NP−40;1mM Na3VO4;1mM PMSF;1mM DTT;10μg/mlロイペプチン;および10μg/mlアプロチニン)を用いて氷上で30分間、溶解させ、4℃で10分間、回転させた。可溶性溶液をチューブへと分離し、残渣のペレットを廃棄した。タンパク質濃度を、Bio−Radタンパク質アッセイ(BioRaD Laboratories、Hercules、CA)を用いて測定した。等量のタンパク質(GST−MUC16−CD融合タンパク質または安定な細胞系抽出物)を、ドデシル硫酸ナトリウムポリアクリルアミドゲル電気泳動(SDS−PAGE)によって分離し、4℃の低温室においてBioRad転写装置を用いてニトロセルロース膜へ転写した。膜を、0.1%Tween−20を含むPBS(PBST)中3%ウシ血清アルブミン(BSA)で4℃、一晩、ブロッキングした。膜を、室温で1時間、一次抗体(1:1000希釈)で探索し、その後、PBSTで3回、洗浄した。その後、対応する二次抗体である、ヒツジ由来の抗マウスIgG西洋ワサビペルオキシダーゼ(HRP)連結型全抗体(GE Healthcare、UK)(1:5000希釈)を用いて、室温で1時間、膜を染色した。膜をPBSTで3回、洗浄し、Western Lightning(登録商標)化学発光試薬(ECL、Perkin Elmer、Waltham、MA)を用いて、室温で1〜5分間、発色させ、シグナルをKodak BioMax Film上に現像した。
精製されたモノクローナル抗体を、ヨードゲン法を用いて131Iで標識し、サイズ排除クロマトグラフィーによって精製した(22)。飽和結合研究を、無傷OVCAR−3細胞の基質を用いて放射標識抗体で実施した。簡単に述べれば、10個の試験溶液を調製し(3連で)、それらは、総体積500μLのPBS(0.2%BSA;pH7.4)中に漸増量の放射性ヨウ素標識抗体、300000〜500000細胞を含有した。細胞を、ガラス繊維膜を通す急速濾過によって単離し、氷冷トリス緩衝食塩水で洗浄した。細胞を、加えられた総活性の標準物質と共に、ガンマカウンターにおいてカウントした。放射標識抗体の各濃度について、非特異的結合を、100nMの非修飾抗体の存在下で決定した。データを、最小二乗回帰法(Origin、Microcal、Software Inc.、Northampton、MA)で分析し、Kd値およびBmax値を決定し、スキャッチャード変換を実行した。
組織マイクロアレイは、本発明者らの施設内で構築するか、または内部で入手できないならば、商業的研究所から購入するかのいずれかであった。簡単に述べれば、既存のパラフィン包埋組織のコア針生検を、いわゆるドナーブロックから入手し、その後、Kononenら、および続いてHedvatらによって改変された技術を用いることによって、レシピエントパラフィンアレイ「マスター」ブロックへ再配置した(23〜24)。Beecher Instruments Inc.(Sun Prairie、WI)製の手動で操作されるTissue Arrayer MTA−1を用いて、直径が0.6〜1.0mmと測定された試料円形スポット(コア)を作製した。コアを、お互いから0.3〜0.4mm離してアレイした。対照組織の層を、エッジ効果を避けるために実際の組織マイクロアレイの周りに戦略的に置いた。各組織マイクロアレイの特定の組成は、下記に描写されている。卵巣癌、前立腺癌、肺腺癌、膵臓の粘液性新生物、ならびに浸潤性腺管癌および浸潤性小葉乳癌腫についての組織マイクロアレイのスライドを、ホルマリン固定パラフィン包埋組織から4um切片を切ることによって調製した。正常な成人および胎児の組織マイクロアレイを商業的供給源(Biomax、US)から入手した。OVCAR3細胞を陽性対照として用いた。
標準OC125(Ventana、Tuscon、AZ)および新規なモノクローナル抗体の両方に関して組織マイクロアレイで免疫組織化学検査を実施した。組織マイクロアレイの切片を4ミクロンに切断し、Superfrost/Plus顕微鏡スライド(Fisher brand)上に置き、60°のオーブンで少なくとも60分間、焼いた。その後、スライドを脱パラフィンし、蒸留水に水和させ、pH6.00におけるクエン酸緩衝液中に97℃で30分間、浸漬し、流水で2〜5分間、洗浄し、蒸留水に希釈された3%過酸化水素中で5分間、インキュベートした。スライドを蒸留水で1分間、洗浄し、リン酸緩衝食塩水(PBS)、pH7.2の浴槽へ移し、各5分間で2回交換し、PBS中に希釈された0.05%BSA中に最低1分間、置いた。組織切片の周囲を乾燥させた後、正常血清を、2%BSA/PBS中に1:20希釈で添加し、湿度チャンバー内、室温で最低10分間、インキュベートした。その後、血清を、切片を乾燥させないように、吸い出し、1:1000の希釈度における約150ラムダの新規抗体を組織上に置いた。スライドを湿度チャンバー内、4℃で一晩(約15〜18時間)、インキュベートした。一次抗体を、各10分間の3回のPBS交換を用いて洗い流した。二次抗体のVector laboratories(Burlingame、Ca)製のビオチン化α−マウスを、1%BSA/PBS中1:500希釈で添加し、湿度チャンバー内、室温で45〜60分間、インキュベートした。上記のように再び、3回のPBS交換を用いて上記抗体を洗い流した。その後、スライドを、PBS中に希釈されたジアミノベンジジン(DAB)の浴槽へ5〜15分間、移した。その後、スライドを水道水中で1分間、洗浄し、Harris改変ヘマトキシリン(Fisher)を用いて対比染色し、1%酸アルコールで脱色し、アンモニア水中で青色にし、各2分間のそれぞれが95%エタノール、100%エタノール、およびキシレンの3回の交換で脱水し、永久封入剤を用いてカバーグラスで覆った。
OC125およびM11などの市販されている抗体は、複合グリコシル化依存性エピトープを標的にする。本発明者らの仮説は、グリコシル化は組織特異的であるということである;それゆえに、パラフィン固定組織におけるペプチド指向性抗体の利用を試験し、MUC16発現の存在率を調査することは重要であった。3つの候補抗体の4H11、9C9、および4A5を、OVCAR3細胞系ペレットを用いて特徴づけた。その3つのうち、4H11抗体は、最小量のバックグラウンド染色をもち、複数の希釈度において最も強く、最もびまん性および一貫性が高い染色パターンを示し、したがって、それを、病理学コア施設においてヒト組織に用いるために最適化した。
抗MUC16モノクローナル抗体の生成および特徴づけ
個々のペプチドおよび組換えGST−ΔMUC16c114タンパク質の両方を用いるELISAに基づいたスクリーニングによって、MUC16指向性モノクローナル抗体を単離し、続いて、単一細胞クローンの連続サブクローニングを行った。

免疫組織化学検査の結果:
高特異的結合親和性を示したため、抗体9C9、4A5、および4H11を、OVCAR3細胞系を用いる免疫組織化学検査での有用性について特徴づけた。3つのうち、4H11抗体を、その他の2つの抗体と比較して、そのロバストで、高感度で、かつ特異的な染色パターンに基づいて、選択し、ヒト組織に用いるために最適化した。
40人の患者由来の原発腫瘍、転移腫瘍、および再発腫瘍を表す419個のコアで構成される2つの高病期、高悪性度の漿液性卵巣癌腫組織マイクロアレイスライドを、OC125および4H11モノクローナル抗体の両方を用いて染色した(図2)。OC125組織マイクロアレイは、3〜5染色を有する279個(66%)のコア、1〜2染色を有する99個(24%)のコア、および染色なしの41個(10%)のコアを示した。4H11組織マイクロアレイは、3〜5染色を有する236個(56%)、1〜2染色を有する91個(22%)、および染色なしの92個(22%)を示した。その2つの抗体は、233個(56%)のコアにおいて一致、161個(38%)において不定、および25個(6%)において不一致であった。25個の不一致のコアのうち、12個(不一致の症例の48%、全症例の3%)は、OC125より高い4H11の陽性を示した。9個は、4ポイントの差だけ不一致であり、3個は5ポイントの差だけ不一致であった。不一致と不定のコアが合わせて、合計186個あり、そのうちの48個(26%)が、4H11に関してOC125より高い染色を示した。4H11およびOC125の両方の染色パターンは細胞質および膜であったが、OC125の膜パターンは、症例の大部分において、4H11より強くかつより良く画定された。不一致の症例から、他の症例より4H11のより高いレベルを実証された。
4H11の抗体の特異性を試験するために、4H11染色について、様々な他の組織もまた調べた。乳房(患者の数は入手できなかった)の浸潤性腺管癌の50個のコアのうち、2個(4%)のみが4以上の4H11染色のスコアを示し、OC125染色について3〜5のスコアを有するものはなかった。OC125に関する染色パターンはほとんど頂端/管腔であり、いくらかの顆粒状細胞質染色があった。細胞質内管腔を有するいくつかの腫瘍もまた、OC125染色に反応した。4H11は、膜を染色強化することなしに、よりびまん性の細胞質濃染(blush)を示した。
他の器官由来の腫瘍はどちらの抗体とも反応性ではなかった。肺腺癌TMAは、生存腫瘍を含有する86人の患者由来の237個のコアを有した。膵臓TMAにおいて、腺管内乳頭粘液性新生物(IPMN)および浸潤性腺管癌腫を含む膵臓粘液性腫瘍を含有する21人の患者由来の92個のコアがあった。前立腺癌TMAにおいて、169個のコアがあった(患者の数は入手できなかった)。これらの癌組織マイクロアレイのいずれも、OC125も4H11のどちらに対しても有意な結合を生じなかった。この情報は表3に要約されている。

D.正常組織
正常な成人の結腸、直腸、子宮頸外部(ectocervix)、小腸、卵巣、肝臓、膵管、脾臓、腎臓、および皮膚においてOC125または4H11での染色はなかった。OC125および4H11の両方が、子宮頸管内腺(endocervical gland)(OC125管腔、4H11細胞質で弱い)、食道腺(管腔)、気管支上皮(OC125管腔、4H11細胞質内顆粒)、および胸腺小体(細胞質)を染色した。4H11は、胃腺の、特に陰窩において、細胞質内顆粒状パターンで、弱〜中程度の染色を実証した。4H11のみでの斑状細胞質内染色を示した他の器官は、前立腺、精巣の精細管、および膵臓の島細胞であった。膵島細胞における染色は特に強く、一貫していた。4H11での肝臓、腎臓、および脳の非特異的染色もあった。OC125で染色され、かつ4H11で染色されなかった症例はなかった。
MUC16抗原に向けて遺伝的に標的化されたT細胞の養子移入後のSCID−Beigeマウスにおける確立した腹膜卵巣腫瘍の根絶の成功
目的:卵巣癌と診断されたほとんどの患者は、最終的には彼らの疾患で死亡する。このため、この悪性腫瘍の処置への新規なアプローチが必要とされている。腫瘍関連抗原へ標的化される、人工的T細胞受容体であるキメラ抗原受容体(CAR)をコードする遺伝子の導入によりエキソビボで遺伝子改変された、患者自身のT細胞の養子移入は、卵巣癌の処置に適用可能な癌治療への新規かつ有望なアプローチである。
卵巣癌は、世界中で6番目に最も頻度の高い癌であり、女性における癌関連死亡原因の第7位である(1、2)。手術および化学療法による複数様式(multimodality)治療にも関わらず、卵巣癌腫を有するたいていの患者は、予後が良くない。このため、この疾患を処置するための代わりとなるアプローチが緊急に必要とされている。
細胞系およびT細胞
OV−CAR3腫瘍細胞系を、10%熱不活化FBS、非必須アミノ酸、HEPES緩衝液、ピルビン酸塩、およびBME(Invitrogen)を補充したRPMI 1640(Invitrogen、Grand Island、NY)中で培養した。PG13およびgpg29レトロウイルスの産生細胞系を、10%FCSを補充したDMEM(Invitrogen)中で培養し、以前に記載された(3)NIH−3T3人工の抗原提示細胞(AAPC)を、10%熱不活化ドナー仔ウシ血清を補充したDMEM中で培養した。T細胞を、BD Vacutainer CPTチューブ(Becton Dickinson、Franklin Lakes、NJ)中に、製造業者の指示に従って、IRB承認プロトコール#95−054の下、健常ドナーの末梢血から採取した。全ての培地に、2mmol/LのL−グルタミン(Invitrogen)、100ユニット/mLのペニシリン、および100μg/mLのストレプトマイシン(Invitrogen)を補充した。T細胞を、20IU/mlのIL−2(Novartis Pharmaceuticals、East Hanover、NJ)を補充した上記のようなRPMI 1640で培養し、示される場合、培地に、10ng/mLのインターロイキン15(R&D Systems、Minneapolis、MN)を補充した。
原発性ヒト腹水由来癌細胞を、IRB承認プロトコール#97−134の下、新たに診断された進行性漿液性卵巣癌腫の手術を受けた卵巣癌患者から入手した。腫瘍細胞を、患者の腹水から、室温で10分間の600gでの遠心分離によって単離した。細胞を、1×PBSで1回、洗浄し、将来の分析のために、10%FBSを補充したRPMI 1640培地中で培養した。
4H11モノクローナル抗体の重鎖可変領域および軽鎖可変領域は、ハイブリドーマ細胞系4H11に由来した。4H11 RNAから作製したcDNAを利用して、本発明者らは、他のところで記載されているように、改変型プライマーを利用するRACE PCRによってVHコード領域を単離した(39、40)。VL鎖可変領域を、Orlandiらによって記載されているように改変型プライマーを利用する標準PCRによってクローニングした(41、42)。生じたVHおよびVL断片を、TopoTA PCR 2.1クローニングベクター(Invitrogen)へサブクローニングし、配列決定した。その後、VHおよびVL断片を(Gly4Ser)3スペーサードメインにライゲーションし、4H11 scFvを作製し、それをオーバーラップPCRによってヒトCD8リーダーペプチド(CD8L)に融合した(9、41)。MUC−CD標的化4H11 CARを構築するために、CD8L−4H11 scFvのコード領域を、T細胞受容体CD3ζシグナル伝達ドメインに融合した、ヒトCD8ヒンジドメインおよび膜貫通ドメインに融合し(4H11z CARが生成)、または代替としてCD28膜貫通ドメインおよび細胞質シグナル伝達ドメインに融合した(4H11−28z CARが生成)(3、9、43)。その後、生じたCAR構築物を、改変型MMLVレトロウイルスベクターSFGへサブクローニングした(44)。形質導入されたgpg29線維芽細胞由来のVSV−G偽型(preudotyped)レトロウイルス上清を用いて、安定なPG13テナガザル白血病ウイルス(GaL V)エンベロープ偽型レトロウイルス産生細胞系を構築した(41)。
単離された健常ドナー末梢血単核細胞(PBMC)に対し、レロネクチン(retronectin)をコーティングした非組織培養プレート上で、2μg/mlの植物性血球凝集素(PHA)(Sigma.St.Louis、MO)で活性化し、レトロウイルスで形質導入した(45)。簡単に述べれば、6ウェル非組織培養プレート(BD Biosciences、San Jose、CA)をRetroNectin(RN)(Takara Biomedicals、Otsu、Japan)で、製造業者の指示に従って、コーティングした。PHA活性化から48時間後、1mlの補充されたRPMI培地中1×106個のT細胞のアリコートを、1mlのSFGレトロウイルス上清と共に、RNをコーティングしたプレートの各ウェルに入れた。T細胞を、毎日新鮮なレトロウイルス上清を加えながら、3日間連続して、毎日、30℃で1時間、2000gで遠心分離した(45)。7日目に遺伝子移入をFACSによって評価した。
刺激による増殖およびサイトカイン放出をインビトロで評価するために、形質導入されたT細胞を、6ウェル組織培養プレート(BD Biosciences)において、サイトカインを補充せずに10%FBSを補充したRPMI培地中、コンフルエントなNIH 3T3 AAPC上でレトロウイルスによる形質導入を行なった後7日間、共培養した。インビボ研究のための十分な数のCAR改変T細胞を生成するために、形質導入されたT細胞を、以前に記載されているように、20IU/mLのIL−2および10ng/mLのIL−15を補充したRPMI培地中、B7.1+AAPC(3T3(MUC−CD/B7.1))上で共培養した(3、43)。患者T細胞を、ヒトCD3/CD28ビーズ(DYNAL(登録商標)、Invitrogen、Carlsbad、CA)を用いて、製造業者の指示に従い、活性化し、かつ増殖させた。
0.1mol/LのDTT(Sigma)による還元条件下でのT細胞溶解産物のウェスタンブロット分析を、以前に記載されているように行った(46)。簡単に述べれば、形質導入されたT細胞を、PBS中で洗浄し、ミニコンプリートプロテアーゼインヒビターを用いて、製造業者の指示に従い(Roche Diagnostics、Indianapolis、IN)、放射免疫沈降アッセイ(RIPA)緩衝液(Boston BioProducts、Worcester、MA)中に再懸濁した。生じたタンパク質を、6×還元ローディング緩衝液(Boston BioProducts、Worcester、MA)の添加および100℃で10分間の加熱後、12%SDS−PAGEミニゲル(Bio−Rad、Hercules、CA)で分離した。その後、分離されたタンパク質を、Immobilon膜に転写し、抗ヒトCD3ζ鎖モノクローナル抗体(BD Biosciences)を用いて探索した。抗体結合を、ヤギ抗マウス西洋ワサビペルオキシダーゼ結合体化抗体でそのブロットを探索し、続いて、Western Lighting Chemiluminescence Reagent Plus(Perkin−Elmer Life Sciences、Boston、MA)を用いて製造業者の指示に従って、発光を検出した。
インビトロでの改変T細胞の細胞傷害性を、DELFIA(登録商標)EuTDAアッセイ(PerkinElmer LAS,Inc、Boston、MA)を用い、製造業者の推奨に従って評価した。細胞傷害性を、示された比でのエフェクターT細胞対標的OV−CAR3(MUC−CD)細胞または原発腫瘍細胞(E:T)において2時間で評価した。これらのアッセイにおけるエフェクターT細胞はCD8+CAR+T細胞の数を表す。
サイトカインアッセイを、Luminex IS 100システムを利用するIL−2およびIFNγを検出するためのマルチプレックスHuman Cytokine Detectionアッセイ(Millipore Corporation、Billerica、MA)を用いて製造業者の仕様書に従って、実施した。サイトカイン濃度を、IS 2.3ソフトウェア(Luminex Corp.、Austin、TX)を用いて評価した。
全てのインビボ研究において、8〜12週齢のFOX CHASE C.B.−17(SCID−Beigeマウス)(Taconic、Hudson、NY)に最初、3×106個のOV−CAR3(MUC−CD)腫瘍細胞か、または生物発光画像化(BLI)研究のために、3×106個のOV−CAR3(MUC−CD/GFP−FFLuc)腫瘍細胞のいずれかを腹腔内に注射した。その後、3×107個のCAR+T細胞を、示されている通り、腫瘍注射後1日目または7日目に、腹腔内または静脈内に注射した。腹囲の増加、立毛、および刺激に対する応答の低下によって評価される苦痛(distress)についてマウスをモニターした。苦しんでいるマウスを安楽死させた。全てのマウス研究を、Institutional Animal Care and Use Committee承認プロトコール(#00−05−065)の関係において行った。
BLIを、Living Imageソフトウェアを備えたXenogen IVIS画像化システム(Xenogen;Alameda、CA)を用いて実施した。簡単に述べれば、OVCAR3(MUC−CD/GFP−FFLuc)腫瘍を担持するマウスに、200μlのPBS中に懸濁されたD−ルシフェリン(150mg/kg;Xenogen)を腹腔内に注射し、10分後、2%イソフルラン麻酔下で画像化した。画像収集を、25cmの視野において中間ビニングレベルで0.5分の露光時間で行った(3、43)。
T細胞および腫瘍細胞の全てのフローサイトメトリー分析を、Cellquestソフトウェアを備えたFACScanサイトメーター(BD Biosciences)を用いて実施した。T細胞を、CAR特異的ポリクローナルヤギAlexa Fluor 647抗体(Molecular probes、Eugene、OR)、フィコエリトリン標識抗ヒトCD4、CD8、B7.1(Caltag Laboratories、Burlingame、CA)、B7.2(Invitrogen、Camarillo、CA)、4−1BBL、およびOX40抗体(Ancell Corporation、Bayport、MN)を用いて分析した。3T3(MUC−CD)細胞およびOV−CAR3(MUC−CD)細胞を、Alexa Fluor 647標識4H11抗体(MSKCCモノクローナル抗体コア施設において作製および標識された)で染色した。
CAR+T細胞を、CellTrace(商標)CFSE細胞増殖キットを用いて製造業者の推奨(Molecular Probes、Eugene、OR)に従って、CFSEで染色した。CFSE標識T細胞の増殖を、FACSによって分析した。CFSE標識T細胞のインビボでの検出のために、卵巣腫瘍を40μmの細胞ストレーナー(BD Falcon、Franklin Lakes、NJ)を介して解離させ、抗体染色およびFACS分析の前に2%FBS/PBSで2回、洗浄した。
生存データを、GraphPad Prismソフトウェア(GraphPad Prism software、San Diego、CA)を用いる対数順位解析によって評価した。サイトカインデータを、スチューデント片側t検定によって解析した。
本発明者らは、MUC−CD抗原に特異的なモノクローナル抗体を産生する4H11ハイブリドーマを用いてMUC−CD抗原に対して標的とする第1世代(4H11z)および第2世代(4H11−28z)のCARをコードするSFGレトロウイルスベクターを構築している(図11A)。本発明者らは、ζ鎖特異的抗体で探索される、生じたPG−13レトロウイルス産生細胞(SFG−4H11zおよびSFG−4H11−28z)のウェスタンブロット分析によって、適切なサイズのCARタンパク質の発現を確認した(データ未呈示)。
4H11z+T細胞および4H11−28z+T細胞のヒト卵巣癌腫腫瘍を標的にし、かつ溶解する能力を評価するために、本発明者らは、MUC−CD抗原を発現するように遺伝子改変されたヒトOV−CAR3細胞系を利用し、そのことにより、4H11が標的にするMUC−CD抗原を発現する臨床的卵巣腫瘍試料の大部分をより良く反映している(48)。本発明者らは、最初に、MUC−CD標的化T細胞による特異的な溶解を検証し、無関係の第1世代および第2世代のCD19標的化19zl CARおよび1928z CARを発現する対照T細胞と比較して、OV−CAR3(MUC−CD)腫瘍細胞を標的にする、4H11z CAR改変T細胞および4H11−28z CAR改変T細胞の類似した著しい細胞傷害活性が実証された(図13A)。4H11−28z CARを発現するように改変された健常ドナーT細胞は同様に、19−28z形質導入T細胞と比較して、新鮮に単離された腹水由来MUC−CD+卵巣癌腫細胞の溶解を示した(図13B)。さらに、4H11−28z CARを発現するように改変された患者の末梢血T細胞は同様に、対照19−28z CARを発現するように改変されたT細胞と比較して、同じ腹水試料由来の自己一次MUC−CD+腫瘍細胞を溶解した(図13C)。
4H11z+T細胞および4H11−28z+T細胞のインビボの抗腫瘍活性を評価するために、本発明者らは次に、OV−CAR3(MUC−CD)腫瘍細胞のSCID−Beigeマウスへの腹腔内注射によって、同所性異種移植卵巣癌腫瘍モデルを作製した。未処置のままであった場合には、これらのマウスでは、腫瘍細胞注射後3週間までに、著しい腹水および複数の結節性腹膜腫瘍が発生した(図14A)。未処置の腫瘍を担持する全てのマウスは、腫瘍細胞注射後7週間までに苦痛の徴候により安楽死させなければならなかった。
4H11−28z+T細胞が、より臨床的に関連した腫瘍負荷量を根絶することができるかどうかをさらに評価するために、本発明者らは、養子T細胞治療の7日前に注射された、十分確立した腹腔内OV−CAR3(MUC−CD/GFP−FFLuc)腫瘍を担持するSCID−Beigeマウスを処置した。もう一度、本発明者らは、MUC−CD標的化T細胞での治療が、全ての処置されたマウスにおいてBLI顕性疾患を著しく根絶したことを見出し(図16A)、腫瘍細胞注入後120日間までのBLI画像化(未呈示)により評価した場合、8匹の処置されたマウスのうち5匹が、最終的に、進行性疾患の再発をおこし、3匹のマウスは疾患がないままであった(図16B)。これらのデータは、進行性疾患の状況においてさえもMUC−CD標的化T細胞により媒介される強力なインビボ抗腫瘍活性を実証している。
患者腫瘍試料の広範な分析に基づいて、卵巣癌腫は、比較的免疫原性腫瘍であるようにみえる。具体的には、研究者らは、手術および化学療法後の予後と、処置前の腫瘍試料における腫瘍浸潤性エフェクターT細胞(TIL)の量との間に直接的相関があることをそこに見出している(25、49、50)。さらに、別の研究者らが、治療後の予後と、腫瘍内のTreg(それらは、次に、おそらく、腫瘍特異的エフェクターTILの抗腫瘍機能を抑制する(26、28、51))の処置前レベルとの間の逆相関を記載している。これらの所見のどちらも、最初の治療の前と後の両方での疾患進行の制御における、内因性エフェクターT細胞の腫瘍に対する応答についての役割を意味し、卵巣癌腫が、卵巣腫瘍細胞抗原に対して標的化した自己T細胞の養子注入による死滅化に感受性が高い可能性があるという主張を強く支持している。
マウスおよびハムスターにおけるマウスMUC16モノクローナル抗体の産生
本発明者らは、モノクローナル抗体がマウスおよびハムスターにおいて生じるための、マウスMUC16ゲノムの3つの異なる領域を選択した。マウスMUC16の選択された領域は、ペプチド1(配列番号21、細胞質ドメインの外部領域)、ペプチド2(配列番号22、第1のシステインループ)、およびペプチド3(配列番号23、第2のシステインループ)であり(図20A)、それのヒトMUC16との比較は、図20Bに示されている。KLHとのより良い結合体化のために、ペプチド1(配列番号21)およびペプチド3(配列番号23)のN末端のペプチド配列にシステインを付加した。個々のペプチドを、Promegaキットを用いてKLHに結合体化した。これらの3つの結合体化されたペプチドをプールし、5匹のマウスおよび4匹のハムスターへ免疫した。1回の免疫ごとに3週間の間隔をあけて5回の免疫を施した。これらの動物由来の血清を、個々のペプチド(配列番号21、22、および23)との特異的な反応性についてELISAによって試験した。陽性選択された動物を1ヶ月間、休ませ、その後、プールされたペプチド免疫原(配列番号21、22、および23)で静脈内にブーストし、4日後、脾臓を採取した。脾細胞をハイブリドーマパートナーと混合し、様々なクローン密度でマイクロタイタープレートへ蒔いた。プレートを37℃、5%CO2で10日間、培養し、その後、クローンを選択した。これらの選択されたクローンからの上清を、個々のペプチド(配列番号21、22、および23)との特異的な反応性についてELISAによって試験した。陽性クローンの上清を、2つのマウス細胞系(ID8およびBR5−FVB1)およびヒト細胞系(OVCAR−3)を用いるFACS、ウェスタンブロット、および画像化により試験した。
表4は、マウスMUC16ペプチド抗原ペプチド1(配列番号21)、ペプチド2(配列番号22)、およびペプチド3(配列番号23)に対するマウスおよびハムスターのモノクローナル抗体の概要を示す。非常に強い抗原応答は、ペプチド1(配列番号21)に関して見られた。
