Description
本発明は、個体におけるアポトーシス促進タンパク質をコードする核酸の制御された発現の為の核酸に関する。
特に、核酸の発現は、少なくとも1つの必須アミノ酸が不足している食事の消費時に制御されうる。
最近、遺伝子導入ベクターにおける技術的改善及び適切な遺伝子送達システムの開発は、遺伝子治療における実質的な臨床的成功がもたらされている。しかしながら、遺伝子治療が広範な疾患群に対して広く受け入れられた処置法になる前に、幾つかの障壁が克服されなければならない。利用可能なベクター系に関する安全性(例えば、遺伝毒性、免疫応答)の懸念に加えて、主な制限は、信頼のある、非常に特異的な、単純な且つ安全な様式で導入遺伝子の発現を外因的に調節することが現在できないことである。1つの戦略は、用量依存的な方法で、例えば経口的に利用可能な良好な耐容性の誘導物質によって、遺伝子発現を調節することであろう。さらに、遺伝子発現の誘導及び抑制の両方が可逆的であるべきである。
有害事象の場合に、注入された細胞を迅速に取り除くことが可能であれば、科学界は、細胞療法が癌処置及び再生医療において役割を果たす可能性があるという事実に同意する。より具体的には、十分に確保された 分子スイッチが、多くの種類の癌の治療法を提供するという大きな期待を満たすために、養子キメラ抗原受容体(CAR:chimeric antigen receptor)T細胞に基づく処置の為に緊急に必要とされるであろう。そのようなスイッチはまた、幹細胞に基づく、特に造血幹細胞に基づく、及びiPS細胞に基づく治療法の可能性を確保し且つ再生医療の分野を促進する為に非常に貴重であるだろう。
養子T細胞治療は最初に、ワクチン接種単独によって得られることができるものよりも多い数のT細胞を達成する為の腫瘍特異的T細胞の単離及びエクス・ビボ増殖を含む。次に、腫瘍特異的T細胞は、腫瘍特異的抗原のT細胞認識及び癌細胞破壊を介して残存腫瘍を圧倒する能力をそれらの免疫系に与えようとして、癌を有する患者に注入される。言い換えれば、そのような治療は、患者自身の免疫細胞を操作して腫瘍を認識し、そしてそれらの腫瘍を攻撃することを含む。
癌処置の為に使用される養子T細胞治療の幾つかの形態がある:腫瘍浸潤リンパ球(TIL:tumor infiltrating lymphocytes)を培養しそして拡大すること、1つの特定のT細胞クローンを単離しそして拡大すること、並びに腫瘍抗原を強力に認識し且つ腫瘍細胞を破壊する為にキメラ抗原受容体(CAR)を発現するように操作されたT細胞を用いること。CAR T細胞は、臨床試験において最も有望であり、50以上のCAR臨床試験が米国だけで現在登録されている(clinicaltrials.gov)。注目すべきことには、キメラ抗原受容体(CAR)-リダイレクトされたT細胞は、治療抵抗性白血病を有する患者において長期の恒久性の寛解及び顕著な奏効率をもたらしている。
現在まで、CAR技術は、各患者自身のT細胞からの治療用製品のカスタムメイド製造を通じて管理されている。しかしながら、この患者特異的な自己のパラダイムは、CAR技術の大規模な展開における重要な制限因子である。同種移植の状況において、無関係の第三者ドナーT細胞からの「既製の(off-the-shelf)」CAR T細胞の産生の為のプラットフォームが現在開発されている。
CAR T細胞はクリニックにおいて大きな初期の見込みを示しているが、これらの開発は幾つかの重篤な且つさらには致命的な副作用を伴っている(Gargett and Brown,Front Pharmacol.,2014年10月 28;5:235)。
移植後の再発を有する患者におけるドナーT細胞注入は移植片対白血病効果を介して疾病の寛解をもたらすことができることが観察されているが、これは一般に、非造血組織に対する同種免疫の結果としてGVHD(graft versus host disease:移植片対宿主病)の展開に関連付けられている(Kolb,Blood,2008年12月1日;112(12):4371-83)。
分化抗原を標的とするT細胞は、同じ抗原を発現する非悪性細胞をまた認識し、有害事象を結果として生じることが予測されることができるため、標的ではあるが腫瘍外(On-target but off-tumor)の有害作用が生じうる。例えば、メラノサイト分化抗原、例えばMART-1及びgp100、を標的とするT細胞で処置されたメラノーマ患者はしばしば、白斑及びぶどう膜炎を発症した(Teulings等,2014年;Pigment Cell Melanoma Res.2014年11月;27(6):1086-96)。
標的ではあるが腫瘍外(On-target but off-tumor)の毒性は、直ちに生命を危うくすることができる。肺転移及び肝転移を伴う結腸直腸癌を有する患者は、HER2特異的CAR T細胞注入の15分以内に呼吸困難を発症し、そしてその後5日後に多臓器不全で死亡した。
これらの各場合において、腫瘍外部位における比較的低いレベルの抗原発現にも関わらず、有害作用が生じ、従って高い結合力及び効力を有するリダイレクトされたT細胞を使用することにおける有害性の可能性を強調する。
T細胞治療がより有効になるにつれて、急性毒性がまたより明白になり、且つ発熱、厳しさ、低血圧及び低酸素によって特徴付けられるサイトカイン放出症候群を引き起こしうる。この症候群は、CD19+悪性細胞の認識に応じて大規模のT細胞活性化の結果として、多くのCD19 CAR T細胞研究において観察されている。
T細胞治療の魅力の1つは、導入された細胞が存続しそして拡大し、それにより持続的な治療効果を媒介する見込みがあることである。しかしながら、如何なる有害作用がまた同様に持続し、細胞が増殖するにつれて悪化し、そして細胞増殖に関連付けられるだろう(Tey,Clin Transl Immunology,2014年6月20日;3(6):e17)。
Zhou等によって認められている通り、細胞治療は癌処置に有効でありうるが、それらの拡大、正常臓器の損傷、及び悪性形質転換の可能性は懸念の元である。イン・ビボで異常なT細胞を条件付きで取り除く能力はこれらの懸念を改善し、そして細胞療法の適用を広げるであろう(Methods Mol Biol.2015年:1317:87-105)。
従って、その可能性を満たす為に、養子細胞移入技術は、特にCAR T細胞のレベルで「自殺」安全性要素を組み込む必要がある。以下において議論されるであろう通り、幾つかのアプローチが開発されているが、臨床現場において安全に移される為の要求を完全に満たすものではない。
ESC 細胞の可能性は、造血系の疾病の状況において大部分実証されている。同様に、人工多能性幹細胞(iPSC:induced pluripotent stem cell)技術は、それらの無限の自己再生能力と3つの胚性胚葉の全てに由来する細胞型に分化する為の能力とに基づいて、再生医療に大きな期待を寄せた。明らかに、iPSCに基づく治療法が、患者特異的な再生医療の為の有望な道を提供する。
Assawachananont等は最近、マウス胚性幹細胞又は人工多能性幹細胞由来の3D網膜組織が、外顆粒層(ONL:outer nuclear layer)を欠く高度な網膜変性モデル (rd1)においてさえも、完全な内側セグメント及び外側セグメントを有する構造化された外顆粒層(ONL:outer nuclear layer)を発達させたことを示した。宿主−移植片シナプス結合は、免疫組織化学によって観察され、進行した網膜変性疾患の為の網膜シート移植治療の為の「概念の証明」を提供した(Stem Cell Reports.2014年4月24:2(5):662-74)。
これらの結果は、非ヒト 霊長類における研究とともに、自己のiPSC由来の網膜色素上皮 (RPE:retinal pigment epithelial)細胞シートのヒトへの移植に着手することを上記著者等に導いた。この臨床治験は、iPS細胞ベースの製品の最初の歴史的移植であるとして歓迎された。
患者は、両眼に一連の18の抗血管内皮増殖因子 (VEGF:anti-vascular endothelial growth factor) 眼内注入を行って、該疾患の一定の再発に対処した。網膜下の線維症組織の除去及びiPS細胞由来のRPE 細胞シートの移植後、患者は、6ヶ月の時点で新血管形成の再発を経験せず、そして頻繁な抗VEGF注射を受けなかった。彼女の視力は安定されており、少なくとも移植後のほぼ1年後の少なくとも2015年9月において安全に関する懸念を有していなかった。
しかしながら、この試験について懸念が生じた:「いまだ、iPS 細胞から作られた組織はそれ自体の懸念を有しており、且つそれはどの国も臨床試験の為のそれらを承認することを中止していた。体の免疫系がそれらを攻撃することができ、又は彼らは、まだ多能性状態にある幾つかの細胞を含んでいてもよく、且つ癌性増殖を引き起こす幾つかの細胞を含んでいてもよい(Jeanne Loring,Scripps Research Institute in La Jolla,California)。
明らかに、RPEシートの製造に従事しているiPS細胞に、堅牢なビルトイン安全システムを装備することが非常に重要である。CAR T細胞注入の状況において上記で強調されている通り、そのような系を保有するRPE 細胞を用いた臨床試験(及びその後の処置)は、かなり安全であろう。そのような安全デバイスはまた、眼の疾病の為の治療法の出現をかなり促進するだけでなく、再生医療の分野全体を刺激するだろう。
事実、既存の「自殺」系は、それらの注意点を有する。
臨床的に試験されている最初の自殺遺伝子は、分裂細胞に対して毒性であるガンシクロビルのガンシクロビルトリホスフェートへの転化を媒介する単純ヘルペスウィルスチミジンキナーゼ (HSVtk:herpes simplex virus thymidine kinase)である。しかしながら、安全スイッチとしてのHSVtkは、多くの欠点、例えば、ガンシクロビルによるHSV-TKの活性化が比較的遅く、イン・ビトロで完全な効果を有する為に3日を必要とするという事実、を有する(Marin等,Hum Gene Ther Methods.2012年12月;23(6):376-86)。
最も重要なことに、ウィルスTK遺伝子産物は、形質導入細胞が免疫適格個体において宿主免疫系によって拒絶される原因となりうる固有の免疫原性を有する(Berger等,Blood.2006年3月15日;107(6):2294-302)。さらに、ガンシクロビルが造血幹移植の免疫不全レシピエントにおけるCMV感染を処置する為に使用される場合、この自殺遺伝子の使用は、形質導入細胞の望ましくない欠失を結果として生じるであろう(Bonini等,Mol Ther.2007年7月;15(7):1248-52)。
別の自殺遺伝子系は、低分子の、生物不活性の、二量化の化学誘導(CID:chemical induction of dimerization)薬物であるAP1903と共に、誘導性カスパーゼ9(iCasp9:inducible caspase 9)遺伝子からなる。iCasp9遺伝子は、ヒトFK506-結合タンパク質に由来する薬物結合ドメインに融合した、アポトーシス促進分子であるヒトカスパーゼ 9 タンパク質の細胞内部分を含む(Straathof等,Blood.2005年6月1日;105(11):4247-54)。AP1903の静脈内投与は、このキメラタンパク質の薬物結合ドメインの架橋を生成し、それは今度はカスパーゼ9を二量化し、そして下流の実行者であるカスパーゼ3分子を活性化し、細胞アポトーシスを結果として生じる。
SCIDマウス-ヒト異種移植モデルを用いた初期のイン・ビボ実験において、単回投与が3日目までに循環ヒト GFP+ T細胞の99%以上を死滅させた。重要なことに、iCasp9を介した死滅は、30分以内に現れる初期アポトーシスアネキシンV+ 細胞で非常に急速であることが見出されており、イン・ビトロでの24 時間のCID処置によって完全な効果が観察された(Marin等,2012年)。
造血幹細胞及び前駆細胞に関して (Stem Cells.2015年1月;33(1):91-100)、Cyntia Dunbar(NIH)は、アカゲザルにおけるiCasp9を発現する造血細胞の安定した生着に続いて、iCasp9を活性化することができる二量体化の化学誘導物質であるAP1903の投与は、全ての造血系統におけるベクター含有細胞を長期的に特に取り除かれ、HSPCレベルでの活性を示唆することを報告した。75〜94%のベクター含有細胞が良好な耐容性の AP1903の投与により取り除かれ、しかし完全なアブレーション(ablation)の欠如は、残存細胞におけるiCasp9発現を低下させることに関連していた。耐性機構の更なる研究は、ベクターで形質導入され且つAP1903アブレーションに対して耐性である、造血細胞株におけるBcl-2の上方制御を示した。これらの結果は、臨床開発との高い関連性を有するモデルにおいて、HSPCを標的とされた遺伝子治療設定に対してiCasp9を利用する安全性アプローチの可能性と限界との両方を示した。
iCasp9自殺系の現在の有効性は幾つかの特定の状況において適しているかもしれないが、それは腫瘍形成の過程を十分に克服しないであろう。
いずれにせよ、主な問題は、その固有の免疫原性の問題である。
Malcom Brennerのチームは、iCasp9自殺系を推進した。iCasp9自殺遺伝子のヒト起源は、それを異種起源からの自殺遺伝子よりも抗原原性をおそらく低くする。遺伝子導入細胞に対する免疫応答の証拠は見出されず、それは我々の患者において長期間に亘って安定したレベルで持続したが、それは他の臨床設定における構築物のいずれの要素の免疫原性も排除することができない(Di Stasi等;N.Engl.J.Med.2011年11月3日;365(18):1673-83)。
本質的に、iCasp9タンパク質は、合成の20個のアミノ酸ペプチドの為に潜在的に免疫原性であり、及びこのペプチドと2つのペプチド部分との間のヒンジは非ヒト 起源のものである。ごく少数の患者のみを含む初期臨床試験において有害事象は観察されなかったが、より多くの患者が登録されるときに免疫学的事象が起こらないという保証は無く、保証さえもないだろう。
明らかに、新規な自殺系が必要である。
幾つかの栄養関連の研究は、生理学的機能の調節における(AA:amino acids)の役割、特にAAによる遺伝子発現の調節に関与するメカニズムに関して、を識別することに焦点を当てている。1つの必須アミノ酸(EAA:essential amino acid)が不足している食事を摂った後、制限的なEAAの血中濃度は急速且つ大幅に減少し、アミノ酸応答経路と呼ばれる遍在する適応過程を引き起こす。この経路の最初の工程は、セリン51上の真核生物開始因子2 (eIF2α:eukaryotic initiation factor 2)のアルファーサブユニットをリン酸化する、哺乳類のGCN2 タンパク質キナーゼの荷電されていないtRNAによる活性化であり、活性化転写因子(ATF4:activating transcription factor)の翻訳の上方制御を導く。一旦誘発されると、ATF4は、アミノ酸応答要素 (AARE:Amino Acid Response Element)への結合を通じて特定の標的遺伝子の転写を活性化する。GCN2-eIF2α-ATF4経路は、欠如しているEAAの投与によって急速に止めることができる。
これらの知見を利用して、特許出願 国際公開第2013/068096号は、少なくとも1つのAARE調節配列及びミニマルプロモーターを含む誘導性プロモーターの制御下にある関心のある遺伝子を含む発現カセットを開示した。

本発明者等は、遺伝子治療に理想的に適した調節系を達成する為に、1つの必須アミノ酸が不足している食事に対する栄養適応経路の顕著な特徴を評価した。本発明者等は、食事特異的アミノ酸飢餓に基づくそのような系は、合成転写因子又は調節タンパク質の発現を必要とせず、また薬理学的誘導物質の投与も必要としないことを見出した。それは、生理学的に非毒性であり且つ臨床応用に適している。この新規な栄養ベースの調節系は、ヒトの遺伝子治療における主要な残りの障害の1つを解決する為の能力を備えた生理学的アプローチとして成り立ち、且つ「自殺戦略」に基づく治療アプローチの為の「安全スイッチ」を提供する。
本明細書の実施例に示されているとおり、AARE駆動発現系は順応性であり、当該系において各個々の必須アミノ酸が潜在的な誘導物質として作用することができる。単一のサイクルを使用するという状況においてさえ、特定の実験/状況に最も適切であろう誘導過程の増幅に従って「欠損している必須アミノ酸」を選択する為の手段を提供するという点で本発明者等はこの点は重要であると考える。
下記の実験データにおける重要な部分は、アポトーシス促進タンパク質TRAILに関する。対応する一連の実験は、癌を扱う問題に対処するためだけでなく、バックグラウンド活動の問題及びモデルシステムの動力学に対処する為のモデルシステムとしても実施された。言い換えれば、臨床的に関連する為には、遺伝子調節系は、ベクターの安全な用量内で、誘導物質の広い用量範囲に亘って調節されるべきであり、且つ低レベルのバックグラウンド発現を示すべきである。
以下の説明から明らかになるであろう通り、AARE/自殺遺伝子系は自殺暗線デバイスとして作用するのに理想的に適しており、ありうる免疫応答を結果として生じない。そのようなデバイスは、CAR T細胞の機能にも幹細胞/iPS細胞の成熟/分化にも必要でない。しかしながら、そのような安全デバイスの存在は、これらの対応する治療において欠くことのできないセーフガードを表すであろう。
さらに、AAREに基づく安全デバイスは、本質的に完全に安全である。CAR T細胞(又は iPSC由来の細胞)を破壊するであろう自殺遺伝子の予期せぬ発現は、患者が治療用の「既成の(off-the-shelf)」CAR T細胞の別の注入を容易に受けることができることを考慮すると、患者を悩ませるものでない。
最後に、AAREに基づく自殺系は、安全スイッチの分野及び開発をかなり広げる。カスパーゼの全ファミリーが考慮されることができる。その上、AARE 自殺系は自殺タスクを実行する為に非ヒト起源のタンパク質に必ずしも依存しない。自殺導入遺伝子の発現は、対応する細胞の実行が不可逆的に続かなければならず、それにおいて自殺遺伝子が内因的に発現され、如何なる免疫反応の時間も許されない。従って、自殺遺伝子のリストは様々な起源からの「毒」を含むことができ、ただし、該遺伝子産物が分泌されず、それらの発現が細胞の死滅と密接に関連付けられている。

実施例
1/方法。
1.1/倫理規定。

流体力学的注射が、マウスの10%身体容積に対応する生理食塩水緩衝液中に50μgのpGL3-2XAARETRB3-Tk-LUC プラスミドを希釈することによって調製さて、そして6週齢の雄のBalB/Cマウスの尾静脈に5秒間に亘って投与された(1群当たり6匹)。注射の24時間後に、16時間絶食されたマウスは、対照食餌又はイソロイシンが欠乏した食餌を6時間与えられた。実験の最後に、全身及び切除された肝臓の生物発光が生物発光画像法システム(IVISスペクトル,PerkinElmer)を用いることによって測定された。

マウス胎児線維芽細胞(MEF:Mouse embryonic fibroblasts)、HeLa細胞及びGli36-luc細胞が、10%のウシ胎児血清を含むダルベッコの改変イーグル培地F12(DMEM F12:Dulbecco’s modified Eagle’s medium F12)(Sigma)中、37℃で培養された。指示がある場合には、ロイシンを欠くDMEM F12(DMEM F12 Base)(Sigma)が用いられた。アミノ酸飢餓を含む全ての実験において、10%の透析された牛血清が用いられた。Gli36-Luc細胞が、Shah K博士(Harvard Medical School,Boston,MA)の贈り物である。GCN2-/-及びPERK-/-MEFsは、D.Ron博士及びH.Harding博士(Institute of Metabolic Science,Cambridge,UK)によって与えられた。PKR-/-MEFsは、John C Bell博士(Ottawa health research institute,Canada)からのものである。KO MEFsはPCR及びウェスタンブロット解析によって、及びGli36-Luc細胞はルシフェラーゼアッセイによって確認された。全ての細胞株が、マイコプラズマフリーであった。Gli36-luc細胞が、ポリブレン(5 μg/mL)の存在下で、10のMOIを用いることによって、LV-AARE-eGFPベクター又はLV-AARE-TRAILベクターのいずれかで形質導入された。感染の48時間後に、細胞が10cm皿に移され、そして実験目的の為に維持された。
1.8/免疫ブロット分析及び抗体。
ウェスタンブロットが、Maurin等 (Cell reports,2014年,6,438-444)において以前に記載された通りに行われた。使用された一次抗体は、抗ホスホ-eIF2α(Abcam,ab32157)、抗ATF4(Santa Cruz,sc-200)、抗アクチン(Santa Cruz,sc-1616R)、切断された抗PARP(Cell signalling tech.,5625)、抗TRAIL(Cell signalling tech.,3219)であった。

1.9/一過性形質転換及びルシフェラーゼアッセイ。

操作の容易の為に、2XAARE TRB3-Tk-TRAIL構築が、SEQ ID NO: 8によって記載された通り、pENTR プラスミドに最初に実行された。
1.11/細胞生存、アポトーシス及びELISAアッセイ。
細胞の生存率が、XTT細胞生存率キット(Cell signaling tech.,9095)を用いて測定された。アポトーシスが、製造者の指示に従って、ANXA5/PE/7-AADアポトーシス検出キット(BD Biosciences,559763)を用いてフローサイトメトリー分析によって評価された。ELISAアッセイに関して、処理の16時間後に培地が回収され、そしてヒトTRAIL/TNFSF10 Quantikine ELISAキット(R&D systems,DTRL00)を用いてTRAILタンパク質決定の為に使用された。

1.12/統計。

2/結果
2.1/アミノ酸制限に応じて転写制御する異種系の最適化及びイン・ビボ検証
以前の結果に基づいて、EAA可用性によって制御される新しい遺伝子発現が最適化された。以前に報告された通り、Trb3遺伝子、Chop遺伝子及びAtf3遺伝子は、GCN2-eIF2α-ATF4 経路の活性化後に上方制御され、そしてそれらの発現を誘発する為にATF4の特異的なAAREへの補充を意味する(図1)。これらのAAREは、Chop及びAtf3プロモーター内にコア配列の単一コピーとして又はTrb3プロモーター中の3つのコピーの繰り返しとして存在する。
以前の結果に基づいて、EAA可用性によって制御される新しい遺伝子発現が最適化された。以前に報告された通り、Trb3遺伝子、Chop遺伝子及びAtf3遺伝子は、GCN2-eIF2α-ATF4 経路の活性化後に上方制御され、そしてそれらの発現を誘発する為にATF4の特異的なAAREへの補充を意味する(図1)。これらのAAREは、Chop及びAtf3プロモーター内にコア配列の単一コピーとして又はTrb3プロモーター中の3つのコピーの繰り返しとして存在する。
レポーター遺伝子としてルシフェラーゼを用いて、2つの最小プロモーター(チミジンキナーゼ及びβ-グロブリン)と融合されたこれらのAAREの異なる組み合わせが、トランスフェクションされたマウス胎児線維芽細胞(MEF:Mouse Embryonic Fibroblasts)において試験された。最も高い応答性は、TK プロモーターに関連付けられた2つのコピーのTrb3 AARE(コア配列の6反復)で得られ、低いバックグラウンドレベルでロイシン飢餓に応答して6倍の誘導を生じた(図示せず)。β-グロブリンミニマルプロモーターを有する構築物が、TKプロモーターでの場合よりも3倍高い対応するバックグラウンドレベルで、同様の誘導レベルを提供した(図示せず)。従って、全ての更なる実験は、TKミニマルプロモーターに関連付けられた2つのTrb3 AAREで実行された。この構築物は、AARE駆動発現系と命名された(図2)。
最後に、マウス(MEF) 細胞又はヒト(HeLa)細胞において安定して組み込まれている場合、アミノ酸飢餓によるこの構築物の誘導性の維持が確認された(図示せず)。GCN2以外に、eIF2αが哺乳動物組織において下記の3つの他のキナーゼによってリン酸化されることができる:PKR(dsRNA及びサイトカインによって活性化される)、PERK(小胞体ストレスによって活性化される)、又はHRI(ヘム欠乏によって活性化される)。HRIは、赤血球系細胞特異的様式で発現される;それ故に、注意が、偏在的に発現されるキナーゼGCN2、PKR及びPERKの役割に集中された。ロイシン飢餓に応答して、ルシフェラーゼ誘導がGCN2-/-細胞において完全に廃止され、一方PERK-/-細胞及びPKR-/-細胞において効果が観察されず、導入遺伝子調節がGCN2 発現に厳密に依存することを示す(図示せず)。次に、我々は、ロイシンの濃度に関してルシフェラーゼ発現のイン・ビトロ誘導を試験した。レポーター遺伝子発現の誘導は、ロイシン濃度に反比例する(図示せず)。最も通目すべきことに、このイン・ビトロ実験において、転写活性化を引き起こすロイシン濃度は、ロイシン欠乏食の摂取後に哺乳動物において得られる濃度と同様であり、それはまたイン・ビボでも同様に有効でありうる。
有機体が食餌中の遊離アミノ酸を迅速に吸収することを考慮して合成食が調製され、その中でタンパク質画分が遊離アミノ酸の混合物によって置き換えられる。この状態において、該食餌が1つのEAAを欠いているとき、血中の子のEAAのレベルは急速に減少するはずである。この遺伝子調節系の概念を動物に転換する為に、我々は最初に、食後の血中ロイシン含量に対するロイシン欠乏食の影響を試験した。図3は、対照食の摂取後の食後期間中に血中ロイシンレベルが増加したことを示す。これとは際立って対照的に、ロイシン血症は、ロイシンが欠乏した食餌を摂取してから30分後には早くも劇的に減少した。
AARE駆動発現系のイン・ビボ検証は、Chaveroux等(Science signaling,2015年,8(374):rs5)によって以前に設計されたAARE駆動ルシフェラーゼマウスモデルを利用することによってさらに評価された。ロイシンを含まない食餌の摂取は3時間で腹部内の生物発光における著しい増加を結果としてもたらし、且つ少なくとも12時間維持された(図4)。16〜24時間の絶食期間は、導入遺伝子発現に影響を及ぼさなかった(図5)。以前の研究は、GCN2-eIF2α-ATF4経路がロイシンが欠乏した食餌の摂取後にイン・ビボで迅速に活性化されることを示した。例えば、Trb3をコードする肝臓mRNAは、ロイシンを含まない食事の開始後1時間で有意に誘発される。測定可能な生物発光を得る為の遅延は、十分なルシフェラーゼを蓄積する必要があるためである。以前のイン・ビトロの結果から予想される通り、この実験は、AARE駆動発現系がイン・ビボでも同様に機能することを明らかに示す。
2.2/AARE駆動発現系の最適栄養誘導の決定
哺乳動物において、9個のEAAが食餌中に供給されなければならず、それらのいずれか1つが欠乏することは、AARE駆動発現系の潜在的な誘導物質となる。9個のEAAのそれぞれを独立して枯渇させた食餌の効果が、AAREによって駆動されるルシフェラーゼ発現のレベルに関して比較された。第1に、欠乏しているEAAの血中濃度が食餌の摂取後に有意に低下することが確認された。減少の程度は、アミノ酸によって異なる(図示せず)。ルシフェラーゼ活性の誘導率は、特定の食餌に動物を切り替えた6時間後に、イン・ビボでの生物発光画像法によってAARE駆動ルシフェラーゼマウスにおいて測定された。誘導率は、イソロイシン又はトリプトファン飢餓の場合、5〜50を超えて変化した。予想通り、絶食されたマウスにおいて又は非必須アミノ酸、例えばアラニン、が欠乏した食餌を与えあれたマウスにおいて、ルシフェラーゼ誘導は観察されなかった。これらの結果はAARE駆動発現系の順応性を強調し、その中で各個々のEAAが潜在的な誘導物質として働くことができる。

2.3/長期導入遺伝子発現の為の「栄養に基づく」プロトコル
遺伝子治療は、導入遺伝子の長期発現を必要としうる。長期EAA欠乏が生理学的に関連性がないことを考慮して、導入遺伝子の長期発現を維持する能力は、AARE動発現系の順応性を利用することによって試験された。そのために、欠けているEAAのそれぞれを交換するアミノ酸が欠乏した食餌のパルスが行われた。このパラダイムにおいて、AARE駆動ルシフェラーゼ マウスは、図6に記載されている通り、6日間、栄養サイクルに付された。腹部の生物発光を観測することが毎日行われた(図7)。生物発光に定量化は、導入遺伝子発現が食餌チャレンジの24時間後に基底レベル近くに戻ったことを明らかに示した。再誘導は、欠けているAAに依存して、最大レベルで同様の動態に従った。アミノ酸が1つのサイクルから他と異なるという条件で、EAA欠乏食餌の給餌サイクルはタンパク質代謝に影響を及ぼさない。体重、除脂肪体重及び体脂肪量のパーセント並びに筋肉量が、上記の周期的栄養プロトコル(図8に示さされている通り、4栄養サイクル)に24日間付されている動物においてモニターされた。図9〜図12に示されている通り、これらの生物学的パラメータはいずれも対照に対して修正されなかった。

2.4/栄養に基づく調節系は、イン・ビボでの遺伝子導入の観点において有効である。
次に、遺伝子導入実験設定においてAARE駆動発現系を調節する栄養素の能力が、遺伝子治療に関連する様々な組織を標的とすることによって調査された。GCN2-eIF2α-ATF4 経路は偏在的であるけれども、或る器官はEAAを含まない食餌に対して、他のものよりもより敏感である。EAA欠乏食餌を与えられた遺伝子導入マウスからのデータは、この経路が、脳並びに、肝臓及び膵臓を含む多くの代謝器官において機能的であることを示した(図示せず)。遺伝子導入マウスに基づく以前の実験と同様に、動物が一晩の飢餓、引き続きイソロイシン欠乏食を6時間与える栄養プロトコルを与えられた。
次に、遺伝子導入実験設定においてAARE駆動発現系を調節する栄養素の能力が、遺伝子治療に関連する様々な組織を標的とすることによって調査された。GCN2-eIF2α-ATF4 経路は偏在的であるけれども、或る器官はEAAを含まない食餌に対して、他のものよりもより敏感である。EAA欠乏食餌を与えられた遺伝子導入マウスからのデータは、この経路が、脳並びに、肝臓及び膵臓を含む多くの代謝器官において機能的であることを示した(図示せず)。遺伝子導入マウスに基づく以前の実験と同様に、動物が一晩の飢餓、引き続きイソロイシン欠乏食を6時間与える栄養プロトコルを与えられた。
一連の実験において、AAREによって駆動されるルシフェラーゼを担持するプラスミドの最初の流体力学的注入は、Balb/C マウスの尾静脈内で調査され、肝臓トランスフェクションを許した。腹部領域における及び集められた肝臓における生物発光は、対照食餌と比較して6時間の食餌性イソロイシン飢餓に応答して、劇的に誘発された(図13)。肝臓タンパク質抽出物のルシフェラーゼ アッセイは、ルシフェラーゼ活性のこの増加を確認した(図14)。
第2セットの実験において、肝臓組織が、AAREよって駆動されるルシフェラーゼを担持するレンチウィルスベクターで形質導入された。発光の明瞭な誘導が、誘導物質食餌に応答して膵臓において観察され、それはタンパク質抽出物におけるルシフェラーゼ活性を測定することによって確認された(図15及び図16)。マウスの他の部分においてシグナルは検出されなかった。最後に、脳内のAARE駆動発現系の調節を試験する為に、AARE-駆動Luc発現系のレンチウィルス注射が、ラットの海馬、定位注射によって容易にアクセス可能である脳領域において行われた。左海馬は、AARE-Luc配列を含むレンチウィルス粒子を受け取り、一方、右海馬は、AARE配列無しの粒子を受け取った。栄養プロトコルの完了時に、ルシフェラーゼ活性が脳抽出物内において測定された。図17において示されている通り、AARE-駆動 Luc 発現系を有するベクターを含有する左海馬の脳抽出物は、増加したルシフェラーゼ活性を示し、一方、右海馬及び、左海馬の非注射領域は、バックグラウンド活性を発現した。
これらのイン・ビボ研究の状況において、AARE駆動発現系は、ハロフギノンと、プロピル-tRNAシンターゼ18を阻害することによりアミノ酸飢餓を模倣するGCN2の薬理学的 活性化因子との腹腔内注射後に、肝臓において及び膵臓において薬理学的に誘発されることができることが観察された(図示せず)。GCN2の薬理学的活性化は、栄養プロトコルに対する代替案を提供することができる。しかしながら、ハロフギノンの特異的、薬物動態的、及び潜在的な有害作用の慎重な評価が待たれている。
2.5/危険な治療因子の発現を調節する為の「栄養に基づく」プロトコルの適用
幾つかの例において、治療的因子のレベルは、患者の必要性及び/又は望ましく名毒性作用の場合に排除されるその発現に従って厳密に調整される必要がありうる。TRAILが、関心のある遺伝子として選択された。TRAILは、アポトーシスを開始する為に特定のデス受容体で結合することによってパラクリン様式で作用する分泌サイトカインである。TRAILは、特にグリア芽腫細胞において精力的に研究されてきた。それは短い生物学的半減期(30分)を有し、且つ全身投与後に体から急速に除去される。しかしながら、TRAILへの正常なヒト細胞の長期暴露は、依然として有毒でありうる。

その状況において、長期の「栄養に基づく」プロトコルは、TRAIL発現の調節に対する適切な解決策を表しうる。ヒトGli36-ルシフェラーゼ細胞は、グリア芽腫細胞の為のモデルとして使用された。恒常的にルシフェラーゼを発現するこれらの細胞は、AARE-駆動TRAIL発現系で形質導入された。第1に、TRAIL タンパク質発現は、ロイシン飢餓に応答して、培地中に分泌され、周囲の細胞にパラクリン効果を及ぼし、そしてアポトーシスを誘発することが確かめられた(図示せず)。
最初に、我々は、ヌードマウスにおけるGli36-ルシフェラーゼ グリア芽腫異種移植片に対するTRAIL発現の効果を試験した。レンチウィルスベクターは腫瘍内に直接的に注射され(細胞移植後1週間)、そして上記の長期「栄養に基づく」プロトコルが2週間適用された。腫瘍展開は、生物発光画像法によって評価された。注目すべきことには、TRAIL発現は、総レベルでは明らかな毒性無しに腫瘍成長を妨げた(-EAA群)が、対照食餌を与えられた同様の動物は阻害を示さなかった(Ctrl群)(図18及び図19)。腫瘍からのタンパク質の医務のブロット分析は、-EAA群からのマウスのみがアポトーシスマーカー切断されたPARPの上昇を示し、アポトーシス過程がこれらの細胞内で生じ且つ対応する対照において生じなかったことを示す(図20及び図21)。
重要なことに、TRAILタンパク質は、-Ile食餌期間の6時間後に、-EAA群からの腫瘍においてのみ検出された。予想された通り、栄養プロトコルそれ自体は、腫瘍成長に影響を及ぼさなかった(図示せず)。まとめると、これらの知見は、導入遺伝子発現のパルスを生成する為のAARE駆動発現系の能力を検証し、それによって長期/断続的な遺伝子治療処置を許す。
3/議論

この栄養に基づく調節系の展開は、アミノ酸欠乏を感知する適応的GCN2-eIF2α-ATF4シグナル伝達経路を利用する。酵母から人まで保存されているこの経路は、雑食動物が1つのEAAにおいて制限された食事(単一の植物タンパク質源のみが利用可能である場合に野生で頻発に発生する可能性がある状況)に直面した場合に、引き起こされる。
GCN2-eIF2α-ATF4経路の活性化は専らEAA不均衡食事の消費に応答して生じ、そして他のいかなるヒトの栄養状態にも関与していない。生理学的条件、例えば空腹時、は、それがGCN2 経路を誘発するような程度までアミノ酸血中濃度に影響を及ぼさない。長期間の絶食に応答して、全てのアミノ酸の血中濃度は、肝臓、そして次に筋肉中の増加したタンパク質分解を含む補償機構を通じで維持されるであろう。

誘導物質食事は容易に吸収される遊離アミノ酸で構成されている故に、栄養に基づく調節系は所望の遺伝子の発現を正確に調整する為に頑強で且つ柔軟な手段を提供し、それによって、急速なEAA血流速度論及び治療的導入遺伝子の誘発をもたらす。所与の導入遺伝子の発現のレベルは、食事において欠けている特定のEAAに応じて、依存するであろう。午前中に患者において摂取された遊離アミノ酸からなる食事は、患者にとってチャレンジであるとは予想されない。すぐに食べられる食事パッケージを使用することは、補助食品を摂取することに似ているだろう。この臨床状況において、市場で入手可能であり且つメープルシロップ尿症 (MSUD:Maple syrup urine disease)において確認されているロイシンを含まない食事の医学的処方が、AARE系と併せて容易に使用されることができることはまた特に注目に値する。さらに、患者の個々のニーズ及び嗜好に合わせた新規な処方が開発されることができる。誘導期間後、患者は数時間後に通常の食事を再開することができた。必要に応じて、導入遺伝子の持続的な発現は、毎日又は数日ごとに、食事を交互にすることによって得られることができる。
導入遺伝子発現の適時の外因性調節は、遺伝子弛緩/手術とは対照的に薬理学的遺伝子治療法の状況において特に適していると思われる。後者の戦略は、真正の機能的対応物による欠陥遺伝子の置換を許し、それによって外因性調節の必要性を排除する。対照的に、慣用的な薬理学的処置と同様である薬理学的遺伝子治療法は、導入遺伝子発現の長期/断続的な調節を必要としうる。適切な状況は、神経変性疾患の状況においてかなりの注目を集めているトロピック(tropic)因子によって表される。特に、グリア細胞株由来神経栄養因子(GDNF:glial cell line-derived neurotrophic factor)は、成人の脳におけるドーパミン作動性ニューロンの生存に必須であることが示されており、及び遺伝子治療アプローチがパーキンソン病について評価されている。
所与の調節系のダイナミクスは、遺伝子導入を通じて与えられた所与の薬の薬理学的特定に取り組む上で重要な特徴である。臨床的に適切である為には、遺伝子調節系は、安全な用量のベクター内で、広い用量範囲の誘導物質に亘って調節されるべきであり、且つ低レベルのバックグラウンド発現を示す。これに関して、結果のセクションに記載されたアポトーシス促進TRAILサイトカインの研究は、導入遺伝子発現と結果として生じた生理学的/臨床的効果との間の直接的且つ定量的な比較を提供した。
具体的には、誘発された腫瘍において発現されたTRAILのレベルは、食事性イソロイシンに非存在下では、対照的条件下で得られたかすかなシグナルよりも100倍強いと推定された。注目すべきことには、TRAIL発現は、肉眼的レベルで明らかな毒性無しに腫瘍成長を阻止したが、対照食餌を与えられた動物は腫瘍阻害を示さなかった。栄養プロトコルに関連付けられたそのような高い誘導比は、導入遺伝子発現レベルを最適な治療レベルに適応させること、及び必要とされる場合にバックグラウンド活性を無効にすることにおいて大きな柔軟性を提供する。従って、臨床現場において、(i)治療ベクターの投与量、(ii)治療導入遺伝子を駆動するミニマルプロモーターの強度、(iii)対応するRNAの(不)安定化、(iv)全ての有効性及び安全性の懸念を満たすであろうEEAが欠乏した食事の選択の取り扱いを通じて、効率的且つ安全な用量の治療用分子を生成する立場にあるべきである。
AARE系は、特定の状況において、eIF2α-ATF4シグナル経路がERストレス応答の3つの分岐のうちの1つの一部であるという事実を考慮すると、内因的に制御された系として作用しうる。従って、疾病組織内に厳密にベクターを送達した後、AARE依存性転写は、アルツハイマー病又はパーキンソン病を患っている患者の或る腫瘍又は或る脳領域などの状況において内部的に活性化されることができる。これらの例において、治療遺伝子の発現は、EAA欠乏食餌に頼ること無しに、内因的に引き起こされる可能性がある。代替的に、治療因子がパラクリン効果を有する分泌タンパク質である場合に、治療効果は、近隣の健全な組織内へのベクターの注射、そしてそれに続く食事の誘導によって得られうる。
最後に、栄養に基づく調節系は、ベクターバックボーンの状況において、例えば多くの種類の細胞を効率的に形質導入し且つ遺伝子治療試験において現在使用される最も有望なウィルスベクターのうちの1つであるレンチウィルスベクターの状況において、有効であることが示された。この発見は、他のベクターがAARE系を発現する能力について他のベクターを試験することを要求し、それはさらに、栄養を調節された系の柔軟性及び可能性を例示しうる。明らかに、AARE系は、遺伝子治療の分野における新しい概念を強調し、合成の食事は治療用導入遺伝子発現の厳密で且つ頑健な時間的制御を可能にすることができ、それによって遺伝子治療プロトコルの臨床実体への転換における頻繁なハードルをロック解除する。

Claims (16)
- 個体におけるアポトーシス促進タンパク質をコードする核酸の制御された発現の為の核酸であって、
ミニマルプロモーターと少なくとも1つのAARE(アミノ酸応答要素)核酸とを含む調節ポリヌクレオチド、ここで、上記調節ポリヌクレオチドは、少なくとも1つの必須アミノ酸が不足している食事の摂取に応じて個体において活性化される、及び
上記調節ポリヌクレオチドの制御下に置かれる、アポトーシス促進タンパク質をコードする核酸、
を含む、上記核酸。 - 上記アミノ酸応答要素(AARE)核酸が、配列SEQ ID No:1、SEQ ID No:2、SEQ ID No:3、SEQ ID No:4及びSEQ ID No:5の核酸を含む群において選択される、請求項1に記載の核酸。
- 上記調節ポリヌクレオチドが少なくとも2つのAARE核酸を含む、請求項1又は2に記載の核酸。
- 上記アポトーシス促進タンパク質がTRAILである、請求項1〜3のいずれか1項に記載の核酸。
- アポトーシス促進タンパク質をコードする核酸の制御された発現の為の核酸ベクターであって、請求項1〜4のいずれか1項に記載の核酸を含む、上記核酸ベクター。
- 請求項1〜4のいずれか1項に記載の核酸又は請求項5に記載の核酸ベクターを含む送達粒子。
- 標的とされた細胞の膜において暴露された標的受容体への結合の為に適した1以上のリガンドをその表面に含む、請求項6に記載の送達粒子。
- (i)請求項1〜4のいずれか1項に記載の核酸、又は請求項5に記載の核酸ベクター、又は請求項6若しくは7に記載の送達粒子と、(ii)医薬的に許容される添加剤とを含む医薬組成物。
- 請求項1〜4のいずれか1項に記載の核酸又は請求項5に記載の核酸ベクターを含む宿主細胞。
- 医薬品としての使用の為の、請求項1〜4のいずれか1項に記載の核酸。
- アポトーシスを少なくとも1つの標的細胞内に誘発する為の活性剤としての使用の為の、請求項1〜4のいずれか1項に記載の核酸。
- 上記標的細胞が腫瘍細胞である、請求項11に記載の使用の為の核酸。
- 腫瘍を処置及び/又は阻止する為の活性剤としての使用の為の、請求項1〜4のいずれか1項に記載の核酸。
