Abstract
Description
本発明は、B型肝炎ウイルス(HBV)に対する処置又はワクチン接種のための改良された組換えワクチン接種用ベクター、並びに、該組換えを含む医薬組成物又はワクチンに関する。本発明はまた、HBVに対するワクチン接種法に使用するための組換えワクチン接種用ベクター、並びに、HBVに対するワクチン接種のためのキットに関する。
2億4千万人以上の人々が、慢性B型肝炎ウイルス(HBV)感染に罹患している。認可された処置選択肢は、ヌクレオシ(チ)ド類似体及びインターフェロンαである。ヌクレオシ(チ)ド類似体は、B型肝炎を制御するが治癒せず、高額で長期の処置を必要とし、これには耐性ウイルスの出現を伴う可能性がある。インターフェロンα療法は副作用によって制限され、患者の15〜20%しか治癒されない。
本発明は、B型肝炎ウイルス(HBV)に対する処置又はワクチン接種のための新規な組換えワクチン接種用ベクター、並びに、該MVAを含む医薬組成物又はワクチンに関する。本発明はまた、組換えMVAを使用したHBVに対するワクチン接種法にも関する。本発明の手段及び方法は、被験者においてHBVに対する抗体応答並びにT細胞応答の両方を誘発すると考えられ、誘発された免疫応答は、多種多様なHBV遺伝子型及び血清型に対して有効であることがさらに考えられる。
マウス及びワクチン接種
C57BL/6野生型(wt)及びHBVトランスジェニックマウス(F. Chisari、The Scripps Institute、ラホヤ、CA州、米国によって親切にも提供された、HBV1.3.32株(Guidotti et al., J Virol 1995; 69:6158-69)(HBV遺伝子型D、亜型ayw))は、施設の指針に従って特殊な病原体フリーの条件下、社内の飼育から得られた。タンパク質によるワクチン接種のために、マウスに、50μlのPBS中に31.91μgの合成ホスホロチオエート化CpG ODN 1668及び/又は25若しくは50μgのポリ[ジ(エチルフェノキシカルボン酸ナトリウム)]ホスファゼン](PCEP)と混合された、組換え酵母HBsAg又はE.coli HBcAg(APP Latvijas BiomedicT nas、リガ、ラトビア)を用いて皮下に免疫化した。MVAによるワクチン接種のために、マウスに、500μlのPBS中1×108感染単位のそれぞれの組換えMVAを用いて腹腔内にワクチン接種した。
脾臓細胞及び肝臓に関連したリンパ球(LAL)を以前に記載(Stross et al., Hepatology 2012; 56:873-83)されているように単離し、H2−kb拘束性ペプチド又はH−2Db拘束性ペプチド(図10)(jpt Peptide Technologies、ベルリン、ドイツ)又は組換えHBsAg(親切にもRheinbiotech-Dynavax、デュッセルドルフ、ドイツによって提供された)を用いて1mg/mlのブレフェルジンA(シグマアルドリッチ社、タウフキルヘン、ドイツ)の存在下で5時間刺激した。細胞を、臭化エチジウム・モノアジド(インビトロジェン社、カールスルーエ、ドイツ)を用いて生死について染色し、抗CD16/CD32−Fcブロック(BDバイオサイエンシーズ社、ハイデルベルク、ドイツ)を用いて遮断した。表面マーカーを、PBにコンジュゲートさせた抗CD8a抗体及びPEにコンジュゲートさせた抗CD4抗体(eBiosciences社、エヒング、ドイツ)を用いて染色した。細胞内サイトカイン染色(ICS)を、FITC抗IFNγ抗体(XMG1.2)、PE−Cy7抗TNFα抗体、及びAPC抗IL2抗体(eBiosciences社、エヒング、ドイツ)を用いてCytofix/Cytopermキット(BDバイオサイエンシーズ社、ハイデルベルク、ドイツ)を製造業者の推奨に従って使用して実施した。
HBsAg、HBeAg、抗HBs抗体、及び抗HBc抗体の血清中レベルを、1:20の希釈率でAXSYM(商標)アッセイ(アボットラボラトリーズ社、アボットパーク、IL州、米国)を使用して決定した。リアルタイムのポリメラーゼ連鎖反応による血清中HBV力価の定量は、以前に記載(Untergasser et al., Hepatology 2006; 43:539-47)の通りに実施された。
記載(Lucifora et al., J Hepatol 2011; 55:996-1003)のように分化及び培養したHepaRG細胞を、ワクチン接種を受けたマウスの血清の連続希釈液(1:33、1:100、1:333、及び1:1000)の存在下、200個のDNAを含有しエンベロープを有するHBV粒子/細胞(亜型ayw)に2回感染させた。陽性対照として、0.8の国際単位のHepatect(商標)CP抗体(バイオテストファーマGmbH社、ドライアイヒ、ドイツ)を使用した。感染から24時間後、細胞をPBSで3回洗浄し、分化培地 1mlを加えた。血清を感染から4日後、7日後、及び10日後に回収し、HBsAgを1:20の希釈率でイムノアッセイによって検出した。
統計分析は、プリズム5ソフトウェア(グラフパッド社、サンディエゴ、米国)を使用して実施された。結果は、平均値±平均の標準誤差として表現される。グループ間の差異が、両側スチューデントt検定を使用して統計学的有意性について分析された。
組換えMVAを、以前に記載(Staib et al., Biotechniques 2003; 34:694-6, 698, 700)のように相同的組換え及び宿主範囲の選択によって作製した。完全HBcAg(遺伝子型D、亜型ayw)及びHBsAgのオープンリーディングフレーム(遺伝子型D、亜型ayw又はadw)を、MVA導入用プラスミドpIIIΔHR−PH5又はpIIIΔHR−P7.5にクローニングし、これによりHBVタンパク質を初期/後期ワクシニアウイルス特異的プロモーターPH5(HBcAg ayw/HBsAg ayw/HBsAg adw)又はP7.5(HBsAg ayw)の制御下に配置した。各ウイルスの構築後、遺伝子の発現、挿入されたDNAの配列、及びウイルス純度を確認した。ワクチン製剤の作製のために、MVAをCEF中で慣用的に増殖させ、ショ糖を介した超遠心分離によって精製し、1mMのトリス−HCl(pH9.0)を用いて再構成し、標準的な方法(Staib et al., Methods Mol Biol 2004; 269:77-100)に従って滴定した。
NIH−3T3マウス線維芽細胞(CRL−1658)を、10%FCS、100U/mlのペニシリン及び100μg/mlのストレプトマイシンの補充された、RPMI 1640培地中で培養した。細胞を、感染から16時間後に溶解緩衝液(50mMのトリス−HCl[pH8.0]、150mM NaCl、1%ノニデットP−40、0.02%のNaN3、及び100μg/mlのフェニルメチルスルホニルフルオライド)中に収集し、12%ドデシル硫酸ナトリウム−ポリアクリルアミドゲル電気泳動(SDS−PAGE)上で分離し、ニトロセルロース膜(0.45μM;バイオラッド社、ミュンヘン、ドイツ)上にブロットした。膜を抗HBc抗体(抗血清H800;親切にもH. Schallerによって提供された)、抗HBs抗体(Murex HBsAgバージョン3;アボット社、アボットパーク、IL州、米国)又は抗アクチン抗体(シグマ社、ミュンヘン、ドイツ)をそれぞれ1:10000、1:50、及び1:10000希釈率で用いて4℃でインキュベートした。セイヨウワサビペルオキシダーゼにより標識された二次マウス抗体及びウサギ抗体(ディアノバ社、ハンブルク、ドイツ)を1:5000の希釈率で21℃で1時間使用した。抗体を、5%脱脂粉乳を含有しているリン酸緩衝食塩水で希釈した。増強化学発光を指示されている通りに(ロシュ社、マンハイム、ドイツ)使用した。
HBV特異的免疫応答の誘導に対するHBV抗原負荷の影響を調べるために、HBV1.3.32トランスジェニック(HBVtg)マウスを、ワクチン接種前にそれらの血清中HBeAgレベルに従って低度の抗原血症、中程度の抗原血症、及び高度の抗原血症のグループに分類した(図10)。ブーストから6日後、HBsAg/MVA−Sの投与を受けたマウスにおける抗HBs抗体力価は、中程度の抗原血症グループのマウスと比較して、低度の抗原血症グループにおいてより高く、高度の抗原血症マウスにおいては依然として検出不可能であった(図2A)。MVA−Sによるブースト後から35日間モニタリングした場合でさえも、高度の抗原血症マウスは、検出可能な抗HBs抗体力価を発生せず、HBsAgは低いレベルで持続した(図11)。抗HBs抗体が過剰量のHBsAgと複合体を形成し、よって検出を回避し得るかどうかを研究するために、本発明者らは、沈殿した131個のタンパク質複合体を尿素を用いて溶かし、イムノアッセイを繰り返した。これにより、本発明者らは、ワクチン接種を受けた高度の及び中程度の抗原血症HBVtgマウスにおいてHBsAg−抗HBs抗体の免疫複合体を見い出したが、低度の抗原血症のHBVtgマウスにおいては見出さなかった(図14)。しかしながら、HBcAg特異的抗体は、HBcAg/MVA−コアのワクチン接種を受けた全てのグループのマウスの血清中に検出されたが、力価は再び、抗原血症とは逆相関する傾向を示した(図2B)。ワクチン接種により誘導されたCD4+T細胞応答を分析するために、本発明者らはHBsAgを用いて脾臓細胞及び肝臓に関連したリンパ球(LAL)を刺激した。本発明者らは、wtマウスにおいてHBsAg特異的CD4+T細胞の高い出現頻度を見い出したが、本発明者らはどのグループのHBVtgマウスにおいてもHBV特異的CD4+T細胞を検出しなかった(図2C、D)。
CD8+T細胞の重要なエフェクター機能は、IFNγに加えてIL−2及びTNFαの産生、並びに、ペプチド刺激に応答して脱顆粒する能力(これはCD107aの表面発現によって分析され得る)を含む。S190ペプチドによる刺激時に、HBsAg/MVA−Sを用いての免疫化によりwtマウス及び低度の抗原血症のHBVtgマウスにおいて誘導された、IFNγ+S190特異的脾臓CD8+T細胞は、同じような比で脱顆粒し(それぞれ72.3%及び73.4%)(図4A)、中程度の抗原血症マウスに由来する脾臓及び肝臓のCD8+T細胞においてIL−2及びTNFαの同等な発現を示し、これは、ペプチドによる刺激時に程度は低いが脱顆粒することもでき(62%)、しかしIL−2の発現は欠失していた(図4B)。
ポリホスファゼンアジュバントPCEPが、CpGの免疫刺激作用を増強するであろうことを調べるために、PCEPをCpGの代わりに使用したか、又はタンパク質ワクチン製剤及び組み合わせたHBsAgとHBcAgのためにCpGに加えた。
次に、体液性免疫応答及び細胞性免疫応答に対する様々なアジュバントの効果を決定することを目的とした。それ故、野生型CH57Bl/6マウスを、様々なアジュバント製剤中で複合体を形成した粒子状のHBcAg及びHBsAgを用いてプライムした。グループ1〜3において(n=3)、HBsAgを水酸化アルミニウムと複合体を形成させ、HBcAg、CpG又はポリホスファゼンアジュバント又はその両方と合わせた。グループ4〜6(n=3)を、CpG若しくはポリホスファゼンのいずれか又はその両方を用いるがアルミニウムは全く用いずに免疫賦活された粒子状HBcAg及びHBsAgを使用してワクチン接種した。28日目に、全ての動物を、亜型adwのコア及びSを発現するMVAを用いてブーストした。
マウス血清を、ワクチン接種後にその中和能について分析した。HBsAg亜型adwを用いてワクチン接種されたマウスは、最大1:1000という高い希釈率でさえもHBV亜型aywを交差中和することができた(図15E)。
次に、多重抗原性MVAを使用して1つ、2つ、及びいくつかのHBV抗原に対する免疫応答の誘導の比較を目指した。意外なことに、S又はSとPolのいずれかに対する体液性免疫応答並びに細胞性免疫応答は、コアがMVAワクチンベクターによって共発現された場合に改善された。
高い力価のHBV抗原を発現するHBVトランスジェニックマウスのHBVxfs株を、全てのHBV RNAの3'領域を標的化しているHBV特異的siRNAを用いて処置した。siRNAによる処置により、HBeAg及びHBsAgのレベルは90%減少した。8週間後、動物を、タンパク質によるプライム−MVA-HBVによるブーストのワクチンを用いてワクチン接種することにより、抗HBs抗体及びHBV特異的T細胞を誘導した(図27)。タンパク質ワクチンとして、粒子状HBsAg及びHBcAgを、CpG及びPCEPを用いて免疫賦活した。MVAワクチンベクターは、HBV S及びコアタンパク質の完全なオープンリーディングフレームを発現した。対照は、siRNAなし、ワクチン接種なし、及びその組合せであった。
最初のセットの実験において、本発明者らは、HBVtgマウスにおいて満足できる免疫原性を示しかつ免疫寛容を破壊することができるであろう、異種のタンパク質によるプライム/MVAによるブーストのワクチン接種のための最低のMVA用量を評価することを目指した。それ故、低度及び中程度の抗原血症HBVtgマウスのグループを、ビス−(3’,5’)−サイクリック二量体アデノシン一リン酸(c−di−AMP)を用いて免疫賦活された15μgの粒子状HBcAgを用いて2週間間隔で2回免疫化した。28日目に、マウスを、4回の異なる用量のMVA−コア(それぞれ、3×106、1×107、3×107、1×108PFU)を用いてブーストした(図29A)。様々なMVA用量を用いた免疫化処方計画によって惹起された体液性免疫応答及び細胞性免疫応答を、ブースト免疫化から7日後(35日目)に評価した。
平衡のとれたTh1/Th2 CD4+T細胞応答を誘導する安全かつ効果的なアジュバントがないことは、臨床試験を開始する際の障害であり得る。さらに、新規に同定された細胞質内のパターン認識受容体STINGのトリガーは、治療的ワクチン接種のための興味深い代替選択肢である。それ故、本発明者らは、治療的B型肝炎ワクチンのための新規アジュバント候補としてのc−di−AMPの有効性を研究することを目指した。この目的のために、中程度及び高度の抗原血症HBVtgマウスのグループ(n=7)のマウスは、2回のタンパク質によるプライム及びMVAによるブースト免疫化を受けた。タンパク質によるプライミングのための粒子状HBsAgとHBcAgを合わせ、c−di−AMP又は以前に確立されたCpGとポリホスファゼン(PCEP)の組合せを用いて免疫賦活した。28日目に、マウスを、MVA−S/コアの混合物を用いてブーストした(図30A)。c−di−AMPの注射及び「空の」MVA(MVAwt)の投与を受けたHBVtgマウス(n=4)を対照として使用した。体液性免疫応答及び細胞性免疫応答を誘導するワクチン製剤の有効性を34日目(ブーストから6日後)に比較した。
治療的な異種のタンパク質によるプライム/MVAによるブーストのワクチン接種におけるタンパク質によるプライミングのための適切なアジュバントのために、スクリーニングを拡大した。本発明者らの目的は、治療用B型肝炎ワクチンのための2つの新規アジュバント候補(ポリ−LCIC及びRIG−Iリガンド)の有効性とc−di−AMPの有効性を比較することであった。さらに、本発明者らは、最も効果的な適用経路を発見するために様々な免疫化プロトコールを研究した。この目的のために、低度及び中程度の抗原血症HBVtgマウスのグループ(n=5)が、2回のタンパク質によるプライム及びMVAによるブーストの免疫化を受けた(図30A)。タンパク質によるプライミングのための粒子状HBsAg及びHBcAgを組合せ、c−di−AMP、ポリ−LCIC又はRIG−Iリガンドを用いて免疫賦活した。28日目に、マウスを、MVA−sAgとMVA−コアの混合物を用いてブーストした。免疫化は、専ら筋肉内(i.m.)経路によって実施されたか、又はタンパク質によるプライミングは皮下(s.c.)に投与され、その後、腹腔内(i.p.)ブーストが投与された。様々なワクチン製剤の有効性及び適用経路を、34日目(ブーストから6日後)における体液性免疫応答及び細胞性免疫応答の誘導に関して比較した。
さらに、HBsAg、HBcAg(主なHBVの遺伝子型であるA、B、C、Dを網羅する配列)、及びHBVポリメラーゼのRTドメインを発現する新規に構築されたポリシストロン性MVA(MVA−HBVvac)のインビボでの免疫原性を評価した。2つのポリシストロン性ワクチン接種用構築物の図解が作成された:全ての主要なHBV遺伝子型のHBVのコア及びS並びにHBVポリメラーゼのRTドメインを網羅しているHBVVac、並びに、HBVのコア及びSを発現するC/S(C/S)(図32A)。タンパク質の発現はウェスタンブロットによって確認された。図32Bは、HBVVAc又はS/Cのいずれかを発現する様々な組換えMVAクローンによるSの発現を示す。
MVA系ワクチンベクターのインビボでの免疫原性を向上させるために、CD70を二シストロン様に発現するMVAベクターを構築した(図34A)。

- 投与が非経口経路又は粘膜経路による、請求項15〜28のいずれか一項に記載の使用のためのMVAウイルス。
- 投与が筋肉内であり、工程(i)及び/又は工程(i’)がアジュバントの投与を含み、ここでの該アジュバントはサイクリックdi−AMPを含む、請求項29記載の使用のためのMVAウイルス。
- 投与が皮下又は筋肉内であり、工程(i)及び/又は(i’)がアジュバントの投与を含み、ここでの該アジュバントはポリI/C又はRIG−Iリガンドを含む、請求項29記載の使用のためのMVAウイルス。
- 請求項1〜14のいずれか一項に記載の組換えワクチン接種用ベクター又はMVAウイルスを含む、ワクチン又は医薬組成物。
- 組換えワクチン接種用ベクターが、請求項9及び11〜14のいずれか一項に記載のMVAウイルス又は請求項10記載のサルモネラ株である、請求項32記載のワクチン。
- ワクチンが非経口用又は粘膜用ワクチンである、請求項32又は33記載のワクチン。
- (i)
(a)B型肝炎ウイルス遺伝子型A由来のエンベロープタンパク質(ここでの該エンベロープタンパク質は好ましくはB型肝炎ウイルス遺伝子型A血清型adw由来の小型又は大型のエンベロープタンパク質であり、ここでの該小型又は大型のエンベロープタンパク質は好ましくは小型のエンベロープタンパク質である);及び/又は
(b)B型肝炎ウイルス遺伝子型D由来のコアタンパク質(HBc抗原)(ここでの該コアタンパク質は好ましくはB型肝炎ウイルス遺伝子型D血清型aywに由来する)
を含むタンパク質組成物;
(ii)請求項32〜34のいずれか一項に記載のワクチン
を含むキット。 - タンパク質組成物が非経口投与に適し、該組成物は好ましくはPCEP及び/又はCpGアジュバントである少なくとも1つのアジュバントを含む、請求項35記載のキット。
- タンパク質組成物が粘膜投与に適し、ここでの該組成物は好ましくは、CTA1DD、dmLT、PCEP、ポリI/C、RIG−Iリガンド、c−di−AMP、c−di−CMP、及びc−diGMP又はその組合せからなる群より選択されるアジュバントを含む、請求項35記載のキット。
