Abstract
Description
嗅覚神経線維は、深い上部の狭い鼻通路の限られた部分を支配し、一方、三叉神経は、鼻の深い上部及び後部に感覚的及び副交感神経的な神経刺激を提供する。これらの脳神経線維に沿った薬物輸送は、血液脳関門(BBB)を迂回して中枢神経系(CNS)(参照文献15、23)への潜在的な直接通路を提供する可能性があり、この部分は、従来の鼻スプレー装置では適切に標的とすることができない(参照文献15、26)。
静脈内(IV)投与が社会の行動や認知に影響を及ぼす可能性がある(それはおそらく、血液吸収と、その後のBBBへの作用を経て)ことを示す早期研究にもかかわらず、OTが認知機能に及ぼす影響を評価するその後のヒト研究では、鼻腔を介してOTを投与する方法を用いてきた。鼻腔内供給がOTを投与する一層適切な手段であるという強い理論的根拠はあるが、鼻腔内供給は、鼻腔内投与(すなわち鼻から脳へ)及び静脈内投与(すなわちBBBを通した輸送)の薬力学(PD)効果の制御された比較は行われていない。
図1(a)〜図1(c)は、本発明の一実施形態による手動作動式経鼻供給装置を示す。供給装置は、ハウジング115と、被験体の鼻腔にフィットさせるためのノーズピース117と、使用中の被験体が息を吐き出し、それにより、例えばマウスピース119を介した被験体による息の吐き出しの際に、被験体の鼻気道へと入り鼻気道を通る空気流の供給を可能とするためのマウスピース119と、被験体の鼻腔に物質を供給するように手動で作動可能な供給ユニット120とを含む。
図1(c)に特に示されているように、ボディ要素128は、ピボット135と、スライド面137とを備えている。ピボット135は、この実施の形態では、バルブシート129の一方の側である下側に設けられていて、その下側にバルブ要素131の一端145が旋回する。スライド面137は、この実施形態では、バルブシート129の他方の側である上側に配置されていて、その上側に向けてバルブ要素131の他端147が摺動可能とされている。
バルブ要素131は、細長いアーム141と、アーム141により支持されるバルブ部材149とを備えている。細長いアーム141は、この実施形態では可撓性アームであり、その一端145(この実施形態では下端)はボディ要素128のピボット135を軸に旋回し、その他端(即ち上端)はボディ要素128のスライド面137に摺動可能に係合する。
この実施形態では、アーム141は、第1の(ここでは下方の)アーム部151を含み、アーム部151は、ここでは内側に偏っており、これによりバルブ要素131が閉鎖休止位置にあるとき、下方アーム部151は、ハウジング115の長手方向の軸に対し内側に傾き、以下に詳細に説明するように、バルブ要素131を開位置に動かすために手動で作動されたときに物質供給ユニット169と係合可能とされる。
この実施形態では、バルブ部材149は、シール161と、支持体163を備える。このシール161は可撓性又は弾性を有する素子であり、バルブ要素131が閉位置にあるときにバルブシート129によって規定されるようにバルブ開口130を閉鎖するように作用する。支持体163はシール161の中央領域を支持している。
この実施形態では、出口ユニット167は、被験体の鼻気道に物質を供給するためのノズル171を備える。この実施形態では、ノズル171は、エアロゾルスプレーを提供するように構成される。代替的な実施形態では、液体の供給のために、ノズル171を、液体ジェットを液体の列として供給するように構成することができる。好ましい実施形態では、出口ユニット167の遠位端は、被験体の鼻腔に少なくとも約2cm、好ましくは少なくとも約3cm、より好ましくは約2cm〜約3cm伸びるように構成される。
物質を含み、物質供給ユニット169の作動部分としてハウジング115内の開口123から延びる物質収容チャンバ173と、ここでは物質含有チャンバ173の押し込みによって、典型的には被験体の指又は親指による押し込みによって作動可能な機械式供給ポンプ175を備える。これにより、物質含有チャンバ173から出口ユニット167へ、及びノズル171から、測定された量の物質をここではエアロゾルスプレーとして供給する。
図2(a)〜図2(e)及び図3(a)〜図3(e)は、説明した実施形態のノーズピース117を示す。
図2(e)に特に示すように、ノーズピース117は、2つのボディ部202、204(ここでは第1の内側ボディ部202、第2の外側ボディ部204)を有する。第1の内側ボディ部202は、ここではプラスチック材料である。第2の外側ボディ部204は、ここではゴム又はエラストマー材料のような、より軟質で弾力性のある材料であり、内側ボディ部202の遠位端の周りに配置され、先端要素206を画定する。
この実施形態では、外側ボディ部204は、ショアA高度42を有する熱可塑性エラストマー(TPE)、ここではVersaflex(RTM)OM1040X-1(GLS / PolyOne Corporationによって供給される)から形成される。
図3(a)〜図3(e)に特に示されているように、この実施形態では、内側ボディ部202は、そこを通る流路209を規定するベース部208と、その遠位の、最も前方の端部にあり、ノーズピース117の先端106を支持する突出部212を備える。
この実施形態では、ベース部208の遠位の最前端は、ノーズピース117の長手方向軸に対してテーパ状になっているか傾斜している表面210を画定する。これにより、ベース部208の表面210は、突出部212の遠位端から離れる方向に傾斜し、ベース部208は、突出部212の先端、ベース部208は、突出部212と対向する側の方が短くなっている。
突出部112は、サジタル方向の剛性を付与するように構成され、この構成は、ノーズピース117の先端206が鼻弁の上部領域において肉質組織を開放させることを可能とするとともに、鼻弁の解放領域を拡大し、横方向に柔軟性を与えて、ノーズピース117の先端206を鼻弁に挿入することを容易にする。この実施形態では、音響比測定(AR)による測定によると、ノーズピース117は、拡張されていない状態及び休止状態における鼻弁の領域の少なくとも2倍の領域への鼻弁の領域の拡張を提供する。
この実施形態では、突出部212は、サジタル方向の長さd1が横方向の長さd2よりも大きいブレードの形態を有する。この実施形態では、サジタル方向の長さd1は、横方向の平均長さd2の1.5倍である。一実施形態では、サジタル方向の長さd1は、横方向の平均長さd2の1.7倍である。この実施形態では、サジタル方向の長さd1は、横方向の平均長さd2の1.9倍である。この実施形態では、サジタル方向の長さd1は、横方向の平均長さd2の2倍である。この実施形態では、突出部212は、約2mmのサジタル方向の長さd1を有する。この実施形態では、突出部212は、約1mmの横方向の長さd2を有する。この実施形態では、突出部212は、ボディ部214と、ボディ部214の長さd1よりもサジタル方向に短い長さd3を有する先端部分216とを有し、ここではその内側エッジに段差を画定する。
この実施形態では、横方向の長さd2は、突出部212の近位端から遠位端まで約1.1mmから約0.8mmに減少する。
無作為化され、二重盲検、二重ダミーの、クロスオーバー研究を実施し、18人の健康な男性成人を無作為に割り当て、16人が4回の単一投与を完了した。
これらは、
(1)図1(a)〜図1(c)の装置を用いて供給された8IUのOTの液体スプレーの鼻腔内投与(以下、8IU−OT)、
(2)図1(a)〜図1(c)の装置を用いて供給された24IUのOTの液体スプレーの鼻腔内投与(以下24IU−OT)、
(3)1IUのOTの静脈内供給(以下、IV)、及び
(4)図1(a)〜図1(c)の装置を用いて供給された偽薬(placebo)の液体スプレーの鼻腔内投与(以下、偽薬)
である。
この研究は、感情刺激、特に扁桃体活動の提示によって指標付けられた、社会認知及び行動についてのOTの薬力学(PD)効果を比較したものである。
OTの行動及び認知効果の神経相関を調べるために、研究者らは、機能的磁気共鳴イメージング(fMRI)のような脳撮像ツールを採用した。
認知資源配分と瞳孔拡張(参照文献119、120)との間に確立された関係が与えられると、瞳孔測定は感情刺激に対する関与の非侵襲的な生物学的測定を提供する。研究は、鼻腔内のOTが、瞳孔拡張を増強すること、及び社会的手がかりの特徴を強調することを示している(参照文献121)。しかし、扁桃体活動と瞳孔に指数化された認識の関与との関係はまだ探究されておらず、OTの効果をよりよく理解するのに役立つかもしれない。
主な成果は、特に扁桃体活動に関連した顔の感情表現の評価であり、副次的な成果には、薬物動態(PK)の分析結果及び信頼性レーティングが含まれていた。
この研究は、8IU−OT及び24IU−OTの投与が怒りの知覚強度に及ぼす主な影響について仮説を立て、この効果は、曖昧な感情的な刺激では、曖昧さの少ない感情的な表現での刺激に比べて顕著になる。
この研究は、8IU−OT及び24IU−OTの用量依存性効果を調べた。この研究ではまた、OTが同じ顔の刺激の信頼度に及ぼす影響についても調査した。
8IU−OT及び24IU−OT投与後の社会的認知の調節は、同等の血液曝露(exposure)を生じるIV−OTの後ではなく、8IU−OT及び24IU−OT投与が、BBBを通過するよりも、少なくとも部分的に脳に直接作用するという証拠を提供する。適格のある参加者は、18〜35歳の男性で、良好な身体的及び精神的健康状態であった。除外基準には、過去14日間の何らかの薬物の使用、アルコール又は薬物乱用の履歴、物理的(腎臓、心臓、内分泌、肺、肝臓、神経、胃腸、血液及び代謝障害を含む)又は精神疾患、及びIQが75未満である。男性ボランティア57名が適格性について評価され、18名の参加者が20−30歳で選ばれた(M=23.81、SD=3.33)。2人の参加者は入会後に退去したが(最初のセッションの後に1人が退去し、もう1人が3つのセッションを終了して退去した)、これらの参加者からのデータは分析に含まれていない。

参加者は、四期、4つの処置法のラテン平方法(ACDB、BDCA、CBAD、DABCが4:4:4:4の比)を用いて、4つの処置シーケンスのうちの1つに無作為に割り付けられた。このとき、処置の間に少なくとも6日の期間を与えられ、潜在的な持ち越し効果を防止した。参加者及び研究チームの両方は、データ収集中は、視覚的に一致する装置及びIV装置を用い、処置について見えないようにした(blind)。
各実験セッションの初めに、除外及び包含基準が確認され、State-Trait Anxiety Inventory 54が施行された。鼻腔の循環によってセッション間で鼻腔の環境が大きく異なることがないことを確認するために、血液サンプルを採取して日常的な尺度を評価し、音響鼻腔計測法(AR)が行われた(スクリーニング中の手続毎に)。

顔の評価のために、参加者は第1の質問(Ql)を受けた。それは次のいずれかであった。
・この人はどのくらい怒っているか?(回答:怒っていない〜非常に怒っている)、又は、
・この人はどのくらい幸せか?(回答:幸せではない〜非常に幸せ)
・あなたはこの人をどれくらい信用しますか?(回答:全く信用しない〜非常に信用する)。
各質問の平均評定は、各感情カテゴリ内でセッションごとに平均化され、7つの行動変数(Ql:幸せな顔/幸せ、幸せな顔/怒っている、曖昧な顔/幸せ、曖昧な顔/怒っている、怒った顔/幸せ、怒った顔/怒っている、Q2:信用する)を出力した。これらの刺激及び質問は、感情認識の3つのレベルを評価するために選択された。
・曖昧であること
・対応する手がかりとレーティングにより、曖昧でないこと(例えば、怒った顔の怒りのレーティング)
矛盾する手がかりとレーティングにより、曖昧でないこと(例えば、幸せな顔の怒りのレーティング)
形状の評価のために、参加者は次のいずれかの質問を受けた。
・(Q1)この形状はどのくらい黄色いか?(回答:黄色ではない/とても黄色ではある)、又は
・この形状はどのくらい青色か?(回答:黄色でない/とても黄色である)
・どのくらいこの色が好きですか?(回答:全く好きでありません/とても好きです)
顔のランク付けと同じように、参加者は、1〜5までの視覚アナログスケール(VAS)で回答をランク付けした。ランク付けは、各質問の提示においてランダム化されたVAS上のカーソルの位置により行われた。
脳画像データは、8チャンネルヘッドコイル(GE Healthcare、ミルウォーキー、米国ウィスコンシン州)を用いた3TGeneral Electric Signa HDxtスキャナーで収集した。

各OT条件(48スライス、面内分解能3.75×3.75mm、スライス厚3.2mm、ギャップなし)における各個体について528ボリュームの1回の実行結果を収集した。
以下のパラメータを有するサジタル高速スポイルドグラジエントエコー(FSPGR)シーケンスを使用して、共同登録目的で使用されるT1強調量を取得した。

瞳孔測定データは、60Hzのサンプリングレートで、MR対応コイル搭載赤外線視線追跡システム(NNLEye Tracking Camera、NordicNeuroLab、Bergen、ノルウェー)を用いて収集した。データは、iViewXソフトウェア(SensoMotoric Instruments、Teltow、ドイツ)を用いて記録し、その際、瞳孔測定記録の開始を刺激提示に同期させる刺激コンピュータからのトリガを用いた。
分析は、IBM SPSS Statisticsバージョン22(IBM Inc.)を用いて薬物動態を決定し、出力された測定値において処置の影響を調べた。感情発現評価、薬物動態、状態不安、及び信頼性の分析のために、最近の鼻腔内交差性向精神薬試行(参照文献95)と一致する線形混合モデル(LMM)アプローチが採用された(参照文献58)。すべてのモデルは、非構造化マトリックスを使用してフィッティングされた。有意な主効果(すなわち、p<0.05)に関しては、5%の誤発見率(FDR)を用いて複数の比較を訂正する臨界p値の調整を用いて事後試験を行った(参照文献59)。
更に、血漿OT、AVP、コルチゾール濃度及び状態不安に対する処置の影響を調べるために、LMMには3つの固定因子(処置、時間、処置×時間)、1つの反復因子(処置)が適合した。複数の処置条件の間で鼻環境が変化するか否かを調べるために、繰り返し測定MANOVAを3つの従属変数(MCA、TVO−5、及びTV2−5)で実施した。
フォローアップのペアワイズ比較(q=0.05、改訂された臨界値p<.017)は、曖昧な顔の怒りのレーティングが、偽薬処置(p=0.011;=17%、SE減少は6%)、及び24IU−OT処置(p=0.003;平均減少=17%、SE減少は5%)と比較して、8IU−OT処置条件において有意に低下したことを示した。他の感情カテゴリ又は信頼性レーティングでは、処置の主な影響は認められなかった(図5(b)〜図5(f))。

刺激カテゴリは、怒りのレーティング評価の低下に対する刺激カテゴリの影響を評価するためのLMMにおける固定された効果、及び反復的された効果の両方であった。
8IU−OTと偽薬処置の割合の変化を比較するLMMについては、刺激タイプ[F(2、14.42)=4.79、p=0.025;図6(a)]について主要な効果があった。
8IU−OT処置と24IU−OT処置の間で割合の変化を比較するLMMについては、刺激タイプ[F(2、14.05)=7.01、p=0.007;図6(b)]について主要な効果があった。
8IU−OT、24IU−OT、IV−OT及び偽薬の投与後の平均OT血漿中濃度の時間変化を(平均標準誤差を表す誤差バーとともに)表2及び図7(a)に示す。4(処置)×6(時間)のLMMについては、OT血漿濃度[F(3、88.71)=4.25、p=0.007]に対する処置の重要な主な効果があった。
8IU−OT、24IU−OT、IV−OT及び偽薬の投与後の平均AVP血漿濃度の時間変化を(平均の標準誤差を表す誤差バーとともに)表3及び図7(b)に示す。4(処置)×6(時間)のLMMでは、AVP血漿濃度[F(3、82.42)=4.55、p=0.005]に対する処置の重要な主な効果があった。
フォローアップペアワイズ比較(q=0.05、p<0.083の改訂された臨界値)は、偽薬処置(p=0.008)及びIV−OT(p=0.008)と比較して、24IU−OT処置後に血漿AVP濃度が有意に低下したことを明らかにした。また、IVI−OT(p=0.023)と比較して8IU−OT処置後においても有意に減少したことを明らかにした。[F(5、90.63)=1.81、p=0.12]又は処置×時間の相互作用、F(15、82.46)=1.03、p=0.434では有意な主な効果はなかった。
8IU−OT、24IU−OT、IV−OT及び偽薬投与後の平均コルチゾール血漿濃度を(平均標準誤差を表す誤差バーとともに)表4及び図7(c)に示す。
フォローアップペアワイズ比較(q=0.05、p<0.017の改訂された臨界値)は、偽薬処置(p=0.01)及び24IU−OT(p<0.001)と比較して、IV−OT処置後の有意に増加したコルチゾール濃度を明らかにしたが、8IU−OTではそうではなかった。コルチゾール血漿濃度[F(5、90.07)=2.4、p=0.04]に時間の有意な主な効果があったが、有意なフォローアップペアワイズ比較は見出されなかった。最後に、有意な処置×時間相互作用はなかった[F(15、84.72)=0.421、p=0.969]。
重要なことに、現在の所見は社会的認知の調節において、低用量のOTが高用量より有効であることを示唆した最初のものである。更に、これらの結果は、この研究の供給装置を用いてOTが鼻腔内に供給され、脳に到達し、社会的認知に影響を及ぼすのに対して、末梢投与OTは、同様に血漿OT濃度を増加させるが、そのような効果はなかった、という行動に関する証拠を提供する。
解析は、R統計パッケージ(Version3.1.1; R Development Core Team、2014)を使用して、鼻弁の断面積の役割を調べ、前部と後部との間の接合部におけるスリット状構造である各鼻腔の領域を薬力学的に評価した。鼻弁の断面積がセッションごとに有意に変動したかどうかを調べるために反復測定ANOVAを最初に実施した(スクリーニングセッション及び各処置セッション)。更に、個人の全体的な大きさ及び年齢によって断面積が異なる可能性があるため、ピアソン相関係数を計算して、スクリーニング時のこれらの因子間の関係を評価した。
8IU−OT、24IU−OT、IV−OT及び偽薬処置の後に、怒っている曖昧な顔に対する応答と鼻弁の平均断面積との間の相関関係を決定した。この研究では、左右の鼻腔の両方に投与を行ったので、左右の鼻腔のそれぞれについて平均断面積を求め、更にこれら左右の鼻腔の平均値の合計から平均断面積を求めた。
表6に示すように、血漿と鼻弁の平均断面積との関係もまた計算された。社会認知評価の直前(処置後40分まで)のベースラインのOT、AVP、及び血清レベルとの間の変化スコアも計算され、全体的な利用可能性について、OT、AVP及びコルチゾールの鼻弁の断面積の影響を調査した。表6は、鼻弁の平均断面積とオキシトシン、バソプレッシン、及びコルチゾールの血漿濃度の関係を示す。
8IU−OT処置後の曖昧な顔の怒りのレーティングと鼻弁の平均断面積との関係を図8に示す。
さらに、処置条件のいずれの後においても、鼻弁の断面積とOT、AVP又はコルチゾールの血漿濃度との間には関係性はなかった。本研究は、社会的認知に対するOTの効能が、24IU未満の規定されたより低用量のOTを鼻腔内投与する場合に、鼻弁の断面積の制御によって影響され得ることを証明している。一実施形態では、この制御は、呼気流の有効圧力と、鼻弁を開く際のノーズピースの構造的効果によって得られる。
従来のfMRIデータのfMRI前処理を、fMRIデータをノイズ除去するために、FMRIBのICA−base X−Noisefier法(FIX)を使用した、独立成分分析(ICA)及び自動分類を使用して実施した。
個人の成分は、MELODIC(独立構成要素への多変量探索的線形最適化分解)の時間連結アプローチを用いてグループ分けされ、40個の成分において固定化されたモデルの順序とされた。
次に、二重回帰を実行して、個人の成分の空間マップ及び対応する時間経過を推定した。これは、図9のAに示すように、二重回帰後のすべてのデータセット(t>5)の1つのサンプルt検定を反映する。
OT条件(IU08−OT、IU24−OT、IV−OT及び偽薬処置のF検定)の主な効果の評価のために、ボクセルワイズ一般線形モデル(GLM)テストが実行された。次に、補正されていないボクセル的なp<0.01の最大のクラスタを同定した。OT条件の主な効果を示す2つの最大のクラスタは、それぞれ図9Bに示すように左右の扁桃内に局在する。
予想の通り、反復測定ANOVAは、両方のクラスタにおける状態の有意な主効果を明らかにした(p=0.0032及びp=0.0039)。
ボックスプロットは、OT条件の主な効果は、IU08−OT対偽薬によって引き起こされ、IU08−OT処置における扁桃体の接続性の増加を示している。これはまた、事後ペアワイズ比較(t=−2.54、p=0.016、及びt=−2.24、p=0.033)によっても支持された。
fMRIデータの共同登録のための正確な脳抽出量を得るために、表面再構成及び完全な脳セグメンテーション(参照文献123)を含むT1強調データのためにFreeSurfer(http://surfer.nmr.mqh.harvard.edu)を使用した。FRRIBソフトウェアライブラリ(FSL;http://fsl.fmrib.ox.ac.uk/fsl/fslwiki/(参照文献124))をfMRIデータの処理に使用した。最初の5ボリュームは廃棄された。FMRIBのエキスパート分析ツール(FEAT)バージョン6.0(参照文献128)を用いてfMRIデータの前処理を行った。これには、MCFLIRT(参照文献124)を使用したモーション補正、FWHM 7mmのガウスカーネルを使用するSUSAN(参照文献125)による空間平滑化、及び100秒の時間ハイパスフィルタが含まれていた。自動化された雑音除去(以下を参照)を実行するために、独立成分(MELODIC ICA (参照文献126)への多変量探索的線形最適化分解を使用して、単一セッション独立成分分析(ICA)を行った。
次に、平均の扁桃体コントラスト―パラメータ推定値(COPE)が左右の扁桃体マスクから、FSLを提供されたHarvard−Oxfordアナトミーアトラスに基づいて抽出され、その値をSPSSのより高次の線形混合モデルに提出して、状態及び処置の主な効果のためのテストを行った(下記参照)。
同一のLMM手法を用いて、平均瞳孔径の違い、怒っている顔と形状の間、幸せな顔と形状の間、及び幸せな顔と怒っている顔の間の、左右の扁桃体活動のコントラストについてのCOPE値を調べた。モデルフィッティング後の標準化された残差を異常値について調べた。2.58未満、2.58より大きいZスコアを分析から除外した。これらの閾値を超える異常値は、扁桃体活動データセット(怒った顔、幸せな顔、曖昧な顔、及び形状刺激の提示中の右扁桃体データからそれぞれ1つの値、左扁桃体の曖昧な顔及び形状データからのそれぞれから、2つの値)から除外された。
対立仮説の両方の証拠の強さを調べた。
怒り[F(3、15)=0.57、p=0.64]、幸せ[F(3、15)=0.62、p=0.62]、又は感情的に曖昧な顔[F(3、15)=1.33、p=0.3]を処理している間、平均瞳孔径に対する処置の有意な主効果はなかった。
最後に、表9に示されるように、いかなる処置の後における、怒りのレーティングの強さと、右偏桃体の活動との間に有意な関係はなかった。また、表10に示すように、いずれかの処置後の鼻弁寸法と右扁桃活性化との間に有意な関係はなかった。
これらの効果は、幸せな顔及び曖昧な顔の提示中の右扁桃体活動の処置の主な効果が有意であり、且つ有意性の境界にあるため、否定的な社会的刺激に特有ではない可能性がある。その後の8IU−OT処置と偽薬との事後比較は、統計的有意性の境界にあった。OT処置後の正及び負の両方の評価された刺激の提示中の右扁桃活性の観察された減少は、OTが接近に関連する行動を増加させるという仮説と一致する(参照文献114、118)。
重要なことに、末梢投与されたOTの1IU−OTが扁桃体活動に影響を及ぼすという証拠は見出されなかった。末梢のOTがBBBを通過できるかどうかについての矛盾する証拠があるが(参照文献140、141)、我々の研究は、OTがこの障壁を少量通過するとしても、この量は偽薬と比較して扁桃体活動を調節するのに十分ではないことを示唆する。
要するに、本研究は、驚くべきことに、記載された供給方法で鼻腔内に送達された、低いドーズ量のOTが扁桃体活動を調節することを示し、またこの結果は、鼻腔内OT用量が低い方が、より社会的認知及び行動の調節を容易にすることができ、OTの末梢活動には重大な神経的帰結がないように見える、ということを示唆する更なる証拠を提供するものである。

Claims (71)
- オキシトシン、その非ペプチドアゴニスト及び/又はそのアンタゴニストを用いて、ヒト被験体における社会的認知及び/又は行動に関する状態を調節するための供給装置及び方法であって、
被験体の第1の鼻腔にノーズピースを提供し、
24IU未満のオキシトシン、その非ペプチドアゴニスト及び/又はそれらのアンタゴニストを、前記ノーズピースを介して、三叉神経によって刺激される鼻弁の後部の上部領域に投与するための供給ユニットを提供する
ことを含む供給装置又は方法。 - 被験体が吐き出して被験体の口腔咽頭軟口蓋を閉鎖させるマウスピースを提供する、
請求項1に記載の供給装置又は方法。 - 前記マウスピースが前記ノーズピースに流体的に接続され、それにより、呼気が前記マウスピースから前記ノーズピースを通って前記第1の鼻腔に供給される、請求項2に記載の供給装置又は方法。
- 前記呼気によって生成される空気流が、前記被験体の鼻の後端部の周りに、前記被験体の第2の鼻腔から供給される、請求項3に記載の供給装置又は方法。

- オキシトシン、その非ペプチドアゴニスト及び/又はそのアンタゴニストが液体として供給される、請求項1に記載の供給装置又は方法。
- オキシトシン、その非ペプチドアゴニスト及び/又はそのアンタゴニストが液体スプレーとして供給される、請求項1に記載の供給装置又は方法。
- オキシトシン、その非ペプチドアゴニスト及び/又はそのアンタゴニストが粉末として供給される、請求項1に記載の供給装置又は方法。
- オキシトシン、その非ペプチドアゴニスト及び/又はそのアンタゴニストが粉末スプレーとして供給される、請求項1に記載の供給装置又は方法。
- 15IU未満のオキシトシン、その非ペプチドアゴニスト及び/又はそのアンタゴニストが投与される、請求項1に記載の供給装置又は方法。
- 12IU未満のオキシトシン、その非ペプチドアゴニスト及び/又はそのアンタゴニストが投与される、請求項1に記載の供給装置又は方法。
- 10IU未満のオキシトシン、その非ペプチドアゴニスト及び/又はそのアンタゴニストが投与される、請求項1に記載の供給装置又は方法。
- 1IUを超えるオキシトシン、その非ペプチドアゴニスト及び/又はそのアンタゴニストが投与される、請求項1に記載の供給装置又は方法。
- 2IUを超えるオキシトシン、その非ペプチドアゴニスト及び/又はそのアンタゴニストが投与される、請求項1に記載の供給装置又は方法。
- 4IUを超えるオキシトシン、その非ペプチドアゴニスト及び/又はそのアンタゴニストが投与される、請求項1に記載の供給装置又は方法。
- 前記第1の鼻腔内の前記鼻弁を拡張させる、請求項1に記載の供給装置又は方法。
- 前記鼻弁は、少なくとも20mm2の非拡張断面積から前記ノーズピースを挿入することによって拡張される、請求項18に記載の供給装置又は方法。
- 前記鼻弁は、少なくとも40mm2の非拡張断面積から前記ノーズピースを挿入することによって拡張される、請求項18に記載の供給装置又は方法。
- 前記鼻弁は、少なくとも60mm2の非拡張断面積から前記ノーズピースを挿入することによって拡張される、請求項18に記載の供給装置又は方法。
- 前記鼻弁は、少なくとも70mm2の非拡張断面積から前記ノーズピースを挿入することによって拡張される、請求項18に記載の供給装置又は方法。
- 前記鼻弁は、少なくとも80mm2の非拡張断面積から前記ノーズピースを挿入することによって拡張される、請求項18に記載の供給装置又は方法。
- 前記鼻弁は、少なくとも90mm2の非拡張断面積から前記ノーズピースを挿入することによって拡張される、請求項18に記載の供給装置又は方法。
- 前記鼻弁は、少なくとも100mm2の非拡張断面積から前記ノーズピースを挿入することによって拡張される、請求項18に記載の供給装置又は方法。
- 前記鼻弁は、前記ノーズピースを挿入することによって、前記鼻弁の非拡張断面積の1.5倍の断面積を有するように拡張される、請求項18に記載の供給装置又は方法。
- 前記鼻弁は、前記ノーズピースを挿入することによって、前記鼻弁の非拡張断面積の1.75倍の断面積を有するように拡張される、請求項18に記載の供給装置又は方法。
- 前記鼻弁は、前記ノーズピースを挿入することによって、前記鼻弁の非拡張断面積の2倍の断面積を有するように拡張される、請求項18に記載の供給装置又は方法。
- 前記鼻弁は、少なくとも100mm2の断面積を有するように前記ノーズピースを挿入することによって拡張される、請求項18に記載の供給装置又は方法。
- 前記鼻弁は、少なくとも120mm2の断面積を有するように前記ノーズピースを挿入することによって拡張される、請求項18に記載の供給装置又は方法。
- 前記鼻弁は、少なくとも150mm2の断面積を有するように前記ノーズピースを挿入することによって拡張される、請求項18に記載の供給装置又は方法。
- 前記鼻弁は、少なくとも180mm2の断面積を有するように前記ノーズピースを挿入することによって拡張される、請求項18に記載の供給装置又は方法。
- 前記鼻弁は、少なくとも200mm2の断面積を有するように前記ノーズピースを挿入することによって拡張される、請求項18に記載の供給装置又は方法。
- 前記ノーズピースは、前記鼻弁へと延びる先端を有し、前記鼻弁を拡張させるように作用する、請求項18記載の供給装置又は方法。
- 前記ノーズピースは、そこを通過する流路を設定するベース部と、少なくとも一部において前記ノーズピースの先端を提供し、サジタル方向において剛性を与えて前記先端に前記鼻弁の上部領域において肉質組織を開放させることを可能とし、これにより前記鼻弁の開放領域を拡大させるとともに、前記サジタル方向と直交する横方向に柔軟性を与えて前記鼻弁への前記先端の挿入を容易にする、前記ベース部の遠位端にある突出部と、を含むボディ部を備えた、請求項34に記載の供給装置又は方法。
- 前記ボディ部は、プラスチック材料、任意にはアクリロニトリルブタジエンスチレン(ABS)プラスチックから形成される、請求項35に記載の供給装置又は方法。
- 少なくとも前記突出部の周りに配置された外側ボディ部を更に備える、請求項35に記載の供給装置又は方法。
- 前記外側ボディ部は、弾性材料、任意にはゴム又はエラストマー材料で形成される、請求項37に記載の供給装置又は方法。
