Abstract
Description
本発明は、抗プログラム細胞死(PD−1)抗体である抗癌剤と他の抗癌剤の組合せを投与することを含む、対象における腎癌を処置する方法に関する。ある態様において、他の抗癌剤は、抗細胞毒性Tリンパ球抗原−4(CTLA−4)抗体、抗血管形成チロシンキナーゼ阻害剤またはそれらの組合せである。
ヒト癌は、免疫系により認識可能な可能性のある新規抗原を産生する、多数の遺伝的および後成的変更を保有している(Sjoblom et al., 2006)。Tリンパ球およびBリンパ球からなる適応免疫系は、多種多様な腫瘍抗原に応答する広い能力および精巧な特異性を備えた、強力な抗癌潜在力を有する。さらに、免疫系には、相当な柔軟性および記憶成分が見られる。適応免疫系の全てのこれらの特性利用の成功は、全癌処置手法の中で、免疫療法を独特なものとする。
本発明は、対象に、(a)PD−1に特異的に結合し、阻害するAbまたはその抗原結合部分;および(b)他の抗癌剤の組合せの治療有効量を投与することを含む、腎癌に罹患している対象を処置する方法を提供する。ある態様において、他の抗癌剤は、(i)CTLA−4に特異的に結合し、阻害するAbまたはその抗原結合部分、(ii)抗血管形成TKIまたは(iii)それらの組合せである。ある態様において、腎癌はRCCである。ここに開示する治療法のいずれかのある態様において、抗PD−1 Abはニボルマブである。他の態様において、抗PD−1 Abはペンブロリズマブである。ここに開示する治療法のいずれかのある他の態様において、抗CTLA−4 Abはイピリムマブである。他の態様において、抗CTLA−4 Abはトレメリムマブである。さらに他の態様において、TKIはスニチニブ、パゾパニブ、ソラフェニブ、アクシチニブまたはチボザニブである。
本発明は、患者に抗PD−1 Abまたはその抗原結合フラグメントと他の抗癌剤の組合せを投与することを含む、腎癌患者を処置する方法に関する。ある態様において、他の抗癌剤は、抗CTLA−4 Ab、抗血管形成TKIまたはこの任意の組合せである。
本発明がより迅速に理解され得るように、まずいくつかの用語を定義する。本明細書で使用する限り、ここで他に明示的に提供されない限り、次の用語の各々は、下記意味を有する。さらなる定義は、本明細書をとおして示される。
PD−1は、活性化T細胞およびB細胞により発現され、免疫抑制を仲介する重要な免疫チェックポイント受容体である。PD−1は、CD28、CTLA−4、ICOS、PD−1およびBTLAを含む受容体のCD28ファミリーのメンバーである。PD−1の2つの細胞表面糖タンパク質リガンド、プログラム死リガンド−1(PD−L1)およびプログラム死リガンド−2(PD−L2)が同定されており、これらは、抗原提示細胞ならびに多くのヒト癌で発現され、PD−1への結合によりT細胞活性化およびサイトカイン分泌を下方制御することが示されている。PD−1/PD−L1相互作用の阻害は、前臨床モデルにおいて強力な抗腫瘍活性に介在する。
本発明の抗CTLA−4抗体は、ヒトCTLA−4に結合してCTLA−4とヒトB7受容体の相互作用を妨害する。CTLA−4とB7の相互作用は、CTLA−4受容体担持T細胞を不活性化させるシグナルを伝達するため、相互作用の妨害は、このようなT細胞の活性化を効率的に誘発、増強または延長し、それにより免疫応答を誘発、増強または延長する。
本発明はまた、腎癌の処置のための抗PD−1抗体と標準治療の組合せ治療も含む。標準治療は、抗PD1抗体処置の前、中途または後のあらゆる時点で対象に実施できる。いくつかの例において、標準治療およびPD−1抗体処置をさらなる処置とまた組合せる。種々のタイプの癌のための標準治療は、当業者に周知である。例えば、米国の21の主要な癌センターの連盟であるNational Comprehensive Cancer Network(NCCN)は、NCCN Clinical Practice Guidelines in Oncology (NCCN GUIDELINES(登録商標))を出版し、これは、広範な癌の詳細な標準治療処置についての最先端情報を提供する(NCCN GUIDELINES(登録商標), 2014参照)。
複数のラインの標的化治療で進行している患者に有効な薬剤ならびに現在の標準処置を越える長い期間生存を延ばす治療に対する明確な要求が存在する。IL−2およびIFN−α以外の免疫療法に対するより新しいアプローチは、最近、有望であることが示されている。伝統的化学療法剤および放射線療法が期待外れであることが証明され、腎臓腫瘍が免疫応答を惹起できることが知られており、RCCは自然な退縮が最も一般的である腫瘍の一つであるために、免疫療法はmRCCの患者で最初に試験された(Elhilali et al., 2000;Inman et al., 2013)。現在、RCCの処置のための、T細胞調節、免疫プライミングおよび自然免疫を含む多様な免疫療法戦略が臨床開発中である(Rosenblatt and McDermott, 2011; Inman et al., 2013)。PD−1受容体またはそのPD−L1リガンドを標的とするAbを使用して、T細胞免疫抑制を逆転させる方法も、RCCの処置に非常に有望であることが証明されている(Topalian et al., 2012a;Brahmer et al., 2012;WO2013/173223号)。
薬剤、例えば抗PD−1および抗CTLA−4 Abの組合せを含む免疫療法アプローチは、黒色腫の処置に高度に有効であることが証明されている(Wolchok et al., 2013;WO2013/173223号)。類推すれば、RCC患者は、異なる免疫療法剤の組合せまたはこのような薬物と標的化藥剤または手術、放射線、標準的癌化学療法またはワクチンを含む他の処置の組合せから利益を受けることができる可能性がある。しかしながら、免疫療法を他の抗癌剤と組合せたとき、驚くべきかつ予想外の合併症が観察されることがある。例えば、BRAF V600E変異を有する2名の黒色腫患者の抗PD−1剤(それぞれニボルマブおよびMK−3475)での第一選択治療は、顕著な毒性を生じなかったが、疾患進行時のベムラフェニブ(ゼルボラフ(登録商標))での処置は、ベムラフェニブ処置コースの初期に重度の過敏性薬疹と多臓器傷害をもたらした(Johnson et al., 2013)。一方の患者はその後急性炎症性脱髄性多発ニューロパチーを発症し、他方は低用量ベムラフェニブ再チャレンジによりアナフィラキシーを発症した。
本発明の治療剤は、組成物、例えば、AbまたはTKIおよび薬学的に許容される担体を含む医薬組成物を構成し得る。ここで使用する“薬学的に許容される担体”は、生理学的に適合性である任意かつ全ての溶媒、分散媒体、コーティング、抗細菌および抗真菌剤、等張および吸収遅延剤などを含む。好ましくは、Abを含む組成物用の担体は静脈内、筋肉内、皮下、非経腸、脊髄または上皮投与(例えば、注射または点滴による)に適するものであり、一方TKIを含む組成物用の担体は、非経腸ではない、例えば、経口、投与に適するものである。本発明の医薬組成物は、1以上の薬学的に許容される塩、抗酸化剤、水性および非水性担体および/または防腐剤、湿潤剤、乳化剤および分散剤のようなアジュバントを含み得る。
本発明は、腎癌を有する対象を処置する方法を提供する。ある態様において、本発明は、組合せ治療を投与することを含む、腎癌または腎癌を有する対象を処置する方法を含む。ある態様において、本方法は、対象に治療有効量の:(a)PD−1受容体と特異的に結合し、PD−1活性を阻害するAbまたはその抗原結合部分である抗癌剤;および(b)他の抗癌治療剤の組合せを投与することを含む。ある態様において、本発明は、処置を必要とする対象における腎癌または腎癌を有する対象を処置する方法であって、対象に治療有効量の:(a)抗PD−1抗体またはその抗原結合部分;および(b)他の抗癌剤の組合せを投与することを含む、方法に関する。他の態様において、本発明は、腎癌または腎癌を有する対象を処置する方法に関し、該方法は、対象に治療有効量の:(a)抗PD−1抗体またはその抗原結合部分;および(b)ここに開示する標準治療を投与することを含む。RCCは腎臓腫瘍の約90%を構成するため、ある態様において、腎癌はRCCである。他のある態様において、対象はヒト患者である。
本発明方法において使用するのに適する抗PD−1 Abは、PD−1と高特異性および親和性で結合し、PD−L1およびまたはPD−L2の結合を遮断し、PD−1シグナル伝達経路の免疫抑制性作用を阻害するAbである。ここに開示する治療法のいずれにおいても、抗PD−1または抗CTLA−4“抗体”は、それぞれPD−1またはCTLA−4受容体に結合し、リガンド結合を阻害し、免疫系を上方制御する完全Abに類似する機能的特性を示す、抗原結合部分またはフラグメントである。ある態様において、抗PD−1 Abまたはその抗原結合部分は、ヒトPD−1との結合についてニボルマブと交差競合する。他の態様において、抗PD−1 Abまたはその抗原結合部分は、キメラ、ヒト化またはヒトモノクローナルAbまたはその一部である。ヒト対象を処置するためのある態様において、Abはヒト化Abである。ヒト対象を処置するための他の態様において、AbはヒトAbである。IgG1、IgG2、IgG3またはIgG4アイソタイプのAbを使用し得る。
RCCを処置するための本発明方法のある態様において、他の抗癌剤、すなわち、抗PD−1 Abと組合せて使用する抗PD−1 Ab以外の抗癌剤は抗血管形成TKIである。ソラフェニブ、スニチニブ、パゾパニブ、アクシチニブおよびチボザニブを含む、RCCの処置に承認されているまたは臨床試験において有効性が証明されている種々のTKIを、抗PD−1 Abとの組合せ治療法で使用し得る。しかしながら、これらの組合せの有効性および安全性が前臨床または臨床試験で評価されるまで、予想外の毒性があるかまたは単剤療法に対して有意に増強された有効性が生じ得ない可能性がある。RencaマウスRCCモデルにおけるここに開示する前臨床データは、抗PD−1 Abは、RCC腫瘍増殖の阻害にスニチニブと相乗的に作用するが、抗PD−1 Abとソラフェニブの組合せは、このマウスRCCモデルにおいてソラフェニブの低レベルの抗腫瘍活性を有意に増強しなかったことを示す(図1および2)。さらに抗PD−1 Abとスニチニブの組合せがマウスモデルにおけるRCCの処置に高度に有効であることを証明するために、ヒトRCC患者におけるこの組合せならびにニボルマブとパゾパニブの組合せの有効性および安全性を、臨床試験で確認している。
本組合せ治療法のある態様において、抗PD−1 Abまたはその抗原結合部分の治療有効量は、抗血管形成TKIと組合せて、静脈内注入により約週1回、約2週間毎に1回、約3週間毎に1回または約月に1回に投与する約0.1〜約10.0mg/kg体重の範囲の用量の範囲を含む。ある態様において、抗PD−1 Abを、3週間毎に1回、2mg/kgの用量で投与する。他の態様において、抗PD−1 Abを、約3週間毎に1回、約5mg/kgの用量で投与する。
または約800mgの用量で投与する。ある態様において、パゾパニブを、1日1回、食事なしで推奨用量の800mgで投与する。
本開示はまた、RCCの処置のための組合せ治療法も提供し、ここで、抗PD−1 Abを、CTLA−4と特異的に結合し、CTLA−4活性を阻害するAbまたはその抗原結合部分である他の抗癌剤と組合せる。抗PD−1 Abであるニボルマブと抗CTLA−4 Abであるイピリムマブの組合せが、ORRにより測定して、ニボルマブまたはイピリムマブ単独で観察される活性より有意に高いレベルの臨床活性を生じることがここで示されている(実施例6参照)。したがって、ある態様において、抗PD−1 Abと組合せて使用する抗CTLA−4 Abはイピリムマブである。ある態様において、抗CTLA−4 Abはトレメリムマブである。他の態様において、抗CTLA−4 Abまたはその抗原結合部分は、ヒトCTLA−4への結合についてイピリムマブと交差競合するAbまたはその一部である。ある他の態様において、抗CTLA−4 Abまたはその抗原結合部分は、キメラ、ヒト化またはヒトmAbまたはその一部である。さらに他の態様において、抗CTLA−4 Abまたはその抗原結合部分は、ヒトIgG1またはIgG4アイソタイプのものである重鎖定常領域を含む。ある態様において、抗CTLA−4 Abは、ヒトIgG1アイソタイプのものである重鎖定常領域を含む。
当業者は、抗PD−1抗体またはその抗原結合フラグメントでRCCを処置する、記載する方法を、RCCの他の既知処置と組合せることができることを認識する。例えば、抗PD−1 Abまたはその抗原結合フラグメントを、パクリタキセル、ドセタキセル、ビンクリスチン、エリブリン、エストラムスチン、エトポシド、イクサベピロン、カバジタキセル、ビンクリスチンリポソーム、ビノレルビン、ビンクリスチン、ビンブラスチンまたはテニポシドのような有糸分裂阻害剤と組合せることができる。上記のように、IFN−αまたはIL−2を含む、種々のサイトカインも癌の処置に有用であることが知られている。抗PD−1 Abまたはその抗原結合フラグメントを、IFN−α、IFN−b2、ペグ化IFNb2(PegIFN−b2)またはIL−2のようなサイトカインと組合せ得る。ビンフルニンを含むが、これに限定されない当分野で知られる他の小分子処置も適し得る。ある態様において、抗PD−1抗体およびTKIまたは抗CTLA−4抗体の組合せ処置を含むここに記載する抗PD−1抗体を、SOC処置を含む、ここに記載する任意の他の処置と組合せで使用する。
治療使用のための抗PD−1 AbおよびTKIを含むキットも本発明の範囲内である。キットは、一般にキットの内容物の意図されている使用および使用指示のためのラベルを含む。用語ラベルは、キットに関してまたはキットと共に届けられるまたは他の方法でキットに添付される、文書または記録媒体を含む。したがって、本発明は腎癌に罹患している対象を処置するためのキットを提供し、キットは:(a)PD−1受容体に特異的に結合し、PD−1活性を阻害するAbまたはその抗原結合部分である0.1〜10mg/kg体重の範囲の用量の抗癌剤;(b)(i)抗血管形成TKI、例えば、4週間投薬と続く2週間休薬のスケジュールの1日1回12.5mg、25mgまたは50mg用量のスニチニブ、1日1回200mg、400mgまたは800mg用量のパゾパニブまたは1日1回または2回400mgの用量のソラフェニブである他の抗癌剤の一定用量;または(ii)0.1〜10mg/kg体重の用量範囲のCTLA−4に特異的に結合し、阻害する抗体またはその抗原結合部分;および(c)ここに開示する組合せ治療法のいずれかにおいて抗PD−1 Abおよび他の抗癌剤を使用するための指示を含む。ある態様において、抗PD−1、抗CTLA−4 Abおよび/またはTKIは、単位用量形態に共包装され得る。ヒト患者の処置のためのある態様において、キットはここに開示する抗ヒトPD−1 Ab、例えば、ニボルマブまたはペンブロリズマブを含む。他の態様において、キットは、ここに開示する抗ヒトCTLA−4 Ab、例えば、イピリムマブまたはトレメリムマブを含む。
マウスモデルにおける抗PD−1およびTKIでの腎癌の処置
材料および方法
動物
雌8〜12週齢Balb/cおよびC57/BL6マウス(Harlan Laboratories, Federick, MD)を全試験で使用した。マウスを、特に断らない限り無菌マイクロアイソレーターケージで飼育し、無菌餌および水を自由に与えた。
スニチニブ(スーテント(登録商標))は、腎細胞癌の処置についてFDAで承認されている、いくつかの受容体チロシンキナーゼ(VEGFR−1、VEGFR−2およびVEGFR−3、PDGFR−αおよび−β、c−Kit、FLT−3、CSF−1RおよびRETを含む)を標的とする、経口投与可能な、小分子、多キナーゼ阻害剤である。スニチニブは、骨髄由来サプレッサー細胞の低減(Ko et al., 2009; Ko et al., 2010)およびT制御性サプレッサー細胞の低減(Hipp et al., 2008; Ozao-Choy et al., 2009)を含む、免疫調節効果を示すことが報告されている。
腫瘍細胞を皮下(SC)にインプラントした。平均腫瘍体積が約100mm3に達したとき、マウスを、8動物の群に無作為化した。媒体(対照)、スニチニブ(120mg/kg)またはソラフェニブ(200mg/kg)を、1日1回、14日間経口投与した(QD×14、PO)。MAb 4H2を、4日毎に10mg/kg、計4回(Q4D×4)、腹腔内(IP)投与した。腫瘍を週に2回測定し、腫瘍体積を、長さ×幅2/2)として計算した。相乗作用は、最高単剤超過(EOHSA)として定義した、すなわち、抗腫瘍効果が、単剤療法で観察された効果より有意に大きいならば、組合せを相乗的と見なした(P<0.05、ウィルコクソンの符号順位検定)。
インプラントしたRCCマウス腫瘍の中央体積に対する抗PD−1およびスニチニブまたはソラフェニブの単剤療法または組合せ治療としての処置効果を図1に示す。スニチニブ単剤療法は、このRencaマウスRCCモデルで活性を示し、処置最終日までに85%の腫瘍増殖阻害(TGI)をもたらした(33日目;表1参照);しかしながら、腫瘍は、治療中止後、進行性に増殖した(図1参照)。単剤療法として投与したとき、抗PD−1 mAbはこのモデルで不活性であったが、抗PD−1とスニチニブの組合せは有意な抗腫瘍活性(101%のTGI)を生じ、インプラント後50日のモニタリングの最後までに完全腫瘍退縮(表1)または腫瘍増殖の著しい遅延(図1)をもたらした。同じモデルの2つの別々の試験で観察されたこれらの結果は、腫瘍増殖阻害におけるこの組合せの効果が、各治療剤個々により示される阻害レベルの合計よりもかなり大きいために、マウスRCC(Renca)腫瘍モデルにおけるニボルマブマウスサロゲート抗PD−1 mAbとスニチニブの相乗的相互作用を示す。
抗PD−1およびスニチニブでの処置後の腫瘍の免疫細胞浸潤
材料および方法
免疫組織化学およびフローサイトメトリー分析を使用して、腫瘍の免疫細胞浸潤を評価した。
凍結切片をアセトン/メタノール混液で固定し、リン酸緩衝化食塩水(PBS)で濯いだ。切片を、内在性ペルオキシダーゼに対して遮断し、一次抗体(抗CD8、抗CD4、抗FoxP3および抗PD−L1)を添加し、一夜インキュベートした。発色現像を、褐色ポリマーベース検出系(Biocare Medical)を使用して行い、ヘマトキシリンを、細胞核の可視化のための青色対比染色として使用した。
腫瘍浸潤性免疫細胞を単離し、単細胞懸濁液として調製し、次いで、染色し、T細胞サブセット(調節性および活性化)および骨髄系細胞(すなわち、CD3、CD4、CD8、FoxP3、CD25およびCD69の発現について)の発現についてフローサイトメトリーで分析した。
対照マウスにおいて、免疫組織化学(IHC)分析は、腫瘍末梢にCD4+およびCD8+T細胞を検出した(図4)。動物にスニチニブを投与したとき、優勢に末梢へのおよび脈管構造に沿ったCD4+およびCD8+T細胞の流入があった。対照的に、マウスに抗PD−1 mAbとスニチニブの組合せを投与したとき、腫瘍全体にCD4+およびCD8+T細胞の大きな浸潤が観察された。抗PD−1 mAbとスニチニブの組合せは、各薬剤単独または対照媒体と比較して、腫瘍浸潤性CD4+およびCD8+T細胞の数を増やした(図4)。対照的に、対照群に対して、腫瘍への増加したT細胞浸潤は、抗PD−1 mAbまたはソラフェニブ単独でまたは抗PD−1 mAbとソラフェニブの組合せで見られなかった(データは示していない)。スニチニブおよびソラフェニブは免疫調節効果を示すことが示されており、これは、これらの薬剤を抗PD−1 Ab免疫チェックポイント阻害剤と組合せたときに見られる異なる治療効果に関係している可能性がある。
抗原特異的免疫応答のインビボ細胞毒性の増強
材料および方法
細胞毒性アッセイ
AH1抗原を発現するCT26マウス結腸癌モデルを本実験に使用して、処置が抗原特異的T細胞に何らかの影響を有するか否かを決定した。CT26細胞のSCインプラント5日日、マウスを媒体で、または3日離して2回投与する抗PD−1 mAb 10mg/kg(Q3D×2)のIPで、1日1回5日のスニチニブ120mg/kg(QD×5)を伴いまたは伴わずに処置した。細胞死を、最終処置後2日目および4日目に調べた。
結果
スニチニブ、パゾパニブまたはイピリムマブと組合せたニボルマブで転移RCCを処置するためのフェーズ1臨床試験
まとめて、実施例1〜3に開示するデータはRCCを有する対象を処置するための、ニボルマブまたはペンブロリズマブのような抗PD−1 Abとスニチニブ、パゾパニブ、ソラフェニブ、アクシチニブまたはチボザニブのような抗血管形成TKIの組合せの研究を支持した。これらのデータは、スニチニブの免疫調節特性が、有効な抗腫瘍免疫応答を支持する腫瘍微小環境の変化を起こし得て、これが抗PD−1 Abにより拡張および増強されることを示唆する。抗PD−1 mAbとソラフェニブの組合せ治療は、マウスRCCモデルでソラフェニブの抗腫瘍活性を有意に増強せず、相乗的有効性がもたらされなかったが(図1)、この組合せは良好に耐容され、ヒト患者で有効である可能性があった。スニチニブ、パゾパニブまたはイピリムマブと組合せたニボルマブの安全性および有効性は、現在、mRCCを有する対象における進行中のフェーズ1試験(NCT01472081)で評価中である。パゾパニブは、経口で利用可能な多キナーゼ阻害剤(VEGFR−1、VEGFR−2、VEGFR−3、PDGFR−αおよび−βおよびc−KIT)であり、ニボルマブとイピリムマブの組合せは、ニボルマブでの単剤療法と比較して、黒色腫の処置において有意に増強された有効性が示されている(Wolchok et al., 2013;WO2013/173223号)。
試験デザイン
mRCCを有する対象におけるニボルマブ+スニチニブ、パゾパニブまたはイピリムマブのフェーズ1オープンラベル試験は進行中である。試験は、5平行処置アーム:ニボルマブ+スニチニブ(アームS)、ニボルマブ+パゾパニブ(アームP)、ニボルマブ3mg/kg+イピリムマブ1mg/kg(アームI−1)、ニボルマブ1mg/kg+イピリムマブ3mg/kg(アームI−3)およびニボルマブ3mg/kg+イピリムマブ3mg/kg(アームIN−3)を含む。アームSおよびアームPの処置は、用量漸増(先に処置された対象)と、続く用量拡張(未処置対象)の2部で実施されている。I−1アームおよびI−3アームの処置は、各アームが先に処置されたならびに未処置の対象の両者からなる、単一初期コホートである。アームI−1およびI−3の拡張コホートならびにアームIN−3は最小限に処置された(未処置を含む)対象からなる。アームSおよびアームPの試験デザインを図10Aに要約し、アームI−1、I−3およびIN−3の試験デザインを図10Bに要約する。
試験集団は、固形癌効果判定基準(RECIST)1.1により少なくとも1つの測定可能な病変を有する、明細胞成分を伴う組織学的に確認された転移腎細胞癌を有する≧18歳の対象である。RECIST 1.1により定義された測定可能な病変を有する非明細胞RCC対象を、アームSおよびアームPの用量漸増コホートのみに認める。有利なまたは中間リスクMSKCC(Memorial Sloan Kettering Cancer Center)予後スコアおよびカルノフスキー・パフォーマンス・ステータス(KPS)≧80%もアームS、PおよびI−1およびI−3オリジナルコホートに必要である(アームI−1およびI−3拡張およびIN−3について、あらゆるMSKCC予後スコアが許容される)。
対象は、本試験の用量漸増部分に適格であるために、進行型/転移設定で少なくとも一つの先の全身治療レジメンを受けていなければならず、一方、本試験の用量−拡張部分について適格であるために対象は未処置でなければならない。先にパゾパニブを受けた用量漸増期に参加する対象は、ニボルマブ+スニチニブの投与に割り当て、一方、先にスニチニブを受けた対象は、ニボルマブ+パゾパニブの投与に割り当てた。先にパゾパニブおよびスニチニブを受けた対象は適格ではない。全身ステロイドまたは何らかの他の免疫抑制性薬剤で慢性的に処置されている対象または抗PD−1、抗PD−L1または抗PD−L2、抗CD137、抗CTLA−4または類似の薬剤で先に処置された対象も試験から除外する。
用量レベルを表2および3。ニボルマブの患者内用量漸増は許されない。
試験した全用量の安全性プロファイルおよび耐容性が特徴付けされ、ニボルマブ+スニチニブおよびニボルマブ+パゾパニブの組合せ投与のMTDがある処置アームについて規定されたら、MTDでのニボルマブ用量が5mg/kg以上である場合のみ、そのアームでコホート拡張を5mg/kgで開始する。いずれかのアームにおいて、ニボルマブのMTDが<5mg/kgであると決定されたら、そのアームはさらに拡張しない。処置アームあたりさらに20対象を、スニチニブまたはパゾパニブと組合せたニボルマブについて5mg/kg用量レベルで処置し、さらなる安全性情報を得る。
前処置対象および未処置対象の両者が参加に適格であり、約20対象を各アームで処置するが、先に抗PD−1、抗PD−L1または抗PD−L2、抗CD137、抗CTLA−4または他の類似薬剤で処置された対象は除外した。I−1、I−3およびIN−3アームの用量を下の表4に詳述する。患者内用量漸増または減量はいずれの薬物でも許可されない。
最小限処置された(未処置を含む)対象登録に適格であり、約25対象を各アームで処置する。各処置サイクルは6週間である。ニボルマブおよびイピリムマブを3週間毎に4回投与し、次いでニボルマブを導入治療の4回目の投与の3週間後または導入治療の4回目の投与が処置遅延により投与されなかったならば、113日目以後に開始して2週間毎に投与する。
進行型または転移設定において先の処置を受けた対象を、上記のようにアームSおよびPの用量漸増コホートに割り当てる。一つの全身前治療を受けている対象を、漸増期アームSおよび/またはPについて、これらのアームがオープンであるときに認定されていない限り、少なくとも2つのイピリムマブ組合せアーム(I−1およびI−3)に、これらのアームの両者がオープンであるとき交互の様式で割り当てる。一方のアームのみがオープンであるとき、これらの対象をオープンアームに割り当てる。
全4アーム(拡張アームSおよびP;I−1およびI−3)が登録のためにオープンであるとき、未処置対象を、登録が完了するまでオープンアームに1:1:1:1比で無作為に割り当てる。
I−1およびI−3拡張およびIN−3について、最小限に処置された(未処置を含む)対象を、登録が完了するまでオープンアームの一つに、1:1:1比で割り当てる。有害事象をNCI CTCAE v4.0に従い段階分けする。疾患評価を、最初の4つの評価について治験薬の最初の投与から6週間毎(±1週間)に次いで、疾患進行まで12週間毎(±1週間)に行う。応答評価をRECIST 1.1に従い行う。アームSおよびアームPの用量漸増相について、対象を、許容されない毒性、疾患進行までまたはインフォームドコンセントの撤回まで処置する。
安全性結果評価指標
組合せの安全性/耐容性結果評価指標およびMTDの決定が、本試験の主要評価項目である。重要な安全性評価は、(1)治験薬の最後の投与から100日後までに生じる有害事象(AE)の発生(または最後の投与から100日後までベースラインに回復しない、安定しない、戻らないまたは不可逆性であると考えられる薬物関連AEについてはそれより長い)、(2)治験薬の最後の投与から100日後までに生じる重度有害事象(SAE)(または最後の投与から100日後までベースラインに回復しない、安定しない、戻らないまたは不可逆性であると考えられる薬物関連SAEについてはそれより長い)および(3)血液学、血清化学および尿検査を含む、最悪毒性グレードによる臨床検査の頻度(アームSおよびアームPについてはスクリーニング、ベースライン、サイクル1〜4の1日目、8日目、22日目および29日目、サイクル5の1日目および22日目に評価;アームI−1およびアームI−3についてはスクリーニング、ベースライン、サイクル1〜2の1日目、8日目、22日目および29日目、サイクル3+の1日目、15日目および29日目に評価;全アームで処置終了時および最後の投与100日後のフォローアップ来院に評価)。
抗腫瘍活性は、本試験の二次評価項目であり、RECIST 1.1に基づく客観的奏効率(ORR)および応答期間(DOR)により測定する。腫瘍応答は、最初の4評価についてはスクリーニング、治験薬の最初の投与から6週間毎(±1週間)および疾患進行まで12週間毎(±1週間)の腫瘍評価に基づく。
SアームおよびPアームで示された抗腫瘍活性
7対象を、アームS N2およびS N5の各々で処置し、進行または許容されない毒性まで、S(50mg、4週間投与、2週間休薬)と組合せてニボルマブ(それぞれ2mg/kgまたは5mg/kg)を受けた。DLTは観察されず、MTDに達しなかった。それゆえに、耐容性に基づき、S N5アームを、19のさらなる未処置患者に拡張した(Sアームについて計n=33)。アームPは、20対象をN2で登録し、P(1日800mg)と組合せた2mg/kgニボルマブを受けた。4つの4DLT(ALT/AST上昇[n=3]、疲労[n=1])が観察され、このアームの閉鎖に到った。実施した用量漸増を図11に説明する。
無増悪生存期間(PFS)の比率対時間のプロットを図13に示す。24週目のPFSの比率はアームSで79%およびアームPで55%であった。Sアーム(S+N;n=33)の中央PFSは55.3週間(95%CI 47.9−72.4週間)およびPアーム(P+N;n=20)は31.4週間(95%CI 12.1−48.1週間)であった。
腫瘍組織コレクションは、後向きであった(Sアームで33サンプルがおよびPアームで20サンプルが利用可能であった)。表面PD−L1発現を、ウサギ抗ヒトPD−L1 mAb、28−8を使用するDako免疫組織化学アッセイにより測定した(WO2013/173223号)。ポジティブPD−L1シグナルのカットオフは、腫瘍膜染色の1%または5%であった。PD−L1発現状態に従い分類した2アームの患者で観察されたORRを表6に示す。
グレード3〜4関連有害事象(AE)が、アームSの27/33患者(82%)およびアームPの14/20患者(70%)で観察された。大部分の毒性は、TKIの既知プロファイルと一致した。最も一般的な関連グレード3〜4 AEはアームSでALT上昇および高血圧(各18%)、低ナトリウム血症およびリンパ球数減少(各15%)およびアームPでALTおよびAST上昇および下痢(各20%)および疲労(15%)であった。グレード3間質性肺炎が1患者(アームS、N5)で生じた。グレード3〜4関連AEは、アームSで11/33患者(33%;3 N2、8 N5)およびアームPで4/20患者(20%)を治療中断させた。グレード5処置関連AEは観察されなかった。肝毒性は、処置アルゴリズムを使用して制御可能であった。
イピリムマブアームで示された抗腫瘍活性
mRCCを有する患者(有利なまたは中間MSKCCスコア;カルノフスキー・パフォーマンス・ステータス≧80%;未処置または先の免疫腫瘍学処置以外、あらゆる数の先の治療)を種々の処置アームに無作為化した。1mg/kgイピリムマブおよび3mg/kgニボルマブ(I−1アーム)で処置した47対象、3mg/kgイピリムマブおよび1mg/kgニボルマブ(I−3アーム)で処置した47対象および3mg/kgイピリムマブおよび3mg/kgニボルマブ(IN−3)で処置した6対象が応答について評価可能であった。大部分の患者(n=54;54%)は、先の全身治療を受けていた(I−1:22;I−3:26;IN−3:3)。表7に示すようにORがI−1およびI−3アームで観察された。ORRおよび対応する両側正確95%CIを、Clopper and Pearsonの方法により計算した。全体として、I−1アームにおける16(34%)患者が確認されたORを示し、I−3アームにおける17(36%)患者が確認されたORを示した。
PFSの比率対時間のプロットを図16に示す。中央PFSはI−1アームで30.3週間であり、I−3アームで36.0週間であった。PFS率は、カプランマイヤー法で概算し、対応する95%CIはGreenwood式に基づき導いた。
腫瘍組織サンプルを遡及的に集め、表面PD−L1発現をmAb、28−8を使用するDako免疫組織化学アッセイにより測定した(WO2013/173223号)。ポジティブPD−L1シグナルのカットオフは、腫瘍膜染色の1%または5%であった。PD−L1発現状態に基づき分類した種々の患者で観察されたORRを表8に示す。
グレード3〜4関連有害事象(AE)がI1、I3およびIN−3アームのそれぞれで16/47(34%)、30/47(64%)および5/6(83%)観察された。I1およびI3アームでそれぞれ5(11%)および10(21%)が、関連AEのために処置を中止した。IN−3アームでは有害事象により中止した患者はいなかったが、しかしながら全6患者が免疫介在有害事象の処置のためにステロイドを必要とし、さらなる患者をIN3アームに登録しなかった。最も一般的なグレード3〜4関連有害事象はリパーゼ上昇(20%、n=20)、ALT上昇(11%、n=11)、下痢(9%、n=9)およびAST上昇を含む(6%、n=6)。グレード3〜4間質性肺炎は見られなかった。

Claims (49)
- 腎癌に罹患している対象を処置する方法であって、対象に治療有効量の:
(a)プログラム細胞死(PD−1)受容体に特異的に結合し、PD−1活性を阻害する抗体またはその抗原結合部分である抗癌剤;および
(b)他の抗癌剤
の組合せを投与することを含む、方法。 - 腎癌が腎細胞癌である、請求項1に記載の方法。

- 顆粒球性骨髄系細胞がCD11b+/Ly6C−/Ly6G+発現により特徴付けられる、請求項45または46に記載の方法。
- 抗PD−1抗体またはその抗原結合部分および他の抗癌剤の投与が、未処置対象または抗PD−1抗体またはその抗原結合部分または他の抗癌剤の単剤療法で処置された対象と比較してT制御性細胞を減少させる、請求項1〜33および35〜47のいずれかに記載の方法。
- T制御性細胞がCD45+/CD4+/CD25+/FoxP3+発現により特徴付けられる、請求項48に記載の方法。
