Abstract
Description
本開示の特性および特徴は、添付された特許請求の範囲に具体的に指摘された要素および組み合わせによって実現および達成され得る。
この明細書に援用され、この明細書の一部を構成する添付図面は、本開示の例示的実施形態の例を示し、前記説明と共に、本開示の原理について説明する役目を果たす。
本開示の実施形態は、人体の組織壁に一定の熱エネルギーを送達するために構成された装置およびその方法に関する。例えば、開示した装置の実施形態は、喘息を含むが、これに限定されない慢性または可逆性の閉塞性肺疾患のうちの少なくとも1つの症状を低減するために、過剰な気道平滑筋(ASM:airway smooth muscle)を減少させる、縮小させる、減量する、または他の場合には排除するために、制御された方法で肺内の気道の壁に熱エネルギーを送達することを容易にし得る。
本願において開示した実施形態は、長尺状装置と一緒に用いることができ、前記長尺状装置は、患者の身体内の標的部位に開示した実施形態を導入および送達する手段として機能する内視鏡システムであり得る。しかしながら、本開示の実施形態は、気管支鏡、トロカール、内視鏡、カテーテルシース、腹腔鏡、結腸内視鏡、尿道鏡などのような他の導入装置、シースまたはシステムと一緒に用いられてもよいことに留意すべきである。
遠位キャップ18は可撓性の生体適合性材料から形成され得る。そのような材料の例としては、シリコーン、ニチノールのような形状記憶合金などが挙げられ得るが、これらに限定されるものではない。
作動部材22は任意の適当な生体適合性材料から形成され得る。これに代わって、作動部材22は、ロッド、ひも、結び目、または往復運動が可能な他の適当な機構であってもよい。適当な材料の例としては、ニチノール、他の形状記憶合金、ステンレス鋼、シリコーンなどが挙げられるが、これらに限定されるものではない。
支持体204の近位端部は、作動部材22のまわりに配置されたスペーサー206のような長尺状部材にさらに接続されるか、または他の場合には結合されてもよい。スペーサー206は脚部16の近位部分から遠位部分まで実質的に延在するように構成され得る。スペーサー206はバスケット14の過剰な拡張を防止するための支持を提供するように構成されてもよい。
図3は、圧縮位置にある停止部材201を備えた医療装置10の断面図である。医療装置10の近位端部のハンドルが、引っ張られるか、または他の場合には作動されて、近位方向の引力を作動部材22に印加するにつれ、突起202および作動部材22は医療装置10の近位端部に向かって移動し得る。その結果として、突起202は停止部材201を支持体204に対して押しつけて圧縮させ得る。作動部材22の近位方向の移動により、バスケット14は半径方向に外側へ拡張し得る(例えば、バスケット14の近位端部および遠位端部(図示せず)は、第1位置から第2位置へ移動し得る)。いくつかの実施形態において、脚部16の遠位端部は、突起202に結合されており、突起202が近位方向に移動するにつれ、近位方向へ移動し得る。第1位置において、バスケット14は圧縮されていてもよい(例えば、バスケット14は、閉鎖しているように見え、脚部16は、医療装置10の近位端部から遠位キャップ18の遠位端部までほぼ一直線に延びるように見え得る)。第2位置にある間、バスケット14は、脚部16が半径方向に外側に撓められるように拡張され得る。停止部材201、突起202および支持体204がない場合には、例えば医療装置10のハンドルを引っ張るか、または他の場合には作動させることによって作動部材22に過剰な力を印加してしまう可能があり得、例えばバスケット14の過剰拡張に起因する脚部16や気道壁への損傷をもたらす。しかしながら、突起202は支持体204によって制限される固定量だけ停止部材201を圧縮し得るので、脚部16が所定の限度を越えて移動することが防止され得る。これは、バスケット14の過剰拡張および脚部16の不適当な撓みを回避するのを助け得る。さらに、スペーサー206の固定された長さにより、脚部16は制御された方法で撓むことができ得る。従って、バスケット14の過剰拡張はさらに回避され、脚部16および気道への潜在的な損傷もさらに回避され得る。
Claims (20)
- 長尺状部材の近位端部と遠位端部との間に少なくとも部分的に延びる管腔を有する長尺状部材と、
前記長尺状部材の遠位部分に結合された複数の脚部と、
前記複数の脚部の各々に接続され、かつ前記長尺状部材の遠位端部の遠位側に位置する遠位端部品と、
前記遠位端部品に結合され、かつ前記管腔を通って前記複数の脚部の間に延びる作動部材であって、前記作動部材の往復運動は、前記複数の脚部を第1位置から第2位置に可逆的に移動させるように構成されている、作動部材と、
前記作動部材に結合され、かつ前記作動部材の移動を制限するように構成されている停止部材とを備える、医療装置。 - 前記停止部材は、前記長尺状部材の長手軸線に沿った前記作動部材の移動を制限するように構成されている、請求項1に記載の医療装置。
- 前記停止部材の少なくとも一部は、前記遠位端部品内に配置されている、請求項1に記載の医療装置。
- 前記停止部材は、前記作動部材の一部を受容するように構成された管腔を画定する、請求項3に記載の医療装置。
- 前記作動部材は、前記作動部材の遠位端部に配置された突起をさらに備え、前記突起は前記停止部材を圧縮させるように構成されている、請求項4に記載の医療装置。
- 前記突起は前記作動部材の遠位端部に取り付けられた円板であり、前記円板は前記停止部材の管腔の幅より大きな幅を有する、請求項5に記載の医療装置。
- 前記円板は前記停止部材の遠位端部に係合して、前記作動部材の移動を制限する、請求項6に記載の医療装置。
- 前記作動部材のまわりに配置され、かつ前記停止部材に接続された支持体をさらに備え、前記支持体は前記停止部材の近位端部を支持するように構成されている、請求項4に記載の医療装置。
- 前記停止部材は、前記複数の脚部の各々の少なくとも一端に形成されている、請求項1に記載の医療装置。
- 前記停止部材は、前記複数の脚部の近位側に配置されている、請求項1に記載の医療装置。
- 前記停止部材は、前記作動部材の2つの部分の間に配置されている、請求項1に記載の医療装置。
- 前記複数の脚部は第1位置において圧縮されており、かつ第2位置において長手軸線から外側に撓むように構成されている、請求項1に記載の医療装置。
- 前記停止部材は、
前記複数の脚部が第1位置にあるときには第1形態にあり、
前記複数の脚部が第2位置にあるときには第2形態にある、請求項1に記載の医療装置。 - 長尺状部材の近位端部と遠位端部との間に少なくとも部分的に延びる管腔を有する長尺状部材と、
前記長尺状部材の遠位端部から延びる複数の脚部であって、前記脚部の少なくとも1本はエネルギーを送達するための電極を備え、前記複数の脚部の各々は遠位端部を備える、複数の脚部と、
前記複数の脚部の各々の遠位端部に接続された遠位端部品と、
前記遠位端部品に結合され、かつ前記管腔を通って前記複数の脚部の間に延びる作動部材であって、前記作動部材は前記複数の脚部を第1位置から第2位置に可逆的に移動させるように構成されており、
前記作動部材に近位方向の力が印加されると、前記複数の脚部は第1位置から第2位置へ移動し、
前記近位方向の力が解放されると、前記複数の脚部は第2位置から第1位置へ移動する、作動部材と、
前記作動部材に結合され、かつ前記作動部材の移動を制限するように構成されている停止部材とを有する、医療装置。 - 前記停止部材の少なくとも一部は、前記遠位端部品内に配置されている、請求項14に記載の医療装置。
- 前記作動部材は前記作動部材の遠位端部に配置された突起をさらに備え、前記突起は、前記作動部材が近位方向に移動されると、前記停止部材に係合するように構成されている、請求項15に記載の医療装置。
- 医療装置を用いて体内管腔の壁にエネルギーを送達する方法であって、
前記体内管腔に医療装置を挿入する工程、
前記医療装置の遠位端部において拡張可能部材を半径方向に拡張するために作動部材を近位方向に引っ張る工程と、
前記作動部材に結合された停止部材を圧縮することにより、前記拡張可能部材の半径方向の拡張を制限する工程と、
前記拡張可能部材を介して前記体内管腔の壁にエネルギーを印加する工程とを備える、方法。 - 前記医療装置の長手軸線に沿った前記作動部材の移動を制限することにより、前記拡張可能部材の半径方向の拡張が制限される、請求項17に記載の方法。
- 前記停止部材の少なくとも一部は、前記医療装置の遠位端部品内に配置される、請求項17に記載の方法。
- 前記停止部材は、前記作動部材の一部を受容するように構成された管腔を画定する、請求項19に記載の方法。
