Abstract
Description
Claims (22)
- 心臓の循環補助のための器具であって、保持手段とポンプとを含み、
前記保持手段は、カテーテルによって、好ましくは血管内の方法を使用することによって、大腿の通路および/または経皮での経心室、経中隔、経心尖もしくは経静脈の通路を通して、心臓の左心室の流出路および/または右心室の流出路において、前記保持手段が心臓内に移植され得るように構成されていて、前記保持手段は、係留手段を含み、前記係留手段は、大動脈弁および肺動脈弁のそれぞれの心室側で、血液の流れる方向で、大動脈弁および肺動脈弁のそれぞれの下側の交連三角部で固定され得るものであり、
前記ポンプは、カテーテルによって、好ましくは血管内の方法を使用することによって、大腿の通路および/または経皮での経心室、経中隔、経心尖もしくは経静脈の通路を介して、前記ポンプが前記保持手段に固定され得るように構成されていて、前記ポンプは、
(a)前記保持手段が、大動脈弁の下側の交連三角部において、前記係留手段によって固定された後、前記保持手段へと取り外し可能に導入され得るか、あるいは、
(b)変形可能かつ拡張可能な前記係留手段にしっかりと接続される、
器具。 - 前記保持手段は、好ましくは少なくとも1つのフックを有する少なくとも1つの拡張可能なステントを含み、前記ステントは、必要に応じて存在するフックを有し、バルーンによって拡張可能であってもよく、あるいは、好ましくは、前記ステントは、左心室および/または右心室の交連三角部の繊維組織への係留を引き起こし得る自己拡張力を発生させて、前記ポンプが、取り外し可能に挿入され得るか、または前記保持手段にしっかりと接続されるように構成されている、請求項1に記載の器具。
- 前記ポンプは、電気モータを含み、好ましくは、前記電気モータは、固定子を形成する前記ステントに取り付けられている第1の手段と、回転子を形成する前記第1の手段の内側の中心に配置されている第2の手段とを含む、請求項1または2に記載の器具。
- 前記電気モータの第1の手段および第2の手段が、電気コイルおよび/または永久磁石を含み、前記電気コイルは、供給ラインによって、電力供給装置に接続され得る、請求項3に記載の器具。
- 前記ポンプは、フレキシブルサスペンション手段を含む電気モータを含み、前記フレキシブルサスペンション手段は、移植のために変形可能であり、前記フレキシブルサスペンション手段によって、前記電気モータを好ましくは取り外し可能に前記ステントに挿入することができ、前記電気モータは、供給ラインによって、電力供給装置に接続され得る、請求項1〜3の少なくとも1項に記載の器具。
- 前記ポンプは、回転シャフトを含み、前記回転シャフトにおいて、少なくとも1つのブレードが配置され、前記ブレードは、前記ポンプを移植するために変形可能であり、前記ポンプが挿入される状態では、前記回転シャフトと一緒に回転可能であるように構成されていて、作動位置では、自動的に真っ直ぐになるように、かつ/または真っ直ぐにされ得るように構成されていて、前記ポンプの作動中では、大動脈弁および肺動脈弁のそれぞれの方向に血液を送り出し得るように構成されている、請求項1〜5のいずれか1項に記載の器具。
- 前記ポンプは、回転式血液ポンプを含み、好ましくは、中心インペラを有する小型軸流ポンプあるいは小型遠心フローポンプを含み、そして/または、前記保持手段は、保持アームを有するステントグラフトを含み、前記ポンプは、前記保持アームを有するステントグラフトに取り付けられ、前記ポンプは、このようにして、大動脈弁または肺動脈弁の下側の左心室の流出路または右心室の流出路に固定されている、請求項1〜6のいずれか1項に記載の器具。
- 心臓の左心室の流出路および/または右心室の流出路に移植することができる心臓の循環補助のための器具を電源および/または制御装置に接続するための電気供給ラインであって、特に請求項1〜7の少なくとも1項に記載の器具への電気供給ラインであり、前記電気供給ラインは、好ましくは、経皮的に配置されるガイドワイヤによって心臓から引っ張ることができるように構成されていて、なおかつ前記経皮的に配置されるガイドワイヤのための心臓の開口部を封止することができるように構成されている、電気供給ライン。
- 前記電気供給ラインが、心臓の左心室および/または右心室の頂部を通して心臓の左心室および/または右心室から、好ましくは、さらに患者の皮膚表面の任意の箇所へとガイドされ得るように構成されている、請求項8に記載の電気供給ライン。
- 前記電気供給ラインが、心臓の左心室および/または右心室の側面の壁部を通して、あるいは左心室および/または右心室の下方の壁部を通して、心臓の左心室および/または右心室から、好ましくはさらに患者の皮膚表面の任意の箇所へとガイドされ得るように構成されている、請求項8に記載の電気供給ライン。
- 前記電気供給ラインが、心中隔の内部に延び、好ましくはさらに患者の皮膚表面の任意の箇所へとガイドされ得るように構成されている、請求項8に記載の電気供給ライン。
- 前記電気供給ラインが、心臓の左心室から、心中隔を通して、右心室、次いで、さらに右心室の自由壁を通して、好ましくはさらに患者の皮膚表面の任意の箇所へとガイドされ得るように構成されている、請求項8に記載の電気供給ライン。
- 前記電気供給ラインが、心臓の左心室から、心中隔を通して、右心室へとガイドされ、そこから、三尖弁を通して、右心房へとガイドされ、次いで、大静脈を通して、大きな末梢静脈へとガイドされ、皮下組織の貫通口を通して、好ましくはさらに患者の皮膚表面の任意の箇所へとガイドされ得るように構成されている、請求項8に記載の電気供給ライン。
- 前記電気供給ラインが、心臓の左心室から、僧帽弁を通して、左心房へとガイドされ、次いでさらに、心房中隔を介して、右心房へとガイドされ、次いで、大静脈を介して、大きな末梢静脈へとガイドされ、皮下組織の貫通孔を通して、好ましくはさらに患者の皮膚表面の任意の箇所へとガイドされ得るように構成されている、請求項8に記載の電気供給ライン。
- 心臓の循環補助のための器具、好ましくは請求項1〜7の少なくとも1項に記載の器具と、供給ライン、好ましくは請求項8〜14の少なくとも1項に記載の供給ラインとを取り扱うためのシステムであって、第1のカテーテル手段を含み、前記第1のカテーテル手段が、前記器具を移植するため、かつ/または患者の心臓の左心室および/または右心室に前記デバイスを新たに配置または調整するためにある、システム。
- 前記カテーテル手段が、ガイドワイヤを含み、前記ガイドワイヤは、直接あるいは静脈系を通して、心臓から前記供給ラインを引っ張るために使用され得るように構成されている、請求項15に記載のシステム。
- コネクタであり、特に、請求項8〜14のいずれか1項に記載の電気供給ラインのためのプラグインコネクタであって、前記コネクタは、患者の皮下組織に配置されるために構成されていて、前記コネクタは、電気供給ラインの移植される部分と、外部にガイドされる前記電気供給ラインの部分との間での取り外し可能な電気接続を提供するために構成されている、コネクタ。
- 固定手段であり、特に請求項1〜7のいずれか1項に記載の器具のための固定手段であって、心臓内での心臓補助システムのための拡張可能なステントを含み、前記ステントは、最初に挿入された後、血管内のワイヤを反対方向に引っ張ることによって、部分的に変形して、前記器具が、左心室の流出路および/または右心室の流出路に再配置および/または調整された後、再拡張するために構成されている、固定手段。
- 各心室の出口弁の弁下位置に好ましくは請求項1〜7のいずれか1項に記載の左心室または右心室を援助する器具を移植するための最小侵襲の方法であって、
大きな動脈もしくは静脈を通して、あるいは直接的に心臓を通して、比較的に大きな器具を挿入する工程、および
循環補助のために心臓の弁下位置に前記デバイスを配置する工程
を含む、方法。 - 血管内でのオーバー・ザ・ワイヤー技術を用いる各心室の出口弁の弁下位置に、好ましくは請求項1〜7のいずれか1項に記載の左心室または右心室を援助する器具を移植するための方法であって、弁下位置の所望の着地位置において正確に展開するための正確な器具の制御のために、ガイドワイヤの両端部における制御を可能にするための1つまたは2つの別々のポイントにおいて、血管系または直接的に心臓にアクセスする工程を含む、方法。
- 前記ワイヤに反対方向のトラクションをかける工程とともに、前記器具の位置を調整して補正する工程をさらに含み、前記ワイヤの両端部へのトラクションによって、前記器具の部分的な変形が可能となる、請求項20に記載の方法。
- 前記器具のドライブラインによる心臓または血管系のアクセス穿刺部位の封止および止血の方法であって、前記ドライブラインは、前記心室に前記器具を配置するための手順に使用されるアクセスカテーテルよりも大きな直径を有するものであり、前記ドライブラインをワイヤに沿って引っ張る工程と、前記ドライブラインによる血管/心臓のアクセス部位を封止する工程とを含む、方法。
