代謝過程の途絶は、化学療法のしばしば起こる副作用である。事実、高投与量の化学療法で使用される薬剤は、多くの細胞過程に影響を与える。肝臓や消化管などの、化学感受性組織に集中している代謝過程は、とくに途絶の大きな危険性に面している。
BMTのよく見られる合併症は、肝内性肝静脈閉塞症(HVOD)である。HVODは黄疸、肝臓の大きさの増大、及び正常な肝臓血流の阻害に関連する。HVODは、患者の約20から40%で起こり、深刻な罹患率と死亡率に関連する。
カルバミルリン酸合成酵素I(CPSI)は、尿素回路を介した尿素生成の最初の関与段階を触媒する、律速酵素である。CPSIは高度に組織特異的であり、ほとんど肝臓と腸に限定されて産生され、機能する。ゲノム的にコードされたCPSIは、細胞質内で産生され、ミトコンドリアへと輸送され、そこで、成熟した160kDaの単量体に切断される。該酵素は、補助因子であるN‐アセチルグルタミン酸(NAG)を使用して、2つのATP分子の消費で、アンモニアと炭酸水素塩とを結合させ、カルバミルを形成する。

いくつかの実施態様において、ここで開示される主題は、本明細書中に記載したカルバミルリン酸合成酵素I(CPSI)遺伝子の多型性に基づいた治療方法に関連するものである。前記治療方法は、次善尿素回路(例えば、骨髄移植毒性)によって媒介、又は調整される疾患の治療、及び予防において、一酸化窒素前駆体の投与、並びにここで開示される主題の単離精製されたポリヌクレオチドを使用する、遺伝子治療アプローチを含む。
また、ここで開示される主題の目的は、対象の組織に由来するゲノムDNA、及びcDNAを含む、核酸の分析を通して獲得される情報に基づいて、脊椎動物の対象、特にヒト対象におけるCPSI表現型の決定を提供することである。
ここで開示される主題の他の目的は、CPSI表現型を決定する、迅速な技術を提供することである。
ここで開示される主題のさらなる目的は、該CPSI多型に関連し、かつ付随した臨床的な症候群の診断、及び治療方法を提供することである。
ここで開示される主題の目的のいくつかは、上文に規定されており、他の目的は、以下に最もよく記載された添付図面、及び実施例とともに、記載が進むにつれて明らかとなるであろう。
ここで開示されているものは、カルバミルリン酸合成酵素I (CPSI)の多型の驚くべき発見であり、該酵素は、尿素回路の最初の律速段階を触媒する。特に、該多型は、CPSIにおいて、1405番目のアミノ酸(異型接合性=.44)のスレオニン/アスパラギンのアミノ酸置換によって特徴付けられる。
また、ここで開示されているものは、CPSI遺伝子の一塩基置換が、CPSIの多型の原因になっているという、驚くべき知見である。特に、CPSI遺伝子のエキソン36におけるCからAへのトランスバージョンは、ACCからAACにトリプレット暗号を変化させ、そのコード化CPSIポリペプチドによってコードされているT1405N変換を引き起こす。
CPSIに関する現在利用可能な構造情報のほとんどは、ラットCPSI酵素の研究に由来する。ラットCPSI酵素、及びヒトCPSI酵素はそれぞれ、1500残基の単一ポリペプチドから構成され、かつ約95%の配列同一性を示す。ラットCPSIポリペプチド、及び核酸配列情報は、Nyunoya, H.らの論文(Journal of Biological Chemistry 260:9346-9356 (1985))、及びGENBANK(登録商標)のアクセッション番号AH005315, M12335, M12328, M12327, M12326, M12325, M12324, M12323, M12322, M12321, M12320, M12319, M12318、及びM11710に開示されており、本明細書中に引用により組み込まれている。ラットCPSIに関する構造情報は、配列相同性、並びに基質、及び補助因子の結合研究に由来している;しかし、利用できる結晶データはない。

ヒトのCPSI mRNAは、165kDa、1500アミノ酸のプレタンパク質をコードしている。この前駆体のアミノ末端は38残基を含み、8個の塩基性残基、及び1個の酸性残基を含み、該成熟酵素の開始前にPro−Glyの4残基を有する。(Nyunoya, H.らの論文(Journal of Biological Chemistry 260:9346-9356 (1985)); Lagace, M.らの論文(Journal of Biological Chemistry 262:10415-10418 (1987))を参照されたい)この高度に保存されたシグナル配列は、酵素が、ミトコンドリアマトリクスに入ることを促進し、次にそこで切断され、160kDaの成熟酵素を産生する。
CPSIの酵素活性は、妊娠5〜10週までのヒト胎児の肝臓において初めて検出された(Moorman, A. F.らの論文(Histochemical Journal 22:457-468 (1990))。妊娠20週までに、CPSI濃度は、正常な成人での濃度の約50%に達し、生まれるまでそれを維持し、その後20歳の成人での濃度へと徐々に増加する(Raiha, N. C. R. and Suihkonen, J. の論文(Acta Paediatrica Scand 57:121-127 (1968)))。CPSIの組織発現は、本来肝臓に限定され、腸、及び腎臓において微量な活性を有する。肝臓が、成人において、その成熟房状構造に成長したとき、CPSIは、小静脈門脈末端の周りの実質細胞に区分化される(Moorman, A. 本明細書中、及び特許請求の範囲で使用されているように、"多型"という用語は、母集団における、2以上の一般的に決定された選択的配列、又は対立遺伝子の発生を意味する。多型マーカーは、相違が発生する位置の遺伝子座である。典型的なマーカーは、少なくとも2つの遺伝子座を有し、各々は1%よりも高い頻度で起こる。多型遺伝子座は、一塩基対ほど小さくてもよい。
身体試料は、該CPSI遺伝子が、4340番目の塩基において、CからAへのトランスバージョンを含むかどうかを決定するために試験することができる。試験に適した身体試料は、生体検査から得られたDNA、RNA、又はタンパク質を含むものを含有し、例えば、生体検査組織の肝臓、及び腸;又は出生前の血液;又は胚組織を含む。
変性、アニーリング、及び伸長産物合成のステップは、必要なだけ繰り返して、検出に必要な程度に標的多型性遺伝子座核酸配列を増幅する。生成される特異的核酸配列の量は、指数関数的種類に蓄積するであろう(PCR. A Practical Approach, ILR Press, Eds. McPhersonらの文献. (1992))。
ここで開示される主題のいくつかの実施態様において、対象由来試料と該CPSI多型の存在を検出する試薬とを接触させ、かつ該試薬を検出することを含む、高アンモニア血症(その危険性がある)の素因、又は高い感受性を有している対象を診断する方法を提供する。
ここで開示される主題に従って、精製され、かつ単離されたCPSI−コード化ポリヌクレオチド、及びそれによってコードされたCPSIポリペプチドを提供する。特に提供されるCPSI−コード化ポリヌクレオチドは、CPSI遺伝子の4340番目の塩基、すなわちエキソン36内におけるCからAへのトランスバージョンを含む、CPSIコードポリヌクレオチドを含む。それは、トリプレット暗号をACCからAACに変化し、かつコード化CPSIポリペプチド中でT1405N変換を導く。特に、該T1405N変換を含むコード化CPSIポリペプチドも提供する。 従って、対立遺伝子多型ポリヌクレオチド、及びそれよってコードされたポリペプチドを、ここで開示される主題に従って提供する。さらにまた、前記CPSIポリペプチドをコードしているCPSI−コード化ポリヌクレオチドであるように、生物学的に活性なCPSIポリペプチドを、ここで開示される主題に従って提供する。例証的な生物学的活性は、該尿素回路の第一段階を仲介する生物学的活性、及び抗CPSI抗体と交差反応する生物学的活性を含む。
いくつかの実施態様において、提供されるCPSIポリヌクレオチド、及びポリペプチドは、脊椎動物、及び無脊椎動物から単離される。従って、制限されないが哺乳類、酵母、及び細菌ホモログを含むCPSIホモログを、ここで開示される主題に従って提供する。CPSIメンバーの代表的な哺乳類ホモログは、ラット、及びヒトホモログを含むが、これに限定されない。
パーセント類似性は、例えば、ウイスコンシン大学遺伝学者コンピューターグループ(the University of Wisconsin Geneticist Computer Group)から利用可能なGAPコンピュータープログラムを使用した配列情報の比較によって決定され得る。該GAPプログラムは、Smith ら(Adv. Appl. Math. 2:482 (1981))によって改正された、Needlemanら(J. Mol. 先に記載したように、改質と変化を、本明細書中で記載したCPSIタンパク質、及びペプチドの構造で行い、類似、又は他の所望の特性を有する分子を得てもよい。例えば、特定のアミノ酸は、細胞の核中のような構造の相互作用能の大きな損失がなければ、タンパク質構造中の他のアミノ酸に置換されてもよい。相互作用能、及びタンパク質特性は、タンパク質の生物学的機能活性を規定するので、特定のアミノ酸配列置換を、タンパク質配列(又は、もちろん、その基礎をなすDNAコード配列)内で行い、かつ類似、又は同等の対抗する特性(例えば、アンタゴニスト対アゴニスト)を有するタンパク質を得ることができる。従って、それらの生物学的有用性、又は活性の大きな損失がなければ、該CPSIタンパク質、及びペプチド(又は基礎をなすDNA)の配列において様々な変化を行うことができる。
議論は、アミノ酸変化により生じる機能的に均等なポリペプチドに焦点を当てているが、遺伝暗号が変質し、かつ2以上のコドンが、該同一アミノ酸をコードし得ることも考慮して、これらの変化が、該コード化DNAの変更によってもたらされ得ることは明らかであろう。
本明細書中に記述したCPSIタンパク質、及びペプチドに対する改質は、部位特異的変異誘発法などの技術を用いて行ってもよい。部位特異的変異誘発法は、基礎をなすDNAの特異的突然変異誘発を介して、個々のペプチド、又は生物学的機能均等性のタンパク質、又はペプチドの調製に有用な技術である。該技術は、例えば、1以上の先の考慮を組み込み、DNAに1以上のヌクレオチド配列変化を導入することによって配列変異を調製し、かつ試験する、迅速な能力を提供する。部位特異的突然変異は、所望される変異のDNA配列、並びに十分な数の近接したヌクレオチドをコードしている特異的オリゴヌクレオチド配列の使用を介して、変異の生成を可能にし、横断された欠失接合点の両端において、安定な二本鎖の形成に十分なサイズ、及び配列複雑性のプライマー配列を提供する。
典型的に、長さが約17〜30ヌクレオチドのいくつかの実施態様のプライマーは、変更した配列の両端の該接合点上の約5〜10残基とともに使用され得る。
本明細書中で記述したCPSIペプチジル化合物に加えて、本発明者らは、他の立体的類似化合物を考案し、該ペプチド構造の重要部分を模倣することができることも意図している。そのような化合物を、ここで開示される主題のペプチドとして、同じ様式で使用してもよい。従って、これも、機能均等性である。構造的機能均等物の創出を、当業者に公知のモデリング、及び化学的デザインの技術によって達成することができる。前記立体的類似構造の全てが、ここで開示される主題の範囲内に含まれることが理解されるであろう。
該遺伝子自身を使用して、該遺伝子産物を導入する場合、導入における従来の方法は、その関連した制御配列と共に所望の遺伝子を組み込んだ組換えベクターの使用を介するであろう。組換えベクターの調製法は、当業者に周知であり、例えば、特に本明細書中に引用により組み込まれているSambrookらの論文(1989)など、多くの参考文献に記述されている。

CPSI遺伝子を、ここで開示される主題に従って、遺伝子療法に使用することができる。宿主細胞への核酸のリポソームのトランスフェクションを含む、例証的な遺伝子療法は、米国特許第5,279,833号、第5,286,634号、第5,399,346号、第5,646,008号、第5,651,964号、第5,641,484号、及び第5,643,567号に記載され、それぞれの内容は、引用により本明細書中に組み込まれている。
窒素クリアランスの役割に加えて、該尿素回路は、強力な血管拡張剤である一酸化窒素(NO)前駆体として機能するアルギニンの体の本質的な源である。次善尿素回路機能の治療方法は、シトルリンなどの一酸化窒素前駆体の投与による治療を含む、ここで開示される主題に従って提供される。典型的に、該次善尿素回路機能は、本明細書中で開示した多型に関連がある。該次善尿素回路機能は、高アンモニア血症、又はシトルリン、及び/又はアルギニン産生低下をさらに含む。

いくつかの実施態様において、ここで開示される主題は、ここで開示される主題のポリペプチド、又はポリヌクレオチド、及び生理学的に許容し得るキャリアーを含む、医薬組成物を提供する。いくつかの実施態様において、医薬組成物は、生物学的に活性なCPSIポリペプチドをコードしているポリヌクレオチドを含む。もしくは、提供される医薬組成物は、先に記載したように、用量中にシトルリン、又はアルギニンを含む。
注射可能な製剤、例えば、無菌の注射可能な水溶液、又は油性の懸濁液は、適切な分散剤、又は湿潤剤、及び懸濁化剤を用いる公知技術に従って配合される。また、該無菌の注射可能な製剤は、例えば1,3−ブタンジオールの溶液のような、無毒性の非経口に許容し得る希釈剤、又は溶媒の、無菌の注射可能な溶液、又は懸濁液であり得る。
いくつかの実施態様において、ここで開示される主題は、ここで開示される主題のポリペプチド、又はポリヌクレオチドを用いた抗体免疫反応性を提供する。いくつかの実施態様において、ここで開示される主題の抗体は、モノクローナル抗体である。抗体の調製、及び特徴付け手段は、当業者に周知である(例えば、抗体研究室マニュアル(Antibodies A Laboratory Manual)、E. Howell、及びD. Laneの文献、Cold Spring Harbor Laboratory, 1988を参照されたい。)。いくつかの実施態様において、抗体は、該CPSI多型を含む、CPSIの異なる形態間を区別する。
当業者に周知のように、特定の免疫原に対する免疫原性を、アジュバントとして公知の免疫反応の非特異的刺激物質の使用により増強することができる。例証的なアジュバントには、完全フロイントアジュバント(Freund's adjuvant)、不完全フロイントアジュバント、水酸化アルミニウムアジュバントがある。
骨髄腫細胞を、融合促進に適した条件下で、ここで開示される主題の抗原/ポリペプチドを注入したマウス、又はラットの脾臓由来の正常抗体産生細胞と結合させる。融合条件は、例えば、ポリエチレングリコールの存在下である。得られた融合細胞は、ハイブリドーマ細胞である。このような骨髄腫細胞、ハイブリドーマ細胞を、培養液中で無制限に増殖させる。
もう1つの方法として、ここで開示される主題は、ここで開示される主題のポリペプチドの検出方法を提供する。該方法は、該ポリペプチドを、上記方法に従って調製した抗体と免疫反応させて、抗体ポリペプチド接合体を形成し、かつ該接合体を検出することを含む。

ここで開示される主題は、生物学的試料の、CPSIポリペプチドの存在に対するスクリーニング方法を提供する。いくつかの実施態様において、該CPSIポリペプチドは、尿素回路における活性、抗CPSI抗体との交差反応性、又はここで開示される主題に従う他の生物学的活性を有する。スクリーニングされる生物学的試料は、細胞外又は細胞内体液、或いは、細胞若しくは組織抽出物又はホモジネートなどの生体液であり得る。また、生物学的試料は、単離された細胞(例えば培養液中)、或いは、組織試料又は組織学試料中などの回収細胞であり得る。組織試料を、液体培地中で懸濁化するか、又は顕微鏡用スライドなどの固体支持体上に固定することができる。肝臓組織は、特に意図された組織である。
スクリーニングアッセイ方法に従って、生物学的試料を、存在が評価されるポリペプチドとの抗体免疫反応に曝露する。通常、曝露は、該抗体、及び候補ポリペプチド、双方を含む液体培地中で混合することにより達成される。該ポリペプチドとともに該抗体、又は該試料、どちらかが、固体支持体(例えば、カラム、又はマイクロタイタープレート)に付くことができる。
該生物学的試料を、生物学的反応条件下で、抗体ポリペプチド接合体形成に十分な時間、該抗体に曝露する。生物学的反応条件は、イオン組成、濃度、温度、及びpHなどを含む。
曝露時間は、とりわけ、使用される該生物学的条件、抗体及びポリペプチド濃度、及び該試料(例えば、体液、又は組織試料)の性質で変わるであろう。曝露時間決定手段は、当業者に周知である。通常、該試料が体液であり、かつその試料中のポリペプチド濃度が約10−10Mである場合、曝露時間は、約10分〜約200分である。
いくつかの実施態様において、該抗体に付着されている標識を検出することにより、検出を達成する。例証的かつ周知の前記標識には、放射性ラベル(例えば、32P、125I、14C)、二次抗体、又はホースラディシュペルオキシダーゼがある。抗体への標識の付着方法は、技術的に周知である。商業的キットが利用できる。
他の態様において、ここで開示される主題は、CPSIポリペプチドとの抗体免疫反応性の存在に対する、生物学的試料のスクリーニング方法を提供する。いくつかの実施態様において、該CPSIポリペプチドは、尿素回路における活性、抗CPSI抗体との交差反応性、又はここで開示される主題に従う他の生物学的活性を有する。前記方法に従って、生物学的試料を、生物学的条件下で、抗体−ポリペプチド接合体形成に十分な時間で、CPSIポリペプチドに曝露し、該形成される接合体を検出する。
核酸分子、特にプローブ分子を、ここで開示される主題のコード化CPSIポリペプチドの被検核酸源に対するオリゴヌクレオチドとして、ハイブリダイズに使用することができる。好ましくは、該CPSIポリペプチドは、尿素回路における活性、抗CPSI抗体との交差反応性、又はここで開示される主題に従う他の生物学的活性を有する。該プロービング(probing)は、該オリゴヌクレオチドを、プロセシングするCPSI遺伝子の被検核酸源にハイブリダイズすることにより達成される。いくつかの場合において、該プローブは、単一プローブのみで構成され、他の場合において、該プローブは、特定のアミノ酸配列、又は該ポリペプチドの配列をベースとしたプローブの収集物で構成され、かつ該遺伝暗号に固有の重複性の多様性を説明する。
他の態様において、ここで開示される主題は、生物学的試料中における、ここで開示される主題のポリペプチドの存在を検出するための診断アッセイキットを提供する。該キットは、該ポリペプチドと免疫反応することができる一次抗体を含む第一容器を含有する。

下記物質、及び方法を、実施例1〜3のそれぞれに使用する。また、追加の物質、及び方法を、各実施例に記載する。
(臨床/患者採用)
移植後の急性肺損傷、及び多臓器障害の機序を理解する目的の該BMT肺外傷後移植(BMT-Lung Injury Following Engraftment)(LIFE)研究において、バンダービルト大学医療センター(Vanderbilt University Medical Center), Nashville, TennesseeでBMTを受けた200以上の患者を登録した。悪性腫瘍の治療のためにBMT、又はPBSCTを継続的に受けている患者の同意を求めた。臓器不全(HVODを含む)、及び反転の規定を、予期して規定し、かつ入院中に同時にデータを回収した。研究登録者(化学療法を受ける前)と、切除化学放射線治療終了後の移植日(骨髄注入前)との血漿、細胞ペレット、及び尿を回収した。
血液、及び尿を、回収後すぐに遠心分離した。全ての試料を氷浴し、次に分析まで−70℃で保存した。これらの保存条件下で、グルタミン、システイン、及びホモシステインが減少することが知られているので、これらを該分析に使用しなかった。血漿アミノ酸を、バンダービルト診断研究室(Vanderbilt Diagnostic Laboratories), バンダービルト大学, Nashville, Tennesseeで測定した。簡潔に言うと、スルホサリチル酸でタンパク質を沈殿させ、かつ0.45μm ACRODISC(商標)4フィルターを通して濾過することにより、血漿のタンパク質フリー抽出液を調製した。Beckmann 7300アミノ酸分析器(Beckmann, Palo Alto, California)の4成分pH、及びイオン強度勾配クエン酸リチウム緩衝液系を用いて、陽イオン交換クロマトグラフィーによりアミノ酸を分離した。ニンヒドリンを用いたアミノ酸のカラム誘導体化後に、570nmで一級アミンアミノ酸、及び440nmで二級アミンを検出した。標準物質で装置を較正することにより定量した(Sigma, St. Louis, Missouri)。
血漿アミノ酸値を、平均±SEMとして示した。スチューデントt検定を用いて、基準と化学療法後のアミノ酸値とを比較した。対立遺伝子頻度を、カイ二乗分析を用いて、HVOD患者とそうでない患者とを比較した。
(患者)
患者を、バンダービルト大学でBMTリフト研究(BMT Lift Study)に登録した患者から識別した。DNAを、移植前血液、又はスパン尿試料(spun urine samples)から単離した。HVOD状態を、バルティモア基準(Baltimore criteria)を用いて決定した。

CからAへのトランスバージョンは、ピリミジンCをプリンAに置き換え、Msl Iサイトを破壊する。CPSIの第35番目のイントロン内からのプライマー、及び該CPSI遺伝子のエキソン36からの外来プライマーの使用は、該変化を含む領域を包含した387bpフラグメントを確実にPCR増幅する。この組合せは、強力な増幅を与える。また、PCR Ready-to-Go(商標)ビーズを、増幅に使用する(Pharmacia)。
該ゲル中のDNAフラグメントを検出するために、銀染色技術を適用した。この安価で迅速な方法は、電気泳動後すぐにバンドの視覚化を可能にする。
(統計分析)
有意な試料サイズを、該結果にカイ二乗分析を実施して得た。Hardy-Weinburg式を使用して、該遺伝子型の予想される頻度を計算した(p2+2pq+q2)。P値は、自由度2を用いて、標準カイ二乗の表から得た。
(CPSIエキソン多型(T1405N)の対立遺伝子は、HVODの存在、又は非存在に対して、Hardy-Weinburg平衡にない。)
ここで開示される主題に従って、該CPSImRNA(約.44異型接合性)の3'末端近くの共通の多型を同定した。この変化の配列分析は、該トリプレット暗号がACCからAACに変化する、4340番目の塩基のCからAへのトランスバージョンを示していた。これは、1405番目のアミノ酸で、スレオニンからアスパラギンへの置換を生じる(本明細書中では、"T1405N"と呼ぶ)。このスレオニンは、アロステリックドメイン内に存在し、それは、該サイン配列(signature sequence)PV(A/S)WP(T/S)(A/Q)Eに先行し、補助因子n−アセチル−グルタミン酸(NAG)の結合に重要な配列である
表2、及び3は、HVOD+、及びHVOD−の患者の間における、該T1405N多型の遺伝子型分析の結果を示す。該C対立遺伝子(本明細書中では、CPSIa対立遺伝子、又はスレオニンコード化対立遺伝子と呼ぶ。)は、実験集団において、頻度.62を有し、かつA対立遺伝子(本明細書中では、CPSIb対立遺伝子、又はアスパラギンコード化対立遺伝子と呼ぶ。)は、頻度0.38を有する。表中のカイ二乗値は、4.3(P=0.1)であり、該多型は、おそらく、HVODに対してHardy-Weinburg平衡にない
ことを示している。従って、これらは、HVODを有するBMT患者における、該T1405N対立遺伝子の分布の不平衡の証拠を提供し、該多型が、BMT毒性に感受性のある対象の同定に使用することができることを示している。
骨髄移植毒性は、著しい罹患率、及び死亡率を示す。HVODは、BMT患者の乏しい予後診断に関連する。この研究は、該CPSI酵素とHVODの発症との間の関連性を評価するために着手された。該T1405N多型は、CPSI機能に影響を及ぼす。該集団のその広い分布は、両形態が、正常状態の適切な尿素回路機能を提供することを示唆している。代謝のストレッサーの添加(高投与量の化学療法など)は、有効閾値以下の低いCPSI効率を供給する。従って、該データの分析は、HVODが、該アスパラギンコード化対立遺伝子よりも該スレオニンコード化対立遺伝子を有する患者において発症するようであることを示唆している。該スレオニンコード化対立遺伝子は、げっ歯類のCPSIに共有される。
(骨髄移植後合併症患者のカルバミル合成酵素Iの生化学的、及び遺伝的変更)
骨髄移植(BMT)、及び末梢血幹細胞移植(PBSCT)は、選ばれた悪性腫瘍の一次療法として次第に使用されるようになった。造血再構成するための幹細胞支持の使用は、潜在的な致死的癌を根絶する実質的な増加投与量の化学療法を可能にする。感染予防、及び移植片対宿主疾患予防の改善に関して、化学療法に誘発される臓器機能不全が、この治療の広範囲使用の重大な障害として残っている。
(臨床/患者採用)
移植後の急性肺損傷及び多臓器障害の機序を理解する目的の整合臨床生化学実施調査である、該骨髄移植肺外傷後移植(Bone Marrow Transplant-Lung Injury Following Engraftment)(BMT−LIFE)研究において、過去3年に渡って、バンダービルト大学 医療センターでBMTを受けた200患者を登録した。臓器不全、及び反転の規定を、予期して規定し、かつ入院中同時に、及びBMT60日後までにデータを回収した。除外基準は、活性ウイルス、及び造血幹細胞支持(PBSCT、又はBMTのどちらか)を用いた先の増加投与量の治療を含む。
60患者の−8、及び0日(前処置、及び移植日)の冷凍保存した血漿試料のアミノ酸分析を行った。最初に、予備実験のために患者試料をランダムに選択した。;次に分析試料を、CPS−Iの該SNP AA遺伝子型(下記参照)の特別患者、及びHVOD、及びALIのBMT後合併症を有する追加の患者を含めて、特に充実させた。スルホサリチル酸でタンパク質を沈殿させ、かつ0.45μmのAcrodisc4(Gelman Sciences, Ann Arbor, Michigan)を通して濾過することにより、血漿のタンパク質フリー抽出液を調製した。
試料を、除タンパクし、かつカドミウムビーズとともにインキュベートして硝酸塩を亜硝酸塩に変換した後に、患者のサブグループにおいて、改質グリース試薬を使用して、血漿NOxを測定した。
(T1405N多型の検出)
CPS1の第36番目のエキソン(CGGAAGCCACATCAGACTGG(配列番号:15))、及びイントロン(GGAGAGTGAAACTTGACAATCATC(配列番号:16))内からのオリゴヌクレオチドプライマー、及びゲノムDNA変化含有領域を含む251bpフラグメントを確実に増幅するポリメラーゼ連鎖反応(PCR)を、バフィーコート調製、又は尿沈渣から得た。プライマーの組合せは、PCR Ready−to−Goビーズ(Pharmacia)、及び下記のPCRサイクル条件を用いて、再現可能な増幅を与えた:55℃で1分アニール、75℃で1分伸長、及び94℃で1分変性の35サイクルである。
化学療法前と後との血漿アミノ酸濃度、及び患者グループ間の濃度を、スチューデントt検定、又はウィルコクスン順位和検定(Wilcoxon's Rank Sum Test)を用いて比較した(該データが、正常に分布していない場合)。CPSIの遺伝子型の分布を、全グループの対立遺伝子頻度を計算し、かつP2分析を用いて、特に選択したサブグループのHardy-Weinberg平衡の証拠を探索することにより、グループに渡って比較した。特定の臨床転の存在、及び非存在により分けられた患者グループ(例えば、HVOD、ALI、及び死亡)において、感受性、特異性、予測値、及び相対リスク評価を、特定のアミノ酸値を用いて構築された2×2分割表から得た。
BMT−LIFE研究において、200患者を登録した。52%が自家移植を受け(平均年齢46±1歳)、48%が同種異系移植片を受けた(平均年齢40±1歳)。同種移植を受けた患者の24%が、適合非血縁ドナーからの移植片を受けた。自家グループの患者のほぼ3分の2が女性であり、この集団の乳がんの増加罹患率を反映している。移植の適応症は多様であるが、該患者の79%が、乳がん、白血病、又は非ホジキンリンパ腫に対して移植された。BMT前に使用される異なる準備措置は、CTC(シクロホスファミド、チオテパ、カルボプラチン)、BuCy(ブスルファン、シクロホスファミド)、CVP16TBI(シクロホスファミド、エトポシド、全身照射)、CBVP16(シクロホスファミド、ビス−クロロエチルニトロソ尿素、エトポシド)、TC(チオテパ、シクロホスファミド)を含む。
本実施例にあるデータは、BMT後の患者のHVODとALIとの間の密接な関連性を
反映する。HVODのほぼ3分の2の患者は、ALIの基準値を満たしている。この研究において、ALI患者の68%(26/38)は、機械喚起を必要とする。Rubenfelt、及びCrawfordは、重要な生存を報告しており、BMT後の機械喚起を必要とする患者の
%のみが、抜管、続いて30日生存で該病院から退院と規定している。Rubenfeld, G. D.、及びCrawford, S. W.の論文, Annals of Internal Medicine (1996) 125:625-33を参照されたい。
HVODは、増加投与量化学療法の主な投与量制限毒性がある。BMTの3週間以内に発症する体液貯留、黄疸、腹水、及び有痛性肝拡大により臨床的に特徴付けられる。これらの臨床的基準を満たす、これらの非生存患者の検視研究は、>80%のケースにおいて、組織学的確認を提供し、かつ増進した局所血栓症が、HVODの病因の起因事象であり得るという考えと一致している。
(アルギニン/シトルリン補充療法)
尿素回路産生物(アルギニン、及びシトルリン)のさらなる低下、及び前駆体(アンモニア、グルタミンなど)の増加は、BMT関連毒性に寄与する多型に起因する。該BMTライフ研究の一部として、BMTを受けた10患者において、シトルリン、及びアルギニン濃度を測定した。
BMTにおいて使用される高投与量化学療法は、尿素回路酵素の正常機能を崩壊させ、かつ又はHVOD関連毒性の発症、どちらかに寄与する。この情報をさらに評価するために、誘導化学療法終了前、及び後に、BMTを受けた10患者からの保存血漿の分析を行った。アミノ酸プロフィールを、全ての試料から決定した。特定の注目は、該尿素回路中間体シトルリン、アルギニン、及びオルニチンに払われる。表4に示すように、前処理基準平均24±3μmol/L〜後処置平均8±1μmol./L(P<0.001)の全患者のシトルリン濃度の顕著な低下。血漿アルギニン濃度は、幾つかの患者にアルギニン含有非経口的栄養法を使用したにもかかわらず、91±6μmol./L〜70±6μmol./L(P<0.05)の平均から減少した。
従って、ここで開示される主題に従って、BMTを受けた患者のHVOD毒性、及び/又は発症の低下方法を提供する。この方法は、該BMT患者に、該患者のアルギニン、及びNO合成の増強に有効な量のアルギニン、及び/又はシトルリン、いくつかの実施態様において、シトルリンを投与することを含む。該患者のアルギニン、及びNO合成の増強は、BMTに関連したHVODの発症を減少し、かつ/又は実質的に抑制する。シトルリンは、より容易にNOに変換されるように与えられる、代表的な補充剤である。
(機能性全長CPSI発現クローンの構築)
多くの戦略を試みた後に、全コード領域を含むヒトCPSI cDNA発現クローンを構築した。図6、及び7は、該発現クローンの構築に使用される方法の概略図である。このクローンは、完全に配列決定されており、かつ技術的に特徴付けられているコンセンサスCPSI配列からの変化は全くない。
これらの構築体を使用して、COS細胞、及び発現系としてそれぞれのCPSI/PC DNA 3.1を用いて対立遺伝子(T1405、N1405)でコードされた組換えCPSIタンパク質の安定供給を提供する。酵素学的に活性なCPSIは、図8に示した系を用いて製造した。
最初に該組織を、0.75M KCl中でホモジナイズする。ATP、及びNAGを含む小分子を、SEPHADEX(商標)G25カラム(Boehringer)を通して除去する。該反応混合物は、炭酸水素アンモニウム、ATP、マグネシウムDTT、n−アセチルグルタミン酸(NAG)、及びトリエタノールアミンを含む。全ての試薬濃度を変えることができ、かつHepG2細胞での実験は、低及び高濃度NAG(0.50mM)で活性低下を示す。予備COS−7細胞発現実験において、NAGがないと、測定可能な酵素活性がないことを示した。
(バルプロ酸治療患者に見られるアンモニア上昇に対する、T1405N多型と尿素回路中間体との関連性)
バルプロ酸(VPA)は、特に欠神発作の治療又は他の発作障害の補助療法に一般的に使用される発作薬である。VPS治療の毒性は、複雑かつ多変量のプロセスであり、かつ恐らく、幾つかの代謝崩壊を示す。高アンモニア血症、並びに、肝臓微小空胞変性及び壊死は、最も一般的に報告されている深刻な医学的合併症である。
少数の患者にのみ、毒性高アンモニア血症の発現が伴うが、有意な罹患率、及び死亡率を有し、かつ幾つかの死が、この合併症に起因している。無症候性高アンモニア血症の発現(血漿アンモニア濃度が60μmol/Lよりも大きい)は、VPA投与1時間以内に生じるが、比較的よく見られる。
VPAが高アンモニア血症を引き起こす機序は、討論の対象であり、かつ現在、多くの異なる説は、技術的サポートがある。腎臓モデルは、グルタミン代謝変化により、増加アンモニアが該肝臓に取り込まれることを提案しているが、多くの他の理論は、尿素回路機能の別の局面に焦点を合わせている。例えば、Warterらの論文, Revue Neurologique, 139:753-757 (1983)を参照されたい。該尿素回路は、ヒトの中のアンモニアを除去するための主要な機序であるので、高アンモニア血症は、何らかの方法で、VPA及び/又はその代謝産物と尿素回路機能及び許容能阻害との相互作用から生じるものと考えられる。
VPA誘発尿素回路欠損の機序は、通常、ミトコンドリアのカルバミル合成酵素I(CPSI)を中心に回転する。VPA過剰投与後の深刻な毒性を有する患者は、50%正常CPSI活性であることを発見した(Bourrierらの論文, Prese Medicale 17:2063-2066 (1988))。出願人は、中断後の即時逆転を有するバルプロ酸を投与した時に悪化した、幾つかの温和なCPSI欠損患者を観察した。
N−アセチルグルタミン酸(NAG)は、CPSIに必要なアロステリック補助因子である。NAGAは、CPSIの細胞分布に似た細胞分布を有するミトコンドリア中のグルタミン酸とアセチルCoAから合成される(Shigesadaらの論文, Journal of Biological Chemistry 246: 5588-5595 (1971))。それは、グルタミン酸(アミノ酸異化作用由来)とアセチルCoAとから合成される。NAG利用能の変更によりCPSI活性低下を予測する、幾つかの方法がある。NAG合成酵素の遺伝子欠如が観察され、かつ、この酵素は、プロピオニルCoA、又はコハク酸などの他の基質により完全に阻害されることが知られ
ている(Bachmannらの論文, New England Journal of Medicine 304:543 (1981);Kamounらの論文, Lancet 48 (1987);Coudeらの論文, J. Clin. Invest. 64:1544-1551 (1979);Rabierらの論文, Biochem. And Biophys. Research Comm. 91:456-460 (1979);Rabierらの論文, Biochimie 68:639-647 (1986))。CPSIが増加量のプロピオニルCoAの存在により競合的に阻害され、かつVPA療法が血液のプロピオン酸濃度を増加させることは、実験的に示されている(Coulterらの論文, Lancet 1 (8181): 1310-1311 (1980);Gruskayらの論文, Ped. Res. 15:475 (1981);Schmidt, R. D., Clin. Chim. Acta. 74:39-42 (1977))。また、VPA曝露は、アセチルCoA、及びグルタミン酸、双方の濃度が低下することにより、無処置肝細胞中のNAG濃度を低下することが示されている(Coudeの論文, Biochem. J. 216:233-236 (1983))。グルタミン濃度の低下は、ピルビン酸デヒドロゲナーゼ、及びピルビン酸カルボキシラーゼに起因する。
(CPSIのさらなる多型の検出)
CPSIメッセージの変異分析で開発された技術を用いて、10の非CPSI欠乏、無関係の患者を、該コード領域のさらなる多型に対してスクリーニングした。これは、リンパ芽球腫、及び線維芽細胞株由来の"非正統的"転写物を用いて行った。該集団の広範な効果を有する多型は、このサイズ試料において明らかとなるべきである。本明細書中、及びクレーム中に使用される用語"多型"は、集団内の2以上の遺伝子的に決定された別の配列、又は対立遺伝子の発生を意味する。多型性指標は、相違のある遺伝子座である。例証的な指標は、少なくとも2つの対立遺伝子を有し、各々が、1%未満の頻度で生じることである。従って、提供される多型性指標は、制限断片長多型、可変数の縦列反復(VNTR's)、超可変領域、ミニサテライト、ジヌクレオチド反復、及びテトラヌクレオチド反復を含む。
上述の4つの重なりRT/PCR産物を、変異のスクリーニングに使用する。制限地図の慎重な分析により、100〜250bpの範囲の切片に開裂する各フラグメントに対する3つの制限酵素の選択を可能にする。このサイズのフラグメントは、一本鎖高次構造多型(SSCP)分析に適している。該酵素は、各断片がその長さに渡って一様に評価され得るように選択される。
に記載されているように、一本鎖フラグメントの立体配座変化の検出を最適化する。銀染色でDNA検出を行い、かつ該ゲルを移動度シフトに対して記録する。シフトした全てのフラグメントの位置に基づいて、サイクル−配列決定プロトコル(cycle-sequencing protocol)を用いて直接、該RT/PCR産物の配列分析を行う。Taqポリメラーゼエラーに起因する変異の可能性を除去するために、新たなRT産物を、各ケースにおいて増幅し、かつ配列決定する。コード化メッセージの全4,600塩基を、この方法で迅速に配列決定する。不明確な領域を含む全ての領域を配列決定し、予測配列の変化を探す。
頻度を確立するために、これらの実験において検出された多型を、Centre d'Etude Polymorphsim Humanise(CEPH)親パネルに対して遺伝子型特定する。全ての変化を、コドン使用のこれらの効果に対して調査し、かつミスセンス変異の結果生じるものを、本明細書中に開示したCPSI特性データを用いて調査する。
この系を用いて、CPSI産生、及び活性の変化のインビトロ効果を観察する。
T344A多型を、CPSI中で検出した。オリゴヌクレオチドプライマーを第10番目のエキソン(U1119:tactgctcagaatcatggc 配列番号:17)、及びイントロン(LI10+37: tcatcaccaactgaacagg 配列番号:18)から使用して、変化を含む91bpフラグメントを増幅した。PCRサイクル条件は、59℃で1分間アニール、72℃で1分間伸長、及び94℃で1分間変性の35サイクルである。患者を、AA、又はTTの同型接合性SNP遺伝子型を有するもの、又は異型接合性(AT)であるものにクラス分けした。この多型の成人集団分布は、35%AA、44%AT、及び21%TTである。
(持続性肺高血圧症を有する新生児患者のカルバミルリン酸合成酵素Iの生化学的、及び遺伝的変更)
この実施例は、病気時の新生児持続性肺高血圧症(PPHN)の病因における内在性NO産生制限の役割を調査する。内在性NOは、尿素回路中間体アルギニンの産物である。アルギニンの産物は、該尿素回路の律速酵素であるカルバミルリン酸合成酵素(CPSI)に依存する。新生児は、半分以下の正常尿素回路機能を有し、酵素形態、及び機能の小さい変化に特に影響されやすい。CPSIの共通のエキソンの多型(T1405N)を観測しており、それは、該尿素回路の第一段階を通る流れに影響を及ぼす。
この実施例において、PPHNを発現した新生児が、対応するコントロールよりも低いNO前駆体(アルギニン、及びシトルリン)を有するかどうかを試験した。また、PPHN患者が、AA(アスパラギン/アスパラギン)CPSI遺伝子型よりも低い機能に関連するCC(スレオニン/スレオニン)、又はAC(アスパラギン/スレオニン)CPSI遺伝子型を優性に有するかどうかを分析した。
バンダービルト新生児集中治療部(Vanderbilt Neonatal Intensive Care Unit)に入院している、>2kg、>35週、及び<72時間の47新生児(心エコー検査で記録された肺高血圧症を有する新生児(n=22)、及び有しない新生児(n=25))を登録した。呼吸困難の重症度の臨床的に重要な測定を記録した。アンモニア濃度、及び血漿アミノ酸プロフィールを得た。未変性MDE(商標)ゲルにPCR増幅DNAを流し、遺伝子型を測定した。
PPHN患者は、平均アルギニンが21.5μmol/lであったが、そうでない患者は平均38.3μmol/l(p=0.0004)であった。該シトルリン平均は、それぞれ6.1μmol/l、及び10.3μmol/l(p=0.02)であった。アルギニン、及びシトルリン濃度は、酸素付加指数、機械換気日間、及び追加O2を要求する日間で測定されたように、低酸素血症の重症度に逆に関連していた。T1405NのPPHN患者の遺伝子型の分析は、5CC、17AC、及び0AAを示したが、該コントロールは、7CC、16AC、及び2AAであった(予測された集団対立遺伝子頻度を用いて、カイ二乗p=0.005)。該CC遺伝子型の乳児は、アルギニン、及びシトルリン平均(21.5μmol/l、及び5.8μmol/l)が、該AA遺伝子型の乳児(31.5μmol/l、及び13.5μmol/l)よりも低くく、該酵素の2つの形態間の機能的相違と一致する。
(結論)
この実施例は、病気新生児のPPHN発現が、該尿素回路中間体アルギニン、及びシトルリンの不適当な利用能に関連することを示している。該CPSI DNAのT1405N多型は、酵素機能低下、及び続くNO前駆体濃度低下を導く。
カルバミルリン酸合成酵素(CPSI)は、該尿素回路の律速段階を触媒し、それによってアルギニン、及びシトルリンを含む該尿素回路中間体の組織濃度を決定する。本明細書中に開示したように、該CPSI遺伝子の広範分布したC〜Aエキソン多型は、重大なN−アセチルグルタミン酸結合ドメインの近くの1405位置で、変換スレオニンをアスパラギンに変化させる。CPSIの該アスパラギン含有体は、酵素機能研究において、さらに効率的な反応速度論を表すというデータが示されている。
Beckmann 7300アミノ酸分析器(Beckmann, Palo Alto, California)の4成分pH、及びイオン強度勾配クエン酸リチウム緩衝液系を用いて、陽イオン交換クロマトグラフィーによりアミノ酸を分離した。ニンヒドリンを用いたアミノ酸のカラム誘導体化後に、570nmで一級アミンアミノ酸、及び440nmで二級アミンを検出した。標準物質で装置を較正することにより定量を達成した(Sigma, St. Louis, Missouri)。シトルリン、及びアルギニンを、該尿素回路を介する中間体のフラックスの測定可能な指数として検出した。
試料を除タンパクし、かつカドミウムビーズとともにインキュベートして硝酸塩を亜硝酸塩に変換した後に、患者のサブグループにおいて、改質グリース試薬を使用して、血漿NOxを測定した。
(SNP検出)
CPS1の第36番目のエキソン(U4295−配列番号:15)、及びイントロン(LI36−配列番号:16)内からのオリゴヌクレオチドプライマー、及びゲノムDNA変化含有領域を含む251bpフラグメントを確実に増幅するポリメラーゼ連鎖反応(PCR)を、全体の血液製剤から得た。プライマーの組合せは、Taqポリメラーゼ(Promega)、及び下記のPCRサイクル条件を用いて、再現可能な増幅を与えた:67℃で1分アニール、72℃で1分伸長、及び94℃で1分変性の35サイクルである。ホルムアミド処置後、試料を、未変性MDE(商標)ゲル(FMC, Rockland, Maine)中、4℃で5時間、電気泳動にかけ、次に硝酸銀で染色して、DNAフラグメントを検出した。患者を、CC又はAAの同型接合性SNP遺伝子型、或いは異型接合性(AC)を有する者に分類した。未変性ゲル電気泳動、及び直接配列分析を用いた遺伝子型特定は、上述のように生じた。従って、T1405N多型の成人集団分布は、45%CC、44%AC、及び11%AAと決定された。
PPHNの3乳児を分析から除外した。肺生検時に肺胞毛細管形成異常が発見された乳児は、肺高血圧症の解剖学的病因があると考えられていた。別の乳児を誤って先天性横隔膜ヘルニアで登録し、かつ第3を、TPN開始後に、199時間齢で登録した。該コントロールグループの1乳児を、プラーダー−ヴィリ症候群になる低血圧症の原因を示す核型分析後に、分析から除外した。
(子ブタの経静脈シトルリン供給は、血漿アルギニン濃度を増加する。)
以前に、経静脈シトルリンを臨床モデルに使用していない。この実施例は、子ブタの血清アルギニン濃度に対してIVシトルリン、及びその効果の安全性を評価した。5〜21日齢の標的最小重量4kgの9デュロック種ブタの全てを利用した。全ての子ブタを麻酔導入、及び気管開口術を施した。中心線を大腿動脈に置き、かつ持続的に血流力学的にモニターした。シトルリン(600mg/kg IV)を、5子ブタに投与した。コントロール動物には、生理食塩水を与えた。シトルリン投与前、及び投与数時間後に血清アミノ酸を抜取った。
IVシトルリン投与1〜2時間後で血清アルギニン濃度がピークになり、かつ3時間後に基準以上を維持した。コントロールと比べて、全ての時間点で有意性に達していた(p<0.001)。血行動態の不安定化がないことが観察された。
上記データに基づき、IVシトルリンの単独投与後の血漿シトルリン、及びアルギニン濃度に対して、薬物動態を計算した。薬物動態データは、半減期(t1/2)、排出定数(Kel)、分布体積(Vd)、及び血漿クリアランス(CLp)を含む。
血漿シトルリン濃度は、急速に増加し、かつt1/2=1.5時間、Kel=.462時間−1、Vd=2.25L、及びCLp=1.05L/時間を示した。しかし、血漿アルギニンに対するシトルリンの効果は、NOシンターゼの基質であるので興味深かった。血漿アルギニン濃度の濃度曲線を、図13に示した。この曲線に基づいた、血漿アルギニンの薬物動態は、下記のようであった:t1/2=18時間、Kel=.039時間−1、Vd=2.85L、及びCLp=0.11L/時間である。長い半減期、及び遅いクリアランスは、IVシトルリンの単独投与が、血行動態に有害な影響なく、平等に長い間隔に渡って、血漿アルギニン濃度増加の維持に有効であることを示している。
(先天性心臓外科手術における経口シトルリン供給)
本実施例は、シトルリン供給が血清シトルリン濃度を増加し、内在性NO産生を介して、手術後の肺高血圧症のリスクを低下させるかどうかの評価に関連するものである。特に、本実施例は、経口シトルリンの手術中供給が、血清シトルリン濃度を増加し、該尿素回路を介した一酸化窒素の大きな産生を導き、それによって、手術後の肺高血圧症のリスクを低下するかどうかの測定に関連するものである。
無作為化した、プラセボ対照、二重盲検の研究を行った。先天性心臓障害の外科的矯正を受け、かつ手術後の肺高血圧症を発現するリスクのある40乳児/小児が、経口シトルリン、又はプラセボを受けた。シトルリン、又はプラセボの5回投与量(1.9g/m2/投与)を、手術前に、手術直後に投与し、次に12時間ごとに3回投与量を投与した。血清シトルリンの一次的指標を、5つの時間点で測定した。全身血圧、血清アルギニン及び一酸化窒素代謝産物、CPSI遺伝子型、及び肺高血圧症の存在/非存在の二次的結果測定を得た。
患者は、有意な副反応の証拠なく、シトルリン投与を許容する。経口シトルリン供給は、心配バイパス後に、血清シトルリン、及びアルギニン、双方を有意に増加させる。正常値以上の血清シトルリン濃度は、手術後の肺高血圧症のリスク低下に関連する。
(患者登録)
無作為化した、対照、二重盲検の研究において40患者を登録した。登録には、先天性心臓障害矯正の6つの外科的手法のうちの1つを受けた全ての6歳未満の乳児、又は小児を考慮した。望ましい外科的手法は、1)左心室低形成症候群(HLHS)、又はHLHSの変種に対するノーウッドI手法、2)両方向性グレン、3)改質フォンタン、4)房室中隔欠損修復(AVSD)、5)大血管転換手法がある。除外基準は、1)外科的に処理されない狭い有意な肺動脈、2)以前の肺動脈ステント配置、3)以前の肺動脈血管形成、4)有意な左側AV値逆流(left sided AV valve regurgitation)、5)肺静脈戻り異常(pulmonary venous return abnormalities)、又は6)肺静脈狭窄を含む。
肺高血圧症を、平均肺動脈圧が全身平均血圧の少なくとも1/2であり、かつ25mmHg超過であるものとして規定した。直接肺動脈圧測定を、両方向性グレン又は改質フォンタン手法後の患者状態における具体的な上大静脈中心線、或いは段階Iノーウッド手法を受けた患者を除いた他の全患者の経胸腔的肺動脈カテーテル、どちらかから得た。中心線は、心臓麻酔医により配置され、かつ肺の線は、心胸部外科医により直接配置された。
全ての医師(外科医、心臓病専門医、集中治療専門医、及びPI)、調査看護師、PCCU看護スタッフ、及び患者は、無作為化スキーム、及び処置腕割当(treatment arm assignments)が分からない。臨床データ、及び患者特徴を、研究結果の知見前に診療記録から得た。
シトルリン投与は、全身低血圧症の理論的危険性がある。48時間研究期間時に、1時間毎に全身血圧をモニターした。有害事象を、平均血圧の基準から25パーセントよりも大きい減少として規定した。患者を、容量蘇生法(volume resuscitation)、及び/又は変力/血管収縮サポートを用いて症候的に処置した。患者を、低血圧症が処置に応答しなくなるまで、除外させなかった。
40患者を、手術直前に、無作為にプラセボ、又はシトルリンのどちらかを受けさせた。バンダービルト病院臨床薬学(Vanderbilt Hospital Clinical Pharmacy)の治験薬サービス(Investigational Drug Service)で、以前の4ブロック無作為入れ換えに従って、乱数発生コンピューターを用いて無作為化を行った。モデルを治療(ITT)する意図で患者を登録した。
.8g/m2、及び全投与量9.5g/m2として、12時間毎に1.9g/m2投与量で5回投与した。この投与量は、該尿素回路欠陥を有する乳児/小児に投与した、現在のシトルリン補充治療により決定され、急性肺損傷のリスクのところで成人骨髄移植患者の進行中の臨床試験で投与された投与量と同じである(バンダービルト大学医療センター、Brian Christman MD)。
血液3ミリリットルを、5回の時間点で各患者から得た:手術直前及び手術直後、次に手術後12、24、及び48時間である。手術前の血液試料を、麻酔導入、及び動脈又は中心静脈カテーテルの配置の後に回収した。手術直後の試料を、小児発症治療部(PCCU)到着時に回収し、かつ次の試料を、該PCCU到着後にそれぞれの時間間隔で回収した。クエン酸塩加チューブで、試料を回収し、氷上に置き、かつ処理まで4℃で保存した。血漿と細胞とを分離するために、回収3時間以内に試料を遠心分離した。血漿試料を、さらなる実験室分析まで−70℃で冷凍した。
結果データ確認のための重要な要素は、中心静脈(CVL)、又は動脈経路(AL)に対する接触性であった。登録患者の両親は、血液試料が手術に必要な線から得られ、かつ中心静脈評価が医療的に必要ない時、追加の採血がないであろうことを保証した。結果変数の測定が利用できない時は、本研究の投与を続けなかった。
血清シトルリン、アルギニン、及び全ての他のアミノ酸の濃度を、タンパク質フリー抽出液のアミノ酸分析により測定した。7300アミノ酸分析器(Beckmann, Palo Alto, California, アメリカ合衆国)を用いて、陽イオン交換クロマトグラフィーにより、アミノ酸を分離した。該分析器の較正は、患者試料の試験前に終了させた。
一酸化窒素代謝産物濃度を、比色分析非酵素的アッセイ(colorimetric nonenzymatic assay)(Oxford Biomedical Research, Oxford, Michigan, アメリカ合衆国)を介して分析した。最初に、血漿試料を、硫酸亜鉛溶液を用いて除タンパクした。次に、硝酸塩を、カドミウムビーズとともにインキュベーションして、亜硝酸塩に還元した。次に遠心分離し、該上清にグリース試薬スルファニルアミド、及びN−(1−ナフチル)エチレンジアミンを連続的に加えた(14)。次に、各試料の吸光度を、540nMで測定し、かつ次に、該コントロールとして希釈亜硝酸ナトリウムの標準曲線を利用して、一酸化窒素代謝産物濃度を測定した。
人工心肺後の乳児、及び小児状態の平均シトルリン濃度は、以前に報告され、手術後12時間で20.7 +/− 13.0umol/Lである。40患者の試料サイズは、両側の有意性、及びα=0.05を用いて、シトルリン(n=20)とプラセボ(n=20)との差13umol/L(1SD)を検出するように、87%のパワー(1−β)を有するであろう。{試料サイズを、PSパワー、及び試料サイズプログラムを用いて計算する(Dupont WD、及びPlummer WD:PSパワー、及び試料サイズプログラムは、インターネットで無料で利用できる。Controlled Clin Trials,1997;18:274; バージョン 2.1.30}
(患者登録)
40患者を問題なく登録し、かつシトルリン(n=20)、又はプラセボ(n=20)を無作為に受けた。48時間の研究時間の間で、無作為化は、基準で、シトルリンとプラセボグループとの間の有意な差はなかった(表1)。
研究集団(N=40)の年齢の中央値は、55%男性、及び90%白色人種で8.5ヶ月であった(IQR 4−29 mo)。外科的介入は、ノーウッド段階I(8%)BDG又はフォンタン(53%)、VSD又はAVSD修復(25%)、及び大血管転換修復(15%)である。研究グループ内のT1405N多型の該CPSI遺伝子型分布は、5AA(12.5%)、23AC(57.5%)、12CC(30%)であり、一般集団で予測された分布に類似している。
平均血圧は、該シトルリン、及びプラセボグループ間で違いはなかった(P=0.530)(図14)。48時間の研究期間で死亡はなかったが、3患者は、外科的修復の30日以内に、手術後の合併症から死亡した。該患者のシトルリンとプラセボグループ間に有意な違いはなかった(2対1,P=0.487)。全ての死は、薬剤投与研究とは無関係であることがわかった。無作為にシトルリンを受けた1患者を、手術直後に除外した。なぜならば、有意な外科的合併症は、膜型人工肺(ECMO)を介するサポートを要求し、かつ48時間研究期間内のさらなる修復のためのORに戻るためである。治療意図を維持した。患者を、データ分析中の手術前の結果測定を包含した該シトルリングループで表したが、しかし、患者データがないために、手術後の結果分析から外した。
血清シトルリン濃度中央値は、経口シトルリンを受けた患者においてプラセボと比較した場合、手術直後(36umol/L IQR 28〜48umol/L対26umol/L IQR 24〜35umol/, P=0.012)、及び手術後12時間(37umol/L IQR 18〜83umol/L対20umol/L IQR 15〜29umol/L, P=0.015)、双方で有意に高かった(図15)。血清シトルリン濃度は、手術直後、及び手術後12時間、双方のプラセボグループの基準から有意に低下した(それぞれ、32umol/L IQR 25〜44umol/L対26umol/L IQR 24〜35umol/L、及び20umol/L IQR 15〜29umol/L, P=0.020、及びP<0.001)。一方、血清シトルリン濃度は、手術直後のシトルリン供給での基準から上昇し、かつ手術後12時間で濃度が上昇し続けた(それぞれ、29umol/L IQR 25〜34umol/L対36umol/L IQR 28〜48umol/L、及び37umol/L IQR 18〜83umol/L, P=0.014、及びP=0.184)。
手術後12時間より後の結果測定は、データ点の損失により不明瞭となった。9患者は回復し、かつ手術後24時間で一般小児科フロアに移動した。
手術後12時間での平均血清アルギニン濃度は、経口シトルリンを受けた患者において、プラセボと比較して有意に高かった(36+/−24umol/L対23+/−13umol/L, P=0.037)(図16)。血清アルギニン濃度は、手術後12時間のプラセボグループの基準から有意に低下した(38umol/L IQR 30〜52umol/L対34umol/L IQR 15〜45umol/L、及び22umol/L IQR 13〜33umol/L, P=0.077、及びP<0.001)。一方、血清アルギニン濃度は、該シトルリングループの基準からの有意な低下はなかった(それぞれ、33umol/L IQR 25〜54umol/L対33umol/L IQR 22〜41umol/L、及び30umol/L IQR 15〜56umol/L, P=0.533、及びP=0.533)。
一酸化窒素代謝産物の濃度は、シトルリン、及びプラセボグループ間での手術直後(42umol/L IQR 27〜72対40umol/L IQR 27〜55umol/L, P=0.430)、又は手術後12時間(45umol/L IQR 28〜81対43umol/L IQR 20〜68umol/L, P=0.518)で違いはなかった。
9患者が、手術後に肺高血圧症を発現した。それは、プラセボグループにおいて6患者(67%)、及び処置グループにおいて3患者(33%)であった(P=0.451)。肺高血圧症の全患者は、血漿シトルリン濃度が、先に報告した6歳小児のシトルリン濃度の標準より低く(30umol/L IQR 23〜37umol/L)、かつシトルリン供給でえら得た中央値濃度よりも低かった(37umol/L, P=0.036)。肺高血圧症患者は、T1405N多型の予測されたCPSI遺伝子型分布を有しておらず、0AA(0%)、6AC(67%)、3CC(33%)であった。しかし、少数の肺高血圧症患者のために有意ではない(P=0.743)。
患者は、有意な副反応の証拠なく、シトルリン投与を許容する。経口シトルリン供給は、プラセボを受けた患者のNO前駆体の有意な低下と比較して、シトルリン濃度を有意に増加させ、かつ手術前のアルギニン濃度を維持する。正常以上の血清シトルリン濃度を維持した患者は、シトルリン投与又は素因により、手術後の肺高血圧症を発現しない。

Claims (14)
- 心臓障害の外科的矯正のための心臓手術に起因する尿素回路中間体の形成の低下を伴う肺高血圧症を治療又は予防するための医薬組成物であって、治療的有効量のシトルリンを含み、経静脈投与用に製剤されている医薬組成物。
- 前記心臓障害が、大きな非制限的な心室中隔欠損であるか、又は過剰な肺の血流の結果である請求項1記載の組成物。
- 前記心臓障害が、過剰な肺の血流の結果である先天的な心臓欠陥である請求項1記載の組成物。
- 前記心臓手術が、心肺バイパス、両方向性グレン手法、改質フォンタン手法、ノーウッドI手法、大血管転換手法、及びこれらの組合せからなる群から選択される手法である、請求項1記載の組成物。
- 前記組成物が、先天的な心臓欠陥の外科的修復後、哺乳動物の低い術後肺血管緊張を維持する請求項1記載の組成物。
- 前記術後肺血流が完全に受動的である請求項1記載の組成物。
- 前記手術の術前、術前後、及び/又は、術後投与用に製剤されている請求項1記載の組成物。
- 投与により、術後肺血管緊張の上昇を防ぐ請求項1記載の組成物。
- 術後の肺血流の低下、及び/又は、心拍出量の低下を防ぐ請求項1記載の組成物。
- 投与により、持続性の胸水、胸膜若しくは縦隔の廃液管における長期の必要性、長期の換気、長期の集中治療室滞在、又はこれらの組合せの予後を防ぐ請求項1記載の組成物。
- 前記尿素回路中間体がアルギニンである請求項1記載の組成物。
- 前記シトルリンが、100mgから30,000mgの範囲の投与量で投与される請求項1記載の組成物。
- 前記シトルリンが、250mgから1,000mgの範囲の投与量で投与される請求項13記載の組成物。
- 前記尿素回路中間体がシトルリンである請求項1記載の組成物。
