Abstract
Description

Claims (35)
- 折り畳み可能な補綴心臓弁のための送達装置であって、
その中において移動空間を画定する筐体、
前記移動空間内において長手方向に可動であるキャリッジアセンブリ、
前記筺体に結合され、かつ、前記筐体に対して回転可能である配備アクチュエータ、および、
回転する態様で前記配備アクチュエータに固定された結合アセンブリであって、前記結合アセンブリが、前記配備アクチュエータの回転が前記キャリッジアセンブリを前記長手方向に移動させる係合位置と、前記配備アクチュエータの回転が前記キャリッジアセンブリを前記長手方向に移動させない係合解除位置とを有する、結合アセンブリ
を含む、操作ハンドルと、
周囲において区画が画定された第1のシャフトであって、前記第1のシャフトが、前記筺体に動作的に接続され、前記区画が、組み付けられた状況において前記弁を受けるように適合された、第1のシャフト、および、
前記キャリッジアセンブリに動作的に接続された遠位シースであって、前記遠位シースが、前記区画を被覆する閉鎖状況と、前記弁を配備するために前記区画を露呈させる開放状況との間で可動である、遠位シース
を含む、カテーテルアセンブリと
を備え、
前記キャリッジアセンブリの、前記移動空間内における前記長手方向の移動が、前記遠位シースを前記閉鎖状況と前記開放状況との間で移動させる、
送達装置。 - 前記キャリッジアセンブリが、前記キャリッジアセンブリの本体から延在して前記結合アセンブリに螺合するねじ山付きロッドを含む、請求項1に記載の送達装置。
- 第1の方向における前記配備アクチュエータの回転が、前記移動空間内において前記長手方向に近位に前記キャリッジアセンブリを移動させ、前記第1の方向とは反対の第2の方向における前記配備アクチュエータの回転が、前記移動空間内において前記長手方向に遠位に前記キャリッジアセンブリを移動させる、請求項2に記載の送達装置。
- 前記結合アセンブリが、複数のねじ山付きスプリットナット部分を有するスプリットナットを含み、前記スプリットナット部分の各々が、互いから離れて、かつ、前記ねじ山付きロッドから離れて、線形に摺動可能であり、前記スプリットナットが、前記スプリットナット部分のねじ山が前記ねじ山付きロッドに係合される係合位置と、前記スプリットナット部分の前記ねじ山が前記ねじ山付きロッドに係合しない係合解除位置とを有する、請求項2に記載の送達装置。
- 前記結合アセンブリが、前記スプリットナット部分に結合されたリングを含み、前記リングがカム面を有し、前記スプリットナット部分が前記係合位置と前記係合解除位置との間を移動するときに、前記スプリットナット部分が、前記カム面に沿って摺動可能である、請求項4に記載の送達装置。
- 前記配備アクチュエータが、前記長手方向に対して平行に延在する中心軸の周りを回転可能なノブである、請求項2に記載の送達装置。
- 前記キャリッジアセンブリが、前記キャリッジアセンブリの本体から延在して前記結合アセンブリに螺合する歯付きラックを含む、請求項1に記載の送達装置。
- 前記配備アクチュエータが、前記長手方向に対して垂直に延在する中心軸の周りを回転可能なノブである、請求項1に記載の送達装置。
- 前記操作ハンドルがさらに、ロック位置および解放位置を有するリシースロックを含み、前記ロック位置における前記リシースロックが、前記キャリッジアセンブリの、前記長手方向における移動を、前記移動空間内の停止位置に制限し、前記解放位置における前記リシースロックが、前記キャリッジアセンブリの、前記停止位置を越えた移動を許容する、請求項1に記載の送達装置。
- 前記キャリッジアセンブリの、前記停止位置への移動が、前記弁が完全に配備されないように、前記閉鎖状況と前記開放状況との間の状況まで前記遠位シースを移動させる、請求項9に記載の送達装置。
- 前記区画が第1の長さを有し、前記移動空間内の前記停止位置が、前記キャリッジアセンブリの移行距離に対応し、前記移行距離が前記第1の長さ未満である、請求項9に記載の送達装置。
- 前記折り畳み可能な補綴心臓弁が第2の長さを有し、前記移行距離が、前記第2の長さの約80%から約90%の間である、請求項11に記載の送達装置。
- 前記カテーテルアセンブリがさらに、前記遠位シースに付設され、かつ、前記キャリッジアセンブリに動作的に接続された外側シャフトを含み、前記外側シャフトが、前記第1のシャフトを少なくとも部分的に取り囲む、請求項1に記載の送達装置。
- 前記操作ハンドルがさらに、前記第1のシャフトを前記筺体に対して近位に移動させるように適合された機構を含む、請求項1に記載の送達装置。
- 前記第1のシャフトが、前記遠位シースに付設され、かつ、前記キャリッジアセンブリに動作的に接続され、前記カテーテルアセンブリがさらに、前記筺体を前記区画に接続し、かつ、前記第1のシャフトを少なくとも部分的に取り囲む外側シャフトを含む、請求項1に記載の送達装置。
- 前記カテーテルアセンブリがさらに、それを経由して長手に延在する内腔と、前記内腔内に位置付けられた挿入物と有する非外傷性先端を含み、前記第1のシャフトが、前記挿入物の遠位端と前記非外傷性先端を形成する材料との間に固定された、外向きにフレア状の遠位端を有する、請求項1に記載の送達装置。
- 前記挿入物が複数のリブを有し、各リブが、前記挿入物の円周の周囲を連続的に、または非連続的に延在する、請求項16に記載の送達装置。
- 前記非外傷性先端が、その長手方向において凹状に先細りにされた外側表面を有する、請求項16に記載の送達装置。
- 患者内に折り畳み可能な補綴心臓弁を送達する方法であって、
カテーテルアセンブリおよび操作ハンドルを有する送達装置を提供することであって、前記カテーテルアセンブリが、組み付けられた状況において前記弁を受けるように適合された区画を含み、前記操作ハンドルが、その中において移動空間を画定する筺体、前記移動空間内において第1および第2の長手方向に可動であるキャリッジアセンブリ、前記筺体に結合され、かつ、前記筺体に対して回転可能である配備アクチュエータ、ならびに、回転する態様で前記配備アクチュエータに固定された結合アセンブリを含む、カテーテルアセンブリおよび操作ハンドルを有する送達装置を提供するステップと、
前記カテーテルアセンブリの前記区画内に前記弁を装填して、前記カテーテルアセンブリの遠位シースで前記区画および前記弁を被覆するステップと、
前記弁が前記患者内の目標箇所に位置決めされるように、前記患者内に前記カテーテルアセンブリを挿入するステップと、
前記移動空間の第1の部分に沿って前記第1の長手方向に、前記操作ハンドルの前記キャリッジアセンブリを移動させることによって、前記弁を部分的に配備するステップと、
前記移動空間の第2の部分に沿って前記第1の長手方向に、前記キャリッジアセンブリの移動を継続することによって、前記弁を完全に配備するステップと
を含む、方法。 - 前記操作ハンドルがさらに、前記キャリッジアセンブリから延在して前記結合アセンブリに螺合するねじ山付きロッドを含み、前記配備アクチュエータが、前記筐体に対して長手に制約され、部分的に配備する前記ステップが、前記配備アクチュエータを回転させることを含む、請求項19に記載の方法。
- 前記結合アセンブリが、複数のねじ山付きスプリットナット部分を有するスプリットナットを含み、前記スプリットナット部分の各々が、互いから離れて、かつ、前記ねじ山付きロッドから離れて、線形に摺動可能であり、前記方法がさらに、前記スプリットナット部分を、前記スプリットナット部分のねじ山が前記ねじ山付きロッドに係合しない係合解除位置から、前記スプリットナット部分の前記ねじ山が前記ねじ山付きロッドに係合される係合位置に移動させることを含む、請求項20に記載の方法。
- 前記結合アセンブリが、前記スプリットナット部分に結合されたリングを含み、前記リングがカム面を有し、前記スプリットナット部分を移動させる前記ステップが、前記スプリットナット部分を前記カム面に沿って前記係合解除位置から前記係合位置に摺動させることを含む、請求項21に記載の方法。
- 前記配備アクチュエータが、前記第1および第2の長手方向に対して平行に延在する中心軸の周りを回転可能なノブである、請求項20に記載の方法。
- 前記操作ハンドルがさらに、前記キャリッジアセンブリから延在して前記結合アセンブリに係合する歯付きラックを含み、前記配備アクチュエータが、前記筺体に対して長手に制約され、部分的に配備する前記ステップが、前記配備アクチュエータを回転させることを含む、請求項19に記載の方法。
- 前記配備アクチュエータが、前記第1および第2の長手方向に対して垂直に延在する中心軸の周りを回転可能なノブである、請求項24に記載の方法。
- 前記カテーテルアセンブリがさらに、周囲において前記区画が画定された第1のシャフトと、前記キャリッジアセンブリを前記遠位シースに接続し、かつ、前記第1のシャフトを少なくとも部分的に取り囲む外側シャフトとを含み、前記第1のシャフトが、固定する態様で前記筺体に接続され、前記遠位シースが、前記キャリッジアセンブリに動作的に接続され、前記弁を部分的に配備する前記ステップおよび前記弁を完全に配備する前記ステップの各々が、前記外側シャフトを前記筺体に対して近位に移動させることを含む、請求項19に記載の方法。
- 前記カテーテルアセンブリがさらに、周囲において前記区画が画定された第1のシャフトと、前記筺体を前記区画に接続し、かつ、前記第1のシャフトを少なくとも部分的に取り囲む外側シャフトとを含み、前記第1のシャフトおよび前記遠位シースが、前記キャリッジアセンブリに動作的に接続され、前記弁を部分的に配備する前記ステップおよび前記弁を完全に配備する前記ステップの各々が、前記第1のシャフトを前記筺体に対して遠位に移動させることを含む、請求項19に記載の方法。
- 前記操作ハンドルがさらに、ロック位置および解放位置を有するリシースロックを含み、前記ロック位置における前記リシースロックが、前記キャリッジアセンブリの、前記第1の長手方向における移動を、前記移動空間内の停止位置に制限し、前記解放位置における前記リシースロックが、前記キャリッジアセンブリの、前記第1の長手方向における、前記停止位置を越えた移動を許容する、請求項19に記載の方法。
- 前記キャリッジアセンブリを、前記第1の長手方向とは反対の前記第2の長手方向に移動させることにより、前記弁をリシースすることをさらに含む、請求項19に記載の方法。
- 前記目標箇所が、前記患者の本来の大動脈弁輪である、請求項19に記載の方法。
- 挿入する前記ステップが、前記患者の大腿動脈を経由して前記カテーテルアセンブリの前記遠位シースを挿入することを含む、請求項19に記載の方法。
- 挿入する前記ステップが、前記患者の心尖を経由して前記カテーテルアセンブリの前記遠位シースを挿入することを含む、請求項19に記載の方法。
- 折り畳み可能な補綴心臓弁のための送達装置であって、
周囲において区画が画定された第1のシャフトであって、前記第1のシャフトが、長手方向に延在し、その遠位端において外向きにフレア状の部分を有し、前記区画が、組み付けられた状況において前記弁を受けるように適合された、第1のシャフト、
前記第1のシャフトの少なくとも長手部分を取り囲む外側シャフトであって、前記外側シャフトが、前記第1のシャフトに対して前記長手方向に摺動可能である、外側シャフト、
前記第1のシャフトおよび前記外側シャフトのうちの1つに付設され、かつ、前記第1のシャフトの長手部分を取り囲む遠位シースであって、前記遠位シースが、前記区画を被覆する閉鎖状況と、前記弁を配備するために前記区画を露呈させる開放状況との間で前記長手方向に可動である、遠位シース、ならびに、
前記第1のシャフトの前記遠位端に付設された非外傷性先端であって、前記非外傷性先端が、それを経由して長手に延在する内腔と、前記内腔内に位置付けられた挿入物とを有し、前記第1のシャフトの、前記外向きにフレア状の部分が、前記挿入物の遠位端と前記非外傷性先端を形成する材料との間に固定された、非外傷性先端
