1つの実施形態においては、本発明のバイオマーカーは、気相イオンを検出する質量分析器を用いる方法である質量分析法によって検出しうる。質量分析法の例は、飛行時間型、磁気セクター、四極子フィルター、イオントラップ、イオンサイクロトロン共鳴、静電セクター分析器、前記のハイブリッドまたは複合等である。具体的な実施形態においては、質量分析法はマトリクス支援レーザー脱離イオン化/飛行時間型(MALDI−TOF MSまたはMALDI−TOF)を含む。他の実施形態においては、方法はMALDI−TOFタンデム質量分析法(MALDI−TOF MS/MS)を含む。さらに他の実施形態においては、質量分析法は当業者が考慮することのできる他の適切な方法と組み合わせることができる。たとえば、MALDI−TOFは本明細書に記載するようにトリプシン消化およびタンデム質量分析法と共に利用することができる。他の実施形態においては、質量分析法は多重反応モニタリング(MRM)または定量的MRMである。
他の実施形態においては、本発明のバイオマーカーはイムノアッセイによって検出および/または測定することができる。イムノアッセイは、バイオマーカーを捕捉するために抗体などの生体特異的捕捉試薬を必要とする。多くの抗体が市販されている。抗体は、たとえば動物をバイオマーカーによって免疫することなどの、技術上周知の方法によっても生成することができる。バイオマーカーは、その結合特性に基づいてサンプルより分離することができる。代替的に、ポリペプチドバイオマーカーのアミノ酸配列が既知の場合は、ポリペプチドを合成し、さらに技術上周知の方法によって抗体を生成するために用いることができる。

本発明のバイオマーカーは、他の適当な方法で検出することができる。この目的のために用いることのできる検出パラダイムは、光学的方法、電気化学的方法(ボルタメトリー法および電流滴定法)、原子間力顕微鏡、およびたとえば多極共鳴分光法などの高周波法を含む。光学的方法の例示は、共焦点および非共焦点顕微鏡に加えて、蛍光、発光、化学発光、吸光度、反射率、透過率、および複屈折または屈折率(例:表面プラスモン共鳴法、偏光解析法、共鳴ミラー法、回折格子結合器導波管法または干渉法)である。
本発明は、脳損傷を診断するためのバイオマーカーの使用に関する。より具体的には、本発明のバイオマーカーは、たとえば個人、被験者または患者の脳損傷を診断するためなどに、脳損傷状態を判定、認定、および/または評価するための診断検査において使用することができる。具体的な実施形態においては、脳損傷状態は、たとえば個人、被験者または患者のSCIを診断するためなどに、患者の無症候性脳損傷状態すなわちSCI状態を判定することを含むことができる。より具体的には、脳損傷(例:SCIまたは顕性脳損傷)を診断する際に検出しようとするバイオマーカーはASTN1、BAI3、CNDP1、ERMIN、GFAP、GRM3、KLH32、MAGE2、NRG3、NRGN、OMG、SLC39A12、RTN1およびMT3を含むが、これに限定されない。SBDP150、SBDP150i、NSE、SlOOβ、MAP2、MAPI、MAP3、MAP4、MAP5、MBP、Tau、NF−L、NF−M、NF−H、UCH−Ll、NSE、NeuN、CNPase、α−インターネキシン、CB−I、CB−2;ICAM、VAM、NCAM、NL−CAM、AL−CAM、C−CAM;シナプトタグニン、シナプトフィジン、シナプシン、SNAP;CRMP−2、CRMP−I、CRMP−3、CRMP−4 iNOS、およびβlll−チューブリンを含むがこれに限定されない関連技術において既知である他のバイオマーカーを、本明細書に記載するバイオマーカーと組み合わせて用いてもよい。
本発明のバイオマーカーは、患者の脳損傷状態を評価、判定、および/または認定(本明細書では互換的に使用される)するための診断検査において用いることができる。語句「脳損傷状態」は、脳損傷を有していないことを含めた状態の任意の判別可能な発現を含む。たとえば、脳損傷状態は、患者の脳損傷の有無、脳損傷を発症するリスク、脳損傷の病期または重症度、脳損傷の進行(例:経時的な脳損傷の進行)および脳損傷の治療の有効性またはこれに対する反応(例:治療後の脳損傷の臨床的経過観察および監視など)を制限なく含む。この状態に基づき、追加的な診断検査または治療手技または治療法を含むさらなる手順を指示しうる。
具体的な実施形態においては、本発明は患者が脳損傷を発症するリスクを判定するための方法を提供する。バイオマーカーの百分率、量またはパターンは、たとえば高、中、低などの多様なリスク状態の特徴である。脳損傷を発症するリスクは、関連バイオマーカーを測定し、その後それらを分類アルゴリズムに投入するか、または参照値、すなわち具体的なリスクレベルと関連する事前に定義したバイオマーカーのレベルまたはパターンと比較することによって判定する。
他の実施形態においては、本発明は患者の脳損傷の重症度を判定するための方法を提供する。脳損傷の各等級または病期は、特徴的なバイオマーカーのレベルまたはバイオマーカー群の相対的レベル(パターン)を有する可能性がある。脳損傷の重症度は、関連バイオマーカーを測定し、その後それらを分類アルゴリズムに投入するか、または参照値、すなわち具体的な病期と関連する事前に定義したバイオマーカーのレベルまたはパターンと比較することによって判定する。
1つの実施形態においては、本発明は患者の脳損傷の経過を判定するための方法を提供する。脳損傷の経過は、脳損傷の進行(悪化)および脳損傷の後退(改善)を含む、脳損傷状態の経時的変化を意味する。バイオマーカーの量または相対的な量(例:パターン)は経時的に変化する。たとえば、バイオマーカー「X」が脳損傷に伴って増加することがある一方で、バイオマーカー「Y」が脳損傷に伴って減少することもある。したがって、脳損傷または非脳損傷に向かうこれらのバイオマーカーの経時的な増加または減少の傾向は、状態の経過を示す。このため、本方法は、患者の1つまたはそれ以上のバイオマーカーのレベルを、たとえば1回目および2回目などの少なくとも2つの測定時に測定し、かつ変化があれば比較することを包含する。脳損傷の経過はこれらの比較に基づいて判定する。
脳損傷状態を認定する方法の一定の実施形態においては、方法は状態に基づく患者の治療を管理することをさらに含む。そのような管理は、脳損傷状態を判定することに続く医師または臨床家の措置を含む。たとえば、医師が脳損傷の診断を下す場合、その後に一定のモニタリング法を実施するであろう。本発明の方法を用いた脳損傷の経過の評価は、その後一定の脳損傷治療法を必要とすることもある。代替的に、非脳損傷の診断の後で、患者が罹患していることもありうる具体的な疾患を判定するためにさらなる検査を行うこともありうる。また、診断検査の結果が脳損傷に関する結論に至らない場合はさらなる検査を要請することもある。
他の実施形態においては、本発明は医薬品の治療効果を判定するための方法を提供する。これらの方法は医薬品の臨床試験を実施する際、および医薬品を使用している患者の進行をモニタリングする際に有用である。治療法または臨床試験は、医薬品を特定の投与法で投与することを包含する。投与法は、医薬品の1回投与または医薬品の経時的複数回投与を包含しうる。医師または臨床研究者は、患者または被験者に対する医薬品の効果を投与の過程に渡ってモニタリングする。医薬品が病状に対して薬理学的影響を有する場合、本発明の1つまたはそれ以上のバイオマーカーの量または相対量(例:パターンまたはプロフィール)は、非脳損傷プロフィールに変化しうる。したがって、投与クール中、患者における1つまたはそれ以上のバイオマーカーの経過を追跡することができる。これにより、本方法は薬物療法を受ける患者の1つまたはそれ以上のバイオマーカーを測定すること、およびバイオマーカーレベルを(例:種々の脳損傷状態に対応する事前に定義したバイオマーカーレベルとの比較により)患者の脳損傷状態と相関させることを包含する。本方法の1つの実施形態は、たとえば1回目および2回目など、薬物療法クール中の少なくとも2つの測定時に1つまたはそれ以上のバイオマーカーのレベルを判定し、かつバイオマーカーのレベルに変化があれば比較することを包含する。たとえば、1つまたはそれ以上のバイオマーカーのレベルは医薬品の投与の前後、または医薬品の投与中の異なる2つの測定時に測定することができる。治療法の効果はこれらの比較に基づいて判定する。治療が有効である場合は1つまたはそれ以上のバイオマーカーは正常に向かう傾向である一方、治療が無効である場合は1つまたはそれ以上のバイオマーカーは脳損傷を示す傾向であろう。
一部の実施形態においては、「既知のサンプル」などのサンプルを用いて生成したデータはその後分類モデルを「トレーニング」するために用いることができる。「既知のサンプル」とは事前に分類されたサンプルである。分類モデルを形成するために用いるデータは「トレーニングデータセット」と呼ぶことができる。分類モデルを形成するために用いる「トレーニングデータセット」は生データを含むこともあれば、または事前に処理したデータを含むこともある。分類モデルは、一旦トレーニングすれば未知のサンプルを用いて生成したデータにおけるパターンを認識することができる。分類モデルはその後未知のサンプルを分類に分類するために用いることができる。これは、たとえば特定の生物学的サンプルが一定の生物学的状態と関連するか否か予測する際など(例:疾患対非疾患)に有用となることがある。
他の態様においては、本発明は脳損傷状態を認定するためのキットであって、当該キットが本明細書に記載するバイオマーカーを検出するために用いられるキットを提供する。具体的な実施形態においては、キットはASTN1、BAI3、CNDP1、ERMIN、GFAP、GRM3、KLH32、MAGE2、NRG3、NRGN、OMG、SLC39A12、RTN1、およびMT3を含むがこれに限定されない本発明のバイオマーカーに対する抗体を含むELISAキットとして提供される。
SCDおよびSCIを有する小児は重大な学習欠損を有しかつ脳卒中のリスクが高い。SCIの早期検出により、IQの低下を含むさらなる脳損傷および認知神経科学的欠損を予防するための治療法の実施が可能となるであろう。定義によれば、SCI患者は微小梗塞と一致する脳損傷のMRIおよび病理学エビデンスを有する。したがって、顕性脳卒中を有する非SCD患者で示されているように、SCI患者を特定する特異な血中バイオマーカーが存在する可能性が高い。本実施例の目標は、SCIの血中バイオマーカーとしての実証のためのリードプロテインを特定するために、SCI患者血漿サンプルと非SCI患者血漿サンプルの臨床的ペア群を探索するバイアスのないプロテオミクス技法を用いることである。
被験集団は、MRIによりSCIを有する(SCI群)またはSCIがない(非SCI群)と宣告された、SIT治験(24施設)に参加しているSCDを有する6〜12歳の男児および女児を含んだ。SIT治験の一環として、MRIおよび神経学的検査の他に、患者はCBC、Hb/Hct、ヘモグロビン電気泳動およびその他の臨床的検査の判定を受ける。治験に組み入れる際、クエン酸処理血漿を調製し、凍結し、ジョンズ・ホプキンズ大学のBiologic Repositoryに−70℃で保存する。HbはSCIの独立リスク因子であることが示されているので、本実施例の初期探索コホートは、表1に示すように、そのいずれもがHb<8.1g/dLのSCIを有する患者50例およびSCIのない患者50例を含む。
SCIを有する小児における血漿発現の変化に伴うタンパク質を同定するため、SCI小児および非SCI小児の個々の血漿サンプル250μLを、事前に入手することのできた新世代のLC10ポリクローナル抗体アフィニティカラム(IgY高容量LC10カラム、ベックマン・コールター)を用いてアフィニティ除去処理した。この新世代カラムは除去処理できる血漿の量が2倍となっているので(125μLから250μL)、下流の敏感度が上昇する。この新規カラムをProteomeLab PPS Proteome Partitioning System(ベックマン・コールター)と組み合わせて、フロースルーおよび結合タンパク質を採取する。除去処理した血漿各250μLにつきフロースルータンパク質を7mL採取する。5mLループを用いて除去処理したタンパク質400μgを、線形アセトニトリル勾配を用いるC18非多孔性カラムと接続したRP HPLC(ProteomeLab PPS Proteome rtitioning System)で分画化し、30分間で60分画を捕集した。
上述のRP HPLC分画に含有されるタンパク質を同定するために、各RP HPLC分画のLC/MS/MS(LTQ Orbitrap)分析を用いて非SCI/SCI血漿タンパク質データベースを策定した。簡単に述べると、1例の患者に由来するRP HPLC分画プールをSpeedVacを用いて完全に乾燥し、中和緩衝液に再懸濁し、一晩トリプシン消化した。トリプシン消化後、RP HPLC分画をLC/MS/MS(Thermo LTQ−Orbitrap)で分析した。トリプシン消化前の未処理タンパク質のRP HPLC分画より、それぞれ50〜100種類のタンパク質を含有する分画を生成した。タンパク質同定の信頼性を高めるために、X!Tandem、OMSSA、およびSageN SEQUESTを用いて、IPIタンパク質データベース(完全トリプシン消化した最新版と3.6)と照合して各タンデムMSスペクトルRAWファイルを検索し、さらにタンパク質およびペプチド同定結果を、MaspectrasおよびScaffoldで各患者別に編集した。SCIおよび非SCIサンプル中のペプチドカウントスペクトルが存在するか/しないかまたは1.5倍を上回るその変化(上昇または減少)に基づいて、初期リードタンパク質を同定した。
半定量的スペクトルカウントの1.5倍変化というカットオフ値を用いて、SCI小児の血中タンパク質を以下の表1に列挙する。これらのペプチドの同定は期待値<10−1を有する。

SCIを有する小児における血漿発現の変化に伴うタンパク質を同定するため、SCI小児および非SCI小児の個々の血漿サンプル250μLを、事前に入手することのできた新世代のLC10ポリクローナル抗体アフィニティカラム(IgY高容量LC10カラム、ベックマン・コールター)を用いてアフィニティ除去処理した。この新世代カラムは除去処理できる血漿の量が2倍となっているので(125μLから250μL)、下流の敏感度が上昇する。この新規カラムをProteomeLab PPS Proteome Partitioning System(ベックマン・コールター)と組み合わせて、フロースルーおよび結合タンパク質を採取する。除去処理した血漿各250μLにつきフロースルータンパク質を7mL採取する。各患者由来の血漿計750μLを除去処理し、除去処理したタンパク質>1mg(プールカラムフロースルー21mLとして)を下流RP−HPLC分画化に提供した。4mLループを用いて、フロースルータンパク質各4mLの独立したラン5回(血漿750μLの除去処理より採取)を、線形アセトニトリル勾配を用いるC18非多孔性カラムと接続したRP HPLC(ProteomeLab PPS Proteome rtitioning System)で分画化し、30分間で60分画を捕集した。各分画のタンパク質濃度を高めるために、5回のRP−HPLCランの各RP−HPLC分画を同じプレートに捕集する。大きな除去処理タンパク質体積(21mL)を処理する反復RR−HPLC分画化は面倒であるが、代替法であるサイズ排除遠心分離よりも優れている。サイズ排除遠心分離では、(a)遠心分離カラムへの非特異的なタンパク質の結合;および(b)タンパク質<10kDa(アンジオテンシンII、ブラジキニンおよびエンドセリンなどの血管作用ペプチドを含み、SCIの病理において非常に重要となりうる)の固有損失から、許容できないタンパク質損失が生じる。この強度のタンパク質損失は、本実施例に記載するような探索ベースの手法では許容できない。対照的に、反復RP−HPLC分画化は非常に再現性が高い。
上述のRP HPLC分画に含有されるタンパク質を同定するために、各RP HPLC分画のLC/MS/MS(LTQ Orbitrap)分析を用いて非SCI/SCI血漿タンパク質データベースを策定した。簡単に述べると、1例の患者に由来するRP HPLC分画プールをSpeedVacを用いて完全に乾燥し、中和緩衝液に再懸濁し、一晩トリプシン消化した。トリプシン消化後、RP HPLC分画をLC/MS/MS(Thermo LTQ−Orbitrap)で分析した。各RP HPLC分画は50〜100種類のタンパク質を含有する。(Maspectrasの)タンパク質データベースを策定し、2群間のタンパク質発現の差について調査し、その後の分析のために機能的および細胞型特異的タンパク質にコメントした。SCI患者および非SIC患者由来の特異的RP−HPLC分画を、目的の潜在的候補タンパク質を含有すると一旦特定したならば、質量分析技法である多重反応モニタリング(MRM)を用いてこれらの分画中のタンパク質を定量した。簡単に述べると、トリプシン処理後に、サンプルをバイオマーカータンパク質のペプチドシグネチャーを用いるLC/MS/MS同定および定量に投入した。WebベースのMaspectrasデータベースを用いて、生データ、処理したデータおよび分析およびタンパク質プロファイリング用最終プロテオミクスデータを保存した。
Unigene(NCBI)のESTタグに基づくNCBIの脳特異的タンパク質リストを用いて、SCI小児の血中脳タンパク質104種類を同定した。表2を参照されたい。これらのペプチドの同定は期待値<0.9を有する。
本明細書に記載のデータ、さらには心臓手術を受けた成人のコホートから取得したデータのさらなる分析より、無症候性脳損傷を診断/評価するために個別に、または複合して用いることのできる以下のバイオマーカーパネルが得られた。
鎌状赤血球症(SCD)は、多臓器損傷を特徴とする慢性溶血性貧血である。脳卒中は、SCDにおいて脳に対して最も顕著に発生しうる損傷である。経頭蓋ドップラー速度が上昇した患者を特定しかつ長期的な赤血球輸血を実施することにより、小児の脳卒中リスクは有意に低下している。Adams他、339 N. ENG. J. MED. 5-11(1998)。MRI上でサイレント脳梗塞(SCI)の存在により確認される脳損傷のリスクが長期的赤血球輸血によって軽減されるかは、現在研究中である。King他、50 PEDIATR. BLOOD CANCER 599-602(2008)。脳卒中リスクのバイオマーカーは、明らかに鎌状赤血球症の診療を変容させている。
(患者)
サイレント梗塞輸血治験(SIT治験、ClinicalTrials.gov NCT00072761)のためにスクリーニングされた鎌状赤血球症を有する5〜14歳の小児の横断サンプル(HbSSおよびHbSβ0)を検討した(n=259)。SIT治験は、鎌状赤血球症およびSCIを有する小児に対する3年間輸血プログラムの多施設無作為化対照試験である。主要評価項目は顕性脳卒中あるいは新規または進行性SCIの発生を含む。全ての患者はインフォームドコンセントに署名した。SCIは、正常な神経学的検査およびT2強調画像の2視野で目視できるMRIシグナル異常によって定義される。シグナル異常は、1次元で3mm以上でなければならない。SCI状態は、神経放射線科医および脳神経科医のパネルによって宣告される。Vendt他、22(3)J. DIGIT. IMAGING 326-43(2009)を参照されたい。陽性および陰性対照患者は、ジョンズ・ホプキンズ病院の外来および入院病棟から選択した。陽性対照血漿サンプルは、入院した小児または顕性脳梗塞または脳手術で入院した成人から採取した。陰性対象は、ジョンズ・ホプキンズ病院のHarriet Lane小児科外来の5〜16歳の小児から選択した。外来記録を審査して何らかの急性疾患、神経学的障害、または喘息、肥満および行動/気分障害以外の慢性疾患を有する患者を除外した。IRBの承認を受けた研究により、これらの対照についての非特定血液サンプルおよび臨床データを取得した。
血液をACDまたはEDTA管に採取し、SIT治験プロトコルに従って1500gで8分間遠心分離し、ジョンズ・ホプキンズ大学SIT治験用Biologic Repositoryに分析時まで−70℃で保存した。ProteomeLab Protein Partitioning System(PPS、ベックマン)上のLC10 IgYカラム(ベックマン・コ−ルター、カリフォルニア州フラートン)を用いて、血漿500μLから12種類の血漿大量タンパク質を除去処理した。IgYカラムのフロースルーを、連続アセトニトリル勾配(PPS、ベックマン)を用いるC18カラム(Jupiter, Phenomenex)を通す逆相HPLCで39分画に分離した。分画を乾燥させ(SpeedVac、サーモサイエンティフィック、マサチューセッツ州ウォルサム)、37℃で一晩トリプシン消化した(プロメガ、ウィスコンシン州マジソン)。各サンプルのスペクトルをLC/MS/MS(LTQ−Orbitrap、サーモサイエンティフィック)により取得した。SEQUEST(Sorcerer、SageN、ニュージャージー州ペニングトン)およびヒトIPIデータベースバージョン3.4を用いてスペクトルを検索し、タンパク質を同定した。Protein Centerを用い、タンパク質同定カットオフ値として信頼水準<0.9により検索後分析を実施した。
電気化学発光サンドイッチイムノアッセイ(メソスケールディスカバリー、メリーランド州ゲッサーズバーグ)を用いて、非希釈血漿サンプル中のGFAPを2回ずつ測定した。各ウェルにつきPBS30μL中のモノクローナル抗GFAPブレンドSMI−26(コーバンス、ニュージャージー州プリンストン)100ngを、標準結合MSDプレート中で一晩インキュベートして捕捉した。スルフォダグ(メソスケールディスカバリー)と直接コンジュゲートしたポリクローナル抗GFAP(ダコ、カリフォルニア州カーピンテリア)をPBS中1μg/Lで用いて検出した。標準曲線は、1%ウシ血清アルブミン(SeraCare Life Sciences、マサチューセッツ州ミルフォード)で4倍希釈した精製GFAP(カルビオケム、カリフォルニア州ラ・ホーヤ)を用いて作成した。測定値の変動を最小化するため、対照および鎌形赤血球症患者サンプルは、できるだけ同一のサンプルプレート上で測定した。分析は、Sector Imager 2400(メソスケールディスカバリー)上でメーカーのプロトコルにしたがって実施した。
GFAP濃度を群間比較するためにスチューデントのt−検定を用いた。自然対数に変換したGFAP値は正規分布に近似し、これらをパラメトリック分析に用いた。割合の検定はフィッシャーの正確検定を用いて実施した。分析はStata v10.1(Stata社、テキサス州カレッジステーション)を用いて実施した。

SCIを有するSIT治験被験者より採取した6血漿サンプル中4サンプルで、SEQUEST検索によりGFAPペプチドを同定した。図2を参照されたい。
Petzoldによって報告されさらにMSDプラットフォームに適合化された抗体試薬を用いたGFAP測定法を開発した。Petzold他、287 J. IMMNOL. METHODS 169-77(2004)を参照されたい。この方法の検証を実施した。至適抗体濃度、プレートの種類、およびブロッキング材料を判定する実験の後、本明細書に報告された全てのGFAP値に対して用いられた最終的な測定は、ブランクウェルのバックグラウンドより2SD高いと定義した0.0011ngと低い検出限界値を有した(実験回数n=19)。0.01ng/mLにおける信号雑音比は1.17であった(実験回数n=19)。算出濃度±既知の濃度の20%となる最小希釈度と定義される定量下限値は0.026ng/mLであった(実験回数n=3)。日間再現性は10ng/mLにおいて2.4%であり、かつ0.156ng/mLにおいては3.4%であった(実験回数n=21)。GFAPを10ng/mLで添加した血漿は、BSAにおいて生成した標準曲線と比較して49.8%±22.9%の回収率を示す。血漿中のGFAPが室温で安定であるか少なくとも48時間判定し、測定値がインビトロタンパク分解のアーチファクトでないことを確認した。t=0、6、24、および48時間における同一の分取物の連続測定は、2ng/mLにおいてCV2.5%、0.2ng/mLにおいて12.1%を示し、血漿中のGFAPの安定性を証明する。
GFAP測定法が脳損傷の状況における血漿GFAPの上昇を検出することを実証するため、急性脳卒中、脳生検または部分脳切除患者の0〜5日後の小児および成人に由来する血漿サンプルより構成される陽性対照集団を測定した。図3は、脳卒中(中央値0.446ng/mL、範囲0.029〜4.29ng/mL)、脳生検(中央値0.96ng/mL、範囲0.162〜0.93ng/mL)および脳切除(中央値1.29ng/mL、範囲0.122〜24.1ng/mL)についてのGFAPレベルを示す。小児におけるGFAPの血漿濃度は不明であるので、一般の小児科外来からの年齢マッチングした対照(5〜16歳)を用いて正常範囲を確立した。健康対照被験者はスクリーニング用に血液を採取されており、かつ急性疾患はなかった。健康対照のうち46/47例はアフリカ系であった。年齢マッチングした正常対照におけるGFAPの95パーセンタイルは0.244ng/mLであった。陽性対照のうち74%は、正常対照の95パーセンタイルよりも高いGFAP値を有していた(p<0.0001)。
いくつかの研究はGFAPを急性脳損傷のマーカーとして評価している。SCIおよび脳卒中は、鎌状赤血球症において発生することが十分に立証されたCNS事象である。図4に示すように、SCI群の小児も非SCI群の小児もGFAP濃度が上昇していた。SCI陽性患者とSCI陰性患者の間でGFAP濃度に統計的な差はなかった(p=0.69)。GFAPが年齢マッチング健康対照の95パーセンタイルを上回って上昇しているSCI患者の割合は、GFAPを有する非SCI患者の割合よりも高かったが、この差は統計的に有意ではない(13.4%対7.6%、p=0.14)。
SITT血漿サンプルは特異で貴重なリソースであるものの、実証の目的に対しては、試験組み入れ時の血漿サンプルとSCI脳病変の発症との間に予測可能な時間的関係がないという点で特別な問題を呈する。小児期SCD集団において非症候性および顕性脳卒中のマーカーとしてのGFAPをさらに実証するために、図5に、臨床的に髄膜炎と疑われる熱性疾患中に急性脳卒中を呈した11歳のホモ接合型鎌形赤血球症患者(非SITT患者)からの連続血漿GFAP濃度を示す。CSFを検査したところ感染および出血は陰性であった(6WBC/μL、2RBC/μL)。脳卒中の臨床診断より32時間前に採取した血漿で初期GFAP濃度を測定した(1.5ng/mL、正常小児の95パーセンタイルの6倍)。脳卒中の臨床診断より25時間後のCSFのGFAPレベルは44.6ng/mLであった。参照として、17歳未満の全小児患者のCSF中のGFAPの研究では、急性損傷がない場合のCSF中GFAPレベルは0.1〜0.5ng/mLであることが示された。Osterlundh他、50 PEDIATR. BLOOD CANCER 793-98(2008)。脳卒中の臨床診断より29時間後に実施したMRIは右前大脳動脈梗塞を示した。図6を参照されたい。赤血球交換後もGFAPレベルが低下を続けたことから、この患者はGFAPが急性脳損傷のみのマーカーである可能性があることも証明する。脳卒中より1週間後、やはりホモ接合型鎌状赤血球症である患者の8歳の妹も熱性疾患を発症し、その間の妹の血漿GFAP濃度は0.088ng/mLであった。
鎌形赤血球症におけるGFAPの脳損傷に対する特異度をさらに探索するため、HbSS患者17例の連続血漿サンプルをGFAPについて検討した(n=40)。入院の理由は急性胸部症候群、疼痛発作、インフルエンザ、発熱、骨髄無形成発症(PCRでパルボウイルスB19を確認)および持続勃起症であった。SCDを有する疾患小児でGFAPが常に上昇することは確認されておらず、39/40サンプルで濃度が正常範囲にあった(範囲0.01〜0.24ng/mL)。最高値(0.44ng/mL)は発熱で入院した4歳の患者に認められた。この数値は正常範囲の95パーセンタイルを上回るものの、正常対照の最高値と同等である。
鎌形赤血球症のバイアスのない血漿プロテオームスクリーンを用い、既知の脳損傷マーカーGFAPを同定した。鎌形赤血球症患者の血漿からの脳特異性の高いタンパク質の探索により、本実施例で用いられるバイオマーカー探索の実験的手法が実証される。
本実施例は、ECMOを受ける小児において血漿GFAPレベルが急性神経学的損傷と関連するか判定するために実施した。これは、2008年4月から2009年8月までにジョンズ・ホプキンズ病院の26床PICUにおいてECMOを受けた小児を対象とした予測的観察的コホート研究であった。任意の適応によりECMOを必要とし、かつECMOの過程で非分画化ヘパリンの連続点滴を受けた生後1日から18歳未満までの全ての小児を、本研究の対象とした。22例の患者をGFAP分析に組み入れた。年齢の中央値は生後10日であり(範囲:生後1日〜16歳)、男児は12例(54.5%)であり、かつ12例(54.5%)がアフリカ系アメリカ人であった。主要な疾患カテゴリーは循環器系以外の内科が14例(64%)、またECMOの適応症は呼吸不全が12例(54.5%)、心不全が6例(27.3%)、ECPRが3例(13.6%)、および敗血症が1例(4.6%)であった。
低酸素性虚血性脳症(HIE)は2.5/1000正期産出生児で発生する新生児脳卒中のサブセットである。残念なことに、分娩期胎児モニタリングおよび分娩後脳画像検査では、周産期脳損傷の乳児を迅速に特定することができない。重要なことに、生後6時間以内に頭部冷却を受けた中等度から重度のHIEを有する新生児は生存率が改善すると共に神経発生的障害が低下することがある。HIEリスクの高い新生児を治療する治療法は開発されつつあるものの、リスクの高い小児において特定の脳損傷を迅速に特定するか、HIE療法の有効性を追跡するか、または新規治療法を評価するバイオマーカーは知られていない。出生の時点で実施することのできる迅速検査は、これらの検査的処置から恩恵を得、治療効果を鑑別し、かつ早期に予後情報を提供することを目的としてHIEリスクの高い乳児を特定する際に大きな臨床的恩恵となるであろう。過去12ヶ月間に新生児集中治療室に入院した、染色体が正常かつ重大な先天的奇形のない予測的103例の予備分析を実施した。低酸素性虚血性脳症(HIE)、脳室周囲白質軟化、てんかん性発作、および頭蓋出血を含む神経学的損傷を有する新生児(n=27)を、神経学的損傷のない新生児(n=76)と比較した。これらの神経学的損傷を受けた新生児および損傷のない新生児は、妊娠期間(損傷乳児33.7±5.7週、非損傷乳児33.2±4.5週、p=0.67)または出生体重(損傷乳児2264±1129g、非損傷乳児1961±966g、p=0.18)に差がなかった。以下の表4を参照されたい。
GFAPはヒト星状細胞の主要な中間フィラメントであり、かつ中枢神経系に対する特異的マーカーである。患者血清中におけるその出現は、外傷性脳損傷および脳卒中後の患者の神経学的転帰を予測することが確認されている。虚血後の脳浮腫は血液脳関門の欠損を引き起こし、またGFAPは虚血プロセスにおいて星状細胞内で著明にアップレギュレートされる。本実施例の目的は、生後24時間以内に得られた血清GFAPレベルを用いて神経学的損傷を有する新生児を特定できるか判定することであった。
心肺バイパス(CPB)を受けた小児は、症例の30〜70%が神経学的損傷を経験する。McQuillen他、38 STROKE 736-41(2007);およびMahle他、106 CIRCULATION I-109-I-114(2002)を参照されたい。体外膜型酸素供給(ECMO)は、同様の支援方式を代表し、症例の10〜60%で神経学的損傷のリスクを伴う。Cengiz他、33(12)CRIT. CARE MED. 2817-24(2005);およびIbrahim他、69(1)ANN. THORAC. SURG. 186-92(2000)を参照されたい。脳特異的タンパク質であるグリア線維性酸性タンパク質(GFAP)は、これらのECMO中の脆弱な患者において、神経学的損傷についての血漿バイオマーカーの役割を果たす可能性もあるという仮説が立てられる。
脳卒中、脳MRIの異常、および神経発達欠損として顕在化する脳損傷は、先天性心疾患(CHD)の外科的矯正後の乳児の30%から70%に発生する。Andropoulos他、139 J. THORAC. CARDIOVASC. SURG. 543-56(2010);およびMcQuillen他、38 STROKE 736-41(2007)を参照されたい。1室または2室CHDの新生児期修復の研究(n=62)では、患者の39%が術前に損傷を有し(ほとんどが脳卒中)、また35%が術後に新規病変を有すると報告した。McQuillen他、38 STROKE 736-41(2007)を参照されたい。本発明者らは、GFAPが、先天性心疾患の外科的修復を目的とした心肺バイパス中の脳損傷の診断的/予後判定的バイオマーカーの役割を果たす可能性があるか検討している。心肺バイパス血清サンプルは、20例の連続乳児症例(生後2日〜3ヶ月)より、先天性心疾患の外科的修復を目的とした心肺バイパスを受ける前、心肺バイパス中(30分おき)、および心肺バイパス後に採取した。
本試験の目的は、小児において血漿グリア線維性酸性タンパク質が体外膜型酸素供給中の脳損傷および死亡と関連しているか判定することであった。2008年4月から2009年8月までに体外膜型酸素供給を必要とした生後1日から18歳までの予測的患者を検討した。ジョンズ・ホプキンズ大学で開発した電気化学発光イムノアッセイを用いてグリア線維性酸性タンパク質を測定した。対照サンプルは、健康な小児99例(0.5〜16歳)および神経学的損傷のない新生児集中治療室乳児59例から採取した。対照においては、グリア線維性酸性タンパク質濃度の中央値は0.055ng/mL(四分位数間範囲、0〜0.092ng/mL)であり、グリア線維性酸性タンパク質の95パーセンタイルは0.436ng/mLであった。体外膜型酸素供給中の患者において、カニューレ留置より6、12時間後および24時間毎に血漿グリア線維性酸性タンパク質を測定した。体外膜型酸素供給を受けた22例の小児を組み入れた。年齢の中央値は生後7日であり(四分位間数範囲、生後2日から9歳)、また体外膜型酸素供給の主な適応症は:心不全、22例中6例(27.3%);呼吸不全、22例中12例(54.5%);体外循環式心肺蘇生22例中3例(13.6%);および敗血症、22例中1例(4.6%)であった。22例中7例(32%)の患者は急性神経学的損傷(頭蓋内出血、脳死、または画像によって診断される脳浮腫)を発症した。22例中15例(68%)は退院まで生存した。体外膜型酸素供給群では、ピークグリア線維性酸性タンパク質レベルは脳損傷を有する小児の方が脳損傷のない小児よりも高く(中央値5.9ng/mL対0.09ng/mL、p<0.04)、かつ非生存者の方が退院時生存者よりも高かった(5.9ng/mL対0.09ng/mL、p=0.01)。グリア線維性酸性タンパク質>0.436ng/mLの正常値に対する脳損傷オッズ比は11.5であり(95%信頼区間、1.3〜98.3)、かつ死亡オッズ比は13.6であった(95%信頼区間、1.7〜108.5)。
(試験デザイン)
本研究は、2008年4月から2009年8月までに大学病院小児科センターである単独の三次診療施設の26床小児集中治療室でECMOを受けた小児を対象とした予測的観察的コホート研究であった。何らかの適応症によりECMOを必要とした18歳未満の患者が本研究の対象であった。このコホートは、この集団における脳損傷バイオマーカーを検討することを副次的な目的とした、ECMO中の神経学的損傷の凝血関連リスク因子の研究のために開始された。除外基準は、ヘパリン起因性血小板減少症の病歴およびECMO中の抗凝血を目的とした直接トロンビン阻害薬の使用歴であった。患者が安定した後のECMOカニューレ留置より6時間以内に、両親または法律上の保護者が小児集中治療室にいる場合にのみ同意を申し入れた。電話で同意を取得することはなかった。組み入れた患者についてそれぞれ人口統計学的データ、臨床データ、臨床検査データ、画像検査データ、および生存データを収集した。ECMO回路は:シリコンレザバー付きカスタムパック1/4インチまたは3/8インチフレキシブル塩化ビニルチューブ(メドトロニック、ミネソタ州ミネアポリス)、ブラダーボックス(ジョンズ・ホプキンズ病院、メリーランド州ボルチモア)、0.8m2から4.5m2の膜式酸素供給装置(メドトロニック)、熱交換器(メドトロニック)、およびローラーポンプ(Sorin Cardiovascular USA、コロラド州アルバダ)より構成された。本研究は、ジョンズ・ホプキンズ大学治験審査委員会により承認された。
静脈血サンプル(クエン酸ナトリウム3.2%に5mL)をEMCO開始後6、12および24時間、さらにその後EMCOカテーテル抜去まで連日採取した。1時間以内に遠心分離した後、血小板量の低い血漿を−80℃で保存した。ジョンズ・ホプキンズ大学でメソスケールディスカバリープラットフォーム(メソスケールディスカバリー、メリーランド州ゲーサーズバーグ)上で開発し、かつPetzold他、287 J. IMMUNOL. METHODS 169-77(2004)の測定法に基づく電気化学発光サンドイッチイムノアッセイを用いたGFAP測定には、未希釈血漿サンプル50μLを2回ずつ用いた。捕捉抗体としてモノクローナル抗GFAPブレンドSMI−26(コ−バンス、ニュージャージー州プリンストン)各ウェル100ngを、メーカーによって、または実験室でリン酸緩衝化生理食塩水を用いて1晩インキュベートしてコーティングした標準的な結合プレート(メソスケールディスカバリー)において用いた。スルフォダグ(メソスケールディスカバリー)と直接コンジュゲートしたポリクローナル抗GFAP(ダコ、カリフォルニア州カーピンテリア)を、リン酸緩衝化生理食塩水中に1μg/Lで検出のために用いた。プレートはSector Imager 2400(メソスケールディスカバリー)で読み取った。標準曲線は、1%ウシ血清アルブミン溶液(SeraCare Life Sciences、マサチューセッツ州ミルフォード)中の精製GFAP(カルビオケム、カリフォルニア州ラ・ホーヤ)を用いて構築した。至適抗体濃度、プレートの種類、およびブロッキング材料を決定する実験の後、最終的なGFAP値の測定法は線形定量範囲0.040〜40.0ng/mLの標準曲線を有した。本GFAP測定法は、2SDがブランクウェルのバックグラウンドを上回ると定義される検出下限値が0.011ng/mLであった(実験回数n=19)。数値<0.040ng/mLはゼロとして報告した。0.01ng/mLにおける信号雑音比は1.17であった(実験回数n=19)。算出濃度±既知の濃度の20%となる最小希釈度と定義される定量下限値は0.040ng/mLであった(実験回数n=3)。日間再現性は10ng/mLにおいて2.4%であり、かつ0.156ng/mLにおいては3.4%であった(実験回数n=21)。GFAPを10ng/mLで添加した血漿は、ウシ血清アルブミンにおいて生成した標準曲線と比較して49.8%±22.9%の回収率を示す。GFAP測定法の実証は、廃棄された正常および陽性対照診断検体を用いて実施した。このGFAP測定法実証試験は、ジョンズ・ホプキンズ病院治験審査会で同意の放棄とともに別個の申請で承認された。
主要独立変数は、小児の正常値の95パーセンタイルを上回る血漿GFAPの上昇であった。主要転帰は、患者がECMOサポートを受ける間に、小児神経科医が脳画像検査および/または神経学的検査によって診断する、脳死と一致する頭蓋内出血、脳梗塞または脳浮腫と定義される、ECMO中の急性神経学的損傷の発症であった。新生児および泉門が閉鎖していない乳児の頭部超音波検査を毎日実施するのは院内プロトコルであり;より年齢の高い小児は脳コンピュータ断層撮影または磁気共鳴画像検査を担当医の裁量で撮影した。全ての画像検査は、小児放射線科医が通常の診療の一環として審査した。副次転帰は神経学的転帰および退院時までの生存であった。神経学的転帰は、小児脳機能カテゴリー(PCPC)を用いて測定した。Fiser他、28 CRIT. PEDIATR. 68-74(1992)を参照されたい。PCPCは、小児集中治療における認知能力の変化を評価することを目的としてグラスゴー転帰スケールから開発された6点スケールである。PCPCの6カテゴリーは、1)正常、年齢に対して適切な神経発達機能;2)軽度の脳障害、3)中等度の脳障害;4)重度の脳障害;5)昏睡または植物状態、および6)脳死である。Fiser他、28 CRIT. CARE MED. 2616-20(2000);およびFiser他、121 J. PEDIATR. 68-74(1992)を参照されたい。熟練した小児クリティカルケア専門医が、入院時および退院時の患者の病状のチャートを審査することより、遡及的にPCPCを当てはめた。良好な神経学的転帰は、先験的に、退院時のPCPCが1または2であるか、または入院時のPCPCから変化しないと定義した。
探索的記述データ解析を実施して患者特性およびECMO過程の特性を検討し、被験者におけるGFAP値の分布を記述し、かつGFAPレベルが95パーセンタイルを上回る被験者の割合を判定した。クラスカル・ワリス検定を用いて正常対照のGFAP濃度を年齢カテゴリー間で比較した。患者は各転帰で2つのカテゴリー:急性神経学的損傷を有する者と損傷のない者、退院時の神経学的転帰が良好な者と不良な者、および退院時生存者と非生存者に分類した。マン・ホイットニーU検定を用いて、これらの群間でピークGFAPの中央値を比較した。フィッシャーの正確検定を用いて、群間でピークGFAPの95パーセンタイルを上回る症例および下回る症例の百分率を比較した。患者別のクラスター化によるロジスティック回帰を用い、全連続GFAPデータポイントを用いて脳損傷および死亡のオッズを概算した。クラスター化分析のために、被験者は画像検査による脳損傷まで急性神経学的損傷がないか、または初回の神経学的検査が脳死と一致するかでコード化した。その後の所見は、急性神経学的損傷を有するとしてコード化した。オッズ比(OR)および95%信頼区間(CI)を提供する。p値0.05は有意と見なした。統計分析はSTATA10.1(StataCorp、テキサス州カレッジステーション)を用いて実施した。
(患者特性)
条件を満たす患者46例のうち22例を組み入れた;18例の患者の両親は小児集中治療室にいなかったか、または6時間の同意取得期間内に同意について完全に話し合うことが不可能であり、5例の患者は同意を辞退し、1例の患者は試験に利用できるサンプルがなかった。評価した22例の患者の人口統計学的および臨床的特性を図14に示す。
既知の神経学的損傷のない正常小児対照において、GFAPレベルは中央値が0.055ng/mL(IQR、0〜0.092ng/mL)であった。GFAPレベルは年齢カテゴリーにわたって同様であった(新生児:中央値0.041ng/mL[IQR,0〜0.096ng/mL]、生後6ヶ月から4歳:中央値0.046ng/mL[IQR、0〜0.117ng/mL]、5〜16歳:中央値0.057ng/mL[IQR、0〜0.088ng/mL]、p=0.7)。小児における正常値の95パーセンタイルカットオフ値(血漿GFAP≦0.436ng/mL)は、乳児および小児158例のサンプル:既知の遺伝的障害または頭蓋内病変のない新生児集中治療室内の生後<4日の新生児59例、および健康小児検診のためにジョンズ・ホプキンズ病院小児科外来を受診した生後6ヶ月から16歳の健康小児99例を用いて決定した。当該測定法は、脳腫瘍切除(n=13)、脳生検(n=3)、および脳卒中の患者(n=12)における神経学的損傷の検出についてさらに実証した。陽性対照サンプル中のGFAPレベルは、全般的に正常対照よりも1から55倍高かった。
EMCO開始より12時間以内の初期GFAPレベルの中央値は0.07ng/mL(IQR、0〜0.155ng/ml)であった。カニューレ留置より24時間の間にGFAP濃度が異常であった患者が22例中3例あり;2例は心停止が持続して右頸部血管の静動脈カニューレ留置によるECPRを受けた患者であり、かつ1例はダブルルーメン右頸静脈カニューレ留置により静静脈ECMOに置かれた既知の神経学的損傷のない患者であった。22例のうち他の19例は、カニューレ留置より24時間のGFAPレベルが低かった。サンプルが3件またはそれ以上である小児のECMO最終日のGFAPレベルの中央値は0.07ng/mLであった(IQR、0.053〜0.795ng/mL;n=17)。新生児と小児および生後>30日の乳児のピークGFAP濃度を比較したところ同様であった(それぞれ0.155ng/mLと0.162ng/mL、p=0.48)。
ECMOは、頭蓋内出血、脳梗塞および脳死を含む脳損傷のリスクの高い手技である。Cengiz他33 CRIT. CARE MED. 2817-24(2005);Conrad他、51 ASAIO J. 4-10(2004);およびCilley他、78 PEDIATRICS 699-704(1986)。ECMO中の患者に対するそのような損傷の適時評価のための手段を欠くことが多い。急性の神経学的発作は危険な病状の患者においては大きな懸念であるが、多くの共同作業が進行中ではあるものの通常の臨床診療で利用できる脳損傷バイオマーカーはまだない。Kaneko他、80 RESUSCITATION 790-94(2009);およびLaskowitz et al., 40 STROKE 77-85(2009)を参照されたい。本研究で用いられる血漿GFAPバイオマーカーは、脳に対する高い特異性、血液が小体積であることによるサンプル採取の容易さ、高速度処理、高精度定量、低コスト、および測定法に必要とされる技術的専門性が最小限であることなど、多くの利点を有する。したがって、連続GFAP測定は神経学的状態および潜在的な神経保護的介入に対する反応をモニタリングし、急性脳損傷の迅速な診断を補助し、さらに転帰を予測するために用いることも可能である。
(被験者)
これは、治験審査委員会の承認を受け、単独の三次診療大学病院の新生児集中治療室(NICU)に入院した新生児を検討した予測的コホート研究である。被験者は妊娠第36〜41週に出生した生存出生、非奇形、無症候乳児であった。本研究はジョンズ・ホプキンズ病院で出生した新生児、さらにはメリーランド州内で出生して生後6時間以内にNICUに搬送された新生児を含んだ。
GFAP測定用に採取した検体は臍帯血および新生児血清を含んだ。臍帯血については、分娩時に常用的に採取する臍帯静脈血サンプルより少量を分取した。新生児サンプルについては、臨床適応の検査が終了した後に連日の臨床検査から残りの血清分画を採取した。新生児血清検体は、NICU入院時(生後6時間以内)、およびその後非神経学的損傷対照は生後連日4日間、また全身冷却を受けたHIE新生児は連日7日間採取した。
メソスケールプラットフォーム(メソスケールディスカバリー、メリーランド州ゲッサーズバーグ)を用いて、GFAP用に電気化学発光サンドイッチイムノアッセイを開発した。Pelinka他、57 J. METHODS 169-77(2004)を参照されたい。血清サンプルは2回測定し、平均濃度を分析に用いた。定量下限値は0.04ng/mLであり;これ以下の数値は0と報告した。
カテゴリー変数にはχ2検定またはフィッシャーの正確検定、および連続変数には対応のないスチューデントのt−検定を用いて比較した。ノンパラメトリックデータのためのウィルコクソン順位和検定を用いてGFAPレベルを比較した。有意性はp<0.05に設定した。線形回帰を用いて、非神経学的損傷対照集団のGFAPレベルに対する妊娠期間の影響を判定した。
(被験集団および臨床的特性)
2009年4月28日から2010年7月11日までの期間中、NICUに652例の入院があり、このうち連続23例の新生児が全身冷却に適した中等度/重度の臨床的HIEと診断された。これら23例の新生児は、生後1週間以内に、非神経学的適応症でNICUに入院した新生児と妊娠期間により1:1でマッチングした。冷却を受けた新生児の平均(SD)妊娠期間は38.7(1.5)週であり、対照は39(1.4)週であった。
血清量が不足、その日に採血を実施していない、または外部の病院で分娩したため臍帯血を採取していないために、対照については所望の138測定時のうち54測定時(39.1%)、冷却を受けた被験者については207測定時中50測定時(24.2%)でGFAPレベルを測定することができなかった。全身冷却で治療された臨床的に中等度から重度のHIE新生児を対照と比較したところ、生後6時間以内のNICU入院時および生後1,3,および4日の新生児の血清中の平均GFAPレベルは、それぞれp=0.032、p=0.013、p=0.013、およびp=0.003で有意に上昇した。図19を参照されたい。
全身冷却を受けた23例の新生児は、いずれもそのNICU入院中に臨床的脳MRI検査を受けた。分娩後にこれらのMRIスキャンを実施した時期に差はなく、異常MRI群では平均(SD)が生後7.4(3.7)日であり、また正常MRI群では生後7.4(3.9)日であった(p=0.49)。HIEを示唆する所見は23例中8例の脳MRIスキャンに見られた(35%)。異常脳MRI新生児は胎盤剥離後に娩出された可能性の方が有意に高いものの、HIEの臨床マーカーはいずれもMRIの異常と関連しなかった。図20を参照されたい。
本実施例は、中等度から重度の臨床的HIE、脳損傷のMRIエビデンスの発生、および機能的転帰と相関する血清バイオマーカーとしての脳特異性タンパク質GFAPの初の使用を記載する。HIEは1000正期産出生児につき2.5例発生し、脳性麻痺症例全体のうち14.5%は分娩時低酸素性虚血と関連するものの、これらの損傷の時期、持続期間および転帰については十分に定義されていない。Graham他、199 AM. J. OBSTET. GYNECOL. 587-95(2008)を参照されたい。
脳損傷のバイオマーカーには、経頭蓋ドップラーに例示されるように、鎌状赤血球症の診療を変換させる能力がある。鎌状赤血球症(SCD)患者から採取した血漿のバイアスのないプロテオミクススクリーンによって、無症候性脳損傷のバイオマーカーが得られるであろうという仮説が立てられた。12種類の最も含有量の高いタンパク質を血漿から除去処理した後、サイレント梗塞輸血(SIT)治験の一環として採取した血漿の質量分析スクリーンにより、トロンボスポンジン−1(TSP−1)および可溶性L−セレクチン(SELL)が得られた。市販のELISAを用いたところ、TSP−1およびSELLは正常年齢マッチング対照(n=23)とSCD患者(n=123)間で差がなかった。しかし、SIT治験にスクリーニングされた5〜14歳のSCD被験者の無作為サンプリングSCI患者(n=62)および非SCI患者(n=51)は、TSP−1およびSELLが有意に上昇することを示した(それぞれp=0.013およびp=0.021)。これらのデータより、鎌状赤血球症における脳損傷の実験的バイオマーカー探索手法が実証され、かつSCIの血流バイオマーカーが始めて記載される。
鎌状赤血球症(SCD)は、多臓器損傷を特徴とする慢性溶血性貧血である。脳卒中は、SCDにおいて脳に対して最も顕著に発生しうる損傷である。経頭蓋ドップラー速度が上昇した患者を特定しかつ長期的な赤血球輸血を実施することにより、小児の脳卒中リスクは有意に低下している。King他、50 PEDIATR. BLOOD CANCER 599-602(2008);およびAdams 他、339 N. ENGL. J. MED. 5-11(1998)を参照されたい。MRI上でサイレント脳梗塞(SCI)の存在により確認される脳損傷のリスクが長期的赤血球輸血によって軽減されるかは、現在研究中である。King他、50 PEDIATR. BLOOD CANCER 599-602(2008)。脳卒中リスクのバイオマーカーは、明らかに鎌状赤血球症の診療を変容させている。
(患者)
サイレント梗塞輸血治験(SIT治験、ClinicalTrials.gov NCT00072761)のためにスクリーニングされた鎌状赤血球症を有する5〜14歳の小児の横断サンプル(HbSSおよびHbSβ0)を検討した(n=259)。SIT治験は、鎌状赤血球症およびSCIを有する小児に対する3年間輸血プログラムの多施設無作為化対照試験である。主要評価項目は顕性脳卒中あるいは新規または進行性SCIの発生を含む。全ての患者はインフォームドコンセントに署名した。SCIは、正常な神経学的検査およびT2強調画像の2視野で目視できるMRIシグナル異常によって定義される。シグナル異常は、1次元で3mm以上でなければならない。SCI状態は、神経放射線科医および脳神経科医のパネルによって宣告される。陽性および陰性対照患者は、ジョンズ・ホプキンズ病院の外来および入院病棟から選択した。陽性対照血漿サンプルは、入院した小児または顕性脳梗塞または脳手術で入院した成人から採取した。陰性対象は、ジョンズ・ホプキンズ病院のHarriet Lane小児科外来の5〜16歳の小児から選択した。外来記録を審査して何らかの急性疾患、神経学的障害、または喘息、肥満および行動/気分障害以外の慢性疾患を有する患者を除外した。IRBの承認を受けた研究により、これらの対照についての非特定血液サンプルおよび臨床データを取得した。
血液をACDまたはEDTA管に採取し、SIT治験プロトコルに従って1500gで8分間遠心分離し、ジョンズ・ホプキンズ大学SIT治験用Biologic Repositoryに分析時まで−70℃で保存した。ProteomeLab Protein Partitioning System(PPS、ベックマン)上のLC10 IgYカラム(ベックマン・コ−ルター、カリフォルニア州フラートン)を用いて、患者15例(探索コホート、非SCI n=7、およびSCI n=8)より採取した血漿500μLから12種類の血漿大量タンパク質を除去処理した。IgYカラムのフロースルーを、連続アセトニトリル勾配(PPS、ベックマン)を用いるC18カラム(Jupiter,Phenomenex)を通す逆相HPLCで39分画に分離した。分画を乾燥させ(SpeedVac、サーモサイエンティフィック、マサチューセッツ州ウォルサム)、37℃で一晩トリプシン消化した(プロメガ、ウィスコンシン州マジソン)。各サンプルのスペクトルをLC/MS/MS(LTQ−Orbitrap、サーモサイエンティフィック)により取得した。Xtandem!およびヒトIPIデータベースバージョン3.4を用いてスペクトルを検索し、タンパク質を同定した。PASS(Integrated Analysis社、メリーランド州ベセスダ)を用い、信頼水準<0.9をタンパク質同定カットオフ値として検索後分析を実施した。
TSP−1およびSELLは、メーカーのプロトコルにしたがい、サンドイッチELISA(R&Dシステムズ)を用いて希釈血漿サンプルで2回測定した。SCIおよび非SCI患者サンプルを無作為に選択した。
血漿濃度を群間比較するためにスチューデントのt−検定を用いた。
(SCD血漿中のTSP−1およびSELLの質量スペクトル同定)
TSP−1およびSELLはそのSCDにおける役割が十分に理解されていない白血球接着分子として機能するが(Ugochukwu他、30(4)INT. J. LAB. HEMATOL. 312-16(2008);およびBrittain他、97(7)BLOOD 2159-64(2001))、血中SELLは小児の脳外傷の高感度かつ特異的バイオマーカーであることが示されている。探索コホート(患者数n=15)の検索にXtandem!を用いたところ、それぞれ10例および13例の患者においてTSP−1については14の特異なペプチド、およびSELLにおいては3種類のペプチド。Xtandem!Log(e)スコアの範囲は−1.1から−10.1であった。TSP−1(図23)およびSELL(図24)のそれぞれについてのペプチドの代表的スペクトルを示す。
いくつかの研究により、脳内の原位置TSP−1は脳損傷の回復にとって重要であり、かつTSP−1はSCD赤血球の内皮細胞への接着性を高める役割も果たすことができることが示されている。表6および図25に示すように、TSP−1レベルは、年齢マッチングした正常小児とSCD小児間で比較するとき、濃度に有意差がない(p=0.49)。しかし、表7および図26に示すように、SCI陽性患者とSCI陰性患者の間でTSP−1濃度の統計的に有意差な上昇があった(p=0.013)。
SELLは、そのSCDにおける役割が十分に理解されていない白血球接着分子であるが、血流中SELLは小児脳外傷の高感度かつ特異的バイオマーカーであることが示されている。Lo他、26(9)J. NEUROTRAUMA 1479-87(2009)。SELLはSITT探索コホート15例中13例に認められたので、SELLがSCD血漿中に存在することおよびSCIにとっての重要性が実証された。平均SELLレベルは人種および年齢マッチング対照とSCD患者の間で有意差がない。しかし、SCIおよび非SCI群を比較するとき(図27)、平均SELLレベルはSCI群で有意に上昇していた(p=0.021)。
鎌形赤血球症における血漿プロテオームのバイアスのないスクリーンを用いたところ、2つの血中タンパク質TSP−1およびSELLが、SCD小児におけるSCIと有意に関連するとして同定された。鎌形赤血球症患者の血漿からのこのような特異性の高いタンパク質の探索により、本実施例で用いられるバイオマーカー探索の実験的手法が実証される。
(ヒトNRGN特異性アプタマーの同定)
指数関数的濃縮によるリガンドの系統的発生法(SELEX)の手順を用いて、ヒトNRGN特異性アプタマーを同定した。簡単に述べると、フィルター固定により一本鎖RNAのプールから特異的アプタマーを選択した。RNA−NRGN標的複合体はニトロセルロースフィルターと結合することが可能であり、かつ遊離RNAが通過して濾過された。特異的アプタマーをフィルターより回収し、PCR増幅した。数ラウンドの選択の後、NRGNに対する親和性の最も高い特異性アプタマーを濃縮し、シークエンシングにより同定した。
一本鎖DNAオリゴプールを化学的に合成した。DNAオリゴは、2つの定常配列に挟まれた40量体ランダム中心核を有し、またライブラリはライブラリの5’および3’保存末端を標的とする1対のプライマーによって増幅することができる。ライブラリの配列は:

まず一本鎖DNAライブラリをSel2 5’プライマーでアニーリングし、配列はインビトロ転写のためのT7プロモーターを含む5’−GGG GGA ATT CTA ATA CGA CTC ACT ATA GGG AGG ACG ATG CGG−3’(配列番号2)であった。ギャップは、37℃で1.5時間実施するKlenow反応で埋めた。反応物は、フェノール:クロロホルム:イソアミル(25:24:1)およびクロロホルム:イソアミル(24:1)抽出各1回によって精製し、またさらにCentricon 30を用いて4℃で濃縮した。濃縮プロセス中、TE緩衝液pH7.4を用いて反応物を2回洗った。最終OD260を測定し、濃度を算出した。
13mm Swin−Lokフィルターホルダー(ワットマン)(直径13mm)、孔径0.45μmのHAWPニトロセルロースディスクフィルター(ミリポア)を組み立てた。フィルターは、Hepes pH7.4を20mM、NaClを50mMおよびCaCl2を2mM含有する洗浄緩衝液1mLで事前に湿らせた。500pmolのRNAを100μLの1×結合緩衝液(0.1%BSAを添加した以外は洗浄緩衝液と同じ処方)で希釈し、さらにフィルターホルダーのレザバーに適用した。フィルターホルダーを50mLコニカルチューブに密封し、37℃で30分間インキュベートした。インキュベーション後、1mLシリンジを用いてフィルターユニットを通過させることによりRNAを回収し、また100μLの1×結合緩衝液を用いて1回洗った。フィルターを通過したRNAを回収した;事前洗浄RNAの総量は約180μLであった。
結合反応は、50pmolヒトNRGNタンパク質を事前洗浄RNAに添加することによって構築され、RNA:タンパク質の分子比率は約10:1であった。総体積は1×結合緩衝液中で200μLとした。反応物を37℃で15分間インキュベートした。新しいフィルターホルダーを組み立て、上述の方法で事前に湿らせ、結合反応物をフィルターに適用し、5mLシリンジを用いて結合サンプルを通過させて押し出し、さらにフィルターを5mL洗浄緩衝液で洗った。
フィルターホルダーを分解し、さらにフェノール:クロロホルム:イソアミル(25:24:1)600μLを入れた1.5mL遠心管にフィルターを移した。遠心管をボルテックスミキサーで約1分間激しく振盪した後、室温で30分間インキュベートした。H2Oを200μL添加し、再度ボルテックスミキサーで振盪した後、14,000rpmで10分間遠心分離した。回収したRNAを含有する上清をクロロホルム400μLで1回抽出した後、エタノール500μL、3M酢酸Na20μL(pH5.2)およびグリコーゲンブルー(Ambion、5mg/mL)3μLを添加することで沈殿させ、その後−80℃で一晩インキュベートした。14000rpmで20分間4℃で遠心分離することによりRNAを回収した後、75%エタノール1mLで1回洗い、その後RNAを遠心分離して風乾した。乾燥したRNAペレットをH2O20μLに溶解した。
回収したRNA5μLを用いて第1のDNA鎖を合成した。プライマーSel2 3’2μmolを反応物に添加した。Sel2 3’プライマーの配列は:5’−TCT CGG ATC CTC AGC GAG TCG TC−3’(配列番号3)である。反応物にロシェより入手した逆転写酵素(カタログ番号:10 109 118 001)を用い、条件はメーカーが推奨する通りとした。
濃縮PCR生成物1μgを用いて、次のラウンドの選択に用いるRNAを生成し、インビトロ転写のプロトコルの後に上述の操作を実施した。合計10ラウンドの選択を実施した。
6種類のクローンをシークエンシング用に選択し、また全ての配列の品質は非常に高かった。DNAアラインメント分析後、NRGN−A4のヌクレオチドが1個異なっていたことを除いて(下線部)6種類のクローンのうち5種類はほぼ同一であった。類似性の低い配列NRGN−A6は、整合性の良い他の5クローンと比較すると、その全てが2Tに富むモチーフを有し、これらがCC/CA(囲み部分)によって分離されていることを示した。NRGN−A1およびNRGN−A6を実証の対象に選択した。以下にアプタマーの配列を示す:
同定された配列に従ってNRGN−A1およびNRGN−A6 RNAを化学的に合成し;RNA3’末端にビオチンリンカーを付加した。異なる2種類の戦略を用いてNRGNアプタマーとNRGN組換えタンパク質の相互作用を試験した。詳細を以下に記載する。
SELEX手順で用いたものと同じメカニズムに基づいてRNA−タンパク質複合体をニトロセルロース膜上に保持することが可能であり、ドットブロットを用いてビオチン標識RNAアプタマーを検出した。まず、His−NRGN組換えタンパク質の連続2倍希釈を作製し、タンパク質の量の範囲は10pmolから0とし;その後さらに各サンプルをNRGNアプタマーRNA1pmolと混合した。最終結合反応の体積を1×結合緩衝液Fで20μLに維持し、反応物を37℃で15分間インキュベートした。
ビオチン標識アプタマーをストレプトアビジン粒子に固定し、さらにプルダウン測定法を実施した。アプタマーをTEN100緩衝液(10mM Tris−HCl、pH7.5、1mM EDTA、100mM NaCl)で1pmol/μLに希釈し、65℃で5分間加熱した後、室温で20分間放置してRNAをその天然コンホメーションにフォールディングさせた。ストレプトアビジン磁性粒子(ロシェ、11641778001)を2倍体積のTEN100緩衝液で3回洗った後、アプタマーサンプルを添加して回転させながら室温で30分間インキュベートした。インキュベーション後、粒子をTEN100緩衝液で3回洗い、その後1×結合緩衝液Fで1回平衡させた。種々の量のNRGNタンパク質(0〜2nmom)を1×結合緩衝液で希釈し、アプタマーを固定した磁性粒子に添加した後、37℃で15分間インキュベートした。
質量分析定量MRM測定法用のニューログラニンシグネチャーペプチドを開発した。ペプチドの配列および転移を以下の表に示す。標識GPGPGGPGGAGVAR(配列番号7)をサンプルに添加して、シグネチャーペプチドGPGPGGPGGAGVAR(配列番号7)の濃度を測定するための標準曲線を作製した。ペプチドKGPGPGGPGGAGVAR(配列番号8)もモニタリングして、トリプシン消化による誤った開裂がないことを確認する。
ヒトNRGN cDNAクローンはOrigeneより購入した(カタログ番号:RC201209)。コード配列を、制限酵素(Sgf I+Mlu I)フラグメントスワッピングによりデスティネーションベクター(Origene、pEX−N−His、カタログ番号PS100030)にクローニングし、pEX−N−His−NRGN発現プラスミドを生成した。コード配列およびリーディングフレームはDNAシークエンシングにより確認した。
ジョンズ・ホプキンズ大学において、モノクローナル抗体の産生のために上記の組換えニューログラニンを用いてマウスを免疫した。30のクローンをスクリーニングし、クローン30.5.2を図32に示す直接ELISAにおいて高希釈度ニューログラニンと結合すると特定した。
濃度75ng/ウェルのニューログラニン抗ヒトモノクローナル抗体(ジョンズ・ホプキンズ)を捕捉抗体として用い、かつ濃度0.5μg/mLのニューログラニンに対するウサギポリクローナル抗体(ジョンズ・ホプキンズ)を検出抗体として用いた。濃度1μg/mLのSULFO−TAG抗ウサギ抗体(MSDカタログ番号R32AB−1)を濃度1μg/mLで標識レポーターとして用いた。GST_NRGN組換えタンパク質(ジョンズ・ホプキンズ)を、開始濃度20ng/mL、その後PBS/1%BSAの7希釈用に対して1:2で標準物質として用いた。PBS/1%BSAをブランクとして用いた。本測定法の標準曲線を図33に示す。
上記のニューログラニンサンドイッチ測定法を用いると、心呼吸器不全のために27日間ECMOサポートを受ける乳児から採取した血清サンプル。乳児は連日頭部超音波測定を実施し、死亡時には脳損傷がないと考えられた。剖検では、脳に多発性皮質梗塞がありそれらは超音波では診断されなかった。図34に示すように、GFAPレベルはECMOサポートの粗さを通じて変化しなかった。しかし、ニューログラニンレベルは14日間のECMOサポートの間に初期値の15倍のピークまで上昇した。ニューログラニンは灰白質、ニューロンマーカーであるので皮質灰白質損傷に対する感度は白質損傷マーカーGFAPよりも高かった。これは、ニューログラニンが急性皮質脳損傷の血中バイオマーカーでありかつGFAPと組み合わせると脳白質損傷を脳灰白質損傷と識別することができるというエビデンスを提供する。

前記の事前に定義したレベルのうち1つとの相関が診断を提供することを特徴とする比較することという段階を含む前記方法。 - 請求項20に記載の前記方法であって、前記のバイオマーカーパネルがOMGおよびMT3をさらに含むことを特徴とする前記方法。
- 請求項20に記載の前記方法であって、前記バイオマーカーパネルがASTN1、BAI3、CNDP1、ERMIN、GRM3、KLH32、MAGE2、NRG3、OMG、SLC39A12、RTN1、およびMT3をさらに含むことを特徴とする前記方法。
- 患者の脳損傷状態を判定するための方法であって:
a.前記患者からサンプルを採取すること;
b.バイオマーカーパネルがASTN1、BAI3、CNDP1、ERMIN、GFAP、GRM3、KLH32、MAGE2、NRG3、NRGN、OMG、SLC39A12、RTN1、およびMT3を含むことを特徴とする、前記患者から採取した前記サンプル中の前記バイオマーカーパネルのレベルを測定すること;および
c.前記バイオマーカーパネルのレベルを脳損傷を有すること、脳損傷がないこと、脳損傷が進行すること、および脳損傷が後退することからなる群から選択される1つまたはそれ以上の脳損傷状態と相関する同じバイオマーカーパネルの事前に定義したレベルと比較することであって、
前記の事前に定義したレベルのうち1つとの相関が前記患者の前記脳損傷状態を判定することを特徴とする比較することという段階を含む前記方法。 - 患者のSCI状態を判定するための方法であって:
a.前記患者からサンプルを採取すること;
b.バイオマーカーパネルがASTN1、BAI3、CNDP1、ERMIN、GFAP、GRM3、KLH32、MAGE2、NRG3、NRGN、OMG、SLC39A12、RTN1、およびMT3を含むことを特徴とする、前記患者から採取した前記サンプル中の前記バイオマーカーパネルのレベルを測定すること;および
c.前記バイオマーカーパネルのレベルをSCIを有すること、SCIがないこと、SCIが進行すること、およびSCIが後退することからなる群から選択される1つまたはそれ以上のSCI状態と相関する同じバイオマーカーパネルの事前に定義したレベルと比較することであって、
前記の事前に定義したレベルのうち1つとの相関が前記患者のSCI状態を判定することを特徴とする比較することという段階を含む前記方法。 - 患者の脳損傷を診断するための方法であって:
a.前記患者からサンプルを採取すること;
b.前記患者から採取した前記サンプル中の1つまたはそれ以上のバイオマーカーのレベルを測定すること;および
c.前記1つまたはそれ以上のバイオマーカーのレベルを脳損傷を有する患者と相関する同じバイオマーカーの事前に定義したレベルおよび脳損傷のない患者と相関する同じバイオマーカーの事前に定義したレベルと比較することであって、
前記の事前に定義したレベルのうち1つとの相関が診断を提供することを特徴とする比較することという段階を含む前記方法。 - 請求項1に記載の前記方法であって、前記の1つまたはそれ以上のバイオマーカーがアストロタクチン1(ASTN1)、脳血管新生阻害物質3(BAI3);カルノシンジペプチダーゼ(CNDP1);ERMIN;グリア線維性酸性タンパク質(GFAP);代謝型グルタミン酸受容体3(GRM3);ケルヒ様タンパク質32(KLH32);メラノーマ抗原ファミリーE,2(MAGE2);ニューレグリン3(NRG3);ニューログラニン(NRGN);乏突起膠細胞ミエリン糖タンパク質(OMG);溶質輸送体ファミリー39(亜鉛輸送体)、メンバー12(SLC39A12);レティキュロン(RTN1);およびメタロチオネイン(MT3)からなる群から選択されることを特徴とする前記方法。
- 請求項1〜26のうちいずれかに記載の前記方法であって、前記の測定する段階がイムノアッセイ、免疫ブロット法、免疫沈降測定法、免疫染色法、定量測定法、免疫蛍光測定法、または化学発光測定法を含むことを特徴とする前記方法。
- 患者の脳損傷状態を判定するためのキット方法であって:
a.患者から生物学的サンプルを採取するための基材;および
b.ASTN1、BAI3、CNDP1、ERMIN、GFAP、GRM3、KLH32、MAGE2、NRG3、NRGN、OMG、SLC39A12、RTN1、およびMT3からなる群から選択される1つまたはそれ以上のバイオマーカーのレベルを測定するための手段を含む前記キット。
