Abstract
本開示は概して、CGRP受容体アンタゴニストである式(I)の化合物、(R)−N−(3−(7−メチル−1H−インダゾール−5−イル)−1−(4−(1−メチルピペリジン−4−イル)ピペラジン−1−イル)−1−オキソプロパン−2−イル)−4−(2−オキソ−1,2−ジヒドロキノリン−3−イル)ピペリジン−1−カルボキシアミド(その医薬的に許容される塩を含む)に関連する。本開示はまた、該化合物をCGRPが関連する障害、例えば、片頭痛、神経原性血管拡張、神経原性炎症、熱傷、循環性ショック、閉経に伴う紅潮、喘息のような炎症性呼吸器疾患症候群、慢性閉塞性肺疾患(COPD)、および癌の治療に用いるための医薬組成物および方法に関連する。
Description
本願は米国仮出願番号第61/319,015号(2010年3月30日出願)の利益を主張する。
本発明は、本発明の化合物の全ての医薬的に許容される塩の形態を包含する。医薬的に許容される塩とは、その対イオンが該化合物の生理活性または毒性に著しく寄与することがなく、故に薬理学的同等物として機能するものである。これらの塩は、市販の試薬を用い、一般的な有機化学的技法により製造することができる。陰イオン塩の形態は、例えば、酢酸塩、アシストレート(acistrate)、ベシレート(besylate)、臭化物、塩化物、クエン酸塩、フマル酸塩、グルクロン酸塩、臭化水素酸塩、塩酸塩、ヨウ化水素酸塩、ヨウ化物、乳酸塩、マレイン酸塩、メシル酸塩、硝酸塩、パモ酸塩、リン酸塩、コハク酸塩、硫酸塩、酒石酸塩、トシル化物およびキシナホ酸塩である。陽イオン塩の形態は、例えば、アンモニウム塩、アルミニウム塩、ベンザチン塩、ビスマス塩、カルシウム塩、コリン塩、ジエチルアミン塩、ジエタノールアミン塩、リチウム塩、マグネシウム塩、メグルミン塩、4−フェニルシクロヘキシルアミン塩、ピペラジン塩、カリウム塩、ナトリウム塩、トロメタミン塩、および亜鉛塩である。

遊離塩基固形物を炭酸バッファー、およびエタノールアミンバッファーと混合した。第3のサンプルのpHはHClで調整した。25℃に設定したインキュベーター内に置いたミキサーで固形物を媒体と混合した。平衡化後、サンプルの上清を採取し、適当に希釈し、HPLCで分析した。
pH溶解性プロフィールの測定中は、結晶の化合物Iをゲル相に変換した。水と平衡状態にある結晶相が存在しないと、熱力学的に安定な溶液として化合物Iを製剤化することが可能なpH範囲の正確な評価が妨げられる。水と平衡化することが可能な化合物Iの結晶性遊離塩基の形態を製造する試みは継続中である。ゲル相で収集したデータを表1に列挙する。
温度の影響を表5に要約する。温度上昇に伴う分解速度の上昇は、ほぼ加水分解の亢進によるものである。最も高い温度において分解は最終的に分解産物Cに到達したが、最も低い温度では分解は分解産物Aで停止した(図2)。
安定性に対する光の影響を表6に要約する。光曝露されたサンプルにおいて7個の分解産物が記録されたが、その内の3個は他のいずれの条件においても観察されなかった。加水分解(分解産物A)は光曝露により促進された。
分解速度に対するpHの影響を表7に要約する。分解速度の加速はpHと逆相関し、主に加水分解の亢進によるものであった。pH4において分解は加水分解産物Cにまで進んだが、pH5および6では、加水分解産物Aで停止した。
薬物含有量を増大させることにより加水分解速度が減速し、加水分解が分解産物Aの形成に制限された(表9)。分解産物Gは、高濃度のサンプルにおいてのみ検出されたものであるが、本研究中の濃度においては著しく増量することはなく、単なる不純物と考えられる。分解産物Jは、最後のタイムポイントにおいてのみ検出することができた。
結合アッセイ
SK−N−MC細胞膜のホモジネートを受容体の供給源として用いた。クローニングされたヒトCGRP受容体と同一の配列の内因性のCGRP受容体を発現している(Aiyar et al., 2001)ため、ヒト神経芽腫SK−N−MC細胞をインビトロアッセイに用いた。細胞を37℃、5%CO2において、アール塩およびL−グルタミンを含有する10%ウシ胎児血清添加MEM中でコンフルーエンスに達するまで増殖させた。細胞をリン酸緩衝生理食塩水で洗浄することにより回収し、10mM Tris(pH7.4)および5mM EDTAからなる低浸透圧性溶解バッファー中、4℃において5−10分間インキュベートした。細胞を回収し、ポリプロピレンチューブに移し、ポリトロン(登録商標)を用いてホモジナイズした。ホモジネートを32,000xgで30分間遠心した。0.1%哺乳類プロテアーゼ阻害剤カクテルを含有する冷却した低浸透圧性溶解バッファーでペレットを再懸濁し、タンパク濃度を定量した。次いで、膜ホモジネートを分注し、アッセイ当日まで−80℃で保存した。
化合物Iは、SK−N−MC細胞膜に発現される内因性CGRP受容体への[125I]CGRPの結合の濃度依存的な阻害を示した。平均Kiは22.7±1.6pMであった。
方法
飽和結合実験を用い、内因性CGRPペプチドと化合物Iとの相互関係の性質を詳細に解析した。簡単に述べると、2種類の濃度(30pMおよび100pM)のいずれか一方の濃度の化合物Iの非存在下(コントロール条件)または存在下(試験条件)において、[125I]CGRP濃度を漸増しながら[125I]CGRPのSK−N−MC細胞膜調製物への結合を測定した。Kellソフトウェア(Biosoft、ケンブリッジ、UK)を用いて飽和データを双曲線方程式で解析し、解離定数(Kd)および結合部位の最大数(Bmax)を推定した。[125I]CGRPの結合パラメーター(Kd、Bmax)に対する化合物I添加の影響を測定し、比較した。
化合物Iは濃度依存的に[125I]CGRPの結合の解離定数Kdを増大させた(親和性は低下)が、[125I]CGRPの結合部位の最大数(Bmax)は有意に変化しなかった。これにより、[125I]CGRPのヒト受容体への結合の化合物Iによる阻害が競合的なメカニズムであることが示唆された(表10)。
方法
CGRP受容体複合体はGタンパク質のGsクラスと共役する。CGRPのこの複合体への結合により、Gs依存的なアデニル酸シクラーゼの活性化を介したサイクリックAMP(アデノシン 3’5’−環状一リン酸)の産生が誘起される。
化合物Iは、接着SK−N−MC細胞全体におけるCGRP刺激性サイクリックAMP産生の濃度依存的な阻害を示し、IC50は38.6±4.2pM、Ymaxは95.4(±1.3)%であった。観察された最大阻害は約100%であり、CGRP受容体の完全なアンタゴニズムが示唆された。
方法
シルド解析を化合物Iのアンタゴニズムの性質の特徴付けに用いた。CGRP刺激性cAMP産生の用量応答をCGRP単独、あるいは異なる濃度の化合物Iの存在下におけるCGRPについて得た。特に、CGRPによる用量依存的cAMP産生を5種類の濃度の化合物Iの存在下または非存在下においてアッセイした。化合物Iの濃度をY軸上の「用量比−1」に対してX軸上にプロットした(用量比は化合物Iの存在下におけるCGRPのEC50をCGRP単独のEC50で割ったものである)。次いで、対数変換したXおよびY軸について線形回帰を行った。傾きは1から大きく外れなかったため、競合的アンタゴニズムであることが示唆される。Kbはアンタゴニストの解離定数である。
化合物Iのシルド解析により、傾きの平均が1.02±0.04であり、アンタゴニストの解離定数Kbの平均が21.5±9.4pMであることが分かった。漸増濃度の化合物Iの存在下においてCGRPの濃度応答が右方向に平行シフトしたことにより、CGRP刺激性cAMP産生の化合物Iによる競合的アンタゴニズムが示唆される。シルドプロットの傾きが1.02であることにより、化合物IとCGRP機能の競合的な相互作用がさらにサポートされる。21.5pMのKbは結合のKi(22.7pM)と一致している。
臨床状態(片頭痛に関連するCGRP放出が治療の開始に先行する状態)を模倣するエクスビボにおける手段を獲得するため、まず、血管をCGRPにより拡張し、次いで、化合物Iにより拡張を回復させた。この回復プロトコルでは、アンタゴニストの後処理によりCGRP誘発性動脈拡張が回復される(アゴニストの単回投与およびアンタゴニストの複数回投与を用いる)。簡単に述べると、ワイヤーを取り付けた動脈輪をカリウムイオン(内因性の張力を模倣するため)で収縮させ、CGRPで完全に拡張し、漸増濃度のCGRPアンタゴニスト化合物Iで拡張を回復させる。化合物Iによる後処理は、エクスビボにおけるヒト頭蓋内動脈の完全なCGRP誘発性拡張の回復に有効であった。
ヒト動脈の剖検サンプルは組織調達業者から得た。全ての血管は氷冷HEPESバッファー(組成(mM):NaCl 130、KCl 4、KH2PO4 1.2、MgSO4 1.2、CaCl2 1.8、グルコース 6、NaHCO3 4、HEPES 10、EDTA 0.025)中で輸送された。受領時に、血管をカルボゲン(5% CO2および95% 酸素)で飽和した冷クレブスバッファー(組成(mM):NaCl 118.4、KCl 4.7、KH2PO4 1.2、MgSO4 1.2、CaCl2 1.8、グルコース 10.1、NaHCO3 25)中に置いた。
血管から結合組織を洗浄し、4−5mmの長さの円筒状のセグメントに切断する。次いで、該血管を2つのステンレス製のフック間の組織浴内に設置し;一方を固定し、他方をフォースディスプレイスメントトランデューサー(force displacement transducer)に連結する。血管の張力はデータ収集システム(Powerlab, AD Instruments, Mountain View, CA)を用いて連続的に記録される。クレブスバッファーおよび設置した血管を含む組織浴は温度37℃、pH7.4にコントロールされ、カルボゲンが連続的にバブルされる。一定の安静時張力(0.25から0.5g)となるまで動脈のセグメントを約30−45分間平衡化する。アッセイ前に血管を100mM KClで予備刺激(条件付け)し、次いで洗浄した。
化合物Iは、エクスビボにおけるヒト頭蓋内動脈のCGRP誘発性拡張に対し強力かつ完全な回復(EC50=880±50pM)を示した。
あるCGRP濃度範囲に対する機能的アンタゴニズムを評価するため、組織浴内の個々の動脈輪を化合物Iでプレインキュベートし、次いで、血管の完全な拡張が達成できるCGRPの濃度応答曲線を作成した(アゴニストの複数回投与およびアンタゴニストの複数回投与を用いる)。アンタゴニストのより高い濃度においてCGRP濃度応答曲線の「右側シフト」が見られ、アンタゴニストの存在を上回って血管の完全な拡張の達成により高濃度のアゴニストが必要となった。簡単に述べると、ワイヤーを取り付けた動脈輪をアンタゴニストでプレインキュベートし、次いで、KCl(内因性の張力を模倣するため)で収縮させ、漸増濃度のCGRPを加えて完全に拡張させる。化合物Iによる前処理はエクスビボにおけるヒト頭蓋内動脈のCGRP誘発性拡張の阻害に有効であり、CGRP濃度応答曲線に平行な右側シフトを示した。
ワイヤーを取り付けた各動脈輪を単一濃度(0.1−30nM)のアンタゴニスト化合物Iで30分間プレインキュベートし、次いで、10mM KCl(内因性の張力を模倣するため)で収縮させ、漸増濃度のCGRPを加えて血管の完全な拡張を達成させる。KCl収縮により、CGRP添加前のアンタゴニストのプレインキュベート時間が約45分となるよう安定化することができる。
化合物Iは、エクスビボにおけるヒト頭蓋内動脈のCGRP濃度応答曲線の平行な右側シフトを引き起こした。シルド解析によると、Kbは91pMであった。これらの結果はインビトロにおける結合(Ki=22.7pM)および機能(Kb=21.5pM)アッセイとよく一致する。
新規CGRP受容体アンタゴニストのインビボでの効果を評価するため、マーモセットにhαCGRPを一連の4回の静脈内注射(45分間隔)で投与した。1回目のものはベースラインコントロールとしてのものであり、続いて試験化合物の皮下送達を行った。次の3回のCGRP投与により、インビボでのCGRPの機能的アンタゴニズムの評価が可能となる。本研究では、化合物Iの強力かつ持続性のCGRPアンタゴニズムが示された。
マーモセットを麻酔し、45分間隔(−30、15、60および105分)のhαCGRPの静脈内注射(IV)により顔面血流を増加させた。0分の時点において送達された試験化合物のhαCGRP誘発性顔面血流変化に対する効果は、レーザードップラー血流計で測定した。有効な化合物であれば、15、60および105分で見られるhαCGRP誘発性の顔面血流の上昇を抑制するであろう(−30分で見られるhαCGRPのベースラインに対する効果に比べて)。
体重350−650gの成熟オスおよびメスのコモンマーモセット(Callithrix jacchus)を対象として用いた。
導入チャンバー(4−5%の急速導入、1−2.5%で維持;Solomon et al., 1999)内でイソフルラン吸入により動物を麻酔した。麻酔は挿管および換気(血中ガスモニタリングを行いながら)により空気:酸素(50:50)およびイソフルランを送達して維持した。体温は、直腸計を用い、自動温度制御板上に置くことにより38±0.5℃に維持した。脱毛クリームまたは剃毛により、顔の片側または両側の毛皮の小さな領域(約1.5平方cm)を除去した。手術領域をクリップし、ベタジンで処理した。試験化合物およびCGRP受容体アゴニストhαCGRPを投与するため、IV管を伏在静脈に挿管した。さらに、このIV管により、血中ガスモニタリングおよび化合物の血中濃度分析用の血液サンプル(最大2.5mL、10%)を採取することができる。血糖値を維持するため、5%デキストロース溶液がIV投与される。麻酔の深さは、非侵襲的アームカフ法およびパルスオキシメータでそれぞれ血圧および心拍数を測定することによりモニターした。顔面血流のピークフラックスを安定させるため、グアネチジン5−10mg/kg IV(必要に応じて5mg/kg IVを補充)を投与した(投与しない場合、繰り返し刺激することにより次第に減少する(Escott et al., 1995))。微小血管血流は、自己接着性レーザードップラー血流プローブを顔面の皮膚に接着させることによりモニターした。該プローブは、2つのレーザービーム間を横切った赤血球の数をそれらの速度とかけあわわせたものを記録する(フラックスの変化として報告される)。
試験化合物は頸筋のSC(0.1−0.6 ml/kg)により投与された。CGRP受容体アゴニスト、hαCGRPは10μg/kgの投与量でIV(1ml/kg)により投与された。
インビボでの有効性および作用時間を調べるため、薬剤送達の30分前(−0.5時間)にhαCGRP(10μg/kg IV)を投与することにより、顔面血流のコントロールとしての上昇を誘起した。次いで、化合物Iを0時間の時点(0分)で投与し、hαCGRPを45分間隔で〜2時間繰り返した(データは投与後0.25、1および1.75時間で収集した)。化合物Iは0.003、0.01および0.03mg/kgの投与量においてSCで投与した。血漿サンプルは各hαCGRP投与の直前に採取した。試験後、動物は、完全に覚醒し歩行可能な状態になるまで動物を保温し続ける温度制御板上に置かれた輸送ケージに戻した。14−21日間の休養およびウォッシュアウト期間後、動物を再度試験に用いることができる。
化合物I(0.003−0.03mg/kg、SC)は、マーモセットの顔面血流のCGRP誘発性増加に対し用量依存的な阻害を示した。強力な(53−80%)阻害は0.03mg/kgにおいて投与後0.25、1および1.75時間で見られた。著しい(35−40%)阻害は、0.01mg/kgにおいて投与後全ての時点で見られた。0.003mg/kgでは、穏やか(20%)だが有意な阻害が0.25時間で見られたが、それ以降の試験時間では見られなかった。
化合物Iおよび化合物III:ラットにおける鼻腔内刺激の1週間比較実験
本研究は、オスのラットに1週間鼻腔内投与した場合の化合物Iと化合物IIIの鼻腔内刺激の可能性を比較するために行われた。オスのラット(10/群)に、化合物Iまたは化合物III溶液(25、75または175mg/L/225mMコハク酸、0.02%塩化ベンザルコニウム、1.25%無水デキストロース、pH5.8−6.2)を1日1回、100μL/鼻孔の投与量で鼻腔内注入した。この投与パラダイムを用い、5、15または35mgの固定用量の試験化合物を毎日投与した。結果として、ラットが成長するにつれ、体重で補正した投与量は減少した。コントロール群の1群にはコハク酸ベヒクルを投与し、シャムコントロール群には生理食塩水を鼻腔内注入により投与した。評価されたパラメーターは、臨床観察、体重、摂餌量、毒物動態学、および鼻の組織の組織学的評価を含む。
化合物Iおよび化合物IIについて、鼻腔内刺激を直接比較した。オスのラット(6/群)に化合物Iおよび化合物IIの(225mM コハク酸、1.25% 無水デキストロース、pH 5.8−6.2)溶液(75または175mg/L)を1日1回、12.5、25または100mL/鼻孔の投与体積において鼻腔内投与した。この研究では、鼻甲介の組織学的評価が評価した唯一のエンドポイントであった。
CGRPアンタゴニストの鼻腔内(IN)投与経路は、迅速な作用発現の可能性を有する非侵襲性送達が可能となるため、魅力的である。透過性の高い鼻腔上皮バリアー、良好な灌流状態にある粘膜組織、および限定された代謝能/組織滞留時間は、経口投与による吸収が非常に低い化合物Iのような化合物の鼻腔内送達をサポートする非常に有用な特性である。
3−3.5kgの範囲のオス ニュージーランドホワイト3羽の群に、薬剤を以下の処方の1つにおいて単回投与した:0.5 mg/kg、30秒間のIVボーラス注入、または0.3−3mg/kg、シリンジマイクロスプレーによるIN投与。IN投与する前に、セボフルランの吸入麻酔により軽く鎮静させた。ウサギは2−5分以内に意識を回復した。一連の血液サンプルをヘパリン含有バキュテナーに、投与前2、5、10、15、30分および投与後1、2、4、6、および24時間後に採取した。血液サンプルは直ちに4℃で遠心し、分離した血漿をさらなるLC/MS/MS分析まで−80℃で保存した。
薬物動態学的プロフィールにより、溶液として噴霧されると化合物Iはウサギの鼻孔から迅速に吸収されることが示唆された。ピーク濃度に到達するまでの時間(Tmax)は、実験した全ての投与量において0.2−0.3時間(15−20分)であった。0.3、1、および3mg/kgにおける絶対的生物学的利用能は13から30%の範囲にあり、Cmaxは0.12から2.0μMの範囲であった(表14)。
本発明の別の一態様は、化合物Iを医薬的に許容される佐剤、担体、または希釈剤と共に含む医薬組成物である。
Claims (7)
- 治療上の有効量の(R)−N−(3−(7−メチル−1H−インダゾール−5−イル)−1−(4−(1−メチルピペリジン−4−イル)ピペラジン−1−イル)−1−オキソプロパン−2−イル)−4−(2−オキソ−1,2−ジヒドロキノリン−3−イル)ピペリジン−1−カルボキシアミドを医薬的に許容される佐剤、担体、または希釈剤と共に含む医薬組成物。
- 治療上の有効量の請求項1の化合物またはその医薬的に許容される塩を患者に投与することを特徴とする、CGRPまたはCGRP受容体シグナリングレベルの異常が関連する病態の治療方法。
- 該病態が片頭痛である、請求項3の方法。
- 該病態が神経障害性疼痛である、請求項3の方法。
- 治療上の有効量の請求項1の化合物またはその医薬的に許容される塩を患者に投与することを特徴とする、増殖性疾患の治療方法。
- 該増殖性疾患が乳癌または神経膠腫である、請求項6の方法。
