Abstract
Description
しかし、冠動脈バイパス手術は、合併症の大きなリスクを伴う。冠動脈バイパス手術後の急性腎損傷(AKI:acute kidney injury)の発症率は、10〜20%に及ぶ(Mehta et al., Circulation 2006, 114: 2208-2216)。これらの個体の1〜5%は、手術後に透析を必要とする。手術後のAKIの病因は多因子性であると思われており、心臓外科手術後の罹患率の増加および長期死亡率との関連性は十分に確立されている(Brown et al., Annals of Thoracic Surgery 2008, 86: 4-11; Kourliouros et al., European journal of Cardiothoracic Surgery 2009, 近刊)。
a)該患者の試料中のGDF−15および/またはトロポニンの量を測定する工程、
b)該測定された量を好適な基準量と比較し、それにより、該患者が急性腎損傷に罹患するリスクを予測する工程、
を含む方法に関する。
第2に、リガンドが目的のペプチドまたはポリペプチドの酵素活性の基質としても用いられている場合、酵素反応産物を測定してもよい(例えば、タンパク質分解酵素の量は、例えばウェスタンブロットにより、切断された基質の量を測定することによって測定することができる)。あるいは、リガンド自体が酵素的性質を呈していてもよく、「リガンドとペプチドまたはポリペプチド」との複合体またはペプチドまたはポリペプチドによってそれぞれ結合されたリガンドを、好適な基質に接触させ、それにより、強度信号の発生によって検出を可能にしてもよい。酵素反応産物の測定のために、基質の量が飽和されていることが好ましい。また、反応前に、基質を検出可能な標識で標識してもよい。好ましくは、適当な期間で試料を基質に接触させる。適当な期間とは、産物が検出可能(好ましくは測定可能)な量産生されるのに必要な時間を指す。産物の量を測定する代わりに、所与の(例えば検出可能な)量の産物の出現に必要な時間を測定することができる。
上述したように、基準量は、後にAKIに罹患したことが分かる患者の手術を受ける前に採取された試料から得られたものであってもよい。バイオマーカGDF−15および/またはトロポニンの前記基準量と本質的に同じかそれより多い量の場合に手術後にAKIのリスクが高まることを示すのが好ましい。また、上述したように、基準量は、後にAKIに罹患し血液透析が必要であることが分かる患者の手術を受ける前に採取された試料から得られたものであってもよい。バイオマーカGDF−15および/またはトロポニンの前記基準量と本質的に同じか多い量の場合に手術後に血液透析を必要とするAKIのリスクが高まることを示すのが好ましい。
「ナトリウム利尿ペプチド」という用語は、心房性ナトリウム利尿ペプチド(ANP:Atrial Natriuretic Peptide)型および脳性ナトリウム利尿ペプチド(BNP:Brain Natriuretic Peptide)型ペプチドおよび同じ予測可能性を有するそれらの変異体を含む。本発明に係るナトリウム利尿ペプチドは、ANP型およびBNP型ペプチドおよびそれらの変異体を含む(例えば、Bonow, R.O.(1996). New insights into the cardiac natriuretic peptides(心臓ナトリウム利尿ペプチドへの新しい洞察). Clin Chem 2004; 50: 867-73)。従って、時間経過または関連の特性に応じて、ナトリウム利尿ペプチドの活性または不活性形態のいずれか一方を測定するのが有利かも知れない。本発明に係る最も好ましいナトリウム利尿ペプチドは、NT−プロBNPまたはその変異体である。上に簡単に述べたように、本発明に従って参照されているヒトのNT−プロBNPは、好ましくは、ヒトのNT−プロBNP分子のN末端部分に対応する76個のアミノ酸長を含むポリペプチドである。ヒトのBNPおよびNT−プロBNPの構造は、既に先行技術(例えば、国際公開第02/089657号、第02/083913号、Bonow 1996, New Insights into the cardiac natriuretic peptides(心臓ナトリウム利尿ペプチドへの新しい洞察). b)測定された量を基準量と比較して、それにより、患者が急性腎損傷に罹患するリスクを予測する工程、
を含む。
さらに、本発明は、リスク患者が外科的処置に適しているか否かを決定するための方法に関し、本方法は、
a)患者の試料中のGDF−15および/またはトロポニンの量を測定する工程、
b)測定された量を好適な基準量と比較して、それにより、患者が外科的処置に適しているか否かを決定する工程、
を含む。
さらに、本発明は、急性腎損傷に罹患するリスクを予測するための、外科的処置を受ける予定の患者の試料中のバイオマーカGDF−15および/またはトロポニンの使用に関する。
さらに、本発明は、外科的処置を受けたリスク患者が急性腎損傷に罹患するリスクを予測するための、前記患者の試料中のGDF−15に特異的に結合する検出物質の使用に関する。
a)患者の試料中のGDF−15および/またはトロポニンの量を測定するための分析部、
b)測定された量を好適な基準量と比較するため、および患者が急性腎損傷に罹患するリスクを予測するための評価部、
を含む。
前記装置は、好ましくは、適用される試料中のGDF−15および/またはトロポニンの量の測定のための分析部、結果データを処理するための評価部を含む。好ましくは、評価部は、格納された基準量を有するデータベースと、コンピュータに明らかに組み込まれている場合に、測定された量とデータベース内に格納されている基準量との比較を行うコンピュータプログラムコードとを含む。より好ましくは、評価部は、比較の結果をリスク予測に対して割り当てるさらなるコンピュータプログラムコードを含む。このような場合、好ましくは、評価部は、基準量がリスクに対して割り当てられているさらなるデータベースを含むことも想定される。
a)患者の試料中のGDF−15および/またはトロポニンの量を測定するための分析物質、
b)分析物質で測定された量を好適な基準量と比較し、患者が急性腎損傷に罹患するリスクをさらに予測することができる評価部、
を含む。
a)患者の試料中のトロポニンTの量を測定する工程、
b)測定されたトロポニンTの量を好適な基準量と比較する工程、
c)工程b)の結果に基づいて前記患者がMIに罹患している否かを診断する工程、
を含む。
本明細書に引用されている全ての参考文献は、それらの開示内容全体および本明細書で具体的に述べられている開示内容に関して、参照により本明細書に組み込まれる。
実施例1
患者、材料および方法
冠状動脈バイパス移植(CABG)手術を受ける予定の全部で126人の継続患者が調査に参加した。68人の男性と58人の女性(年齢の中央値:68歳(52〜81歳)がいた。血清クレアチニンレベルは、全ての患者で正常であった。全ての患者は、内腔の50%を超える少なくとも1つの狭窄によって示されている2つ以上の血管疾患に罹患していた。急性腎損傷の死亡率および発症に関して患者を30日間追跡調査した。調査の終点は以下のとおりであった:急性腎損傷(AKI、手術後3日以内の少なくとも0.3mg/dlのクレアチニンの増加)、血液透析の新しい必要性または死亡(手術後30日以内)。手術前および手術直後ならびに手術から1日、2日および3日後に血液を採取した。試料を30分間遠心分離し、分析まで摂氏−20℃で保存した。
実施例2
個々の患者症例
症例1:
複数の狭窄を有する三枝病変に罹患している62歳の男性にCABGを行った。この患者は、長期の動脈性高血圧症に罹患しており、真正糖尿病または喫煙履歴はなかった。この患者の脂質は正常範囲内であった。但し、この患者には肥満症の病歴があった。手術の前後に、以下の結果が得られた:
手術前のGDF−15およびNT−プロBNP量は、手術後にAKIのリスクが高まることを示す基準量には満たなかった(それぞれ1717pg/mlおよび1119pg/ml)。このトロポニンT量は、AKIのリスクの増加から除外され得ることをさらに示していた(13.3pg/ml未満のTnT)。
症例2:
複数の狭窄を有する二枝病変に罹患している58歳の男性にCABGを行った。この患者は、過去に喫煙者であり、動脈性高血圧症に罹患していたが、真正糖尿病には罹患していなかった。この患者の脂質は正常範囲内であった。手術の前後に、以下の結果が得られた:
手術前に測定したGDF−15量は、腎代替療法を必要とするAKIのリスクが高いことを示していた。トロポニンT量は、AKIのリスクが高いことを示す閾値(25.5pg/ml)を僅かに下回っていた。NT−プロBNP量は、AKIのリスクが高いことを示していた。総合すると、3つのマーカーのうち2つは、AKIのリスクが高いことを示していた。1つのマーカーによって血液透析の必要性も予測されていた。従って、AKIの発症は予想に合致している。
症例3:
広範囲にわたる三枝病変に罹患している62歳の男性にCABGを行った。この患者は、手術前に呼吸困難に陥り、また、過去に症候性心不全を発症し、それをACE阻害薬、β遮断薬および利尿薬で治療したことが報告されていた。この患者の腎機能は限界であり、ループ利尿薬を用いて改善した。この患者のリスク因子には、喫煙、動脈性高血圧症および肥満症が含まれていたが、最近になって減量した。手術の前後に、以下の値が得られた:
トロポニンTおよびNT−プロBNPは、血液透析を必要とするAKIのリスクが高いことを示していた。手術前のGDF−15量は、血液透析を必要とするAKIのリスク(2573pg/ml)を示す基準値を僅かに下回っていた。従って、3つのマーカー全てが、AKIの発症を正確に予測し、2つのマーカーは、血液透析を必要とするAKIの深刻な症例も予測していた。
Claims (15)
- 外科的処置を受ける予定のリスク患者が急性腎損傷に罹患するリスクを予測するための方法であって、
a)該患者の試料中のGDF−15および/またはトロポニンの量を測定する工程、
b)該測定された量を基準量と比較して、それにより、該患者が急性腎損傷に罹患するリスクを予測する工程、
を含む前記方法。 - リスク患者が外科的処置に適しているか否かを決定するための方法であって、
a)該患者の試料中のGDF−15および/またはトロポニンの量を測定する工程、
b)該測定された量を基準量と比較して、それにより、該患者が外科的処置に適しているか否かを決定する工程、
を含む前記方法。 - ナトリウム利尿ペプチドの量をさらに測定して基準量と比較する、請求項1または2に記載の方法。
- 該ナトリウム利尿ペプチドはNT−プロBNPである、請求項3に記載の方法。
- GDF−15および/またはトロポニンの代わりに、肝臓型脂肪酸結合タンパク質(L−FABP)または腎損傷分子1(KIM−1)を測定する、請求項1に記載の方法。
- 外科的処置を受けたリスク患者が急性腎損傷に罹患するリスクを予測するための方法であって、
a)該患者の試料中のGDF−15の量を測定する工程、
c)該測定された量を基準量と比較して、それにより、該患者が急性腎損傷に罹患するリスクを予測する工程、
を含む前記方法。 - ナトリウム利尿ペプチドおよび/またはトロポニンの量をさらに測定して基準量と比較する、請求項6に記載の方法。
- 該ナトリウム利尿ペプチドがNT−プロBNPである、請求項7に記載の方法。
- 該急性腎損傷が血液透析を必要とする、請求項1〜8のいずれかに記載の方法。
- 該外科的処置が冠状動脈バイパス移植である、請求項1〜9のいずれかに記載の方法。
- バイオマーカGDF−15、バイオマーカトロポニン、外科的処置を受ける予定の患者の試料中のGDF−15に特異的に結合する検出物質および/またはトロポニンに特異的に結合する検出物質の、急性腎損傷に罹患するリスクを予測するための使用。
- バイオマーカGDF−15、バイオマーカトロポニン、リスク患者の試料中のGDF−15に特異的に結合する検出物質および/またはトロポニンに特異的に結合する検出物質の、該患者が外科的処置に適しているか否かを決定するための使用。
- バイオマーカGDF−15および/または外科的処置を受けたリスク患者の試料中のGDF−15に特異的に結合する検出物質の、該患者が急性腎損傷に罹患するリスクを予測するための使用。
- 外科的処置を受ける予定のリスク患者が急性腎損傷に罹患するリスクを予測するための装置であって、
a)該患者の試料中のGDF−15および/またはトロポニンの量を測定するための分析部、
b)該測定された量を好適な基準量と比較するための、および該患者が急性腎損傷に罹患するリスクを予測するための評価部、
を含む前記装置。 - 外科的処置を受ける予定のリスク患者が急性腎損傷に罹患するリスクを予測するためのキットであって、
a)該患者の試料中のGDF−15および/またはトロポニンの量を測定するための分析物質、
b)該分析物質によって測定された量を好適な基準量と比較し、該患者が急性腎損傷に罹患するリスクをさらに予測することができる評価部、
を含む前記キット。
