Description
TNF−αは、感染症、および自己免疫性疾患の際に観察されるもののような炎症プロセスにおいて中心となるメディエーターである。それは、損傷の過程において、白血球細胞、マスト細胞、内皮およびいくつかの他の組織によって放出される。IgE/抗原複合体で刺激されたマウスのマスト細胞を使用する細胞ベースのアッセイは、λ−カラゲナンではなくι−カラゲナンおよびκ−カラゲナンの両方がTNF−α放出を阻害するということを実証した(図1)。
1=刺激されていないもの;2=IgE/抗原刺激;3=λ−カラゲナン前処置+後のIgE/抗原刺激;4=κ−カラゲナン前処置+後のIgE/抗原刺激、5=ι−カラゲナン前処置+後のIgE/抗原刺激。Y軸はTNF−αの濃度(pg/ml単位)を映す。
マスト細胞を、様々な濃度でι−カラゲナンとインキュベーションした。60分後、この細胞を、IgE/抗原複合体で刺激した。細胞を37℃で6時間インキュベーションし、この上清中のTNF−αを市販のマウスTNF−α ELISA(ベンダー・メド・システムズ(Bender−Med−Systems))によって測定した。結果を図2に示す。エラーバーは4つの独立のウェルの間の標準偏差を示す。1=刺激されていないもの;2=IgE/抗原で刺激されたもの、3=ι−カラゲナン 200μg/ml、4=ι−カラゲナン 66μg/ml、5=ι−カラゲナン 6.6μg/ml、6=ι−カラゲナン 0.6μg/ml。Y軸はTNF−αの濃度(pg/ml単位)を表す。
アレルギー性鼻炎およびいくつかの種類の植物の花粉に対するアレルギーの強い症状を伴うI型アレルギーの判明している病歴をもつ29歳の患者に以下の治療計画を受けさせた:0.12% ι−カラゲナンを含有する点鼻薬を毎日晩に投与し、花粉の季節の間はこの投薬量を増加させた。この患者は、この点鼻薬を使用するときくしゃみの頻度が大きく減少すると報告し、妨げられない睡眠が再び可能となった。加えて、この患者は鼻内粘膜の炎症の減少を報告した。この患者はさらに、それまでの花粉の季節とは極めて対照的に付加的な薬物、とりわけ鼻腔内鬱血除去薬、抗ヒスタミン薬およびコルチコステロイド剤は、もはや必要ではないと報告した。
風邪感染に対する予防をもたらすことを意図して、26歳の志願者に0.12% ι−カラゲナンを含有する点鼻薬を使用することを依頼した。投薬手順を遵守して、この志願者は数ヶ月間、毎日少なくとも2回スプレーした。この志願者は、重篤な風邪感染の病歴およびいくつかの種類の植物の花粉に値するアレルギー反応の病歴を有していた。驚くべきことに、かなり厳しい花粉の季節にもかかわらず、自分は予想とは逆にアレルギー性鼻炎(季節性鼻アレルギー)の症状をまったく患っていない、とこの志願者は報告した。しかしながら、この志願者は、点鼻薬を使用していない身体の他の部分ではアレルギー反応の症状を報告した。これらの症状には、眼のかゆみおよび皮膚の発赤が含まれていた。従ってこれらの観察から、例えば点鼻薬として本発明に係るι−カラゲナン組成物の繰り返された局所的な投与、すなわち粘膜投与はアレルギー性鼻炎(季節性鼻アレルギー)に罹るリスクにある個体に予防的および/または治療的保護をもたらす可能性があるということを推測することができる。植物の花粉は主に春の季節の間放出されるため、従って罹患した個体は、本発明の点鼻薬を予防的に投与することを適時に開始することにより、季節性鼻アレルギーの症状を緩和することができる可能性があり、または恐らくは季節性鼻アレルギーの急激な発生を完全に防ぐことができる可能性がある。
生理食塩水未満(すなわち0.5%)のリン酸緩衝化された食塩水中の0.12重量%のι−カラゲナンを含む溶液に0.09重量%のエスチンを補った。この溶液の300μl(270μg エスチンを含有する)をニュージーランド白ウサギの1つの前鼻孔の中へと1日4回、5日間移した。最後の付与の後、30分、1時間、2時間、4時間、6時間、および12時間の時間点で、この動物から血漿を採取した。エスチンの血漿中濃度は6時間でピークを迎え、最高約90ng/mlの最大濃度に到達した。この濃度は、ヒトにおいて、経口投与後に得られる濃度と比べて著しく高い。50mg エスチンで1日2回繰り返して処置したヒト患者におけるエスチンの血漿レベルは20ng/ml未満(10ng/ml 平均レベル、16〜18ng/ml 最大濃度)であった。これらの結果は、カラゲナンはエスチンなどの所望の非カラゲナン活性物質との併用粘膜投与の際に佐剤機能をもたらす可能性があるということを示す。従ってカラゲナンは、経口投与の際にわずかしか生物が利用可能ではない生理活性化合物のバイオアベイラビリティーを改善するために、粘膜投与に適した医薬組成物の製造において使用される可能性がある。
Claims (17)
- アレルギー状態または疾患の予防的または治療的処置における医薬としての使用のためのカラゲナンであって、ただし前記カラゲナンはι−カラゲナンおよび/またはκ−カラゲナンを含み、かつλ−カラゲナンを実質的に含まない、医薬としての使用のためのカラゲナン。
- 前記疾患または状態はアレルギー性鼻炎、アレルギー性結膜炎、食物アレルギー、またはアレルギー性喘息である、請求項1に記載の医薬としての使用のためのカラゲナン。
- 粘液の損なわれた吸着能力の回復または補強における医薬としての使用のためのカラゲナンであって、ただし前記カラゲナンはι−カラゲナンおよび/またはκ−カラゲナンを含み、かつλ−カラゲナンを実質的に含まない、医薬としての使用のためのカラゲナン。
- 前記粘液は、気道、消化管、および眼のうちの少なくとも1つにある、請求項3に記載の医薬としての使用のためのカラゲナン。
- アレルギー状態または疾患の予防的または治療的処置のための医薬組成物の製造における使用のためのカラゲナンであって、ただし前記カラゲナンはι−カラゲナンおよび/またはκ−カラゲナンを含み、かつλ−カラゲナンを実質的に含まない、カラゲナン。
- 前記アレルギー状態または疾患はアレルギー性鼻炎、アレルギー性結膜炎、食物アレルギー、またはアレルギー性喘息である、請求項5に記載のカラゲナン。
- 粘液の損なわれた吸着能力の改善または回復のための医薬組成物の製造における使用のためのカラゲナンであって、ただし前記カラゲナンはι−カラゲナンおよび/またはκ−カラゲナンを含み、かつλ−カラゲナンを実質的に含まない、カラゲナン。
- 前記粘液は気道、消化管、および眼のうちの少なくとも1つにある、請求項7に記載のカラゲナン。
- レシピエントの粘膜表面に抗アレルギー性有効量のカラゲナンを含む医薬組成物を投与する工程を含む、アレルギー状態または疾患の予防的または治療的処置方法であって、ただし前記カラゲナンはι−カラゲナンおよび/またはκ−カラゲナンを含み、かつλ−カラゲナンを実質的に含まない、方法。
- 前記投与は、20cP(0.02パスカル秒)以下、好ましくは1〜10cP(0.001〜0.01パスカル秒)の予め選択された粘度の溶液として、または泡沫、ゲル、クリーム、ドロップ、薬用キャンディー、錠剤、カプセル剤、チューインガム、または乾燥粉末としての前記組成物を前記粘膜へと、特に気道、消化管および眼のうちの少なくとも1つの粘膜へと、所望のまたは所定の期間にわたって1日に少なくとも1回投与することによって行われる、請求項9に記載の方法。
- 抗アレルギー性有効量のカラゲナンを含む抗アレルギー性医薬組成物であって、ただし前記カラゲナンはι−カラゲナンおよび/またはκ−カラゲナンを含み、かつλ−カラゲナンを実質的に含まない、抗アレルギー性医薬組成物。
- 0.05〜5重量%、通常0.1〜2重量%の、ι−カラゲナンもしくはκ−カラゲナンまたはその両方の混合物を含む、請求項11に記載の組成物。
- 非カラゲナン生理活性成分をさらに含む、請求項12に記載の組成物。
- 前記成分は、天然植物またはミネラル源から誘導された薬草から製した薬剤またはホメオパシー薬剤の群から選択される、請求項13に記載の組成物。
- 好ましくはメントール、ミント、スペアミント、ペパーミント、ユーカリ、ラベンダー、柑橘類、レモン、ライム、またはこれらのいずれかの組み合わせからなる群から選択される香料をさらに含む、請求項12に記載の組成物。
- 二クロム酸カリウム、ガムまたはデンプンなどの増粘剤、デンプングリコール酸ナトリウムまたは架橋ポビドンなどの崩壊剤、ステアリン酸マグネシウムなどの離型剤、乳化剤、界面活性剤、薬学的に許容できる賦形剤もしくは担体、アンチケーキング剤、造粒剤、防腐剤、着色料からなる群から選択される少なくとも1つの添加剤をさらに含む、請求項12に記載の組成物。
- 点鼻薬として適合された、請求項1から請求項16のいずれか1項に記載の組成物。
