骨軟骨欠損の治療に適用するための2相プラグの物理特性評価

2相プラグの取扱い性
この調査では、緩衝液溶液中のプラグ材料水和の進行を評価し、またプラグ材料への溶出緩衝液長時間飽和状態の時間に対する影響を測定するという2つの目的のために、2相プラグ(Orthomimeticのコンドロミメティックプラグ)の取扱い性を評価した。メチレンブルー染料を使って可視化しプラグ材料の水和を実証した。
MQddH2Oを用いて酢酸ナトリウム緩衝液(20mM酢酸ナトリウム、pH5.99)を作った。200μLの17.4M酢酸(シグマ06911ME)でpHを6.0に調整し、次に2Lに調整した。次にこの酢酸ナトリウム緩衝液を0.22μmの滅菌フィルターで濾過した。
では、プラグ材料に物理的観点からの大きな収縮または変化は認められないことが明確になった。溶出緩衝液中に入れると横向きの状態からすぐに上部が上を向くというプラグ再配向から明らかように、プラグの先端相は、底部相に比べより大きな浮力を示した。プラグ材料は、全ての時点で優れた形状記憶を示した。実験全体を通して、いかなる微粒子状物質も溶出緩衝液の曇りも記録されなかった。
2相プラグ材料からのrhPDGF−BB(組換えヒト血小板由来増殖因子−BB)放出の評価

100μLの希釈した捕集試薬を96−ウエルプレート(コーニング3590)の各ウエルに添加した。接着性プレートカバーで覆い、希釈した捕集試薬を室温でオービタルシェーカーを使って一晩コートした。
Orthomimeticsの2相(RIVERSIDE(登録商標))プラグが組換えヒト血小板由来増殖因子BB(rhPDGF−BB)特性に与える影響
この調査では、サイズ排除および逆相HPLCならびにELISAで決定されたrhPDGF−BB受容体に対する結合親和性により、OrthomimeticsのRIVERSIDE(登録商標)プラグと組み合わせた後の、rhPDGF−BBの構造と機能の健全性を評価した。
2つのRIVERSIDE(登録商標)プラグを4分割した。プラグを100μLのrhPDGF−BB(例えば、1.0mg/mLを20mM酢酸ナトリウム緩衝液に溶解)に浸漬し4分割プラグのそれぞれに試料1〜6のラベルを付けるか、または同じ容積の酢酸
ナトリウム(NaAc)緩衝液に浸漬し(対照7、8)、室温で10分間インキュベートした。
自動注入装置を使って100μLの試料を自動サンプラーからサイズ排除カラムに注入し、0.05M酢酸ナトリウム中の0.4M NaCl、pH4.0と平衡したカラムで流速0.8mL/分、室温で溶出した。クロマトグラフィーの時間中、試料を4℃に保持した。
試験手順QCT004に従って、各90μLの試料を、最初、200mMジチオトレイトールと4Mグアニジン塩酸塩で50℃、5分間変性し、次にC18逆相カラムに注入し0.06%トリフルオロ酢酸中の24〜45%アセトニトリル勾配、1.2mL/分で溶出した。214nmの吸光度を使ってデータ収集した。
溶出緩衝液を使って4倍希釈の試料を10ng/mLに希釈し、2倍希釈系列により合計7希釈液を2組調整した。次に、このアッセイのためにR&D Labから入手した標準プロトコルに従い、DuoSetアッセイを使ってPDGF検出アッセイを行った。
SEC(サイズ排除クロマトグラフィー)から得られたrhPDGF−BBプロファイル
はその元の構造の変化および/またはプラグから溶出した可溶性成分の存在を反映していた。全塩濃度でプラグ材料から溶出した多少の低分子量物のバックグラウンドがあった。しかし、溶出緩衝液中に塩が存在するときのみ、材料からrhPDGF−BBが放出され、rhPDGF−BBがイオン相互作用によってプラグに付着していることを示した。低塩濃度(例えば、0.28M NaCl)では、溶出時間約8分で小さな高分子量ピークが現れ(ブランク溶出では認められない)、おそらく一部のrhPDGF−BBの凝集が起こっていることを示すと思われた。この現象は高塩濃度(例えば、0.56M NaCl)では認められない。

A.調査I
この調査には、3つの群のボーア交配種去勢雄ヤギが含まれる。第一群(群1)は、二相性生体適合性マトリクスプラグ(コンドロミメティック(Orthomimetics))だけを与えた6〜8匹のヤギで構成される。また、第2の群(群2)および第3群(群3)も、6〜8匹のヤギからなり、二相性生体適合性マトリクスプラグに含まれる2種類

この調査の目的は、rhPDGF−BBと組み合わせた二相性生体適合性マトリクスプラグ/インプラント(例えば、コンドロミメティック(Orthomimetics))を使って骨軟骨欠損修復強化の影響を測定することであった。

この調査は、用量範囲決定と有効性調査として計画され、5つの外科的群を含む。すなわ
ち、軟骨欠損に対し治療をしない対照群、20mM酢酸ナトリウム(緩衝液)で飽和したコンドロミメティックI型コラーゲンインプラントを含む対照群、0.030mg/mL
rhPDGF−BBの緩衝液中溶液の0.5ccで飽和したコンドロミメティックI型コラーゲンインプラントを含む実験群、0.15mg/mL rhPDGF−BBの緩衝液中溶液の0.5ccで飽和したコンドロミメティックI型コラーゲンインプラントを含む実験群、および1.0mg/mL rhPDGF−BBの緩衝液中溶液の0.5ccで飽和したコンドロミメティックI型コラーゲンインプラントを含む実験群である。これらの群の動物への割り付けを下の表10に示す。
1)動物麻酔と手術準備
1つの(1)右大腿内側顆状突起中に骨軟骨欠損を導入した。基本的な外科手術は全動物に対し同じとした。全手術は厳密な無菌状態下で行った。全動物で手術12時間前に食物と水を断った。
手術前および死体解剖前の健康状態選別用の血液収集に加えて、追加の1つの血液チューブを手術の日と安楽死の日にクロットチューブに集め、血清を集めて少なくとも血清の2mLを、調査番号、動物番号、および収集日付のラベルのついたクライオバイアル中にいれ、−70〜−80℃で保存した。
全動物に対して、手術時間中に関節液の色と粘度の目視評価を記録した。採取可能な十分な量がある場合は、関節液試料を集め、調査番号、動物番号、収集者のイニシャル、および収集日付を記したラベルを貼ったクライオバイアル中に入れ、−70〜−80℃で保存した。
この手術法は、遠位の右大腿の1/3から脛骨プラトーのレベルまでの、脛骨脊椎を横切った曲線状側部皮膚切開で構成された。この方法を使って、皮膚を明確に切開し、口を開いて後膝関節への外側膝蓋骨経由アプローチを可能とした。膝蓋骨と膝蓋靭帯の外側境界に平衡に切開を行った。これを大腿筋膜の外側から大腿二頭筋の頭蓋境界に沿って後膝関節の外側筋膜へと拡張した。大腿二頭筋および付随の外側筋膜を後退させ、切開を関節襄まで到達可能とした。関節を拡張し、膝蓋骨をはずして内部の後膝関節を曝した。
19mm遠位で滑車溝の内側陵に整列した位置と確定した。提供された手術器具を使って、直径8mmX深さ8mmの骨軟骨欠損を導入した。欠損部を大量の無菌食塩水で洗い流した。いずれの試験物質を置く前にも、関節の残存部分を注意深く洗い流し、関節を紙で吸い取り乾燥した。
2相基質インプラント/rhPDGF−BB:移植の前に、3種類の用量の内の1つのrhPDGF−BB(0.030mg/mL、0.15mg/mL、または1.0mg/mLrhPDGF−BBの緩衝液中溶液)または20mM酢酸ナトリウム緩衝液を、2相コラーゲンインプラントと組み合わせた。これは、ステンレス鋼ボウル中で前述の適切な試験物質の0.5ccを無菌のコラーゲンインプラントに添加することにより達成した。水和コラーゲンインプラントを室温で5〜15分間置き、次に、外科用鉗子で静かに欠損位置に移した。過剰のrhPDGF−BB溶液は全てシリンジで吸い出し欠損部位に絞り出した。
手術後の不快感の兆しのあるどの動物に対しても少なくとも毎日2回の手術後チェックを行った。動物に不快感による苦悩の兆しが少しでもあるようなら、追加の手術後鎮痛薬を与えた。全ての処置は適切な調査ドキュメンテーションに記録された。安楽死のときまで、毎日の臨床的観察を行い、各動物について記録を取った。ルーチン臨床観察の一環として全動物に対し術後チェックを行った。
動物は術後84±3日目(12週目)人道的に屠殺された。屠殺直前に体重を記録した。ジアゼパム0.22mg/kgとケタミン10mg/kgのIV注入により全身麻酔を誘導した。この後、心拍停止が確認されるまで、麻酔下の動物に濃塩化カリウム(KCl)IV過量投与を行った。
目視評価の直後、左および右膝窩リンパ節と一緒に、試料を10%リン酸塩で緩衝化した

microCT80システム(SCANCO USA、サウスイースタン、ペンシルベニア州)とメーカーの分析ソフトウェアを使ってマイクロCTを走査し、分析を行った。マイクロCT分析の評価項目には、肋軟骨下ゾーン全体の骨充填および中心窩洞の骨体積/全体積(BV/TV)の評価が含まれる。
目視剖検
屠殺まで動物を毎日2回観察した。全動物を術後2週間で屠殺し、マイクロCTと組織分析用試料を採取した。

rhPDGF−BB治療群または欠損未充填治療群に比較して、PDGF治療群で修復組織の隣接する元の軟骨との用量依存的統合;8)全治療群で、主に線維軟骨からなる修復組織;9)欠損未充填治療群を除く全治療群について、欠損の軟骨ゾーンの中央部でヒアリン様軟骨の存在。これは、II型コラーゲンのための免疫組織化学染色の陽性により示され、またサフラニンO染色により測定されたグリコサミノグリカン含量により示された。;および10)コラーゲンインプラント含有全治療群で、欠損空隙内の少量の残留コラーゲンインプラント。
ヤギモデルにおける骨軟骨欠損修復のための軟骨下骨再構成のマイクロCT評価
調査の目的は、骨軟骨欠損モデルにおけるヤギの大腿顆状突起の軟骨下骨修復角度を評価することであった。定量的因子(例えば、骨体積、骨梁幅、等)を測定し形成された硬骨の量と質を評価した。この調査で用いた治療群は実施例7のものと同じであった。
大腿内側顆状突起用マイクロCT走査手順
各大腿内側顆状突起を51.2mmの褐色樹脂試料ホルダーに入れ装填した。各顆状突起を気泡ゴムで固く包み試料ホルダー中で安定化させた。包んだ顆状突起を試料チューブに挿入し欠損側を上に向け、チューブの長軸と平行にした。顆状突起の安定性をチューブ内の回転と横方向の移動によりチェックした。装填と各顆状突起の安定性チェック後、10%中性緩衝化ホルマリンを添加し試料を完全に浸漬したが、チューブの先端に2〜3mmの空気層を残した。試料チューブをプラスチックチューブキャップでシールした。マイクロCT中で、シールした試料チューブの方向マーカーをユーザー側に向けた。
たは300スライス(7.5mm深さ)を描画した。再構築した欠損部位を、各分析に対し次の寸法の環状輪郭を描画して輪郭に合わせた:元の欠損の中心管を中心として、160ピクセルx160ピクセル(0.1266cm2、4mm径)、240ピクセルx24

全体積、骨体積、材料平均密度、連結密度、骨梁数、骨梁肉厚、および骨梁間隔の定量的測定値を、Scanco microCT80 machine(サウスイースタン、ペンシルベニア州)の分析プログラムを使って評価した。治療群と群当たりの動物の数は実施例7と同じで、表16に概要を示した。定量分析を多重分析基準を使って元の欠損の中心管上で行った。この基準には、直径8mmX深さ6.25mmのシリンダー、直径6mmX深さ6.25mmのシリンダー、直径4mmX深さ6.25mmのシリンダー、直径8mmX深さ7.5mmのシリンダー、および直径10mmX深さ6.25mmのシリンダーが含まれる。各分析基準では、全容積(輪郭に合わせたシリンダーの容積)を一定に保持した。連結密度または骨梁間隔に有意差は観察されなかった。全分析基準に対し、治療群間の骨体積に有意差は認められなかったが、500μg rhPDGF−BB治療群7試料のうち4試料に欠損空隙の全幅に架かる実質的な骨ブリッジングが認められた。このタイプのブリッジングは、残りの治療群では観察されなかった。厚さ8mmX深さ6.25mm輪郭に対し、500μg rhPDGF−BBで治療した試料の骨梁数(図10A)は0μg rhPDGF−BB治療群に比較して、有意に増加した。直径8mmX深さ7.5mm輪郭に対し、0μg rhPDGF−BB、15μg rhPDGF−BB、および欠損未充填治療群に比較して、500μg rhPDGF−BB治療群で骨梁数(図10C)は、有意に増加した。厚さ4mmX深さ6.25mm輪郭に対し、0μg rhPDGF−BB治療群に比較して、75μg rhPDGF−BBで治療した試料で、骨梁幅(図10D)が、有意に増加した。また、0μg rhPDGF−BB、75μg rhPDGF−BB、および500μg rhPDGF−BB(直径10mmX深さ6.25mm輪郭)および0μg rhPDGF−BB、15μg rhPDGF−BB、および75μg rhPDGF−BB治療群(直径8mmX深さ7.5mm輪郭)に比較して、欠損未充填治療群で骨梁幅が有意に増加した。
数点の分析により、残りの治療群に比較して、欠損未充填治療群で骨梁幅の有意な増加が明らかになった。この差は、欠損未充填治療群において、周囲にある内在性硬骨の欠損空隙への崩壊に起因する。この場合、欠損プロファイルの崩壊のない残りの治療群での新生硬骨に比較して内在性硬骨が輪郭内で見つかっている。多重輪郭解析により、他の治療群に比較して、500μg rhPDGF−BB治療群で認められた有意な骨梁数の増加、および、12週目の早い時点での骨ブリッジングの促進の存在は、rhPDGF−BBが骨新形成に対し有意な影響を与えることを示しているものと考えられる。この軟骨下骨再構成促進は、修復促進を生じ、コラーゲン基質と組み合わせた場合、rhPDGF−BBが骨軟骨欠損修復治療に有望である可能性があることを示唆している。
膝骨軟骨欠損の外科的修復を目的とした二相性生体適合性マトリクスとrhPDGF−BBの使用に関する安全性評価のためのパイロットヒト臨床試験
この調査の主目的は、高荷重負荷と低荷重負荷膝骨軟骨欠損の治療のために、hPDGF−BBおよび二相性生体適合性マトリクス(例えば、コンドロミメティック(Orthomimetics))の安全性を確認し、性能調査を行うことである。この調査の2つ目の目的は、rhPDGF−BBと二相性生体適合性マトリクスの移植に対する外科手術および臨床成績の測定値(ICRS−国際軟骨修復学会フォーラム(International Cartilage Repair Society Form)、VAS−視覚的アナログスケール(Visual Analogue Scale)、Cincinnati Rating、KOOS−膝関節損傷と変形性関節症転帰スコア(Knee Injury and Osteoarthritis Outcome Score))を評価することである。

- 二相性生体適合性マトリクスおよび血小板由来増殖因子(PDGF)を含む骨軟骨欠損治療のための組成物であって、前記二相性生体適合性マトリクスが足場材料を含み、さらに前記足場材料が骨相および軟骨相を含む多孔質構造を形成する組成物。
- 前記骨軟骨欠損が軟骨および軟骨に隣接した硬骨中にあり、前記軟骨が関節軟骨、線維軟骨、または弾性軟骨を含む請求項1記載の組成物。
- 前記骨軟骨欠損が軟骨および軟骨に隣接した硬骨中にあり、前記軟骨に隣接した硬骨が軟骨下骨または海綿骨を含む請求項1記載の組成物。
- 前記骨相がカルシウムリン酸塩およびコラーゲンを含む請求項1〜3のいずれか1項に記載の組成物。

- 哺乳動物の骨軟骨欠損を治療する方法であって、前記哺乳動物に骨軟骨欠損の少なくとも1つの部位に対する二相性生体適合性マトリクスおよび血小板由来増殖因子(PDGF)を含む有効量の組成物を投与することを含み、前記二相性生体適合性マトリクスが足場材料を含み、前記足場材料が骨相および軟骨相を含む多孔質構造を形成する方法。
- 前記骨軟骨欠損が軟骨および軟骨に隣接した硬骨中にあり、軟骨が関節軟骨を含む請求項14に記載の方法。
