Description
1つの態様では、本開示は、妊娠ヒト被験者における子癇前症の発症リスクを評価する方法に関する。本明細書に記載の方法は、妊娠第1期中の妊婦から得られた生体試料中のレラキシン、および場合によりC反応性タンパク質(CRP)のレベルの測定を用いる。子癇前症は、女性が妊娠中に入院する主な理由の1つであることから、医療制度に対する高コストと関連している。子癇前症は、特に母親の血圧が140/90mmHgを超えた場合に、母体または胎児の健康上の懸念のため帝王切開による妊娠の早期終結をもたらすことが多いため、子癇前症またはこの症状で入院する妊婦の予後はこれまで深刻であった。今日現在、妊娠の終結以外に子癇前症の治癒はない。この問題を軽減するため、本開示は、子癇前症の発症可能性またはリスクを評価するのに使用できるテストを提供する。好ましい実施形態では、テストは、女性をモニターでき、子癇前症が完全に進展するのを防ぐための適切な介入を行えるように妊娠早期(例えば、第1期)で行われる。子癇前症のリスク増大を早期に認識することは、担当医師が妊娠患者の状態を発症から安定化できるようにする。高血圧を予防または低下させるための療法形態での介入は、同様に、母親および子どもの死亡リスクを低下させ、妊娠の早期終結のリスクをさらに低下させる。
用語「内因性レラキシン」または「天然レラキシン」は、本明細書では互換的に使用され、当技術分野でよく知られている自然発生のペプチドホルモンレラキシンを指す。女性では、レラキシンは卵巣の黄体、胸部ならびに、妊娠中は、胎盤、絨毛膜、および脱落膜によっても産生される。内因性レラキシンレベルは、黄体による産生の結果、排卵後に上昇し、黄体期中期および後期にピークに達する。受胎できない月経周期では、レラキシン濃度は検出不能なレベルまで減少する。しかし、受胎できる月経周期では、レラキシン濃度は急増し、第1期にピークに達する。レラキシン濃度は次いで緩やかに減少し始めるが、妊娠期間中上昇したままである。ヒト被験者に関して本明細書では用語レラキシン(天然レラキシンおよび内因性レラキシン)は、特に規定のない限り、H2レラキシンを指す。
子癇前症(preeclampsia)(またはpre−eclampsia)は、低酸素状態になる浅く着床した胎盤が原因となり得、胎盤からのアップレギュレート炎症性メディエーター(血管内皮で作用する)の分泌を特徴とする免疫反応をもたらす。浅い着床は、胎盤に対する母体免疫系の反応から生じる。この理論は、胎児およびこの胎盤由来の父親抗原に対する確立した免疫寛容の欠如を示唆する証拠に関連している。子癇前症の幾つかのケースでは、母親は、母体の免疫反応をダウンレギュレートするための、胎盤が分泌するタンパク質に対する受容体が欠けていると考えられる(Moffettら、Placenta Suppl. A:S51〜6頁、2007年)。しかし、子癇前症の多くのケースでは、胎盤に対する母体の反応は、正常な着床が起こるのを可能にしているように思われる。慢性高血圧症または自己免疫疾患などの基礎疾患から生じる炎症のベースラインレベルが高い女性は、妊娠の炎症影響に対する寛容が低くなり得る可能性がある。
内因性または天然レラキシンは、インスリンにサイズおよび形が類似したペプチドホルモンである。より具体的には、レラキシンは、インスリン遺伝子スーパーファミリーに属するエンドクリンおよびオートクリン/パラクリンホルモンである。コードされたタンパク質の活性型は、ジスルフィド結合(2つの鎖間および1つの鎖内)により結合された、A鎖およびB鎖から成る。故に、構造は、ジスルフィド結合の配置がインスリンに酷似している。ヒトでは、3つの既知の非対立レラキシン遺伝子、レラキシン−1(RLN−1またはH1)、レラキシン−2(RLN−2またはH2)およびレラキシン−3(RLN−3またはH3)がある。H1およびH2は、高い配列相同性を共有する。この遺伝子に関して記載された異なるアイソフォームをコードする2つの選択的スプライス転写変異体がある。H1およびH2は、生殖器官で異なって発現される(米国特許第5,023,321号;およびGaribay−Tupasら、Molecular and Cellular Endocrinology 219:115〜125頁、2004年参照)のに対し、H3は主に脳内で見出される。レラキシンペプチドファミリーおよびこの受容体の進化が、当技術分野で記載されている(Wilkinsonら、BMC Evolutionary Biology 5(14):1〜17頁、2005年;およびWilkinsonおよびBathgate、第1章、Relaxin and Related Peptides、Landes Bioscience and Springer Science+Business Media、2007年参照)。
レラキシンの特徴的な機能は、妊娠中の子宮頸部および膣の細胞外マトリックスのリモデリングならびに満期時の胎膜の破裂に関与する生化学的プロセスの制御を含む、女性生殖器官生理機能と関連がある。これらの修飾は、子が産道を通過できるようにし、難産(すなわち、分娩中の胎児下降もしくは子宮頸部拡張の著しい遅れもしくは停止またはこの両方)を予防する。さらに、レラキシンは、子宮および胎盤の成長を促進し、血管発生および子宮内膜の増殖に影響する(Parryら、Adv Exp Med Biol、612:34〜48頁、2007年)。
CRPは、身体での炎症に反応して肝臓により産生される血漿タンパク質を指す。炎症は、損傷、感染または高血圧などの状態に起因し得る。CRPは、先天性免疫系の一部および慢性全身性炎症マーカーと見なされている。CRPはまた、心臓血管イベントの独立した予測因子でもある。CRPは、アルブミン合成がIL−6および循環において増加する他のサイトカインに因り減少するのと同時に、急性期反応物質として正常妊娠で増加すると考えられる。特に、IL−1およびIL−6レベルは子癇前症でより高く、これはCRPレベルも子癇前症でより高い理由を説明し得る。この知見は、子癇前症中に悪化する、正常妊娠の炎症反応と一致する。故に、血清レラキシンレベルに加えて血清CRPレベルについて妊婦をテストすることは、これらの女性が子癇前症をどれ程発症する可能性があるかのさらなる洞察を提供することができる。もう1つの因子としてCRPを用いることにより、本明細書に記載のH2レラキシンテストの感度が増大する。
CART(分類および回帰木)は、実験例に開示された試験中に妊婦から得られたデータを分析するのに使用された。子癇前症予測に使用されるパラメータは、レラキシンおよび、場合により、C反応性タンパク質(CRP)である。図1に見ることができるように、妊娠中の在胎期間は3つの期間、すなわち、0〜15週(第1期)、15〜25週(第2期)、および25〜35週(第3期)に分けられる。疾患が十分に進展するのを防ぐ治療を開始するため、子癇前症マーカーについてできる限り早期に妊婦をテストすることが有益である。故に、第1期に女性をテストすることは、妊娠第2または第3期中に女性をテストするより好ましい。テストがより早期にできない場合、妊娠第2および/または第3期に女性をテストできる場合もある。
子癇前症は危険な状態であり、妊娠中、分娩中および分娩後最大6週の間のいつでも出現し得るが、子癇前症は最終期(第3期)に最も頻繁に生じ、ほとんどの場合妊娠20〜35週までは生じない。子癇前症は、徐々に発症、または極めて突然発症し得る(数時間で再燃することすらある)が、徴候および症状は数カ月間気づかれない場合がある。子癇前症が無症候性で、日常的な血圧チェックおよび/または尿検査中に突然出現し、赤ん坊は予定日が近い(36週後)ならば、陣痛が誘発され、赤ん坊が出産され、および母親は注意深くモニターされる。子癇前症が妊娠早期に生じる場合、この影響はさらにより深刻である。管理下で母親の血圧を維持するため、例えば、安静、薬物療法およびさらには入院が指示される場合がある。可能な限り子宮で維持されるのが赤ん坊の一番の利益になる。不幸にも、子癇前症の唯一の治癒は赤ん坊の出産であり、予定日前に赤ん坊を出産するのが母親の一番の利益になり得る。医師は、ベータ遮断薬、カルシウムチャネル遮断薬、ヒドララジン、アルファメチルドーパ、クロニジン、および稀なケースでは、ラシックスまたは利尿剤(ウォーターピル)などの降圧剤を処方することができるが、後者は一般に勧められない。血圧が薬物療法および治療で管理することができない、ならびに母親および/または乳児の健康が危険な状態にあるならば、生存可能な赤ん坊が早産され得るように乳児の肺の成熟を助けるため、母親はステロイドを与えられる場合がある。
早期検出の1つの利点は、該疾患によりなりやすい女性群を研究試験および/または臨床試験のため濃縮すること、および予防対策のテストを容易にすることである。本開示は、子癇前症および疾患進行および疾患との闘い方をより良く理解するための臨床および/または研究試験のため患者の濃縮集団を選択することを含む、新規のスクリーニングプロセスを可能にする。女性の濃縮集団は、低レラキシンレベルのテストにより定義することができ、科学的および/または臨床的に意義のある結果を得るためにはるかに少ない患者が必要とされる。妊娠第1または第2期中または子癇前症症状の出現前の妊婦が、血流中のレラキシンレベルを検査されてもよく、約500pg/ml未満のレラキシンレベルを有する者が、濃縮患者集団に選択される。この有益な選択プロセスがなければ、新規の薬剤または作用物質が子癇前症に効果を有するかどうかを判定するのに、数百人の女性のスクリーニングを必要とするであろう。故に、本開示は、子癇前症を発症するより高い可能性を有する妊婦を濃縮集団から選択すること、および新規作用物質の有効性についてこれらの女性をテストすることを含む、臨床および/または研究試験中に子癇前症を治療または予防する新規作用物質のスクリーニング方法を提供する。濃縮集団には、約500pg/ml未満である血流中レラキシンレベルを有する、妊娠第1および第2期中または子癇前症症状の出現前の女性が含まれる。
レラキシン、レラキシンアゴニスト、レラキシン模倣剤および/またはレラキシン類似体は、本開示の方法で使用される医薬品として調製される。生物学的もしくは薬学的に活性なレラキシン(例えば、合成レラキシン、組換えレラキシン)またはレラキシンアゴニスト(例えば、レラキシン類似体またはレラキシン様モジュレーターまたはレラキシン模倣剤)のレラキシン受容体への結合に関連した生物学的反応を刺激することができる任意の組成物または化合物が、本開示で医薬品として使用され得る。調製および投与法に関する一般的詳細は、科学的文献に十分に記載されている(Remington's Pharmaceutical Sciences、Maack Publishing Co、イーストン Pa.参照)。薬学的に活性なレラキシンを含有する医薬製剤は、医薬品の製造のための当技術分野で既知の任意の方法により調製することができる。本開示の方法で使用される薬学的に活性なレラキシンまたはレラキシンアゴニストを含有する製剤は、皮下(SQ)、筋肉内、静脈内、舌下、局所、経口および吸入を含むがこれらに限定されない、任意の従来許容可能な方法で投与するために調製されてもよい。説明例が以下に記載される。1つの好ましい実施形態では、レラキシンは皮下(SQ)投与される。
本開示の方法で使用される薬学的に活性なH2レラキシンまたは薬学的に有効なH2レラキシンキメラ、アゴニスト、または模倣剤を含有する製剤は、皮下、筋肉内、静脈内、舌下、局所、経口および吸入を含むがこれらに限定されない、任意の従来許容可能な方法で投与することができる。投与は、薬剤の薬物動態および他の特性ならびに患者の健康状態により変わるであろう。一般的ガイドラインが以下に示されている。
レラキシンおよび/またはCRPのエピトープを特異的に認識できる抗体の産生方法が、本明細書に記載されている。このような抗体には、ポリクローナル抗体、モノクローナル抗体(mAb)、ヒト、ヒト化またはキメラ抗体、単鎖抗体、Fabフラグメント、F(ab’)2フラグメント、Fab発現ライブラリーにより生成されたフラグメント、抗イディオタイプ(抗Id)抗体、および上記いずれかのエピトープ結合フラグメントが含まれ得るが、これらに限定されない。レラキシンおよび/またはCRPに対する抗体の産生のため、様々な宿主動物を、レラキシンタンパク質もしくはCRPタンパク質、またどちらか一方の部分を注射して免疫化することができる。このような宿主動物には、ウサギ、マウス、およびラットが含まれ得るが、これらに限定されない。フロイント(完全および不完全)、水酸化アルミニウムなどのミネラルゲル、リゾレシチンなどの界面活性物質、プルロニックポリオール(pluronic polyol)、ポリアニオン、ペプチド、油エマルション、キーホールリンペットヘモシアニン、ジニトロフェノール、ならびにBCG(カルメットゲラン桿菌)およびコリネバクテリウム・パルブム(Corynebacterium parvum)などの潜在的に有用なヒトアジュバントを含むがこれらに限定されない、様々なアジュバントを、宿主種に応じて、免疫学的反応を増加するのに使用することができる。

卵巣の黄体から放出されるペプチドホルモンであるレラキシンは、妊娠中の強力な血管拡張剤である。血清H2濃度は第1期中に上昇し、第2期早期にピークに達し、著しい母体腎臓および全身血管拡張と一致する。対照的に、不十分な血管拡張の適応および全身性血管抵抗性の増加が、子癇前症の顕著な特徴である。この例は、妊娠中の血清レラキシンの測定、および第1期中の血清レラキシンの低下と子癇前症の発症リスクの増加との関連の判定を示す。
CART(分類および回帰木)を用いて、表2に示した特徴を有する69人の妊婦から得られたデータを分析した。この拡大分析は、実施例1の最初の試験被験者、および数人の追加被験者から得られたデータに基づく。子癇前症予測に使用したパラメータは、最初はH2レラキシンレベルであり、次いでC反応性タンパク質(CRP)レベルも使用した。図1に見ることができるように、妊娠中の在胎期間は、3つの期間、すなわち、0〜15週(第1期)、15〜25週(第2期)、および25〜35週(第3期)に分けられた。図1は、子癇前症女性(HPUおよびHP群)における血清レラキシン濃度を在胎年齢に関して示す。線は同じ被験者由来の試料を結ぶ。三角形は、子癇前症を後期に発症し、最初の15週に内因性H2レラキシンレベルが500pg/mlより低かった妊婦由来の試料を表す。四角形は、子癇前症を後期に発症し、および内因性H2レラキシンレベルが約500pg/mlであった妊婦由来の試料を表す。菱形は、子癇前症を発症しなかった妊婦由来の試料を表す。これらの女性で、最初の15週に500pg/mlより低いH2レラキシン濃度であった者はほとんどいなかったことに注意すべきである。
妊婦由来の血清試料は妊娠早期に採取する。血清H2レラキシン濃度およびCRP濃度はELISAにより判定する。図3に示されたようなアルゴリズムを用いて、子癇前症を発症する可能性を判定する。例えば血清H2レラキシンレベルが300pg/mlならば、患者は子癇前症のほぼ確実な発症を予防するための治療を受ける。血清レラキシンレベルが600pg/mlならば、CRPレベルも検査される。CRPレベルが1.5mcg/mlより高くおよび13.5mcg/ml未満ならば、妊娠は正常妊娠と見なされる。しかし、妊娠被験者のCRPレベルがこの範囲外であるならば、妊娠被験者は子癇前症を発症するリスクが高い。

- 妊婦が子癇前症を発症するリスクが高いかどうかを評価する方法であって、
a)子癇前症の発症前の前記妊婦から得られた生体試料中のH2レラキシン濃度を測定するステップと、
b)前記H2レラキシン濃度が妊婦の下位四分位濃度のカットオフ値未満である場合、前記妊婦は子癇前症を発症するリスクが高いと判定するステップと
を含む、方法。 - 前記生体試料が血漿または血清を含む、請求項1に記載の方法。
- 前記H2レラキシンが、前記H2レラキシンに対する抗体を用いて測定される、請求項2に記載の方法。

- 請求項22に記載のモノクローナル抗体、マイクロプレート、および試料のH2レラキシン濃度を測定するための説明書を含む、免疫アッセイキット。
- 前記免疫アッセイが、さらにポリクローナル抗レラキシン抗体を含むH2レラキシン捕捉アッセイである、請求項23に記載の免疫アッセイキット。
