Claims (15)
- 末梢オピオイド受容体アンタゴニストおよび少なくとも1つの添加剤を含む粒子であって、該粒子の粒径が30−1000nmである、粒子。
- (a)オピオイド受容体アンタゴニストを含む、内側のより大きな粒子;および
(b)少なくとも1つの界面活性剤および少なくとも1つの添加剤を含む、外側のより小さな粒子
を含み、外側粒子の粒径が100−500nmの範囲にある、ヘテロ粒状物質の粒子である、請求項1記載の粒子。 - オピオイド受容体アンタゴニストがメチルナルトレキソン、第四級または第三級モルフィン誘導体、ピペリジン−N−アルキルカルボキシレート、カルボキシ−ノルモルフィナン誘導体、または第四級ベンゾモルファンであって、第四級モルフィナンがN−メチルナルトレキソン、N−メチルナロキソン、N−メチルナロルフィン、N−ジアリルノルモルフィン、N−アリルレベロルファン、またはN−メチルナルメフェンの第四級塩である、請求項1または2記載の粒子。
- 添加剤が以下の成分:
(a)多糖またはポリフォスフェートであってもよい、ポリマー;
(b)疎水性添加剤;
(c)(i)酸性および中性のpHで正に帯電されていてもよく、または(ii)酸性および中性のpHで負に帯電されていてもよい、親水性添加剤
を含む、請求項1または2記載の粒子。 - 酸性および中性のpHで正に帯電されている親水性添加剤がキトサンであり、および/または酸性および中性のpHで負に帯電されている親水性添加剤がトリポリリン酸ペンタナトリウム(TPP)である、請求項4記載の粒子。
- オピオイド受容体アンタゴニストがメチルナトレキソンであり、添加剤がキトサンである、請求項1〜5のいずれか一項に記載の粒子。
- 粒子が腸溶性コーティング剤にて含まれ、腸溶的に被覆された粒子を形成するか、またはヘテロ粒状物質の粒子の外側のより小さな粒子が腸溶性コーティング剤として処方されている、請求項1〜6のいずれか一項に記載の粒子。
- 腸溶性コーティング剤がポリマーおよび/またはアセチル化単糖を含む、請求項7記載の粒子。
- ポリ(カプロラクトン)であってもよい、持続放出性剤を含む、請求項1〜8のいずれか一項に記載の粒子。
- 界面活性剤がホスファチジルコリンまたは非イオン性界面活性剤を含む、請求項2記載の粒子。
- 請求項1〜10のいずれか一項に記載の粒子と、医薬上許容される担体とを含む、医薬組成物。
- 経口投与可能であり、および/または懸濁液またはカプセルにて含まれる、請求項11記載の医薬組成物。
- 請求項1に記載の複数の粒子を製造する方法であって、
各粒子が末梢オピオイド受容体アンタゴニストを含む粒子では、
(a)末梢オピオイド受容体アンタゴニストを水に溶かし、末梢オピオイド受容体アンタゴニスト溶液を形成し;
(b)該末梢オピオイド受容体アンタゴニスト溶液を第1添加剤を含む溶液に加え、末梢オピオイド受容体アンタゴニスト/第1添加剤溶液を形成し;および
(c)該末梢オピオイド受容体アンタゴニスト/第1添加剤溶液を第2添加剤を含む溶液に加え、それで複数の粒子を製造する
ことを含み、
さらに以下の
(d)該懸濁液を遠心分離に付し、そうしてその中にある液体を末梢オピオイド受容体アンタゴニストを含む粒子より分離し;
(e)上澄みを取り出し;および
(f)該粒子を凍結乾燥に付してもよく、
任意で該粒子を腸溶性コーティング剤にてカプセル化してもよく、あるいは
(i)末梢オピオイド受容体アンタゴニストを含む、内側のより大きな粒子;および(ii)少なくとも1つの界面活性剤および少なくとも1つの添加剤を含む、外側のより小さな粒子を含み、外側粒子の粒径が100−500nmの間にある、ヘテロ粒状物質の粒子では、
(a)複数の第1粒子を含む水性懸濁液を調製し;
(b)末梢オピオイド受容体アンタゴニストを含む少なくとも1つの第2粒子を複数の第1粒子を含む水性懸濁液に分散させ;および
(c)工程(b)の生成物を噴霧乾燥させ、
ここで第2粒子の粒径は複数の第1粒子の平均粒径よりも大きく、
さらに該水性懸濁液を濃縮することを含んでもよい、方法。 - 患者におけるオピオイド誘発の副作用を防止するか、またはオピオイド受容体活性により媒介される障害を処置するのに用いるための、請求項1〜10のいずれか一項に記載の粒子、または請求項11または12に記載の医薬組成物であって、副作用が便秘、不快、掻痒、尿閉、腸閉塞、術後腸閉塞、まひ性腸閉塞、分娩後腸閉塞、胃腸障害、突発性便秘、腸運動低下、閉塞、胃運動低下、胃腸運動阻害、胃内容排出の阻害、胃内容排出の遅延、不完全な排出、発汗または掻痒作用であり;または障害が癌、炎症性障害、免疫抑制、心血管障害、慢性炎症、慢性痛、鎌状赤血球貧血、血管損傷、網膜症、胆汁分泌の低下、膵臓分泌物の減少、胆道のけいれん、および胃食道逆流の増加である、粒子または医薬組成物。
- 患者が慢性オピオイド患者である、請求項14に記載の方法に用いるための粒子または医薬組成物。
