Abstract
Description
Claims (21)
- 弁輪形成リングとホルダとの組立体であって、
縫合糸を通すことが可能な外側カバーを含む弁輪形成リングと、
テンプレートであって、該テンプレートは、上側近位面と、下側遠位面と、該テンプレートに一致接触して該弁輪形成リングを受け取るようなサイズを有し、かつ受け取るように適合されている周縁部とを有し、該テンプレートは、該近位面から上方に延びる対の離間壁により規定される、該周縁部に隣接する単一切り取りウェルと、該テンプレート周縁部に隣接する2つの離間クリートとをさらに含み、各クリートは、該切り取りウェルから該周縁部の周囲に円周方向に少なくとも90°に位置する、テンプレートと、
可撓性連結フィラメントと
を備え、
該可撓性連結フィラメントは、該2つの離間クリートに固定される該可撓性連結フィラメントの自由端と中間部分とを有し、該中間部は、該弁輪形成リング外側カバー上の少なくとも2点を貫通し、1つだけの位置において該テンプレートの該近位面上に出現し、該切り取りウェルを横断して懸架されている、組立体。 - 前記2つの離間クリートは、前記切り取りウェルから、前記周縁部の周囲において円周方向に等距離で配置される、請求項1に記載の組立体。
- 前記テンプレート周縁部は、前記弁輪形成リングを受け取るための実質的に角度を付けられたチャネルを規定し、該チャネルは、略軸方向に延びる遠位壁と該テンプレートの外側範囲を規定する外側に延びる近位突起とにより形成されている、請求項1に記載の組立体。
- 前記遠位壁と前記近位突起との間の前記角度を付けられたチャネルの挟角は、前記周縁部の周囲で変動する、請求項3に記載の組立体。
- 前記テンプレート周縁部は、外側に延びる近位突起を含み、前記2つの離間クリートの各々は、前記近位面より下に凹状である前記近位突起を通る第1の対の穴によって形成される、請求項1に記載の組立体。
- 前記第1の対の穴の各々と前記切り取りウェルとの間に位置する、前記近位突起を通る第2の対の穴をさらに含み、前記フィラメントは、各第1の対の穴に対して縛り、前記弁輪形成リング外側カバーを貫通し、該第2の対の穴を上下に通って輪状になり、そして、前記テンプレートの前記近位面の上に出現するまで該近位突起の下にあり続け、前記切り取りウェルを横断して懸架される、請求項5に記載の組立体。
- 前記テンプレートは、ハンドル部材が接続する連結器をさらに含み、該連結器は、前記切り取りウェルとは直径方向において反対の前記周縁部に隣接して位置する、請求項1に記載の組立体。
- 前記弁輪形成リングは、前記縫合糸を通すことが可能な外側カバーにより囲まれる略硬質の内核を含み、前記テンプレート周縁部は、前記テンプレートの外側範囲を規定する外側に延びる近位突起によって部分的に形成され、該弁輪形成リングが該周縁部に一致接触して受け取られた場合に、該近位突起は該硬質の内核から半径方向外側に延び、該縫合糸を通すことが可能な外側カバーは、該近位突起から外側に延びている、請求項1に記載の組立体。
- 前記切り取りウェルの近く、かつ、該切り取りウェルの両側において2つの自由端において前記テンプレートに対して固定されている第2の可撓性連結フィラメントをさらに含み、該第2の可撓性連結フィラメントの一部分は、該テンプレートの近位面より上を該2つの自由端の間で延び、該第2の可撓性連結フィラメントの一部分は、該切り取りウェルを横断して懸架される、請求項1に記載の組立体。
- テンプレートを備え、
該テンプレートは、上側近位面と、下側遠位面と、該テンプレートに一致接触して該弁輪形成リングを受け取るようなサイズを有し、かつ受け取るように適合されている周縁部とを有し、該テンプレートは、該近位面から上方に延びる対の離間壁により規定される、該周縁部に隣接する単一可撓性連結フィラメント切り取りウェルを含み、該テンプレートは、ハンドル部材が接続する連結器をさらに含み、該連結器は、該切り取りウェルとは直径方向において反対の該周縁部に隣接して位置している、
弁輪形成リングホルダ。 - 前記テンプレート周縁部は、長軸および長寸法と、短軸および該長寸法より小さい短寸法とを有し、前記単一切り取りウェルは、該短軸に沿って位置している、請求項9に記載のホルダ。
- 前記テンプレート周縁部は、該周縁部内の断面積の大部分を占める少なくとも1つの視界窓を囲む、請求項9に記載のホルダ。
- 前記テンプレート周縁部は、前記弁輪形成リングを受け取るための実質的に角度を付けられたチャネルを規定し、該チャネルは、略軸方向に延びる遠位壁と該テンプレートの外側範囲を規定する外側に延びる近位突起とにより形成されている、請求項9に記載のホルダ。
- 前記遠位壁と前記近位突起との間の前記角度を付けられたチャネルの挟角は、前記周縁部の周囲で変動する、請求項13に記載のホルダ。
- 前記テンプレートは、該テンプレート周縁部に隣接して2つの離間クリートを含み、各クリートは、前記切り取りウェルから前記周縁部の周囲に円周方向に少なくとも90°に位置し、可撓性連結フィラメントが固定され得る、請求項9に記載のホルダ。
- 弁輪形成リングを標的弁輪に送達する方法であって、
送達するために弁輪形成リングとテンプレートとの組立体を準備することであって、
該テンプレートは、上側近位面と、下側遠位面と、周縁部とを有し、該周縁部は、該周縁部に一致接触して該弁輪形成リングを受け取るようなサイズを有し、かつ、受け取るように適合され、該テンプレートは、該近位面から上方に延びる対の離間壁により規定される、該周縁部に隣接する単一切り取りウェルをさらに含み、該テンプレートに固定されている可撓性連結フィラメントが、該弁輪形成リングを該周縁部に対して保持し、該切り取りウェルを橋架し、該テンプレートは、ハンドル部材が接続する連結器を該周辺面上にさらに含み、該連結器は、該切り取りウェルとは直径方向において反対の該周縁部に隣接して位置している、ことと、
ハンドル部材を該連結器に連結することと、
該弁輪形成リングとテンプレートとの組立体を該標的弁輪に近接させるように遠位に進めることと、
該可撓性連結フィラメントを該切り取りウェルにおいて切断することにより、単一の切断ステップで該弁輪形成リングを該テンプレートから解放することと
を含む、方法。 - 前記テンプレート周縁部は、長軸および長寸法と、短軸および該長寸法より小さい短寸法とを有し、前記単一切り取りウェルは、該短軸に沿って位置している、請求項13に記載の方法。
- 前記弁輪形成リングは、僧帽弁輪のためのものであり、前記テンプレート周縁部は、前記短軸を横断して前方区分に対向する後方区分を有し、前記単一切り取りウェルは、前記後方区分上に位置する、請求項14に記載の方法。
- 前記テンプレート周縁部は、該周縁部内の断面積の大部分を占める少なくとも1つの視界窓を囲み、前記方法は、該視界窓から前記標的弁輪を視認することを含む、請求項13に記載の方法。
- 前記テンプレート周縁部は、前記弁輪形成リングを受け取るための実質的に角度を付けられたチャネルを規定し、該チャネルは、略軸方向に延びる遠位壁と該テンプレートの外側範囲を規定する外側に延びる近位突起とにより形成されている、請求項13に記載の方法。
