Abstract
【解決手段】 腫瘍内部または腫瘍を含む領域内部において低酸素領域を作製し、この局所領域内部における低酸素活性化生体還元剤(例 チラパザミン)の活性化を増強する。活性化された低酸素活性化生体還元剤は、腫瘍細胞内部でDNA鎖切断を触媒することにより低酸素領域において腫瘍細胞を死滅させる。その活性が限局されているので、生体還元物質の全身投与の結果として一般的に起こる副作用を低減する。

Description
血管新生阻害剤および血管破壊剤の両者によるこれらの腫瘍血管標的物質の中心的課題は、腫瘍細胞から血管サポートを奪い去り腫瘍が低酸素を生じ次に壊死を受けることにある。腫瘍中低酸素類の発生は従って腫瘍細胞死を誘発する重要要件である。しかし、低酸素腫瘍細胞は低酸素環境で生存を促す解糖系酵素の産生またはVEGFおよび脈管形成を誘発する他の脈管形成因子の産生を誘発する低酸素誘発因子(Hypoxia−Induced Factor)(HIF)1−αの安定化のようなさまざまな低酸素反応を発生させる(14,15)ので、腫瘍中低酸素は細胞死を誘発するには十分でない。低酸素中の腫瘍細胞で産生されるもうひとつの因子である一酸化窒素(NO)は、血管拡張を誘発し従って同様に腫瘍血液供給を改善する(16,17)。NOはまた脈管形成にも強く関連している(18,19)。これらの代償的メカニズムの結果、血管新生阻害剤およびVDAに対する薬物抵抗性が生じることになる。
腫瘍脈管構造を標的とする治療効率を増強する可能な戦略が数種ある。ひとつは従来の化学療法といずれかの物質群を併用することであり、それは現在血管新生阻害剤とともに使用される通常の戦略となっている。VEGFに対するモノクローナル抗体であるベバシズマブは通常、結腸直腸癌、非小細胞肺癌および乳癌治療に化学療法と併用される。VDAは現在さまざまな固形腫瘍に対する第二相治験に入っているが、これまでFDA承認を得たものはない。ひとつの新しい戦略は、VDAと血管新生阻害剤を併用することである。これは、VDAがVEGF上昇を誘発しその後骨髄内皮始原細胞を血行循環に移動させ従って破壊された腫瘍血管修復に関与するという観察に基づいている(20)。VEGF経路の阻害剤を用いることで前記移動をブロックし治療効果を増強できた(11,20)が、この戦略は臨床状況で証明されていない。
チラパザミン作用について提案されているメカニズムを下図に示した(24−28)。チラパザミンの1電子還元はチトクロームP−450、NADH−チトクロームP−450レダクターゼおよび他のフラビンタンパク質または金属タンパク質を含む酵素類による還元作用により進行する。この1電子転移反応の産物はフリーラジカル(IまたはII)であり、それは酸素により酸化されスーパーオキサイドおよび親薬物チラパザミンを生ずる。これとは別に、このフリーラジカル(IまたはII)は水素抜き取り反応中間体(III)を介してもうひとつ電子を得て安定なモノ−N−オキサイド(SR4317)を形成できる。これらの多様な結末が、低酸素環境におけるチラパザミンの選択的代謝につながる。前記第二の電子を大分子から得ることが、低酸素細胞への致死的傷害の原因であると示唆された。別の可能な経路は、中間体IIからのヒドロキシルラジカルの放出で直接SR4317を産生し、それはさらに類似反応によりSR4330に代謝される。
前臨床および臨床の両者のためチラパザミンの重要な開発が行われてきた。チラパザミンの動物研究は、その潜在的副作用として骨髄毒性、臭覚神経壊死、および網膜変性が含まれることを明らかとした(21)。3週に1度チラパザミンを静注投与する第一相臨床研究は、最大耐量(MTD)が390mg/m2であることおよび用量が330mg/m2を超えた時の用量制限毒性には可逆性の聴覚毒性や一過性の視覚異常が含まれる(30、31)ことを示した。他の非特異的毒性には、筋肉痙攣、悪心、嘔吐および下痢などが含まれる。血小板減少症グレード1が450mg/m2を投与した患者1例で観察され、どの患者でも白血球減少症が全く観察されなかった(30)。

“塞栓形成”とは、物質(リピオドール、ゲルフォーム、血栓等)を注入して、腫瘍を含む領域に血液を供給する動脈側枝の閉塞を誘発しその結果腫瘍細胞が適切な血流を得ることができず死亡することによって、例えば肝細胞癌に血液を供給する肝動脈のような確認可能な動脈側枝により血液を供給される腫瘍または腫瘍を含む領域に用いられる限局された療法を意味する。前記領域および周辺正常臓器または組織の血液供給の分析により、周辺臓器/組織が塞栓形成後に血液供給不足により有意な傷害をこうむるかどうかを判定する。例えば、正常肝臓には二重の血管すなわち肝動脈と門脈により血液が供給され、従って、正常肝で有意な傷害を結果として生ずることなく肝動脈またはその側枝の閉塞が可能となる。この操作は一般的に介入担当の放射線科医によって実施され、この放射線科医は鼠径部大腿動脈からカテーテルを配置しカテーテル先端をフルオロスコープX線ガイダンスにより腫瘍に血液を供給する肝動脈側枝まで進める。いったん腫瘍に血液供給する動脈側枝が造影剤注入により確認できると、リピオドールまたはゲルフォームのような塞栓形成物質を注入し側枝を閉塞させる。この操作は、肝細胞癌治療のための標準的移動局所療法である(40−43)。
本発明の他の態様においては、低酸素を誘発するVDAとともに低酸素活性化生体還元剤を用いる。本発明の実施で用いることができるVDAにはコンブレタスタチン誘導体類(9)、(5S)−5−(アセチルアミノ)−9,10,11−トリメトキシ−6,7−ジヒドロ−5H−ジベンゾ〔a,c〕−シクロヘプテン−3−イル ジヒドロゲンホスフェート(ZD6126)(44)、DMXAA(5,6−ジメチルキサンテノン−4−酢酸(9)、(N−〔2−〔4−ヒドロキシフェニル〕アミノ〕−3−ピリジニル〕−4−メトキシベンゼンスルホンアミド)(E7010またはABT−751)(45)、シス−3、4’、5−トリメトキシ−3’−アミノスチルベン(スチルベン5c)およびシス−3、4’、5−トリメトキシ−3’−ヒドロキシスチルベン(スチルベン6c)またはそれらの誘導体のようなスチルベン誘導体、および例えば米国特許出願第11/738,813号(Leeら、その完全な内容を参照により本明細書に組み入れる)により記載されたプロドラッグ類(スチルベン5cのモルホリノ−カルバメート誘導体)が含まれるが、それらに限定されない。これらの化合物は、たとえ全身投与しても腫瘍中に選択的に顕著な低酸素を誘発する(20)。VDAの投与はある種の化学的塞栓形成と見なすことができ、それは、上記で述べた標準的塞栓形成で血管を直接閉塞させるのと対比的に腫瘍含有領域中で選択的に塞栓形成という同一目標を達成するために化学物質を用いる。VDAとチラパザミンのような低酸素活性化生体還元剤の併用は、固形腫瘍悪性疾患の治療においていずれかの物質の単独での活性に基づいて予測されるよりも驚くべきことにより効果的である。すなわち、それらの活性が相乗的である(併用物質の腫瘍死滅効果が各物質単独の効果の算術的総計よりも高い)。例えば、チラパザミン後に投与したVDAはチラパザミンの活性化を可能とし、その後の腫瘍細胞死滅をいずれかの物質単独による腫瘍細胞死滅レベルに比較して少なくとも10倍以上高める。この戦略をまた先に述べた塞栓形成と組み合わせることができ、腫瘍低酸素およびチラパザミン活性化誘発においてはるかに効果的である。
本発明のさらに別の態様において、1種以上のHABAを例えば(ベバシズマブのような)脈管内皮増殖因子(VEGF)モノクローナル抗体または(ソラフェニブまたはスニチニブのような)VEGFレセプターチロシンキナーゼ阻害剤のようなAAAと併用する。共投与が腫瘍低酸素時間を長くすることでAAAとHABAの効果の相乗作用を引き起こし、さらに、HABAの治療効果を増強する。
細胞株および腫瘍移植片モデル
本研究に用いた腫瘍細胞株にはUCI−101卵巣癌細胞およびHep3B肝細胞癌細胞が含まれる。細胞を5%CO2湿潤環境中10%ウシ胎児血清、グルタミン、およびペニシリウム/ストレプトマイシン添加IMEMおよびDMEM中で、それぞれ、増殖させる。細胞が増殖し80%コンフルエンシーになった時細胞を1%トリプシンで採取し、リン酸緩衝生理食塩水で3回洗浄し、その後ヌードマウスの皮下に注入した。マウス移植片研究のため、2×106腫瘍細胞をその背部に皮下注入した。ヌードマウスは、説明どおり、NCI Development Therapeutic Programから購入した。腫瘍径はカリバーで長軸および短軸(それぞれ、aおよびb)によりモニタリングし、腫瘍容積は、式ab2/2により計算した。VDAシス−3、4’、5−トリメトキシ−3’−アミノスチルベン(スチルベン5c)(50mg/kg)、チラパザミン(60mg/kg)による治療は、腹腔内注入により実施した。AAAベバシズマブによる治療は、用量10mg/kgの尾静脈注入により実行した。
腫瘍移植片を有するヌードマウスを1%イソフルランと混合酸素で麻酔した。DEC−MRIは、小型動物イメージングを専門とする実験的磁気共鳴(MR)システム(Bruker、Biospec2.35T/40cm)で行った。頚静脈を切開しカテーテルIVを入れ、急性相研究において造影剤注入を行った。バッチ1のマウスは、頚部カテーテルからガドリニウム(OmniScan)50μlを注入し、MRIイメージは毎秒採取し、注入後すぐのMRIシグナル増加の初期速度を調べた。初期速度を決めた後、次のMRI研究ではガドリニウムシグナルの持続的増加に注目し、そのことはまた、組織潅流の定性的および定量的情報を提供する。この研究のため、ガドリニウム20μlを直接尾静脈に注入した。マウスは、注入1分以内にMRI装置トンネルに移した。MRIイメージは30分間、毎分採取した。
マウスを屠殺した後主要臓器と腫瘍を摘出し、10%ホルマリンに固定した。組織をパラフィンに包埋し、切片をH&EおよびCD34抗体で染色し、微小血管密度を定量した。微小血管密度の点数は、Diagnostic Instuments Spot RT CCDカメラ装着Nikon ECLIPSE E800M顕微鏡を用いて倍率200倍のフォトフレーム中CD34−陽性シグナルを計数することによるWeidnerらの方法に基づいていた。
ヌードマウスにUCI−101卵巣癌細胞を皮下注入し、腫瘍移植片を形成させた。腫瘍の大きさが最大直径で8−10mmになった時に移植片を酸素および血流研究に供した。腫瘍に25G針で穿孔し、センサープローブ挿入のためのトンネルを形成させた。センサープローブの先端を腫瘍中心に留置した。腫瘍酸素レベルと血流は、組織酸素、温度および血流をレーザードップラー技術で測定するように設計されている“ベア−ファイバー”タイプのセンサープローブを有するOxyLite 2000デュアルチャンネルモニタリングシステム(Optonix、oxford、UK)で測定した。測定はリアルタイムベースで行い(毎秒100回測定)、酸素レベルと腫瘍血流は、連続的に記録した。針およびセンサープローブ挿入により誘発された初期外傷は血流および酸素化を妨害できるので、各マウスは、試験したVDAシス−3、4’、5−トリメトキシ−3’−アミノスチルベン(スチルベン5c)を50mg/kgでマウスに腹腔内投与する前処理しないで少なくとも20分間、最初に記録した。腫瘍血流および酸素レベルを処理後さらに20分間、記録した。

- 動物中腫瘍細胞を選択的に死滅させる方法であって、
前記動物に低酸素活性化生体還元剤を付与する過程と、
腫瘍内部または1個以上の腫瘍を含む限定された領域内部に、酸素10%以下の低酸素領域を局所的に形成し、それによって前記低酸素活性化生体還元剤が活性化され前記腫瘍または限定された領域内部の前記低酸素領域で腫瘍細胞を死滅させる過程と
を含むことを特徴とする方法。 - 前記低酸素領域を局所的に形成させる前記過程が前記動物に対して血管破壊剤および血管新生阻害剤の1種以上を付与することによって達成されることを特徴とする請求項1記載の方法。

- 前記血管新生阻害剤および血管破壊剤の1種以上が、ベバシズマブ、ソラフェニブ、スニチニブ、アフリベルセプト、IMC−1C11,バタラニブ、N−(2,3−ジヒドロ−3,3−ジメチル−1H−インドール−6−イル)−2−〔(4−ピリジニルメチル)アミノ〕−3−ピリジンカルボキサミド(AMG706)、3−(4−ブロモ−2,6−ジフルオロ−ベンジルオキシ)−5−〔3−(4−ピロリジン−1−イル−ブチル)〕−ウレイド〕−イソチアゾール−4−カルボン酸アミド(CP−547,632)、パゾパニブ(GW−786034)、N−(4−(3−アミノ−1H−インダゾール−4−イル)フェニル)−N’−(2−フルオロ−5−メチルフェニル)尿素(ABT−869)、およびセジラニブ(AXD−2171)、コンブレタスタチン、コンブレタスタチン誘導体、(5S)−5−(アセチルアミノ)−9,10,11−トリメトキシ−6,7−ジヒドロ−5H−ジベンゾ〔a,c〕−シクロヘプテン−3−イル ジヒドロゲンホスフェート(ZD6126)、DMXAA(5,6−ジメチルキサンテノン−4−酢酸)、(N−〔2−〔4−ヒドロキシフェニル)アミノ〕−3−ピリジニル〕−4−メトキシベンゼンスルホンアミド)(E7010またはABT−751)、シス−3、4’、5−トリメトキシ−3’−アミノスチルベン(スチルベン5c)およびシス−3、4’、5−トリメトキシ−3’−ヒドロキシスチルベン(スチルベン6c)またはそれらの誘導体などのスチルベン誘導体、およびスチルベン5cのプロドラッグモルホリノ−カルバメート誘導体から構成される群から選択されることを特徴とする請求項21記載の組成物またはキット。
- 前記血管新生阻害剤および血管破壊剤の1種以上がシス−3、4’、5−トリメトキシ−3’−アミノスチルベン(スチルベン5c)を含むことを特徴とする請求項22記載の組成物またはキット。
- 前記血管新生阻害剤および血管破壊剤の前記1種以上がベバシズマブを含むことを特徴とする請求項22記載の組成物またはキット。
