Abstract
Description
第1および第2の端部を備えた筒状部および第1の突出部を有する主管であって、第1の突出部は筒状部の第1の端部から突出している主管と、
第1の分岐管と、を具備してなり、
この第1の分岐管は、
(a)第1の突出部、あるいは、
(b)第1の突出部に隣接するポジションにおける筒状部に対して接続されており、かつ、そこから延在している。
プロテーゼを提供するステップであって、当該プロテーゼは、
第1および第2の端部を備えた筒状部および第1の突出部を有する主管であって、第1の突出部は筒状部の第1の端部から突出している主管と、
第1の分岐管と、を具備してなり、この第1の分岐管は、
(a)第1の突出部、あるいは、
(b)第1の突出部に隣接するポジションにおける筒状部に対して接続されており、かつ、そこから延在しているものであるステップと、
主管が主血管内に位置し、かつ、第1の分岐管が枝血管の一つの中に位置するように血管の分岐領域にプロテーゼを送り込むステップと、を具備する。
大動脈内に配置するための主管であって、第1および第2の端部を有し、第1の端部は第2の端部の上流側で大動脈根内に配置するためのものである主管と、
主管の第1および第2の端部間に置かれたポジションにおいて主管に接続された第1の分岐管であって、左および右冠状動脈の一方内に配置するためのものである第1の分岐管と、
プロテーゼ大動脈弁と、を具備してなり、
第1の分岐管と主管の接続ポジションと主管の第1の端部との間の距離は、第1の分岐管が左および右冠状動脈の一方内に配置されたとき、主管の第1の端部は、それが大動脈弁を押圧するポジションまで大動脈根内で延在し、大動脈弁を開放ポジションで維持するようなものである。
プロテーゼを提供するステップであって、当該プロテーゼは、
第1および第2の端部を有する主管と、
この主管の第1および第2の端部間に置かれたポジションにおいて主管に対して接続された第1の分岐管と、
プロテーゼ大動脈弁と、を具備してなるものであるステップと、
プロテーゼを分岐領域へと送り込むと共に、主管を大動脈内に、そして第1の分岐管を左および右側冠状動脈の一方内に配置し、これによって、主管の第1の端部で大動脈弁を押圧し、大動脈弁を開放ポジションで維持するステップと、を具備してなる。
主管であって、その第1および第2の端部間で延在する壁を有する主管と、
一つ以上の分岐管とを具備してなり、
主管の壁は、第1のセクションおよび第2のセクションを具備してなり、第2のセクションは第1のセクションよりも、さらにフレキシブルでありかつ/または弾力性に富み、かつ、
一つ以上の分岐管は、第2のセクションに対して接続されると共に、この第2のセクションから延在する。
プロテーゼを提供するステップであって、当該プロテーゼは、
主管であって、その第1および第2の端部間で延在する壁を有する主管と、
一つ以上の分岐管とを具備してなり、
主管の壁は、第1のセクションおよび第2のセクションを具備してなり、第2のセクションは第1のセクションよりも、さらにフレキシブルでありかつ/または弾力性に富み、かつ、
一つ以上の分岐管は、第2のセクションに対して接続されると共に、この第2のセクションから延在しているものであるステップと、
主管が主血管内に位置し、かつ、第1の分岐管が枝血管の一つの中に位置するように、プロテーゼを血管の分岐領域へと送り込むステップと、
を具備する。
遠位端部および近位端部を有するカテーテルコアと、
プロテーゼと係合すると共にそれを押しやるためにコアカテーテルの遠位端部に設けられたプッシュ要素と、
プロテーゼの一つ以上の分岐管を押しやるための一つ以上のアクチュエータと、を具備してなり、
アクチュエータはプッシュ要素に対して動作可能に設けられる。
主管および一つ以上の分岐管を具備してなるプロテーゼを提供するステップと、
遠位端部および近位端部を有するコアカテーテルおよび遠位端部のプッシュ要素を提供するステップであって、一つ以上のアクチュエータはプッシュ要素に対して動作可能に設けられたものであるステップと、
プッシュ要素がプロテーゼを分岐領域へと押しやるようにコアカテーテルを動かすステップと、
アクチュエータが一つ以上の分岐管を一つ以上の枝血管内へと押しやるように、プッシュ要素に対して一つ以上のアクチュエータを動かすステップと、を具備してなる。

Claims (38)
- 血管の分岐領域に配置するためのプロテーゼであって、
第1および第2の端部を備えた筒状部と、前記筒状部の前記第1の端部から突出する第1の突出部と、を有する主管と、
第1の分岐管と、を具備してなり、
前記第1の分岐管は、
(a)前記第1の突出部、あるいは、
(b)前記第1の突出部に隣接するポジションにおける前記筒状部に対して接続され、かつ、そこから延在していることを特徴とするプロテーゼ。 - 前記筒状部の前記第1の端部から突出する第2の突出部と、
第2の分岐管であって、
(a)前記第2の突出部、あるいは、
(b)前記第2の突出部に隣接するポジションにおける前記筒状部に対して接続され、かつ、そこから延在している第2の分岐管と、
を具備してなることを特徴とする請求項1に記載のプロテーゼ。 - 前記分岐管は、前記突出部に対して接続され、かつ、前記突出部から延在していることを特徴とする請求項1または請求項2に記載のプロテーゼ。
- 前記分岐管は、前記筒状部に対して接続され、かつ、前記筒状部から延在していることを特徴とする請求項1または請求項2に記載のプロテーゼ。
- 前記大動脈が前記右および左側冠状動脈と出会う、前記大動脈根の分岐領域に配置するために構成されたものであり、前記主管および前記分岐管はそれぞれ大動脈および前記冠状動脈内に配置されるものであり、前記突出部は前記大動脈弁の小葉部のボウル状キャビティ内に突出するようになっていることを特徴とする請求項1ないし請求項4のいずれか1項に記載のプロテーゼ。
- 前記突出部は、前記筒状部の前記第1の端部において、前記筒状部の外周を取り囲むように延在するリムあるいは環状領域から延在していることを特徴とする請求項1ないし請求項5のいずれか1項に記載のプロテーゼ。
- 各突出部は、前記リムまたは環状領域の外周の半分未満から延在していることを特徴とする請求項6に記載のプロテーゼ。
- 各突出部は、前記リムまたは環状領域の外周の三分の一未満から延在していることを特徴とする請求項6に記載のプロテーゼ。
- 各突出部は葉状部であることを特徴とする請求項1ないし請求項8のいずれか1項に記載のプロテーゼ。
- 前記突出部の一つ以上は、前記筒状部の延長方向と交差する方向に延在していることを特徴とする請求項1ないし請求項9のいずれか1項に記載のプロテーゼ。
- 前記突出部の一つ以上は、前記筒状部の延長方向と同じ方向に延在していることを特徴とする請求項1ないし請求項10のいずれか1項に記載のプロテーゼ。
- 前記主管は、この主管の壁を支持するための一つ以上のZステントを具備してなることを特徴とする請求項1ないし請求項11のいずれか1項に記載のプロテーゼ。
- 前記Zステントの一つは、前記筒状部の前記第1の端部に隣接して設けられており、かつ、前記突出部の外縁を画定していることを特徴とする請求項12に記載のプロテーゼ。
- プロテーゼ弁を具備してなることを特徴とする請求項1ないし請求項13のいずれか1項に記載のプロテーゼ。
- 前記弁は三つの弁小葉部を具備してなり、各弁小葉部の側縁は、三つの放射状に延在する隆起部を形成するために、前記隣接するバルブ小葉部の側縁と解放可能に係合するようになっていることを特徴とする請求項14に記載のプロテーゼ。
- 前記側縁は、前記筒状部の前記第1の端部に隣接して、かつ、突出部が筒状部の前記第1の端部から突出する領域間に、前記筒状部に対して接続された交連ポイントを有することを特徴とする請求項15に記載のプロテーゼ。
- 前記筒状部の外側にシーリング要素を具備してなることを特徴とする請求項1ないし請求項16のいずれか1項に記載のプロテーゼ。
- 前記分岐管の少なくとも一つは、フレキシブルでありかつ/または弾力性に富む前記突出部の壁セクションにつながっていることを特徴とする請求項1ないし請求項17のいずれか1項に記載のプロテーゼ。
- 前記壁セクションは、前記筒状部の壁よりも、さらにフレキシブルでありかつ/または弾力性に富むことを特徴とする請求項18に記載のプロテーゼ。
- 前記壁セクションは環状であることを特徴とする請求項19に記載のプロテーゼ。
- 前記壁セクションは、前記筒状部の壁とは異なる厚み、組織および/または化学的特性を有することを特徴とする請求項18ないし請求項20のいずれか1項に記載のプロテーゼ。
- 前記主管は壁支持枠を具備してなり、かつ、前記壁セクションは、前記筒状部の前記壁よりも少ない支持を前記支持枠から受けることを特徴とする請求項18ないし請求項21のいずれか1項に記載のプロテーゼ。
- 収縮状態と拡張状態との間で動作可能であることを特徴とする請求項1ないし請求項22のいずれか1項に記載のプロテーゼ。
- プロテーゼは、プロテーゼが収縮状態に置かれたとき、前記突出部の壁が、それにつながるかあるいは隣接する前記分岐管を包囲するよう構成されていることを特徴とする請求項23に記載のプロテーゼ。
- 血管の分岐領域に配置するためのプロテーゼであって、
主管であって、この主管の第1および第2の端部間で延在する壁を有する主管と、
一つ以上の分岐管と、を具備してなり、
前記主管の前記壁は第1のセクションおよび第2のセクションを具備してなり、前記第2のセクションは、前記第1のセクションよりも、よりフレキシブルでありかつ/または弾力性に富み、かつ、
前記一つ以上の分岐管は、前記第2のセクションに接続されており、かつ、前記第2のセクションから延在していることを特徴とするプロテーゼ。 - 前記第2のセクションは、前記第1のセクションの壁材よりも小さな厚み、それとは異なる組織および/または異なる化学的特性を有する壁材を具備してなることを特徴とする請求項25に記載のプロテーゼ。
- 前記主管は壁支持枠を具備してなり、かつ、前記第2のセクションは、前記第1のセクションよりも少ない支持を前記支持枠から受けることを特徴とする請求項25または請求項26に記載のプロテーゼ。
- 前記支持枠は、前記壁と一体である一つ以上のZステントから形成されることを特徴とする請求項27に記載のプロテーゼ。
- 前記第1および第2のセクションは、それぞれ第1および第2の周壁セクションであることを特徴とする請求項25ないし請求項28のいずれか1項に記載のプロテーゼ。
- 第3の周壁セクションを具備してなり、前記第2の周壁セクションは、前記第1および第3の周壁セクションよりも、よりフレキシブルであり、かつ/または弾力性に富むことを特徴とする請求項29に記載のプロテーゼ。
- 大動脈が右および左側冠状動脈と出会う大動脈根の分岐領域に配置するためのプロテーゼであって、
前記大動脈内に配置するための主管であって、第1および第2の端部を有し、前記第1の端部は前記第2の端部の上流側で大動脈根内に配置するためのものである主管と、
前記主管の前記第1および第2の端部間に置かれたポジションにおいて前記主管に接続された第1の分岐管であって、前記左および右側冠状動脈の一方内に配置するためのものである第1の分岐管と、
プロテーゼ大動脈弁と、を具備してなり、
前記第1の分岐管と前記主管との接続ポジションと、前記主管の前記第1の端部との間の距離は、前記第1の分岐管が前記左および右側冠状動脈の一方内に配置されたとき、前記主管の前記第1の端部は、それが大動脈弁を押圧するポジションまで大動脈根内で延在し、大動脈弁を開放ポジションで維持するようなものであることを特徴とするプロテーゼ。 - 前記左および右側冠状動脈の他方内に配置するための、前記主管に接続された第2の分岐管を具備してなることを特徴とする請求項31に記載のプロテーゼ。
- 前記主管と前記第1の分岐管との接続ポジションと、前記主管の前記第1の端部との間の距離は、1cmないし2cmであることを特徴とする請求項31または請求項32に記載のプロテーゼ。
- 血管の分岐領域に対してプロテーゼを送り込むためのカテーテル器具であって、前記プロテーゼは主管と一つ以上の分岐管とを具備してなり、
前記カテーテル器具は、
遠位端部および近位端部を有するコアカテーテルと、
前記プロテーゼと係合しかつそれを押しやるために、前記コアカテーテルの前記遠位端部に設けられたプッシュ要素と、
前記プロテーゼの前記一つ以上の分岐管を押しやるための一つ以上のプッシュアクチュエータと、を具備してなり、
前記アクチュエータは、前記プッシュ要素に対してスライド可能に設けられていることを特徴とするカテーテル器具。 - 前記プッシュ要素は、前記コアの延長方向と実質的に直交する方向に延在する側方領域を具備してなることを特徴とする請求項34に記載の器具。
- 前記アクチュエータは、前記プッシュ要素の前記側方領域内に設けられた孔内にスライド可能に配置されていることを特徴とする請求項35に記載の器具。
- 前記分岐領域へと前記プロテーゼを送り込む間、前記コアカテーテル、押し込み要素および前記プロテーゼの上に配置するための補強ライナースリーブを具備してなることを特徴とする請求項34ないし請求項36のいずれか1項に記載の器具。
- 前記筒状部の壁材を支持するスカフォードを具備してなり、前記スカフォードは、二つ以上のワイヤリングと、隣接するワイヤリングを連結する連結スカフォード部材と、を含み、前記連結部材は、前記プロテーゼの一方側に面して配置され、かつ、対向する側には配置されないことを特徴とする請求項1ないし請求項33のいずれか1項に記載のプロテーゼ。
