Abstract
【解決手段】本発明は、膝プロテーゼ(100)に関するものであって、人間の膝関節の構造および機能をより忠実に模擬し得ることを特徴としている。これにより、特に、制御された態様でもって脛骨の内部回転を許容することによって、より自然な態様で膝をより大きく曲げ得るという利点と;自然のスクリューホームメカニズムを模擬し得るという利点と;脛骨と大腿骨とをより自然な態様で関節結合させ得るという利点と;をもたらす。好ましい実施形態においては、本発明による膝プロテーゼは、大腿骨部材(200)と脛骨部材との間に配置されたインサート(400)を具備している。

Description
大腿骨の先端部上に適合し得るよう構成された大腿骨部材であるとともに、側方の顆構造と、中間の顆構造と、を備えているような、大腿骨部材と;
この大腿骨部材の側方顆構造に対して協働し得るよう構成された側方基端表面と、大腿骨部材の中間顆構造に対して協働し得るよう構成された中間基端表面と、を備えてなる受領構造と;
を具備している膝プロテーゼにおいて、
大腿骨部材が、側方顆構造の幾何学的構造が、中間顆構造の幾何学的構造とは、異なるものとされ、
受領構造が、側方基端表面の幾何学的構造と中間基端表面の幾何学的構造とが互いに異なるものとされ、これにより、実質的に0°という屈曲角度から実質的に130°という屈曲角度にまで膝を屈曲した際に、大腿骨部材に対して脛骨が内部回転することを補助し得るようになっているような、膝プロテーゼが提供される。
この大腿骨部材が、
大腿骨に対してのインターフェースをなす内表面を有した前方部分と、
大腿骨に対してのインターフェースをなす内表面を有した後方部分を備えた側方顆構造と、
大腿骨に対してのインターフェースをなす内表面を有した後方部分を備えた中間顆構造と、
を備え、
双方の内表面が、大腿骨の解剖学的軸に対して実質的に平行な先端並進移動を可能としつつ、大腿骨部材内に大腿骨の少なくとも一部を物理的に捕捉し得るものとされ、
大腿骨部材のすべての内表面が、設置時に後方部分回りにおいて大腿骨上において大腿骨部材を回転させたときには、大腿骨部材によって、大腿骨の切除部分を物理的に除去し得るものとされているような、膝プロテーゼが提供される。
と大腿骨との間の他の自然な関節結合を可能とする。

Claims (22)
- 膝プロテーゼであって、
大腿骨の先端部上に適合し得るよう構成された大腿骨部材であるとともに、側方の顆構造と、中間の顆構造と、を備えているような、大腿骨部材と;
この大腿骨部材の前記側方顆構造に対して協働し得るよう構成された側方基端表面と、前記大腿骨部材の前記中間顆構造に対して協働し得るよう構成された中間基端表面と、を備えてなる受領構造と;
を具備している膝プロテーゼにおいて、
前記大腿骨部材が、前記側方顆構造の幾何学的構造が、前記中間顆構造の幾何学的構造とは、異なるものとされ、
前記受領構造が、前記側方基端表面の幾何学的構造と前記中間基端表面の幾何学的構造とが互いに異なるものとされ、これにより、実質的に0°という屈曲角度から実質的に130°という屈曲角度にまで膝を屈曲した際に、前記大腿骨部材に対して脛骨が内部回転することを補助し得るようになっていることを特徴とする膝プロテーゼ。 - 請求項1記載の膝プロテーゼにおいて、
前記幾何学的構造の相違の少なくとも1つが、サイズの相違であることを特徴とする膝プロテーゼ。 - 請求項1記載の膝プロテーゼにおいて、
前記幾何学的構造の相違の少なくとも1つが、形状の相違であることを特徴とする膝プロテーゼ。 - 請求項1記載の膝プロテーゼにおいて、
前記受領構造が、大腿骨に対しての脛骨の少なくともいくらかの屈曲角度範囲において、前記大腿骨部材の前記側方顆構造に対して協働し得るよう構成された基端後方表面を備えており、これにより、大腿骨に対しての脛骨の内部回転が可能とされていることを特徴とする膝プロテーゼ。 - 請求項1記載の膝プロテーゼにおいて、
前記受領構造が、脛骨上に取り付けられた部材を備えていることを特徴とする膝プロテーゼ。 - 請求項1記載の膝プロテーゼにおいて、
前記受領構造が、脛骨インプラントの一部であることを特徴とする膝プロテーゼ。 - 請求項1記載の膝プロテーゼにおいて、
前記受領構造が、前記大腿骨部材と脛骨インプラントとの間に介装され得るよう構成されたインサートの一部であることを特徴とする膝プロテーゼ。 - 請求項4記載の膝プロテーゼにおいて、
前記基端後方表面が、凸状をなす部分を有していることを特徴とする膝プロテーゼ。 - 請求項4記載の膝プロテーゼにおいて、
前記基端後方表面が、実質的にフラットであるような部分を有していることを特徴とする膝プロテーゼ。 - 請求項4記載の膝プロテーゼにおいて、
前記基端後方表面が、実質的に凹状をなす部分を有していることを特徴とする膝プロテーゼ。 - 請求項7記載の膝プロテーゼにおいて、
さらに、脛骨インプラントを具備していることを特徴とする膝プロテーゼ。 - 請求項1記載の膝プロテーゼにおいて、
脛骨の前記内部回転が、実質的に130°以上の屈曲角度においては、大腿骨に対して実質的に8°以上であることを特徴とする膝プロテーゼ。 - 請求項1記載の膝プロテーゼにおいて、
前記受領構造が、前記大腿骨部材の前記側方顆構造に対して協働する領域においては、前記大腿骨部材の前記中間顆構造に対して協働する対応領域と比較して、より厚いことを特徴とする膝プロテーゼ。 - 請求項1記載の膝プロテーゼにおいて、
前記中間顆構造の最先端表面と前記側方顆構造の最先端表面との間を結ぶラインが、実質的に0°という屈曲角度においては、前記膝プロテーゼが埋設されている脚の機械的軸に対する垂直ラインに対して、約3°という角度をなすものとされていることを特徴とする膝プロテーゼ。 - 請求項1記載の膝プロテーゼにおいて、
前記受領構造が、基端表面上に突出部分を備え、
前記大腿骨部材が、前記突出部分の前方表面に対して協働し得る表面を有したカムを備え、
これにより、いくらかの屈曲角度範囲において、前方安定化力がもたらされていることを特徴とする膝プロテーゼ。 - 請求項1記載の膝プロテーゼにおいて、
前記受領構造が、基端表面上に突出部分を備え、
前記大腿骨部材が、前記突出部分の後方表面に対して協働し得る表面を有したカムを備え、
これにより、脛骨と大腿骨との間における50°以上という屈曲角度範囲において、大腿骨に対しての脛骨の内部回転の実行が補助されていることを特徴とする膝プロテーゼ。 - 請求項16記載の膝プロテーゼにおいて、
前記カムが、非対称な形状を有し、
これにより、前記カムの側面が、中間面よりも大きなものとされていることを特徴とする膝プロテーゼ。 - 請求項1記載の膝プロテーゼにおいて、
前記受領構造が、丸められた基端表面に対して隣接するエッジを備えていることを特徴とする膝プロテーゼ。 - 請求項1記載の膝プロテーゼにおいて、
前記受領構造と前記大腿骨部材とが、約155°という屈曲角度でもって大腿骨と脛骨とを屈曲させ得るよう構成され、
これにより、その155°という屈曲角度においては、脛骨が、大腿骨に対して実質的に8°以上という角度でもって内部回転することを特徴とする膝プロテーゼ。 - 請求項1記載の膝プロテーゼにおいて、
前記大腿骨部材の前方表面の側方部分が、前記大腿骨部材の前方表面の中間部分と比較して、より前方に延出され、
これにより、前記大腿骨部材の前記前方表面が、膝蓋骨の亜脱臼を防止し得るよう、構成されていることを特徴とする膝プロテーゼ。 - 請求項1記載の膝プロテーゼにおいて、
前記受領構造が、円弧状掃引表面を有した基端側面を備え、
前記掃引表面が、脛骨と大腿骨との間の少なくともいくらかの屈曲角度範囲において、前記基端表面上における前記大腿骨部材の側面構造の案内を補助し得るよう構成されていることを特徴とする膝プロテーゼ。 - 請求項1記載の膝プロテーゼにおいて、
前記大腿骨部材の前記中間顆構造の基端表面が、実質的に完全な伸展時においては、前記受領構造の基端表面を2mm以上超えないようにして、延出されることを特徴とする膝プロテーゼ。
