Abstract

Description
本願は、2002年6月3日に出願された米国仮出願番号60/385,274を参考として援用し、かつその優先権および利益を主張する。
本発明は、概して、心臓内欠損を処置するためのデバイスおよび関連方法に関する。より詳細には、本発明は、心臓内欠損の経皮的閉塞のための生物学的組織足場を有する心臓内閉塞器、およびその関連方法に関する。
例えば、本発明は以下の項目を提供する。
(項目1)
心臓内欠損の経皮経管処置のための心臓内閉塞器であって:
近位閉塞シェルを支持する近位支持構造体;および
遠位閉塞シェルを支持する、該近位支持構造体に連結された遠位支持構造体、

前記支持構造体のうちの少なくとも1つが、生物吸収ポリマーを含む、項目1に記載の閉塞器。

前記支持構造体のうちの少なくとも1つが、生物分解ポリマーを含む、項目1に記載の閉塞器。

前記近位支持構造体が複数の外側に伸びる近位アームを含み、前記遠位支持構造体が複数の外側に伸びる遠位アームを含む、項目1に記載の閉塞器。

患者における心臓内欠損の経皮経管処置法であって:
心臓内閉塞器を提供する工程であって、該閉塞器は以下:
近位閉塞シェルを支持する近位支持構造体;および
遠位閉塞シェルを支持する、該近位支持構造体に連結された遠位支持構造体、
を備え、該閉塞シェルのうちの少なくとも1つが、生物学的組織足場を含む、工程;
該心臓内閉塞器を該心臓内欠損付近に配置する工程;および
該心臓内欠損と該心臓内閉塞器とを係合して、該心臓内欠損を実質的に閉塞する工程、を包含する、方法。

前記心臓内欠損と係合させる工程が、前記近位閉塞シェルおよび前記遠位閉塞シェルを、該心臓内欠損の異なる側に配置することを包含する、項目8に記載の方法。
(項目10)
前記心臓内欠損が卵円孔開存である、項目8に記載の方法。
(項目11)
前記心臓内欠損が患者の心房中隔欠損である、項目8に記載の方法。
(項目12)
前記心臓内欠損が患者の心室中隔欠損である、項目8に記載の方法。

前記心臓内欠損が患者の左心耳である、項目8に記載の方法。
(項目14)
心臓内欠損の経皮経管処置のための心臓内閉塞器の製造法であって:
近位支持構造体および遠位支持構造体を含む総合支持構造体を提供する工程;
第1および第2の生物学的組織足場を提供する工程;
該第1の生物学的組織足場を該近位支持構造体に連結する工程;および
該第2の生物学的組織足場を該近位支持構造体に連結する工程、
を包含する、方法。

前記生物学的組織足場を連結する工程が、前記支持構造体に前記生物学的組織足場を縫い付けることを包含する、項目14に記載の方法。
(項目16)

本発明は、心臓内欠損(例えば、卵円孔開存、心房中隔欠損、心室中隔欠損、および左心耳)の修復のための、心臓内閉塞器を提供する。この心臓内閉塞器は、構造フレームワークおよびそれに接着した生物学的組織足場を備える。
、および遠位閉塞シェル20を支持するための遠位支持構造体34を備える。1つの実施形態において、近位支持構造体24および遠位支持構造体34の両方は、それぞれの閉塞シェル18、20を各々支持するための、外側に延びるアームを供える。例えば、図2Aに示されるように、近位支持構造体24は、4つの外側に延びるアーム26を備え、遠位支持構造体34も同様に、4つの外側に延びるアーム36を備える。1つの実施形態において、各々の外側に延びるアームは、中心点45から放射状に間隔を空けて配置された3つ以上の弾力コイル43を有する結果として、弾力により偏っている。あるいは、他の弾力性支持構造が使用され得る。1つの実施形態において、8つのアーム26、36は、ワイヤ52により機械的に一緒に固定されている。あるいは、他の手段(例えば、レーザー溶接)が、8つのアーム26、36を一緒に固定するのに使用され得る。4つのアーム26、36を示す図2Aに示される心臓内閉塞器10の断面図を、図2Bに示す。
