Abstract
Description
第1及び第2の部材は、可撓性があり、ガイドワイヤを前進させるために近位部分を遠位部分に向かって付勢するように、手動で変形させることができ、従事者は、近位および遠位部分を保持し、第3の選択的な前進技法をもたらす。
・しかしながら、貯留チューブ104と近位端部160との嵌合は、使用中、医師による取り扱いを容易にするために、図2において紙面で示される貯留チューブ104の平面が回転することを許容し、貯留チューブ104が長手軸112を中心にして容易に回転できるように、十分に緩い(ルース:loose)ものでなければならない。前記雄型接続具172は、チューブのサイズが異なれば変わるものである。雄型接続具172を貯留チューブ104内に係留するためのより高い把持強度をもたらすように、前記雄型接続具172には、また、長手軸112から離間して伸びる複数の瘤(ナッブ:nub)があってもよい。
・好ましくは、先端部120の遠位端部から近位部分108の近位端部160にまで至るアドバンサー102の全長は、約100ミリメートルである。
・イントロデューサ装置200を安定させるために、他方の手を用いて、ユーザは好ましくはイントロデューサ装置200を保持する。或る好ましい方法によればアドバンサー102の先端部120は、図5に示すように、既に患者内に挿入されているイントロデューサ装置200の近位端部に挿入される。前記好ましい方法では、前記で詳しく説明したような、貯留チューブ104及びガイドワイヤ107を有するアドバンサー102又は組立体100が、好ましくは用いられる。ガイドワイヤ107の湾曲した遠位端部107aは、遠位部分110の遠位端部118から既に伸長しており、イントロデューサ装置200の管腔202内に挿入されるに先立って真っ直ぐに矯正されなければならない。一旦、先端部107aが管腔202内に挿入されると、ユーザは、自身の親指と他の一指、好ましくは人差し指とを用いて、ワイヤ107をしっかりと掴み制御可能に送給する。このようにしてワイヤ107が中央部分154を通って移動し、図6における矢印「B」の方向へ、アドバンサー102を通ってその外へ押し出される。
・その後、ユーザは自身の親指をガイドワイヤ107から外し、ガイドワイヤ107が所望の長さにわたって前進させられるまで、そのプロセスを繰り返す。
Claims (12)
- 長手軸を有する本体部であって、
ユーザが、ガイドワイヤと直接に摩擦を伴って係合するように、親指と他の指の一つとを使って前記ガイドワイヤを前記長手軸に沿って前進させることができるようにするのに適合した第1部分と、
該第1部分に結合された第2部分であって、ユーザが、前記ガイドワイヤと直接に摩擦を伴って係合するように、親指および他の指の一方だけを使って、当該ガイドワイヤを前記長手軸に沿って前進させることができるようにするのに適合し、前記ガイドワイヤを前方へ付勢する間、親指または他の指が当該ガイドワイヤをそれに対して付勢するガイドワイヤ・プラットフォームを形成する、第2部分と、
を有する本体部を備える、ことを特徴とするガイドワイヤ・アドバンサー。 - 前記第1部分と前記第2部分は前記長手軸回りに独立して回転可能であることを特徴とする請求項1に記載のガイドワイヤ・アドバンサー。
- 第1部分と第2部分のうちでより遠位側のものは、ガイドワイヤが直接にイントロデューサ内に入って前進時に患者内に入ることができるようにするために、イントロデューサと協働するのに適合した、遠位端部に帰結する、ことを特徴とする請求項1に記載のガイドワイヤ・アドバンサー。
- 前記遠位端部はフラストコニカル状であることを特徴とする請求項1に記載のガイドワイヤ・アドバンサー。
- 前記第1部分は前記第2部分の遠位側に配置されていることを特徴とする請求項1に記載のガイドワイヤ・アドバンサー。
- 前記第1部分が、
長手方向に伸長する第1通路を有する遠位部分と、
長手方向に伸長する第2通路を有する近位部分と、
前記遠位部分と前記近位部分の間に伸長する第1及び第2部材を有する部分であって、該第1および第2部材は、両者間に、ガイドワイヤが近位部分から遠位部分に伸びるとき手動での関与のために露出される開口を形成する、中央部分と、
を備える、ことを特徴とする請求項1に記載のガイドワイヤ・アドバンサー。 - 前記遠位側の通路は、摩擦を伴って前記ガイドワイヤと係合するのに適合しており、前記第1及び第2部材の変形による前記開口部の圧縮が、前記近位部分を前記遠位部分の方へ移動させて、第1及び第2通路を通ってガイドワイヤを前進させる、
ことを特徴とする請求項6に記載のガイドワイヤ・アドバンサー。 - 前記第1及び第2部材は弓形であることを特徴とする請求項6に記載のガイドワイヤ・アドバンサー。
- 前記開口部は、親指と他の指とが、前記開口部を通って伸長するガイドワイヤを掴んで、前記第1及び第2通路に沿って前記ガイドワイヤを前進させることができるように、十分に大きい、ことを特徴とする請求項6に記載のガイドワイヤ・アドバンサー。
- 前記第2部分は、長手方向に伸長する第1通路を有する遠位部分と長手方向に伸長する第2通路を有する近位部分とを有し、前記プラットフォームは、伸長して前記近位部分と前記遠位部分とを連結し、前記長手軸の一方側に配置されている、ことを特徴とする請求項1に記載のガイドワイヤ・アドバンサー。
- 前記プラットフォームは、ガイドワイヤが配設される長手軸に整列した浅い切欠を有する隆起した凸状部を備えている、ことを特徴とする請求項1に記載のガイドワイヤ・アドバンサー。
- 請求項1から11の何れかに記載されたガイドワイヤ・アドバンサーと、
前記本体部の近位端部に連結されたガイドワイヤ貯留チューブと、
少なくとも部分的に前記ガイドワイヤ貯留チューブ内に配置されたガイドワイヤと、
を備えることを特徴とするガイドワイヤ・アドバンサー組立体。
