本願は、本願明細書に援用する2004年11月24日出願のスコット・エム・ハンプトン(Scott M. Hampton)およびアンドレアズ・レイフ(Andreas Reiff)による「EYE IMPLANT WITH MEDICINE RELEASE」という表題の米国仮特許出願第60/630,751号の利益を、米国特許法第119条(e)に準じて主張するものである。
目の多くの慢性疾患は、視力喪失または失明を含む長期にわたる損傷を引き起こすかもしれず、また引き起こす可能性がある。2つの主要な疾患カテゴリとして、非感染性慢性炎症性眼疾患と、加齢性黄斑変性症または糖尿病性網膜症などの変性血管障害とに区別することができる。最近の研究により、炎症機序が目の変性疾患に関わっていることが示唆されており[19、20、21、22、23]、したがってこれらのカテゴリは、略式というよりも描写的であり、重複した特徴を持ちうることが示唆されている。

黄斑変性症は、先進国世界の高齢者集団における最も一般的な失明原因である。黄斑変性症では、黄斑部の感光細胞が正常に機能せず、時間が経つうちに働きを停止してしまう。黄斑変性症は、60歳を超える高齢者に最も多く発生し、この場合、加齢性黄斑変性症(AMDまたはARMD)と呼ばれるが、子供を含む全年齢で発症しうる。AMDの最も一般的な初期兆候として視力がかすみ、次に、直線が波打って見えるようになり、そして最後に、視力および感色性を失うに至る。黄斑部は、中心の視野を与える網膜の一部であり、変性すると、視力が部分的に失われるか、または完全に失われる。AMD症例の約85〜90%は、ドルーゼンと呼ばれる脂肪性沈着物の黄色っぽい斑点が黄斑部上に見える乾燥型または萎縮型である。AMD症例の残りは、滲出型であり、網膜の背後の目の一部である脈絡膜内に新たに形成する血管から網膜内に漏出があるためそう呼ばれる。通常は、脈絡膜内の血管は、栄養物を網膜に届け、廃棄物を網膜から運び去る。ときには、黄斑部の下にある脈絡膜内の細かい血管が増殖し始めるが、これは、脈絡膜血管新生、またはCNVと呼ばれる過程である。原因は不明である。これらの血管は増殖すると漏出を生じ、黄斑部内の細胞は損傷を受けて死滅することがある。レーザー光凝固術は、眼科医が黄斑下新血管形成由来の漏出の治療を行うために使用する手技である。残念なことに、滲出型AMDの患者の約半分しか、レーザー光凝固術の対象とならず、レーザー光凝固術は、滲出型黄斑変性症の治療として行われるうちの約半分しか効果がない。効果がある場合、効き目は、平均して約1年間持続する。

Claims (61)
- 炎症性または変性眼疾患を治療するための、眼球内薬物送達用移植物であって、
a.第1の末端部分、反対側の第2の末端部分、外面、内面、および該第1の末端部分と該第2の末端部分との間に定義される長さLを有し、該第1の末端部分に第1の開口部、該第2の末端部分に反対側の第2の開口部を備えたキャビティを形成する、本体部分と、
b.該キャビティ内に収容される固形物であって、デポー製剤材料および有効な量の少なくとも1つの治療化合物または薬剤からなる固形物と
からなり、
該移植物が生体の目に埋め込まれると、該有効な量の少なくとも1つの治療化合物または薬剤が長期間にわたって該第1の開口部および反対側の該第2の開口部のうちの少なくとも一方を通して該移植物の周囲環境に放出されることを特徴とする移植物。 - 前記本体部分は、ポリサルフォン、ポリエーテルイミド、ポリイミド、ポリメタクリル酸メチル、シロキサン、その他のアクリレート、ポリエーテルエーテルケトン、これらの化合物の共重合体、および生体適合性移植可能ポリマーから選択された不活性ポリマー材料から作られる請求項1に記載の移植物。

- 前記可溶性結合剤物質は、デンプン、セルロース、およびキトサンを含む変性多糖類、糖類および変性糖類、例えば、トレハロース、ショ糖、ショ糖エステル、ポリアルコール、ポリ(ビニルアルコール)、グリセロール、フィブリン、フィブロネクチン、ゼラチン、コラーゲン、コラゲノイド、酒石酸塩、ジェラン・ガム、ヘパリン、カラギーナン、ペクチン、キサンタン、デキストラン、マルトデキストリン、ポリ(エチレングリコール)、ポリ(プロピレンオキサイド)、ポリ(ブチレンオキサイド)、プルロニック類、変性アルギン酸塩ヒドロゲル、カルボポール、ポリ(リシン)、タンパク質、ポリペプチド、多価電解質、プロテオグリカン、およびこれらの任意の共重合体または組合せのうち少なくとも1つからなる請求項57に記載の方法。

- 前記少なくとも1つの治療化合物または薬剤は、抗体、ナノボディ、抗体断片、シグナル伝達経路阻害剤、転写因子阻害剤、受容体拮抗薬、小分子阻害剤、オリゴヌクレオチド、融合タンパク質、ペプチド、タンパク質断片、Gタンパク質結合受容体(GPCR)などの細胞表面受容体のアロステリック調節因子、細胞表面受容体内在化誘導因子、およびGPCR逆作動薬のうちの少なくとも1つからなる請求項51に記載の方法。
