Abstract
【解決手段】導電流体(50)の存在下で一個以上の電極端子(58)に対して高周波数(RF)電気エネルギを印加し、組織構造の構造を除去、縮小または改変する。本発明の1つの側面では、患者の脊椎内におけるヘルニア様の椎間板を治療すべく椎間板組織に対して十分な電気エネルギを印加して椎間板の体積を減少させて脊髄の神経への圧力を軽減する方法が提供される。一実施例では、上記高周波電圧は、髄核の一部即ち環の外側への突出部分、又は、環内における髄の一部または全て、のいずれかを切除するに十分である。別実施例では、上記電極端子を上記環内に進入させ、上記髄核内のコラーゲン繊維を縮小または収縮させるべく高周波電圧を印加し、髄核を収縮させ脊髄の神経へ衝突しないようにする。

本出願の優先権は両者ともに1998年2 月20日に出願された米国特許出願第09/026,851号および第09/026,698号( 代理人整理番号:S-2 およびS-3)に由来するが、これらの出願は1996年7 月18日に出願された米国特許出願第08/690,159号( 代理人整理番号:16238-001610) の一部継続出願であり、それらの全ての開示内容は全ての目的のための参考として本明細書に合体される。
本発明は電気外科の分野に関し、特に、脊椎の領域における組織を治療する高周波数の電気エネルギを採用する外科治療用の装置および方法に関する。本発明は特に、ヘルニア様の椎間板(herniated discs)の治療に適している。
Claims (65)
- 患者の脊椎内のヘルニア様の椎間板の近傍に電極端子を位置決めする段階、および、
椎間板の体積を減少し脊髄神経に対する圧力を解放するのに十分な高周波数の電圧差を、前記電極端子と戻り電極との間に印加する段階を備える、患者の脊椎内のヘルニア様の椎間板を治療する方法。 - 前記ヘルニア様の椎間板の環における開口を介して前記電極端子を導入する段階、および、
前記電極端子と戻り電極の間に十分な高周波数の電圧を印加して髄核の一部を切除する、印加段階を更に備える、請求項1記載の方法。 - 前記環の外側における前記ヘルニア様の椎間板の突出部分に隣接して、前記電極端子を位置決めする段階、および、
前記電極端子と戻り電極の間に十分な高周波数の電圧を印加して前記突出部分の少なくとも一部を切除する、印加段階を更に備える、請求項1記載の方法。 - 前記ヘルニア様の椎間板の環における開口を介して前記電極端子を導入する段階、および、

- 前記電極端子と前記戻り電極との間に導電流体を供給する流体供給要素を更に備える、請求項58記載の装置。
- 前記目的部位から流体および組織断片を吸引除去する吸引除去用要素、および、
前記吸引除去用要素の末端または該末端近傍にある吸引除去用電極であって、前記吸引除去用要素内へ吸引除去される組織断片を切除する高周波数の電源に連結される吸引除去用電極を更に備える、請求項58記載の装置。 - 前記吸引除去用電極は、吸引除去内孔の遠隔の開口にまたがり延びているワイヤのメッシュを備える、請求項64記載の装置。
