Abstract
【解決手段】細胞実験研究、動物実験研究、臨床前及び臨床実験研究において、細胞親和性の非均質分子脂質はバイオ分子のミサイルとして、ターゲットの癌細胞に進入し、癌細胞のアポトーシスを促進することが可能であり、且つ癌細胞血管生成を抑制する作用と人体免疫機能を高める作用を示している。細胞親和性の非均質分子脂質は既に592 名の癌患者の治療に用いられている。その結果、患者に対し、当該薬は無毒、高い効率、高い生活品質と高い生存率の面において、有益な役割を果たしていることを実証している。治療案は局所注射、動脈点滴注射と静脈点滴注射を含み、肝臓癌、肺癌、皮膚癌、乳癌、脳腫瘍、結腸直腸癌、胃癌、頭頸部癌、白血病、悪性リンパ腫、肉腫、悪性黒色腫、骨髄腫及び転移癌等の治療に用いる。

Description
低下させ、安定性の上昇及び陰電性をもたらし、包含される薬物の漏出を促進する。それによって、保持される薬物が減少し、リポソームは結集して沈殿を生じ易くなり、毒性が現れる。従って、リポソーム膜材料として用いるレシチンが高純度で0.2 以下の酸化指数を有すると好都合である。
請求項3に記載の発明は、請求項1に記載の医薬組成物を、腫瘍を治療する薬の製造に用いることを特徴とする薬物の用途を特徴とする。
アラキドン酸 9 〜12重量%
リノレン酸 5 〜7 重量%
ドコサヘキサエン酸 12〜26重量%
エイコサペンタエン酸 8 〜14重量%
オレイン酸 30〜38重量%
パルミチン酸 8 〜15重量%
ステアリン酸 4 〜10重量%
ビタミンA 0.7 〜1.5 重量%
ビタミンD 0.3 〜1.0 重量%
ビタミンE 0.8 〜3.1 重量%
スクワレン 0.5 〜2.1 重量% 。
CHMLの作り出しは上述の成分をホモジナイズすることである。調製して二種類の医薬品:(1 )CHML 100g/殺菌注射用水2,000ml として、アンプル一本あたり2ml に100mg のCHMLを含むもの;(2 )CHML 100g/殺菌注射用水4,000ml として、アンプル一本あたり10mlに250mg のCHMLを含むもの、とする。以下のステップに基づき、二種類の薬品を調製する。
2.活性炭を加える。
3.100 ℃まで加熱し、15〜30分間沸騰させる。
上述の混合物を0.45μmの膜でろ過し、遠心して、0.22μmの膜でろ過することによって、二種類の澄み切った透明な薬品を得る。
局所注射について、腫瘍であることが明らかな患者に、35〜70mg/cm2(腫瘍面積)の局所注射で治療を行う。2 〜3 回/ 週、3 〜4 週間で1クールの治療とする。2 〜4 週間休憩後、次のクールの治療を行う。脳腫瘍患者の治療は1.4 〜1.8mg/cm3 (腫瘍体積)の局所注射を用いる。3 〜4 回/ 週、4 週間で1クールの治療とする。2 〜4 週間休憩後、次のクールの治療を行う。
腸癌の患者には、7 〜10mg/kg/回/ 日、7 回/ 週、10〜15日で1クールの治療とする。2 〜4 週間休憩後、次のクールの治療を行う。
皮膚癌、乳癌、肉腫、悪性黒色腫及びその他の可視腫瘍に対して好適な局所注射治療であって、その具体的な治療案が次の通りである。
2.メチレンブルー(methylene blue)を用いて腫瘍を0.5 ×0.5 cmに画線する。
3.CHML 100mg/ 本、CHML 250mg/ 本、2%の塩酸リドカイン5ml 、5%のブドウ糖塩化ナトリウム注射液(5%GNS)15ml を十分均等に混ぜる。
1.脳腫瘍腔内にオンマヤ(OMMAYA)リザーバを置く。
2.CHML 100mg/ 本、CHML 250mg/ 本、5%のGNS 200ml を十分均等に混ぜる。
4.毎週3 回注射する。4 週間で1クールの治療とする。2 〜4 週間休憩後、次のクールの治療を行う。腫瘍が完全に消失するまで治療を続ける。
1.血管造影法(DSA) を用いて、カテーテルを腫瘍動脈に挿入し、動脈ポンプで点滴注射する。
3.CHML混合液を毎日8 時間腫瘍動脈に点滴し、毎週7 日間注射し、25〜30日間で1クールの治療とする。2 〜4 週間休憩後、次のクールの治療を行う。
1.血管造影法(DSA) を用いて、カテーテルを腫瘍動脈に挿入し、動脈ポンプで点滴注射する。
3.CHML混合液を毎日10時間腫瘍動脈に点滴し、毎週7 日間注射する。10日間で1クールの治療とする。2 〜4 週間休憩後、次のクールの治療を行う。
1.CHML 200〜400mg/2 〜4 本、CHML 500〜1,000mg/2 〜4 本、5%のGNS 400 〜800ml を十分均等に混ぜる。
局所注射について、視覚的に腫瘍ということがわかる患者に対して、35〜70mg/cm2(腫瘍面積)の局所注射で治療を行う。2 〜3 回/ 週、3 〜4 週間で1クールの治療とする。2 〜4 週間休憩後、次のクールの治療を行う。脳腫瘍患者の治療は1.4 〜1.8mg/cm3 (腫瘍体積)の局所注射を用いる、3 〜4 回/ 週、4 週間で1クールの治療とする。2 〜4 週間休憩後、次のクールの治療を行う。
腫瘍治療の有効率(完全緩和CR+部分緩和PR)は次の通りである。肝臓癌77% 、肺癌68% 、皮膚癌94% 、乳癌83% 、脳腫78% 、結腸直腸癌80% 、胃癌50% 、頭頸部癌78% 、白血病83% 、悪性リンパ腫71% 、肉腫43% 、悪性黒色腫67% 、及び骨髄腫50% 。II級又はII級以上の毒性反応・副作用はなかった。実証によれば、CHMLは有効であり、患者の耐性が良く、II級又はII級以上の毒性反応・副作用が生じなかった。

材料及び方法
1.CHML薬品:医薬品の臨床試験の実施の基準(GCP)[米国食品医薬品監督管理局(FDA )文書、企業向けガイダンス、E6 Good Clinical Practice: Consolidate Guidanc
e, 1996 ]に基づき、米国のグローリー食品薬品社(Glory F & D Co Ltd)より次のロットの薬品9709147 、9803077 、9907077 、20000707、20010707及び20020707が提供された。CHMLの全ての成分は植物及び動物から抽出され、バイオ脂質技術によって調製された。
2.1 患者の資料:癌のステージ分類標準は国際対がん連合(UICC)TNM 悪性腫瘍ステージ分類、世界保健機関(WHO )脳腫瘍ステージ分類、及びフランス〜アメリカ〜イギリス(FAB )白血病ステージ分類に基づく。全ての患者の情報は表1を参照。患者の選択の標準は以下の通りである。
C. 生存見込み時間≧3 ヵ月。
D. 年齢:成年、14〜81歳。
2.3 患者は必ず以下の倫理規定を理解しなければならない。つまり、患者は、他の腫瘍専門家の評価により「がんが既に末期であり、その患者に対する現時点での有効な治療方法がない」と告知された者を除き、臨床試験に参加している腫瘍病院で利用可能かつ現在利用可能な方法論で為されうる他の有効な治療を受けることができる。。
2.5 投与方法及び使用量。
A. 局所注射。皮膚癌、乳癌、肉腫、悪性黒色腫及びその他の可視腫瘍に対する治療に用いられ、その具体的な治療案は以下の通りである。

d. 毎週の月、水、金曜にそれぞれ1回、週に3 回注射する。4 週間で1クールの治療とする。2 〜4 週間休憩後、次のクールの治療を行う。腫瘍が完全に消失するまで治療を続ける。

2.6 治療効果の評価はWHO がん治療結果報告標準に基づく。
部分緩和(PR):腫瘍病巣最大の垂直両径の積の縮小は50% 以上、又は肝肥大の単一な径の縮小は少なくとも30% 及び新病巣無し、且つ4 週間以上維持する。
安定(NC):腫瘍の縮小は25% 以下、又は腫瘍治療開始後その増大が25% より小さい。
2.7 毒性反応及び検査、米国国立がん研究所(NCI)毒性反応・副作用報告指針に基づき、全ての患者に以下の検査を行わなければならない。
E. 呼吸器系検査:毎週1回異常肺症状(BRMP)と肺機能(BRMP)を治療完了後1 週間まで検査する。もし正常ならば、2 週間に1回前述検査を行い、3 ヵ月間追跡調査する。
1.治療効果について、計1,424 クールの治療を行った。352 名の患者に2 クールの治療を行った。240 名の患者に3 クールの治療を行った。各患者に対する6 ヵ月間の追跡調査観察については表3 〜15を参照されたい。患者に対する治療の有効率は表2 を参照し、詳細は表3 〜15を参照されたい。異なるタイプの癌患者に対する治療の有効率は以下の通りである。肝臓癌は135 名で77% 、肺癌は102 名で68% 、皮膚癌は67名で94% 、乳癌は65名で83% 、脳腫瘍は65名で78% 、結腸直腸癌は61名で80% 、胃癌は30名で50% 、頭頸部癌は23名で78% 、白血病は18名で83% 、悪性リンパ腫は14名で71% 、肉腫は7 名で43% 、悪性黒色腫は3 名で67% 、及び骨髄腫は2 名で50% となっている。

Claims (5)
- 9 〜12重量% のアラキドン酸と、5 〜7 重量% のリノレン酸と、12〜26重量% のドコサヘキサエン酸と、8 〜14重量% のエイコサペンタエン酸と、28〜38重量% のオレイン酸と、8 〜15重量% のパルミチン酸と、4 〜10重量% のステアリン酸と、0.7 〜1.5 重量% のビタミンA と、0.3 〜1.0 重量% のビタミンD と、0.8 〜3.1 重量% のビタミンE と、0.5 〜2.1 重量% のスクワレンとからなる医薬組成物。
- 前記各組成成分が、自然界の動物と植物から抽出されて得られることを特徴とする請求項1に記載の医薬組成物。
- 請求項1に記載の医薬組成物を、腫瘍を治療する薬の製造に用いることを特徴とする薬物の用途。
- 前記腫瘍は肝臓癌、肺癌、皮膚癌、乳癌、脳腫瘍、結腸直腸癌、胃癌、頭頸部癌、白血病、悪性リンパ腫、肉腫、悪性黒色腫、骨髄腫又は転移癌から選ばれることを特徴とする請求項3に記載の用途。
- 前記薬物のタイプは局所注射剤タイプ、動脈点滴注射剤タイプ又は静脈点滴注射剤タイプから選ばれることを特徴とする請求項3に記載の用途。
