イコデキストリン中のグルコース分子の大多数は、α(1−4)グルコシド結合によって直線的に結合されており(>90%)、一方小分画(<10%)はα(1−6)結合で結合されている。イコデキストリンの分子量の分配を、ゲル透過クロマトグラフィーによって行った。イコデキストリン中のα(1→6)グルコシド結合およびα(1→4)グルコシド結合の分布を、核磁気共鳴分光法によって評価した。揮発性および半揮発性の有機不純物を高速液体クロマトグラフィーおよび質量分析計によって試験した。

カイコ幼虫血漿(SLP)試験(Wako Pure Chemical Industries,Ltd.,Osaka,Japan)によってペプチドグリカン(PG)定量を行った。例えば、Tsuchiya M,Asahi N,Suzouki F:Detection of peptidoglycan and B−glucan with silkworm larvae plasma test,FEMS Immunol Medical Micrbiol 1996;15:129−34;および米国特許第4,970,152号を参照のこと。SLPは、プロフェノールオキシダーゼ(PPO)カスケード(昆虫の自己防衛機構)のあらゆる要因を含む。PPOカスケードは、ペプチドグリカンによって開始され、ここでPPOは、最終的に、フェノールオキシダーゼへと活性化される。このフェノールオキシダーゼ活性は、3,4−ジヒドロキシフェニルアラニンを基質として用いて、比色定量測定によって検出される。イコデキストリン溶液中のペプチドグリカンの検出限界は、7.4ng/mLであることが見出された。SLP試験は、エンドトキシンを検出しない。
イコデキストリン中のペプチドグリカンについて、効果が観察されないレベル(NOEL)を、ラットモデルにおいて決定した。全部で45匹の雄のSprague−Dawleyラット(Harlan Inc;Indianapolis,IN,USA)、(体重255〜280g)を、9つの等しい群に分けた。各群は、0ng/mL、1ng/mL、5ng/mL、10ng/mL、50ng/mL、100ng/mL、500ng/mL、1000ng/mLまたは5000ng/mLのペプチドグリカンを加えた(spike)、35mL/kgの用量のイコデキストリンの単回腹腔内注射を受けた。Staphylococcus aureus(Toxin Technology Inc.,Sarasota,FL,USA)由来のペプチドグリカンを、これらの実験において使用した。注射6時間後、ラットを屠殺し、腹腔内の流体を、重量および体積を決定することによって定量的に回収した。腹水を有核細胞数および分画(differential count)、総タンパク質、およびIL−6濃度およびTNF−α濃度について分析した。
2000年7月と2002年9月との間に製造されたイコデキストリンの数百のバッチに関する販売後の監視データを分析し、ペプチドグリカンレベルと無菌性腹膜炎の発生との間の関連を決定した。百万あたりの病訴(complaints per million(CPM))を効果的に示すのに十分な病訴頻度を蓄積するために、データを対数スケールでのペプチドグリカンレベル、すなわち、≦7.4ng/mL、>7.4〜15ng/mL、>15〜30ng/mL、>30〜60ng/mL、および≧60ng/mLへと分類した。各ペプチドグリカン濃度範囲に対して、全病訴と全販売単位とを計算した。販売単位CPMを、全病訴÷全販売単位×100万として計算した。負の二項回帰を使用して、CPMとペプチドグリカンレベルとの間の関連を評価した。SASプロシージャーGENMOD (SAS Institute,Cary,NC,USA)を使用して、統計分析を行った。
2000年にBAXTER HEALTHCARE CORPORATIONの世界的ファーマコビジランスシステムを通じて受けた報告によると、無菌性腹膜炎の頻度[(病訴数÷処置患者数)×100]は、0.095%であった。2001年における報告症例において、2002年3月における1.04%のピーク頻度への定常的な増加が存在した。患者は、見かけ上、稀に熱性もしくは中毒性であった;腹痛は軽度〜なしであった;透析液は曇っており、かつ細胞性であり、透析液細胞数は、300/mm3から3500/mm3まで変動した(種々のパーセンテージの好中球、リンパ球、およびマクロファージを有し、好塩基球を有さなかった)。細菌性腹膜炎と異なり、低分子溶質についての限外濾過または腹膜透過性における変化はなかった。血液および透析液培養物は、常に陰性であった。抗生物質は、その地方の医師によって様々に実施された。あらゆる臨床徴候は、イコデキストリンを中止することで解消した。いくつかの症例では、イコデキストリンを再開し、そして無菌性腹膜炎が再発した。BAXTER HEALTHCARE CORPORATIONは、2002年5月に、数百バッチのイコデキストリンの自発的な全世界的リコールを開始した。
リコールされたイコデキストリンバッチの大規模な化学的研究および物理的研究を行った。これらの分析は、無菌性腹膜炎に関連するバッチと有害な臨床事象に関連しないバッチとの間での、イコデキストリンの分子量の分布、グルコシド結合の分岐のパーセントにおける何らかの差異を明らかにせず、また揮発性および半揮発性の有機不純物を追跡しなかった。イコデキストリン含有透析液の病訴バッチの全ての溶液構成要素は、製品の仕様の範囲内であり、かつ現行の薬局方標準を満たした。
コントロール排液と比較して、無菌性腹膜炎を有する患者からの透析液排液において、IL−6濃度の明らかな上昇が観察された(それぞれ、>5000pg/mL 対 59pg/mL)。コントロールと比較して、タンパク質濃度の上昇が病訴サンプルにおいて示された(それぞれ、236mg/dL 対 125 mg/dL)。病訴サンプルとコントロールサンプルとの間で、イコデキストリンとその代謝産物(2〜7の重合化の程度を有するグルコースポリマー)における差はなかった。これらの結果は、イコデキストリンもその代謝産物も、おそらく無菌性腹膜炎の原因ではなかったことを示した。
無菌性腹膜炎に関連する全てのイコデキストリンサンプルにおけるエンドトキシンレベルは、LAL試験によって決定される場合、製品の限度の範囲内(<0.25EU/mL)であることが見出された。ウサギパイロジェン試験において、病訴イコデキストリンバッチもしくは非病訴イコデキストリンバッチのいずれにも、温度上昇はなかった。しかし、インビトロPBMCアッセイにおけるIL−6応答の増加は、以下の表1に示されるように、病訴イコデキストリン含有透析液バッチにも、腹膜透析液の病訴バッチを製造するために使用されるイコデキストリン原材料にも観察された。
数百のリコールされたイコデキストリンのバッチの分析は、41%のバッチがSLPアッセイによって検出されたペプチドグリカンによって汚染されていたことを示した。ペプチドグリカン濃度は、検出限界の7.4ng/mLから303ng/mLの範囲に及んだ。マルトデキストリンからのイコデキストリンの製造は、加熱および酸性化を必要とするので、選好性の生物の存在に関する微生物学的研究を行った。イコデキストリンの初期工程が、好熱性、好酸性のグラム陽性生物であるAlicyclobacillus acidocaldariusで汚染されることが見出された。Alicyclobacillusは、汚染性ペプチドグリカンの源であった。最終に近い生成物に対して適用された加熱および滅菌濾過手順は、細菌を消失させたが、ペプチドグリカン汚染物は消失しなかった。イコデキストリン中のペプチドグリカンレベルとPBMCアッセイにおいて観察されたIL−6応答との間に正の相関が見出された(図1を参照のこと)。ドナー間でIL−6応答にかなりの変動性が見られ、このことは、ペプチドグリカンに対する感受性の範囲を示唆する。
NOELを確立するために、ラットにおいてペプチドグリカンの腹腔内投与の効果を調べた。これは、イコデキストリン製造のための規制的仕様を確立するために使用され得る。総タンパク質、白血球および好中球は、ペプチドグリカンを含まないイコデキストリンを受容したコントロールラットと比較して、イコデキストリン+ペプチドグリカン処置ラットの腹水において上昇した。腹水における好中球の浸潤は、100ng/mLにおいてNOELを有し、用量依存性の増加を示した(図2を参照のこと)。炎症性サイトカインは、TNF−αに関して10ng/mLにおいてNOEL値を有し(図3を参照のこと)、そしてIL−6に関して100ng/mLにおいてNOELを有し(図4を参照のこと)、どちらも用量依存性の増加を示した。ペプチドグリカンに対する最低のNOELは、10ng/mLであったTNF−α応答に対してであった。
以下の表2に示されるように、イコデキストリン中のペプチドグリカン濃度とCPMとの間に、正の相関を見出した。
2002年5月において、BAXTER HEALTHCARE CORPORATIONは、>10ng/mLの濃度でペプチドグリカンに汚染されたイコデキストリンの全てのバッチをリコールし、そしてそのリコールの時点でペプチドグリカンについてアッセイしていなかった全てのロットをリコールした。さらに、アッセイの検出限界(<7.4ng/mL)より下のペプチドグリカンについての内部の規制的仕様を、製品の発売に対して実施した。ペプチドグリカンおよび好熱性細菌に対する日常的な連続的モニタリングを実施した。製造された最近のバッチ全てにおけるペプチドグリカン濃度は、SLP試験の検出限界より下であり、そしてPBMCアッセイによってIL−6誘発性因子を欠いている。
Claims (31)
- 腹膜透析溶液を製造するための方法であって、該方法は、以下の工程:
グルコースポリマーを提供する工程;
該グルコースポリマーに試薬を添加する工程であって、該試薬は、ペプチドグリカンと反応し得る、工程;
該ペプチドグリカンの量を決定する工程;および
該ペプチドグリカンが十分低いレベルで存在することが決定される場合、該グルコースポリマーを使用して該腹膜透析溶液を作製する工程
を包含する、方法。 - 前記反応が、セリンプロテアーゼカスケードを開始させる、請求項1に記載の方法。
- 前記セリンプロテアーゼカスケードが、プロフェノールオキシダーゼカスケードを含む、請求項2に記載の方法。
- 前記試薬が、カイコ幼虫血漿に由来する、請求項1に記載の方法。
- 前記ペプチドグリカンの量が、前記ペプチドグリカンと前記試薬との間の反応に対する比色定量測定によってさらに決定される、請求項1に記載の方法。
- 前記十分低いレベルのペプチドグリカンが、約10ng/mL未満である、請求項1に記載の方法。
- 前記グルコースポリマーベースの溶液が、イコデキストリンを含有する、請求項1に記載の方法。
- 前記試薬が、前記グルコースポリマーベースの溶液に添加される、請求項7に記載の方法。
- 請求項1に記載の方法であって、前記ペプチドグリカンが前記十分低いレベルで存在していないことが決定される場合、該ペプチドグリカンを除去して、該ペプチドグリカンの十分低いレベルを提供する工程をさらに包含する、方法。
- 前記グルコースポリマーがイコデキストリンを含む、請求項1に記載の方法。
- 患者に腹膜透析を提供する方法であって、該方法は、以下の工程:
腹膜透析溶液が十分低いレベルのペプチドグリカンを有することを保証するための試薬を利用して、該腹膜透析溶液を調製し、該患者において腹膜炎を防止する工程;および
該患者に該腹膜透析溶液を提供する工程
を包含する、方法。 - 前記ペプチドグリカンの十分低いレベルが、約10ng/mL未満を含む、請求項11に記載の方法。
- 前記腹膜透析溶液が、グルコースポリマーベースの溶液を含む、請求項11に記載の方法。
- 前記グルコースポリマーベースの溶液が、イコデキストリンを含有する、請求項13に記載の方法。
- 前記腹膜透析が、自動腹膜透析および連続携行式腹膜灌流からなる群より選択される、請求項11に記載の方法。
- 前記患者が、腹膜透析の間、腹膜炎に関してモニタリングされる、請求項11に記載の方法。
- 腹膜炎の発症に相関のあるIL−6応答を判定するために、透析排液が前記患者から回収される、請求項16に記載の方法。
- 請求項11に記載の方法であって、腹膜透析の間に使用する前の前記腹膜透析溶液中において前記ペプチドグリカンの量が約10ng/mLを超えるか否かを決定するために、前記試薬が使用される、方法。
- 前記試薬が、カイコ幼虫血漿に由来する、請求項18に記載の方法。
- 腹膜炎を引き起こすのに十分なレベルを超えるグラム陽性生物の存在に関して、腹膜透析溶液を試験する方法であって、該方法は、以下の工程:
該腹膜透析溶液に試薬を添加する工程であって、該試薬は、ペプチドグリカンと反応してセリンプロテアーゼカスケードを開始させ得る、工程;および
該ペプチドグリカンの量を決定する工程
を包含する、方法。 - 前記セリンプロテアーゼカスケードが、プロフェノールオキシダーゼカスケードを含む、請求項20に記載の方法。
- 前記試薬が、カイコ幼虫血漿に由来する、請求項21に記載の方法。
- 前記腹膜透析溶液が、グルコースポリマーベースの溶液を含む、請求項20に記載の方法。
- 前記グルコースポリマーベースの溶液が、イコデキストリンを含有する、請求項23に記載の方法。
- 前記試薬が、前記グルコースポリマーベースの溶液を作製するために使用される原材料形態のグルコースポリマーに添加される、請求項23に記載の方法。
- 前記グルコースポリマーベースの溶液が、約10ng/mLを超える前記ペプチドグリカンの量に関して試験される、請求項23に記載の方法。
- ペプチドグリカンと反応し得る試薬を含有する、グルコースポリマー組成物。
- 前記試薬が、前記ペプチドグリカンと反応してセリンプロテアーゼカスケードを開始させ得る、請求項27に記載のグルコースポリマー組成物。
- 前記セリンプロテアーゼカスケードが、プロフェノールオキシダーゼカスケードを含む、請求項28に記載のグルコースポリマー組成物。
- 前記試薬が、カイコ幼虫血漿に由来する、請求項27に記載のグルコースポリマー組成物。
