Abstract
Description
(1.発明の分野)

本発明は、血管系における狭窄領域の拡張のための改善された装置および方法を提供する。その狭窄領域は、しばしば、繊維性の、石灰化された、またはさもなければ硬化したプラークもしくは従来の血管形成術バルーンを使用して拡張することが困難であり得る型の他の狭窄物質の領域を含む。その方法および装置は、しばしば、その動脈血管系、特に冠状動脈血管系の処置においてそれらの最大の用途を見出すが、静脈系および/または末梢血管系の処置、ステント挿入されていない小血管および/または血管分枝の処置、口病変部の処置、ならびにISRの処置においてもまた用途を見出し得る。

Claims (49)
- 掻き取りカテーテルであって、以下:
近位端および遠位端を有するカテーテル本体;
該カテーテル本体の遠位端の近くで半径方向に拡張可能なシェル;ならびに
該シェル上に保持される、非軸方向掻き取り構造体、
を備える、掻き取りカテーテル。 - 前記掻き取り構造体が、少なくとも1つの連続掻き取り要素を備え、該連続掻き取り要素は、前記シェルの周囲の境界を定め、そして少なくとも2cmの長さを有する、請求項1に記載のカテーテル。
- 前記掻き取り構造体が、2cm以下の長さを有する複数のセグメントを備える、請求項1に記載のカテーテル。
- 前記掻き取り構造体の少なくとも一部分が、前記半径方向に拡張可能なシェル構造体の周囲の境界を定める、請求項1〜3のいずれか1項に記載のカテーテル。
- 前記掻き取り構造体の少なくとも一部分が、前記シェルの上で螺旋状に配置されている、請求項4に記載のカテーテル。
- 前記掻き取り構造体の少なくとも一部分が、前記シェルの上で蛇行パターンで配置されている、請求項4に記載のカテーテル。
- 前記掻き取り構造体の少なくとも一部分が、前記シェルの周りを囲んで配置されている、請求項4に記載のカテーテル。
- 前記拡張可能なシェルが、拡張可能な領域を有し、そして前記掻き取り構造体が、該拡張可能な領域の百分率の20%未満を覆う、請求項1に記載のカテーテル。
- 前記百分率が、1%〜5%の範囲である、請求項8に記載のカテーテル。
- 前記掻き取り構造体の少なくとも一部分が、ワイヤを備える、請求項1に記載のカテーテル。
- 前記掻き取り構造体が、前記シェルの外側表面に固定されるか、または該外側表面内に包埋される、請求項1に記載のカテーテル。
- 前記掻き取り構造体が、前記シェルの周囲の境界を少なくとも部分的に定めるケージ構造体内に組み込まれる、請求項1に記載のカテーテル。
- 前記ケージ構造体が、少なくとも1つの点において、前記カテーテル本体に直接取り付けられる、請求項12に記載のカテーテル。
- 前記ケージ構造体が、前記シェルに取り付けられない、請求項13に記載のカテーテル。
- 前記ケージ構造体が、前記シェルの一端の近くにおいて、前記カテーテル本体に取り付けられ、そして該シェルの他端において、軸方向に自由にスライドする、請求項14に記載のカテーテル。
- 前記ケージ構造体が、弾性であり、そして前記拡張可能なシェルが潰された場合に、該シェルの上で半径方向に閉じられるように配置されている、請求項12に記載のカテーテル。
- 前記ケージの少なくとも一部分が、超弾性材料から構成される、請求項15に記載のカテーテル。
- 前記シェルおよび前記掻き取り構造体のアセンブリが、冠状血管を通して進められる場合にねじれることなく90°の角度での屈曲を可能にするために十分に可撓性である、請求項1に記載のカテーテル。
- 前記非軸方向掻き取り構造体が、1つ以上の掻き取り要素を備え、該掻き取り要素の累積長さの70%未満が、拡張される場合に、前記シェル上で軸方向に整列する、請求項1に記載のカテーテル。
