Abstract
ロチゴチンと、濃度1〜140ミリモル/Lの少なくとも1種の塩化物塩とを含む、pH4〜6.5の組成物を、パーキンソン病治療用のイオン導入装置製造に使用することにより、従来の受動拡散系で得られるよりも高いヒトの角質層を介するロチゴチン流量を得ることが可能になった。
Description
本発明は、ドパミン受容体アゴニスト、ロチゴチン(rotigotine,INN)のイオン導入デリバリーを使用するパーキンソン病の症候群を治療または軽減する効果的な方法に関する。
パーキンソン病は、主として黒質のドパミン作用性ニューロンの変性により惹起されると考えられている。要するに、これは尾状核における緊張性ドパミン分泌とニューロン活性のドパミン関連適応能の喪失をもたらし、かくして脳のある領域においてドパミンの欠乏をきたす。このようにして生じた神経伝達物質であるアセチルコリンとドパミンの不均衡は結局、疾患関連症候群をもたらす。通常、運動系障害と見なされるが、現在、パーキンソン病は、運動系および非運動系の両方を含めたより複雑な障害と考えられている。この、人を衰弱させる病気は、振戦、運動緩慢、硬直、運動異常、歩行障害および言語障害を含む主な臨床的特色によって特徴づけられる。患者によっては、これらの症候に痴呆を伴うことがある。自律神経系のかかわり合いでは、起立性低血圧、発作性潮紅、熱調節の問題、便秘、膀胱および括約筋制御の喪失を生じる。パーキンソン病には、動因喪失や抑鬱のような心理学的障害も伴うことがある。

パーキンソン病の初期運動欠損は黒質ドパミン放出細胞の変性の始まりで突き止めることができる。このニューロンの変性は、黒質を線条体と連結するドパミン作用性経路における欠陥を生じる。病気の進行と共に、難治性運動、自律性、精神的異常が進行しえ、これは線条体の受容体メカニズムに進行性変性のあることを示している。

さらに、近年利用されているパーキンソン病療法の主な欠点は、「変動症候群(fluctuation syndrome)」の究極的発症であり、運動異常となる運動性のオン(on)期間と運動低下または運動不能のオフ(off)期間が交互することにより特徴づけられるオール・オア・ノン(all or none)症状をもたらす。経口抗パーキンソン療法で予測できない、または不規則な「オン−オフ」現象を示す患者がL−ドパおよび他のドパミンアゴニストの静脈内投与に対して予測できる有益な応答を有することから、薬剤の血漿濃度における変動が「オン−オフ」現象の原因であることが示唆される。ドパミン受容体アゴニストであるアポモルフィン(apomorhine)およびリスライド(lisuride)の連続注入(infusion)により「オン−オフ」変動の頻度も改善される。しかし、この投与法は不都合である。したがって、より一定した血漿レベルを与える、局所投与のような他の投与法が有益であり、過去に示唆されている。

を有する化合物、(−)−5,6,7,8−テトラヒドロ−6−[プロピル−[2−(2−チエニル)エチル]アミノ]−1−ナフタレノールの国際一般的名称[International Non-Proprietary Name (INN)]である。従前、受動経皮療法系(passive transdermal therapeutic system (TTS))によりロチゴチンを投与することが知られていた。このようなロチゴチン投与用の受動経皮療法系は、例えば、特許文献1および2に記載されている。しかし、これらの受動経皮療法系で得られるロチゴチンの流量(flux)は必ずしも全ての患者に満足されるものではない。
パーキンソン病の治療に使用されてきたもう1つのドパミンアゴニストはR−アポモルフィンである。R−アポモルフィンは、構造式:
を有する化合物、(R)−5,6,6(a),7−テトラヒドロ−6−メチル−4H−ジベンゾキノリン−11,12−ジオールの国際一般的名称(INN)である。
R−アポモルフィンのイオン導入投与システム開発のいくつかのアプローチが従来から記載されている(例えば、非特許文献1および2参照)。しかし、これらの努力にもかかわらず、1.4〜10.7ng/mLの治療濃度範囲の下限濃度のみが得られただけである。
さらなるドパミンアゴニストは、ロピニロール(ropinirole)である。ロピニロール(INN)は、構造式:
を有する(4−[2−ジプロピルアミノ)エチル]−1,3−ジヒドロ−2H−インドール−2−オン)である。ロピニロールのイオン導入投与は可能と考えられていたが、治療範囲の下限の流量のみを得ることが可能であった(非特許文献3参照)。
多くの患者は、アポモルフィンまたはロピニロールのイオン導入デリバリーを用いて可能な濃度より著しく高い濃度を必要としている。

R−アポモルフィンのイオン導入投与システム開発のいくつかのアプローチが従来から記載されている(例えば、非特許文献1および2参照)。しかし、これらの努力にもかかわらず、1.4〜10.7ng/mLの治療濃度範囲の下限濃度のみが得られただけである。
さらなるドパミンアゴニストは、ロピニロール(ropinirole)である。ロピニロール(INN)は、構造式:
多くの患者は、アポモルフィンまたはロピニロールのイオン導入デリバリーを用いて可能な濃度より著しく高い濃度を必要としている。
パーキンソン病の広範な症候群と重篤度の差異に鑑み、パーキンソン病患者の皮膚を介するロチゴチン流量を調整し、かつドパミン受容体の一定な受容体刺激を可能にする方法が強く要望されている。好ましくは、そのようなシステムは、受動経皮デリバリー系により達成されるよりも高いロチゴチン流量を可能にするものである。
アポモルフィンのイオン導入デリバリーでの落胆させられた経験からすると、ロチゴチンのイオン導入デリバリーが、従来の受動拡散系よりも高いばかりでなく、実際に医薬上有効な薬剤用量のデリバリーを可能にするロチゴチンの血漿レベルを提供できたことは驚くべきことである。本発明を用いて得られた結果は、パーキンソン病の効果的な治療が提供できることを合理的に期待させる。本明細書における「治療」なる用語は、完全治癒に導く本当の原因治療よりも、パーキンソン病の症候群の治療または軽減を意味する。

本発明は、ロチゴチンと、濃度1〜140ミリモル/Lの少なくとも1種の塩化物塩とを含む、pH4〜6.5の組成物の、パーキンソン病治療用のイオン導入装置製造のための使用を提供するものである。
イオン導入法は電気的手段により種々のイオンを皮膚内に導入することである。受動経皮デリバリーと比較すると、イオン導入法は、パーキンソン病治療に有用ないくつかの利点を提供する。
− イオン導入法は、電流の調整により流量を必要な治療速度にプログラムすることができる。
− イオン導入法は、必要に応じて、イオン導入デリバリー・システムをオンまたはオフにすることにより、簡単に、迅速に薬剤の投与を開始または終了することができる。
− イオン導入法は、電流の調整により流量を必要な治療速度にプログラムすることができる。

イオン導入流量はいくつかのパラメーターに影響されるので、最適な流量を達成するためには、これらのパラメーターを別々に最適化することが重要である。
驚くべきことに、pH4〜6.5の、ロチゴチンと、濃度1〜140ミリモル/Lの少なくとも1種の塩化物塩とを含む組成物をイオン導入装置のドナー・チャンバーで用いると、治療範囲に十分はいる流量が達成できることを見出した。
ドナー・コンパートメントにおける電解質濃度を減じることにより、低い電流密度において目的のイオン導入流量を達成するか、単位面積当たりの経皮用量の増加が可能となる。
ロチゴチンのイオン導入デリバリーの実施可能性を評価する研究の間に、pHが増加するとロチゴチンの溶解度が低下することが判明した。しかし、驚くべきことに、治療的に適正な速度がpH4〜6.5の間で、非常に低いロチゴチン濃度において達成されることが判明した。
ヒトの角質層を介する最適な流量を提供するためには、ドナー相における電極反応用の十分なCl−イオンの濃度を与えることも必要である。しかし、塩化物塩の添加は電極反応を維持するが、ロチゴチンの溶解度を減じる。かくして、1〜140ミリモル/L、好ましくは50〜100ミリモル/L、さらに好ましくは60〜80ミリモル/Lの塩化物塩濃度が最適である。
驚くべきことに、pH4〜6.5の、ロチゴチンと、濃度1〜140ミリモル/Lの少なくとも1種の塩化物塩とを含む組成物をイオン導入装置のドナー・チャンバーで用いると、治療範囲に十分はいる流量が達成できることを見出した。

ロチゴチンの濃度は、患者の要求、パーキンソン病治療における治療効果を得るのに要求される流量によって変動しうる。しかし、最適な治療には、好ましくは少なくとも0.5mg/mL、さらに好ましくは0.5〜3mg/mLである。
本発明の組成物においては、医薬上許容される全ての塩化物塩を使用できる。本発明の好ましい具体例において、塩化物塩はNaCl、塩化トリエチルアンモニウムおよび塩化トリブチルアンモニウムから選択される。ロチゴチンのより高い流量をもたらすので、塩化トリエチルアンモニウムおよび塩化トリブチルアンモニウムが特に好ましい。
本発明の特に好ましい具体例において、イオン導入装置のドナー相に使用する組成物は、0.5〜3mg/mLのロチゴチンと、60〜80ミリモル/Lの濃度の、塩化トリエチルアンモニウムおよび塩化トリブチルアンモニウムの少なくとも1種を含んでなり、ドナー相はpH4.5〜5.5を有する。
本発明の組成物においては、医薬上許容される全ての塩化物塩を使用できる。本発明の好ましい具体例において、塩化物塩はNaCl、塩化トリエチルアンモニウムおよび塩化トリブチルアンモニウムから選択される。ロチゴチンのより高い流量をもたらすので、塩化トリエチルアンモニウムおよび塩化トリブチルアンモニウムが特に好ましい。
本発明の特に好ましい具体例において、イオン導入装置のドナー相に使用する組成物は、0.5〜3mg/mLのロチゴチンと、60〜80ミリモル/Lの濃度の、塩化トリエチルアンモニウムおよび塩化トリブチルアンモニウムの少なくとも1種を含んでなり、ドナー相はpH4.5〜5.5を有する。
本発明の他の態様においては、ロチゴチンと、濃度1〜140ミリモル/Lの少なくとも1種の塩化物塩とを含む、pH4〜6.5の組成物のパーキンソン病治療用のイオン導入装置を、必要とする患者の皮膚に適用することを特徴とするパーキンソン病の治療方法が提供される。
本発明においては、従来のいずれものイオン導入装置が使用できる。そのようなイオン導入装置は、例えば、V. Nair, O. Pillai, R. Poduri, R. Panchagnula, "経皮イオン導入療法、パートI:基礎的原理および考察", Methods Find. Exp. Clin. Pharmacol. (1999), 21(2), 139-151に記載されている。
イオン導入の間に使用する電流密度は患者の要求によって変動し、用いるイオン導入装置および組成物に依存する。適当な電流は付き添う医師が決定しうる。一般に、適当な電流密度は、好ましくは200〜500μA/cm2である。
本発明においては、従来のいずれものイオン導入装置が使用できる。そのようなイオン導入装置は、例えば、V. Nair, O. Pillai, R. Poduri, R. Panchagnula, "経皮イオン導入療法、パートI:基礎的原理および考察", Methods Find. Exp. Clin. Pharmacol. (1999), 21(2), 139-151に記載されている。

R. van der Geestら(R. van der Geest, M. Danhof, H.E. Bodde, "輸送セルを介する新しいイオン導入連続流の確認およびテスト", J. Control. Release (1998), 51, 85-91)記載のスリー・チャンバー・フロー−スルー拡散セル(three-chamber flow-through diffusion cells)を用いてロチゴチン投与のイン・ビトロ・イオン導入研究を行った。アクセプター・コンパートメントの両側にヒト角質層(SC)を位置させた。5,000Daのカット・オフを有する透析膜を支持膜として使用した。各外側チャンバーの容量は約2mLで、アクセプター・コンパートメントの容量は0.54mLであった。2つの外側チャンバーは銀プレート(アノード)または銀/塩化銀(カソード)ドライバー電子を有していた。ドナー相は5mMクエン酸緩衝液(2.1mMクエン酸ナトリウム2水和物および2.9mMクエン酸)で緩衝されたロチゴチン溶液からなっていた。
この構成を用い、ドナー・チャンバーのpH5、電流密度500μA/cm2、アクセプター・チャンバーのpH7.4、温度20℃、ドナー・チャンバーのNaCl濃度70ミリモル/Lで、ドナー相における種々の薬剤濃度でのロチゴチン流量を測定した。

Claims (9)
- ロチゴチンと、濃度1〜140ミリモル/Lの少なくとも1種の塩化物塩とを含む、pH4〜6.5の組成物の、パーキンソン病治療用のイオン導入装置製造のための使用。
- ロチゴチンの濃度が、少なくとも0.5mg/mLである請求項1記載の使用。
- ロチゴチンの濃度が、0.5〜3mg/mLである請求項1または2記載の使用。
- 塩化物塩が、NaCl、塩化トリエチルアンモニウムおよび塩化トリブチルアンモニウムから選ばれる請求項1〜3いずれか1項記載の使用。
- 塩化物塩が、塩化トリエチルアンモニウムまたは塩化トリブチルアンモニウムである請求項4記載の使用。
- 塩化物塩の濃度が、60〜80ミリモル/Lである請求項1〜5いずれか1項記載の使用。
- 組成物が、イオン導入装置のドナー相に使用される請求項1〜6いずれか1項記載の使用。
- イオン導入装置のドナー相における組成物が、ロチゴチン0.5〜3mg/mLと、塩化トリエチルアンモニウムおよび塩化トリブチルアンモニウムの少なくとも1種60〜80ミリモル/Lとを含み、ドナー相のpHが、4.5〜5.5である請求項1〜7いずれか1項記載の使用。
- ロチゴチンと、濃度1〜140ミリモル/Lの少なくとも1種の塩化物塩とを含む、pH4〜6.5の組成物のパーキンソン病治療用のイオン導入装置を、必要とする患者の皮膚に適用することを特徴とするパーキンソン病の治療方法。
