Description
本発明は、多様な疾患の治療の為のインシュリン様成長因子(IGFs)のあるアゴニストの使用に関する。
インシュリン様成長因子I及びII(各々,IGF−I及びIGF−II)はインビボにおいて、細胞増殖,細胞分化,細胞死の阻害,及びインシュリン様活性を含む多数の作用を媒介する(Clark 及びRobinson, Cytokine Growth Factor Rev., 7: 65-80(1996); Jones 及びClemmons, Endocr. Rev., 16: 3-34(1995))。これらの分裂促進及び代謝応答の殆どは、インシュリン受容体と密接に関連しているα2β2異種四量体であるIGF−I受容体の活性化により始まる(McInnes 及びSykes, Biopoly., 43:339-366(1998); Ullrich ら,EMBO J., 5:2503-2512(1986))。両タンパク質は、チロシンキナーゼスーパーファミリーの一員であり、共通の細胞内シグナルカスケードを共有する(Jones 及びClemmons, 同上)。IGF−インシュリンハイブリッド受容体は単離されているが、それらの機能は不明である。IGF−Iとインシュリン受容体は,特定のリガンドとナノモルレベルの親和力で結合する。IGF−Iとインシュリンは、親和力が100−1000倍低いにもかかわらず,各々の非同族受容体と交差反応が可能である(Jones 及びClemmons, 同上)。IGF−I受容体の細胞外部分の一部を表す結晶構造が、最近、報告されている(Garrett 等., Nature, 394: 395-399(1998))。
さらに、(1−27、gly4、38−70)−hIGF−Iと称される変異体は、ヒトIGF−IのC領域の残基28−37が4残基グリシン架橋によって置換されており、IGFBPと結合するがIGFレセプターとは結合しないことが発見されている(Bar 等., Endocrinology, 127: 3243- 3245(1990))。
さらに、Cascieri 等., Biochemistry, 27: 3229-3233(1988)は、3個が1型IGF受容体への親和力が低下していた、IGF−Iの4個の変異体について開示した。これらの変異体は:(Phe23、Phe24、Tyr25)IGF−I(1、2型IGF及びインシュリン受容体への親和力がヒトIGF−Iと同等)、(Leu24)IGF−I及び(Ser24)IGF−I(ヒト胎盤1型IGF受容体、胎盤インシュリン受容体、及びラット及びマウス細胞の1型IGF受容体への親和力がIGF−Iよりも低い)、そしてdesoctapeptide(Leu24)IGF−I(位置24の芳香族性の消失がhIGF−Iのカルボキシ末端のD領域の欠損と連結している)。これら4個の変異体は、ヒト血清結合タンパク質へ標準的な親和力を有している。
これらの成果にもかかわらず、IGF−IのIGFBP結合エピトープの概論は、統一的ではなく、解像度が低い。これまでの研究は、殆どが相同的なインシュリンの領域をIGF−I或いは切断タンパク質(例、des(1−3)−IGF−I)挿入することに関したのもで、ミスフォールディングと真の結合決定基に起因される作用を区別するものではなかった。すべてのこれらの研究結果を結びつけることは、放射線標識リガンド結合分析からバイオセンサー分析の範囲の異なった技術を使用してIGF変異体とIGFBPの複合体を分析するという事実から、さらに複雑なことである。
さらに、IGF−Iを末期の慢性腎疾患の状況へ投与すると、腎臓の機能が数日間に渡って増強することが可能である。末期の慢性腎疾患が、透析又は移植のみで治療することが可能な状況であること、そしてその発生が急増していることから、これは重要である。糖尿病患者と高齢者は、この状況を有する傾向にある。およそ60%末期慢性腎疾患は患者血液透析、約10%は腹膜透析、そして残りの約30%が移植を受ける。毎年、アメリカでは、50,000人以上の患者が透析治療を始める。毎年、さらなる末期腎疾患になった25%の患者が透析を受けることを拒否する。これら透析を受けている患者の看護の費用は、現在、平均して一月当たり2億ドルである。さらに、患者は透析によって支障のある生活様式を見せる。IGF−Iが末期慢性腎疾患を体験している方々の腎臓機能を増強することが可能であるにもかかわらず、IGF−Iの短期投与によって誘導された腎糸球体の濾過速度と腎血漿の流量の増加は長期投与の間は持続せず、副作用の発生率は高い(Miller等., Kidney International, 46: 201-207(1994))。
ある環境下において、上昇した循環IGF−Iのレベルが、腎臓機能の長期に渡る変化に関連している又は直接の原因であることは知られている。例えば、末端肥大症患者の循環GHとIGF−Iの上昇を伴うインシュリンとPAHのクリアランスの増大は、一年に渡って持続する(Kkos等., Acta Endocrinol., 21: 226-236(1956))。クレアチンのクリアランスの増大が、GH非感受性のラロン型小人萎縮症へIGF−Iを投与した最初の12日間に起こった。この増大は、次の59日間に渡って進行性であった(Walker等., J.Pediatr., 121: 641-646(1992))。
慢性腎疾患などの慢性疾患の治療の為の断続的なIGF−Iの投与は、U.S. Pat. Nos. 5,565,428及び5,741,776に開示されている。
腎臓に対するIGF−Iの影響についての完全な考察としては、例えば、Hammerman及びMiller, Am. J. Physiol., 265:F1-F14(1993)及びHammerman及びMiller, J.Am.Soc.Nephrol., 5: 1-11(1994)を参照のこと。
従って、一実施態様では、 本発明はIGF−I変異体を提供し、それは天然配列ヒト型IGF−Iの位置16、25、又は49のアミノ酸残基、 又は位置3及び49のアミノ酸残基がアラニン、グリシン、又はセリン残基へ置換されている。
また、ここに提供されているのは、容器、好ましくは製薬的に許容可能な容器に含まれる変異体を含む組成物である。好ましくは、この組成物は無菌である。
さらに、ここに提供されるのは、上記に記載した変異体を哺乳類へ投与することを含む哺乳類のGH/IGF軸の調節不全を特徴とする疾患の治療方法である。哺乳類は、好ましくはヒトで、疾患は好ましくは腎疾患、さらに好ましくは腎不全である。
また、ここで考察されているのは、ここで取り上げられるペプチドと哺乳類のGH/IGF軸の調節不全を特徴とする疾患の治療の為に組成物を使用する使用者への説明書を含む、製薬的に許容される組成物を含有する容器を含むキットである。疾患が腎疾患である場合は、このキットは任意的にさらに腎臓活性分子を含有する容器を含む。
ここに示すペプチドの同定の為に、ヒトIGF−Iは一価の繊維状ファージミド粒子に表示され(US Pat. Nos. 5,750, 373 及び 5,821,047)、その完全なアラニンスキャンニング突然変異誘発(Cunningham及びWells, Sience, 244: 1081-1085 (1989); US Pat No. 5,834,250)をファージディスプレイ法によりおこなった(「ダーボ−アラスキャン」)(Cunningham等., EMBO J. 13: 2508-2515(1994); Lowman, Methods Mol. Biol., 87: 249-264(1998))。IGF−ファージミド変異体は、IGF−IにあるIGFBP−1及びIGFBP−3の為の結合決定基の地図を描く為に使用された。IGFBP−1或いはIGFBP−3に特異的に結合してインビボにおけるこれらタンパク質のクリアランス半減期の調節し、タンパク質分解安定性の向上させ、又は組織分布の変更をする結合タンパク質特異的IGF変異体を発生する為に、アラニンスキャンニングは結合タンパク質の為の特異的決定基を明らかにする。さらに、多様なIGF−結合抗体、又は他のペプチドやタンパク質でIGF−Iと結合するもののエピトープを地図化することは有益であると言える。
A.定義
ここで使用される治療目的の為の“哺乳類”とは、哺乳類に分類されるすべての動物に関してであり、ヒト、家畜、及び飼育動物、そして動物園、スポーツ用、又はペット動物、例えば犬、馬、猫、羊、豚、牛などを含む。好ましい哺乳類はヒトである。“大人ではない”という用語は、哺乳類において出生時(低体重乳児を含む)から思春期を含み、後者はまだ潜在成長力に達していない場合に相当する。
“ペプチド”とは、少なくとも2個のアミノ酸を有するIGF−Iアゴニスト/IGF−II変異体を含み、少なくとも50個のアミノ酸を有するポリペプチドを含む。その定義は、ペプチド誘導体、それらの塩類、又は光学異性体を含む。
ここで使用される“腎臓活性分子”は、電解質の再吸収及び保持を促進、又は他の点では腎疾患を治療する為に作用する。例は下記に提供される。
ここの本発明は、一つの側面において、野生型ヒトIGF−Iの位置16、25、又は49のアミノ酸、或いは天然配列ヒトIGF−Iの位置3及び49がアラニン、グリシン、及び/又はセリン残基と置換されたことに関する。好ましくは、当該の1個又は2個のアミノ酸がアラニン又はグリシン残基、最も好ましくはアラニンと置換される。
精製IGF及びそのアミノ酸配列から、例えば組み替えDNA技術によってIGF親分子のペプチジル変異体であるIGF変異体の生成が可能である。これらの技術は、簡略化された形で、天然又は合成であろうと、ペプチドをコードする遺伝子を取り出す;遺伝子を適切なベクターへ挿入する;ベクターを適切な宿主へ挿入する;遺伝子発現の為に宿主を培養する;それによって生産されたそしてペプチドを回収又は単離すことを意図する。好ましくは、回収されたペプチドは、その後、相応する程度まで精製される。
そして、親DNAは、宿主細胞の形質転換に使用される適切なプラスミド又はベクターへ挿入される。一般には、宿主との関連において、宿主と適合する種由来の複製及びコントロール配列を含むプラスミドベクターが使用される。ベクターは、通常、形質転換細胞の形質選択を提供できるタンパク質又はペプチドをコードする配列と同様に、複製部位を持つ。
例えば、大腸菌は、大腸菌由来のプラスミドであるpBR322を使用することによって形質転換される(Mandel等., J.Mol.Biol.53:154(1970))。プラスミドは、アンピシリン及びテトラサイクリン耐性遺伝子を含み、それ故に選択が容易な方法を提供する。他のベクターは、発現にしばしば重要な異なったプロモーターのような、異なった特徴を含む。例えば、プラスミドpKK223−3、pDR720、及びpPL−lambdaは、tac、trp、又はPLプロモーターを持つ、程なく入手可能な発現ベクターである(Pharmacia biotechnology)。
他の好ましいベクターは、前記に示したベクターの適切な特徴を組み合わせることによる標準的技術を使用することで構築が可能である。適切な特徴とは、プロモーター、リボゾーム結合部位、decorsin又はornatin遺伝子、或いは遺伝子融合(プロテインAのZドメイン及びdecorsin又はornatinとそのリンカー)、抗生物質耐性マーカー、及び適切な複製開始点を含む。
原核生物に加えて、酵母培養菌、又は多細胞生物由来の細胞のような真核生物が使用することができる。基本的には、そのような全ての細胞が機能する。しかし、関心は脊椎動物細胞についてがもっとも大きく、培養(組織培養)での脊椎動物細胞の増殖は再現性のある方法になっている(Tissue Culture, Academic Press, Kruse and Patterson, editors(1973))。このような有用な宿主株化細胞は、VERO及びHeLa細胞、チャイニーズハムスター卵巣(CHO)由来の株化細胞、W138,293,BHK、COS−7、及びMDCK株化細胞である。
遺伝子融合は、必須ではないが、後のこれら遺伝子産物の精製のみならず大腸菌における異種ペプチドの発現を容易にする(Harris, Genetic Engineering, Willianson, R., Ed.(Academic Press, London, Vol.4, 1983), p.127; Ljunguist等., Eur.J.Biochem., 186: 557-561(1989)及びLjungquist等., Eur.J.Biochem., 186: 563-569(1989))。プロテインA融合は、プロテインAの結合、より詳しくはプロテインAのZドメインのIgGへの結合が融合タンパク質の精製の為の「親和性取っ手」を提供することから、頻繁に使用される。多くの異種タンパク質が大腸菌内で直接に発現されると分解されることが知られているが、融合タンパク質として発現された場合は安定である(Marston, Biochem J., 240: 1(1986))。
もう一つの方法としては、融合タンパク質のタンパク質分解による切断を使用することが可能である(Cater, Protein Purification: From Molecular Mechanism to Large-Scale Process, Ladish等., eds.(American Chemical Society Symposium Series No.427,1990), Ch13, page 181-193)。
第Xa因子、トロンビン、及びサブチリシン又はその変異体、他の多数のプロテアーゼが、融合タンパク質の切断に首尾よく使用されている。典型的には、プロテアーゼによる切断を受けやすいペプチドリンカーは、「他の」のタンパク質(例えばプロテインAのZドメイン)と所望するペプチドとの間に挿入される。組み替えDNA技術を使用すると、リンカーをコードするヌクレオチド塩基対は、他のタンパク質をコードする遺伝子又は遺伝子断片の間に挿入される。タンパク質分解による正確なリンカーを含む部分精製融合タンパク質の切断は、その結果、天然融合タンパク質、又は還元型或いは変性融合タンパク質についておこなうことができる。
変性及び再折り畳みが必要な場合は、通常は、ペプチドは塩酸グアニジンのようなカオトロピック剤によって処理され、その後、例えばペプチドが天然構造へと再折り畳むような適切な割合、pH、及び温度において、還元及び酸化型ジチオスレイトール又はグルタチオンを含有する酸化還元緩衝液によって処理される。
その他のカップリング法は、N,N'−ジシクロヘキシルカルボジイミド又はN,N'−ジイソプロピルカルボジイミドなどの適したカップリング剤を使用することを含む。他のカップリング剤で、当該技術に熟練している者に明らかなものは、E.Gross & J.Meienhofer, The Peptide: Analysis, Structure, Biology, Vol.I: Major Method of Peptide Bond Formation(Academic Press, New York, 1979)に記載されている。
α−アミノ保護基は(a)カップリング反応が用いられている条件下においては、α−アミノ機能を不活性にする、(b)側鎖保護基を除かない及びペプチド断片の構造を改変しない条件下においては、カップリング反応後は容易に除去可能でなけれならない、(c)カップリング直前の活性化においては、ラセミ化の可能性を除去しなければならない。側鎖保護基は(a)カップリング反応が用いられている条件下においては、側鎖機能基を不活性にする、(b)α−アミノ保護基を除去の際に用いられている条件下では、安定でなけれならない、(c)ペプチド鎖の構造を改変しない条件下では、所望のアミノ酸ペプチドの完成時には容易に除去可能でなければならない。
有用なアミノ酸保護基の種類は:
(2)Lysに存在する側鎖アミノ基については、保護は、BOC、p-クロロベンジルオキシカルボニル等といった上記(1)に述べた任意の基によってなされる。
(3)Argのグアニジド基については、保護は、ニトロ、トシル、CBZ、アダマンチルオキシカルボニル、2,2,5,7,8-ペンタメチルクロマン-6-スルホニル又は2,3,6-トリメチル-4-メトキシフェニルスルホニル、又はBOCによってなされる。
(4)Ser、Thr、又はTyrのヒドロキシル基については、保護は、例えば、Cl−C4アルキル、例えばt-ブチル;ベンジル(BZL);置換BZL、例えばp-メトキシベンジル、p-ニトロベンジル、p-クロロベンジル、o-クロロベンジル、及び2,6-ジクロロベンジル。
(5)Asp又はGluのカルボキシル基については、保護は、例えば、BZL、t-ブチル、シクロへキシル、シクロペンチルなどの基を用いたエステル化によってなされる。
(6)Hisのイミダゾール窒素については、トシル部が好ましく使用される。
(7)Tyrのフェノール性ヒドロキシル基については、テトラヒドロピラニル、tert-ブチル、トリチル、BZL、クロロベンジル、4-ブロモベンジル、又は2,6-ジクロロベンジルなどの保護基が好ましく使用される。好ましい保護基は2,6-ジクロロベンジルである。
(8)Asn又はGlnの側鎖アミノ基については、キサンチル(Xan)が好ましく用いられる。
(9)Metについては、アミノ酸は好ましくは保護せずに残す。
(10)Cysのチオ基については、p-メトキシベンジルが典型的に用いられる。
各保護アミノ酸又はアミノ酸配列は固相反応器に過剰に導入し、ジメチルホルムアミド(DMF)又はCH2Cl2又はそれらの混合物の媒体中でカップリングを適切に実施させる。不完全なカップリングが生じた場合、次のアミノ酸のカップリングに先立つN-アミノ保護基を除去する前にカップリング方法を繰り返す。合成の各段階でのカップリング反応の成功を監視してもよい。合成を監視する好ましい方法は、Kaiser等, Anal. Biochem., 34: 595 (1970)に記載されたニンヒドリン反応である。カップリング反応は、周知の方法、例えばBIOSEARCH 9500(商品名)ペプチド合成機を使用して自動的に実施できる。
保護基を除去することなくペプチドを切断することが望まれる場合、保護ペプチド−樹脂にメタノール分解を施して、C-末端カルボキシル基がメチル化された保護ペプチドを生成させることができる。このメチルエステルは、次いで、温和なアルカリ条件下で遊離のC-末端カルボキシル基を与える。次いで、ペプチド鎖上の保護基を液体フッ化水素などの強酸で処理することにより除去する。メタノール分解についての特に有用な技術は、Moore等, Peptides, Proc. Fifth Amer. Pept. Symp., M. Goodman及びJ. Meinhofer, 編, (John Wiley, N.Y., 1977), p. 518-521のものであり、そこでは保護ペプチド−樹脂がクラウンエーテルの存在下でメタノール及びシアン化カリウムで処理される。
また、N-末端α-アミノ基に存在する保護基も、保護ペプチドが支持体から切断される前又は後に優先的に除去してよいと認められる。
本発明のポリペプチドの精製は、典型的には従来の手法、例えば分離用HPLC(逆相HPLCを含む)又は他の周知のクロマトグラフィ技術、例えばゲル浸透、イオン交換、分配クロマトグラフィ、アフィニティクロマトグラフィ(モノクローナル抗体カラムを含む)、又は向流分配によって達成される。
本発明のペプチドは、環化によって立体配置的に安定化させてもよい。ペプチドは通常、一のペプチドの−及びC-末端ドメインを本発明の他のペプチドの対応するドメインに共有結合させることにより環化され、2又はそれ以上の反復ペプチド配列を含み、各反復ペプチドが実質的に同じ配列を有するシクロ-オリゴマーを形成する。さらに、環化したペプチド(シクロ-オリゴマー又はシクロ-モノマーのいずれか)は、架橋して1−3の環状構造を形成し、その中には2から6のペプチドが含まれる。ペプチドは好ましくはα-アミノ及び主鎖カルボキシル基(頭部から尾部)を介して共有結合せず、−及びC-末端ドメインに位置する残基の側鎖を介して架橋する。よって結合部位は一般に残基の側鎖間になるであろう。
Glu及びLys側鎖も、環状又は二環状ペプチドの調製において架橋され;このペプチドはp-メチルベンズヒドリルアミン樹脂上の固相化学によって合成される。ペプチドは樹脂から切断されて脱保護される。環状ペプチドは、希釈メチルホルムアミド中のジフェニルホスホリルアジドを用いて形成される。これに代わる方法については、Schiller等, Peptide Protein Res., 25: 171-177 (1985)を参照のこと。米国特許第4,547,489号参照。
望ましい環状又は重合性ポリペプチドは、ゲル濾過に次ぐ逆相高圧液体クロマトグラフィ又は他の従来の手法により精製される。ペプチドは滅菌濾過され、従来からの薬理学的に許容される媒体中に処方される。
4つの異なる置換基に結合した炭素原子から生成されたペプチドが不斉の場合、ペプチドはジアステレオマー、エナンチオマー又はそれらの混合物として存在しうる。上記の合成は、出発材料又は中間体としてラセミ化合物、エナンチオマー又はジアステレオマーを用いてもよい。そのような合成から得られるジアステレオマー生成物は、クロマトグラフィ又は結晶化法によって分離しうる。同様に、エナンチオマー混合物は、同様の技術又はこの分野で知られた他の技術によって分離してよい。各不斉炭素原子は、存在する場合は、2つの立体配置R又はSの一方でよく、それらともに本発明の範囲内である。
治療に使用されるペプチドは定式化され、そして良い医療診断、患者個人の臨床状況(特にペプチドによる治療の副作用)、疾患の種類、ペプチドの到達部位、投与方法、投与予定、及び開業医が把握している他の要因を考慮することと一致する方法で投薬される。従って、ここにおける目的の為のペプチドの有効量は、このような考慮によって定められ、量は哺乳類と所望する効果にとって薬の生物学的利用能とならなければならない。
一般的な考え方としては、一服の非経口投与された薬学的に有効なペプチドの量は、用量反応曲線によって測定可能な範囲にある。例えば、IGFBPに結合したIGF又は血中IGFは、服用を決めるための治療を受けている哺乳類の体液によって測定が可能である。代替方法としては、増加するペプチドを患者へ投与し、IGF−I及びIGF−IIのために患者の血清レベルを調べることができる。下記の実施例3にある、ヒト血清中のIGFBPをIGF−Iトレーサーに置換することを参照のこと。
特に、ペプチドの適切な服用を決める一つの方法は、体又は血中などの生物的体液中のIGFレベルの測定を伴う。それらを測定する方法は、RIA及びELISAを含むすべての方法によっておこなうことが可能である。IGFレベルを測定した後、単回投与又は複数回投与の使用により体液はペプチドと接触する。この接触段階の後、IGFレベルは体液中で再測定される。体液中のIGFレベルが所望する効力が生じるための十分な量によって低下した場合、その場合は、その分子の服用は最高の効力が生じるように調製可能である。この方法は、インビトロ又はインビボで実行できる。好ましくは、この方法は、インビボで実行される、すなわち体液が哺乳類から抽出され、IGFレベルが測定された後、ここでのポリペプチドは単回又は複数回投与の使用によって投与され(すなわち、接触回数は哺乳類への投与によって達成される)、そしてIGFレベルは、哺乳類から抽出された体液から測定される。
服用を決めるもう一つの方法は、血中の“遊離”又は活性IGFのレベルを測定することであろう。ある用途においては、“遊離”IGFのレベルは、効力及び有効服用量又は服用の適切な指標である。
(a)体液を1)ペプチドの存在下において、IGF結合部位がIGF結合タンパク質との結合に利用可能な状態でペプチド上に残存するような固相担体へ付着したポリペプチド(例えば、ペプチド上のエピトープの第一抗体)に対して特異的なペプチドを検出する媒介と接触させ、その結果によって媒介とIGF結合タンパク質間に複合体を形成する;及び2)一定の期間中に、IGF結合タンパク質上の全ての利用可能なIGF結合部位を飽和するの十分なペプチドと接触させ、その結果によって飽和複合体を形成する;
(b)ペプチドがIGF結合タンパク質と結合している時に、結合に利用可能なIGF結合タンパク質(IGFBP上のエピトープに特異的な第二抗体のような)へ特異的な検出可能なラベルされた第二の媒介と飽和複合体を接触させる;及び
(c)生物体液中のIGFBPの指標として結合しているラベル化媒介の総量、及びそれから体液中に存在する結合したペプチドの総量、IGF結合タンパク質、結合したIGF,又は活性IGFの指標として結合しているラベル化媒介の総量の数量的な分析。
前記の方法により投薬量を決定すると、一般的に、使用するペプチドの総量の見積もりが可能である、すなわち、前記に記したように、この方法はかなりの治療上の自由裁量が前提としているが、患者の体重のkgに基づいて、約10μg/kg/日から200μg/kg/日が使用可能である。例えば、慢性腎疾患の治療においては、一日の投与量は、好ましくは約10から160μg/kg、より好ましくは20から100μg/kg、そして最も好ましくは約25から75μg/kgである。
ペプチドは、徐放性システムによって適正に投与される。徐放性組成物の適正な例は、造形品の形の半浸透性ポリマー基質、例えばフイルム、又はマイクロカプセルを含む。半浸透性ポリマー基質は、ポリ乳酸(U.S. Pat. No. 3,773,919. EP58, 481)、L-グルタミン酸とγ-エチル-L-グルタミン酸の共重合体(Sidman等., Biopolymers, 22,547-556(1983))、ポリ(2-ヒドロキシエチルメタクリル酸)(Langer等., J.Biomed.Mater.Res., 15:167-277(1981), 及びLanger, Chem.Tech., 12: 98-105(1982))、エチレンビニル酢酸(Langer等., 同上)又はポリ-D-(-)-3-ヒドロキシ酪酸(EP 133,988)を含む。徐放性組成物は、また、リポソーム包含ペプチドを含む。ペプチドを包含するリポソームは、それ自体で知られている方法で調整される:DE 3,218,121;Epstein等., Pro. Natl. Acad. Sci. U.S.A., 82: 3688-3692(1985);Hwang等., Proc. Natl. Acad. Sci. U.S.A., 77: 4030-4034(1980);EP 52,322;EP 36,676;EP 88, 046;EP 143,949;EP 142,641;Japanese Pat. Appln. 83-118008;U.S. Pat. Nos. 4,485,045 及び4,544,545;及びEP 102,324。通常、リポソームは脂質含有量が約30モルパーセントコレステロールを越える小さな単層形(約200から800オームストロング)で、その選択された部分は最も効果的な治療のために調節されている。
非経口投与のためには、一実施態様は、ペプチドは、一般的に、製薬的、或いは非経口的に許容可能な担体、すなわち使用投薬量及び濃度が投与対象者にとって無毒、そして製剤中の他の成分と共存可能な担体と供に所望する純度でそれぞれを単位投与量の注射可能な形態(溶液、懸濁液、又は乳濁液)の中で混合することによって製剤化される。例えば、製剤は、好ましくは酸化剤、及びポリペプチドに対して有害だと知られている他のペプチドを含まない。
一般的に、製剤は、ペプチドを均等且つ密接に液体担体又は細粒固体担体、或いは両方と接触させることによって調製される。そして、必要ならば、産物は所望の製剤形態へ成形される。好ましくは、担体は非経口担体で、さらに好ましくは、溶液は、受給者の血液と等張である。そのような担体媒介物の例としては、水、生理食塩水、リンガー液、緩衝化された溶液、デキストロース液を含む。非溶液媒介物、不揮発性油及びオレイン酸エチルなどもここでは有用である。
ペプチドは、一般的に、これら媒介物中でpH4.5又はおよそ4.5から8で製剤化される。前記の賦形剤、担体、又は安定化剤のいくつかの使用が、塩類やペプチドを形成する結果となることは理解される。最終調製は、安定な液体又は凍結乾燥固形物となる。
治療投与に使用されるペプチドは、無菌でなければならない。無菌性は、無菌濾過膜(例えば、0.2ミクロンの膜)による濾過によって容易に達成する。一般的に、治療組成物は、例えば静脈注射用の溶液袋、又は皮下注射針により穴をあけることが可能な栓を有するバイアルなど、無菌引き込み口を有する容器へ配置される。
ここにおけるペプチドと、血中IGFを増加させる又はペプチドの効果を促進する一つ或いはさらに他の適正な試薬による併用療法も、本発明の一部である。一般的に、これらの試薬は、ここにおけるペプチドが生成IGFを放出することを可能にする。例えば、ペプチドとの組み合わせでは、他の活性分子を投与することが望ましい。例えば、消耗又は異化状態のためには、ペプチドはMEGASETMのような食欲促進剤と共に投与することが可能である。
その上、ペプチドは適正に投与され、投与のための受容体又は抗体或いは抗体断片と結合する。
さらなる併用療法は、ヒト成長ホルモン、IGFBP−3、IGFBP−5のような成長ホルモンを含む。
腎疾患では、ペプチドは、例えば心房性ナトリウム利尿ペプチド(ANP)、ANP類似体、又は受容体活性を有する或いは有しないそれの全ての変異体、urodilatin、ヒトB型ナトリウム利尿性ペプチド(BNP)、アンジオテンシン受容体アンタゴニスト、バソプレシン及びその類似体のような電解質の再吸収と保持を促進する腎活性分子、及び抗体又はペプチドアンタゴニストのようなエンドセリンアンタゴニストとともに適切に投与される。一例は、BQ−123(Ihara等., Life Science, 50: 247-250(1992); JP 51-94254A 1993年8月3日公開; Webb等., Biochem. Biophys. Res. Comm., 185: 887-892(1992))で、エンドセリンA受容体の潜在的及び特異的遮断剤であり、エンドセリン−1、CT−1ではなく、マウスLIF、又はフェニレフリンによって誘導される肥大性活性のみを遮断する環状ペンタペプチドである。その他の例は、Ihara等., Biochem. Biophys. Res. Comm., 178: 132-137(1991)によって記載されているBQ−123の親化合物である。さらなる例は、EP 647,236;EP647,449;EP633,259(phenyl-sulfonyl-amino-pyrimidine derivaties);EP601,386(sulfonamide compounds);U.S. Pat. No.5,292,740(phenylsulfonylamidopyrimidines);及びU.S. Pat.No. 5,270,313(phenyl-sulfonyl-aminopyrimidine derivaties)に記載のものを含む。その上、アンジオテンシン変換酵素(ACE)阻害剤は、腎疾患のIGF−I治療に関連して有益である。
ここにおける発明は、ペプチドをコードする核酸を使用する哺乳類のための遺伝子治療を使用することも意図している。一般的には、遺伝子治療は、哺乳類のIGFレベルを増加(過剰発現)させるために使用される。ペプチドをコードする核酸は、この目的のために使用される。一度、アミノ酸配列が明らかになると、遺伝子コードの縮重を使用することによって幾つかの核酸分子を作ること、及びその中から遺伝子治療ヘ使用するものを選択することが可能である。
遺伝子治療の目的の為に、核酸(選択的にベクターに含まれる)を患者の細胞へ導入する二つの主要な方法がある:インビボ及びエキソビボである。インビボデリバリーにおいては、普通は、核酸は直接にプチドを必要とする患者の部位ヘ注入される。エキソビボの治療においては、患者の細胞が取り出され、核酸はこれら単離された細胞へ導入され、そして改良された細胞は患者へ直接又は、例えば、患者へ移植される多孔質膜によってカプセル化される。U.S. Patent Nos. 4,892,538 及び5,283,187を参照のこと。
最近、好まれるインビボの核酸転移技術は、ウイルスベクターによるトランスフェクション(アデノウイルス、,単純疱疹Iウイルス、又はアデノ随伴ウイルスなど)及び脂質を基礎とするシステム(遺伝子の脂質媒介転移に有用な脂質は、DOTMA、DOPE、及びDC−Chol、など)である。ある状況では、細胞表層膜又は標的細胞ヘ特異的な抗体、標的細胞上の受容体へのリガンドなどの標的細胞を標的とする薬剤を核酸のソースへ供給することが望ましい。リポソームが使用された場合は、エンドサイトーシス(飲食作用)と関連している細胞表層膜タンパク質と結合するタンパク質は、標的化及び/又は取り込みを容易にすることに使用される、例えば、キャプシドタンパク質又はそれの断片、特定の細胞の向性、サイクリングで内部移行を受けるタンパク質への抗体、及び細胞内局在化を標的とし、細胞内半減期を増進するタンパク質。受容体媒介エンドサイトーシスの技術は、例えば、Wu等., J.Biol.Chem., 262:4429-4432(1987);及びWanger等., Proc.Natl.Acad.Sci.USA, 87:3410-3414(1990)に記載されている。最近、知られている遺伝子標識及び遺伝子治療の手順は、Anderson等., Science,256:808-813(1992)を参照のこと。WO93/25673及びそれに引用されている参考文献も参照のこと。
本発明は、下記の実施例の参照によってさらに十分に理解できる。しかし、それらは、本発明の範囲を制限するように解釈されるべきではない。ここにおいて言及された全ての文献及び特許の引用文は、参考文献によって明白に含まれている。
IGF−I及び構造変異体のアラニン−スキャンニング突然変異誘発
序:
アラニン−スキャンニング突然変異誘発法(上掲のCunningham及びWells)を用いてIGF−Iの各側鎖のβ炭素を越えた部分を取り除いた。ついでIGFBP−1又はIGFBP−3に対するペプチドの結合の自由エネルギーへのこれらの原子の寄与を競合ファージELISA法によって検査した。このアッセイでは、IGFBP−1又はIGFBP−3を用いてIGF−ファージ変異体がIGFBP−1又はIGFBP−3被覆免疫吸着プレートへ結合するのを阻害する。結合タンパク質の滴定列から結合性(IC50)を計算することができる。幾つかの変異体はまたBIACORETMアッセイにおける直接結合性についても調べた。
実施例では、通常のαアミノ酸は、中間体と最終生成物を指すとき、標準的な一文字又は三文字アミノ酸コードによって記述される。通常のαアミノ酸とはmRNAの指示下でタンパク質内に導入されるアミノ酸を意味する。標準的な略語はメルクインデクス、10版、pp Misc-2−Misc-3に列挙されている。特に断らない限り、通常のαアミノ酸はα炭素原子において自然のもしくは「L」立体配置を有している。「D」がコードの先にくる場合は、これは通常のαアミノ酸とは反対の鏡像異性体を表している。ノルロイシン(Nle)及びオルニチン(Orn)のような改変された又は普通でないαアミノ酸は米国特許商標庁オフィシャルガゼット1114TMOG、1990年5月15日に記載されているように示した。以下に記載するIGF変異体を用いた実験の結果に基づいて、ここに請求されたタイプの分子が治療される患者における活性IGF量を増加させるであろうと予測される。
ファージミドベクターの作成と突然変異誘発
成熟ヒトIGF−Iをコードする遺伝子を、PCRプライマー5'-AGCTGCTTTGATATGCATCTCCCGAAACTCTGTGCGGT-3'(配列番号:4)及び5'-GAGCGATCTGGGTCTAGACAGATTTAGCGGGTTTCAG-3'(配列番号:5)を用いてpBKIGF2B(米国特許第5342763号)から増幅した。得られた断片をNsiI及びXbaIで切断し、先にNsiI及びXbaIで消化させたpH0753中に結合させた。pH0753はphGHam−g3の派生体(Lowman等, Biochemistry, 30: 10832-10838 (1991))であり、アルカリホスファターゼプロモータ(PhoA)の更なるXbaI部位がオリゴヌクレオチド5'-AAAAGGGTATGTAGAGGTTGAGGT-3'(配列番号:6)を用いて欠失させられている。IGF−1オープンリーディングフレームを含む連結ベクターpH0753は、pIGF−g3と命名した。これは、大腸菌バクテリオファージM13由来の遺伝子IIIタンパク質の断片(残基249−406)に融合した二重突然変異G1S−A70Vを有するIGF−1をコードしている。このIGFBP−1及び−3へのIGF−I変異体の結合性は野生型IGF−Iとは区別できないことが分かった。アラニン突然変異誘発は鋳型として一本鎖プラスミドpIGF−g3を用いて実施した(Kunkel等, Methods Enzymol., 204: 125-139 (1991))。システインとアラニンを除くIGF−Iの全ての残基が個々にアラニンに置換された。得られた作成物をDNA配列決定によって検証した。
免疫吸着プレート(Nunc, MAXISOR PTM, 96ウェル)を、pH7.2のPBSバッファー中1μg/mLのIGFBP−1又はIGFBP−3を100μl/ウェルで用いて4℃にて一晩かけて被覆した。ついでプレートを室温で2時間の間0.5%のTWEEN20TM/PBS(また結合バッファーとして使用)で阻害した(ウシ血清アルブミンのようなタンパク質様阻害剤は潜在的なIGF又はIGFBP汚染を防止するために避けた)。ファージミドベクターで新しく形質転換した大腸菌細胞(XL1-Blue, Stratagene)をM13−VCSヘルパーファージ(Stratagene)の存在下で5mLの2YT培地(上掲のSambrook等)中で一晩かけて増殖させた。ファージ粒子を収集し、Cabilly, S. (編), Combinatorial Peptide Library Protocols (Humena Press Inc.: Totowa, NJ, 1009), pp.249-264中のLowman, H.B., 「Pharge Dsiplay of Peptide Libraries on Protein Scaffolds」に記載されたようにしてPBSバッファー中に再懸濁させた。ついで、ファージ濃度を基準化し、各変異体に対して0.2−0.4の最大ELISAシグナルを生じさせた(上掲のCabilly, S. (編)のLowman)。可溶型競合体の3倍連続希釈物を、先に決定した濃度でファージを含む結合バッファー(0.5%のTWEENTM20/PBS)で非吸着マイクロタイタープレート(Nunc, F, 96ウェル)上に調製した。競合タンパク質の希釈範囲はIGFBP−1に対して5μmとIGFBP−3に対して500nMで出発して106の規模まで広げた。阻止後、固定化した標的を含むプレートを0.05%のTWEENTM/PBSバッファーで洗浄し、ついで室温で1時間の間80μl/ウェルの前もって混合されているファージ−競合体溶液と共にインキュベートした。洗浄後、結合したファージを、0.5%のTWEENTM/PBS中に一次ウサギ抗ファージポリクローナル抗体と二次ヤギ抗ウサギモノクローナル抗体−西洋わさびペルオキシダーゼ抱合体を含む溶液を80μl/ウェル用いて検出した。o−フェニレンジアミン(Sigma)とテトラメチルベンジジン(Kirkegaard及びPerry)を色素生産物質として使用し、生成物をそれぞれ492及び450nmで検出した。IC50値は、結合データを基準飽和曲線に一致させることにより決定された(上掲のCabilly, S. (編)のLowman)。各IGF−I変異体の少なくとも二つの個々のクローンが検定された。表Iの数は個々に評価されたIC50値の平均±標準偏差を表している。
Mortensen等, Endocrinology, 138: 2073-2080 (1997)により記載されているようにして、ヒトIGFBP−1をCHO細胞中で発現させ、条件培地から精製した。組換えヒトIGFBP−3をまたクローニングし、哺乳動物細胞で発現させた(Wood等, Mol. Endocrinology, 2: 1176-1185 (1988))。条件培地からの精製は、IGFアフィニティーカラム(Martin及びBaxter, J. Biol. Chem., 261:8754-8760 (1986))を用いて、本質的にIGFBP−1に対して記述された手順に従った。
プラスミドpBKIGF2B(米国特許第5342763号)はPphoAプロモータの制御下でlamBのリーダーペプチドに融合したヒト野生型IGF−Iを発現する。部位特異的突然変異誘発の簡単化のために、ファージf1複製起点(F1 ori)をプラスミドpBKIGF2B中に導入した。そのために、f1 oriを含む466bpのBamHI断片をpH0753から切除し(上掲のLowman,1991)、プラスミドpBKIGF2BをEcoRIで線形化した。ベクターと断片の双方をクレノー酵素で処理して、平滑末端連結に先立って付着末端制限部位を充填した。M13VCSヘルパーファージの存在下で一本鎖ファージミドDNAをつくり出す能力について正しい作成物を選択した。得られたファージミドベクターをpBKIGF2B−f1−oriと名付け、Kunkel等, Methods Enzymol., 204: 125-139 (1991)の手順を使用して対象のIGF−Iala−変異体を作成するために鋳型として用いた。突然変異誘発の各工程はDNA配列決定により確認した。
IGF−I変異体の発現は、IGF−I野生型に対して記述されているようなもの(Joly等, Proc. Natl. Acad. Sci. USA, 95: 2773-2777 (1998))であったが、オキシドレダクターゼの一過性の過剰発現が伴わなかった。精製手順は、過去のプロトコールに基づき(Cleland, J. L. (編), Protein Folding In Vivo and In Vitro (American Chemical Society, Washington,DC, 1993), pp.178-188)中の Chang及びSwartz, "Single-Step Solubilization and Folding of IGF-1 Aggregates from Escherichia coli)、僅かに適合化させた。典型的には、6gの湿った細胞ペースト(24時間成長させた2リットルの低ホスフェート培地と等価)を5mMのEDTAを含むpH7.5の25mMのトリス-HClの150ml中に再懸濁させた。細胞をマイクロフルイダイザー(Microfluidics Corp., Newton, MA)中で溶解させ、蓄積したIGF−I凝集物を含む屈折粒子を12000xgで遠心分離により収集した。屈折粒子を溶解バッファーで2回、1%のN−ラウロイル−サルコシン(Sigma)を含む溶解バッファーで2回洗浄して膜タンパク質を抽出し、溶解バッファーで再び2回洗浄した。洗浄した屈折体を、2Mの尿素、100mMのNaCl、20%のMeOH、及び2mMのDTTを含むpH10.4の50mMのCAPS(3−(シクロヘキシルアミノ)−1−プロパンスルホン酸;Sigma)バッファー中におよそ2mg/mlで再懸濁させた。この手順は屈折体の可溶化と続くIGF−I変異体の酸化リフォールディングを組み合わせるものである(上掲のChangとSwartz)。室温で3時間後、リフォールディング溶液を、50kDaの分子量カットオフ性を持つマイクロコンセントレーター膜(Centricon, Amicon)を通して濾過した。殆どの単量体IGF−Iが溶出液中に回収される一方、より高分子の汚染物は未透過物中に集中していた。この時点で、IGF−I画分は、SDS-PAGE分析から判断したところ、>95%の純度であった。IGFスワップ(二つの非天然ジスルフィドを含む;Hober等, Biochemistry, 31: 1749-1756 (1992);Miller等, Biochemistry, 32: 5203-5213 (1993))から正しくジスルフィド結合したIGF−Iを分離するために、リフォールディング溶液を5%の酢酸で酸性化し、4ml/分でDynamaxTMC18半調製用HPLCカラム(Varian;10.0mmID)に充填した。バッファーはH2O/0.1%TFA(A)及びアセトニトリル/0.1%TFA(B)であった。ジスルフィド異性体の分離は次の勾配:0−30%Bを20分、30−45%Bを60分を適用して達成された。IGF−スワップに対する未変性IGF−Iの比は各変異体に対して通常は約2:1であり、IGF−スワップは未変性IGF−Iより先の勾配で溶出した。各変異体の分子量は質量分析により検証した。HPLC精製後に、試料を凍結乾燥し、100mMのHEPESバッファー、pH7.4中におよそ1mg/mlで再構成した。
IGFBP−1とIGFBP−3に対するIGF変異体の結合親和性を、BIACORETM−2000リアルタイム反応速度相互作用分析システム(Biacore, Inc., Piscataway, NJ)を使用して会合(ka)速度と解離(kd)速度を測定することにより決定した。カルボキシメチル化デキストランバイオセンサーチップ(CM5, BIAcore Inc.)をEDC(N-エチル-N'-(3-ジメチルアミノプロピル)-カルボイミドヒドロクロリド)及びNHS(N-ヒドロキシスクシンイミド)で活性化させた。固定化については、20mMの酢酸ナトリウム、pH4.8中のIGF変異体を50μg/mlの濃度でバイオセンサーチップに注入し、およそ450−600RU(共鳴応答単位)の共有的に結合したタンパク質を生成した。未反応群は1Mのエタノールアミンの注入で阻害した。反応速度の測定を、25℃で20μl/分の流量を用いて、流れるバッファー(PBS、0.05%Tween20、0.1%卵白アルブミン、0.1%アジ化ナトリウム)中IGFBP−1かIGFBP−3の何れかの2倍の連続希釈物(1μMで開始)を注入することにより実施した。会合速度(ka)と解離速度(kd)はBIACORETM評価ソフトウェア3.0バージョンにおいて1:1のラングミュアー(Langmuir)TM会合モデルを用いて別個に計算した。平行解離定数(KD)はkd/kaとして計算した。
IGF−1の一価ファージディスプレイ
IGF−Iの70のアミノ酸残基の迅速かつ包括的なアラニンスキャンのために、タンパク質がファージM12の表面に一価的にディスプレイされるかどうかが最初に決定された(Bass等, Proteins, 8:309-314 (1990))。ファージディスプレイ技術は迅速な一本鎖DNA突然変異誘発の利点を、対応するファージ粒子の単なる単離によって、得られた変異体タンパク質の簡単な生成と組み合わせるものである(例えば上掲のCunningham等, 1994)。成熟ヒトIGF−IがM13遺伝子III産物のカルボキシ末端ドメインに融合されたベクターが作成された。この作成物は大腸菌の細胞膜周辺腔に融合タンパク質を向かわせ、タンパク質の一価ディスプレイを可能にするstIIシグナル配列を含んでいる(上掲のBass等;上掲のLowman等, 1991)。クローニングのために、IGF−1の最初のアミノ酸と最後のアミノ酸を変えた:得られた変異体G1S−A70Vは続くアラニンスキャンニング突然変異誘発のための鋳型作成物として使用した。
IGF−I G1S−A70Vをディスプレイするファージ粒子を単離し、IGFBFに対するその親和性についての結合競合ファージELISA法において検定したとき、その実験で決定されたIC50はIGFBP−1に対しては8.5nM、IGFBP−3に対しては0.5nMであった(図.1)。これらの値は野生型IGF−Iを使用するBIACORETM実験により決定された解離定数(上掲のHeding等)とよく一致している。放射性イムノアッセイ(RIA)により測定された野生型IGF−I親和性は、IGFBP−1に対して〜2.8nM、IGFBP−3に対して〜0.8nMであり、更にファージELISA法から導かれたIC50値を裏付けている。また、IGF−I G1S−A70Vをディスプレイするファージ粒子は、マイクロタイタープレート上に固定した11の独立したモノクローナルマウス抗IGF−I抗体によって効率的に捕捉された。これらの結果を併せると、ディスプレイされたIGF変異体が正しく折り畳まれ、ファージ粒子の表面に接近可能であることが示唆される。
G1S−A70VIGF−Iの全ての残基を、4固の未変性アラニン及び6個のシステインを除いて、記述したG1S−A70VIGF−IgIIIベクターを鋳型として使用して、個々にアラニンで置換した。また、単一変異体S1G及びV70A及び野生型IGF−Iを修復する二重変異体を作成した。これらの作成物の各々が大腸菌において発現され、ファージにディスプレイされた。IGFBP−1とIGFBP−3への結合性に対するIC50値は図1に示したような競合ファージELISA法によって測定した。各変異体の少なくとも二つの異なったクローンを試験した。得られたIC50値は表1に列挙し、G1S−A70Vに対する各変異体に対するIC50の減少又は増加は図2にグラフで表す。
結合親和性におけるほんの僅かな緩やかな改善がアラニン置換によって見出された。S1A,D12A及びD45AはIGFBP−1結合性においておよそ2倍の増加を示す一方、S35A及びT41AはIGFNBP−3に対して同様の効果を示した。しかしながら、IC50値の2倍の変化がこれらの実験における精度の限界である。
E3A、G7A、L10A、F25A及びF49はIGFBP−1対IGFBP−3の結合において差次的な効果を示した。これらの5つのIGF−I単一アラニン変異体において、IGFBP−1の相対IC50はIGFBP−3のものとは4倍以上異なっていた(図2;表1、相対的特異性)。E3AとF49Aはこの群において最も大きな相対特異性因子を示した。E3のアラニン置換はIGFBP−3の親和性には実質的に効果を持っておらず(1.4倍)、IGFBP−1への結合は34倍弱くされている。更に劇的なことには、F49Aの親和性はIGFBP−1に対しては100倍以上減少しているがBP−3に対しては3.6倍だけである。この結果は、ファージELISA法による直接の比較において示した。IGF−I F49Aをディスプレイするファージ粒子を可溶型IGFBP−1(図3A)又はIGFBP−3(図3B)の存在下でIGFBP−3被覆ウェルに添加した。IGF−I G1S−A70Vをディスプレイしている対照ファージと比較して、F49Aの結合曲線はIGFBP−1競合において102以上シフトした(図3A)。これに対して、結合曲線はIGFBP−3競合においては同様であり、IC50値の差異はファクター4未満であった(図3B)。よって、E3とF49はIGF−I分子においてIGFBP−1結合のための二つの主要な特異性決定因子である。
残基G7、L10及びF25は、アラニンによって置換されたときIGFBP−3に対するよりもIGFBP−1に対する親和性の顕著な損失を示したが、両方のIGFBPの結合に対して重要であると思われた。IGFBP−3に対しては、IGFBP−3に対してよりもIGFBP−1に対してより強く結合する変異体のような、有意な特異性決定因子は特定されなかった。しかし、変異E9A、D12A、F23A、Y24A、T29A、S34A及びD45AはIGFBP−1結合性についてよりもIGFBP−3に対して僅かに大きな(約2倍)効果を有していた。
ファージELISA法によって得られた結果の妥当性の確認のために、特異的アラニン変異体を発現させ、BIACORETM装置を用いた反応速度分析のために精製した。野生型IGF−Iの解離定数(KD)はIGFBP−1に対しては13nMで、IGFBP−3に対しては1.5nMであると決定された(図5Aと5B;表2)。IGFBPに対する親和性の差異は、IGFBP−3に対するIGF−Iの10倍速い会合速度(ka)のためである(3.2x105対3.2x104M−1s−1)。これらの結果はファージELISAによって測定された絶対IC50値とよく対応している(図1A及び1B;表I)。予想されるように、二重変異体G1S−A70Vは野生型とは本質的に区別ができない反応速度パラメータを示した(表2)。
V11A、R36A及びP39Aは、これらの変異体はファージに正しくディスプレイされなかったので、抗体認識実験(上を参照)に基づいて試験した。R36AとP39Aは双方の結合タンパク質に対して野生型の反応速度を示したが、V11AはIGFBP−1とIGFBP−3の双方に対して親和性の5倍の減少を示した。
更に、可溶型IGF変異体T4Aを試験することが決定された。この残基は以前の刊行物(上掲のBayne等, J. Biol. Chem., 263; 上掲のClemmons等, 1990)においてはIGFBP結合に関連していたが、ここでのファージアッセイでは穏やかな効果を示していた。野生型IGF−Iに対するT4AのKD値の増加はファージELISA法により測定されたIC50比よりおよそ2−3倍高かった(表2)。ファージ及びバイオセンサー分析により得られた結果の間の大きな矛盾はF16Aに対して見られた。この場合、二つの方法は因子4だけ異なっていた。
最初のα−へリックス(螺旋)領域における変異はIGFタンパク質構造に不安定化効果を有していることが示されている(上掲のJansson等, 1997)。如何なる理論にも制約を受けるものではないが、ファージの表面上のg3融合タンパク質は再び折り畳まれ精製された可溶型タンパク質よりも更に安定であるかもしれないと思われる。これは、二つの残基が構造的に感受性のN末端へリックスの外側に位置しているF25AとF49Aに対して得られたBIACORETMの結果によって裏付けられている。KDとIC50におけるそれぞれの変化はこれらの二つの変異体に対してすばらしく一致している(表2)。IGFBPへの結合に対するF49Aの差次的効果はBIACORETM分析によって確認された。70倍の親和性の減少がIGFBP−1結合に対して測定され(図5C;表2)、IGFBP−3結合性は4倍だけ減少した(図5D;表2)。
驚いたことに、IGFBP−3の相互作用は、IGFBP−3がおよそ10倍高い親和性をもってIGF−Iに結合するという事実にも拘わらず、IGFBP−1との相互作用よりも、アラニン置換に影響を受ける度合いは一般にかなり少なかった。P63Aとは別に、何れのアラニン変異体もIGFBP−3親和性の>6倍の低減を示さなかった(図2と表1)。
バイオセンサーの実験においてdes(1−3)−IGF−Iが25倍低減した親和性でもってIGFBP−3に結合することが過去に示されている(上掲のHeding等)。IGF−Iのこの天然に生じる型は最初の3つのN末端残基を欠いており、おそらくはIGFBP結合性の低減のために分裂促進能力の増加を示す(上掲のBagley等)。最初の3つアミノ酸側鎖の何れもIGFBP−3の結合に対するエネルギーを助長しないにもかかわらず(表1)、des(1−3)−IGF−IがIGFBP−3結合において妥協されるので、如何なる理論に制約されるものではないが、バックボーン相互作用が関与しているかも知れないとの仮説が立てられる。
この仮説は、最初の3個のN末端アミノ酸を置換する三重アラニン変異体(Ala(1−3)-IGF−I)をファージ上にディスプレイすることにより試験された。その領域のバックボーンがIGFBP−3との相互作用に寄与しているならば、この変異体は結合できなければならない。しかし、IGFBP−1への結合性はE3側鎖の欠落のために低減していなければならない(表1)。位置1及び2におけるIGFBP−1との潜在的なバックボーンの相互作用を試験するため、対照としてdes(1−2)−IGF−I変異体を産生した。予想されたように、Ala(1−3)-IGF−IはE3Aと同様にIGFBP−1親和性を減少させたが、IGFBP−3親和性における変化は示さなかった(表1;図2)。des(1−2)−IGF−Iに対しては、双方の結合タンパク質に対して親和性の差は観察されなかった。des(1−3)−IGF−I(上掲のHeding等)に関する知見と併せると、これらの結果は、如何なる理論にも限定されるものではないが、IGF−Iの残基3及び4の間のペプチドバックボーンがIGFBP−3との重要な相互作用を媒介する。
IGF−Iの表面上の機能性IGFBP−1及びIGFBP−3結合エピトープがアラニンスキャンニング突然変異誘発により証明された。両方の結合エピトープは図6に示されている。個々のIGF−I側鎖相互作用はIGFBP−3に対してよりもIGFBP−1への結合に対して更に重要な役割を果たしている。二つの主要な結合部分がIGFBP−1に対して見いだされる(図6A)。一つが(G7、L10、V11、L14、F25、I43、及びV44からなる)N末端ヘリックスの上面に位置し、一つが(E3、T4、L5、F16、V17及びL54からなる)下面に位置している。これらの二つの結合部分はF49とR50によって架橋されている。IGFBP−3に対しては、結合エピトープはより散在性であり、G22、F23及びY24を含むようにずれている(図6B)。IGFBP−3の結合性は一般にアラニン置換にはあまり感受性ではない。実際、親和性の最も大なる減少は(P63Aの他、以下参照)G7Aに対して見られる6倍の減少である。最もありそうなことは、如何なる理論にも制約を受けるものではないが、IGF−I主鎖バックボーンから派生する相互作用がIGFBP−3の結合性に寄与していることである。この仮説は更にAla(1−3)−IGF変異体での実験により実証される。単一及び三重アラニン置換はIGFBP−3結合性への効果を持たないが、最初の3個のアミノ酸の欠失の結果、25倍の親和性の減少した(上掲のBagley等;上掲のClemmons等;上掲のHeding等)。要約すると、IGF−Iは異なった結合様式を用いてIGFBP−1とIGFBP−3に結合する;数個のアミノ酸側鎖の相互作用がIGFBP−1への結合に重要である一方、バックボーン相互作用がIGFBP−3への結合に対して主要な活動的な役割を担うように思われる。
タンパク質−タンパク質界面の同様な研究において、僅かな側鎖残基だけが大部分の自由結合エネルギーに寄与することが見いだされた(Clackson及びWells, Science, 267:383-386 (1995); Kelley等, Biochemistry, 34: 10383-10392 (1995))。同じことがIGF−IGFBP−1相互作用に対しても当てはまる。しかし、ここでは、VIIa因子への組織因子の結合に対して注目されたように、重要な側鎖から派生した結合自由エネルギー値の大きさ(ΔΔG)は成長ホルモンの場合よりも小さい(上掲のKelley等)。支配的なΔΔGの寄与を持つ残基は成長ホルモン-レセプター界面におけるようにIGF−I表面上でクラスター形成しないが(上掲のClackson及びWells)、連続的なIGFBP−1結合エピトープをなお形成した(図6A)。これに対して、IGF−I上のIGFBP−3結合エピトープは不連続であり、側鎖は非常に穏やかな個々の結合エネルギーを導いた。
循環中のIGF−Iの大部分はIGFBP−3と酸不安定サブユニット(ALS)と名付けられた第3のタンパク質との複合体として見いだされる(上掲のBachとRechler;Clemmons, Cytokine Growth Factor Rev., 8:45-62 (1997); 上掲のJonesとClemmons)。150kDの分子量のこの三元複合体は脈管構造壁を横断できず、IGFに対する循環リザーバとして作用する。この機構によってIGF−Iの半減期が劇的に増加する(Simpson等, Grwoth Horm IGF Res, 8: 83-95 (1998))。IGFBP−3の量はIGF−Iによってポジティブに調節される。これに対してIGFBP−1の役割はそれ程明確ではない。このクラスの結合タンパク質は一般にはIGFBP−3程豊富ではなく、その量はインスリンによりネガティブに調節される(上掲のBachとRechler;上掲のClemmons, 1997; 上掲のJonesとClemmons)。
IGFBP−1とIGFBP−3への結合に対して重要なIGF−Iの残基を同定した。特定のIGFBPに対する結合特異性を決定する幾つかの残基が見いだされた。最近の刊行物(上掲のLoddick;上掲のLowman等, 1998)では、生物学的利用能のある「遊離の」IGF−Iの増加プールが結合タンパク質から内在性IGF−Iを置換することにより産生された動物実験が報告されている。IGFBP特異的IGF−I変異体はここに記載したように診断のためにまた治療のために使用することができる。
腎疾患の治療に関するある種の変異体の特徴付け
IGF−I変異体の作成
実施例1(及びDubaquieとLowman, Biochemistry, 38: 6386 (1999))において、IGFBP−1、IGFBP−3、又は双方の結合タンパク質への結合親和性が低減されたIGF−I変異体が同定された。特に、IGF−Iの全アラニンスキャンニング突然変異誘発は、IGFBP−1への結合のための特異性決定因子としてグルタミン酸3(E3)及びフェニルアラニン49(F49)、並びにある程度フェニルアラニン16(F16)及びフェニルアラニン25(F25)を同定した。ファージディスプレイアラニンスキャンニングの結果は、位置3と49の側鎖の双方がIGFBP−1との複合体生成のためのかなりの結合エネルギーに選択的に寄与する一方(E3Aに対して〜30倍、F49Aに対して〜100倍の親和性の損失)、IGFBP−3に対する結合エネルギーにおけるその寄与は検出できないか(E3A)、僅かである(F49Aに対して〜4倍)ことが示唆された(実施例1と上掲のDubaquieとLowmanを参照)。
IGFBP−3に対する更に改善された特異性は、点突然変異の効果が結合自由エネルギーへの寄与に対してしばしば相加的であるので、IGF−Iの累積性突然変異によって達成される可能性が高かった(Wells, Biochemistry 29: 8509 (1990))。従って、IGF−Iの二重変異体であるE3A/F49Aは単一の分子に点突然変異E3A及びF49Aを組み合わせることにより作成した。F16Aはより小なるIGFBP-特異性効果を示したが(実施例1及び上掲のDubaquieとLowman)、二重変異体F16A/F49Aもまた作成した。
F49Aを含むIGF−Iの単一アラニン変異体並びにE3A/F49A二重変異体を発現させ、精製し、再び折り畳んで、HPLC分析によって判断して適切なジスルフィド異性体を得た(ここの実施例1及び上掲のDubaquieとLowman)。これらの変異体を特異的結合タンパク質結合・レセプター活性化アッセイにおいて試験した。
IGFBP−1とIGFBP−3に対するこれら変異体の結合親和性をBIACORETM分析を使用して野生型IGF−Iのものと比較した。固定化したIGF−I又は変異体に対するIGFBP−3結合での反応速度実験(表3)を実施例1及び上掲のDubaquieとLowmanに記載されているようにして実施し、F49AIGF−I及び野生型IGF−Iと比較した。この実験では、二重変異体E3A/F49Aは野生型よりIGFBP−3への結合親和性が約20倍弱く、二重変異体F16A/F49Aは約66倍弱かった(表3)。
キナーゼレセプター活性化アッセイ(KIRA)はリガンドによる細胞外刺激時の細胞質IGFレセプターリン酸化の度合いを特異的かつ定量的に監視する(Sadick等, J. Pharm. Biomed. Analysis, 19(6): 883-891 (1998))。幾つかのIGF変異体G1S/A70V、T4A、V11A、F16A、F25A、F16A/F49A、R36A、P39A及びF49Aをレセプター活性化の単一濃度アッセイにおいて試験した。F16A/F49Aを除くこれら変異体のIGFBP−1及びIGFBP−3結合親和性は表II及び上掲のDubaquieとLowmanに示されている。表6はBIACORETM測定からの相対的親和性と特異性をまとめているものである。
KIRAアッセイに対しては、変異体濃度は、光学密度測定に基づき、大まかに13nM(「高濃度」)又は1.3nM(「低濃度」)と推定された。各IGF変異体に対して得られた信号を野生型IGF−Iの基準希釈列の信号と比較し、KIRAアッセイにおいて観察された活性に対応する見かけのIGF−I濃度によって報告した(図8A−8B)。正確な相対的効力は測定されなかったが、これらの結果は、試験した全ての変異体がIGFのI型レセプターを活性化する能力を維持していることを示している。
F49AとE3A/F49Aの相対的効力を決定するために、KIRAアッセイにおいて連続希釈を使用してI型IGFレセプターを活性化するその能力を測定した。図9A−9Bに示されているように、F49AとE3A/F49Aは共に野生型IGF−Iとは区別できないIGFレセプター活性化曲線を示している。最大半減有効濃度(EC50)は、F49Aに対して20.0±1.3ng/ml、E3A/F49Aに対して18.9±0.2ng/ml、野生型IGF−Iに対して19.8±0.6ng/mlであった。これらの結果は、両方のIGF変異体が十分に生物活性であることを強く示唆している。
活性IGF分子の腎臓への蓄積は慢性もしくは急性腎不全において潜在的に有益であろう。これれあの病状はIGFBP−1とIGFBP−2が異常に高レベルで、IGF−I剛性の低減が組み合わされ、最終的に異化作用を導くことにより特徴付けられる(上掲のToenshoff等, 1997)。
F49A及びE3A/F49A IGF−Iの予備的薬理学的性質を評価するために、両方のタンパク質を放射標識し、ラットに静脈内投与した。図10Aは両方の分子が動物の血液から除去される速度の時間経過を示す。そのIGFBP親和性の低減から予測されるように、双方の変異体は野生型ヒトIGF−Iと比較してより速い速度で除去された。興味深いことに、二重変異体(E3A/F49A)は単一変異体(F49A)よりも速く除去され、血清中の主要な結合タンパク質IGFBP−3に対するそれぞれの親和性とよく相関している(表3)。図10Bは異なった器官中のIGF変異体に対する血液対組織比を示している。大多数の放射標識IGF分子が腎臓において検出されたが、肝臓、脾臓、心臓、及び膵臓における放射能レベルは更に低かった。変異体F49AとE3A/F49Aが野生型IGF−Iと比較して腎臓において統計的に有意なより高レベルで蓄積することは明らかである。
F49AとE3A/F49A IGF−Iの円偏光二色性スペクトルを分析して、導入された変異がタンパク質構造に主要な変化をもたらすかどうかを試験した。構造的な不安定化はタンパク分解の感受性の増加に至り、IGF変異体のより速やかな血液浄化速度に対する別の説明を提供する。しかしながら、図11に示されているように、両方の変異体は野生型IGF−Iに対して記録されたものと実質的に同一のスペクトルを有している。IGF−IのNMRスペクトロスコピーから予想されるように、CDスペクトルはα−ヘリックスとランダムコイルの双方のエレメントを明らかにする(Coole等, Biochemistry, 30: 5484 (1991))。IGF−Iの熱安定性は、おそらくは室温で既に存在する比較的高い含量(〜30%)のランダムコイルのために、円偏光二色によって精確に決定することはできない(Jansson等, Biochemistry, 36: 4108 (1997))。両方の変異体のCDスペクトルが野生型IGF−Iからの有意な逸脱を示していないという事実は、導入された変異がIGF−Iの全体構造を変えないことの表れである。
上に提示した証拠から、単一及び二重変異体F16A、F16G、F16S、F25A、F25G、F25S、F49A、F49G、F49S、E3A/F49A、E3A/F49G、E3G/F49A、E3G/F49G、E3A/F49S、E3S/F49A、E3S/F49A、E3S/F49S、E3G/F49S、及びE3S/F49G IGF−Iは、アラニン置換変異体がIGFBP−3と結合する能力を大幅に喪失することなくIGFBP−1に対して低い親和性を示し多くの試験において生物活性であるので、GH/IGF軸調節不全により特徴付けられる疾患を治療するのに有効であると予想される。更に、かかる変異体は、F49AとE3A/F49Aが野生型IGF−Iと比較してまた他の器官と比較して腎臓に統計的に有意なより高いレベルで蓄積し、アラニン置換変異体がIGFBP−1にほんの僅かに結合するだけで、実験的尿毒症においてIGFBP−1及びIGFBP−2遺伝子の発現の増大が見られるので(上掲のToenshoff等, 1997)、腎疾患の治療に効果的であると予想される。
ヒトの治療
この実施例は、IGFBPの一又は複数に結合する外来性に投与されるペプチドが如何にして内在性IGFを置換するかの原理とヒトへの使用に対するここでのペプチドの服用方法を示す。
この実験では12週間の間4種の用量(10、20、40又は80μg/kg)で毎日2回の注射によって組換えヒトIGF−I又はプラセボをヒトII型糖尿病患者に投与した。血液試料を治療前、治療中に2週間毎に、治療の12週後(EP)に採取した。10μg/日のIGF−Iで治療された患者から採取された試料ではIGF−IIを測定しなかった以外は、IGF−I、IGF−II及びIGFBP−3の濃度を全ての試料について測定した。
国際公開第98/45427号の図44は患者の血液中のIGF−II濃度を示している。IGF−Iの上昇量と対照的に、IGF−IIの量はIGF−I濃度の上昇に対するほぼ鏡像的なパターンで低下する。上昇するIGF−I濃度のプラトー効果と同様に、現象するIGF−II濃度もまたプラトーに達した。
国際公開第98/45427号の図45は患者の血液中のIGFBP−3濃度を示している。血液中のIGF−I及びIGF−IIのパターンの明らかな変化と対照的に、IGFBP−3の濃度は統計的に有意な又は明らかな変化パターンを示さなかった。
国際公開第98/45427号の図43と図44の考察から、全IGF濃度(IGF−IプラスIGF−II)が治療では殆ど変化を示していないことが明らかにされる。これは、IGF−1の濃度の上昇がIGF−IIの濃度の低下に密接に合致するためである。3つの図の全の考察により、患者の血液中のIGF−I及びIGF−II濃度の用量依存的変化はIGFBP−3結合タンパク質能の低下により達成されなかったことが分かる(IGFBP−3は血液中の主要な結合タンパク質である)。
IGF−IIの濃度の低下とIGF−I及びIGF−II濃度のプラトー化の明白な説明は、IGF結合タンパク質能力が有限量であり、この実験において用いられたIGF−Iの用量が結合タンパク質からのIGF−IIの用量依存的な置換を引き起こしたことである。
本発明を止む終えずある特定の方法と材料を参照してここで検討した。これら特定の方法と材料の検討は本発明の範囲に対して限定をなすものでは決してなく、本発明の目的を達成するために好適な全ての代替の材料と方法まで拡張されるものと理解されるべきである。
Claims (20)
- 天然配列ヒト型IGF−Iの位置16、25、又は49のアミノ酸残基、或いは位置3及び49のアミノ酸残基が、アラニン、グリシン、又はセリン残基で置換されたインシュリン様成長因子Iの変異体。
- 天然配列ヒト型IGF−Iのアミノ酸残基がアラニン又はグリシンで置換された請求項1に記載の変異体。
- 天然配列ヒト型のIGF−Iのアミノ酸残基が、アラニンで置換された請求項1に記載の変異体。
- 請求項1に記載した変異体を担体に含んでなる組成物。
- 腎臓活性分子をさらに含む請求項4の組成物。
- 担体が無菌である請求項4に記載の組成物。
- 請求項1の変異体の有効量を哺乳類へ投与することを含む、哺乳類の成長ホルモン/インシュリン様成長因子(GH/IGF)軸調節不全を特徴とする疾患の治療方法。
- 疾患が高血糖性傷、腎臓疾患、うっ血性心不全、肝不全、栄養失調、消耗病症群、又は異化状態で、その時の哺乳類のインシュリン様成長因子結合タンパク質IGFBP−1のレベルがこれら疾患ではない状態時のレベルに対して増加した場合の請求項7に記載の方法。
