Claims (1)
- 【特許請求の範囲】 1. 血管を流れる血流を中断することなく、前記血管の壁部に沿って吻合部位 を創出するための装置であって、この装置は以下の構成を含む、 第1の伸張した部材、 前記第1の伸張した部材にスライド可能に取り付けられた第2の伸張した部材 、及び 前記第1及び第2の伸張した部材の遠位部に結合されている変形可能なシーリ ング部材であって、前記シーリング部材は以下の形状間で調節可能であり、 前記血管の壁部に設けられた切開部を通じて前記シーリング部材を前記血管 に挿入し、且つ前記血管から取り出すのに適した伸張した細い形状、及び 前記シーリング部材がカップを形成し、血流途絶領域を創出するために、前 記カップの縁部が、前記切開部の周囲で前記血管の内壁に対するシールを形成す るのに使用される拡張形状、 ここで、前記シーリング部材の形状は、前記血管の外部から、前記第1及び第 2の伸張した部材を相対的に移動させることにより、伸張した形状から拡張した 形状へ、及びその逆へ調節することができる。 2. 前記第1の伸張した部材がチューブであり、前記第2の伸張した部材が、 前記チューブ内でスライドするシャフトである請求項1の装置。 3. 前記シーリング部材が、前記シャフトの遠位端に取り付けられている遠位 端と、前記チューブの遠位端に取り付けられている近位端を有しており、前記シ ャフトが、前記シーリング部材の中空導管を通じて延びている請求項2の装置。 4. 前記シャフトの前記遠位端と前記チューブの前記遠位端が互いに近づく方 向に移動したときに、前記シーリング部材が反転して前記カップを形成する請求 項3の装置。 5. 手術実施者が前記シーリング部材の形状を遠隔的に調節できるように、前 記第1及び第2の伸張した部材に操作可能に結合されているハンドルを更に含む 請求項1の装置。 6. 前記ハンドルが、前記シーリング部材を、少なくとも1つのスプリングの 制御下で、前記伸張した形状から前記拡張した形状へ自動的に変形させるための 、手術実施者が作動すべくなされているトリガーを含む請求項5の装置。 7. 前記シーリング部材が、このシーリング部材が前記伸張した形状から前記 拡張した形状へ変形するときに半径方向に拡張する可撓性の管状部材を含んでい る請求項1の装置。 8. 前記可撓性の管状部材が、可撓性のある不浸透性の材料で少なくとも部分 的にコーティングされたメッシュチューブを含んでいる請求項7の装置。 9. 前記メッシュチューブが、前記拡張した形状をとれるように前形成されて いる請求項7の装置。 10. 前記可撓性の管状部材が、それ自体が反転することにより、前記伸張し た形状から前記拡張した形状へ変形する請求項7の装置。 11. 前記シーリング部材が、端面−側面吻合術を実施するための、前記血管 の壁部に沿った作業領域を提供するのに充分な大きさを有している請求項1の装 置。 12. 前記シーリング部材が、心臓バイパス形成術を実施するための、ヒト大 動脈の壁部に沿った作業領域を提供するのに充分な大きさを有する請求項1の装 置。 13. 前記第1及び第2の部材に沿ってスライド式に展開可能な押し抜き孔あ け装置を更に含み、この押し抜き孔あけ装置は、前記シーリング部材が前記シー ルを形成している間に、前記切開部の周囲で、前記血管の壁部に吻合用の孔を押 し抜くために用いられる請求項1の装置。 14. 前記押し抜き孔あけ装置が、本装置の作動ハンドルに取り外し可能に取 り付けられており、前記作動ハンドルは、前記第1及び第2の 伸張した部材の近位端部に結合されている請求項13の装置。 15. 前記シーリング部材を特定の形状に維持するために、前記チューブと前 記シャフトを一緒に制御可能にロックするためのロック手段を更に含んでいる請 求項1の装置。
