Claims (1)
- 【特許請求の範囲】 1.生物学的組織の生体内修復中隣接した柔らかい組織の脱出から生物学的組織 欠陥を保護するための移植片であって、該移植片は、 非金属基材材料でできた実質的に平坦なシートと、 非金属基材材料でできた実質的に平坦なシートに配置された複数の孔と、を含 み、該複数の孔は、隣接した柔らかい組織が生物学的組織欠陥中へ著しく脱出 することを防止しながら、隣接した柔らかい組織に由来する脈管構造および結 合組織細胞が生物学的組織欠陥中へ増殖するのを許容するようになっているこ とを特徴とする移植片。 2.結合組織細胞は、間葉細胞を含むことを特徴とする請求の範囲第1項に記載 の移植片。 3.移植片は、細胞制御のための少なくとも1つの物質で含浸されていることを 特徴とする請求の範囲第1項に記載の移植片。 4.移植片は、細胞移動に影響を与える化学戦略物質、細胞移動に影響を与える 抑制物質、細胞増殖に影響を与える細胞分裂促進成長因子、および新血管形成 (新しい血管の形成)を促進する因子のうちの少なくとも1つで含浸されてい ることを特徴とする請求の範囲第2項に記載の移植片。 5.骨欠陥を安定化するための固定装置と組み合わせて使用される移植片であっ て、移植片は、骨の湾曲と一致するほど十分に可撓性であり、骨欠陥の巨視的 運動を減少させ、固定装置が骨欠陥に取り付けられるときに周囲の運動が内部 空間中に伝わるのを制限するほど十分に強い基材材料でできたシートを含み、 移植片は、骨欠陥の修復中に隣接した柔らかい組織が骨欠陥中へ脱出すること から骨欠陥を保護するようになっており、さらに、骨欠陥の修復後骨の応力遮 蔽再吸収を防止するようになっていることを特徴とする移植片。 6.再吸収されることが防止される骨は、自己移植片および同種移植片のうちの 1つを含みことを特徴とする請求の範囲第5項に記載の移植片。 7.再吸収されることが防止される骨は、骨欠陥内に新しい再生骨を含むことを 特徴 とする請求の範囲第5項に記載の移植片。 8.基材材料でできたシートは、再吸収性であり、基材材料でできた移植片の再 吸収は、骨欠陥の応力遮蔽の量を減少させ、それによって、骨欠陥近くの新し い骨の再吸収を減少させることを特徴とする請求の範囲第7項に記載の移植片 。 9.固定装置は、再吸収性であり、基材材料でできた移植片の再吸収は、骨欠陥 の応力遮蔽を減少させ、それによって、骨欠陥内の新しい骨の再吸収を防止す ることを特徴とする請求の範囲第8項に記載の移植片。 10.固定装置は、再吸収性であり、約24月以内にその機械的強度を失い、そ れによって骨欠陥が、固定装置或いは移植片のいずれか、またはその両方によ って応力遮蔽されたならば起こるであろう骨欠陥近くの新しい骨の再吸収を防 止することを特徴とする請求の範囲第7項に記載の移植片。 11.基材材料でできたシートは、非再吸収性であり、固定装置がその機械的強 度を失った後、骨欠陥の応力遮蔽を防止するほど十分に可撓性であることを特 徴とする請求の範囲第10項に記載の移植片。 12.基材材料でできたシートは、複数の孔を含み、該孔の各々は、20ミクロ ンから3000ミクロンの間の範囲の径を有することを特徴とする請求の範囲 第5項に記載の移植片。 13.複数の孔の各々は、約1500ミクロンの径を有することを特徴とする請 求の範囲第12項に記載の移植片。 14.基材材料でできたシートは、10ミクロンから2000ミクロンの間の範 囲の厚さを有することを特徴とする請求の範囲第5項に記載の移植片。 15.基材材料でできたシートは、生劣化性の合成材料、および新しい骨が理想 的には成長するであろう骨の領域中へ柔らかい組織が著しく脱出するのを防止 するのに十分な堅さを有する非骨形成、非金属物質でもある生劣化性天然材料 のうちの少なくとも1つからなることを特徴とする請求の範囲第5項に記載の 移植片。 16.骨欠陥領域を、新しい骨が理想的には成長するであろう骨欠陥領域中への 柔らかい組織の増殖から保護するための再吸収性非骨誘導膜であって、該再吸 収世非骨誘導膜は、保護された領域内での新しい骨の成長を容易にし、柔らか い組織が 保護された領域中へ著しく脱出するのを防止するのに十分な堅さを有する基材 材料を含むことを特徴とする再吸収性非骨誘導膜。 17.複数の孔を含み、該孔の各々は、20ミクロンから3000ミクロンの間 の範囲の径を有することを特徴とする請求の範囲第16項に記載の再吸収性非 骨誘導膜。 18.10ミクロンから2000ミクロンの間の範囲の厚さを有することを特徴 とする請求の範囲第16項に記載の再吸収性非骨誘導膜。 19.再吸収性非骨誘導膜は、柔らかい組織が保護された領域中へ著しく脱出す るのを防止するのに十分な堅さを有する合成又は天然の、非骨誘導、非金属物 質を含むことを特徴とする請求の範囲第16項に記載の再吸収性非骨誘導膜。 20.柔らかい組織が骨欠陥の保護された領域中に脱出するのを防止するための 平坦な膜であって、該平坦な膜は、新しい骨が理想的には成長するであろう骨 欠陥領域の内部に配置されるのでなく、骨欠陥領域の外部に配置されるように なっており、それによって、全く新しい骨が保護された領域の内部でのみ成長 するのを容易にし、前記平坦な膜には、複数の孔が配置されており、該複数の 孔の各々は、隣接した柔らかい組織が保護された領域中へ著しく脱出すること を防止しながら、隣接した柔らかい組織に由来する脈管構造および結合組織細 胞が生物学的組織欠陥中へ増殖するのを許容するようになっていることを特徴 とする平坦な膜。 21.結合組織細胞は、間葉細胞を含むことを特徴とする請求の範囲第20項に 記載の移植片。 22.骨欠陥領域内での保護された骨の再生を容易にするための再吸収性保護膜 であって、再吸収性膜は、該再吸収性膜に配置された複数の孔を含み、該複数 の孔の各々は、約1mmよりも大きい径を有することを特徴とする再吸収性膜 。 23.複数の孔は、隣接した柔らかい組織が骨欠陥領域中へ脱出することを防止 しながら、脈管構造および結合組織細胞がこれらの複数の孔を経て生物学的組 織欠陥中へ増殖するのを許容するようになっていることを特徴とする請求の範 囲第22項に記載の再吸収性膜。 24.結合組織細胞は、間葉細胞を含むことを特徴とする請求の範囲第23項に 記載 の移植片。 25.複数の孔の各々は、約1.5mmの径を有することを特徴とする請求の範 囲第22項に記載の再吸収性膜。 26.再吸収性膜は、10ミクロンから2000ミクロンの間の範囲の厚さを有 することを特徴とする請求の範囲第22項に記載の再吸収性膜。 27.再吸収性膜は、柔らかい組織が保護された領域中へ著しく脱出するのを防 止するのに十分な堅さを有する合成又は天然の、非骨誘導、非金属物質を含む ことを特徴とする請求の範囲第22項に記載の再吸収性非骨誘導膜。 28.ほ乳類の身体内に移植され、それによって、骨が理想的には成長するであ ろう骨欠陥の保護された領域中へ柔らかい組織が脱出するのを防止する非骨誘 導膜であって、非骨誘導膜が、該非骨誘導膜内に配置された複数の孔を含み、 該複数の孔の各々は、複数の孔を含み、該孔の各々は、20ミクロンから30 00ミクロンの間の範囲の径を有することを特徴とする非骨誘導膜。 29.非骨誘導膜は、ほ乳類の身体内に再吸収されるようになっていることを特 徴とする請求の範囲第28項に記載の非骨誘導膜。 30.非骨誘導膜は、ほ乳類の身体内に再吸収されるようになっているが、非骨 誘導膜がほ乳類の身体内に移植された最初の時から約24月以内は、再造形さ れないことを特徴とする請求の範囲第29項に記載の非骨誘導膜。 31.ほ乳類の身体内に移植される再吸収性非骨誘導膜であって、再吸収性非骨 誘導膜は、新しい骨の成長が理想的には起こるであろう骨欠陥領域の境界のま わりに移植されるようになっており、再吸収性非骨誘導膜は、柔らかい組織が 骨欠陥領域中へ著しく脱出するのを防止するのに十分な堅さを有することを特 徴とする再吸収性非骨誘導膜。 32.生物学的組織欠陥の修復中隣接した組織の脱出から生物学的組織欠陥を保 護するための可撓性膜であって、移植片は、新しい骨が理想的には成長するで あろう生物学的組織欠陥領域の内部に配置されるのでなく、生物学的組織欠陥 領域の外部に剛性の固定装置によって配置されるようになっており、それによ って、生物学的組織欠陥領域内で新しい骨が成長することを許容し、可撓性膜 は、骨の湾曲 に一致するほど十分に可撓性であり、隣接した柔らかい組織が生物学的組織欠 陥領域中へ脱出するのを防止するほど十分に強いことを特徴とする可撓性膜。 33.可撓性膜に配置された複数の孔を含み、該複数の孔の各々は、20ミクロ ンから3000ミクロンの間の範囲の径を有することを特徴とする請求の範囲 第32項に記載の可撓性膜。 34.複数の孔は、脈管構造および結合組織細胞が生物学的組織欠陥領域中へ増 殖することを許容するようになっていることを特徴とする請求の範囲第33項 に記載の可撓性膜。 35.可撓性膜は、10ミクロンから2000ミクロンの間の範囲の厚さを有す ることを特徴とする請求の範囲第34項に記載の再吸収性膜。 36.可撓性膜は、柔らかい組織が保護された領域中へ著しく脱出するのを防止 するのに十分な堅さを有する合成又は天然の、非骨誘導、非金属物質からなる ことを特徴とする請求の範囲第35項に記載の再吸収性非骨誘導膜。 37.保護された骨再生を容易にするための再吸収性膜であって、該再吸収性膜 は、骨欠陥領域のまわりに巻き付けられるようになっており、それによって、 骨欠陥全体を覆い、取り囲み、骨欠陥領域近くの骨の隣接した領域に重なり合 い、前記再吸収性膜は、隣接した柔らかい組織が骨欠陥領域中へ脱出するのを 防止し、それによって、再吸収性膜が骨欠陥領域のまわりに固着され、および 、骨欠陥領域近くの骨の隣接した領域に固着されたときに、固定装置からの助 けなくして、独立に骨の再生を容易にするほど十分な強度を有することを特徴 とする再吸収性膜。 38.前記再吸収性膜は、骨欠陥領域のまわり、および、骨欠陥領域近くの骨の 隣接した領域の両方に固着されたときに、骨欠陥領域全体を取り囲み、骨欠陥 領域近くの骨の隣接した領域に重なり合う管を形成することを特徴とする請求 の範囲第37項に記載の再吸収性膜。 39 前記再吸収性膜は、骨欠陥領域のまわりに摩擦的に固着されていることを 特徴とする請求の範囲第38項に記載の再吸収性膜。 40.前記再吸収性膜は、クランプ、ステープル、ねじ、縫目、および鋲のうち の少なくともで骨欠陥領域のまわりに摩擦的に固着されていることを特徴とす る請求 の範囲第38項に記載の再吸収性膜。 41.固定装置は、プレート、ねじ、髄内ロッド、および外部固定装置のうちの 少なくとも1つを含むことを特徴とする請求の範囲第38項に記載の再吸収性 膜。 42.骨欠陥領域は、骨折した長い骨からなることを特徴とする請求の範囲第4 1項に記載の再吸収性膜。 43.再吸収性膜は、該再吸収性膜に配置された複数の孔を含み、該複数の孔の 各々は、約1mmよりも大きい径を有することを特徴とする請求の範囲第42 項に記載の再吸収性膜。 44.再吸収性膜の再吸収性は、骨欠陥領域の長期の応力遮蔽を回避し、それに よって、骨欠陥領域およびその近くの新しい骨のその後の骨再吸収を回避する ことを特徴とする請求の範囲第43項に記載の再吸収性膜。 45.柔らかい組織の介置から生物学的組織欠陥領域を保護する方法であって、 該方法は、隣接した柔らかい組織が生物学的組織欠陥領域中へ著しく脱出する ことを防止しながら、隣接した組織に由来する脈管構造および結合組織細胞が 生物学的組織欠陥の境界中へ増殖するのを許容するようになっている複数の孔 を含む再吸収性膜を、生物学的組織欠陥の境界の外部に配置する操作を含むこ とを特徴とする方法。 46.生物学的組織欠陥領域は、骨欠陥領域を含み、再吸収性膜を骨欠陥領域の 境界の外部に配置する操作は、再吸収性膜を長い骨の2つの端のまわりに巻き 付け、それによって、長い骨の2つの端の間の欠損部を取り囲むことを特徴と する請求の範囲第45項に記載の方法。 47.再吸収性膜を長い骨の2つの端のまわりに巻き付けた後に、剛性の固定装 を、長い骨の2つの端の間に固着することを特徴とする請求の範囲第46項に 記載の方法。 48.柔らかい組織の介置から骨欠陥領域を保護する方法であって、該方法は、 隣接した柔らかい組織が骨欠陥領域中へ著しく脱出することを防止しながら、 隣接した組織に由来する脈管構造および結合組織細胞が骨欠陥領域の境界中へ 増殖するのを許容するようになっている複数の孔を含む非金属基材材料ででき た実質的に 平坦なシートを、骨欠陥領域の境界の外部に配置する操作を含むことを特徴と する方法。
