- 【特許請求の範囲】 1. 一般的な丸い形とガイドチャネルを内部に有し、患者の膣の後脳弓に収容さ れるような形状と大きさとを有する前端部と、 前端部から伸びているシャフトと、 ユーザによって保持されて操られるような大きさと形状とを有するシャフトに 接続している後端部と、 を有し、ガイドは患者の膣に挿入されて後脳弓に対して配置され、医療機器はガ イドチャネルを介して患者の頸部の下の膣壁に対して配置されることを特徴とす る医療機器ガイド。 2. ガイドチャネルは前端部の上面から前端部に達している溝であることを特徴 とする請求の範囲第1項に記載のガイド。 3. ガイドは、ガイドをカニューレに取り外し可能に取り付けるための領域を有 することを特徴とする請求の範囲第1項に記載のガイド。 4. 前端部は、頸部を収容するための凹部を有する上面を有することを特徴とす る請求の範囲第1項に記載のガイド。 5. 医療機器が前端部の前を過ぎて所定距離を進むことを防ぐストップを更に有 することを特徴とする請求の範囲第1項に記載のガイド。 6. 医療機器はベレス針であり、所定距離は約15mmであることを特徴とする 請求の範囲第5項に記載のガイド。 7. カニューレと、 カニューレ内部に位置する拡張閉塞器と、 第1の管状の針シャフトと、閉じた鈍い前端部を有する第1の針シャフトに摺 動自在に配置された第1の針先端閉塞器とを有する第1のベレス針アセンブリと 、 第2の管状の針シャフトと、開いた前端部を有する管状の形を有する第2の針 シャフトにおいて摺動可能に配置された第2の針先端閉塞器とを有する第2のベ レス針アセンブリと、 を有し、第1および第2のベレス針アセンブリは、拡張閉塞器に交互に摺動可能 に配置されることを特徴とする医療機器システム。 8. 第2のベレス針アセンブリは、第2の管状の針シャフトの後端部が固定され ているハンドルと、ハンドルに接続されて第2の閉塞器の後端部に同軸に且つ伸 縮自在に接続している管状のバネストップと、第2の閉塞器とバネストップとの 間に接続されて第2の閉塞器を拡張位置に付勢するバネと、を更に有することを 特徴とする請求の範囲第7項に記載のシステム。 9. バネはコイルバネであることを特徴とする請求の範囲第8項に記載のシステ ム。 10. 更に、ハンドルに接続している弁を有することを特徴とする請求の範囲第 8項に記載のシステム。 11. 更に、ハンドルに接続しているガス圧力センサを有す ることを特徴とする請求の範囲第8項に記載のシステム。 12. 組織を貫くための医療機器であって、 ベレス針と、 患者の膣に挿入されて患者の後脳弓に当たるように配置されるような大きさと 形状とを有するロケータ領域を有するポジショナと、 を有し、ポジショナは、可動に取り付けられるベレス針と、ポジショナのベレス 針をポジショナのホームポジションから所定の限られた長手方向の距離を前方に 移動させる手段とを有することを特徴とする医療機器。 13. 移動手段は、ベレス針をポジショナに対して前方に押し込むバネと、ポジ ショナに配置されてベレス針の前方移動をポジショナに対して所定位置で停止さ せるストップ突起とを有することを特徴とする請求の範囲第12項に記載の機器 。 14. 組織を貫くための医療機器であって、 ベレス針アセンブリと、 ベレス針アセンブリが摺動自在に接続される拡張閉塞器と、 を有し、拡張閉塞器は、前端部に格納式の横に伸びるカッターを有することを特 徴とする医療機器。 15. ベレス針アセンブリは、針先端を有する管状の針シャフトに摺動可能に位 置する管状の閉塞器を有し、管状の閉塞器は、針先端で開いた前端部を有するこ とを特徴とする 請求の範囲第14項に記載の機器。 16. 組織を貫通してアクセスする医療機器であって、 カニューレと、 針先端領域と、針先端領域後方の一体成形拡張外側膨張領域とを有するチュー ブ形状フレームと、針先端領域に配置されたバネロードの安全閉塞器とを有する ベレス針および拡張器の組み合わせアセンブリと、 を有し、ベレス針及び拡張器の組み合わせアセンブリは、組織を介して小さい孔 を突き通し、組織の孔をカニューレと同じサイズに拡張するようになっているこ とを特徴とする医療機器。 17. バネロードの安全閉塞器は、針先端領域に位置する開いた前端部を有する チューブ形部材を有することを特徴とする請求の範囲第16項に記載の機器。 18. チューブ形フレームは、拡張外方拡張領域の前に第1の直径を有する第1 の領域と、拡張外方拡張領域の後により大きな第2の直径を有する第2の領域と 、を有することを特徴とする請求の範囲第16項に記載の機器。 19. チューブは、針先端領域と拡張領域とを有する一体成形部材であることを 特徴とする請求の範囲第16項に記載の機器。 20. 中心チャネルと、針の形をした開いた前端部と、中心チャネルへ続く開い た後端部とを有する管状の形を有するフレームと、 開いた後端部に接続している圧力信号器と、 を有し、圧力信号器は、大気圧に対する開いた前端部でのガス圧力の差をユーザ に合図することを特徴とするベレス針アセンブリ。 21. フレームに取り付けたバネロードの安全先端閉塞器を更に有し、安全先端 閉塞器は開口部を有することを特徴とする請求の範囲第20項に記載のアセンブ リ。 22. 患者の腹膜壁を介してカニューレを挿入する方法であって、 患者の膣に腹膜壁を介してベレス針アセンブリを挿入して腹膜壁に孔を形成す る行程と、 孔のサイズを拡張する行程と、 サイズが拡張された孔にカニューレをすべらせる行程と、 からなることを特徴とする方法。 23. 孔のサイズを拡張する行程は、第1の直径を有する第1の領域を有するベ レス針アセンブリの管状のフレーム部と、より大きな第2の直径を有する第2の 領域と、第1および第2の領域の間の拡張膨張領域と、を有し、拡張領域は、管 状のフレーム部を孔に通すにつれて、孔のサイズを拡張することを特徴とする請 求の範囲第22項に記載の方法。 24. 孔のサイズを拡張する行程は、孔を介してベレス針アセンブリの拡張閉塞 器をすべらせて、孔の幅を広くしてくさびでとめる行程を有することを特徴とす る請求の範囲第 22項に記載の方法。 25. カニューレをすべらせる行程は、閉塞器に対してカニューレを孔へとすべ らせることを特徴とする請求の範囲第24項に記載の方法。 26. 更に、ベレス針アセンブリを介してガイドワイヤをベレス針アセンブリの 前端部の外へ出るように挿入する行程を有することを特徴とする請求の範囲第2 2の方法。 27. 更に、ガイドワイヤを孔にとどめながらも、腹膜壁および膣からベレス針 アセンブリを取り外す行程を有することを特徴とする請求の範囲第26項に記載 の方法。 28. 膨張行程は、ガイドワイヤにそって孔へと拡張閉塞器を挿入して孔の幅を 広げてくさびでとめる行程を有することを特徴とする請求の範囲第27項に記載 の方法。 29. 孔のサイズを拡張する行程は、孔の側面にスリットを切れ込む拡張閉塞器 の横方向のカッターから成ることを特徴とする請求の範囲第28項に記載の方法 。 30. カッターは、拡張閉塞器の拡張位置および格納位置の間を移動する刃から 成ることを特徴とする請求の範囲第29項に記載の方法。 31. 更に、膣にガイドを挿入する行程を有し、ガイドは案内スロットを有し、 ベレス針アセンブリを挿入する行程は、ガイドの案内スロットにベレス針アセン ブリを配置する行程からなることを特徴とする請求の範囲第22項に記載の方法 。 32. ベレス針アセンブリを挿入する行程は、ガイドの駆動機構を動かして高速 で腹膜壁を介してベレス針アセンブリを挿入する行程からなることを特徴とする 請求の範囲第22項に記載の方法。 33. 動作行程は、圧縮したバネを解放する行程からなり、挿入行程は、更に、 所定位置までガイドに対するベレス針アセンブリの前方移動を制限する行程から なることを特徴とする請求の範囲第32に記載の方法。 34. 針先端でガス圧力の変化を検出してガス圧力の変化のユーザに合図するこ とによって、ベレス針アセンブリの針先端が適切に腹膜壁を貫通したか否かを判 別する行程を更に有することを特徴とする請求の範囲第22項に記載の方法。 35. 患者の腹膜壁を介して侵入する方法であって、 腹膜壁近傍の患者の組織に対して、案内スロットおよび超音波プローブ先端を 有するガイドを配置する行程と、 超音波プローブ先端の前で患者の身体の一部の超音波プローブ先端に接続して いる超音波ベースユニットで超音波画像を生成する行程と、 組織および腹膜壁を介して超音波プローブ先端の視界へとベレス針を進める行 程と、 からなることを特徴とする方法。
