Abstract
L'invention concerne une prothèse destinée à être utilisée en chirurgie pour la réparation de hernies.Elle comprend une matière textile prothétique préformée (14) supportée le long de sa périphérie par un fil (12) en alliage à mémoire de forme ayant une température de transformation approximativement égale à la température normale du corps, ce qui permet à la prothèse d'être étroitement enroulée en une configuration cylindrique pour être amenée en position au moyen d'un instrument laparoscopique, et de se déployer suivant une configuration prédéterminée en se réchauffant jusqu'à la température du corps.Domaine d'application : traitement des hernies, etc.
Description
L'invention concerne un appareil destiné à être utilisé dans la chirurgie
de réparation de hernies, et elle a trait plus particulièrement à une pièce prothétique de réparation de hernies qui peut être enroulée dans un tube pour être amenée en poisition par voie laparoscopique au moyen d'un trocart, et qui se déploie en une forme globale-
ment plane lorsqu'elle est éjectée du trocart dans la cavité abdominale. On a effectué depuis 1991 de nombreuses inter-
ventions de réparation laparoscopique de hernies portant sur des hernies inguinales, ventrales, ombilicales et dues à des incisions chirurgicales ou des cicatrices, avec beaucoup de succès quant à la satisfaction des patients. En comparaison
avec la chirurgie ouverte, les patients soumis à une inter-
vention laparoscopique souffrent beaucoup moins et reprennent plus rapidement une activité normale.
Dans l'exécution de ces interventions, on utilisait initialement une pièce prothétique réalisée, au moment de l'intervention chirurgicale, à partir d'une toile20 de polypropylène tissée, stérile, qu'on pliait en deux pour former une double couche, puis qu'on coupait à dimensions,
habituellement sous la forme d'un rectangle de 6 cm par 9 cm.
On utilisait des sutures pour relier les quatre angles du rectangle et deux sutures supplémentaires étaient placées approximativement au milieu du bord non plié. Puis on pratiquait entre ces deux sutures supplémentaires une fente calculée pour recevoir les vaisseaux épigastriques infé- rieurs. Après une dissection pour éloigner le sac herniaire des structures homolatérales du testicule et des cordons, la30 formation d'une incision péritonéale et la dissection, ensuite, du pan peritoneal et du sac herniaire à l'écart du défaut constitué par la hernie et des structures de fascia et de cordon entourantes, la pièce était enroulée en un tube et
insérée dans un manchon de trocart qui était ensuite intro-
duit dans un trocart de plus grand diamètre, et était
introduite dans la cavité peritoneale. Des forceps laparos-
copiques étaient ensuite utilisés pour dérouler la pièce et la placer antérieurement à l'anomalie herniaire et autour des vaisseaux épigastriques inférieurs, la toile recouvrant à la fois les espaces herniaires direct et indirect. La pièce de toile était ensuite maintenue en place par agrafage ou suture sur le fascia sous-jacent. Ensuite, le péritoine était fermé sur la pièce afin que la pièce entière de toile soit ainsi recouverte. Bien que l'intervention ci-dessus se soit avérée entièrement satisfaisante en termes de résultat, la nécessité de fabriquer la pièce de toile au moment de l'intervention chirurgicale, et la difficulté ensuite de dérouler et de positionner la pièce de toile par rapport aux espaces herniaires direct et indirect, ainsi que la nécessité d'agrafer ou de suturer ensuite la pièce de toile en place, ajoutent nécessairement un temps et un coût notables,
demandés pour l'exécution de l'intervention.
On a donc besoin d'une pièce pour hernie destinée à être utilisée en chirurgie laparoscopique, préfabriquée pour se conformer aux structures anatomiques, se déployant aisément lorsqu'elle est libérée d'un tube laparoscopique d'introduction, et restant en place sans nécessiter un
agrafage ou une suture sur les fascias sous-jacents. L'inven-
tion répond à ce besoin.
La pièce de réparation pour hernies de l'inven-
tion comporte un cadre en fil métallique qui peut être de diverses conceptions comprenant, à titre non limitatif, la forme d'une boucle fermée, le fil constituant le cadre étant
formé d'un alliage à mémoire de forme. Une matière prothéti-
que synthétique, telle que du polypropylène tissé ou du PTFE expansé ("Gortex"), est attachée au cadre en fil et supportée par celui-ci. Le cadre en fil supportant la toile peut être formé de NiTiNOl ou d'un autre alliage convenable à mémoire de forme et elle peut être attachée à la matière prothétique afin d'avoir la configuration d'un sablier lorsque l'alliage est dans sa forme austénitique et une configuration enroulée, cylindrique, lorsqu'il est dans sa forme martensitique. Le pourcentage en valeur atomique de nickel dans l'alliage est tel que l'alliage présente une température de transformation d'environ 37 C (température du corps). Par conséquent, lorsque la pièce est refroidie, elle peut être aisément façonnée en une configuration cylindrique pour être placée dans un trocart de mise en position. Lorsqu'elle est éjectée du trocart dans la cavité abdominale du patient, elle se
réchauffe jusqu'au point ou l'alliage prend sa forme austéni-
tique afin de revenir dans sa configuration fonctionnelle prédéterminée. La partie centrale rétrécie d'une pièce en forme de sablier laisse la place aux vaisseaux épigastriques inférieurs et aux structures des cordons, tandis que les lobes externes opposés recouvrent les espaces herniaires direct et indirect. Etant donné que le cadre est intégré à la pièce, il ne se déplace pas et il est donc inutile qu'il soit suturé ou agrafé en place. Il reste ancré après avoir été recouvert par le péritoine à la manière d'un sandwich ou
d'une structure stratifiée.
L'invention sera décrite plus en détail en regard des dessins annexés à titre d'exemple nullement limitatif et sur lesquels: la figure 1 est une vue en plan à échelle agrandie de la pièce de réparation de hernies réalisée conformément à l'invention; la figure 2 montre la pièce de la figure 1 dans une configuration enroulée, tubulaire, pour une mise en place endoscopique au moyen d'un trocart; et la figure 3 est une vue en bout de la pièce
enroulée de la figure 2.
En référence a la figure 1, il est illustré une vue à échelle agrandie d'une pièce pour réparation de hernies réalisée conformément à l'invention. La pièce prothétique est désignée de façon générale par la référence numérique 10 et on voit qu'elle comprend un élément de cadre extérieur 12 constitué d'un fil métallique formé d'un alliage métallique à mémoire de forme, avantageusement un alliage de nickel et de titane comportant 49 à 51 %, en valeur atomique, de nickel et le reste de titane. Un tel alliage est désigné comme élément NiTiNOL. En ajustant convenablement la concentration relative du nickel dans l'alliage, on peut amener le cadre 12 en fil métallique à présenter une température de transition entre la forme austénitique et la forme martensitique à environ 37 C, correspondant à la température du corps. Le fil est représenté sous une forme constituée d'une boucle fermée qui est de configuration globalement ovale, mais avec un petit axe restreint pour présenter un profil en forme de
sablier. Bien que cette configuration soit peut-être avanta-
geuse, l'invention n'entend pas être limitée à cette confi-
guration et il n'est pas nécessaire que le cadre forme une
boucle fermée.
Une étoffe prothétique 14, avantageusement constituée de fils tissés de polypropylène ou de PTFE expansé ("Gortex"), est supportée dans le cadre 12 en fil. Le dispositif prothétique 10 peut ainsi être stérilisé à la vapeur. Lors de l'utilisation, les lobes 16 et 18 sont destinés à être placés au-dessus des espaces herniaires direct et indirect, respectivement, et lorsque ceci est réalisé, la partie centrale resserrée, indiquée de façon générale par la référence numérique 20, permet la mise en
place sans gêne des vaisseaux épigastriques inférieurs.
Lorsque le cadre 12 en alliage est refroidi au-
dessous de l'état de transformation de l'alliage à mémoire de forme afin qu'il se trouve dans sa forme martensitique, la prothèse 10 peut être enroulée en hélice pour former une
structure cylindrique comme illustré sur les figures 2 et 3.
Ceci permet d'introduire la prothèse dans la cavité abdomi-
nale au moyen d'un trocart tubulaire. Pendant que le cadre 12 en alliage à mémoire de forme se réchauffe jusqu'à la
température du corps, il se transforme en sa forme austéniti-
que comme illustré sur la figure 1. En utilisant des forceps laparoscopiques, le chirurgien peut saisir la prothèse 10 de la figure 1 et la repositionner jusqu'à ce que les lobes 16 et 18 et la partie centrale rétrécie 20 soient convenablement
placés pour recouvrir la hernie.
Les spécialistes de la technique apprécieront que la prothèse peut être fabriquée sous diverses formes et dimensions pour convenir à des enfants, des adultes, des hommes et des femmes, et en particulier au type de hernies rencontrées. Elle peut être contenue dans un emballage stérile jusqu'au moment de l'utilisation. Bien que le NiTiNOL soit l'alliage à mémoire de forme préférée, d'autres alliages, tels que des alliages or-cadmium, nickel-aluminium et manganèse-cuivre pouraient également convenir. De plus, il n'est pas nécessaire que la matière textile prothétique 14 soit une toile de polypropylene, mais elle peut également comprendre d'autres matières convenables, telles que des films polymeriques à orientation biaxiale, compatibles avec
le corps.
Le grand axe de la prothèse 10 peut être compris dans une plage allant d'environ 6 à 12 cm et le petit axe, resserré, peut être d'environ 4 à 8 cm, sans que ceci ne soit limitatif. Le dispositif peut être roulé étroitement en un cylindre, comme montré sur la figure 2, afin d'être logé dans la lumière intérieure d'un trocart de 10 mm ou moins ou d'une
gaine d'introduction.
Il va de soi que de nombreuses modifications peuvent être apportées à la pièce de réparation décrite et
représentée sans sortir du cadre de l'invention.
Claims (7)
1. Pièce de réparation de hernies, caractérisée en ce qu'elle comporte un cadre (12) en fil métallique formé d'un alliage à mémoire de forme, et une matière textile prothétique (14) attachée au cadre et supportée le long de sa
périphérique par le cadre.
2. Pièce de réparation de hernies selon la revendication 1, caractérisée en ce que le cadre en fil métallique constitue une boucle fermée de forme globalement ovale, mais avec un petit axe réduit lorsqu'il est sous sa
forme à température plus élevée.
3. Pièce de réparation de hernies selon la revendication 1, caractérisée en ce que l'alliage à mémoire de forme comprend du NiTi avec un pourcentage de Ni dans l'alliage compris dans une plage de 49 à 51 % en valeur atomique.
4. Pièce de réparation de hernies selon la revendication 3, caractérisée en ce que l'alliage à mémoire de forme présente une température de transformation d'environ
37 %.
5. Pièce de réparation de hernies selon la revendication 1, caractérisée en ce que le cadre en fil métallique présente la configuration d'un sablier lorsque l'alliage à mémoire de forme est dans sa forme austénitique et une configuration étroitement enroulée lorsqu'il est dans
sa forme martensitique.
6. Pièce de réparation de hernies selon l'une
quelconque des revendications 1 à 5, caractérisée en ce que
la matière textile prothétique est choisie dans un groupe constitué d'une toile tissée de fibres de polypropylene et de
PTFE expanse.
7. Pièce de réparation de hernies selon la revendication 5, caractérisée en ce que la matière textile prothétique est globalement plane et présente une grande dimension longitudinale comprise dans une plage de 6 à 12 cm et une petite dimension transversale comprise dans une plage
de 4 à 8 cm.
