Abstract
L'invention concerne plus spécialement la conformation de la pointe d'un cathéter pour éviter une blessure interne lors de l'introduction chez un patient. Au moins la partie terminale 3 du cathéter 1 est faite de plusieurs couches dont une couche interne 8 en matériau relativement mou et une couche externe 7 en matériau plus dur. A la formation de la pointe, au moyen de meules tournantes 13 par exemple, la couche dure externe 7 est enlevée. La couche 8 qui subsiste sur la pointe, en matériau mou, ne risque pas de blesser la paroi d'un vaisseau par exemple. Applicable notamment à l'angiographie.
Description
L'invention concerne un procédé pour fabriquer un cathé-
ter, comprenant la fabrication d'un corps tubulaire flexible, de forme allongée, possédant au moins un canal central, la mise en place d'un élément de raccordement à une extrémité du corps et la réalisation d'une pointe sur une portion de l'autre extrémité, de manière que cette portion soit pourvue d'un diamètre décroissant
vers la pointe extrême.
Un tel procède est généralement connu. Le corps tubulaire est fabriqué de matériau qui confère au cathéter final les
propriétés désirées, telles que flexibilité et rigidité en torsion.
Une pointe est formée sur l'extrémité de sortie afin de faciliter l'introduction du cathéter chez un patient. Bien entendu, cette pointe n'est pas réalisée de manière qu'il se forme une pointe aigué ou un bord terminal vif. Une telle conformation pourrait provoquer des blessures internes lors de l'introduction. Il existe néanmoins un certain risque de blessure interne, en particulier de
lésion de la paroi d'un vaisseau par exemple, lors de l'introduc-
tion de cathéters fabriqués selon le procédé connu.
L'invention vise à apporter un procédé, du type indiqué au
début, pour fabriquer un cathéter avec lequel ce risque soit dimi-
nué. Selon l'invention, on obtient ce résultat par le fait que
l'on forme le corps, tout au moins dans la zone de l'autre extré-
mité ou extrémité de sortie, d'un certain nombre de couches coaxiales, en choisissant un matériau plus mou pour une couche
interne que pour une couche située plus vers l'extérieur, la réali-
sation de la pointe comprenant l'enlèvement de matériau de la péri-
phérie extérieure. Ainsi, lors de la réalisation de la pointe, la couche située plus vers l'intérieur, c'est-à-dire la couche plus molle, est dégagée, si bien que la pointe extrême du cathéter est formée par ce matériau mou. On obtient la raideur désirée d'au moins la partie terminale en choisissant un matériau plus raide pour la ou les couche(s) située(s) plus vers l'extérieur. La raideur totale de l'extrémité composite est ainsi amenée à la
valeur désirée.
Du fait que la pointe extrême du cathéter rendu pointue est formée par le matériau mou, le risque de blessure interne lors
de l'introduction du cathéter chez un patient est fortement réduit.
Il s'avère que l'on obtient déjà l'effet désiré dans la mesure adéquate lorsque le corps est formé, tout au moins dans la
zone de l'extrémité de sortie, de deux couches coaxiales structu-
relies. Le mode de réalisation selon cette caractéristique de l'invention est préféré pour sa simplicité. Outre les deux couches structurelLes, le cathéter peut comporter en plus une mince couche,
située le plus à l'intérieur, en matériau réduisant la friction.
Le cathéter peut être constitué de deux ou davantage de
couches coaxiales sur toute sa longueur. IL est cependant préfé-
rable qu'une portion terminale comprenant l'extrémité de sortie
soit fabriquée séparément et soit reliée à une partie de base.
Cette dernière peut alors être fabriquée de manière qu'elle possède les propriétés optimales pour ce qui concerne la tenue à la pression et la rigidité en torsion, lesquelles ont une importance
secondaire pour la partie terminale.
IL est avantageux de former la partie terminale par un processus de moulage par injection comprenant l'amenée simultanée
de plusieurs matériaux. Un tel moulage par injection est en lui-
même connu.
Si le nombre de cathéters à fabriquer est relativement important, il est préférable de former la partie terminale par le sectionnement à la longueur désirée d'un matériau de départ produit
par coextrusion en au moins deux couches et sous une longueur rela-
tivement grande. Le sectionnement peut consister en un cisaillement
ou une coupe à longueur par exemple.
L'invention concerne et procure également un cathéter
comprenant un corps tubulaire flexible, de forme allongée, possé-
dant au moins un canal central, le corps étant pourvu d'un élément de raccordement à une extrémité et présentant au moins un orifice de sortie à l'autre extrémité, formant l'extrémité de sortie, laquelle est rendue pointue, de sorte qu'elle possède, sur une portion de sa longueur, un diamètre décroissant vers la pointe extrême.

Selon l'invention, ce cathéter est caractérisé en ce qu'il est constitué, tout au moins à l'extrémité de sortie, d'un certain
nombre de couches coaxiales, comprenant une couche interne de maté-
riau plus mou que celui d'une couche située plus vers L'extérieur, OS la réalisation de la pointe comprenant l'enlèvement de la couche
située plus vers l'extérieur au droit de la pointe extrême.
D'autres caractéristiques et avantages de l'invention
ressortiront plus clairement de la description qui va suivre de
plusieurs exemples de mise en oeuvre non limitatifs, ainsi que des dessins annexés, sur lesquels: - la figure 1 est une vue en perspective schématique d'un cathéter fabriqué selon le procédé de l'invention; - la figure 2 montre schématiquement un appareil de
meulage pour illustrer une étape dans le procédé selon l'inven-
tion; - la figure 3 montre une variante de mise en oeuvre de l'étape illustrée sur la figure 2; - la figure 4 montre partiellement et schématiquement un dispositif de moulage par injection pour la mise en oeuvre du procédé selon l'invention; - les figures 5 et 6 montrent une autre variante de mise en oeuvre de ce procédé; et
- les figures 7 et 8 montrent schématiquement deux exécu-
tions différentes d'une étape du procédé selon l'invention.
Le cathéter 1 représenté sur la figure 1 est un cathéter
destiné à des examens angiographiques. Il comprend un corps 2 tubu-
laire et flexible, de forme allongée, possédant un canal central.
Un élément de raccordement 9 est fixé à une extrémité du corps 2.
L'autre extrémité est comprise dans une partie terminale 3 qui, dans le mode de réalisation illustré du cathéter, a été amenée par déformation plastique sous une forme déterminée, laquelle dépend de l'application envisagée. Le canal central débouche à l'extrémité extrême par un orifice 5. La partie terminale 3 est rendue pointue, de manière que, sur une distance 6 déterminée, elle présente un
diamètre décroissant vers la pointe extrême. Le diamètre peut dimi-
nuer de façon uniforme, comme montré, mais la décroissance peut
également être échelonnée selon un autre mode de réalisation.
Le cathéter décrit jusqu'ici est en lui-même généralement connu. Le procédé pour fabriquer un tel cathéter est également
connu généralement.
Selon un aspect important de l'invention, au moins la partie terminale est formée, près de la pointe extrême ou de l'extrémité de sortie, d'un certain nombre de couches 7, 8
coaxiales structurelles, au nombre de deux dans ce mode de réalisa-
tion; pour la couche interne 8, on a choisi un matériau plus mou que pour la couche 7, située plus vers l'extérieur, et du matériau de la périphérie extérieure a été enlevé lors de la formation de la pointe. Une couche supplémentaire 4, située le plus à l'intérieur,
est une mince couche de matériau réduisant la friction.
La figure 2 montre un exemple de réalisation de la pointe,
constituant une étape du procédé.
Cette figure montre clairement la constitution de la partie terminale 3 de plusieurs couches 7, 8, 4. Pour former la
pointe, on utilise un appareil de meulage 10, illustré schématique-
ment, qui comprend une plaque support 11 solidarisée en rotation
avec un arbre tournant 12 et portant un certain nombre, de préfé-
rence trois meules 13 qui sont chacune montées rotatives sur la plaque support 11, autour d'un axe de rotation incliné par rapport à cette plaque. Il est possible aussi d'utiliser des meules 13 de forme conique et de les monter chacune sur la plaque support 11
autour d'un axe de rotation perpendiculaire à cette plaque.
L'entraînement en rotation des meules 13 est de type planétaire. Un tel appareil de meulage est en lui-même connu et est déjà appliqué,
par exemple, pour former des pointes sur des cathéters connus.
L'appareil de meulage 10 ne sera donc pas décrit plus en détail.
Au lieu de produire une décroissance uniforme du diamètre vers l'extrémité, comme indiqué sur la figure 2, avec formation d'une surface oblique 15 en cône lors de la réalisation de la
pointe, la réduction du diamètre peut également être échelonnée.
Un tel mode de réalisation est illustré schématiquement sur la figure 3. L'extrémité du cathéter 3 est fixée dans ce cas dans un gabarit 16. Celuici est pourvu d'un mandrin 17 conçu pour pénétrer dans le canal central de l'extrémité 3' du cathéter. Le gabarit est équipé d'une meule rotative 18 dont le profile est conformé de manière que la couche 7 plus dure puisse être enlevée par meulage au droit de la pointe extrême du cathéter et que des transitions arrondies soient formées. Comme le cathéter 3 est tourné pendant le meulage, tout son pourtour est travaillé de façon uniforme. Le résultat final est une extrémité présentant une
surface 19 avec un gradin.
Il ressort des figures 2 et 3 que par la réalisation de la pointe au moyen d'un appareil à meuler, avec enlèvement de matériau du pourtour original 14, pour former la surface oblique 15 ou la surface à gradin, la couche externe 7 est enlevée complètement sur
la pointe extrême, de sorte que la couche interne 8 dépasse libre-
ment à cette extrémité.
Conformément à l'invention, on a choisi un matériau mou pour la couche interne 8, avec le résultat que la pointe extrême du cathéter est douce et que, de ce fait, lors de l'introduction chez
le patient, le risque de lésion interne est minimal.
Afin d'obtenir néanmoins la rigidité en flexion désirée sur la partie terminale 3, on choisit du matériau de plus grande raideur pour la couche externe 7 de cette partie terminale. La raideur des couches 7, 8 combinées peut ainsi être amenée à la
valeur désirée.
Le cathéter I est réatisable sur toute sa longueur d'au moins deux couches coaxiales pour certaines applications. Il est cependant préférable de former la partie terminale 3 séparément et de la relier ensuite à une partie de base. Selon un aspect préféré de l'invention, la réalisation d'une partie terminale composée d'un certain nombre de couches, peut consister en un processus de moulage par injection comprenant l'amenée d'un certain nombre de différents matériaux. La figure 4 représente schématiquement un dispositif de moulage par injection permettant l'amenée simultanée de trois matériaux pour réaliser une partie terminale à trois
couches coaxiales.
Un teL dispositif de moulage 20 comporte, de façon connue, un porte-moule 22, 23, dans lequel les deux parties d'un moule 21
sont reçues de manière qu'elles puissent être éloignées et appro-
chées l'une de l'autre dans le sens de la flèche 24. La figure 4 montre les parties du moule 21 à la position de fermeture o elles
sont appliquées l'une contre l'autre.
Les parties du moule 21 délimitent alors une empreinte de
moulage qui peut être remplie de matière plastique à l'état fluide.
Ce remplissage s'effectue à l'aide d'une tête d'injection 25.
S'agissant d'un dispositif de moulage pour l'injection simultanée de plusieurs matériaux, la tête d'injection 25 est une tête multiple, comme représenté. Elle comprend trois canaux d'injection 26, 27, 28 reliés respectivement à des raccords d'alimentation 29, et 31. En choisissant convenablement la pression et la vitesse d'introduction des divers matériaux par la tête d'injection 25, il se forme dans l'empreinte du moule 21 un objet constitué de couches superposées. Pour le procédé dont il est question ici, l'empreinte du moule possède naturellement la forme d'un canal plus long que
large et de section annulaire.
Les figures 5 et 6 montrent schématiquement un autre mode
de fabrication pour au moins la partie terminale.
Dans ce cas, on produit séparément un tube ou tuyau 33 en
matériau mou pour la couche interne et un tuyau externe 34 de maté-
riau plus dur pour la couche externe. Le tuyau 34 est fabriqué avec
un diamètre intérieur légèrement plus petit que le diamètre exté-
rieur du tuyau 33.
Le tuyau 34 est ensuite chauffé puis dilaté à chaud. Apres cela, il est refroidi à l'état dilaté, de sorte qu'il est "gelé" à
cet état dilaté.
Après introduction du tuyau 33 dans le tuyau 34, on fait - passer les deux tuyaux ainsi assemblés à travers un gabarit chauffant 35, ce qui porte la couche externe 34 à une température telle qu'elle est de nouveau "dégelée" et qu'elle reprend sa forme originale. Le matériau à cathéter 36 quittant le gabarit chauffant 35, comporte par conséquent deux couches coaxiales dont celle située à l'intérieur est faite d'un matériau plus mou que la couche externe. Comme cette dernière enserre avec force La couche
interne, on obtient ainsi un ensemble cohérent.
En cas d'application d'un mode de mise en oeuvre du procédé selon l'invention avec fabrication séparée de la partie
terminale, constituée d'au moins deux couches, cette partie termi-
nale doit être fixée ensuite à une partie de base. Celle-ci peut ne
pas comporter de couche en matériau plus mou.
Cette étape du procédé est illustrée schématiquement sur les figures 7 et 8. Le cathéter 40 de cet exemple est composé d'une partie de base 45 et d'une partie terminale 41. Cette dernière et le bout de la partie de base 45 sont enfilés sur un mandrin 51, de manière que leurs faces d'extrémité adjacentes 42 et 43 soient appliquées l'une contre l'autre. On introduit ensuite l'ensemble ainsi obtenu dans un gabarit chauffant 50, de manière que les faces d'extrémité 42, 43 se trouvent au milieu du gabarit. En fermant le circuit électrique pour le chauffage du gabarit 50, le matériau de la partie de base 45 et celui de la partie terminale 41 sont portés à l'état de fusion, de sorte que les deux parties sont soudées ensemble. Apres le refroidissement, elles forment un cathéter d'un
seul tenant.
La figure 8 montre une deuxième possibilité. La partie terminale 41 est enfilée dans ce cas sur un mandrin 54 fixé à un support rotatif 53. Le mandrin dépasse de la partie terminale 41 et le bout de la partie de base 45 est emboîté sur l'extrémité saillante du mandrin. Lorsqu'on fait ensuite tourner le support 53 et avec lui la partie terminale 41 que l'on exerce sur la partie
de base 45 une pression vers la droite, sur la figure 8, en.
empêchant la partie de base de tourner, le frottement produit au niveau des surfaces en contact dégage une telle quantité de chaleur que les deux parties fondent dans la zone de contact et se réunissent. Pour guider et enfermer le matériau en fusion, on utilise un gabarit stationnaire 52. La force de pression à exercer sur la partie de base 45 et la durée pendant laquelle le support 53 doit être entraîné en rotation, sont simplement déterminées

expérimentalement. La soudure obtenue entre La partie terminale et La partie de base est soumise à une finition afin d'obtenir une transition uniforme de La partie de base 45 à La partie terminale 41. La formation de la pointe s'effectuera habitueLLement après La
fixation de la partie terminale 41 à la partie de base 45.
Claims (8)
1. Procédé pour fabriquer un cathéter, comprenant la fabrication d'un corps (2) tubulaire flexible, de forme aLlongée, possédant au moins un canal central, la mise en place d'un élément de raccordement (9) à une extrémité du corps et la réalisation d'une pointe sur une portion de l'autre extrémité, de manière que cette portion soit pourvue d'un diamètre décroissant vers la pointe extrême, carac<érisé en ce que l'on forme le corps (2), tout au moins dans la zone de l'autre extrémité ou extrémité de sortie, d'un certain nombre de couches (7, 8) coaxiales, en choisissant un matériau plus mou pour une couche interne (8) que pour une couche (7) située plus vers l'extérieur, la réalisation de la pointe
comprenant l'enlèvement de matériau de la périphérie extérieure.
2. Procédé selon la revendication 1, caractérisé en ce que l'on forme le corps (2), tout au moins dans la zone de l'extrémité
de sortie, de deux couches (7, 8) coaxiales structurelles.
3. Procédé selon la revendication 1 ou 2, caractérisé en
ce que l'on forme séparément une partie terminale (3; 41) compre-
nant l'extrémité de sortie et on relie cette partie terminale à une
partie de base (45).
4. Procédé selon la revendication 3, caractérisé en ce que l'on forme la partie terminale (3) par un processus de moulage par injection comprenant l'amenée simultanée d'un certain nombre de
matériaux.
5. Procédé selon la revendication 4, caractérisé en ce que l'on forme la partie terminale (3) par le sectionnement à la longueur voulue d'un matériau de départ fabriqué par coextrusion en
au moins deux couches et sous une longueur relativement grande.
6. Cathéter comprenant un corps (2) tubulaire flexible, de forme allongée, possédant au moins un canal central, le corps étant
pourvu d'un élément de raccordement (9) à une extrémité et présen-
tant au moins un orifice de sortie (5) à l'autre extrémité, formant l'extrémité de sortie, laquelle est rendue pointue, de sorte qu'elle possède, sur une portion de sa longueur, un diamètre décroissant vers la pointe extrême, caractérisé en ce qu'il est constitué, tout au moins à l'extrémité de sortie, d'un certain nombre de couches (7, 8) coaxiales, comprenant une couche interne (8) de matériau plus mou que celui d'une couche (7) située plus
vers l'extérieur, la réalisation de la pointe comprenant l'enlève-
ment de la couche (7) située plus vers l'extérieur au droit de la
pointe extrême.
7. Cathéter selon la revendication 6, caractérisé en ce que l'extrémité de sortie (3; 41) est constituée de deux couches coaxiales.
8. Cathéter selon la revendication 6 ou 7, caractérisé en ce qu'il est composé d'une partie de base (45) et d'une partie terminale (3; 41) qui est reliée à la partie de base et que seule la partie terminale possède une couche interne de matériau plus mou.
