Description
1. La présente invention est relative à des articles
polymères modifiés par l'acide hyaluronique ("AH") et pré-
sentant une biocompatibilité améliorée qui permet à ces
articles modifiés d'être utilisés dans de nombreuses appli-
cations in vivo, par exemple pour des dispositifs variés de prothèse, notamment les valves cardiaques artificielles,
les greffes vasculaires, etc...
L'acide hyaluronique est une matière connue qui se
rencontre naturellement et qui est susceptible de nombreu-
ses applications en médecine et en biologie. Par exemple, on pourra se référer au brevet américain N 4 272 522 et
aux documents cités dans celui-ci.
Comme indiqué précédemment, la présente invention
concerne des articles polymères modifiés par l'acide hyalu-
ronique, cette substance étant normalement présente dans
les tissus animaux, pour conférer à de tels articles un de-
gré considérablement plus élevé de biocompatibilité que les
articles non modifiés. La présente invention vise les arti-
cles polymères tels que les produits moulés, les pellicules,
les fibres et les tissus qui contiennent de l'acide hyalu-
ronique ou l'un de ses sels dispersé dans tout l'article et/ou comme revêtement sur la surface de celui-ci. L'acide
hyaluronique peut également être réticulé apres la prépa-
ration de l'article. Les articles polymères ainsi modifies sont par conséquent tout à fait appropriés à être utilisés dans des dispositifs varies de prothèse tels que les valves cardiaques artificielles, les lentilles intra-oculaires,
les greffes vasculaires, les conducteurs pour appareils ré-
gulateurs cardiaques et autres, de même que dans divers ty-
pes de dispositifs distributeurs de médicaments.
Les polymères qui peuvent être modifiés par l'acide hyaluronique, conformément à la présente invention, sont très nombreux et comportent notamment:
1) les polyuréthanes, notamment les poly6therurêtha-
nes et les polyesteruréthanes aliphatiques, aromatiques et araliphatiques; 2) les polyoléfines, notamment le polyethylene; 3) les polymères de vinyle, notament le chlorure de 2.
polyvinyl, l'alcool polyvinylique et l'acétate polyviny-
lique, et leurs copolymères;
4) les polymères acryliques, notamment le méthacry-
late poly-hydroxyéthylique et les copolymères d'acrylate de méthyle et de méthacrylate de méthyle; ) les polyamides; 6) les polyesters;
7) les polysiloxanes.
Il existe de nombreux procédés pour introduire l'aci-
de hyaluronique dans un polymère ou pour appliquer celui-
ci à la surface d'un polymère. Un procédé consiste à dis-
soudre l'acide hyaluronique dans un solvant approprié et
à mélanger la solution obtenue avec une solution du poly-
mère ou une émulsion et, ensuite, à constituer un article,
par exemple une pellicule, à partir du mélange, ou a appli-
quer ce mélange sous la forme d'un revêtement. Les solvants
appropriés pour dissoudre l'acide hyaluronique sont cons-
titués par l'eau, le diméthylsulfoxyde et la diméthylfor-
mamide. La surface du substrat polymère peut être activée,
par exemple par l'acide hyaluronique.
Un autre procédé consiste à ajouter l'acide hyaluro-
nique sous forme solide, de préférence sous la forme d'une poudre, à une solution du polymère, et ensuite à former
un article, par exemple une pellicule, à partir du mélan-
ge obtenu ou à appliquer ce mélange sous la forme d'un re-
vêtement.
Un autre procédé encore consiste à revêtir une matiè-
re particulaire avec de l'acide hyaluronique à partir d'une solution et à introduire les particules ainsi revêtues dans une solution du polymère et ensuite a former un article, par exemple une pellicule, à partir du mélange obtenu ou à appliquer ce mélange sous la forme d'un revêtement. Des exemples de matières particulaires sont constitués par les
résines, la silice, l'alumine, etc.. à échange d'ions.
Enfin, dans chacun des procédés décrits ci-dessus, l'acide hyaluronique peut être réticulé avant ou après le mélange avec le polymère, en utilisant des agents variés
de réticulation.

Les exemples ci-après, dans lesquels tous les pour-
centages sont exprimés en poids, sauf indication contraire, illustrent divers modes de mise en oeuvre de l'invention, sans toutefois en constituer une limitation, l'invention
étant seulement limitée par les revendications annexées.
Dans les exemples 1 à 4, des articles en polyurétha-
ne améliorés ont été préparés et consistent en une couche
de polyuréthane et en une ou deux couches d'un mélange d'a-
cide hyaluronique avec un autre polymère ou des autres poly-
mères pour la formation d'une pellicule élastique.
Exemple 1
Une pièce de pellicule de caoutchouc polyétherurétha-
ne a été activée par trempage dans un bain de diméthylfor-
mamide pendant 10 secondes, puis lavée dans un mélange eau/ acetone (1/30 en volume) pendant une minute puis à l'eau
pendant une minute.
Un mélange contenant de l'acide hyaluronique a été préparé de la manière suivante: 31 g d'une solution aqueuse à i % du sel de sodium de l'acide hyaluronique (préparé à partir de crêtes de coq, contenant 1,15 % de protéines et
présentant une viscosité intrinsèque n de 2 700)ont été.
dilués avec 60 g d'eau puis mélangés à 6,7 g d'une émulsion aqueuse de polyuréthane aliphatique (produit commercialisé
par la Société POLYVINYL CHEMICAL INDUSTRIES sous la réfé-
rence Néo Rez R962).On a ainsi obtenu un mélange transluci-
de et modérément visqueux, contenant 2,96 % de matières so-
lides, le rapport entre l'acide hyaluronique et le polyuré-
thane étant d'environ 1/4. La pellicule de polyuréthane ac-
tivé a été plongéedans le mélange ainsi préparé puis séchée à l'air pendant 30 minutes et, finalement,dans un four à
vide à 55 C et sous une pression de5,7kPa pendant 30 mn.
L'article obtenu est une couche de pellicule de caoutchouc de polyétheruréthane revêtue des deux côtés d'une pellicule
continue consistant en polyuréthane aliphatique et en hyalu-
ronate de sodium.
Exemple 2
Les opérations de l'exemple 1 ont été répétées,à la
différence que 0,3 g d'un composé d'aziridine polyfonction-

nelle (commercialisé par la Société POLYVINYL CHEMICAL IN-
DUSTRIES, sous la référence CX-100) a été ajouté.
L'article obtenu consistait en une couche d'une pel-
licule de caoutchouc de polyétheruréthane revêtue des deux côtés d'une pellicule continue consistant en polyétheruré-
thane aliphatique et en hyaluronate de sodium réticulé.
Exemple 3
Une tige en verre a été revêtue par trempage dans une solution à 10 % de polyétheruréthane (commercialisé
par la Société THE UPJOHN COMPANY, sous la référence Pel-
lethane 80 AE) dans de la diméthylformamide.La pellicule polymère a été coagulée dans un mélange eau/acetone d'une proportion de 3/1 en volume. La pellicule présente sur la
tige a été séchée dans un four à 80 pendant 30 mn. La se-
conde couche a été appliquée par trempage dans le mélange
polyuréthane - hyaluronate de sodium selon l'exemple 1.
L'article tubulaire obtenu consistait en deux cou-
ches dont l'une était une couche de polyétheruréthane et l'autre était une pellicule de mélange d'un polyuréthane aliphatique et d'un hyaluronate de sodium réticulé
Exemple 4-
On a dissout 1,09 g de hyaluronate de sodium séché
à l'air dans un mélange de 4,4 g d'eau et de 67 g de dimé-
thylformamide. On a mélangé 10 g de la solution obtenue avec 10 g d'une solution à 25 % d'un polyétherulréthane
(THE UPJOHN COMPANY, Pellethane 80 AE) dans de la diméthyla-
cetamide. La teneur de cette solution en matières solides était de 13,66 %, le rapport entre le hyaluronate de sodium et le polyuréthane étant de 1/40. Une tige de verre a été trempée dans cette solution visqueuse. Les polymères ont
été coagulés dans un mélange eau/acetone de 3/1 en volume.
La pellicule tubulaire a été lavée dans l'eau pendant 10 mn puis séchée dans un four à vide à 55 pendant 45 mn sous
une pression de 6 kPa.
Les exemples suivants illustrent l'application d'un revêtement à base d'acide hyaluronique sur un tissu textile en polyester (exemple 5) ou sur une pellicule en chlorure
de polyvinyle(exemple 6).

Exemple 5
Une solution contenant 1,5 % d'acide hyaluronique et 1 % d'agent de réticulation CX-100 (voir exemple 2) a été préparée. Un morceau de tissu textile tricoté en Dacron (marque déposée) a été plongé dans cette solution pendant 2 mn. L'excès de solution a été erlevé et l'échantillon a
été séché pendant une nuit. La teneur en acide hyaluroni-
que dans le tissu revêtu a été de 3,7 %.
Exemple 6
Une solution aqueuse de hyaluronate de sodium a été
ajoutée à une émulsion aqueuse acrylique de vinyle (commer-
cialiseepar la Société UNION CHEMICALS DIVISION sous la ré-
férence Amsco-Res 9205) pour obtenir un mélange contenant 0,25 % d'acide hyaluronique. Le mélange a été appliqué par
trempage sur une pellicule de chlorure de polyvinyleet sé-
ché à l'air pendant 2 heures. La pellicule ainsi revêtue
a été traitée dans une solution à 1 % d'agent de réticu-
lation CX-100 dans un mélange eau/acetone contenant 20 %
en volume d'acétone, puis séchée à l'air pendant 3 heures.
On a obtenu une pellicule avec un revêtement d'une bonne
adhérence contenant de l'acide hyaluronique.
Les exemples suivants illustrent un autre aspect d'ob-
tention d'une pellicule de polyuréthane contenant un hyalu-
ronate de sodium, c'est-à-dire en introduisant un hyaluro-
nate de sodium sous la forme d'une poudre finement divisée
dans une solution de polyuréthane, puis par moulage du mé-
lange et précipitation de la pellicule par l'agent de coagu-
lation approprié.
Exemple 7
0,33 g de hyaluronate de sodium, de viscosité intrin-
sèque 3 160, sous la forme d'une poudre finement divisée a été ajouté à 10,69 g d'une solution depolyurétheruréthane/
UPJOHN COMPANY, Peliethane 80 AE) dans de la diméthylacéta-
mide (concentration 16,4 %). Le mélange a été étalé sur une plaque de verre, à l'aide d'une tige en verre, et introduit
dans 100 ml d'alcool éthylique à 95 %. La pellicule de pré-
cipitation, d'aspect blanc laiteux, a été laissée dans l'al-
cool pendant 10 mn, puis enlevée du bain, lavée deux fois 6.
avec de l'éthanol et séchée à l'air. La pe]licu-]e élasti-
que obtenue contenait 9,35 % de hyaluronate de sodium.
L'exemple suivant illustre une variante de la pelli-
cule obtenue selon l'exemple 7, pour obtenir la formation d'une pellicule d'acide hyaluronique réticulé sur la sur- face de la pellicule. Le traitement consiste à placer la
pellicule dans une solution aqueuse de l'agent de réticu-
lation qui réticule l'acide hyaluronique lors du séchage.
Dans ce traitement, les particu3es d'acide hyaluronique
fondent et se dissolvent partiellement dans l'eau pour mi-
grer à la surface de la pellicule. Il en résulte la for-
mation d'une couche d'une solution d'acide hyaluronique
concentré à la surface. Lors du séchage, l'acide hyaluro-
nique est réticulé par l'agent de réticulation et, de cet-
te manière, une couche d'acide hyaluronique réticulé se
forme à la surface de la pellicule.
Exemple 8
La pellicule obtenue à l'exemple 7 a été plongée
dans une solution d'un composéd'aziridine polyfonctionnel-
le dans de l'eau (0,5 % en poids, agent de réticulation CX-
de la Société POLYVINYL CHEMICAL INDUSTRIES) pendant une minute, puis enlevée de la solution et séchée à l'air
pendant 2 heures. La pellicule a ensuite été lavée de ma-
nière répétée dans l'eau puis séchée. La pellicule obtenue
présentait une couche continue d'acide hyaluronique réti-
culé à sa surface, ce qui a été prouvé en teintant la pel-
licule dans une solution aqueuse de bleu d'orthotoluidine.
L ' exemple suivant illustre une variante de
l'approche ci-dessus décrite. Dans cette variante, une ma-
tière consistant en petites particules, par exemple une
résine, silice, alumine, etc.. à échange d'ions a été re-
vêtue d'acide hyaluronique et ces particules ainsi revêtues
ont été introduites dans un polyuréthane.
- Exemple 9
5 g d'une résine à échange d'ions (Dowex 50W-2x), d'une taille de 0,035 à 0,075nmm,ont été mélangés avec 10,6 g d'acide hyaluronique dans de l'eau, à raison de 7,2 mg/ ml. Le mélange a été séché dans un four à vide. Le produit 7. obtenu contenait 1,5 % d'acide hyaluronique. 0,5 g de la résine revêtue a été mélangé avec 5 g de la solution de
polyuréthane utilisée dans l'exemple précédent. La pelli-
cule a été précipitée et traitée comme ci-dessus. La te-
neur de la pellicule en acide hyaluronique était de 0,38 %.
L'exemple suivant illustre une variante des proprié-
tés de surface du caoutchouc de silicone à l'aide d'un re-
vêtement contenant un hyaluronate.
Exemple 10
Un morceau de caoutchouc de silicone (commercialisé par la Société DOW CORNING sous la référence Silastic) a été plongé dans une solution à 10 % de silane contenant une amine (UNION CARBIDE, référence Silane A-1l00) dans du toluène, séché pendant 10 mn à l'air puis dans un four
à 609cpendant 10 mn. Le caoutchouc ainsi traité a été trem-
pé dans une solution contenant 0,5 % de hyalurunate de so-
dium et 0,12 % d'agent de réticulation CX-100. L'échantil-
lon a été séché à l'air puis dans un four à 70 C pendant
mn. L'échantillon modifié présentait une surface mouil-
'able. Cette propriété était permanente comme cela a été
prouvé par lavages répétés de l'échantillon avec de l'eau.
La biocompatibilité des préparations selon l'inven-
tion a été démontrée par l'essai décrit ci-après.
Exeml - Test de compatibilité avec le sang
M X L- 1 {
La fourniture de 3H - sérotonine par des plaquettes
sanguines humaines a été utilisée dans des études prélimi-
naires pour établir la réactivité du produit selon l'exem-
ple 5 avec le sang. Le sang veineux humain normal a été
prélevé dans des seringues en matière plastique et trans-
féré immédiatement dans des tubes en matière plastique con-
tenant 3,8 % de citrate de sodium, à raison d'une partie
de citrate pour 9 parties de sang complet. Du plasma ri-
che en plaquettes a été préparé par centrifugation à 4 C pendant 15 mn, sous une accélération centrifuge de 125 x g,
puis extrait à l'aide d'une pipette sérologique et trans-
féré dans un tube d'essai en matière plastique ou aà base
de silicone. On a ajouté 3E sérotonine (3H-5-hydroxytryp-
tamine, ou 3H-5HT, de la Société NEW ENGLAND NUCLEAR, con-

tenant 26,3 Ci/mmole,lmCi/ml éthanol-eau) à du plasma ri-
puis. che en plaquettes, à raison de 0,2 à 0,5 V 1/ml de plasma,/ fait incuber pendant 15 mn à 37 C. Dans cet essai, on a
utilisé des tubes de test à base de silicone ou en polypro-
pylène; on a utilisé des caillots comme moyen de contrôle
positif, et on a testé des échantillons revêtus et non re-
3 de x vêtus. 1-2ml de 3H-5HT-plasma ont été ajoutés à chacun e8e/ tubes contenant les échantillons à tester; une partie aliquote de 50 l 1 a été enlevée du mélange de contr1ôle pour
la détermination de la radioactivité totale. Apres la pé-
riode d'incubation appropriée (10 à 120 mn), des parties aliquotes de 0,2 - 0,5 ml de la suspension ont été enlevées et centrifugées sur une huile au silicone dans un appareil
Eppendorf pendant 2 mn sous une accélération de 12 000 x g.
50 V 1 du produit surnageant ont été enlevés de chacun des tubes, mélangés avec 5 ml d'un liquide de scintillation et on a mesuré la radioactivité par béta-spectrométrie. La
quantité de3 H-5HT libérée par les caillots ou les échan-
tillons de test constituait l'incrément de radioactivité du
produit surnageant (radioactivité des échantillons expéri-
mentaux moins la radioactivité de commande). Les échantil-
lons revêtus du produit de l'exemple 5 étaient notablement moins réactifs en termes de quantité de H-5HT libéré; les échantillons non revêtus conduisaient à une libération plus
grande de 52 % par rapport aux échantillons revêtus.
Bien entendu l'invention n'est pas limitée à la des-
cription et aux exemples ci-dessus; on pourrait au contrai-
re concevoir diverses variantes sans sortir pour autant de
son cadre.

Claims (8)
1.Composition de matière, caractérisée par le fait qu'elle est constituée par une matière polymère modifiée
par l'inclusion d'acide hyaluronique ou de l'un de ses sels.
2. Composition selon la revendication 1, caractéri-
sée par le fait que la matière polymère appartient au grou-
pe constitué par les: polyuréthane, notamment polyétheruré-
thane ou polyesteruréthane aliphatique, aromatique ou arali-
phatique, polyoléfine, polymère ou copolymère.- vinylique, notamment chlorure de polyvinyle,polymère ou copolymère
acrylique, polyamide, polyester ou polysiloxane.
3. Composition selon l'une des revendications 1 et 2,
caractérisée par le fait que l'acide hyaluronique est lié
de manière covalente à la surface de la matière polymère.
4. Composition selon l'une des revendications 1 à 3,
caractérisée par le fait que l'acide hyaluronique est dis-
tribué dans la structure ou à la surface de la matière poly-
mère.
5. Composition selon l'une des revendications 1 à 3,
caractérisée par le fait que l'acide hyaluronique ou son
sel constitue un revêtement de surface pour la matière po-
lymère. 6. Composition selon la revendication 5, caractérisée
par le fait que l'acide hyaluroniqle à la surface de la ma-
tière polymère est réticulé.
7. Article caractérisé par le fait qu'il est consti-
tué par la composition selon l'une des revendications 1 à
4. 8.Article caractérisé par le fait qu'il est revêtu
par la composition suivant l'une des revendications 5 et 6
9. Article selon l'une des revendications 7 et 8, ca-
ractérisé par le fait qu'il se présente sous la forme d'un produit moulé, d'une pellicule, d'une fibre ou d'un tissu textile.
