Cuando la segunda mordaza 102 está en la posición sujeta, la primera superficie 121 del yunque 111 y la segunda superficie 122 del cartucho de grapas 113 están en alineación yuxtapuesta y definen un espacio de tejido máximo predeterminado “G” (véase la Figura 13).
Mover la segunda mordaza 102 con respecto a la primera mordaza 101 desde la posición abierta a la posición sujeta mientras se mantiene continuamente la alineación paralela entre la primera superficie 121 del yunque 111 y la segunda superficie 122 del conjunto de cartucho 112 aumenta la capacidad del operador para lograr una fuerza de compresión de tejido deseada en el espacio de tejido “G” . Además, el pellizcado del tejido que puede ocurrir alrededor del sitio de un punto de pivote entre un yunque y un cartucho en un conjunto de herramienta convencional puede minimizarse. El mantenimiento continuo de la primera superficie 121 del yunque 111 y la segunda superficie 122 del conjunto de cartucho 112 en alineación paralela aplica una fuerza de compresión distribuida más uniformemente al tejido durante la sujeción, en comparación con un conjunto de herramienta convencional que emplea un punto de pivote entre el yunque 111 y el conjunto de cartucho 112.
Las Figuras 3-7 ilustran la segunda mordaza 102 que define un primer orificio roscado 201 y un segundo orificio roscado 202. El miembro de accionamiento 331 tiene una porción de extremo distal 333 y una porción de extremo proximal 332. La porción de extremo distal 333 soporta una cremallera 203 que tiene un primer lado y un segundo lado. La cremallera 203 define una pluralidad de dientes en cada uno de los lado primero y lado segundo de la cremallera 203 que están configurados para acoplarse engranajes formados en un piñón correspondiente. Un conjunto de accionamiento de aproximación 204 (Figura 3) incluye un primer tornillo de avance 205, un segundo tornillo de avance 206, un primer piñón 207 y un segundo piñón 208. El primer tornillo de avance 205 se recibe dentro del primer orificio roscado 201 y el segundo tornillo de avance 206 se recibe dentro del segundo orificio roscado 202. Un primer piñón 207 se soporta en el primer tornillo de avance 205 y el segundo piñón 208 se soporta en el segundo tornillo de avance 206. El primer piñón 207 se acopla al primer lado de la cremallera 203 y el segundo piñón 208 se acopla al segundo lado de la cremallera 203. El primer tornillo de avance 205 y el segundo tornillo de avance 206 están fijados de manera giratoria a la primera mordaza 101. El movimiento del miembro de accionamiento 331 desde la posición retraída a la posición avanzada avanza la cremallera 203 para girar el primer piñón 207 y el segundo piñón 208. La rotación del primer piñón 207 gira el primer tornillo de avance 205 dentro del primer orificio roscado 201 y la rotación del segundo piñón 208 gira el segundo tornillo de avance 206 dentro del segundo orificio roscado 202 para mover la segunda mordaza 102 en relación con la primera mordaza 101 desde la posición abierta a la posición sujeta.
El primer orificio roscado 201 puede formarse al menos en parte en una primera tuerca 209 que está sujeta de manera fija a la segunda mordaza 102. De manera similar, el segundo orificio roscado 202 puede formarse al menos en parte en una segunda tuerca 210 sujeta de manera fija a la segunda mordaza 102.
La primera mordaza 101 soporta un primer pasador de guía 211 (Figura 7) y un segundo pasador de guía 212. La segunda mordaza 102 define un primer orificio 213 y un segundo orificio 214. El primer orificio 213 está alineado con el primer pasador de guía 211 y el segundo orificio 214 está alineado con el segundo pasador de guía 212. El primer pasador de guía 211 incluye una primera porción de extremo 215 colocada parcialmente en el primer orificio 213 cuando la segunda mordaza 102 está en la posición abierta con respecto a la primera mordaza 101 (Figura 7). La primera porción de extremo 215 del primer pasador de guía 211 se desliza adicionalmente en el primer orificio 213 a medida que la segunda mordaza 102 se aproxima a la posición cerrada con respecto a la primera mordaza 101 (Figura 11). El segundo pasador de guía 212 incluye una segunda porción de extremo 216 colocada parcialmente en
el segundo orificio 214 cuando la segunda mordaza 102 está en la posición abierta con respecto a la primera mordaza 101 (Figura 7). La segunda porción de extremo 216 del segundo pasador de guía 212 se desliza adicionalmente dentro del segundo orificio 214 a medida que la segunda mordaza 102 se aproxima a la posición cerrada con respecto a la primera mordaza 101 (Figura 11). El primer pasador de guía 211 y el segundo pasador de guía 212 mantienen la alineación paralela y evitan la rotación relativa entre la primera mordaza 101 y la segunda mordaza 102.
La porción distal 333 del miembro de accionamiento 331 soporta un extremo de trabajo (por ejemplo, barra en I 334) que define el espacio de tejido máximo “G” . La barra en I 334 tiene una primera barra 335, una segunda barra 336 y un puntal vertical 337 que conecta la primera barra 335 con la segunda barra 336. El miembro de accionamiento 331 es móvil desde una posición retraída (véase, por ejemplo, la Figura 5) hacia una posición parcialmente avanzada para mover el conjunto de herramienta desde la posición abierta a la posición sujeta (véase, por ejemplo, la Figura 9), y es móvil desde la posición parcialmente avanzada hasta una posición totalmente avanzada (véase, por ejemplo, la Figura 13) para disparar grapas desde el conjunto de cartucho 112. La posición parcialmente avanzada del miembro de accionamiento 331 (Figura 9) está definida por una porción de extremo distal 339 de la cremallera 203. El miembro de accionamiento 331 tiene una superficie plana 340 en lados opuestos del mismo entre la porción extrema distal 339 de la cremallera 203 y la porción extrema proximal 332 del miembro de accionamiento 331. Por lo tanto, hacer avanzar el miembro de accionamiento 331 desde la posición parcialmente avanzada (Figura 9) a la posición totalmente avanzada (Figura 13) no gira el primer piñón 207 ni el segundo piñón 208. Cuando el miembro de accionamiento 331 se hace avanzar desde la posición retraída (Figura 5) a la posición parcialmente avanzada (Figura 9), el segundo miembro de mordaza 102 se aproxima completamente a partir de la configuración abierta con respecto al primer miembro de mordaza 101 (Figura 5) en la posición aproximada con respecto al primer miembro de mordaza 101 (Figura 9) por rotación del primer piñón 207 y del segundo piñón 208. A medida que el miembro de accionamiento 331 se mueve adicionalmente a través del conjunto de herramienta 16 desde la posición parcialmente avanzada (Figura 9) a la posición totalmente avanzada (Figura 13), la primera barra 335 se acopla a la primera mordaza 101 y la segunda barra 336 se acopla a la segunda mordaza 102 para impedir el movimiento hacia fuera de las mordazas primera y segunda 101 y 102 más allá de una distancia preestablecida para definir el espacio de tejido máximo “G” . Cuando el miembro de accionamiento 331 alcanza la posición completamente avanzada (Figura 13), se han disparado todas las grapas del conjunto de cartucho 112.
La barra en I 334 puede tener una superficie orientada distalmente que define una cuchilla 338 que tiene un borde afilado configurado para cortar tejido.
Los diversos aspectos del dispositivo de grapado descritos en esta invención también pueden configurarse para trabajar con sistemas quirúrgicos robóticos y lo que se conoce comúnmente como “telecirugía” . Dichos sistemas emplean diversos elementos robóticos para ayudar al cirujano y permitir la operación remota (u operación remota parcial) de instrumentación quirúrgica. Para este propósito pueden emplearse diversos brazos robóticos, engranajes, levas, poleas, motores eléctricos y mecánicos, etc., y pueden diseñarse con un sistema quirúrgico robótico para ayudar al cirujano durante una operación o tratamiento. Dichos sistemas robóticos pueden incluir sistemas conducidos de forma remota, sistemas quirúrgicos automáticamente flexibles, sistemas quirúrgicos flexibles de forma remota, sistemas quirúrgicos articulados de forma remota, sistemas quirúrgicos inalámbricos, sistemas quirúrgicos operados de forma remota modulares o selectivamente configurables, etc.
Los sistemas quirúrgicos robóticos pueden emplearse con una o más consolas que están junto a la sala de operaciones o situadas en una ubicación remota. En este caso, un equipo de cirujanos o enfermeras puede preparar al paciente para cirugía y configurar el sistema quirúrgico robótico con uno o más de los instrumentos descritos en esta invención, mientras que otro cirujano (o grupo de cirujanos) controla de forma remota los instrumentos a través del sistema quirúrgico robótico. Como puede apreciarse, un cirujano altamente experto puede realizar múltiples operaciones en múltiples ubicaciones sin abandonar su consola remota, lo que puede ser económicamente ventajoso y un beneficio para el paciente o para una serie de pacientes.
Los brazos robóticos del sistema quirúrgico se acoplan típicamente a un par de mangos maestros por un controlador. Los mangos pueden moverse por el cirujano para producir un movimiento correspondiente de los extremos de trabajo de cualquier tipo de instrumento quirúrgico (por ejemplo, efectores de extremo, pinzas, cuchillas, tijeras, etc.) que pueden complementar el uso de uno o más de los aspectos de la divulgación descrita en esta invención. El movimiento de los mangos maestros puede escalarse para que los extremos de trabajo tengan un movimiento correspondiente que sea diferente, menor o mayor, que el movimiento realizado por las manos operativas del cirujano. El factor de escala o relación de engranajes puede ser ajustable para que el operador pueda controlar la resolución de los extremos de trabajo del o de los instrumentos quirúrgicos.
Los mangos maestros pueden incluir varios sensores para proporcionar retroalimentación al cirujano relacionada con diversos parámetros o condiciones del tejido, por ejemplo, resistencia del tejido debido a la manipulación, corte o tratamiento de otra manera, presión por el instrumento sobre el tejido, temperatura del tejido, impedancia del tejido, etc. Como puede apreciarse, tales sensores le proporcionan al cirujano una retroalimentación táctil mejorada que simula las condiciones reales de operación. Los mangos maestros también pueden incluir una variedad de
actuadores diferentes para la manipulación o el tratamiento de tejidos delicados potenciando aún más la capacidad del cirujano para imitar las condiciones reales de operación.
Debe entenderse que diversos aspectos divulgados en esta invención pueden combinarse de manera diferentes que las combinaciones presentadas específicamente en la descripción y los dibujos adjuntos. También debe entenderse que, dependiendo del ejemplo, ciertos actos o eventos de cualquiera de los procesos o métodos descritos en esta invención pueden realizarse en una secuencia diferente, se pueden añadir, fusionar o salir por completo (por ejemplo, todos los actos o eventos descritos pueden no ser necesarios para llevar a cabo las técnicas). Claims (9)
- REIVINDICACIONESi. Un conjunto de herramienta (16) para un dispositivo de grapado quirúrgico (10), que comprende:una primera mordaza (101) que tiene una primera superficie (121);una segunda mordaza (102) que tiene una segunda superficie (122) orientada hacia la primera superficie, la segunda mordaza móvil en relación con la primera mordaza entre una posición abierta y una posición sujeta;un miembro de accionamiento (331) que tiene una porción de extremo distal (333), y un conjunto de accionamiento de aproximación (204), el conjunto de herramienta caracterizado porque:la segunda mordaza define un primer y segundo orificios roscados (201, 202); la porción de extremo distal del miembro de accionamiento soporta una cremallera (203) que tiene un primer lado y un segundo lado, el miembro de accionamiento puede moverse desde una posición retraída a una posición avanzada; y el conjunto de accionamiento de aproximación (204) que incluye un primer tornillo de avance (205) recibido dentro del primer orificio roscado, un segundo tornillo de avance (206) recibido dentro del segundo orificio roscado, un primer piñón (207) soportado en el primer tornillo de avance, y un segundo piñón (208) soportado en el segundo tornillo de avance, el primer piñón se acopla al primer lado de la cremallera y el segundo piñón se acopla al segundo lado de la cremallera, en donde el primer tornillo de avance y el segundo tornillo de avance están fijados de manera giratoria a la primera mordaza,en donde el movimiento del miembro de accionamiento desde la posición retraída a la posición avanzada avanza la cremallera para girar el primer piñón y el segundo piñón, y en donde girar el primer piñón gira el primer tornillo de avance dentro del primer orificio roscado y girar el segundo piñón gira el segundo tornillo de avance dentro del segundo orificio roscado para mover la segunda mordaza en relación con la primera mordaza desde la posición abierta a la posición sujeta.
- 2. El conjunto de herramienta de la reivindicación 1, en donde la segunda superficie de la segunda mordaza se mantiene en alineación sustancialmente paralela con la primera superficie de la primera mordaza a medida que la segunda mordaza se aproxima desde la posición abierta a la posición sujeta.
- 3. El conjunto de herramienta de la reivindicación 2, en donde la primera mordaza soporta un primer y segundo pasadores de guía (211, 222) y la segunda mordaza define un primer y segundo orificios (213, 214) que están alineados con el primer y segundo pasadores de guía, en donde el primer pasador de guía y el segundo pasador de guía están configurados para mantener un alineamiento sustancialmente paralelo entre la primera superficie de la primera mordaza y la segunda superficie de la segunda mordaza.
- 4. El conjunto de herramienta de cualquiera de las reivindicaciones anteriores, en donde el primer orificio roscado se forma al menos en parte en una primera tuerca (209) sujeta de manera fija a la segunda mordaza, y el segundo orificio roscado se forma al menos en parte en una segunda tuerca (210) sujeta de manera fija a la segunda mordaza.
- 5. El conjunto de herramienta de cualquiera de las reivindicaciones anteriores, en donde el miembro de accionamiento incluye un extremo de trabajo en la porción de extremo distal del miembro de accionamiento, el extremo de trabajo configurado para mantener una distancia máxima entre la primera superficie de la primera mordaza y la segunda superficie de la segunda mordaza cuando la segunda mordaza está en la posición sujeta.
- 6. El conjunto de herramienta de la reivindicación 5, en donde el extremo de trabajo del miembro de accionamiento está configurado para moverse a lo largo de la primera mordaza y la segunda mordaza para mantener la distancia máxima entre la primera superficie de la primera mordaza y la segunda superficie de la segunda mordaza.
- 7. El conjunto de herramienta de la reivindicación 5 o 6, en donde el extremo de trabajo del miembro de accionamiento define una barra en I (334), incluyendo la barra en I una primera barra (335) configurada para acoplarse con la primera mordaza, una segunda barra (336) configurada para acoplarse con la segunda mordaza, y un puntal vertical (337) que conecta la primera barra y la segunda barra.
- 8. El conjunto de herramienta de la reivindicación 7, en donde la barra en I incluye una superficie orientada distalmente que define una cuchilla (338).
- 9. Un dispositivo de grapado quirúrgico (10), que comprende:un cuerpo alargado (14) que tiene una porción proximal y una porción distal; yun conjunto de recarga (18) que incluye una porción de cuerpo proximal (18a) y el conjunto de herramienta (16) como se define en cualquier reivindicación anterior, la porción de cuerpo proximal adaptada para acoplarse de forma liberable a la porción distal del cuerpo alargado, y el conjunto de herramienta estando apoyado en la porción del cuerpo proximal.
